Abstract
Description

ヒト癌は多数の遺伝的および後成的改変を抱え、免疫系により認識可能である可能性のあるネオ抗原を産生する(Sjoblom et al., (2006) Science 366:268-74)。TおよびBリンパ球からなる適応免疫系は、多様な腫瘍抗原に応答する広い能力および精巧な特異性を備え、強力な抗癌能を有する。さらに、免疫系は、相当な柔軟性および記憶成分を示す。適応免疫系の全てのこれらの特質の利用の成功は、免疫療法を全癌処置モダリティの中で特別のものとする。

本発明の方法に使用するのに適する抗PD-1抗体は、PD-1に高特異性および親和性で結合し、PD-L1の結合を遮断し、PD-1シグナル伝達経路の免疫抑制作用を阻止する、抗体である。ここに記載する治療方法の何れにおいても、抗PD-1または抗PD-L1“抗体”は、それぞれ、PD-1またはPD-L1受容体に結合し、リガンド結合阻止および免疫系上方制御において抗体全体に類する機能的性質を示す、抗原結合部分を含む。ある実施態様において、抗PD-1抗体またはその抗原結合部分は、ヒトPD-1への結合についてニボルマブと交差競合する。他の実施態様において、抗PD-L1抗体またはその抗原結合フラグメントは、ヒトPD-L1への結合についてBMS-936559、MPDL3280A、MEDI4736またはMSB0010718Cと結合を競合する。

自己幹細胞移植(ASCT)および続くブレンツキシマブ ベドチンの不成功後のニボルマブ単剤療法でのcHLの処置
フェーズ2治験において、ASCTおよび続くブレンツキシマブ ベドチン不成功後のcHLを有する患者におけるニボルマブ単剤療法の有効性および安全性を決定した。本治験のプライマリーエンドポイントは、独立放射線審査委員会(IRRC)により評価される客観的奏効率(ORR)を含む。セカンダリーエンドポイントは、1)IRRC評価完全寛解(CR)、部分寛解(PR)および応答の期間(DOR);2)治験医評価ORRおよびDOR;3)安全性;および4)クオリティ・オブ・ライフ(QoL)を含む。
治験設計
本治験は、欧州、カナダおよび米国の34治験施設からのcHLを有する患者における進行中の、多施設-非比較、多コホート、フェーズ2登録治験の一部であった。
患者は、ニボルマブ処置に関連する有効性、安全性およびクオリティ・オブ・ライフ(QoL)について評価された。
最初に計画された治験のサンプルサイズは、40%のORRを仮定し、5%の2側アルファを考慮して、ORRが≦20%であるヌル仮説を排除するための約93%検出力を提供するであろう60患者であった。しかしながら、80患者が治験医からの高度の要請および高率のスクリーニング不成功の可能性を考慮して、登録された。
患者
計80のcHLを有する患者が登録され、ニボルマブ処置を受けた。登録患者のベースライン特性は、表2に提供される。データベースロック時、80患者中51患者(64%)がニボルマブ処置のままであった。受けた回数の中央値は17(範囲3~25)であった。ニボルマブ処置を中止した29患者のうち、中止の主理由は、疾患進行(n=13)およびSCT(同種SCT、n=5;自己SCT、n=1)を含んだ。移植時、最良応答はCR(n=1)、PR(n=3)およびSD(n=2)であった。データカットオフ時、全患者が生存していた。

aASCT前の高用量予備レジメンを除く
客観的奏効率(ORR)
IRRCおよび治験医により評価されたORRは、表3に提供される。最初の客観的応答までの期間中央値は2.1か月(範囲1.6~5.7か月)であった。IRRCにより評価して、80患者中53患者(66%)は腫瘍負荷が減少し、7患者は完全寛解を示した。治験医評価下、58患者(73%)は腫瘍負荷減少を示し、これらの患者中、22患者が完全寛解を有した。完全寛解におけるこの不一致は大部分FDG-PET走査解釈に基づき、3/19治験医評価完全寛解がIRRCにより少なくとも部分寛解として評価されたため、臨床活性解釈に意味があるように影響するとは考えなかった。IRRC ORRは、直近のブレンツキシマブ ベドチンに先に応答しなかった患者の72%(n=43)であった。

8.9か月の中央値フォローアップ後、DOR中央値は7.8か月(95%CI6.6~推定不可能(NE))であった。
解析時、IRRCにより、62%の応答者がニボルマブ処置に応答し続けたと評価された(図2A参照)。1名を除き全員腫瘍負荷のベースラインからの≧50%の減少を有した(図2B参照)。この患者はFDG-PET走査陰性を示した。
図3に示すとおり、ニボルマブ処置患者のPFS中央値は10か月であった。6か月で、PFSは77%(95%CI65%~85%)であり、OS率は99%(95%CI91%~100%)であった。
原因を問わない有害事象(AE)が80患者中79患者(99%)で報告され、重度AE(グレード3~4)が20患者(25%)で報告された。治験薬関連AEは72患者で報告された(表4参照)。間質性肺炎が、最初の投与から最後の投与の35日後に2患者(3%;グレード1/2およびグレード3)で見られ、これは、治験薬関連と考えられた。

ニボルマブでの処置の時間と共に、平均EQ-5D可視アナログスコアは増加した。EORTC QLQ-C30はまた機能的スコア、症状スコアおよび全体的健康スコアにわたるベースラインからの改善の傾向を示した。
ASCTおよびブレンツキシマブ ベドチンに不成功のcHLを有する患者におけるこの登録治験において、ニボルマブ単剤療法は、次の観察をもたらした。1)66.3%の患者がIRRC評価により客観的奏効率(ORR)を示した;2)応答の予備的持続性は励みになり、62%の患者がデータカットオフ時に応答したままであり(応答期間の中央値は7.8か月)、PFS中央値は10.0か月であった;および3)大部分グレード1または2 AEであり、固形腫瘍に対する新規安全性懸念なく、許容される安全性プロファイルであった。
新たに診断された古典的ホジキンリンパ腫を有する患者におけるニボルマブ含有レジメンのフェーズ2治験
目標
この治験の主目標は、新たに診断された進行期cHLを有する患者における、ニボルマブ単剤療法、続くAVD(ドキソルビシン、ビンブラスチンおよびダカルバジン)と組み合わせたニボルマブの全体的安全性および忍容性の評価である。これは、最初の投与と、最後の投与後30日の間における≧グレード3~5処置関連AEを経験した患者として評価した。
これは、新たに診断された進行期cHLを有する患者における、ニボルマブ単剤療法、続くニボルマブとAVDとの組み合わせの安全性および忍容性を評価するための、シングルアーム、オープンラベル、フェーズ2治験である。治験は、北米およびEUの8カ国で、予定された登録は、50名の新たに診断された進行期cHLを有する患者で実施される。

Claims (15)
- ホジキンリンパ腫に由来する腫瘍を有する対象を処置する方法に使用するための、モノクローナルプログラム死-1受容体(PD-1)に特異的に結合する抗体またはその抗原結合部分(“抗PD-1抗体”)であって、対象が治療有効量の該抗PD-1抗体を投与され、ここで、対象は(i)自己幹細胞治療、(ii)ブレンツキシマブ ベドチンおよび(i)と(ii)の両方から選択される先の処置に応答性ではなかったものである、抗PD-1抗体。
- (i)対象が先の処置後部分奏効の達成に不成功であった、(ii)対象が先の処置後の完全奏功後に再発したおよび/または(iii)対象が先の処置後の部分奏効または疾患安定後、疾患進行した、請求項1に記載の使用のための抗PD-1抗体。
- 対象がさらにドキソルビシン、ビンブラスチンおよびダカルバジンを投与される、請求項1または2に記載の使用のための抗PD-1抗体。
- 対象がブレオマイシンを投与されない、請求項1~3の何れかに記載の使用のための抗PD-1抗体。
- ドキソルビシン、ビンブラスチンおよびダカルバジンが約1週、2週、3週または4週に1回投与される、請求項3または4に記載の使用のための抗PD-1抗体。
- ドキソルビシンが約10mg/m2~約40mg/m2、約10mg/m2~約30mg/m2または約20mg/m2~約30mg/m2の用量で投与される;ビンブラスチンが約0.1mg/m2~約10mg/m2、約1mg/m2~約10mg/m2または約5mg/m2~約10mg/m2の用量で投与される;および/またはダカルバジンが約200mg/m2~約500mg/m2、約250mg/m2~約500mg/m2または約300mg/m2~約400mg/m2の用量で投与される、請求項3~5の何れかに記載の使用のための抗PD-1抗体。
- 約2週に1回、ドキソルビシンが25mg/m2の用量で投与され、ビンブラスチンが6mg/m2の用量で投与され、ダカルバジンが375mg/m2の用量で投与される、請求項3~5の何れかに記載の使用のための抗PD-1抗体。
- (i)抗PD-1抗体がヒトPD-1への結合についてニボルマブと交差競合する;(ii)抗PD-1抗体がニボルマブと同じエピトープに結合する;(iii)抗PD-1抗体がニボルマブである;または(iv)抗PD-1抗体がペンブロリズマブである、請求項1~7の何れかに記載の使用のための抗PD-1抗体。
- (i)抗PD-1抗体が約1週、2週または3週に1回、少なくとも約0.1mg/kg~少なくとも約10.0mg/kg体重の範囲の用量で投与される;(ii)抗PD-1抗体が少なくとも約3mg/kg体重の用量で、約2週に1回投与される;または(iii)抗PD-1抗体が、所望により、約1週、2週、3週または4週に1回少なくとも約200mg、少なくとも約220mg、少なくとも約240mg、少なくとも約260mg、少なくとも約280mg、少なくとも約300mg、少なくとも約320mg、少なくとも約340mg、少なくとも約360mg、少なくとも約380mg、少なくとも約400mg、少なくとも約420mg、少なくとも約440mg、少なくとも約460mg、少なくとも約480mg、少なくとも約500mgまたは少なくとも約550mgの均一用量で投与される、請求項1~8の何れかに記載の使用のための抗PD-1抗体。
- ホジキンリンパ腫が9p24.1に遺伝子変化を有する、請求項1~9の何れかに記載の使用のための抗PD-1抗体。
- ホジキンリンパ腫がPD-L1および/またはPD-L2を発現する、請求項1~10の何れかに記載の使用のための抗PD-1抗体。
- PD-L1および/またはPD-L2発現が少なくとも約1%、少なくとも約5%または少なくとも約10%である、請求項11に記載の使用のための抗PD-1抗体。
- 対象が投与開始後少なくとも約1か月、少なくとも約2か月、少なくとも約3か月、少なくとも約4か月、少なくとも約5か月、少なくとも約6か月、少なくとも約7か月、少なくとも約8か月、少なくとも約9か月、少なくとも約10か月、少なくとも約11か月、少なくとも約1年、少なくとも約18か月、少なくとも約2年、少なくとも約3年、少なくとも約4年または少なくとも約5年無進行生存を示す、請求項1~12の何れかに記載の使用のための抗PD-1抗体。
- 対象が初期投与後少なくとも約1か月、少なくとも約2か月、少なくとも約3か月、少なくとも約4か月、少なくとも約5か月、少なくとも約6か月、少なくとも約7か月、少なくとも約8か月、少なくとも約9か月、少なくとも約10か月、少なくとも約11か月、少なくとも約1年、少なくとも約18か月、少なくとも約2年、少なくとも約3年、少なくとも約4年または少なくとも約5年の全生存期間を示す、請求項1~14の何れかに記載の使用のための抗PD-1抗体。
- (a)抗PD-1抗体;および
(b)請求項1~14の何れかにより抗PD-1抗体を使用するための指示
を含む、キット。
