Description

【発明の詳細な説明】 本発明は、内視鏡、特にフレキシブルな内視鏡を用い
て行われる手術の際に結び目を結び付けるために使用さ
れる装置に関する。
従来、フレキシブルな内規鏡検査の際に結び目を結び
付けることは困難であるとされていた。実際、外科医
は、単一の縦に延びたチャンバーを操作降下させ、苦心
して結び目を結び付ける際に、有効な結び目は一般に内
視鏡のチャネルを横切る方向に沿って力を作用させるこ
とを要求する、という問題に直面する。本発明は、この
問題を解決することを目的とする。
本発明は、遠位の端部と近位の端部とを有する内視鏡
と、この内視鏡の可視領域を制限することなく前記遠位
の端部を取り囲む環状部材で構成される糸案内装置とを
備え、前記糸案内装置は互いに横方向に離間配置された
一対の糸案内手段を有し、前記装置は、前記糸案内手段
に通された糸によって形成された結び目を前記内視鏡に
よって見ることを可能とする。
さらに、本発明は、上述したようにその遠位の端部に
糸案内装置が設けられた内視鏡を提供する。
また、本発明は、その遠位の端部に横方向に離間配置
された一対の糸案内手段を有する内視鏡を提供する。
添付の概略図において、 図１は、内視鏡の遠位の端部に設けられた本発明によ
る糸案内装置の斜視図である。
図２は、緩く絡ませた結び目を結び付けるために使用
される、図１の装置及び内視鏡を示す。
図３は、セルフ・ロック型の引結びを結ぶ付けるため
に使用される、図１の装置及び内視鏡を示す。
図１は、その遠位の端部に設けられた糸案内装置２を
有する内視鏡１を示す。内視鏡自身は従来のものでよ
く、図示したタイプはイメージ束10と、２つのイルミネ
ーション束11と、２つの操作チャネル12とを備えてい
る。但し、他の形式の内視鏡を替わりに使用することも
できることを強調しておかなければならない。
装置２は環状断面の短管の形態を有し、この短管は内
視鏡１の遠位の端部を取り囲んでいる。装置２は、いく
つかの従来の内視鏡に与えられた外部糸に絡ませるため
の内部糸を与えることによってプッシュ・フィットとし
て内視鏡１に装着されたり、又は他の適切なやり方で装
着され得る。
装置２は、内視鏡１及び装置２の長手方向の軸心に対
して平行に前記装置の一方の端部から他方の端部に延び
る一対の糸案内通路３を有している。この通路は、手術
の際に使用される通常の種類の糸が通る程度の断面を有
している。図示したように、２つの通路３は好ましくは
装置２の軸心に対して互いにほぼ180゜に配置され、こ
れが結び目を結び付ける際に作用する力を最大にすると
いう点において望ましいが、これは本質的なものではな
い。
図１に示した構成を実際に使用する方法が図２及び図
３から読み取れる。
図２には患者の組織４が付加的に示され、この組織４
は外科医によって糸６が通されたＵ字状の組織５を有し
ている。糸６で結び目８を結び付けることが望まれる。
結び目を結び付ける手順の開始時に、糸６の端部７は患
者の外側にあり、通常は口又は肛門を通って出ている。
糸案内装置２が内視鏡１に装着される前又は後に、端部
７を用いて患者の外側で緩い結び目が結ばれ、そして、
各端部７は、内視鏡の遠位の端部から近位の端部に向け
て、通路３のいずれか一方にそれぞれ通される。
そして、糸案内装置を有する内視鏡は、患者の内部に
挿入され、組織の部分に向かって動かされる。このよう
にして、緩い結び目８は、内視鏡の遠位の端部が組織５
の部分に接近するまで、糸の端部に沿ってスライドす
る。そして、外科医は矢視方向に端部７を引っ張る。す
ると、横方向の成分を有する力が緩い結び目８に作用
し、これによって結び目が締め付けられる。この一連の
工程において、内視鏡の可視チャネルを通して結び目を
見ることができる。なぜなら、結び目は内視鏡の可視チ
ャネルの正面にあり、かつ、可視領域は糸案内装置２に
よって全く制限を受けないからである。
上述した工程は、単一のハーフヒッチの結び目を結び
付ける場合である。１個よりも多くの、例えば２又は３
個のハーフヒッチによって手術用の結び目を形成する場
合もあり、この場合は、図２に示した構成で、内視鏡１
及び糸案内装置２を患者の内部から引き抜いた後に追加
のハーフヒッチを作成して再び挿入し、これを繰り返す
ことによって行うことができる。
図３は、図２と同様の構成のものを使用する場合を示
すが、セルフ・ロック型の引結び8aを結び付ける点が異
なる。結び目自体の相違を除けば、使用される工程は図
２を参照して上述したものと同様である。
図１乃至図３に示した糸案内装置は、内視鏡自体とは
区別される部材であり、内視鏡から取り外すことができ
る。しかしながら、糸案内装置を内視鏡と一体的に設け
たり、又は内視鏡に対して取り外しできないように永久
的に取り付けることもできる。

    (57)【特許請求の範囲】
  1. 【請求項１】遠位の端部と近位の端部とを有する内視鏡
    であって、この内視鏡の可視領域を制限することなく前
    記遠位の端部を取り囲む環状部材で構成される糸案内装
    置を備え、前記糸案内装置は互いに横方向に離間配置さ
    れた一対の糸案内手段を有し、前記装置は前記糸案内手
    段を通された糸によって形成された結び目を前記内視鏡
    で見ることを可能とする、内視鏡。
  2. 【請求項２】前記糸案内手段は前記内視鏡の直径方向で
    あって互いに異なる側に位置することを特徴とする請求
    の範囲の第１項に記載の内視鏡。
  3. 【請求項３】前記環状部材は円筒形であることを特徴と
    する請求の範囲の第１項又は第２項に記載の内視鏡。
  4. 【請求項４】前記糸案内手段のそれぞれは前記環状部材
    を通って延びる通路で構成されていることを特徴とする
    請求の範囲の第１項乃至第３項のいずれかに記載の内視
    鏡。
  5. 【請求項５】前記通路は互いに平行で、かつ前記環状部
    材の回転対称の軸心に対して平行であることを特徴とす
    る請求の範囲の第４項記載の内視鏡。
