Claims (19)
- 近位側でレーザに光学的に接続され、遠位側に光出射口を有する光ファイバーケーブルのための作動チャネルと、前記遠位側で前記光出射口の領域へと開口し、前記近位側で灌注液リザーバと流体連結している灌注液チャネルとを備える内視鏡アセンブリを採用しているレーザ光ベースの砕石術の施術中に温度をモニタリングするためのシステムであって、
前記内視鏡アセンブリと組み合わせることが可能なモジュール部を備え、
前記モジュール部は、
前記灌注液チャネルおよび/または前記灌注液チャネルと流体連結している供給ラインと並んで取り付け可能で、灌注液と非接触で灌注流量を判定する流量センサと、
処理部に接続され、前記光出射口の位置に照射可能なレーザ出力を決定するための基礎となる前記レーザのための動作パラメータを前記処理部に送信可能である入力手段と、
前記灌注液チャネルおよび/または前記灌注液チャネルと流体連結している供給ラインと並んで取り付け可能で、前記灌注液と非接触で前記灌注液の温度を判定する温度センサと、
前記処理部に取り付けられ、少なくとも前記灌注流量と、前記灌注液の温度(TS)と、前記決定された照射レーザ出力とに基づいて、前記光出射口の位置での前記レーザ光ベースの砕石術中に体内で発生する温度Tを数値的に決定する分析部と、
前記決定された温度Tを閾値と比較し、前記閾値を上回った場合、信号を生成する比較部と、
を備える、システム。 - 前記流量センサは、超音波センサの形態を有する、請求項1に記載のシステム。
- 前記入力手段は、手動で操作可能なインタフェースまたはデータインタフェースである、請求項1または請求項2に記載のシステム。
- 前記レーザの動作パラメータは、レーザパルス周波数および個別パルスエネルギーを含む指定可能なパラメータである、請求項1から3のいずれか一項に記載のシステム。
- 前記温度センサは、赤外線センサの形態を有する、請求項1から6のいずれか一項に記載のシステム。
- 前記システムは、前記比較部によって生成された前記信号を触覚的、聴覚的および/または視覚的に認知可能にする生理学的認知可能信号部を備える、請求項1から7のいずれか一項に記載のシステム。
- 前記灌注液チャネルおよび/または前記灌注液リザーバに熱的に結合される冷却部を備え、
前記冷却部は、前記比較部により生成された前記信号によって起動または制御されることが可能である、請求項1から7のいずれか一項に記載のシステム。 - 前記比較部により生成される前記信号によって起動可能な緊急停止スイッチを備える、請求項1から8のいずれか一項に記載のシステム。
- 前記レーザの動作を記録するマイク信号を生成するためにマイク部を備え、
前記マイク部は前記分析部に接続されている、請求項1から9のいずれか一項に記載のシステム。 - 前記システムは、レーザ光ベースの砕石術の施術に適し、使用中の内視鏡アセンブリに取り付けるための後付けキットとして構成されている、請求項1から10のいずれか一項に記載のシステム。
- 前記作動チャネルおよび前記灌注液チャネルは、前記作動チャネルに並んで配置されている前記光ファイバーケーブルが前記灌注液によって直接的に灌流されるように1つのチャネルとして構成されている、請求項1から11のいずれか一項に記載のシステム。
- 前記内視鏡アセンブリは、前記砕石術の施術中に患者の外部に残留する体外領域を有し、
前記モジュール部の前記温度センサおよび前記流量センサは、前記灌注液チャネルまたは前記灌注液チャネルに流体連結されている供給ラインに並ぶ前記内視鏡アセンブリの前記体外領域に取り付けられている、請求項1から12のいずれか一項に記載のシステム。 - 前記灌注液リザーバ内の灌注液の充填レベルを演算またはセンサによって直接検出するためのユニットが備えられ、
前記ユニットは、前記充填レベルに依存する信号を少なくとも1つ生成する、請求項1から13のいずれか一項に記載のシステム。 - 前記ユニットは、前記灌注液リザーバに備えられているセンサおよび/または前記処理部により実装されるソフトウェアベースの分析アルゴリズムであり、前記センサにより検出された前記灌注流量および灌注液が放出される時間の計測長に基づいて前記充填レベルを決定する、請求項14に記載のシステム。
- 前記信号は、請求項7に記載の前記信号部によって、またはさらなる信号手段によって、触覚的、聴覚的および/または視覚的に認知可能に生成可能である、請求項14または請求項15に記載のシステム。
- 前記センサにより検出された前記灌注液の温度(TS)は、前記比較部または前記灌注液の温度(TS)が臨界温度値を下回るまたは上回る場合に信号を生成するさらなる比較部に提供されることが可能である、請求項1から16のいずれか一項に記載のシステム。
- 前記灌注液チャネルは、前記近位側において、電気的に動作可能な灌注液供給部に接続される、請求項1から17のいずれか一項に記載のシステム。
- 前記灌注液供給部は、前記光出射口の位置で発生する前記温度Tおよび/または前記レーザの少なくとも1つの動作パラメータに応じて前記灌注液流量を制御することが可能な手段を提供する、請求項18に記載のシステム。
