NMOSDの治療を必要としている患者を治療する方法に用いられ得るVIB551の用量(ここで、VIB551用量は、循環CD20+ B細胞の少なくとも90%を少なくとも6ヵ月間枯渇させ且つ患者の感染リスクを高めない)は、約300mgの用量であり得る。約300mgは250mg~350mgの用量であってもよく、それは275~325mgの用量であってもよく、それは290~310mgの用量であってもよく、それは205~305mgの用量であってもよく、又はそれは300mgの用量であってもよい。

概要.主要な組入れ基準は、以下であった:NMOSDの診断2、17及び8.0以下のEDSSスコアを有する成人であって、スクリーニング前1年以内の救命治療(コルチコステロイド静注、免疫グロブリン静注、及び/又は血漿交換)を必要とする少なくとも1回の発作歴又はスクリーニング前2年間での救命治療を必要とする少なくとも2回の発作歴のいずれかがある者。AQP4-IgG血清反応陽性及び血清反応陰性患者が適格であった;血清反応陰性参加者は、Wingerchuk 2006判定基準17を満たさなければならなかった。AQP4-IgG血清状態に関して予め計画された募集目標はなかった。募集は、約80%が血清反応陽性、20%血清反応陰性という患者集団の既知の人口統計学を反映するであろうことが仮定された17。全ての参加者が書面によるインフォームドコンセントを提出した。
スクリーニング期間.NMO/NMOSDの診断を有する対象を28日の期間にわたってスクリーニングすることにより、組入れ基準及び除外基準に基づき本研究への参加のその適格性を確立した。適格性判定基準を満たした全ての対象を本研究に無作為化した。
無作為化対照治療期間.スクリーニング期間(56日以下)の後、適格参加者を中央音声/ウェブ自動応答システムにより、それぞれ静脈内VIB551 300mg又はプラセボに3:1で無作為化し(順列ブロック無作為化スキーム)、1日目及び15日目に投与した。参加者、治験責任医師、治験依頼者、判定委員会、及び患者治療又は臨床評価に関与するスタッフは、EDSS評価者を含め、受ける治療に関してマスク化/盲検化され、外観上、VIB551とプラセボとは区別不能であった。
視神経脊髄炎スペクトラム障害発作の判定プロセス。規定の発作診断基準の統一された適用を確実にするため、発作診断及びリアルタイム判定の詳細なプロセスを開発し、それに従い進行した。表6を参照のこと。患者は、予定された研究来院時の間、及び研究来院の合間に2週間毎の(又は予定された来院日に休んだ場合には)フォローアップの電話連絡で、視神経脊髄炎スペクトラム障害発作に関連する新規又は悪化症状に関してモニタされた。
主要評価項目.1日目から、197日目以前に判定委員会が決定した視神経脊髄炎スペクトラム障害発作が発症するまでの時間(日単位)。発作の定義は、視神経脊髄炎スペクトラム障害発作についてのプロトコルに定義された判定基準のうちの少なくとも1つを満たす視神経脊髄炎に関連する新規症状の存在又は視神経脊髄炎に関連する既存症状の悪化であった。
2015年1月~2018年10月まで、24ヵ国の99ヵ所の参加施設で467例の参加者がスクリーニングされた。そのうち231例が登録し、175例がVIB551(AQP4-IgG血清反応陽性、n=161)に、及び56例がプラセボ(AQP4-IgG血清反応陽性、n=52;図1)に無作為化された。2018年9月7日、252例の参加者/67例の判定で認められた発作数の目標が達成される前に、有効性及び条件付き検出力が99%以上であることが明確に裏付けられたため、データモニタリング委員会が登録の中止を勧告した。2018年9月21日、治験依頼者は登録を中止した後、治療の割付けに関して盲検化したままでデータベースをロックした。
本研究は主要評価項目を達成し、NMOSD発作までの時間について、プラセボと比べてVIB551に有利な有意差があった。全体的に見て、プラセボの投与を受けた56例中22例の参加者(39.3%)と比較して、VIB551の投与を受けた174例中21例の参加者(12.1%)が発作を起こした;相対リスクの低減、72.8%;ハザード比0.272(95%信頼区間[CI]、0.150~0.496;p<0.0001);及び治療必要数3.73(95%CI、3.06~5.66)(図8A)。AQP4-IgG血清反応陽性サブ群では、相対リスクの低減は77.3%であった;ハザード比0.227(95%CI、0.121~0.423;p<0.0001);及び治療必要数3.23(95%CI、2.72~4.54)(図8B)。VIB551治療を続けて1年後、85%の患者はNMOSD発作が起こらなかった。RCPとOLPとを合わせた安全性解析では、VIB551治療期間平均値は1.5年(範囲0.2~3.7)である。
intent-to-treat集団全体及びAQP4-IgG血清反応陽性集団についての主要な副次評価項目を表13に提示する。ベースラインからRCPの最終来院時までにEDSSスコアの悪化が生じた対象の比率は、AQP4-IgG血清反応陽性対象では、プラセボ群よりもVIB551群において一層低かった。この差は統計的に有意であり、VIB551群についてのプラセボ群と比較したオッズ比は0.371であった(95%CI:0.1807、0.7633;p=0.0070)。同様に、ITT集団全体でも、全プラセボ群と比べて全VIB551群において一層低い比率の対象がEDSSスコアの悪化を示した。全VIB551群についての全プラセボ群と比較したオッズ比は0.370であった(95%CI:0.1850、0.7389;p=0.0049)。対象は、以下の判定基準のうちの1つを満たす場合、EDSSスコア全体の悪化が起こっていると見なされた:ベースラインスコアが0の対象について、EDSSスコアの2ポイント以上の悪化;ベースラインスコアが1~5の対象について、EDSSスコアの1ポイント以上の悪化;ベースラインスコアが5.5以上の対象について、EDSSスコアの0.5ポイント以上の悪化。
RCPの間のmRSスコアに基づく治療効果をウィルコクソン-マン・ホイットニーオッズ手法により評価した。mRSは、脳卒中又は他の神経学的身体障害の原因に罹患したことがある人の身体障害の程度又は日常活動の介護依存度の測定によく使用される尺度である。mRSは、症状のない完全な健康から死亡に至るまでの範囲の、以下のとおりの0~6の範囲である:0-症状なし;1-幾つかの症状はあるものの、重大な身体障害はなく、通常の活動は全て行うことができる;2-軽度身体障害、自分のことは介助なしに自分で面倒を見ることができ、しかし以前の活動全てを行うことはできない;3-中等度身体障害、多少の介助が必要であるが、自力歩行ができる;4-中等度から重度の身体障害、介助なしには自身の生理的要求を処理することができず、及び自力歩行が困難である;5-重度身体障害、常時介護する必要があり、寝たきり、失禁状態;6-死亡。
総じて、全ての身体部位について疼痛NRSスコア平均値のベースラインから28週までの変化の平均値は、プラセボ群とVIB551群とで同様であった。しかしながら、AQP4-IgG血清反応陽性対象の中では、及びintent-to-treat集団全体では、VIB551群でプラセボ群よりも小さい下肢痛の平均増加量を示す傾向があった。表14を参照のこと。
B細胞(CD20+、移行、ナイーブ、メモリーB細胞、形質芽球及び形質細胞)、T細胞(CD4+及びCD8+)、及びNK細胞を含む末梢血単核球サブセットを28週間の無作為化対照期間(RCP)を通じてフローサイトメトリー(FACS)によりアッセイした。末梢血B細胞及び形質細胞特異的遺伝子発現シグネチャもまた逆転写酵素qPCR(rt-qPCR)により評価した。
VIB551の投与を受けた参加者の71.8%及びプラセボの投与を受けた参加者の73.2%に有害事象が発生した(表15)。ほとんどの有害事象は、輸注関連反応(プラセボ、10.7%;VIB551、9.2%)を含め、VIB551よりもプラセボでより多く見られた。尿路感染症、関節痛、背痛、頭痛、転倒、感覚鈍麻、膀胱炎、及び眼痛が、名目上、VIB551でより高頻度であった。VIB551の投与を受けた参加者の4.6%及びプラセボVIB551の投与を受けた8.9%に重篤有害事象が発生した;2例以上の参加者に報告されたものはなく、無作為化対照期間を通じて死亡は発生しなかった。AQP4-IgG血清反応陽性集団の安全性評価項目は、上記に報告したものと同様であった。
臨床試験の一部として、NMOSD患者の縦断的MRIを系統的に実施した。各患者について、ベースライン時、NMOSD発作から8日以内(患者がNMOSD発作を起こした場合)及びRCPの終了時(6.5ヵ月目)に、脳、視神経及び脊髄のMRIを実施した。MRIは、治療について盲検化された2人の独立した神経放射線科医による新規ガドリニウム増強T1(新規Gad-T1)病変の中央読影とした。発作は専門委員会によって判定された。

- 前記NMOSDが非進行性と特定され、及び、前記患者が前記治療に対するレスポンダーと特定される、請求項78又は79に記載の方法。
- 前記NMOSDが進行性と特定され、及び、前記患者が前記治療に対するノンレスポンダーと特定される、請求項78又は79に記載の方法。
- 前記患者が、NMOSD発作を起こしたことがあると診断される、請求項81に記載の方法。
- 以下を含む、NMOSDと診断された患者のNMOSDの治療に好適であると試験薬剤を特定する方法:
前記試験薬剤による治療の最も早くて1ヵ月前に前記患者の第1のMRI病変数を決定すること、
前記試験薬剤による治療の3~24ヵ月後に前記患者の第2のMRI病変数を決定すること;及び
前記第2のMRI病変数が前記第1のMRI病変数と同じか若しくはそれより少ない場合、NMOSDの治療に好適であると前記試験薬剤を特定し、又は、前記第2のMRI病変数が前記第1のMRI病変数を超える場合、NMOSDの治療に好適でないと前記試験薬剤を特定すること。 - 前記試験薬剤による前記治療の最も早くて2週間前に前記患者の前記第1のMRI病変数が決定される、請求項83に記載の方法。
- 前記試験薬剤による治療の6~12ヵ月後に前記患者の前記第2のMRI病変数が決定される、請求項83又は84に記載の方法。
- 前記試験薬剤による治療の約6ヵ月後に前記患者の前記第2のMRI病変数が決定される、請求項85に記載の方法。
- 以下を含む、NMOSDの治療を必要としている患者のNMOSD関連発作を低減する方法:
NMOSDの治療を必要としている患者にVIB551を投与することであって、前記VIB551が6ヵ月毎に300mgの用量で静脈内投与される、こと;及び
前記患者の前記NMOSD関連発作を低減すること。 - 前記300mg VIB551を6ヵ月毎に投与する2週間前に、前記患者に初期300mg VIB551用量が投与される、請求項87に記載の方法。
- 前記初期300mg VIB551用量と共に前記患者に経口コルチコステロイド薬が共投与される、請求項88に記載の方法。
- 前記経口コルチコステロイド薬が少なくとも2週間にわたって連日投与される、請求項89に記載の方法。
- 前記患者の前記NMOSD関連発作を前記低減することが、第1の期間に前記患者に起こるNMOSD関連発作の回数の、第2の期間と比べた低減を含み、
前記第1の期間が初回VIB551用量の投与後に存在し、
前記第2の期間が前記初回VIB551用量の投与前に存在し、及び
前記第1の期間と前記第2の期間とが等しい時間長さである、
請求項87に記載の方法。 - 前記患者の前記NMOSD関連発作を前記低減することが、第1の期間に前記患者に起こるNMOSD関連発作の回数の、第2の期間と比べた低減を含み、
前記第1の期間が前記初期VIB551用量の投与後に存在し、
前記第2の期間が前記初期VIB551用量前に存在し、及び
前記第1の期間と前記第2の期間とが等しい時間長さである、
請求項88に記載の方法。 - 前記第1及び前記第2の期間が6ヵ月である、請求項91又は92に記載の方法。
- 前記第1及び前記第2の期間が12ヵ月である、請求項91又は92に記載の方法。
- 前記第1及び前記第2の期間が18ヵ月である、請求項91又は92に記載の方法。
- 前記第1及び前記第2の期間が24ヵ月である、請求項91又は92に記載の方法。
- 前記第1及び第2の期間に前記患者に起こる前記NMOSD関連発作が、視神経炎、脊髄炎、又は脳幹発作のいずれか1つ以上を含む、請求項91又は92に記載の方法。
- 前記患者に起こる前記NMOSD関連発作の1つ以上が無症候である、請求項97に記載の方法。
