胞シーケンシング技術はゲノム、トランスクリプトーム、エピゲノミクスやプロテオーム
などのさまざまなレベルで実現されており、多くの科学問題の研究において重要な役割を
果たしている。しかしながら、現在の単細胞シーケンシング技術はデータの品質、スルー
プット、コスト、操作性などの面で多くの課題が存在し、本格的な大規模応用にはまだ多
くの技術的障壁が存在している。

者の重要な特徴は、約40%のBD患者がインスリン/ブドウ糖代謝疾患を同時に患って
いることであることを考慮する(文献:Ruzickova M, Slaney C, Garnham J, & Alda M (
2003) Clinical features of bipolar disorder with and without comorbid diabetes m
ellitus. Canadian journal of psychiatry 48(7):458-461;文献:Hajek T, McIntyre R,
& Alda M (2016) Bipolar disorders, type 2 diabetes mellitus, and the brain. Curr
ent opinion in psychiatry 29(1):1-6,;及び文献: Wysokinski A, Strzelecki D, & K
loszewska I (2015) Levels of triglycerides, cholesterol, LDL, HDL and glucose in
patients with schizophrenia, unipolar depression and bipolar disorder. Diabetes
& metabolic syndrome 9(3):168-176参照)。脳内の重要な分子経路の欠陥が行動に関す
る症状を引き起こすため、これらの欠陥が小島に発生するとインスリン代謝疾患を引き起
こし、両者の合併症を引き起こすのではないかと推測されている。
Gene Cardsデータベースを調べたところ、インスリン代謝とシナプス機能の両
方に97個の遺伝子が出現していることがわかった。NCBI文献検索(www.ncbi.nlm.n
ih.gov/pubmed/)により、97個の遺伝子の中から上記2つの症状に作用する可能性のあ
る18個の遺伝子を選別した。
(文献Mertens J, et al. (2015) Differential responses to lithium in hyperexcitab
le neurons from patients with bipolar disorder. Nature 527(7576):95-99.参照)を
用いて、これらの患者のiPSCs分化ニューロンから、RNA-seqとqRT-PC
R手段を用いて、18個の遺伝子のうち11個の遺伝子のmRNA発現レベルに1.5倍
を超える変化が認められた(図1のA及びBに示されるとおりであり、具体的に示す縦座
標変化値は増加又は減少する)。図1のAは、BD患者のiPSC分化海馬ニューロンに
おける18個の候補遺伝子のトランスクリプトームシーケンシング(RNA-seq)の
結果であり、図1のBはBD患者のiPSC分化海馬ニューロンにおける18個の候補遺
伝子のリアルタイム蛍光定量RT-PCRの結果である。ここで、HCは健常対照群(n
=4人)を表し、BD-IはI型BD患者(n=6人)を表し、ここで、図1のA及びB
の図の横座標の各遺伝子に対応する2列の棒グラフは、左から後ろに向かってそれぞれ健
常対照組及びI型BD患者のデータを表している。
ウンした後、強制水泳試験(FST)を行い、マウスの不動時間を分析し、マウスの不動
時間は、マウスの感情的な絶望の程度を示す指標であり、時間が経つほど絶望的な気分が
深刻になった。その結果を図1のCに示す。図1のCは、候補遺伝子をノックダウン(K
D)した後のFST試験におけるマウスの不動時間(n=10)の結果を示し、t検定法
(Student’s t-test.*P<0.05;error bars,s.e
.m.)が使用さている。
、他の遺伝子のノックダウンでは、このような有意な変化は見られなかった。このことか
ら、Syt7は行動異常の候補リスク因子である可能性が推測された。
脳精神疾患-類似行動におけるSyt7の作用を系統的に研究するために、まず、2種
類の異なる由来のiPSC分化ニューロンについてBD患者体内のSyt7の発現状況を
独立に研究した。図2のA及びBは、文献Mertens J, et al. (2015) Differential resp
onses to lithium in hyperexcitable neurons from patients with bipolar disorder.
Nature 527(7576):95-99に記載されているLR及びNR患者由来の海馬ニューロンにおけ
るSyt7の発現状況を示しており、ここで、LRはlithium(リチウム塩)に反
応するiPSC分化ニューロンを表し、NRはlithiumに反応しないiPSC分化
ニューロンを表す。図2のCは文献Stern S, et al. (2017) Neurons derived from pati
ents with bipolar disorder divide into intrinsically different sub-populations o
f neurons, predicting the patients' responsiveness to lithium. Molecular psychia
tryに記載されているiPSC分化海馬ニューロンにおけるSyt7の発現状況を示す。
その結果、この2種類のニューロンにおいて、qRT-PCR及びウエスタンブロット分
析により健常対照群との差異を検出したところ、すべてSyt7の発現は低下した。この
ことから、異なるサブグループのBD患者にはSyt7欠陥が存在することが確認された
。
マウス)のパフォーマンスを検出した。その結果、同腹の野生型マウスと比較して、Sy
t7KOマウスのほぼ30%が、夜型(ZT 12-24)及び昼型(ZT 0-12)
の両方において、より短い不動時間を示した(図3に示すような持続的な躁状態であり、
英語ではManic-likeで表される)。興味深いことに、60%を超えるマウスは
、夜型では不動時間がより短く、昼型では不動時間がより長いという、変動性行動異常の
表現型を示した(図3に示すような双極性変動の結果であり、英語ではFluctuat
ionで表される)。
KOマウスに集中した。尾懸垂試験(TS)を行い、不動時間を分析し、マウスの不動時
間は、マウスの感情的な絶望の程度を示す指標であり、時間が経つほど絶望的な気分が深
刻になった。試験の結果、Syt7KOマウスは、野生型と比較して、図4のA(背景の
灰色部分は夜型を表し、背景の白色部分は昼型を表し、他の図も同様の表示をしている)
に示すように、夜型では顕著に短い不動時間を示し、昼型ではより長い不動時間を示した
。
、学習性無力感試験(LH)とショ糖嗜好性試験(SPT)を別々に行った。このうち、
LHは抑うつと関連しており、LH試験におけるマウスの脱出失敗回数は、動物が外界か
らのプレッシャーによって抑うつになっていることを検出する指標の一つとなり、脱出失
敗回数が多いほど、抑うつが顕著になっていた。SPTは快感の欠如と関連しており、マ
ウスのショ糖嗜好率はマウスの快感欠如の程度を検出する指標となるが、ショ糖嗜好率が
高いほど、快感が向上していることが示唆された。図4のB及びCに示すように、野生型
マウスと比較して、Syt7KOマウスは、夜型では、より躁状態(LH試験では脱出失
敗回数が少ない)であり、SPT試験ではより多くの快感を獲得し、一方、昼型では、S
yt7KOマウスは常に逆の状態を示した。
、Syt7KOマウスに不安のような行動が観察された。その結果、野生型マウスと比較
して、Syt7KOマウスは、図4のDに示すように、夜型では不安行動が減少し、昼型
では不安行動が増強することが明らかになった。
生するのかを明らかにするために、オープンフィールド試験を用いてマウスを24時間モ
ニタリングした結果、Syt7KOマウスは夜型で活動が増加するが、昼型では活動が減
少し、これは、FST試験や尾懸垂試験で得られた結果に対応している。重要なことに、
Syt7KOマウスの活動の変化が夜型及び昼型で持続的に存在し、マウスの気分異常が
永続的に出現することを示している。
で、Syt7KOマウスの心神喪失-類似行動をPPI試験(プレパルス抑制試験)を用
いて観察した。PPI試験は動物の心神喪失を検出するための古典的な行動パラダイムで
ある。PPI試験では、PPIの割合が低いほど、マウスの心神喪失が深刻であることを
示している。図4のEに示すように、野生型マウスと比較して、Syt7KOマウスは、
夜型でも昼型でも、常にPPIの割合が少なく、光の影響を受けないことから、Syt7
KOマウスでは精神病に類似した行動が持続していることを示し、またSyt7KOマウ
スは類似の音声誘発性けいれん反応を示した。
Syt7欠陥が存在することを示し、さらに重要なことに、Syt7KOマウスは行動異
常を示し、示した夜/昼周期的な特徴が双極性-類似行動のような異常な行動変動に特有
である。
さらにSyt7KOマウスの行動異常とBD特徴との関係を明らかにするため、Syt
7KOマウスを、BD患者の臨床的治療薬と前臨床的治療薬の2種類であるオランザピン
(olanzapine、OLZ)とlithiumで処置し、動物の行動を観察した。
t7KOマウスを処置し、用量依存性(用量はそれぞれ0.2、0.5、1.0mg/k
g)(ANOVA統計法で)を観察した。図5のA及びBに示すように、FST試験では
、非処置群と比較して、医薬品処置群のSyt7KOマウスでは、中等用量のオランザピ
ンの作用は行動異常を変化させるのに十分であり(すなわち、夜型の短い不動時間が反転
した)、さらに、高用量オランザピン処置は昼型の不動時間を延長した。また、低用量オ
ランザピン処置では、Syt7KOマウスの昼型の不動時間が減少し、これは、高用量の
結果とは異なった。理論的な制限はないが、BD患者の治療におけるオランザピンの抗う
つ効果と関連していると考えられる。

のA、B、及びCに示すように、夜型でも昼型でも、高用量オランザピン処置はSyt7
KOマウスの抑うつ状態を緩和し、非処置群のKOマウスと比較して、LH試験における
脱出回数を有意に増加させ、ショ糖嗜好性を減少させ、LDB試験における白箱滞留時間
を減少させたことが示された。また、オランザピン処置WTマウスでも、Syt7KOマ
ウスと類似した結果が得られた。

でも、30mg/kg Li2CO3処置は、図7に示すように、WT及びSyt7KO
マウスのいずれにも作用しなかった。しかし、図7のB及びCに示すように、LH及びS
PTの試験では、Liで処置したWT及びSyt7マウスはともに改善された抗うつ効果
を示し、すなわち、夜型でも昼型でも、非処置対照群と比較して、増加した脱出回数と減
少したショ糖嗜好性を示した。さらに、図7のDに示すように、LDB試験では、Li処
置はWTには作用しなかったが、Syt7KOマウスの行動を変化させ、白箱滞留時間を
減少させ、この変化は夜型でも昼型でも発生する。
な効果があることが示された。
最後に、BD患者のSyt7の状況がiPSC分化ニューロンにおける欠陥と一致する
か否かを調べるために、BD患者の血漿中のmRNAレベルを分析した。20人のBD患
者の検体及び年齢と性別が一致する11人の健常対照群(HC)を準備し、血漿を分離し
た。その後、qRT-PCR方法を用いて血漿中のSyt7mRNAの発現レベルを分析
した。その結果、HC群と比較して、BD群では、Syt7mRNAレベルは著しく低下
しており(図8参照)、BD患者にSyt7欠陥が存在することが明らかになった。
C群と比較して、大部分のBD患者のSyt7mRNAレベルは低下する傾向を示した。
なお、30歳以上の患者では、Syt7mRNAレベルの減少が顕著ではなかった。検体
数は少ないものの、結果はSyt7欠陥が若年成人でより顕著である可能性を示唆してい
る。さらに、タイプ、BD家族歴、薬物療法及び心神喪失の有無に応じてBD患者を群分
けして、分析した。その結果、HC群と比較して、BD-I型及びBD-II型患者の血
漿のSyt7mRNAレベルは有意に減少したことがわかった(図9のA参照)。
レベルの減少が顕著であったが、心神喪失やBD家族歴のあるそれらの患者では、Syt
7mRNAの減少傾向が見られた(図9のB及びC参照)。なお、未治療群の患者では、
検体量が少ないにもかかわらず、Syt7mRNAレベルの顕著な減少が示された。治療
群の患者のSyt7mRNAは、HC群と比較して低下していたが、回復が認められた(
図9のD参照)。

療方法の開発はBDの診療にとって有望である。

Claims (12)
- Syt7遺伝子及び/又はその発現産物を含む、ことを特徴とする双極性障害用のバイ
オマーカー。 - 双極性障害及び/又はその合併症を治療する医薬品の製造におけるSyt7遺伝子及び
/又はその発現産物の使用。 - 前記医薬品は前記Syt7遺伝子の発現量を増加させる、ことを特徴とする請求項2に
記載の使用。 - 野生型細胞よりも、Syt7遺伝子及び/又はその発現産物の含有量が低下する、こと
を特徴とする双極性障害細胞モデル。 - 野生型動物よりも、Syt7遺伝子及び/又はその発現産物の含有量が低下する、こと
を特徴とする双極性障害動物モデル。 - 前記動物はマウス、ラット又は霊長類から選ばれる少なくとも1種を含む、ことを特徴
とする請求項5に記載の双極性障害疾患動物モデル。 - 双極性障害を治療する医薬品のスクリーニング方法であって、
請求項4に記載の双極性障害細胞モデル又は請求項5又は6に記載の双極性障害動物モ
デルに候補薬を投与するステップと、
Syt7遺伝子又はその発現産物の量を向上させる候補薬、又は双極性障害の症状を軽
減させる候補薬をスクリーニングして、双極性障害を治療する前記医薬品とするステップ
とを含む、ことを特徴とする方法。 - Syt7遺伝子又はその発現産物を特異的に検出する試薬を含む、ことを特徴とするキ
ット。 - 前記試薬はプライマー及び/又はプローブを含む、ことを特徴とする請求項8に記載の
キット。 - 双極性障害を診断する又は双極性障害の予後効果を判断するシステムであって、
生物学的サンプル中のSyt7遺伝子又はその発現産物の量を取得する取得手段と、
前記取得手段に接続され、前記生物学的サンプル中のSyt7遺伝子又はその発現産物
の量に基づいて、双極性障害を診断する又は双極性障害の予後効果を判断する判断手段と
を含む、ことを特徴とするシステム。 - 前記判断手段は、さらに、
前記生物学的サンプル中のSyt7遺伝子又はその発現産物の量が正常検体中のSyt
7遺伝子又はその発現産物の量未満であることが、前記生物学的サンプルが双極性障害に
罹患していることを示すか、又は
前記生物学的サンプル中のSyt7遺伝子又はその発現産物の量が少なくとも正常検体
中のSyt7遺伝子又はその発現産物の量以上であることが、前記双極性障害患者の予後
効果が良好であることを示すことを含む、ことを特徴とする請求項10に記載のシステム
。 - 被検体が双極性障害に罹患しているか否かの診断方法であって、前記被検体由来の生物
学的サンプル中のSyt7遺伝子及び/又はその発現産物を検出するステップを含む、こ
とを特徴とする方法。
