Abstract
Description
各参加患者の試験期間は、以下のように最大28日間であった。
・任意の診断用サンプリング源からの逆転写ポリメラーゼ連鎖反応(RT-PCR)によるSARS-COV-2感染が検査室で確認された。
・NIH重症度分類(下記参照)により、重症または重篤に分類された患者。
・臨床試験に参加した5人の患者は、担当医が、クレキサンの最小用量の40mg/日、デキサメタゾン、及び、一部にはレムデシビルを投与して治療した。
・妊娠、授乳、及び出産の可能性のある女性で、試験期間中、許容される避妊手段を用いることを望まない場合。
・ステージ4の重症慢性腎臓病または透析を必要とする(すなわち、推定糸球体濾過量(eGFR)が30未満の)他の臓器不全を合併している場合。
・悪性腫瘍の患者、他の重篤な全身疾患の患者、または、精神病患者。
・他の臨床試験に参加している患者、または本試験と矛盾する可能性のある治験薬(すなわち、生物学的製剤)による治療を受けている患者。
・HIVや結核との重感染している患者。
・免疫抑制状態または免疫抑制作用が分かっている薬剤(次ページの併用禁止薬剤を参照)を服用している患者。
・挿管されている患者(インフォームドコンセントに署名できないため)。
・P/F比が150未満の患者または適格性が変化した患者であって、適格性と実際の薬物送達との間でP/F比が急激に低下した患者。
・無症候性または前駆症状の感染症:分子診断法(例えば、ポリメラーゼ連鎖反応)または抗原検査によるウイルス学的検査でSARS-CoV-2陽性を示すが、無症状の患者。
・軽症:COVID-19に類似した様々な症状及び徴候(例えば、発熱、咳、咽頭痛、倦怠感、頭痛、筋肉痛)の所見があるが、息切れ、呼吸困難、及び胸部画像検査の異常の所見がない患者。
・中等症:臨床的評価または画像診断により下気道疾患の所見があり、海抜0mの室内空気での酸素飽和度(SpO2)が94%以上の患者。
・重症:呼吸回数が30回/分以上、海抜0mの室内空気でのSpO2が94%未満、動脈血酸素分圧/吸気酸素濃度比(PaO2/FiO2)が300mmHg未満、または肺浸潤が50%超の患者。
・重篤:呼吸不全、敗血症性ショック、多臓器不全を有する患者。
Claims (24)
- SARS-CoV-2ウイルスに感染した対象におけるCOVID-19を治療する方法であって、
早期アポトーシス単核細胞濃縮集団を含む組成物を前記対象に投与する投与ステップを含み、
前記投与ステップによりCOVID-19を治療する、方法。 - 請求項1に記載の方法であって、
前記治療は、COVID-19の症状の治療、抑制、発生率の低減、改善、または緩和を含む、方法。 - 請求項2に記載の方法であって、
前記症状は、臓器不全、臓器機能不全、臓器損傷、サイトカインストーム、サイトカイン放出症候群、またはそれらの任意の組み合わせを含む、方法。 - 請求項3に記載の方法であって、
前記臓器は、肺、心臓、腎臓、肝臓、またはそれらの任意の組み合わせを含む、方法。 - 請求項4に記載の方法であって、
前記臓器不全、前記臓器機能不全、または前記臓器損傷は、肺不全、肺機能不全、または肺損傷を含む、方法。 - 請求項5に記載の方法であって、
前記肺機能不全は、急性呼吸窮迫症候群(ARDS)または肺炎を含む、方法。 - 請求項3に記載の方法であって、
前記臓器不全は、急性多臓器不全を含む、方法。 - 請求項3に記載の方法であって、
前記臓器不全の治療は、前記臓器不全の軽減、進行の遅延、抑制、反転、修復、またはそれらの任意の組み合わせを含む、方法。 - 請求項1に記載の方法であって、
前記治療は、前記早期アポトーシス単核細胞濃縮集団を投与されていないCOVID-19対象と比較して、COVID-19対象の生存期間を増加させる、方法。 - 請求項1に記載の方法であって、
前記COVID-19は、軽症、中等症、重症、重篤のCOVID-19を含む、方法。 - 請求項10に記載の方法であって、

前記早期アポトーシス単核細胞濃縮集団は、
(a)24時間を超えて安定しているアポトーシス集団、
(b)非静止非アポトーシス細胞の数が減少した、生存非アポトーシス細胞の細胞活性化が抑制された、または生存非アポトーシス細胞の増殖が低下した、アポトーシス集団、
(c)早期アポトーシス単核濃縮細胞のプールされた集団、または、
(d)それらの任意の組み合わせ、を含む、方法。 - 請求項1に記載の方法であって、
前記投与ステップは、前記早期アポトーシス単核細胞濃縮集団の単回注入を含む、方法。 - 請求項1に記載の方法であって、
前記投与ステップは、前記早期アポトーシス単核細胞濃縮集団の複数回注入を含む、方法。 - 請求項1に記載の方法であって、
