本明細書ではPyLとも称される18F-DCFPyLは、フッ素化されたPSMAを標的とする陽電子放出断層撮影/コンピュータ断層撮影(「PET/CT」)造影剤であって、骨および軟組織転移などの転移の可視化を可能にするものである。驚くべきことに、かかる剤を用いたイメージングは、本明細書に規定されるとおりの対象について高レベルの感度または陽性的中度で再発性または転移性の前立腺がんの有無を決定することに使用することができるということが見いだされている。
「再発性」は、本明細書に使用されるとき、当初の前立腺がんが起こった後で起こる前立腺がんを指すか、または処置を生き延びた前立腺がんを指す。
提供される方法のいずれか1つのいくつかの態様において、対象は、最下点(nadir)を0.2ng/mL以上超える高くなったPSAを伴うものなどの、高いかまたは上昇している前立腺特異的抗原(PSA)レベルを有し得る。かかる対象のいくつかの態様において、対象は、放射線療法、凍結療法、または小線源療法を経たものである。本明細書に提供される方法のいずれか1つのいくつかの態様において、対象が、放射線療法、凍結療法、または小線源療法を経たものであるか、または、方法が、対象を放射線療法、凍結療法、または小線源療法で処置するステップを包含する。

例1- 前立腺がんを有する患者における18F-DCFPyL PET/CTイメージングの前向きフェーズ2/3多施設研究-診断的確性の検査(A PrOspective Phase 2/3 Multi-Center Study of 18F-DCFPyL PET/CT Imaging in Patients with PRostate Cancer - Examination of Diagnostic AccuracY)(OSPREY)
背景
前立腺特異的膜抗原(PSMA)は、前立腺がん(PCa)細胞によって過剰発現させられる膜貫通タンパク質であり、および、新規のPET放射性追跡子18F-DCFPyLを使用して標的化することができる。この例は、PCaを有する男性における骨盤リンパ節転移および他の離れた疾患部位を検出するための18F-DCFPyL PET/CTの診断パフォーマンスを決定することを目指すものであった。
根治的前立腺切除術およびリンパ節切除術を予定された高リスクPCaを有する(コホートA、n=268)、または生検を実行可能である転移性PCaの放射線学的証拠を有する(コホートB、n=117)、385人の男性において、18F-DCFPyL PET/CTを評価した。9mCi(333MBq)の18F-DCFPyLをPET/CTの1~2時間前に投与した。骨盤リンパ節転移を検出するための18F-DCFPyL PET/CTの感度および特異度という複数主要エンドポイント(coprimary endpoints)を、コホートAにおいて評価した。副次的エンドポイントは、両方のコホートにおいて安全性を、コホートAにおいて陽性的中度(PPV)および陰性的中度(NPV)を、およびコホートBにおいて感度およびPPVを包含した。3の中心的な、盲検化された、かつ独立したリーダー(readers)が、18F-DCFPyLスキャンを評価した。組織病理学は、イメージング所見と比較された参照基準としての役割を果たした。図1は、研究デザインおよびコホートに関して情報を提供する。
コホートA(n=252が評価可能)において、18F-DCFPyL PET/CTの感度は、3のリーダーの間で30.6~41.9%の範囲にわたっており(95%CIの下限:19.2~29.7%)、96.3~98.9%(95%CIの下限:93.6~96.0%)の特異度の範囲を伴っていた。加えて、PPVおよびNPVは、夫々78.1~90.5%および81.4~83.8%の範囲にわたっていた。分析をリンパ節転移の中央値>4mmに限ると、感度において顕著な改善があった(51.7~65.5%の範囲にわたる;95%CIの下限:33.5~48.0%)。遠隔転移PCaを有する患者(コホートB、n=93が評価可能)において、感度およびPPVの値は、夫々92.9~98.6%(95%CIの下限:84.0~91.6%)および81.2~87.8%の範囲にわたっていた。薬剤関連の重篤な有害事象は観察されず、および、27人(7.0%)の男性が、≧1の薬剤関連有害事象を経験し、味覚障害(2.1%)および頭痛(2.1%)が最もよく見られた。
骨盤リンパ節診断概要
・ 第一の複数主要エンドポイントである特異度は、96~99%の範囲にわたり、および、前もって特定された限界である80%で統計的に有意であった
・ 第二の複数主要エンドポイントである感度は、31~42%の範囲にわたり、および、前もって特定された信頼限界である40%を満たさなかった
・ 高いPPV(78~91%)およびNPV(81~84%)パフォーマンスは、18F-DCFPyL PET/CTの強い臨床的実用性を実証し、医師に試験結果を信頼するだけの信頼性を提供する
転移診断概要
・ 骨盤外(例として、骨、軟組織、およびリンパ節)の再発性または転移性疾患を表す副次的な効能のエンドポイント
・ この患者集団において、高い感度(93~99%)およびPPV(81~88%)は、遠隔転移病変を的確に検出する強い臨床的実用性を与える
・ 拡大骨盤リンパ節郭清からの骨盤リンパ節および標的とされた生検からの遠隔転移または再発病変を包含する前立腺外病変を検出する18F-DCFPyLのパフォーマンスは、全ての診断パラメーターにわたって統計的に有意であった

主な成果測定:
1.18F-DCFPyL PET/CTイメージング上で同定された少なくとも1つの病変と、評価可能な組織病理学、情報性のある相関イメージング、または放射線療法の後のPSA応答のいずれかとして定義された複合の真の基準(composite truth standard)との、1対1対応を伴う対象のパーセンテージとして定義される、正確なローカライゼーションのレート(Correct Localization Rate)(CLR)。
PyL PET/CTイメージングに続く60日以内に、PyLによる疑いのある病変(単数または複数)の、生検/手術、従来型のイメージング、または局所領域放射線療法のいずれかが遂行される。
2.意図していた前立腺がん処置計画における18F-DCFPyL PET/CTイメージング結果に起因する変更を伴う対象のパーセンテージ。
意図していた前立腺がん処置計画における変更は、PyL PET/CTイメージングの前および後に完了させた医学管理アンケート(Medical Management Questionnaires)に基づく。

・ 男性>/=年齢18歳の
・ 元々の診断について組織病理学的に確認された前立腺癌であって、後続する根治療法を伴う
・ 以下に基づいた根治療法の後の上昇しているPSAに基づいて前立腺がんの再発の疑いがある:
a.根治的前立腺切除術後:≧0.2ng/mLである検出可能なまたは上昇しているPSAであって、確認的なPSA≧0.2ng/mLを伴う(米国泌尿器科学会[AUA]);あるいは
b.放射線療法後、凍結療法後、または小線源療法後:最下点≧2ng/mLを超えて高くなったPSAレベルの増加(アメリカ腫瘍放射線治療学会[ASTRO]-Phoenix)
・ 第1日に対して60日以内前に標準治療精密検査の一部として遂行された従来型のイメージング試験での前立腺がんについて陰性または曖昧な所見
・ 制度的な標準治療(SOCスキャン(単数または複数)(例として、骨盤CT/MRI、全身骨スキャン、NaF、フルシクロビンまたはコリンPET)ごとに、従来型のイメージング所見は第1日に対して60日以内前に完了した。第1日に対して60日前よりも前に遂行したSOCスキャン(単数または複数)は研究のスクリーニング手続として繰り返され得、および第1日よりも前に検査担当医によってレビューされ得る。
・ 検査担当医によって決定されるとおりの余命≧6か月
・ インフォームドコンセントを提供し、およびプロトコル要件を遵守する、能力および意思
・ 何らかの高エネルギー(>300KeV)ガンマ線放射ラジオアイソトープを第1日に対して物理的半減期5回以内前に投与された対象

背景
前立腺特異的膜抗原(PSMA)は、前立腺がん(PCa)細胞によって過剰発現させられる膜貫通タンパク質である。PSMAベースのイメージングは、PCa検出に大いに有望であると見なされており、および、新規のPET放射性追跡子18F-DCFPyLを使用して標的化することができる。この前向きな多施設トライアルは、PCaを有する男性における、骨盤リンパ節転移、および遠隔転移の部位を検出するための、18F-DCFPyL PET/CTの安全性および診断パフォーマンスを決定するためにデザインされた。
リンパ節切除術を伴う根治的前立腺切除術を予定された高リスクPCaを有する(コホートA)、または生検を予定されていた者であって再発性もしくは転移性PCaの放射線学的証拠を有する(コホートB)、385人の男性において、18F-DCFPyL PET/CTを評価した。9mCi(333MBq)の18F-DCFPyLをPET/CTの1~2時間前に投与した。骨盤リンパ節における前立腺がん転移を検出するための18F-DCFPyL PET/CTの感度および特異度という複数主要エンドポイントを、コホートAにおいて評価した。主要な副次的エンドポイントは、有害事象の発生率、コホートAにおける骨盤リンパ節転移を検出するための18F-DCFPyL PET/CTイメージングの陽性的中度(PPV)および陰性的中度(NPV)、コホートBにおける転移または再発の部位内で前立腺がんを検出するための18F-DCFPyL PET/CTイメージングの感度およびPPVであった。3の中心的な、盲検化された、かつ独立したリーダーが、18F-DCFPyLスキャンを評価した。イメージング所見を、参照基準としての組織病理学と比較した。
コホートA(n=252が評価可能)における骨盤リンパ節転移の検出については、18F-DCFPyL PET/CTイメージングは、感度の中央値40.3%(95%CI:28.1~52.5%)、特異度の中央値97.9%(95%CI:94.5~99.4%);および、PPVおよびNPVの中央値夫々86.7%(95%CI:69.7~95.0%)および83.2%(95%CI:78.0~88.0%)を有していた。コホートB(n=93が評価可能)においては、感度の中央値およびPPVは、夫々95.8%(95%CI:87.8~99.0%)および81.9%(95%CI:73.7~90.2%)であった。18F-DCFPyL PET/CTの感度およびPPVを、コホートBにおいては、領域レベル上の異なる病変位置(前立腺、骨盤および骨盤外)において評価した。骨盤領域についての感度およびPPVの中央値は夫々100%(95%CI:N/A)および79.5%(95%CI:67.0~92.0%)であり;および骨盤外領域についての感度およびPPVの中央値は夫々94.9%(95%CI:82.0~99.0%)および86.1%(95%CI:76.0~96.0%)であった。分析のための前立腺領域への局所再発を有する評価可能な対象はいなかった。個々のリーダーによるアセスメントは、下の表に示される。薬剤関連の重篤な有害事象は観察されなかった。27人(7.0%)の男性が、≧1の薬剤関連有害事象を経験し、味覚障害(2.1%)および頭痛(2.1%)が最も多かった。
18F-DCFPyL PET/CTは、良好な毒性プロファイルを有し、および、PCaを有する患者において一般的に良好な耐容性を示した。18F-DCFPyL PET/CTは、遠隔転移性前立腺がんを信頼性よく検出することにおける高い感度、および骨盤リンパ節転移がないことを確認することにおける高い特異度を実証した。これらの疾患セッティングにおける18F-DCFPyLイメージングの、関連した強いPPVおよびNPVは、その潜在的に高い臨床的実用性を示唆する。18F-DCFPyLは、現在のところ、PCaの生化学的再発を有する患者における第三相試験において調査中である。
背景
前立腺がんの的確な検出は、患者管理に必須であるが、それでも未だ標準的なイメージング法は、的確にmPCを検出するパフォーマンスが悪い。18F-DCFPyLは、前立腺がんについての認識されている標的である前立腺特異的膜抗原に選択的に結合する、新規なPET造影剤である。OSPREYは、新たに診断された高リスクの前立腺がんのある(コホートA)、またはmPCと分かっているかもしくは疑いのある(コホートB)、いずれかの患者における、前向きな多施設研究であった。ここでは我々はコホートBに注目する。
再発性またはmPCの生検を予定されていた117人の男性に18F-DCFPyLを受けさせた。患者は、画像誘導生検を経た。18F-DCFPyL PET/CTの感度、陽性的中度(PPV)、および安全性が、コホートBについての主要なエンドポイントであった。18F-DCFPyL PET/CTスキャンを、3の独立した、盲検化された中心的なリーダーによって評価し;および、真の基準(truth standard)としての組織病理学と結果を比較した。
組織病理学と比較したときの18F-DCFPyL PET/CTの感度およびPPVは、夫々92.9~98.6%(95%CIの下限:84.0~91.6%)および81.2~87.8%にわたっていた。解剖学的位置による診断パフォーマンスは、全ての疾患部位において高い感度および高いPPVを示した(下の表を参照)。2人(1.7%)のみのコホートB患者が≧1の薬剤関連AE(味覚障害および全身性発疹)を経験し、両方とも、重症度では軽症(グレード1)であった。
18F-DCFPyL PET/CTは、良好な耐容性を示し、および、節性の、骨の、および/または内臓組織/軟組織の転移を的確に検出することにおける高い感度およびPPVを実証した。陽性18F-DCFPyL PET/CTスキャンは、病理学的に証明された遠隔の疾患を顕す蓋然性が高く、これは、処置計画に良い影響を与えるPETイメージング剤としての18F-DCFPyLの潜在力を実証する。
Claims (18)
- 対象における前立腺がんを処置することまたはその処置管理判断を行う方法であって:
18F-DCFPyL PET/CTで対象における1以上の転移の有無を決定すること、および
1以上の転移の有無に基づいて対象を処置することまたは対象についての処置管理判断を行うこと
を含み、
対象が、転移性もしくは再発性前立腺がんを有するか、または転移性もしくは再発性前立腺がんを有する疑いのあるものである、前記方法。 - 対象が、前立腺外病変を有するか、または転移性もしくは前立腺外病変を有する疑いのあるものである、請求項1に記載の方法。
- 対象が、骨盤転移を有するか、または骨盤転移を有する疑いのあるものである、請求項1または に記載の方法。
- 対象が、遠隔転移を有するか、または遠隔転移を有する疑いのあるものである、請求項1~3のいずれか一項に記載の方法。
- 対象が、節性の、骨の、および/または内臓組織/軟組織の転移を有するものである、請求項1~4のいずれか一項に記載の方法。
- 転移の1つ以上が、4mmよりも大きい、請求項1~5のいずれか一項に記載の方法。
- リンパ節転移の平均または中央値が、>4mmである、請求項6に記載の方法。
- 18F-DCFPyLが、対象へ、PET/CTの1~2時間前に投与される、請求項1~7のいずれか一項に記載の方法。
- 9mCi(333MBq)の18F-DCFPyLが対象へIV注射により投与される、請求項1~8のいずれか一項に記載の方法。
- 前記決定に基づいて対象を処置することをさらに含む、請求項1~9のいずれか一項に記載の方法。
