Abstract
Description
ステントは、プラスチック又は金属で作られている管状のデバイスであり、動物又はヒトの脈管の開存性を確保又は維持する目的で使用される。ステントは、総胆管又は主膵管を治療して閉塞を解除することを目的として、内視鏡的に、又は経皮的な方法によって留置し得る。このデバイスは、胆嚢及び膵臓に貯留する液体のドレナージに効果的に用いられる。入手可能なデバイスのうち、より安価な選択肢はプラスチックステントであるが、その直径は、標準的な内視鏡で対応できる最大許容可能値である12フレンチ(0.156インチ)に限定される。それより大きい脈管のドレナージには、一部のプラスチックステント、又は拡張型金属ステントが使用されることになろう。金属ステントは、バルーン拡張可能なものであってよいし、又は、送達カテーテルから展開されると自己拡張する材料で構成されるものであってよい。いずれのタイプの金属ステントも、ポリマー又はシリコーンで作られているカバー部を伴うものを利用可能することができる。このカバー部は、周囲の脈管腔からの組織の侵入を防止する。
本発明は、コーティング層を含む薬剤溶出ステントシステムであって、該コーティングが、多孔性の表面カバー部と、抗増殖剤、好ましくは大環状トリエン免疫抑制化合物を含む少なくとも1種の治療剤とで構成される上記薬剤溶出ステントシステムを提供する。コーティング層は、放射状に拡張可能な(radial expansion)ステントに、該ステントが該コーティングにより封入されるように適用される。

本明細書で使用される場合、ステントに関連する「コーティングされた」又は「コーティング」という用語は、ステント表面に適用されるカバー部、化合物又は物質を指す。これには、カバー部中又はその表面に存在する薬剤又は治療用化合物、並びに、ステント表面から標的への薬剤の送達を促進する目的でステント表面に適用される任意の他の物質も包含されよう。
本明細書で使用される場合、「ポリマー」という用語は、反復構造単位又はモノマーが共有結合性の化学結合により互いに連結することで全体が構成される物質を指す。
本明細書で使用される場合、「対象」という用語は、ヒトを包含する、アジュバント組成物の投与が望ましい任意の動物を指す。「対象」は、哺乳動物及び非哺乳動物を包含し、その例としては、霊長動物、家畜、コンパニオンアニマル、実験用動物、捕獲された野生動物、爬虫類及び魚類等が挙げられる。

本発明のステントは、好適な生体適合性金属から作製してよく、個体の脈管系に挿入することが可能である。本ステントは、ステンレス鋼、チタン、又は同様に生体適合性を有するこれらの合金等の材料を含む金属ベースを含む。
本発明のステントは、実質的に管状又は円筒状に設計されており、脈管系内又は脈管壁近接部位等の所望の標的に位置決めされると放射状に拡張可能になる。本ステントは、自己拡張型又はバルーン拡張可能であるように形成できる。

(a)表面が多孔性材料である少なくとも1つのシートを用意するステップ、
(b)該少なくとも1つのシートをステントの管腔側又は反管腔側表面の少なくとも一方に載せるステップ、
(c)高温高圧を適用して該ステントを該多孔性材料中に封入するステップであり、それにより、該ステントに多孔性の表面カバー部をもたらすステップ、
(d)該多孔性の表面カバー部に治療剤溶液を適用するステップ、及び
(e)該治療剤溶液が適用された該多孔性の表面カバー部を乾燥させるステップ
を含む、方法を対象とする。
ステントの調製
例A:1つの設計形態において、本発明は、ニッケルチタン合金を含む金属構造体ベースが生体適合性の多孔性メッシュでさらに封入されている拡張可能な人工器官(ステント)を包含する。この人工器官は、展開用バルーンの助けを借りることなく、ヒト又は動物の体温で全径まで自己拡張する。走査型電子顕微鏡による検査下で測定したメッシュ孔径は、1ミクロン~5ミクロンである。この管状の人工器官を、エレクトロスプレー法又は他の手段により平面上に事前にキャスト成形(cast)してから金属製ベアステントへの適用前のサイズにカットしておいた多孔性のPFTE又はポリウレタン材料シート2つの間に挟みこむ。抗増殖薬CRC-015を含むコーティングをアセトンに溶解し、薬剤の適用量を決定する高精度シリンジアプリケーター(precision syringe applicator)を用いてステントに適用する。多孔性のメッシュカバー部は、がん療法の必要条件である、より高用量の薬剤の保持が可能である。次いで、コーティングされたデバイスを室温で24時間乾燥させる。適切な液体溶出媒体中に配置すると、薬剤はメッシュ表面から周囲媒体中に分離する。測定値が長さ40mm、直径7mmの封入されたデバイス例を、4.5mgのCRC-015薬をアセトン溶媒に溶解したものでコーティングした。コーティングされたデバイスを、乾燥させた後、リン酸緩衝液中の18%(w/w)アセトニトリル10mLの入ったガラス容器中に配置した。次いで、この容器を、温度37℃に制御したHeidophモデルPromax1020振盪テーブルに配置し、1分当り200ストロークの振動数で振盪させた。液体媒体の試料を、薬剤含有量についてアッセイした。時間の経過に伴うステントからの薬剤溶出量についての複合曲線から、一貫した値の薬剤が少なくとも60日の期間にわたってステントから放出されることが示されている。
別の設計形態において、コバルトクロム金属メッシュを含むステントを、エレクトロスプレー技術を用いた直接適用によりポリウレタンメッシュでカバーした。乾燥を経たカバードステントは、薬剤適用の準備が整った状態になっている。Sonotek Inc.社製の120kHz Micromistノズルを用いて、CRC-015を含有する薬物製剤を有する10~35ミクロン厚のコーティングを、外側のメッシュ表面上に堆積させる。薬剤でコーティングされた人工器官を、室温で24時間乾燥させた後、圧着装置を用いてバルーンカテーテル上に載せる。ヒトへの使用においては、バルーンの位置を狭窄又は狭窄症の部位に定めて膨らませると、デバイスがその最終径に達するまで展開し、メッシュカバー部に含有されている薬剤が周囲組織中に溶出することが可能になる。
CRC-015をアセトンに溶解して30mg/mLのCRC-015溶液を調製することにより、ポリマーフリーのCRC-015溶液を調製した。次に、パクリタキセルをこの溶液に添加し溶解して、最終的なパクリタキセル濃度を10mg/mLにした。薬剤を配合したこの溶液を用い、開示した方法によりカバードステントをコーティングして、がんにおける2つの異なる代謝過程を阻害することが可能な治療用コンビネーションデバイスを調製する。2種の薬剤を別々又は一緒にカバードステントに適用でき、別々に適用すれば異なる区域で異なる薬剤がもたらされるようになることがわかっている。
CRC-015をアセトンに溶解して20mg/mLのCRC-015溶液を調製することにより、ポリマーを含有するCRC-015溶液を調製した。PLAをこの溶液に添加し溶解して、最終的なPLA濃度を10mg/mLにした。薬剤及びポリマーを配合したこの溶液を用い、先に開示した方法によりカバードステントをコーティングした。ポリマーの添加は、本発明のカバードステントからの薬剤溶出速度をさらに修正するために利用できる。
本明細書において提案の自己拡張型のカバードステントに担持されているCRC-015の抗狭窄性を評価するために、養豚されている豚(farm swine)の表在性大腿動脈(SFA)及び深在性動脈(Profundus Artery)に該ステントを28日間埋め込むことによる試験を実施した。2匹の養豚されている豚[sus scrofa、生後約3カ月]を鎮静、挿管し、イソフルランによる全身麻酔下においた。動物はいずれも、外科手術予定日に2日間先行してアスピリン(80mg)及びPlavix(商標)で前処置し、フォローアップ手順(follow up procedure)の実施までこれを毎日継続した。CRC-015の用量が2.0μg/mm2、直径が6.0mm及び7.0mm、並びに長さが40mmであるカバードステント2種類を、片方の後肢の末梢SFA及び深在性脈管で展開させた。カテーテル及びガイドアクセスには、対側の大腿動脈を用いた。ステントグラフト材料は、2×2 Bioweb Electrospun PTFEであり、賦形剤は含まれていなかった。エレクトロンスパン(electronspun)グラフト材料を高温で加圧して、本明細書において提案のステントを封入した。送達システムはオーバーザワイヤーバルーンタイプであり、デュアルルーメンシャフトを通る0.035”ワイヤーを必要とする。このシャフトは、親水性の表面コーティングを備える。送達システムカテーテルの全長は、150cmであった。剖検に際し、動物は、ヘパリン化の後、ペントバルビタールナトリウムを使用して安楽死させた。脈管系にアクセスし、動脈系にシースを挿入した。生理食塩水を動脈系に注入した。前処置した脈管から流れ出る静脈流出液が透明になり始めたら注入を中止して、処置した動脈を切除した。これらの脈管は、適切にラベル付けし、10%中性ホルマリンで固定し、組織プロセシングのために病理学者に送った。一方の動物では、SFAは閉塞したようであったが、深在性脈管は広く開存していた。2匹目の動物では、SFA及び深在性脈管は両方とも広く開存していた。
本試験の主目的は、CRC-015 DCB(drug coated balloon:薬剤でコーティングされたバルーン)及びCRC-015でコーティングされた胆管ステントグラフトを家畜のブタの胆管で展開することの実現可能性を評価することであった。2匹の養豚されている豚(sus scrofa、生後3カ月以上)を選んだ。その理由は、大きさがヒトに近いことから、ヒトでの試験に使用する器具一式を使用できると考えられたからである。動物は、埋込みが成功した27日後に安楽死させた。評価項目には、管の開存性及び28日時点での炎症応答が含まれた。本明細書において提案のCRC-015 DCB及びCRC 015ステントグラフト送達システムは、十二指腸近くの総胆管に直接的な針穿刺を行うことにより挿入した。穿刺部位は、ステント展開及び送達システム回収の後に縫合した。DCBは、胆管の近位部(胆嚢から近い)で展開し、ステントグラフトは、胆管の遠位部(胆嚢から遠い)で展開した。本試験で使用した被験品(TA:Test Article)は、6mm×40mmのCRC-015DCB、0.035ガイドワイヤー(4μg/mm2)及び6mm×30mmのCRC-015胆管ステントグラフトシステム(4μg/mm2)並びに7mm×30mmのCRC-015胆管ステントグラフトシステム(4μg/mm2)であった。
Claims (15)
- 少なくとも1種の治療剤を標的に送達するために用いられ、前記標的は好ましくは胆管又は膵管である、以下を含む薬剤溶出ステントシステム:
(a)放射状に拡張可能なステント、及び
(b)多孔性の表面カバー部と、抗増殖剤である少なくとも1種の治療剤とにより構成されるコーティング
を含み、前記ステントは前記コーティングにより封入されている。 - 少なくとも1種の治療剤が、大環状トリエン免疫抑制化合物である、請求項1に記載の薬剤溶出ステントシステム。
- 多孔性の表面カバー部が不織布である、請求項1に記載の薬剤溶出ステントシステム。
- 不織布がエレクトロスパン繊維により構成される、請求項5に記載の薬剤溶出ステントシステム。
- エレクトロスパン繊維が、ポリテトラフルオロエチレン(polytetrafiuoroethylene)、フッ素化エチレンプロピレン、Dacron、ポリエチレンテレフタラート、ポリウレタン、ポリカーボネート、ポリプロピレン、Pebax、ポリエチレン、並びに生体ポリマー、例えば、コラーゲン、フィブリン及びエラスチン等を含む又はこれらから成る群から選択されるポリマーから構成される、請求項6に記載の薬剤溶出ステントシステム。
- 多孔性の表面カバー部が表面積の20%~40%の範囲内の多孔度を有する、請求項1に記載の薬剤溶出ステントシステム。
- 多孔性の表面カバー部が反管腔側表面では表面積の20%~40%、管腔側表面では表面積の5%未満の範囲内の多孔度を有する、請求項1に記載の薬剤溶出ステントシステム。
- 少なくとも1種の治療剤が3.0μg/mm2を超える濃度で多孔性の表面カバー部中に存在する、請求項1に記載の薬剤溶出ステントシステム。
- 薬剤溶出ステントシステムを調製するための方法であって、
(a)表面が多孔性材料である少なくとも1つのシートを用意すること、
(b)前記少なくとも1つのシートをステントの管腔側又は反管腔側表面の少なくとも一方に載せること、
(c)高温高圧を適用して前記ステントを前記多孔性材料中に封入し、それにより前記ステントに多孔性の表面カバー部を与えること、
(d)前記多孔性の表面カバー部に治療剤溶液を適用すること、及び
(e)前記治療剤溶液が適用された前記多孔性の表面カバー部を乾燥させること
を含む、上記方法。 - ステップ(c)において、温度が55℃~90℃の範囲であり、圧力が0.13MPa~1.25MPaの範囲である、請求項11に記載の方法。
- ステップ(b)において、シートがステントの管腔側及び反管腔側表面に載せられる、請求項11に記載の方法。
- 多孔性材料のシートをステントの表面上に直接適用することにより、ステップ(a)とステップ(b)とが同時に実施される請求項11に記載の方法。
- 請求項1~10のいずれかに記載の薬剤溶出ステントシステムを胆管又は膵管の中に配置することにより(i)胆管閉塞、又は(ii)膵管閉塞、又は(iii)膵臓がんを処置する方法。
