Abstract
Description
1、プラスミノーゲン活性化経路の成分、プラスミノーゲンを直接活性化し得るか、またはプラスミノーゲン活性化経路の上流成分を活性化することによって間接的にプラスミノーゲンを活性化し得る化合物、プラスミノーゲンまたはプラスミンの活性を模倣する化合物、プラスミノーゲンまたはプラスミノーゲン活性化剤の発現をアップレギュレートすることができる化合物、プラスミノーゲン類似体、プラスミン類似体、tPAまたはuPA類似体および線維素溶解阻害剤の拮抗剤から選択される1つ以上の有効量のプラスミノーゲン経路活性剤を、筋萎縮性側索硬化症(ALS)に罹患している被験者に投与することを含む、筋萎縮性側索硬化症(ALS)を治療する方法。

プラスミノーゲンは治療の用途に用いられるために、自然界から分離及び精製されるものでもよく、標準的な化学ペプチド合成技術によって合成することでもよい。化学的手法によりポリペプチドを合成する際、液相または固相で合成を行うことができる。固相ポリペプチド合成(SPPS)(配列のC末端アミノ酸を不溶性支持体に附着させ、順番に配列中の残りのアミノ酸を添加する)はプラスミノーゲンの化学的合成に適したものである。各種形式のSPPS、例えばFmoc及びBocは、プラスミノーゲンの合成に用いることができる。固相合成に用いられる技術は以下に記載されている:Barany及びSolid-Phase Peptide Synthesis;3-284ページ、The Peptides:Analysis,Synthesis,Biology.第二巻:Special Methods in Peptide Synthesis,Part A.,Merrifield,tら J.Am.Chem.Soc.,85:2149-2156(1963);Stewartら,Solid Phase Peptide Synthesis,2nd ed.Pierce Chem.Co.,Rockford,Ill.(1984);及びGanesan A.2006Mini Rev.Med Chem.6:3-10及びCamarero JAら 2005Protein Pept Lett.12:723-8。簡単に言えば、その上にペプチド鎖が構築されている機能性ユニットにより小さい不溶性の多孔ビーズを処理する。カップリング/脱保護の繰り返し循環後に、附着した固相の遊離N末端アミンと単一のN保護を受けているアミノ酸ユニットをカップリングさせる。それから、該ユニットを脱保護し、他のアミノ酸と接続する新しいN末端アミンを露出させる。ペプチドを固相上に固定したままにし、それからそれを切除する。

本発明の一つの実施形態は製品または薬物キットに係るものであり、糖尿病に起因する心血管疾患および関連疾患を治療するための本発明のプラスミノーゲンまたはプラスミンを含有する。前記製品は好ましくは一つの容器、ラベルまたはプロトコルを含む。適切な容器はボトル、バイアル、注射器などである。容器は各種材料例えばガラスまたはプラスチックから作られることができる。前記容器は組成物を含有し、前記組成物は本発明の疾患または症状を有効に治療し且つ無菌の入口を有する(例えば前記容器は静脈輸液用パックまたはバイアルであり、皮下注射針によって貫通される栓を含む)。前記組成物中の少なくとも一種類の活性化剤がプラスミノーゲン/プラスミンである。前記容器上にあるまたは添付されているラベルは前記組成物を本発明の糖尿病に起因する心血管疾患および関連疾患の治療に用いられると説明するものである。前記製品はさらに薬用緩衝液を含有する第二容器を含み、前記薬用緩衝液は例えばリン酸塩緩衝の食塩水、リンガー溶液及びグルコース溶液を含む。さらには商業及び使用者の角度から見ると必要とされるその他の物質、即ちその他の緩衝液、希釈剤、濾過物、針及び注射器を含むことができる。また、前記製品は使用説明を有するプロトコルを含み、これは例えば前記組成物の使用者にプラスミノーゲン組成物及び疾患の治療に伴うその他の薬物を患者に投与することを指示するものである。
実施例1は、プラスミノーゲンが筋萎縮性側索硬化症のモデルマウスの寿命と生存期間中央値を延長ことに関するものである。
本実施例に使用されているヒトプラスミノーゲンはドナー血漿に由来し、文献[1-3]に記載されている方法に基づいてプロセスを最適化し、精製して得られた。ヒトプラスミノーゲン単体の純度は95%を上回った。以下の実施例でも同じである。
トランスジェニック突然変異SOD1は、孤発性および家族性筋萎縮性側索硬化症(Amyotrophic Lateral Sclerosis、ALS)の臨床で観察される組織病理学的特徴を持っている。本願のALSモデルマウスはB6.Cg-Tg(SOD1-G93A)1Gur/Jトランスジェニックマウス(略してSOD1-G93A)であり、Jackson研究所から購入し、SPFレベルの環境で動物関連の実験を行った。SOD1-G93Aモデルマウスでは、約100日目に後肢振戦が現れ、その後急速に悪化し、50%生存率は157.1±9.3日であった[4]。現在、ALSのメカニズムの研究や新薬開発の前臨床試験研究に広く利用されている。
16週齢のオスSOD1-G93Aマウス9匹を取り、体重によってマウスをランダムに2群に分け、溶媒対照群で5匹、プラスミノーゲン投与群で4匹とした。溶媒対照群マウスに0.1ml/日で溶媒(PBS、pH7.4)を尾静脈注射により投与し、プラスミノーゲン投与群マウスに1mg/0.1ml/匹/日でプラスミノーゲンを尾静脈注射により投与し、マウスの生存を毎日観察して記録した。
その結果、プラスミノーゲン投与群のマウスの平均寿命は164±8.6日であり、溶媒対照群のマウスの平均寿命は153±0日であり、溶媒対照群と比べて、プラスミノーゲン投与群の寿命は約11日長かった(図1A)。プラスミノーゲン投与群のマウスの生存期間の中央値は53±9日であり、溶媒対照群の生存期間の中央値は40±0日であり、溶媒対照群と比べて、プラスミノーゲン投与群の生存期間の中央値は約13日長く、約30%延長された(図1B)。この結果は、プラスミノーゲンがALSマウスの寿命と生存期間の中央値を延長できることを示している。
実施例2は、プラスミノーゲンがALSモデルマウスの神経筋機能を改善することに関するものである。
16週齢のオスSOD1-G93Aマウス9匹を取り、体重によってマウスをランダムに2群に分け、溶媒対照群で5匹、プラスミノーゲン投与群で4匹とした。溶媒対照群マウスに0.1ml/日で溶媒(PBS、pH7.4)を尾静脈注射により投与し、プラスミノーゲン投与群マウスに1mg/0.1ml/匹/日でプラスミノーゲンを尾静脈注射により投与し、34日間連続して投与した。投与し始めた日を0日目とし、投与の6、8、12、16、21、23、27、30、34日目に次のようにサスペンショングリップ強度テストを行い、ALSモデルマウスの神経筋機能に対するプラスミノーゲンの効果を考察し、次のようにALSマウスの神経行動を統計分析した。
サスペンショングリップ強度テスト
サスペンション実験は、一般的にマウスの運動能力(筋力)を評価するために使用される。マウスケージの金属製のふたの上に1匹のマウスを置き、ふたを軽く振ってマウスにふたをつかませてから、ふたをひっくり返す。そして、マウスの後肢がふたを放すまでの潜伏時間を記録する[5]。各マウスに対して3回の実験を行ない、1回の実験の最長期間は90秒であり、各マウスの最長の潜伏時間を統計分析に使用する。
その結果、2つの群のマウスのサスペンション潜伏時間は投与期間中に減少したが、プラスミノーゲン投与群のマウスのサスペンション潜伏時間は常に溶媒対照群のマウスよりも長かく、しかも投与の6、21、および23日目に、溶媒群と比べて、プラスミノーゲン投与群のサスペンション潜伏時間は統計学的に有意または非常に有意であり、P値はそれぞれ0.03、0.02、および0.008であった(図2)。これは、プラスミノーゲンがALSマウスの筋力低下を遅らせることができることを示している。
ALSジスキネジア特性スコア、0点:運動機能障害の兆候なし。1点:尾懸垂中の後肢の著しい震え;2点:歩行異常、75cmの歩行中につま先が少なくとも2回カールするか、または足がケージの底に沿って引きずられる。3点:後肢が少なくとも1日間引きずられ、硬くて弱く(硬い麻痺)、脚を前方に動かすことができない。4点:マウスは仰臥位に置かれ、30秒以内に腹臥位に戻ることができない(死にかけている状態と判断される)[5]。
その結果、プラスミノーゲン投与群のマウスの2点神経行動が現れた時点は、溶媒群より有意に遅く、統計学的差は有意であった(*はP<0.05を表す)(図3)。
上記の結果は、プラスミノーゲンがALS疾患の筋力低下を大幅に遅らせ、ALS疾患の進行を遅らせることができることを示している。
週齢の近い野生型マウス20匹、オスSOD1-G93Aマウス29匹、およびメスSOD1-G93Aマウス31匹を取り、野生型マウスをブランク対照群とし(何ら投与治療はない)、SOD1-G93Aマウスは14週目に後肢振戦が現れた時点から観察記録し、各マウスの発症時刻を記録し、発症から14日後に投与を始めた。発症状況によってすべてのマウスをランダムに溶媒対照群とプラスミノーゲン投与群に分け、溶媒対照群マウスが32匹であり、毎日0.1ml/匹で溶媒(10mMクエン酸ナトリウム緩衝液、pH7.4)を尾静脈注射により投与し、プラスミノーゲン投与群で28匹、毎日1mg/0.1ml/匹でプラスミノーゲンを尾静脈注射により投与し、SPF環境下で35日間連続して投与した。投与し始めた日を0日目とし、投与期間中3日ごとに体重を1回測定し、ALSモデルマウスの体重減少に対するプラスミノーゲンの影響を調べた。
ALSは通常、大幅な体重減少が伴われ、ALSの主な特徴の1つである[5]。毎回体重測定の結果は、0日目の体重で標準化され、つまり、毎回体重測定値/0日目の体重*100で処理された。
その結果、投与中、ブランク対照群のマウスの体重はあまり変動することなく徐々に上昇する傾向があり、溶媒対照群のマウスの体重は徐々に減少し、プラスミノーゲン投与群のマウスの体重は、最初の25日間で大きく変動していたが、ブランク対照群のマウスの体重に近いかわずかに多く、25日後に体重は徐々に減少していたが、溶媒対照群のマウスの体重よりも常に多く、溶媒対照群と比較して、P値は0.001を下回るか0.001に近かった(図4)。これは、プラスミノーゲンがALSモデルマウスの体重減少率を有意に緩和し、ALSの悪化を遅らせることができることを示している。
実施例4は、プラスミノーゲンがALSモデルマウス脊髄の前角の液胞面積を減少させることに関するものである。
週齢の近い野生型オスマウス5匹、およびオスSOD1-G93Aマウス9匹を取り、野生型マウスをブランク対照群とし、SOD1-G93Aマウスは14週目に後肢振戦が現れた時点から観察記録し、各マウスの発症時刻を記録し、発症から14日後に投与を始めた。発症状況によってすべてのマウスをランダムに溶媒対照群とプラスミノーゲン投与群に分け、溶媒対照群マウスが5匹であり、毎日0.1ml/匹で溶媒(10mMクエン酸ナトリウム緩衝液、pH7.4)を尾静脈注射により投与し、プラスミノーゲン投与群で4匹、毎日1mg/0.1ml/匹でプラスミノーゲンを尾静脈注射により投与し、SPF環境下で連続して投与した。マウスが死にかけていたときに取材し、最長の投与は61日間であった。脊髄を採取し、ホルマリン固定液で固定した。固定後の組織をアルコールで段階的に脱水させ、キシレンで透徹化処理した後にパラフィンで包埋した。切片の厚みは3μmであり、切片を脱パラフィンさせ、さらに浸水して水で1回洗浄し、ヘマトキシリンで10分間染色させた後、流水で5分間流した。1%塩酸エタノールで10秒分別させ、流水で10分間流し、0.2%エオシンで10秒染色させ、さらに段階的に脱水させて透徹化して封入させた。切片を光学顕微鏡下で400倍にて観察した。
脊髄の前角における運動ニューロンの変性と死亡は、ALSの主な病理学的特徴の1つである[6]。その結果、ブランク対照群(図5A)マウスの脊髄の前角に一定レベルの液胞面積が示され、溶媒群(図5B)マウスの脊髄の前角の液胞面積がブランク対照群マウスのそれよりも有意に大きく(P<0.001)、投与群(図5C)のマウスの脊髄前角の液胞面積は、溶媒群のマウスよりも有意に低く、しかも統計学的差は非常に有意である(図5D)。これは、プラスミノーゲンがALSモデルマウスの脊髄の前角の空胞面積を減少させ、脊髄の前角の運動ニューロンの死を減少させることができることを示唆している。
実施例5は、プラスミノーゲンがALSモデルマウスの脊髄の前角におけるアセチルコリントランスフェラーゼの発現を促進することに関するものである。
週齢の近い野生型オスマウス5匹、およびオスSOD1-G93Aマウス9匹を取り、野生型マウスをブランク対照群とし、SOD1-G93Aマウスは14週目に後肢振戦が現れた時点から観察記録し、各マウスの発症時刻を記録し、発症から14日後に投与を始めた。発症状況によってすべてのマウスをランダムに溶媒対照群とプラスミノーゲン投与群に分け、溶媒対照群マウスが5匹であり、毎日0.1ml/匹で溶媒(10mMクエン酸ナトリウム緩衝液、pH7.4)を尾静脈注射により投与し、プラスミノーゲン投与群で4匹、毎日1mg/0.1ml/匹でプラスミノーゲンを尾静脈注射により投与し、SPF環境下で連続して投与した。マウスが死にかけていたときに取材し、最長の投与は61日間であった。脊髄を採取し、ホルマリン固定液で固定した。固定後の組織をアルコールで段階的に脱水させ、キシレンで透徹化処理した後にパラフィンで包埋した。組織の切片の厚みは3μmであり、切片を脱パラフィンさせ、さらに浸水して水で1回洗浄した。PAPペンで組織を丸で囲み、3%過酸化水素水で15分間インキュベーションし、0.01M PBSで2回洗い、毎回5分間であった。5%の健常ヒツジ血清液(Vector laboratories,Inc.,USA)で30分間ブロッキングした;時間になった後、ヒツジ血清液を廃棄し、ウサギ抗アセチルコリントランスフェラーゼ抗体(ab178850、Abcam)を滴加して4℃で一晩インキュベーションした後、0.01M PBSで2回洗い、毎回5分間であった。ヤギ抗ウサギIgG(HRP)抗体(Abcam)の二次抗体を室温で1時間インキュベーションし、0.01M PBSで2回洗い、毎回5分間であった。DABキット(Vector laboratories,Inc.,USA)で呈色させ、水で3回洗浄した後にヘマトキシリンで30秒対比染色して、流水で5分間流した。アルコールで段階的に脱水させてキシレンで透徹にし、中性ゴムに封入させ、切片を光学顕微鏡下で400倍にて観察した。
アセチルコリントランスフェラーゼ(chAT)は、コリン作動性ニューロンのマーカー酵素であり、神経細胞で合成される。研究によると、ALS動物モデルの動物脊髄の運動ニューロンおよび患者のchATレベルが低下していることが示されている[7,8]。

実施例6は、プラスミノーゲンがALSモデルマウスの脊髄の前角におけるシナプトフィジンの発現を促進することに関するものである。
週齢の近い野生型オスマウス5匹、およびオスSOD1-G93Aマウス9匹を取り、野生型マウスをブランク対照群とし、SOD1-G93Aマウスは14週目に後肢振戦が現れた時点から観察記録し、各マウスの発症時刻を記録し、発症から14日後に投与を始めた。発症状況によってすべてのマウスをランダムに溶媒対照群とプラスミノーゲン投与群に分け、溶媒対照群マウスが5匹であり、毎日0.1ml/匹で溶媒(10mMクエン酸ナトリウム緩衝液、pH7.4)を尾静脈注射により投与し、プラスミノーゲン投与群で4匹、毎日1mg/0.1ml/匹でプラスミノーゲンを尾静脈注射により投与し、SPF環境下で連続して投与した。マウスが死にかけていたときに取材し、最長の投与は61日間であった。脊髄を採取し、ホルマリン固定液で固定した。固定後の組織をアルコールで段階的に脱水させ、キシレンで透徹化処理した後にパラフィンで包埋した。組織の切片の厚みは3μmであり、切片を脱パラフィンさせ、さらに浸水して水で1回洗浄した。PAPペンで組織を丸で囲み、3%過酸化水素水で15分間インキュベーションし、0.01M PBSで2回洗い、毎回5分間であった。5%の健常ヒツジ血清液(Vector laboratories,Inc.,USA)で30分間ブロッキングした;時間になった後、ヒツジ血清液を廃棄し、ウサギ抗シナプシン抗体(17785-1-AP、Proteintech)をを滴加して4℃で一晩インキュベーションした後、0.01M PBSで2回洗い、毎回5分間であった。ヤギ抗ウサギIgG(HRP)抗体(Abcam)の二次抗体を室温で1時間インキュベーションし、0.01M PBSで2回洗い、毎回5分間であった。DABキット(Vector laboratories,Inc.,USA)で呈色させ、水で3回洗浄した後にヘマトキシリンで30秒対比染色して、流水で5分間流した。アルコールで段階的に脱水させてキシレンで透徹にし、中性ゴムに封入させ、切片を光学顕微鏡下で400倍にて観察した。
シナプトフィジン(synaptophysin)は、軸索の成長とシナプス形成のマーカーとして、シナプス前膜上のリン酸化タンパク質であり、シナプスの柔軟性と密接に関連している。ALSモデルマウスは、臨床症状が現れる前に、明らかなシナプス変性と運動ニューロン細胞体の喪失を示す[9]。
その結果、ブランク対照群(図7A)マウスの脊髄前角で一定量のシナプトフィジンが発現し、溶媒群(図7B)マウスのシナプトフィジンの発現レベルはブランク対照群マウスよりも有意に低く、投与群(図7C)マウスの脊髄前角のシナプトフィジン発現は溶媒群マウスよりも有意に高く、統計学的差は有意であった(P<0.05)(図7D)。これは、プラスミノーゲンがモデルマウスの脊髄前角におけるシナプトフィジンの発現を促進し、シナプス損傷の修復を促進できることを示している。
実施例7は、プラスミノーゲンがALSモデルマウスの脊髄の前角の炎症の修復を促進することに関するものである。

イオン化カルシウム結合アダプター分子-1(Ionized calcium binding adaptor molecule-1、Iba-1)は、中枢神経系のミクログリアの表面マーカーである。中枢神経系の免疫細胞として、ミクログリアは、その病変または損傷した時に神経障害をすばやく感知して活性化される。活性化されたミクログリアは、数と形態に大きな変化があり、損傷部位に移動して、死細胞の食作用や炎症性サイトカインの産生の増加など、さまざまな機能を発揮する[10]。
その結果、ブランク対照群(図8A)マウスの脊髄前角で一定レベルのIba-1が発現し、投与群(図8C)マウスの脊髄前角におけるIba-1の発現レベルは溶媒群(図8B)とブランク対照群マウスよりも有意に高く、統計学的差は有意であった(P<0.05または0.01)(図8D)。これは、プラスミノーゲンがモデルマウスの脊髄前角の炎症の修復を促進できることを示している。
実施例8は、プラスミノーゲンがALSモデルマウスの筋萎縮を改善することに関するものである。

その結果、ブランク対照群(図9A)マウスの腓腹筋線維は構造が完全であり、形状とサイズが比較的に均一であるに対し、溶媒群(図9B)の腓腹筋線維は、局所的な炎症性細胞浸潤(赤い矢印)および筋線維の真円度の変化を伴う重度の萎縮を示す。投与群(図9C)の筋線維萎縮は、溶媒群よりも重症度は低いが、炎症性細胞浸潤もあった。これは、プラスミノーゲンがモデルマウスの筋萎縮を改善できることを示している。
実施例9は、プラスミノーゲンがALSモデルマウスの筋萎縮を改善することに関するものである。

実施例10は、プラスミノーゲンがALSモデルマウスの脊髄の前角におけるSMNタンパク質の発現を促進することに関するものである。

生存運動ニューロン(Survival Motor Neuron、SMN)タンパク質に関する研究は、SOD1-ALSモデルの生存運動ニューロン(Survival Motor Neuron、SMN)タンパク質のレベルが低下し、SMNタンパク質の増加が疾患の表現型を改善できることを示している[11]。
その結果、投与群(図11B)マウスの脊髄前角におけるSMNタンパク質の発現レベルは、溶媒群(図11A)よりも有意に高い。これは、プラスミノーゲンがモデルマウスの脊髄前角におけるSMNタンパク質の発現を促進できることを示している。

Claims (15)
- 筋萎縮性側索硬化症(ALS)に罹患している被験者に治療有効量のプラスミノーゲン経路活性化剤を投与することを含む、筋萎縮性側索硬化症(ALS)を治療する方法。
- 前記プラスミノーゲン経路活性化剤が前記筋萎縮性側索硬化症(ALS)に罹患している被験者に対して、寿命と生存期間の中央値の延伸、筋肉の萎縮と筋力の低下の遅延、体重減少の速度の緩和、脊髄の前角における細胞損傷、変性および壊死の低減、脊髄の前角におけるchATの合成の促進、コリン作動性ニューロン機能の回復の促進、脊髄の前角におけるシナプトフィジンの発現の促進、脊髄の前角におけるSMNタンパク質の発現の促進、脊髄の前角の炎症の修復の促進、およびシナプス損傷の修復の促進から選択される1つ以上の活性を有する、請求項1に記載の方法。
- 前記プラスミノーゲン経路活性化剤が、被験者の筋萎縮、筋力低下、けいれん、および/または線維束性収縮の症状を改善する、請求項1に記載の方法。
- 前記プラスミノーゲン経路活性化剤が、被験者の体重減少を軽減し、および/または生存期間を延長する、請求項1に記載の方法。
- 前記プラスミノーゲン経路活性化剤が、被験者の筋緊張を改善する、請求項1に記載の方法。
- 前記プラスミノーゲン経路活性化剤が、被験者の筋肉機能の回復を促進する、請求項1に記載の方法。
- 前記プラスミノーゲン経路活性化剤が、被験者の脊髄前角のニューロン損傷の修復を促進する、請求項1に記載の方法。
- 前記プラスミノーゲン経路活性化剤が、1つ以上の他の薬剤および/または治療方法と組み合わせて投与される、請求項1~7のいずれか1項に記載の方法。
- 前記プラスミノーゲン経路活性化剤が、静脈内、皮下、筋肉内、髄腔内、鼻吸入、エアロゾル吸入、点鼻薬または点眼薬の投与によって投与される、請求項1~8のいずれか1項に記載の方法。
- 前記プラスミノーゲン経路活性化剤が、プラスミノーゲン活性化経路の成分である、請求項1~9のいずれか1項に記載の方法。
- 前記プラスミノーゲン活性化経路の成分がプラスミノーゲンである、請求項10に記載の方法。
- 前記プラスミノゲンが、配列2、6、8、10または12と少なくとも80%、85%、90%、95%、96%、97%、98%または99%の配列同一性を有し、且つ依然としてプラスミノーゲン活性を有する、請求項11に記載の方法。
- 前記プラスミノーゲンがプラスミノーゲン活性フラグメントを含み、且つ依然としてプラスミノーゲン活性および/またはリジン結合活性を有するタンパク質である、請求項11に記載の方法。
- 前記プラスミノーゲンがGlu-プラスミノーゲン、Lys-プラスミノーゲン、ミニプラスミノーゲン、マイクロプラスミノーゲン、delta-プラスミノーゲンまたはそれらの、プラスミノーゲン活性を保持した変異体から選択されるものである、請求項11に記載の方法。
- 前記プラスミノーゲンが、天然または合成のヒトプラスミノーゲン、または依然としてプラスミノーゲン活性および/またはリジン結合活性を保持した変異体若しくはフラグメントである、請求項11に記載の方法。
