Abstract
Description

細胞、特に遺伝子工学的に改変された免疫細胞、の分化、発達および増殖の制御された操作は、臨床上の重大な関心事である。T細胞は、癌細胞、細胞内病原体、および自己免疫に関与する細胞を認識して殺傷するなどの治療用途で使用するために、遺伝子改変されてきた。癌の治療における遺伝子改変細胞療法の使用は、特定の機能を提供しかつ癌細胞を選択的に攻撃するように誘導された改変T細胞の選択的活性化と拡大によって促進される。養子免疫療法のいくつかの例では、T細胞は患者の血液から分離され、エクスビボで処理され、患者に再注入される。そのため、標的化された改変細胞集団の選択的活性化を可能にする組成物および方法が望まれる。

STATタンパク質は、C末端の保存されたチロシン残基のリン酸化後に、核内に移行し、そこでDNAに結合して標的遺伝子の転写を活性化する、転写活性化因子として作用する。Hennighausen L, Robinson GW. STAT3結合モチーフに加えて、改変型ヒトCD122はヒトCD122を含み、ヒトCD122は天然ヒトCD122またはヒトオルソゴナルCD122であり得る。
改変型オルソゴナルヒトCD122を使用する場合には、オルソゴナルIL2を用いて、改変型オルソゴナルCD122を発現するように操作された細胞の選択的刺激を促進することができる。改変型オルソゴナルCD122を発現するように操作されたT細胞と、該オルソゴナルCD122の対応するオルソゴナルリガンドとの接触は、そのように操作されたT細胞の特異的活性化を可能にする。特に、このオルソゴナルIL2受容体リガンド複合体は、細胞の混合集団、特にT細胞の混合集団において、オルソゴナル受容体を発現するように操作された細胞の選択的拡大(増殖)を提供する。IL2オルソゴナルリガンドは、CD122オルソゴナル受容体の細胞外ドメイン、特にアミノ酸置換H133DおよびY134Fを含むヒトCD122の細胞外ドメインを含む受容体を介した選択的結合およびシグナル伝達を提供する。野生型CD122を発現する細胞に対するオルソゴナルIL2のIL2活性は、オルソゴナルCD122を発現する細胞に対するオルソゴナルIL2の活性と比較して、大幅に低下する。それゆえ、操作された細胞集団に対してオルソゴナルIL2を用いた、オルソゴナルIL2の細胞外ドメインを含む受容体を発現するように操作された細胞の選択的活性化および/または拡大が提供される。

細菌発現系でリーダー配列の非存在下に直接組換えにより産生させると、内因性プロテアーゼがN末端のMet-Ala1残基を欠失させて、「desAla1」オルソゴナルIL2をもたらす。本明細書に開示されるオルソゴナルIL2は、最初の2つのアミノ酸の欠失(desAla1-desPro2)と、N末端修飾、特にシステインのスルフヒドリル基のPEG化、を促進するためのシステイン残基(T3C)によるThr3グリコシル化の置換を含むことができる(例えば、1993年4月27日発行のKatre, et al. 米国特許第5,206,344号を参照されたい)。
本開示のいくつかの態様では、オルソゴナルIL2は、有効性の実質的な損失なしに、ヒトにおけるIL2療法の使用の重大な用量制限のある副作用である血管漏出症候群(vascular leak syndrome)を回避するために、アミノ酸置換を含む。2009年4月7日発行のEpstein, et al. 米国特許第7,514,073B2号を参照されたい。本開示のオルソゴナルIL2に含まれるそのような修飾の例として、R38W、R38G、R39L、R39V、F42K、およびH55Yの1つまたは複数が挙げられる。
いくつかの態様では、オルソゴナルIL2は、オルソゴナルCD122に対するその活性を高めるために、親和性成熟を行うことができる。「親和性成熟」を行ったポリペプチドは、1つまたは複数の残基に1つまたは複数の改変を有するポリペプチドであって、こうした改変を持たない親ポリペプチドと比較して、コグネイトオルソゴナル受容体に対するオルソゴナルポリペプチドの親和性、またはその逆の改善をもたらすものである。親和性成熟は、オルソゴナルIL2の結合親和性を、「親」ポリペプチドと比較して、少なくとも約10%、あるいは少なくとも約50%、あるいは少なくとも約100%、あるいは少なくとも約150%、または1~5倍増加させるために行うことができる。本発明の遺伝子操作されたオルソゴナルIL2は、上述のようにそのコグネイトオルソゴナル受容体を活性化するが、適切なアッセイ条件下で十分な量の分子を用いてELISAおよび/またはFACS解析によって評価した場合、野生型IL2受容体の結合および活性化が大幅に低下している。
上述したように、本開示の組成物は、インビボでの寿命の延長および/または対象における作用持続時間の延長を提供するように改変されたオルソゴナルIL2を含む。寿命および/または作用持続時間の延長のためのそうした改変には、オルソゴナルIL2の一次配列に対する修飾、キャリア分子へのコンジュゲーション(例えば、アルブミン、アシル化、PEG化)、およびFc融合が含まれる。
上述したように、オルソゴナルIL2という用語は、インビボでの寿命の延長および/または対象における作用持続時間の延長を提供するためのオルソゴナルIL2の改変を含む。
いくつかの態様では、オルソゴナルIL2は、オルソゴナルIL2に特定の性質(例えば、対象における作用持続時間の延長)を提供するように改変され、これは、半減期の延長などの所望の薬理学的性質を提供するキャリア分子へのコンジュゲーションによって達成され得る。いくつかの態様では、オルソゴナルIL2は、IgGのFcドメイン、アルブミン、または半減期を延長するための他の分子に、例えば、当技術分野で知られるようなPEG化、グリコシル化、脂肪酸アシル化などによって、共有結合することができる。
いくつかの態様では、オルソゴナルIL2は、インビボでの長期間曝露を促進することが当技術分野で知られているアルブミン分子(例えば、ヒト血清アルブミン)との融合タンパク質として発現される。

いくつかの態様では、本開示のオルソゴナルIL2は、Resh (2016) Progress in Lipid Research 63: 120-131に記載されるように、脂肪酸分子へのコンジュゲーションによってアシル化され得る。コンジュゲート化される脂肪酸の例としては、ミリスチン酸、パルミチン酸、およびパルミトレイン酸が挙げられる。ミリストイル基は通常、N末端グリシンに連結されるが、リジンもミリストイル化され得る。パルミトイル化は通常、遊離システイン-SH基の酵素的修飾により、例えばS-パルミトイル化を触媒するDHHCタンパク質により達成される。セリンおよびトレオニン残基のパルミトレイル化は通常、PORCN酵素を用いて酵素的に達成される。

いくつかの場合には、改変型ヒトCD122は、ヒトCD122の細胞内ドメインのC末端に融合された1つまたは複数のSTAT3結合モチーフを含む。ヒトCD122は、天然ヒトCD122(SEQ ID NO: 1)または上記に開示されるようなオルソゴナルヒトCD122であり得る。

いくつかの態様では、遺伝子操作される免疫細胞はまた、キメラ抗原受容体(CAR)を発現する。CARは、免疫細胞が病変細胞、例えば腫瘍細胞、上の抗原に結合することを可能にし、したがって、その病変細胞を特異的に殺傷することができる。CARの抗原結合ドメイン(ABD)は、1価または多価であってよく、細胞表面腫瘍抗原に特異的に結合する1つまたは複数(例えば1、2、または3つ)のポリペプチド配列(例えば、scFv、VHH、および/リガンド)を含み得る。いくつかの態様では、腫瘍抗原と、そのような細胞表面腫瘍抗原に選択的に結合するABDを含むCARは、当技術分野で公知である(例えば、Dotti, et al., Immunol Rev. 2014 January; 257(1)を参照されたい)。本開示の方法および組成物は、CARのABDが、CD123、CD19、CD20、BCMA、CD22、CD30、CD70、Lewis Y、GD3、GD3、メソテリン(mesothelin)、ROR CD44、CD171、EGP2、EphA2、ErbB2、ErbB3/4、FAP、FAR、IL11Ra、PSCA、PSMA、NCAMなどを含むがこれらに限らない、腫瘍抗原に特異的に結合するCAR療法と併せて有用である。これらの標的と反応する抗体は文献上よく知られており、当業者であれば、CARのABDに組み込むことができる単鎖抗体(例えば、scFv、CDRグラフト化VHHなど)のポリペプチド配列の構築のために、そのような抗体からCDRを単離することができる。

いくつかの態様では、本開示は、1つまたは複数のSTAT3結合モチーフを含む改変型ヒトCD122を発現するように操作された免疫細胞を刺激する方法を提供し、この方法は、該免疫細胞を、改変型ヒトCD122に結合するヒトIL2ポリペプチドと接触させ、それによって、該免疫細胞を刺激する工程を含む。

いくつかの態様では、遺伝子改変T細胞は、治療される個体に関して同種他家である。Graham et al. (2018) Cell 7(10) E155。いくつかの態様では、同種他家の遺伝子改変T細胞は、完全にHLA適合である。しかし、全ての患者に完全適合ドナーがいるわけではないので、HLA型に関係なく全患者に適した細胞製品が代替製品を提供する。
改変型ヒトCD122を発現するように操作された免疫細胞の集団の導入に続いて、コグネイトIL2リガンドをコードする核酸構築物を対象に投与して、コグネイトIL2リガンドへの対象の連続曝露を達成することにより、コグネイトIL2リガンドを対象に提供することができる。いくつかの場合には、該操作された免疫細胞が改変型オルソゴナルヒトCD122を発現し、コグネイトのオルソゴナルIL2をコードする組換えベクターが、対象へのオルソゴナルIL2の延長送達と、そのようなオルソゴナルIL2に関連するコグネイトのオルソゴナル受容体を発現するように操作された対応する細胞の長期活性化とを提供するために導入される。
一態様では、オルソゴナルIL2は、非ウイルスベクター中のオルソゴナルIL2の核酸発現構築物の形態で対象に投与することができ、非ウイルス送達システムにおいて提供され得る。非ウイルス送達システムは、典型的には、核酸カーゴによる標的細胞の形質導入を促進するための複合体であり、核酸は、カチオン性脂質(DOTAP、DOTMA)、界面活性剤、生体物質(ゼラチン、キトサン)、金属(金、磁性鉄)、合成ポリマー(PLG、PEI、PAMAM)などの物質と複合体化される。非ウイルス送達システムの多数の実施態様が当技術分野でよく知られており、例えば、脂質ベクターシステム(Lee et al. 遺伝子改変されたT細胞は、治療用、例えばヒトの治療に適した薬学的組成物として提供され得る。そのような細胞を含む治療用製剤は、凍結されるか、または生理学的に許容される担体、賦形剤もしくは安定剤(Remington's Pharmaceutical Sciences 第16版, Osol, A. 編 (1980))を用いて、水溶液の形態で、投与するために調製され得る。該細胞は、適切な医療行為と一致する方法で製剤化され、投与され、管理される。これとの関連で考慮すべき要素には、治療される特定の障害、治療対象となる特定の哺乳動物、個々の患者の臨床状態、障害の原因、薬剤の送達部位、投与方法、投与スケジュール、および医療従事者には公知の他の要素が含まれる。
本開示の組成物および方法は、追加の治療薬と併用することができる。例えば、治療すべき疾患、障害または状態が腫瘍性疾患(例えば、癌)である場合、本開示の方法は、従来の化学療法剤または他の生物学的抗癌剤、例えばチェックポイント阻害剤(例:PD1またはPDL1阻害剤)もしくは治療用モノクローナル抗体(例:アバスチン、ハーセプチン)と併用され得る。

これらの研究で使用されたhoCD122受容体のオルソゴナルコグネイトリガンド(以後STK-009)は、N末端アラニン残基の欠失を含み、かつアミノ酸置換L18R、Q22EおよびQ126K(成熟野生型hIL2に従って番号付けされた)を含み、さらにN末端プロリン残基にアルデヒドリンカーを有する40kD分岐(2×20kD)PEG分子、40kDaの2アーム分岐PEG-アルデヒド、2本の20kDA直鎖PEG分子を含む40kDA PEG-アルデヒド(Sunbright(登録商標)GL2-400AL3, NOF America Corporation, One North Broadway, White Plains, NY 10601 USA)を付加することにより修飾された、hIL2ムテインの化合物である。
CD19 CAR_T2A_hoRbまたはCD19 CAR_T2A hoRb_GGYRHQを安定的に導入されたT細胞を、RPMI培地+10%FBS中で1時間休ませた。次に、WT IL-2またはSTK-009のいずれかの用量漸増(dose titration)により細胞を20分間刺激し、様々なリン酸化抗原(phospho-antigen)について染色した。その後、細胞をフローサイトメトリー解析にかけた。これらの研究の結果を添付図面の図4および図5に示す。示されるように、CAR T細胞のhoCD122受容体にSTAT3モチーフを追加すると、pERK、pS6Kの増大した活性化およびpSTAT3のより高いベースラインが生じる。
CD19 CAR_T2A_hoRbまたはCD19 CAR_T2A hoRb_GGYRHQを安定的に導入されたT細胞を、CD19標的細胞株であるRaji-ルシフェラーゼと、示されたエフェクター:標的細胞(E:T)の比率で一晩共培養した。細胞生存率は、D-ルシフェリンの光への変換により評価し、生物発光プレートリーダーで読み取った。データは、100%×(自然死RLU - 試験RLU)/(自然死RLU - 最大殺傷RLU)として算出される特異的溶解(specific lysis)として示される。この研究の結果を添付図面の図6に示す。このデータは、CAR T細胞のオルソ受容体にSTAT3モチーフを追加することで、優れた細胞傷害活性が得られることを実証している。

- 改変型ヒトCD122が、天然ヒトCD122と比べて、R41、R42、Q70、K71、T73、T74、V75、S132、H133、Y134、F135、E136、およびQ214から選択される1つまたは複数の残基でさらに改変されている、請求項11に記載のポリヌクレオチド。
- リンカーがジヌクレオチドGGを含む、請求項10に記載のポリヌクレオチド。
- 改変型CD122が、リンカーとSTAT3結合モチーフの少なくとも1つとを含むアミノ酸配列を含み、該アミノ酸配列が、

(a)請求項1に記載のポリヌクレオチドによってコードされる改変型ヒトCD122を発現する細胞、および
(b)ヒトIL2ポリペプチド
を含む、前記キット。 - 改変型ヒトCD122が、SEQ ID NO: 1によりコードされる天然ヒトCD122を含み、かつヒトIL2ポリペプチドが、SEQ ID NO: 2によりコードされる天然ヒトIL2ポリペプチドである、請求項19に記載のキット。
- 改変型ヒトCD122が、オルソゴナルヒトCD122を含み、ヒトIL2ポリペプチドが、オルソゴナルヒトIL2ポリペプチドであり、かつ該オルソゴナルヒトIL2ポリペプチドが、天然ヒトCD122と比較して、改変型ヒトCD122に優先的に結合する、請求項19に記載のキット。
- オルソゴナルヒトIL2ポリペプチドが、天然ヒトCD122に対応する位置の残基T51、R81に、天然ヒトIL2ポリペプチドのアミノ酸以外のアミノ酸による少なくとも1つのアミノ酸置換を含むか、または天然ヒトCD122に対応する位置M23にアラニンを含み、かつ天然ヒトCD122に対応する位置E15、H16、L19およびD20の各々にアミノ酸置換を含む、請求項21に記載のキット。
- オルソゴナルヒトIL2ポリペプチドが、[E15D, E15T, E15A, E15S]、[H16N, H16Q]、[L19V, L19I, L19A]、[D20L, D20M]、[Q22S, Q22T, Q22E, Q22K, Q22E]、[M23A, M23W, M23H, M23Y, M23F, M23Q, M23Y]、[G27K, G27S]、[R81D, R81Y]、[N88E, N88Q]、[T51I]から選択される、天然ヒトIL2の位置に対応する1つまたは複数のアミノ酸置換を含む、請求項19に記載のキット。
- 改変型ヒトCD122が、哺乳動物細胞によって発現される、請求項19に記載のキット。
- 哺乳動物細胞が免疫細胞である、請求項24に記載のキット。
- 免疫細胞がT細胞である、請求項25に記載のキット。
- T細胞が、キメラ抗原受容体(CAR)-T細胞である、請求項26に記載のキット。
- 1つまたは複数のSTAT3結合モチーフを含む改変型ヒトCD122を発現する免疫細胞を刺激する方法であって、該免疫細胞をヒトIL2ポリペプチドと接触させる工程を含む、前記方法。

- 1つまたは複数のSTAT3結合モチーフを含む改変型ヒトCD122を発現する免疫細胞を個体に導入する工程、および
該個体にヒトIL2ポリペプチドを投与し、それによって該個体における免疫応答を活性化する工程
