Description

【発明の詳細な説明】 「産業上の利用分野」 本発明は直接固定型人工股関節に関するものである。
「従来の技術」 一般的に知られているように、人工股関節は本質的に
二つの種類に分類される。
第１の種類は、膠着物により固定されるタイプの人工
股関節である。しかし、このタイプの人工股関節は二つ
の問題を呈示する。
一つは膠着物自体によって引き起こされる麻酔ショッ
ク等の手術中の問題である。
また、もう一つは骨が膠着物と接合することによる汚
染の危険、あるいは、穿孔された骨がもろくなって人工
股関節の交換が必要になった場合に取り外し等の再手術
が困難であること等の手術後の問題である。
一方、第２の種類は、現在広まりつつある「直接固定
型人工股関節（prosthesis with primary fixation）」
と呼ばれるタイプである。このタイプの人工股関節で
は、調整されたピンが骨に強制的に嵌め合わされ、ま
た、ピンの特殊な形状により骨の矯正がなされる。この
タイプの人工股関節は三つの主要部分、すなわち、順
に、大腿骨に挿入される大腿ピン、患部股関節の寛骨臼
に嵌め込まれる杯体、および、球状部を載きかつピンを
杯体に結合して関節接合を適正に保持する関節首を必須
構成としている。
このような人工股関節を提供するにあたって、これま
でに多くの提案がなされている。
例えば、大腿ピンをねじのように大腿骨にねじ込む場
合に、前記ピンを多少切り詰めることによりねじ込み操
作を簡単にする提案である（欧州特許第0010527号およ
び仏国特許第2 295 729号参照）。
「発明が解決しようとする問題点」 しかしながら、上記ようなピンは、しばしば大腿骨の
形状にふさわしくない状態にねじ込まれて調整不良を起
こし、ひいては不具合な応力分布を発生させる。さら
に、これまで使用されていたピンは、長さ不足あるいは
フランジにおける引き戻し現象（drawing−back effec
t）等に起因して、取り外し時に危険を伴うことが指摘
されている。このような欠点は、ねじの取り付け位置で
生じる最大応力によるところの手術後の痛みを引き起こ
す可能性がある。
一方、欧州特許第000549号および仏国特許第2 481 59
6号の両者は、ピンを大腿骨に嵌め込まない代わりに、
ピンの頭部に同軸フランジを設け、その下面を大腿骨に
係合させ、かつ、上面に位置決め固定手段を設けて、ピ
ン組立時に関節首の傾斜姿勢の設定と保持とを行なうこ
とを提案している。この提案は多くの効果を奏するもの
であるが、実際にはフランジが整復部位に係合するか
ら、骨の切除が必要であり、大腿骨の上端、特に、「転
子」として知られる部分に破損を与える。さらに、この
フランジの構造そのものから考えて、関節首の前傾姿勢
があらゆる状況において理想的な向きになり得ない。
本発明は上記の事情に鑑みてなされたもので、その目
的とするところは、上記全ての欠点を排除した直接固定
型人工股関節を提供することにより、上記の不利を克服
することにある。
「問題点を解決するための手段」 かかる目的を達成するために、本発明に係る直接固定
型人工股関節は、次の三つの部分、すなわち、 i.大腿骨にねじ込まれるべく設計されたねじ山付き大腿
ピン、 ii.患部股関節の杯状の窪みに嵌まり込む形状の杯体、 iii.上端に球状部を載くとともにピンの頭部を杯体に連
結して関節接合を適正状態に保持する関節首 を主要構成とし、しかも、ねじ付き大腿ピンが、その末
端部から頭部に向けて漸次半径を増大させる如くテーパ
付けされたチューリップ形の縦断面を有することを特徴
とするものである。
言い替えれば、本発明は直接固定型人工股関節に係
り、そのねじ付き大腿ピンは半径が漸次変化する特殊な
チューリップ形の解剖学的な輪郭形状を有するものであ
る。
ここで「半径が漸次変化する輪郭形状」とは、末端部
から頭部に向かって曲率半径が連続的に漸次減少する縦
断面輪郭を意味する。従って、横断面の半径は末端部か
ら頭部に向けて漸次増加することになる。
この場合、実際には、ねじ付き大腿ピンの横断面形状
が円形であること、また、ねじ付き大腿ピンのねじ山
が、杯体のねじ山同様に台形断面、好ましくは角が丸ま
った台形断面を有することが有利である。
また、前記ねじ山において、ねじの方向に沿う横断面
に対するねじ山上面の傾きは、同横断面に対するねじ山
下面の傾きより小さいことが望ましい。
前記横断面に対するねじ山上面の傾斜角は15ないし25
゜、好ましくは20゜であり、一方、同横断面に対するね
じ山下面の傾斜角は10ないし20゜、好ましくは15゜であ
る。
また、ねじ山付き大腿ピンはその軸線に沿う複数の溝
を備えており、各溝の切り込み角は、ねじ込み、および
抜き出し操作を補助する両方向に傾斜していることが好
ましい。
さらに、関節首とねじ付き大腿ピン頭部との嵌め合い
および関節首と球状部との嵌め合いは、側面の傾斜が小
さい二つの円錐状部分、すなわち円錐状凸部および円錐
穴の嵌め合いによりなされるのがよい。
「作用」 本発明の直接固定型人工股関節は大腿骨の形状に適応
しているので、順応性が優れている。
また、ねじの取り付け位置における最大応力を低減さ
せ、かつ、大腿骨の上端の切除を少なくして直接固定法
を改善する。
そして、結果的に関節首の前傾姿勢を理想的な向きに
する。
「実施例」 以下、本発明の一実施例を図面に基づいて説明する。
本発明に係る人工股関節は、横断面円形をなすねじ付
き大腿ピン（以下単にピンと称す）１を第１の構成要素
としている。このピン１は、その頭部２が末端部３より
太く形成されており、また、本発明によれば、末端部３
から頭部２に向けて半径を漸次増大させるチューリップ
形の縦断面４を備えている。この外側に向って広がる独
特の解剖学的輪郭の曲率は、連続的に累進的に変化して
いる。従って、ピン１の半径も変化している。このピン
１の輪郭形状は、三次多項式に近似できる。好ましい実
施例では、例えば、ピン１の全長を95mmとした場合、末
端部３の直径は18mm（ねじ山も含む）、頭部２の直径は
30mm、これに対し、中央部の直径は20.5mm足らずであ
る。
前記ピン１は在来の一般物質、例えばステンレス、合
金、あるいはセラミック等により形成されている。好ま
しくはピン１はチタニウムで形成され、表面にセラミッ
ク、特に酸化アルミニウムの被覆層を有するのがよい。
前記ピン１には、第３図および第５図に詳細に示すよ
うに、角が丸まった台形状の断面を有するねじ山５が設
けられている。
また、前記ピン１にはその軸線に沿って三本の溝6,7,
8が各々設けられている。これら溝は120゜毎にずれて配
置されており、その切り込み角9,10は、ピン１のねじ込
み操作と抜き出し操作を補助する両方向に傾斜してい
る。実際には、このような切り込み角は鋭い角度をなし
ている。従って、切り欠き刃を形成する三本の溝6.7,8
はタッピング作用（ねじを切る作用）により、ねじ込み
操作、あるいは必要に応じて抜き出し操作を容易にする
ので、このとき使用される膠着物等の量を少なくできる
利点がある。
本発明の他の実施例によれば（第３図および第５図参
照）、ピン１のねじ山５の断面は、角が丸まった台形状
をなしている。このねじ山５の上面11はねじ山の方向、
すなわち、ねじ込み方向に対して傾斜している。この上
面11の傾斜角はねじ山５の下面12の傾斜角よりも小さく
形成されている。上面11と水平面Ｈとのなす角度（傾斜
角）αは、10ないし20゜、好ましくは15゜前後である。
これは傾斜角αを切り込み角の一部となし、かつ、大腿
骨内におけるピン１の固定状態を確実にするためであ
る。水平面Ｈと下面12とのなす角度（傾斜角）βは、15
ないし25゜、好ましくは20゜である。これは効果的な切
り込み角を形成するためである。第５図に示すように傾
斜面11,12間の角部13,13′は丸められている。
ピン１に好適な頭部２の形状は円錐状であり、この頭
部（円錐状凸部）２の側面（軸受面）15は約６゜程度に
傾斜している。この頭部２はこれに符合する形状の雌
型、つまり関節首17の基部16内に形成された円錐穴18に
嵌まり合う。
前記関節首17はピン１と同様の物質、例えばチタニウ
ムで形成されている。この関節首17はその基部16に円錐
穴18を備えている。また、その頭部19は円錐状をなし、
この頭部（円錐状凸部）19の側面（上部軸受面）20は僅
かに傾斜している。この円錐状凸部19は球状部21内に形
成された符合する形状の円錐穴25の中に嵌まり込むよう
になっている。前記上部軸受面20の傾斜角度は、軸受面
15の傾斜角度にほぼ近く、約３゜程度（モールス円錐）
である。このような嵌め合い構造は、機械的分野では既
に周知であり、かつ、人工股関節として既に提案されて
いるとうり（例えば仏国特許1 017 927および欧州特許0
000549参照）、容易に得られるものであり、関節首17の
前傾姿勢を理想的な向きにする。
関節首17は、取り付けねじ23によりピン１の上に取り
付けられている。
関節首17の軸線とピン１とのなす角度は、135゜前後
である。
球状部21は、周知技術により杯体32に嵌まり合い、ま
た、杯体32は患部股関節の杯状の窪みに嵌まり合うよう
になっている。
前記杯体（第１図参照）32は、高密度ポリエチレン製
の在来部材（図示略）を第１の構成要素としている。こ
の在来部材は、球状部21の外面（接触部）22に係合する
よう空洞部を有する形状とされており、この在来部材の
外部は卵形を呈し、かつ、それ自身を杯体32の中に適応
状態に埋設して、受け具を形成している。有利な実施例
では、前記接触部22は気相で付着させられた窒化チタニ
ウムの層、例えば３ないし10ミクロン以上の厚さを有す
る層で構成されている。また、チタニウム製の杯体32
は、前記受け具の形状に実質的に符合する外形を持って
いる。このことは骨盤への力の伝達状態を改善する。ま
た、前記杯体32には、ねじ山５に似た台形状のねじ山3
3、およびタッピング作用を持つ一連の溝34,35が設けら
れている。
また、杯体32のねじ山33はピン１のねじ山５と同じ特
徴、すなわち角が丸まった台形断面を有するという特徴
を持っているのが好ましい。そして、本発明の好ましい
実施例では、ピン１のねじ山５と杯状体32のねじ山33は
６本の突条により構成されている。
上記のように構成された人工股関節において、ピン１
の形状、すなわち、半径が漸次変化するチューリップ形
の特徴ある輪郭形状は、予期した以上に大腿骨上端の皮
質の形状にほぼ完全に適合する。この結果、ピン１の調
整をほどよく行うことが可能となり、人工股関節と骨と
の間の歪分布を改善することができる。
また、前記ねじ山５は人工股関節に適用された場合
に、予想以上に抜群の強度を発揮し、かつ、応力分布を
改善して最大応力を低減させる素晴しい効果を奏する。
「発明の効果」 以上説明したように、本発明の人工股関節は従来のも
のを上回る次のような多くの効果を奏する。
ねじ山のピッチに与えられた特殊な形状、すなわち、
角が丸まった台形状の断面形状により、ねじ山の取り付
け位置での最大応力を消し去るか、あるいは完全に消し
去れなくとも、低減させることができる。従って、手術
後の痛みを緩和することができる。
大腿ピンの長さ方向の輪郭形状は、末端部から頭部に
向けて半径を漸次増大させる形状であるから、従来の円
錐形状のピンよりも、大腿骨の髄の導管に適合するだけ
ではなく、骨に与える変形量が少なく、そのために骨に
発生する応力が緩和され、痛みを軽減することができ
る。しかも、骨に与える変形量が少ないので、骨の成長
を可及的に阻害しないという利点がある。
加えて、中心軸に関して対称でありかつ半径が連続的
に漸次変化することから、大腿ピンを回動させることに
より、ねじ込み操作および抜き出し操作を容易に行うこ
とができる。
ピンにフランジが無いので大腿骨上端の骨の切除が重
要でなくなるから、転子が破損する危険性を小さくする
ことができる。
機械的分野およびこの種の出願でよく知られている低
傾斜の円錐状部分の嵌め合いにより、結果的に、これま
で得ることができなかった関節首の理想的な前傾姿勢を
確保することができる。
ピンのタッピング作用により、少量の膠着物で簡単に
位置調整ができる。
ねじ山付き大腿ピンと関節首とは、所定角度をもって
着脱可能な構成とされているので、転子周辺の湾曲に対
しては、関節首が適合することができる。よって、ねじ
山付き大腿ピンの中心軸を直線状とすることが可能であ
り、ねじ山付き大腿ピンのねじ込み操作および抜き出し
操作を容易に達成することができる。

    (57)【特許請求の範囲】
  1. 【請求項１】大腿骨に挿入されるねじ山付き大腿ピン
    と、患部股関節の寛骨臼に嵌め込まれる杯体と、球状部
    を載くとともに前記ねじ山付き大腿ピンを杯体に連結し
    て関節接合を適正状態に保持する関節首とを必須構成と
    し、 前記ねじ山付き大腿ピンと前記関節首とは、所定角度を
    もって着脱可能な構成とされ、 前記ねじ山付き大腿ピンは、断面がほぼ円形であるとと
    もに中心軸が直線状であり、かつ大腿ピンの長さ方向の
    輪郭形状は、前記中心軸に関して対称であるとともに末
    端部から頭部に向けて半径を連続的に漸次増大させる形
    状であることを特徴とする直接固定型人工股関節。

  6. 【請求項６】前記ねじ山付き大腿ピンは、その軸線の方
    向に沿って配された複数の溝を備え、これら溝の切り込
    み角は、ねじ山付き大腿ピンのねじ込み操作および抜き
    出し操作を補助する両方向に傾斜していることを特徴と
    する特許請求の範囲第１項記載の直接固定型人工股関
    節。
  7. 【請求項７】前記関節首と前記ねじ山付き大腿ピンの頭
    部との嵌め合い、および関節首と前記球状部との嵌め合
    いは、側面の傾斜が小さい二つの円錐状部分、すなわち
    円錐状凸部および円錐穴の嵌め合いによりなされている
    ことを特徴とする特許請求の範囲第１項記載の直接固定
    型人工股関節。
  8. 【請求項８】前記ねじ山付き大腿ピン、関節首、球状
    部、および杯体はチタニウムで形成されていることを特
    徴とする特許請求の範囲第１項記載の直接固定型人工股
    関節。
  9. 【請求項９】前記球状部の外面は、杯体の空洞部内の高
    密度ポリエチレンと協同する窒化チタニウムの層で覆わ
    れていることを特徴とする特許請求の範囲第８項記載の
    直接固定型人工股関節。
