Abstract
Description
本願は、2019年4月10日に出願された米国特許仮出願第62/832,281号の優先権主張出願であり、この米国特許仮出願を参照により引用し、その記載内容全体を本明細書の一部とする。
本発明の一実施形態(図1)に従って脂肪吸引カニューレに連結されている振動器ハンドピースにより得られた摘出効率および脂肪吸引術が施された脂肪の質の科学的評価。この器具をPAL(パワーアシスト脂肪吸引器具)と比較し、そして同一の患者の両側について脂肪吸引術を施した。この実験例では、調和振動ハンドピース器具を一方の側に用い、PALを他方の側に用いた。ほぼ同じ脂肪吸引真空源および摘出カニューレを両方の器具に用い、12分間の脂肪吸引にわたって摘出された脂肪の量を記録し、サンプル組織を幹細胞分析のために実験室に送った。調和振動ハンドピースによる脂肪吸引は、脂肪吸引1分当たりに摘出された脂肪量の観点において、PAL脂肪吸引よりも少なくとも30~45%以上効率的であった。さらに、SVFおよびADSCもまた、PALを用いた場合よりも調和振動ハンドピースを用いて得られたサンプル中の脂肪吸引術が施された脂肪1ミリリットル当たり約30~45%以上であった。
Claims (25)
- 脂肪吸引または体形矯正手技のために患者の組織をほぐす振動手持ち型外科用器械であって、前記器械は、
回転シャフトを備えたモータを有し、
第1の端部および第2の端部を備えた回転可撓性シャフトを有し、前記第1の端部は、前記モータの前記シャフトに作動的に連結され、
前記回転可撓性シャフトの第2の端部に作動的に連結された偏心回転質量体を含む振動アクチュエータを有し、
組織に係合するエンドエフェクタを有し、前記エンドエフェクタは、前記振動アクチュエータに作動的に連結され、前記モータは、前記偏心質量体を回転させ、それにより前記エンドエフェクタが振動して組織をほぐす、外科用器具。 - 前記エンドエフェクタは、前記振動アクチュエータに取り外し可能に取り付けられている、請求項1記載の外科用器具。
- 前記エンドエフェクタは、遠位部分のところに位置する少なくとも1つの開口部および流体を前記患者の体内に注入するか組織を前記患者から吸引するかの一方または両方のためのルーメンを備えたカニューレを有する、請求項1または2記載の外科用器具。
- 前記エンドエフェクタは、前記回転シャフトの長手方向軸線と整列する、請求項1~3のうちいずれか一に記載の外科用器具。
- 前記エンドエフェクタは、前記回転シャフトの前記長手方向軸線からオフセットするとともに前記偏心質量体の回転シャフトからオフセットしている、請求項1~4のうちいずれか一に記載の外科用器具。
- 前記振動アクチュエータは、ハウジング内に収容され、前記エンドエフェクタは、前記ハウジングからオフセットしている、請求項1~5のうちいずれか一に記載の外科用器具。
- 前記モータは、ハウジング内に収容され、前記器械は、前記エンドエフェクタの振動を減衰させるとともに前記モータを振動機構体に連結するよう前記ハウジングと前記振動アクチュエータとの間に延びる減衰機構体をさらに有する、請求項1~6のうちいずれか一に記載の外科用器具。
- 前記可撓性回転シャフトは、前記減衰機構体内に位置決めされた第1のばねおよび前記第1のばねに嵌められた第2の静止ばねを含む、請求項7記載の外科用器具。
- 一端が前記モータの前記シャフトに連結され、他端が前記可撓性シャフトに連結された第1のカップラと、一端が前記可撓性シャフトに連結され、他端が前記回転偏心質量体の回転の中心であるシャフトに連結された第2のカップラとをさらに有する、請求項1~8のうちいずれか一に記載の外科用器具。
- 前記振動アクチュエータの調和振動の振動数または振幅のうちの少なくとも一方を選択的に調節するよう構成されたマイクロコントローラをさらに有する、請求項1~9のうちいずれか一に記載の外科用器具。
- 前記回転偏心質量体は回転時、往復運動を前記エンドエフェクタに与えられた前記振動運動と関連して前記エンドエフェクタに与える、請求項1~10のうちいずれか一に記載の外科用器具。
- 前記回転偏心質量体は、前記エンドエフェクタの往復運動を行わせるよう一端にカム作用面を有する、請求項11記載の外科用器具。
- 前記エンドエフェクタの往復運動を行わせるためにソレノイドをさらに有する、請求項1~12のうちいずれか一に記載の外科用器具。
- 脂肪吸引または体形矯正手技のために患者の組織をほぐす振動手持ち型外科用器械であって、前記器械は、
前記患者の体内への流体注入または前記患者からの物質の吸引のうちの一方または両方のためのルーメンを備えたカニューレと、
モータと、
前記モータに作動的に連結された振動アクチュエータとを有し、前記モータは、前記振動アクチュエータを作動させて振動運動を前記カニューレに与え、その結果、前記振動が多数の軸線で生じるようにする、外科用器械。 - 前記モータは、さらに、前記振動運動と関連して往復運動を前記カニューレにさらに与える、請求項14記載の外科用器具。
- 往復運動を前記カニューレに与えるためのソレノイド装置をさらに有する、請求項14または15記載の外科用器具。
- 前記ソレノイドは、前記振動アクチュエータに連結されている、請求項16記載の外科用器具。
- 前記振動アクチュエータは、偏心回転質量体を有し、前記モータは、前記偏心回転質量体に直接連結されている、請求項14~17のうちいずれか一に記載の外科用器具。
- ユーザの手を保護するための減衰機構体をさらに有し、前記減衰機構体は、受動型コイルを含む、請求項14~18のうちいずれか一に記載の外科用器具。
- 前記カニューレは、前記振動アクチュエータに取り外し可能に取り付けられている、請求項14~19のうちいずれか一に記載の外科用器具。
- 前記振動アクチュエータは、シャフトおよび前記シャフトに回転可能に取り付けられた回転偏心質量体を有し、前記カニューレは、前記シャフトの長手方向軸線からオフセットしている、請求項14~20のうちいずれか一に記載の外科用器具。
- 脂肪吸引を実施しまたは軟組織をほぐす方法であって、
a)モータ、前記モータに作動的に連結された振動アクチュエータ、および前記振動アクチュエータに作動的に連結されたカニューレを有する手持ち型器具を用意するステップと、
b)前記モータを作動させて前記振動アクチュエータの回転を生じさせ、それにより組織内で前記カニューレの振動を行わせるステップと、
c)前記モータを前記振動アクチュエータに連結する減衰機構体によって前記器具のオペレータを振動から遮蔽するステップとを含む、方法。 - 振動により前記軟組織から離れた脂肪を、前記カニューレを通して取り出すステップをさらに含む、請求項22記載の方法。
- 前記カニューレの振動中または振動後、グラフトを前記軟組織中に注入するステップをさらに含む、請求項22または23記載の方法。
- 前記振動の振動数または振幅のうちの少なくとも一方を選択的に調節するステップをさらに含む、請求項22~24のうちいずれか一に記載の方法。
