本発明の対象とする茎部は、長い骨、殊に大腿骨の髄管
内にセメントなしで導入されるものである。
【０００２】
【従来の技術】セメントなしの人工股関節或いは人工肩
関節は、予め決定された向きに設置されねばならない。
事実、外科医は、設置時に手術的に大腿骨茎部の方向を
訂正する可能性を持たない。人工股関節の現在の使用法
では、外科医は大腿骨頚部をある決定された位置で切断
する。頚部の断面が開口部の直径を決定し、該開口部よ
り移植組織或いは大腿骨茎部が導入されねばならない。
【０００３】
【発明が解決しようとする課題】現在提案されている種
々の多くの人工股関節においては下記の問題点がある。
大移植組織或いは大腿骨茎部の設置時に、外科医は頚部
の切断を適応させねばならない。このことは、骨素材の
重要な撤去を強制し、移植組織の維持を弱めることにな
る。解剖学的移植組織を使用する時、導入を可能にする
ために、骨のいくらかの区域は破壊される。これら破壊
された区域はもはや移植組織と一旦設置された人工補綴
物と接触しない。このことは結果として人工補綴物の直
接固着を減じ、こうして移植組織が移動する危機を招
く。本発明がより特別に改善するのはこれらの不都合で
ある。
【０００４】
【発明の目的】本発明による茎部は、骨表面上に完全に
支えられる導入を確実にするために、茎部自体を支える
特別最適な区域を定義するのを目的とする。
【０００５】
【課題を解決するための手段】この目的で、本発明は末
端部と結合部と外髄質部とを有し、これらは後面と前面
及び内側面と外側面を有してなる人工関節用茎部であっ
て、前記結合部の内側面は丸みを有し、この丸みの半径
の中心は、骨幹の軸（ＸＸ’，ＹＹ’ ，ＺＺ’）のう
ちの軸（ＸＸ’，ＹＹ’）の中心に対して側方及び前方
に徐々に移行しており、前記結合部の後面と前面とは骨
幹の軸（ＸＸ’）に対してそれぞれ異なる角度で傾斜し
ており、それらの傾斜角は結合部の全高に亘って一定で
あり、また後面と前面は前記傾斜角を維持しつつ骨幹の
軸（ＺＺ’）方向に湾曲した矢状の面を形成していて、
その湾曲中心及び湾曲半径はそれぞれ異なっていること
を特徴とする。 結合部の後面と前面の軸（ＸＸ’）に対
する傾斜角は７度から１５度の間であることが好まし
い。また、結合部の後面はプラスマイナス５００mm変化
しうる２０００mmの湾曲半径を有することが好ましい。
さらに、結合部の前面はプラスマイナス４０mm変化しう
る２４０mmの湾曲半径を有することが好ましい。本発明
による茎部は人工股関節と人工肩関節に独立して適用さ
れうる。
【０００６】例示として与えられた添付図により、本発
明、その呈する特徴、並びにもたらされうる利点がより
よく理解できるであろう。
【０００７】
【実施例】図１〜図３には茎部１、殊に人工股関節の大
腿骨茎部を示してある。茎部１の外形状は、患者の大腿
骨（図示されていない）の髄管の内部にセメントなしで
導入できるように形成されている。
【０００８】大腿骨茎部１は、大腿骨の骨幹の軸である
XX' 、YY' 、ZZ' で表される軸に対する幾何学的形状で
示されている。注意すべきは、正面はXX'-ZZ' 軸によっ
て限定され、矢状の面はYY'-ZZ' 軸によって限定され、
一方骨幹の軸はZZ' で示されることである。
【０００９】大腿骨茎部１は、茎部の下端をなす末端部
1a、結合部（癒着部）或いは中央部1b、茎部の自由な上
端をなす外髄質部1c、の三区域から構成される。大腿骨
茎部１は後面1d、前面1e、内側面1f、外側面1gを有し、
これらの面の形状は各々、末端部1a或いは結合部1bにお
いて、変化する。
【００１０】末端部1aは、図１の正面と図２の矢状の面
に従って、異なる傾きの円錐形状を呈する。事実、1fと
1gの両面は、1dと1eの両面のように同じ角度では傾いて
いない。
【００１１】結合部1b内では大腿骨茎部１は徐々に太く
なっていく。その際、後面1dと前面1eは、望ましい解剖
学的形状によれば７度から15度の間に含まれる一定の角
度で、骨幹のXX' 軸に対して各々異なる角度で傾いたま
ま保持される。後面1dと前面1eは、茎部１の全高にわた
って一定で且つ骨幹のXX' 軸に対しては７度から15度の
間に含まれる角度の傾きを呈する（図４参照) 。
【００１２】更に認められるのは、後面１ｄと前面１ｅ
は、結合部１ｂ内において、前記傾斜角を維持しつつ骨
幹の軸（ＺＺ’）方向に湾曲した円筒形状の矢状の面を
形成していることである（第２図）。また、それらの湾
曲中心及び湾曲半径はそれぞれ異なっている。後面１ｄ
は例えば、大腿骨茎部１の解剖学的形状を妨げることな
しに、プラスマイナス５００mm変化しうる2000mmの湾曲
半径或いは部分円によって表される。同じく前面１ｅは
例えば、大腿骨茎部１の解剖学的形状を妨げることなし
に、プラスマイナス４０mm変化しうる２４０mmの湾曲半
径或いは部分円によって表される。
【００１３】内側面1fは、図１に示されるように、結合
部1b内に湾曲して延長され、該内側面1fは、末端部1aに
見られたのと同じ一定半径の円形を呈する。これに対
し、結合部1bにおける何段階かの異なる高さで内側面1f
の形状を形作る半径の中心1hは全体が、図５〜図８に示
すように、骨幹のXX' 軸とYY' 軸の中心Ｐに対して側方
に且つ前方に移動する。認められるのは、内側面1fの形
状を形作る各半径の中心1hのこのような移動は、茎部１
に特別な様相を与えて、骨幹のXX' 軸とYY' 軸の中心Ｐ
に対する大腿骨茎部１全体のずれを引き起こすことであ
る。内側面1fの形状の正面における投影は円の一部分で
ある。この投影は、例えばプラスマイナス25mm変化する
半径85mmの円の一部分によって表される。
【００１４】後面1dは、深さが下から上へと増加する縦
溝1i形状の表面様相を有する。これは大腿骨茎部１に課
される回転力に対して抗するようにするためである。各
溝1iは引き伸ばされた逆向きの水滴形状を呈し、つまり
最も幅広の部分が大腿骨茎部１の外髄質部1cのすぐ近く
に位置している（図１参照) 。
【００１５】前面1eは同じく、深さ、幅、長さにおいて
寸法が下から上へと増加する水滴形状1jの表面様相を有
する。これは、大腿骨茎部１の差し込み力に対して抗す
るように、また骨生成の位置的可能性に応じて変化しう
る骨の押し戻し凹みを造り出すようにするためである。
水滴形状1jは、後面1d上に設けられた水滴に比べ長さが
非常に短く、該水滴形状1jの最も幅広の部分が大腿骨茎
部１の外髄質部1cのすぐ近くに位置するように同じく逆
向きである (図３参照) 。
【００１６】大腿骨茎部１の外髄質部1cは、図示されて
いない人工大腿骨頭部を受けるための円錐部分1lを末端
とする軸1kから特に構成される。大腿骨茎部１の外髄質
部1cの幾何学的形状は、人工頚部の長さに応じて、骨幹
軸ZZ' に対する円錐部1lの頂点の距離によって定義され
るレバーの腕に応じて、また骨幹軸ZZ' に対する円錐部
1lの軸の矢状の面（図２参照）内に投影によって定義さ
れるアンチヴァージョン角度(angle d'anteversion) に
応じて、定義される。
【００１７】注意すべきは、パラメータが大腿骨茎部の
異なる長さによって変化することである。故に、外髄質
部1cのレバーの腕がガウス法(gaussienne)に従って回転
し、アンチヴァージョン角度を一定に保持し、頚部の長
さを内髄質部に応じて、該内髄質部自体は大腿骨の骨幹
の穴の直径によって定義されるものであるが、軸受によ
って変化するように保持する。
【００１８】上述した人工関節用茎部は、例えば人工肩
関節に適用され得るが、このために本発明の対象外とな
ることはい。

Claims (11)

    (57)【特許請求の範囲】
  1. 【請求項１】 末端部（１ａ）と結合部（１ｂ）と外髄
    質部（１ｃ）とを有し、これらは後面（１ｄ）と前面
    （１ｅ）及び内側面（１ｆ）と外側面（１ｇ）を有して
    なる人工関節用茎部であって、 前記結合部（１ｂ）の内側面（１ｆ）は丸みを有し、こ
    の丸みの半径の中心（１ｈ）は、骨幹の軸（ＸＸ’，Ｙ
    Ｙ’ ，ＺＺ’）のうちの軸（ＸＸ’，ＹＹ’）の中心
    （Ｐ）に対して側方及び前方に徐々に移行しており、 前記結合部（１ｂ）の後面（１ｄ）と前面（１ｅ）とは
    骨幹の軸（ＸＸ’）に対してそれぞれ異なる角度で傾斜
    しており、それらの傾斜角は結合部（１ｂ）の全高に亘
    って一定であり、また後面（１ｄ）と前面（１ｅ）は前
    記傾斜角を維持しつつ骨幹の軸（ＺＺ’）方向に湾曲し
    た矢状の面を形成していて、その湾曲中心及び湾曲半径
    はそれぞれ異なっている ことを特徴とする人工関節用茎
    部。
  2. 【請求項２】 結合部（１ｂ）の後面（１ｄ）と前面
    （１ｅ）の軸（ＸＸ’）に対する傾斜角は７度から１５
    度の間であることを特徴とする請求項１の人工関節用茎
    部。
  3. 【請求項３】 結合部（１ｂ）の後面（１ｄ）はプラス
    マイナス５００mm変化しうる２０００mmの湾曲半径を有
    することを特徴とする請求項１の人工関節用茎部。
  4. 【請求項４】 結合部（１ｂ）の前面（１ｅ）はプラス
    マイナス４０mm変化しうる２４０mmの湾曲半径を有する
    ことを特徴とする請求項１の人工関節用茎部。
  5. 【請求項５】 末端部（１ａ）は円錐形状を有し、その
    後面（１ｄ）と前面（１ｅ）が、内側面（１ｆ）と外側
    面（１ｆ）のために想定された角度とは異なる角度で傾
    いており、結合部（１ｂ）の後面（１ｄ）と前面（１
    ｅ）が異なる中心の異なる湾曲半径に従って延長し、一
    方内側面（１ｆ）は、正面視における投影で円弧を表す
    丸みを形作る各半径の中心（１ｈ）の角度変位に応じて
    決定される湾曲形状に従って延長し、外髄質部（１ｃ）
    は人工関節頭部を受ける円錐部分（１１）を末端とする
    軸（１ｋ）を有していることを特徴とする請求項１の人
    工関節用茎部。
  6. 【請求項６】 後面（１ｄ）が結合部（１ｂ）内に、茎
    部（１）に課される回転力に抗するように、逆向きの水
    滴形状の一連の溝（ｌｉ）を有し、該溝の深さは該茎部
    の下から上へと増加することを特徴とする請求項１の人
    工関節用茎部。
  7. 【請求項７】 溝（ｌｉ）の最も幅広の部分が、茎部
    （１）の外髄質部の方へ向けられることを特徴とする請
    求項６の人工関節用茎部。
  8. 【請求項８】 前面（１ｅ）が、茎部（１）の差し込み
    力に対して抗するように、また骨生成の位置的可能性に
    応じて変化しうる骨の押し戻し凹みを造り出すように、
    該茎部の下から上へと深さ、広さ、長さにおいて寸法が
    増加する逆向きの水滴形状（１ｊ）の溝を多数備えるこ
    とを特徴とする請求項１の人工関節用茎部。
  9. 【請求項９】 逆向きの水滴形状（１ｊ）の溝が、その
    最も幅広の部分が茎部（１）の外髄質部（１ｃ）の方へ
    向くよう配置されることを特徴とする請求項８の人工関
    節用茎部。
  10. 【請求項１０】 当該人工関節用茎部が人工股関節の大
    腿骨茎部であることを特徴とする請求項１〜９のいずれ
    かに記載の人工関節用茎部。
  11. 【請求項１１】 当該人工関節用茎部が人工肩関節の上
    腕骨茎部であることを特徴とする請求項１〜９のいずれ
    かに記載の人工関節用茎部。
