Abstract
Description
本明細書において、「アセチルコリンエステラーゼ」又は「AChE」は、神経伝達物質として機能するアセチルコリン及びいくつかのその他のコリンエステルの分解を触媒として促進する、EC 3.1.1.7群の加水分解酵素を意味する。
本明細書において、「5-HT4受容体」又は「5-ヒドロキシトリプタミン受容体4」は、HTR4遺伝子によってヒトにおいてコードされ、セロトニン(5-ヒドロキシトリプタミン)に反応してcAMPの産生を刺激するヒトセロトニン受容体のファミリーのメンバーである、タンパク質を意味する。
本明細書において、「神経変性疾患」は、神経細胞の体系的な変性及び脱落に基づく、神経回路ニューラルネットワークの崩壊によって誘導される疾患を意味する。
本発明は、上記の「神経変性疾患」のセクションで規定された神経変性疾患の防止及び/又は治療の際の神経保護剤としての使用のための、上記の「アセチルコリンエステラーゼ阻害剤」のセクションで規定されたアセチルコリンエステラーゼ阻害剤と、上記の「5-HT4受容体アゴニスト」のセクションで規定された5-HT4受容体アゴニストと、を含む医薬組成物に関する。

この実施例は、Aβ(24時間曝露)によって損傷されたラット初代海馬ニューロンにおける、アセチルコリンエステラーゼ阻害活性及び5-HT4受容体アゴニスト活性の両方を有する化合物(すなわち、ドネコプリド(Donecopride))の効果を示す。
海馬ニューロンの初代培養
ラット海馬ニューロンは、Callizot et al. (2013) J. Neurosci. Res. 91706-716に記載されるように培養した。妊娠17日の妊娠した雌のラット(Rats Wistar; Janvier Labs France)を、CO2チャンバー用いて深麻酔、その後頸椎脱臼を用いて、殺した。次いで、胎児を子宮から取り除き、直ぐに、2%ペニシリン(10000U/ml)及びストレプトマイシン(10mg/ml)溶液(PS)、並びに1%ウシ血清アルブミン(BSA)を含む、よく冷えたL15 Leibovitz培地に置いた。海馬ニューロンを、終濃度が0.05%のトリプシン及び0.02%のEDTAの、トリプシン-EDTA溶液で、20分間、37℃で処理した。解離は、DNAse IグレードII(終濃度0.5mg/ml)及び10%のウシ胎児血清(FCS)を含む、4.5g/lのグルコースを有するダルベッコ改変イーグルス培地(DMEM)を加えることによって、停止させた。細胞を、10-mlピペットのチップを3回強制的に通過させることによって、機械的に解離させ、その後、4℃で10分間、515×gで遠心した。上清を捨て、ペレットを、2%のB27サプリメントの溶液、2mmol/lのL-グルタミン、2%のPS溶液、及び10ng/mlの脳由来神経栄養因子(BDNF)を有する、Neurobasal培地からなる規定培養培地(defined culture medium)中に再懸濁した。生存している細胞を、トリパンブルー色素排除試験を使用して、ノイバウアー血球計算器中でカウントした。細胞を、ポリ-L-リシンでプレコートされた96ウェルプレート中に、1ウェルあたり20000個の密度で蒔き、空気(95%)-CO2(5%)インキュベーター中で、37℃で培養した。培地は、2日毎に交換した。
海馬ニューロンを、培養の17日後に、Aβ溶液(以下を参照)に曝露した。Aβ1-42の調製は、Callizot et al. (2013) J. Neurosci. Res. 91706-716によって記載された方法に従って、行った。簡潔には、Aβ1-42ペプチドを、40μMの初期濃度で、上記の規定培養培地中に溶解した。この溶液を、暗い中で、3日間、37℃で、静かに攪拌し、培養培地中で適切に希釈して、使用される濃度(2μM又は0.25μMのオリゴマー(Aβ)にそれぞれ対応する、20μM(プレート1)又は2.5μM(プレート2))にした後、直ぐに使用した。
ドネコプリドフマレート(ドネコプリド)を、96ウェルプレート中での1つの培養で試験した(1条件あたり6ウェル)。化合物は、Aβの適用の1時間前に、プレインキュベーションした。以下の条件を評価した。
プレート1:生存、神経突起ネットワーク、及びホスホタウの評価
中毒(intoxication)の24時間後、海馬ニューロンを、エタノール(95%)及び酢酸(5%)の冷えた溶液で、5分間、-20℃で固定した。0.1%のサポニンで透過処理後、細胞を、
a) 1%のウシ胎児血清及び0.1%のサポニンを含むPBS中で1/1000に希釈された、ニワトリの抗-微小管関連タンパク質2(MAP-2)ポリクローナル抗体(この抗体は、神経細胞体及び神経突起の特異的な染色を可能にし、神経細胞死及び神経突起ネットワークの研究を可能にする)
b) 1%のウシ胎児血清及び0.1%のサポニンを含むPBS中で1/400に希釈された、マウスの抗-ホスホタウ モノクローナル抗体(AT100)
とともに、2時間、インキュベーションした。

a) 1%のウシ胎児血清及び0.1%のサポニンを含むPBS中で1/100に希釈された、マウスの抗-PSD95(95kDaのシナプス後肥厚部(post synaptic density 95kDa))モノクローナル抗体
b) 1%のウシ胎児血清及び0.1%のサポニンを含むPBS中で1/100に希釈された、ウサギの抗-シナプトフィジン(SYN)ポリクローナル抗体
とともに、2時間、インキュベーションした。

全ての値は、平均+/-SEMとして表される。統計学的解析は、一元配置分散分析(One-way ANOVA)後、PLSDフィッシャーの検定(PLSD Fisher’s test)によって実施し、p<0.05を有意であるとみなした。
MAP-2海馬ニューロンの生存及び神経突起ネットワーク、並びにタウの過剰リン酸化に対するドネコプリドの効果
海馬ニューロンの初代培養における、MAP-2ニューロンの生存に対する、ドネコプリドの効果を、以下の表2(Table 1)に示す。

シナプスの形成を評価するために、PSD-95(シナプス後マーカー)及びシナプトフィジン(シナプス前マーカー)の分布を調査した。PSD-95及びシナプトフィジン染色の両方に関して陽性である構造体は、シナプスとみなした。
本研究は、アセチルコリンエステラーゼ阻害活性及び5-HT4受容体アゴニスト活性の組合せが、アルツハイマー病のin vitroモデルの、Aβ 1-42ペプチドによって損傷した、海馬ニューロンの、ニューロンの生存及び神経突起ネットワークに対して有益な効果を有することを示す。
この実施例は、アルツハイマー病の5XFADマウスモデルへの長期投与における、アセチルコリンエステラーゼ阻害活性及び5-HT4受容体アゴニスト活性の両方を有する化合物(ドネコプリド)の抗健忘効果、抗アミロイド効果、及び抗炎症効果を示す。
本研究の目的は、アルツハイマー病のマウスモデルでの長期投与における、アセチルコリンエステラーゼ阻害活性及び5-HT4受容体アゴニスト活性の両方を有する化合物(ドネコプリド)の潜在的な抗健忘効果、抗アミロイド効果、及び抗炎症効果を評価することであった。
マウス
動物実験は、1986年11月24日の欧州共同体の理事会による指令(86/609/EEC)に従って、実施した。動物の苦しみを最小化し、使用するマウスの数を減らすための全ての努力が行われた。5XFADマウスの作製は、Oakley et al. (2006) J. Neurosci. 26:10129-10140に記載された。これらのトランスジェニックマウスは、Swedish (K670N, M671L)、Florida (I716V)、及びLondon (V717I)家族性AD(FAD)変異を内包するヒトAPP(695)、並びに2つのFAD変異(M146L及びL286V)を内包するヒトプレセニリン1(PS1)の両方を過剰発現する。両方の導入遺伝子の発現は、マウスThyプロモーターのニューロン特異的要素によって、制御される。5XFAD系統(B6/SJLの遺伝的バックグラウンド)は、ヘミ接合のトランスジェニックマウスをB6/SJL F1繁殖個体と交配することによって、維持した。5XFAD系統(C57BL/6Jの遺伝的バックグラウンド)は、ヘミ接合のトランスジェニックマウスをC57BL/6J F1繁殖個体と交配することによって、維持した。全ての動物を、尾のゲノムDNAを使用して、PCRによってジェノタイピングした。トランスジェニックマウス及び野生型(WT)マウスは、本発明者らの動物施設内で繁殖され、自由に食餌及び水を手に入れることができ、22~24℃で、12時間の明暗サイクル下で、飼育された。雌の5XFADマウスを、この研究で使用した。各実験において、マウスの年齢は、(2週間の範囲を有する)月で示される。
ドネコプリド(ドネコプリドフマレート、MR 31155)は、CERMN, Caen, Franceにおいて、合成された。
5XFADマウスは、2つの異なるプロトコルに従って、薬剤又はビヒクルを用いた長期間の処置を受けた。各実験において、薬剤又はビヒクル溶液を、1週間に2回、腹腔内投与した(1mg/kg)。「プロトコル1」では、2~4月齢のマウス(群あたり、n=6~7)を、2か月間、化合物で処置した。「プロトコル2」では、1~4月齢のマウス(n=7)を、3か月間、化合物で処置した。各プロトコルの最後に、マウスを、生理食塩水溶液中の100mg/kgのケタミン及び10mg/kgのキシラジンの混合物を用いて麻酔し、PBSを用いて経心的に(transcardially)灌流した。脳を氷上に素早く単離し、嗅球及び小脳を除去し、両半球を分割した。一方の半球をドライアイス上で凍結し、生化学分析のために-80℃で保存し、他方を免疫組織化学染色(IHC)のために4% PFAで後固定した。
マウスを麻酔し、定位固定装置に載せた。頸の皮膚を切り、マイクロ開創器(micro-retractors) (Fine Science Tools, Heidelberg, Germany)を用いて、皮下組織と筋肉とを分けた。次いで、マウスを、頭が身体と約135°の角度を形成するように、置いた(Liu and Duff (2008) J. Vis. Exp. 10:960)。キャピラリーチューブ(ホウケイ酸ガラス、B100-75-10、Sutter Instruments社、Novato, California, USA)を使用して、大槽(cisterna magna)の硬膜に穴をあけた。CSFを毛細管現象によって回収して、0.5mLマイクロチューブに移し、速やかにドライアイス上で凍結して、使用するまで-80℃で保存した。一度解けると、サンプルを、国際出願の国際公開第2008/009868号に記載されるように、60℃で、5分間、加熱し、更なる凍結-解凍サイクルをせずに解析をした。
5XFADマウス及びコントロールの脳半球、前頭皮質、海馬を解凍し、重さを量り、プロテアーゼ阻害剤カクテル(Roche Applied Science, Meylan, France)を有する、4倍量のトリス生理食塩水(50mM Tris-HCl pH=7.4、150mM NaCl)でホモジネートした。生じたホモジネート物を、20分間、54000×gで遠心して、上清(「溶解性分画」)を回収し、分注して-80℃で保存した。ペレットを、10倍量の、50mM Tris-HCl, pH = 7.6中の6M グアニジンHCl (guanidine HCl)の中で、短時間の超音波処理によって、再懸濁し、再度、20分間、26500×gで遠心した。上清(「非溶解性分画」)を分注し、-80℃で保存した(Morishima-Kawashima et al. (2000) Am. J. Pathol. 157:2093-2099)。
Aβ40(ヒトアミロイド-β (1-40)アッセイキット、#27713)又はAβ42(ヒトアミロイド-β (1-42)アッセイキット、#27719)の投薬に関する、IBL International社(Hamburg, Germany)のELISAキットを、メーカーの説明に従って、使用した。反応を、Infinite 2000発光カウンター(Infinite 2000 luminescence counter)を使用して、620nm及び450nmで読み取った。得られた値を、BCAタンパク質アッセイ(Sigma-Aldrich)を使用して測定された、各サンプルのタンパク質濃度に標準化した。
30μmの厚さの切片を、ビブラトーム(vibratome)(Microm HM 650V, Thermo Scientific, Saint Herblain, France)を使用して切り、-20℃で、抗凍結剤培地中で保存した。アミロイド斑の標識のために、前頭皮質、海馬、及び内嗅皮質(entorhinal cortex)のフリーフローティング組織切片(ブレグマ(bregma)からの座標:前頭皮質=1.98mm、海馬=-1.94mm、内嗅皮質=-3.08mm)をPBS中でよく洗浄し、次いで、ブロッキング溶液(PBS; 3% BSA; 0.1% Triton X-100)中で1時間インキュベーションした。切片をヘキスト染料(1:1000, Life Technologies, Saint Aubin, France)を用いて、15分間染色して、細胞核を検出し、次いで、新たに調製したチオフラビンT溶液(#T3516-5G, Sigma-Aldrich)(終濃度:ブロッキング緩衝液中で0.01mg/ml)を用いて、15分間染色した。70%エタノール中で5分間洗浄後、サンプルをポリリジンスライド(poly-lysine slides)上に、カバーガラスとともに載せた。GFAP(glial fibrillary acidic protein;グリア線維性酸性タンパク質)染色に関して、フリーフローティング脳切片を、前述の通りにブロッキングし、ウサギ抗GFAPポリクローナル抗体(1:1000, Z0334, Dako, Les Ullis, France)とともに、4℃で、一晩、インキュベーションした。チオフラビンT染色、及び70%エタノールでの洗浄後、Alexafluor 594 ヤギ抗ウサギ二次抗体(1:1000, A11012, Life Technologies)を2時間加えた。前述のように、PBS洗浄及びマウンティングを実施した。
画像を、AxioImager Z1顕微鏡(Carl Zeiss S.A.S., Marly le Roi, France)を使用して取得した。チオフラビンT染色の解析は、盲検的に実施し、データは、同じ脳領域/動物由来の2~3つの組織切片中の1mm2あたりの粒子の平均個数として表す(Image J software)。GFAP発現を、Image J softwareを用いて定量し、結果を面積率(area fraction)として表す。
マウスの認知能力を、ノベルオブジェクト探索(NOR)試験(Bevins and Besheer (2006) Nat. Protoc. 1:1306-1311)を使用して、試験した。動物を、試験開始前に、薬物処置の間、大規模に処理した。毎日、試験前に、少なくとも1時間の間、試験室に慣れさせた。試験は、薄暗い部屋に置かれたプレキシグラスボックス(Plexiglas box)(幅:35cm、長さ:20cm、高さ:20cm)中で実施した。1日目及び2日目に、10分間/日、各マウスを空のボックスに慣らした。3日目に、(プラスチック玩具で構築された)2つのオブジェクトを、反対の壁から5cm離れた、ケージに置いた。訓練期間の間、各動物を、2つのオブジェクトの間に、壁に向けて置き、次いで、5分間、オブジェクトを探索できるようにさせた。そして、マウスをホームケージに戻し、24時間後に、2つのオブジェクト(familiar)の1つが新しいもの(novel)に置き換えられた、5分間の試験期間を行った。試験全体はビデオ録画され、オブジェクト探索(マウスの鼻がオブジェクトと接触することよって費やされた時間、又は1cm以下(≦1cm)の距離でオブジェクトの匂いを嗅ぐことによって費やされた時間)を盲検的に測定した。2つのパラメータが考慮された:1)訓練期間の間に、動物が、2つの慣れたオブジェクトとふれあうことによって費やされた探索時間(sec)、及び2)試験の間に、動物が、総探索時間と比較して新しいオブジェクトとふれあうことによって費やされた時間({novel/(familiar + novel)}×100)。識別指数も計算した((novel - familiar)/(familiar + novel))。
全ての値は、平均±SEMとして表される。処置の有意な効果は、多重比較群の場合、ANOVA解析とそれに続くテューキーの事後検定(Tukey’s post hoc tests)によって決定した。全てのその他の場合には、対応のないスチューデントのt検定(unpaired Student's t test)を使用した。全ての統計学的試験に関して、P<0.05を有意であるとみなした。解析は、GraphPad Prism 7 (GraphPad Software, La Jolla, CA)を用いて行った。
5XFADの脳のアミロイド量に対するドネコプリドの効果
27匹の雌の5XFADマウス(B6/SJLの遺伝的バックグラウンド)を、この研究に使用した。2~4月齢のマウスに、2か月間、1週間に2回、腹腔内に、ビヒクル又は化合物(1mg/kg)を受けさせた。
溶解性分画:
・Aβ40 = 9.48 ± 1.08 μg/mgのタンパク質、
・Aβ42 = 0.24 ± 0.03 μg/mgのタンパク質;
非溶解性分画:
・Aβ40 = 0.68 ± 0.10 μg/mgのタンパク質、
・Aβ42 = 10.84 ± 0.73 μg/mgのタンパク質。
5XFADマウス脳の溶解性分画において、ドネコプリドの長期投与(1mg/kg、1週間に2回)は、Aβ40のレベルのいかなる変化なしに、Aβ42のレベルを有意に減少させることができた(一元配置分散分析とそれに続くテューキーの検定(Tukey’s tests)によって決定された*p < 0.05)。同様に、非溶解性分画において、ドネコプリドは、マウスの脳において、Aβ42量(laod)を減少させた。非溶解Aβ40のレベルは、薬剤による影響を受けなかった。
28匹の雌の5XFADマウス(C57BL/6Jの遺伝的バックグラウンド)を、この研究で使用した。1~4月齢のマウスに、3か月間、1週間に2回、腹腔内に、ビヒクル又は化合物(1mg/kg)を受けさせた。
1か月齢の雌の5XFAD及びWTの同腹子を、RS 67333、ビヒクル、又はドネコプリド(1mg/kg、3か月間、1週間に2回)を用いて処置し、その後、認識記憶を、ノベルオブジェクト探索(NOR)試験を用いて評価した。馴化期間を処置の最後の3日後に始めて、急性効果への妨害を避けた。訓練期間及び試験の間の間隔は、24時間であった。類似した探索時間及び有意でない識別指数によって示されるように、ビヒクルで処置されたマウスは、障害された記憶力を提示し、新規のオブジェクトと慣れたオブジェクトとを識別することができなかった。RS 67333処置は、ビヒクルで処置されたマウスと比較して、新規のオブジェクトの探索に充てられる時間のより高いパーセンテージによって示されるように、認知障害を元に戻す(慣れたオブジェクトに対して**p<0.01、2元配置分散分析(two-way ANOVA)とそれに続くシダックの検定(Sidak’s test)によって決定した)。同様に、ドネコプリドの長期処置は、記憶力の変化を防ぐことができた(慣れたオブジェクトに対して**p<0.01、2元配置分散分析とそれに続くシダックの検定によって決定した)。2つの化合物で処置された群の識別指数は、ゼロ、チャンスレベルとは異なった(対応のないスチューデントのt検定によって決定した)。訓練期間の間の総探索時間は、3つの群で有意に異ならず、ドネコプリドで処置された5XFADマウスにおける認識記憶の向上は、探索活動の増加に伴うものではないことを示した。
RS 67333又はドネコプリドで処置された(1mg/kg、3か月間、1週間に2回)5XFADマウスの脳脊髄液(CSF)中のAβ42濃度は、コントロールと比較して、有意には異ならず、長期の5-HT4受容体アゴニスト刺激は、CSF中のアミロイドレベルに影響を与えないことを示した(一元配置分散分析とそれに続くテューキーの検定によって決定した)。
前頭皮質、海馬、及び内嗅皮質を、5XFADマウスの脳(前頭部、正中部、及び尾側部)の代表領域、並びにアルツハイマー病によって初期に影響を受け、及びアミロイド沈着が高度に多いヒトの脳領域の代表領域として、解析した。
アストログリアの免疫組織化学染色評価(抗GFAP抗体、グリア線維性酸性タンパク質)は、ドネコプリド及びRS 67333の長期処置(1mg/kg、3か月間、1週間に2回)が、アストログリオーシスを有意に減少させた(それぞれ、ビヒクルを受けたコントロールと比較して、処置を受けた動物の内嗅の脳切片において、GFAP染色の54±10%及び62±6%の減少)を示した。しかしながら、RS 67333又はドネコプリドを用いて処置(1mg/kg、3か月間、1週間に2回)された5XFADマウスの前頭皮質及び海馬におけるGFAP染色は、コントロールと比較して、有意には異ならなかった。統計は、一元配置分散分析とそれに続くテューキーの検定によって決定した。
本研究は、アセチルコリンエステラーゼ阻害活性及び5-HT4受容体アゴニスト活性を有する化合物(ドネコプリド)が、アルツハイマー病のトランスジェニックマウスモデルである5XFADマウスへの長期処置後に、抗アミロイド効果(Aβ42レベル及びアミロイド斑の減少)及び抗炎症効果(アストログリオーシスの減少)を発揮することを実証する。これらの効果は、内嗅皮質及び前頭皮質において、主に顕著である。さらに、ドネコプリドの長期投与は、5XFADマウスにおいて、抗健忘効果を発揮し、記憶障害の出現を防止することができる。
この実施例は、Aβ1-42損傷の存在下における、ラット初代海馬ニューロンに対する、RS67333(5-HT4受容体アゴニスト)及びドネペジルの相乗効果を示す。
海馬ニューロンの初代培養
全ての実験は、国立衛生研究所の実験動物の管理と使用に関する指針(Guide for the Care and Use of Laboratory Animals)に従って実施し、最新の欧州連合規則(指令2010/63/EU)に従った。承諾番号:A1301310。
プレインキュベーション:培養17日目に、Aβ曝露前に、化合物を培養培地中に溶解し、1時間、初代海馬ニューロンとともにプレインキュベーションした。
損傷:海馬ニューロンを、培養の17日後に、Aβ1-42溶液(以下を参照)に曝露した。Aβ1-42調製は、Callizot et al. (2013) J. Neurosci. Res. 91:706-716に記載される方法に従って行った。この研究に使用したAβ1-42のバッチ番号は、10%のオリゴマーを含む(Combes et al. (2015) J. Neurosci. Res. 93:633-643で実施されたようにWBによって定量)。簡潔には、Aβ1-42ペプチドを、上述した規定培養培地中に、40μmol/lの初期濃度で溶解した。この溶液を、暗い中で、3日間、37℃で、静かに攪拌し、培養培地中で適切に希釈して、使用される濃度(2μmol/l又は0.25μmol/lのオリゴマー(AβO)にそれぞれ対応する、20μmol/l又は2.5μmol/l)にした後、直ぐに使用した。Aβ1-42の調製物を加えて、化合物の存在下で、コントロール培地に希釈されて、20又は2.5μmol/lの終濃度(=自動ウェスタンブロットで評価された、2μmol/l又は0.25μmol/lのAβO)にした。
化合物を、96ウェルプレート中での1つの培養で試験した(1条件あたり6ウェル)。化合物は、Aβ1-42の適用の1時間前に、プレインキュベーションした。以下の条件を評価した。
中毒の24時間後、上清を取り出し、直ちに、将来の解析的分析のために、凍結した。上清は、-20℃で、最大6か月保存した。その後、海馬ニューロンを、エタノール(95%)及び酢酸(5%)の冷えた溶液で、5分間、-20℃で固定した。海馬ニューロンを、PBS中で再度2回洗浄し、次いで、0.1%のサポニン及び1%のFCSを含むPBSの溶液を使用して、15分間、室温で、透過処理し、非特異的部位をブロックした。
‐1%のウシ胎児血清及び0.1%のサポニンを含むPBS中で1/1000に希釈された、ニワトリの抗微小管関連タンパク質2(MAP-2)ポリクローナル抗体(この抗体は、細胞体及び神経突起を特異的に染色し、神経細胞死及び神経突起ネットワークの研究を可能にする);及び
‐1%のウシ胎児血清及び0.1%のサポニンを含むPBS中で1/100に希釈された、マウスの抗ホスホタウモノクローナル抗体(AT100)
とともに、2時間、インキュベーションした。
-ニューロンの総数(ニューロンの生存、MAP-2陽性ニューロンの数)
-神経突起ネットワーク(MAP-2陽性ニューロンのμmにおける)
-AT100の領域(μm2、MAP-2陽性ニューロンとの重なり)。
‐1%のウシ胎児血清及び0.1%のサポニンを含むPBS中で1/200に希釈された、マウス抗PSD95(95kDaのシナプス後肥厚部)モノクローナル抗体;
‐1%のウシ胎児血清及び0.1%のサポニンを含むPBS中で1/100に希釈された、ウサギ抗シナプトフィジン(SYN)ポリクローナル抗体
とともに、2時間、インキュベーションした。
‐(PSD95/SYN間で重なっている)シナプスの数。
全ての値は、平均±SEM(standard error of the mean;平均値の標準誤差)として表される。統計学的解析は、一元配置分散分析とそれに続くダネットの検定(Dunnett’s test)又はLSDフィッシャーの検定(LSD Fisher’s test)によって実施した。p<0.05を有意であるとみなした。
この研究は、RS67333とドネペジルとの間の潜在的な相乗作用を研究するように設計した。本研究の第一の工程は、選択されたパラメータ(ニューロンの生存、神経突起ネットワークのインテグリティ、タウの過剰リン酸化、シナプスの保護)における、最も低い活性用量を同定することを目的とした。本研究の第二の工程は、同じパラメータ(ニューロンの生存、神経突起ネットワークのインテグリティ、タウの過剰リン酸化、シナプスの保護)における、RS67333とドネペジルの不活性用量の異なる組合せを試験するように設計した。
Aβ 1-42損傷は、神経突起ネットワークの喪失、タウの過剰リン酸化、及びシナプスの喪失とともに、海馬ニューロンの喪失を誘導する。
・ニューロンの生存: 50nM
・神経突起ネットワーク: 10nM
・タウの過剰リン酸化: 1nM
・シナプス: 1nM
であった。
Aβ 1-42ペプチドの神経毒性を、全てのパラメータについて確認し、全てのパラメータにおいて、工程1で見られた損傷と同様の程度であった(図2を参照)。
・ニューロンの生存が、組合せ#6(10pMのドネペジル及び1nMのRS67333)によって向上した。
・組合せ#4~#8は、神経突起ネットワークの総長を有意に向上することができた。最大効果は、組合せ#6(10pMのドネペジル及び1nMのRS67333)で見られた。興味深いことに、組合せ#6及び#8は、1つのニューロンあたりの神経突起の平均の長さを向上させた。
・ほとんどすべての組合せ(#2から#8)が、AT100エピトープにおける、タウの過剰リン酸化を減少させることができた。最大効果は、組合せ#6(10pMのドネペジル及び1nMのRS67333)で見られた。
・組合せ#6及び#7は、Aβ 1-42損傷からシナプスを保護した。
本研究の目的は、アルツハイマー病のin vitroモデルにおけるRS67333とドネペジル間の潜在的な相乗作用を同定することであった。表11(Table 8)は、相乗作用の結果をまとめている。
・工程1:
‐タウの過剰リン酸化における、RS67333の最も低い活性濃度は、1nMであった。RS67333の最も低い神経保護(ニューロンの生存)濃度は、50nMであった。
‐ドネペジルの最も低い活性濃度は、1nMであった。
・工程2
‐RS67333とドネペジルとの組合せは、全ての調査したパラメータにおいて、明らかな相乗効果を示した。
‐RS67333とドネペジルとの相乗効果は、ドネペジルの第一の活性用量を用いて得られる効果よりも優れている:
ニューロンの生存(相乗作用:80%生存;ドネペジル単独:75%)
神経突起ネットワーク(相乗作用:80%生存;ドネペジル単独:75%)
AT100(相乗作用:141%;ドネペジル単独:165%)。
Claims (14)
- アセチルコリンエステラーゼ阻害剤及び5-HT4受容体アゴニストを含む、神経変性疾患の防止及び/又は治療において、神経保護剤として使用するための、医薬組成物。
- アセチルコリンエステラーゼ阻害剤が、ドネペジル、リバスチグミン、タクリン、及びガランタミンからなる群から選択される、請求項1に記載の医薬組成物。
- アセチルコリンエステラーゼ阻害剤が、ドネペジルである、請求項1又は2に記載の医薬組成物。
- 5-HT4受容体アゴニストが、プルカロプリド、シサプリド、ダゾプリド、モサプリド、レンザプリド、ナロナプリド、ザコプリド、ベルセトラッグ、テガセロッド、メトクロプラミド、シニタプリド、YM-53389、RS-67333、5-メトキシトリプタミン(5-MT)、RS-56532、ML-10302、SSP-002392、BIMU-8、及びDA-6886からなる群から選択される、請求項1から3のいずれか一項に記載の医薬組成物。
- 5-HT4受容体アゴニストが、RS-67333である、請求項1から4のいずれか一項に記載の医薬組成物。
- 神経変性疾患が、アルツハイマー病、パーキンソン病、ハンチントン病、多系統萎縮症、筋萎縮性側索硬化症を含む運動ニューロン疾患、ダウン症候群、前頭側頭型変性症からなる群から選択される、請求項1から5のいずれか一項に記載の医薬組成物。
