Abstract

BL-8040(以前はBKT140と名づけられた、配列番号1)は、高選択性CXCR4アンタゴニストである。その治験薬は、CXCR4と高親和性(IC50 0.54~4.5nM)で結合し、それの機能を阻害する、芳香環でキャッピングされた、14残基の環状合成ペプチドである[1]。ケモカインCXCL12(SDF-1 ストロマ由来因子1)およびそれの受容体、CXCR4は、造血細胞の骨髄(BM)への輸送において中心的役割を果たす[2]。マウスの様々な癌モデルにおいてBL-8040の活性を探索する動物試験は、造血細胞の動員因子としてのそれの活性に加えて、BL-8040が、CXCR4を過剰発現する悪性細胞に対してCXCR4依存性の優先的な抗腫瘍効果を示すことを示している[3]。インビトロおよびインビボでCXCR4をブロックすることについてのBL-8040およびそれの類似体の有効性は、小細胞肺癌、乳癌、悪性黒色腫、神経芽細胞腫、および膵癌についてのインビトロおよびインビボのモデルを含む多数の前臨床試験において実証されている。CXCR4アンタゴニストとして、BL-8040はまた、免疫細胞の腫瘍微小環境への輸送にも影響する。BL-8040の投与が、ナチュラルキラー(NK)細胞、T細胞、およびB細胞のBMおよびリンパ節からその周辺への動員を誘導することが見出された。マウスにおける同系癌モデルを用いて、BL-8040が、免疫学的バリアを除去し、腫瘍微小環境内の免疫細胞の蓄積を可能にし得ることが示された。

ペムブロリズマブは、PD-1受容体との結合の高特異性を有する強力なヒト化免疫グロブリンG4(IgG4)モノクローナル抗体(mAb)であり、それゆえに、PD-1のPD-L1およびPD-L2との相互作用を阻害する。前臨床のインビトロデータに基づいて、ペムブロリズマブは、PD-1に対する高親和性かつ強力な受容体ブロッキング活性を有する。ペムブロリズマブは、許容できる前臨床の安全性プロファイルを有し、進行悪性腫瘍についてのIV免疫療法として臨床開発中である。Keytruda(登録商標)(ペムブロリズマブ)は、いくつかの適応症に渡って患者の処置に適応される。
腫瘍性形質転換の増殖を調節することにおける無傷の免疫監視機構の重要性は、何十年もの間知られている[7]。証拠の蓄積により、癌組織における腫瘍浸潤リンパ球と様々な悪性腫瘍における良好な予後との相関が示されている。特に、CD8+T細胞の存在およびCD8+エフェクターT細胞/FoxP3+制御性T細胞(Treg)の比率は、卵巣、結腸直腸、および膵臓の癌;肝細胞癌;悪性黒色腫;ならびに腎細胞癌などの固形悪性腫瘍における予後の向上および長期生存と相関する。腫瘍浸潤リンパ球は、エクスビボで増殖させて、再注入することができ、黒色腫などの癌において永続的な客観的腫瘍応答を誘導する[8、9]。
マウスモデルにおける治療研究により、単剤療法としてまたは他の処置モダリティーと併用して、PD-1/PD-L1相互作用をブロックする抗体の投与が、腫瘍特異的CD8+ T細胞の浸潤を増強し、最終的に腫瘍拒絶をもたらすことが示されている[18~23]。抗マウスPD-1抗体または抗マウスPD-L1抗体は、扁平上皮癌、膵癌、黒色腫、AML、および結腸直腸癌のモデルにおいて抗腫瘍応答を示している[12、22~25]。そのような試験において、CD8+ T細胞による腫瘍浸潤、ならびにIFN-γ、グランザイムB、およびパーフォリン発現の増加が観察され、PD-1チェックポイント阻害の抗腫瘍活性の根底にある機構が、インビボでのエフェクターT細胞の局所的浸潤およびその機能の活性化を含むことを示している[22]。実験は、同系マウス腫瘍モデルにおける、単剤療法としておよび化学療法と併用しての、抗マウスPD-1抗体のインビボ有効性を確認している(ペムブロリズマブIB参照)。
リポソーム型イリノテカンは、非リポソーム型イリノテカンの薬理学的および臨床的制限を克服するために開発されたトポイソメラーゼ-1阻害剤イリノテカンのリポソームに封入された製剤である[26、27]。5-FUは、膵臓腺癌を有する患者の処置に適応されるヌクレオシド代謝阻害剤である[28]。LVは、葉酸の化学還元型誘導体である。それは、5-FUの治療効果および毒性効果を増強することができる。LVの同時投与は、5-FUの血漿薬物動態を変化させないように思われる。5-FUは、フルオロデオキシウリジル酸へ代謝され、そのフルオロデオキシウリジル酸は、酵素チミジル酸シンターゼ(DNA修復および複製において重要な酵素)と結合して阻害する。LVは、別の還元型葉酸、5,10-メチレンテトラヒドロ葉酸へ容易に変換され、その5,10-メチレンテトラヒドロ葉酸は、フルオロデオキシウリジル酸のチミジル酸シンターゼとの結合を安定化させて、それにより、この酵素の阻害を増強するように働く[29]。IV用リポソーム型イリノテカン注射剤(Onivyde(登録商標))は、ゲムシタビンに基づいた治療後に進行している転移性膵臓腺癌を有する患者において、5-FUおよびLV(5-FU/LV)との併用での使用を認可されている。ゲムシタビンに基づいた治療後に進行している転移性膵臓腺癌を有する患者における中枢的な国際共同第III相NAPOLI-1試験において、5-FU/LVと併用したリポソーム型イリノテカンは、5-FU/LV対照治療と比較して、一次分析(313個のイベント後)および最終分析(382個のイベント後)の時点において、全生存期間(OS)中央値(プライマリーエンドポイント)および無増悪生存期間(PFS)中央値を有意に延ばした。奏効率(ORR)もまた、対照群においてよりリポソーム型イリノテカン+5-FU/LV群において有意に高かった。リポソーム型イリノテカンに基づいた併用療法は、管理可能な安全性プロファイルを有した;最もよく見られるグレード3重症度のTEAEは、実際は、血液学的または胃腸管系であった[26、27]。
膵癌は、低い早期診断率および予後不良である、膵臓の悪性新生物である。それの発生率は近年、上昇しており、それは、現在、一般的な腫瘍の1%~2%を占める。毎年、世界中で約185,000人の人がこの状態と診断されている。それの症状は、通常、非特異的であるため、膵癌は、それが進行期に達するまで診断されない場合が多い。膵癌の唯一の治癒の可能性がある治療は、外科的切除である。残念なことに、たった20%の患者における腫瘍だけが、診断の時点で切除可能である。膵癌の切除を受け、断端陰性を有するそれらの患者の中でさえも、5年生存率はたった10%~25%である[30、31]。患者の中での5年全生存率は、5%未満であり、それは、固形悪性腫瘍の中で最も高い死亡率に相当する。全体の生存期間中央値は、診断から1年未満であり、より新しい治療選択肢の開発の必要性を浮き彫りにしている。

画像診断評価
画像診断は、応答の評価に用いられる。例えば、CTまたはMRIが用いられ得る。その同じ画像診断法が、各対象について処置の間中、用いられるべきである。
処置への対象選択の時点
単剤療法期間の終了時、5日目
併用処置のサイクル2の終了時、およびその後、処置の1年間(サイクル17)まで3サイクルごと、およびその後、処置の終了まで4サイクルごと。
・放射線学的画像診断が最初の進行性疾患を立証したならば、下記の通り処置を継続するという選択肢に関して、進行性疾患を確認するために4週間以上後に、腫瘍評価が繰り返されるべきである。
・繰り返し画像診断が、ナディア(nadir)、安定な、または改善された、以前の新しい病変(最初の進行性疾患の原因として同定された場合)および安定な/改善された非標的病変(最初の進行性疾患の原因として同定された場合)と比較して腫瘍量の20%未満の増加を示すならば、処置は継続/再開され得る。
・繰り返し画像診断が、下記のシナリオのリストのいずれかによる進行性疾患を確認したならば、対象は治療を中止される。
・腫瘍量が増加または減少しているかどうかを決定することにおいて、全ての標的病変、加えて非標的病変が考慮される。
・腫瘍量が、依然として、ナディアと比較して、20%以上増加、および少なくとも5mmの絶対的増加のままである
・最初の進行性疾患を生じた非標的病変が悪化している(定性的)

上記で言及されているように、腫瘍画像診断が最初の疾患進行を示す場合には、その試験サイトは、irRECISTにより、処置を継続することを選択し、4週間以上後に画像診断を繰り返し、腫瘍応答または確認された進行を評価し得る。
特定の実施形態によれば、生検は、スクリーニング時点で実施することができ、遺伝子変異量(tumor mutation burden)(TMB)、PD-L1、およびDNAミスマッチ修復状態について評価される。
試料は、安全性および有効性分析、例えば、CBC、抗薬物抗体力価、およびBL-8040血漿濃度の決定のために収集される。
目的
本試験の目的は、転移性膵臓腺癌を有する患者において、リポソーム型イリノテカン(Onivyde(登録商標))/5-フルオロウラシル/ロイコボリン(5-FU/LV)と併用した、BL8040/ペムブロリズマブの有効性および安全性を評価することである。単独での、および併用で与えられた時のBL-8040の作用機構は、血液試料および腫瘍組織試料を用いてさらに研究される。

試験デザイン
これは、転移性膵臓腺癌を有する患者におけるオープンラベル、単群の第IIa相試験である。本試験は、以下の2つの期間からなる:
・単剤療法期間:1~5日目におけるBL-8040を用いる処置で、1週間

・6人の対象のうちの0人が、最初のサイクルの処置の間にDLTを経験する場合には、その併用処置は、さらなる評価のために適格とみなされ、躊躇なく募集は継続する。
・6人の対象のうちの1人が、DLTを経験する場合には、安全性DLTコホートは、12人の対象まで拡大される。
・6人の対象のうちの2人以上がDLTを経験する場合には、DMCは、その併用処置のリスク/便益比を評価し、以下のうちの1つまたは複数を勧告する:1)安全性DLTコホートは、12人の対象まで拡大されるべきである、2)プロトコールを修正されるべきである、3)処置スケジュールは変更されるべきである、または4)その試験は中止されるべきである。

試験集団

この治験の参加に適格であるために、対象は以下を満たさなければならない:
1.18歳以上

5.以前の処置ライン
第一選択のゲムシタビンに基づいた化学療法での処置を停止した後の客観的なX線検査での進行が実証されている。原発性転移性患者だけが、参加することが許される。膵癌について以前手術を受けた患者は、参加することが許されない。
6.腫瘍がアクセスできないとみなされない限り、またはそうでなければ、生検が患者にとって一番の利益にはならないとみなされない限り、好ましくは肝臓転移からの、評価可能な腫瘍組織試料を提出する意思がある。

11.対象は有効な避妊を用いなければならない:
a.女性対象は、妊娠不可能でなければならず、または、妊娠可能であるならば、試験薬物の摂取前の72時間以内に陰性の尿または血清妊娠検査結果を有しなければならない。尿検査が陽性であり、または陰性と確認することができない場合には、血清妊娠検査が必要とされる。血清妊娠検査は、対象が適格であるために陰性でなければならない。妊娠不可能は、以下として定義される(医学的理由以外による):
・45歳以上、2年間を超える期間、月経を生じたことがない
・子宮摘出および卵巣摘出なしに2年間より長い間の無月経、ならびに治験前(スクリーニング)評価での閉経後範囲内の卵胞刺激ホルモン(FSH)値
・スクリーニング時より少なくとも6週間前の子宮摘出、両側の卵巣摘出、両側の卵管開口術、または両側の卵管結紮後。文書化された子宮摘出または卵巣摘出は、実際の手順の医療記録で確認され、または超音波により確認されなければならない。卵管結紮は、実際の手順の医療記録で確認されなければならず、そうでなければ、対象は、スクリーニング来院時から始まって、試験治療の最後の用量後120日目まで、本試験の間中、2つの十分なバリア法を使用することを進んで受け入れなければならない。
男性対象は、試験治療の最初の用量から始まって、試験治療の最後の用量後120日目まで、十分な避妊方法を使用することに同意しなければならない。
対象は、以下である場合には、治験の参加から除外されなければならない:
1.腺房細胞癌、膵芽腫(pancreaticoblastoma)、悪性嚢胞性新生物、内分泌新生物、扁平上皮癌、ファーターおよび膨大部周囲の十二指腸または総胆管の悪性腫瘍を含む、腺癌以外の膵臓腫瘍を有する。
2.腸閉塞を有する対象。
3.全身性治療を必要とする活動性感染症を有し、または制御されない感染症を有する。
4.進行中であり、または積極的処置を必要とする、公知の追加の悪性腫瘍を有する。例外は、治癒の可能性がある治療を受けている十分に処置された基底細胞癌もしくは扁平上皮癌、または子宮頚部上皮内癌である。
5.試験参加を妨げると予想される基礎疾患を有する。
6.治癒目的で投与される治療に適している疾患を有する。
7.治験薬の試験に現在、参加して、試験治療を受けている最中であり、または処置の最初の用量の4週間以内に、参加して、試験治療を受け、もしくは治験デバイスを用いたことがある。
8.免疫不全と診断され、または治験処置の最初の用量前の7日以内に全身性ステロイド治療もしくは任意の他の型の免疫抑制治療を受けている。
9.試験1日目の前の4週間以内に先行の抗癌モノクローナル抗体(mAb)を受けたことがあり、または4週間より前に投与された作用物質によるAEから回復(すなわち、≦グレード1またはベースライン時)していない。
10.試験1日目の前の2週間以内に先行の化学療法、標的小分子治療、もしくは放射線療法を受けたことがあり、または以前に投与された作用物質によるAEから回復(すなわち、≦グレード1またはベースライン時)していない。
11.本試験に先立つ2年間における、全身性処置(すなわち、疾患修飾性剤、コルチコステロイド、または免疫抑制薬を用いる)を必要とした活動性自己免疫疾患。
12.試験1日目の前の4週間以内に血液製剤(例えば、血小板または赤血球)の輸血またはコロニー刺激因子(例えば、顆粒球コロニー刺激因子[G-CSF]、GM-CSF、または組換えエリスロポエチン)の投与を受けたことがある。
13.ステロイドを必要とした(非伝染性)肺臓炎の病歴を有し、または現在の肺臓炎。
14.間質性肺疾患の病歴を有する。
15.O2飽和<92%(室内空気において)。
16.不安定狭心症、過去3カ月以内の新規発症狭心症、過去6カ月以内の心筋梗塞、および現在のうっ血性心不全 ニューヨーク心臓協会(New York Heart Association)クラスIIIまたはそれ以上。試験開始前の6カ月未満に心室性不整脈もしくはコントロール不良の血圧、または重篤な動脈血栓塞栓症を有する。
17.本治験の結果を混乱させ、対象の本治験の全期間の参加を妨げ得る任意の状態、治療、もしくは検査異常の過去もしくは現在の証拠を有し、または、処置する治験責任医師の意見によれば、参加するのが対象にとって一番の利益とはならない。
18.本治験の要求との連携を妨げると予想される公知の精神医学的または物質乱用障害を有する。
19.スクリーニング来院時から始まって、治験処置の最後の用量後120日目までの治験の計画された期間内において、妊娠中であり、または授乳中であり、または子どもを身ごもり、もしくは父親になることを期待している。ベースラインから72時間以内に陽性妊娠検査結果を有する女性。
20.抗PD-1剤、抗PD-L1剤、もしくは抗PD-L2剤での前治療を受けたことがあり、または対象が以前、Merck MK-3475臨床試験に参加したことがある場合。
21.スクリーニング時点またはスクリーニング前の任意の時点において陽性HIV試験結果を有する。過去の陽性HIV試験結果がない患者は、地方条例により許可されないことがない限り、スクリーニング時にHIV試験を受ける。
22.活動的なB型肝炎(スクリーニング時における陽性B型肝炎表面抗原(HBsAg)検査結果を有するとして定義される)またはC型肝炎(スクリーニング時における陽性HCV抗体試験結果または陽性HCV RNA検査結果を有するとして定義される)をわかっている。
23.慢性B型またはC型肝炎の病歴をわかっている。
24.試験治療の計画された開始時点の30日以内に生ワクチンを受けたことがある。生ウイルスを含有しない季節性インフルエンザワクチンは許可される。

26.ペムブロリズマブおよび/またはそれの賦形剤のいずれかに対して重度の過感受性(≧グレード3)を有する。
27.処置を必要とする臨床的腹水を有する。
それぞれの同意した対象は、スクリーニング時点から試験の終わりまで、または対象が本試験を終了するまでに生じる全ての手順について対象を識別するために用いられる固有の対象番号を受け取る。各対象は、1つの対象番号だけを割り当てられる。対象番号は、異なる対象に再使用されてはいけない。
スクリーニング期間中の任意の段階でエントランス基準を満たすことができない対象は、スクリーニング不適格と定義される。

対象が試験から離脱した場合には、その理由を決定するためにあらゆる努力がなされるべきである。この情報は、対象の症例報告書(CRF)に記録される。時期尚早に試験から離脱した全ての対象は、原因を問わず、全ての早期中止試験来院手順、安全性追跡来院を行い、さらに、生存について長期追跡を継続するべきである。
試験薬の中止は、以下であるCRの確定を達成した対象について検討され得る:
・併用療法で少なくとも24週間、処置されている、および
・最初のCRが宣言された日付を越えて、少なくとも2処置サイクルの併用療法を受けた。
処置期間
処置期間は、以下の2つの期間で構成される:

サイクルは、ペムブロリズマブの処置の3週間サイクルスケジュールにより定義される。
・90分間に渡るIV Onivyde(登録商標) 70mg/m2、その後、30分間に渡るIV LV 400mg/m2、その後、46時間に渡るIV 5-FU 2400mg/m2、2週間ごと。
・IV ペムブロリズマブ 200mg、3週間に1回。
・10日目に始まる、SC BL-8040 1.25mg/kg、週2回で、化学療法投与から少なくとも24時間後。
1.全ての来院時における前投薬および同時投薬の審査。

9.腫瘍画像診断評価は、サイクル2の終わり、およびその後、処置の1年間(サイクル17)まで3サイクルごと、およびその後、サイクル34の終わり/終了来院(2年間の処置)まで4サイクルごとに行われる。
10.バイオマーカー相関性試験のための血清は、サイクル1/2回目のサイクルの化学療法より前の15日目、およびサイクル3に始まって2サイクルごとの21日目において、収集される(免疫細胞、受容体占有率、CA 19-9)。
BL-8040
BL-8040での治験処置は、1日目から始まって毎日5日目まで1週間、単剤療法として1日あたり1.25mg/kgのSC注射により投与される。対象は、1日1回、BL-8040のSC注射を午前中に受ける。鎮痛剤と共のまたは鎮痛剤なしでの、全身性抗ヒスタミン剤での前投薬は、BL-8040に関係した局所注射部位反応および/または全身性反応の発生を最小限するために推奨される。
・併用期間中、BL-8040は、化学療法から少なくとも24時間後の、48時間離れた連続していない日に週2回、SC注射として午前中に投与される。
ペムブロリズマブでの処置は、単剤療法期間後に、併用療法の一部として始まる。併用期間中、ペムブロリズマブは、全ての手順および評価が完了した後に、各3週間の処置サイクルの1日目における30分間のIV注入を用いる200mgの用量として投与される。ペムブロリズマブは、管理上の理由により、予定された各サイクルの1日目の前後3日までに投与され得る。
Onivydeは、LVおよび5-FUの前に投与されるべきである。2週間ごとに、90分間に渡るIV注入としてOnivyde 70mg/m2、その後、30分間に渡るLV 400mg/m2 IV、その後、46時間に渡る5-FU 2400mg/m2 IV。UGT1A1*28アレルについてホモ接合の患者は、Onivyde(登録商標)を50mg/m2で開始し、その用量は、その後のサイクルにおいて、忍容性が認められる場合には、増加させることができる。
安全性追跡来院
終了/早期終了来院後、安全性追跡来院は、最後の試験薬の最後の用量からおよそ30日後(または新しい抗癌処置の開始前の7日以内、いずれか早い方)に、実施される。対象は、安全性追跡来院まで、またはグレード0~1への毒性の解消もしくはAE安定化まで、いずれか遅い方まで、AEについてモニターされる。SAEは、試験終了から90日以内に発生した場合に、および妊娠は、試験終了から120日以内に発生した場合に、またはどちらの場合においても、新しい抗癌治療が開始されたならば、停止後30日以内の場合、報告されるべきである。

有効性評価
画像診断評価
画像診断は、応答の評価のために用いられる。例えば、CTまたはMRIが用いられ得る。同じ画像診断法が、各対象について処置を通して用いられるべきである。
・処置への対象選択の時点
・単剤療法期間の終わり、5日目
・併用処置期間のサイクル2の終わり、およびその後、処置の1年間(サイクル17)まで3サイクルごと、およびその後、処置の終わりまで4サイクルごと。
・放射線学的画像診断が最初の進行性疾患を実証した場合には、腫瘍評価は、以下の通り、処置を継続するという選択肢をもって、進行性疾患を確認するために、4週間以上後に繰り返されるべきである。
・繰り返し画像診断が、ナディア、安定な、または改善した過去の新しい病変(最初の進行性疾患の原因として同定された場合)および安定/改善した非標的病変(最初の進行性疾患の原因として同定された場合)と比較して腫瘍量の20%未満の増加を示す場合には、処置は継続/再開され得る。
・繰り返し画像診断が、下記に列挙されたシナリオのいずれかによる進行性疾患を確認した場合には、対象は治療を中止される。
・腫瘍量が増加または減少したかどうかを決定することにおいて、全ての標的病変、加えて非標的病変が考慮される。
・腫瘍量が、ナディアと比較して、依然として、20%以上増加および少なくとも5mm絶対的増加のままである。
・最初の進行性疾患を生じた非標的病変が悪化している(質的)
・最初の進行性疾患を生じた新しい病変が悪化している(質的)
・最後の評価以降の追加の新しい病変
・疾患進行を示す徴候および症状がない

上記で言及されているように、腫瘍画像診断が最初の疾患進行を示すならば、そのサイトは、処置を継続することを選択し、4週間以上後に画像診断を繰り返し、irRECISTにより腫瘍応答または進行の確認を評価し得る。
特定の実施形態によれば、生検は、スクリーニング時に実施することができ、腫瘍変異量(TMB)、PD-L1、およびDNAミスマッチ修復状態について評価される。

BL-8040
BL-8040は、癌の処置のための新規な治験治療としてBiokine Therapeutics,Ltd.およびBioLineRx Ltd.により共同開発された高選択性CXCR4アンタゴニストである。
注入用ペムブロリズマブ(MK-3475)溶液は、100mg/4mLのペムブロリズマブ(MK-3475)を含有する、使い捨てのI型ガラスバイアルにおいて供給される無菌の非発熱性水溶液である。その製品は、本質的に外来性微粒子を含まない、保存剤なしの溶液である。
Onivydeは注射剤:単一用量バイアル中の、白色~黄味がかった不透明なリポソーム分散体としての43mg/10mL イリノテカン遊離塩基である。
転移性膵臓腺癌(PDAC)を有する患者におけるペムブロリズマブおよび化学療法と併用したBL-8040の安全性および有効性を評価するための第2a相治験
背景
PDACについての現在の処置選択肢は限られている。PD-1/PD-L1アンタゴニストは他の癌型において有望な結果を示しているが、このアプローチは、PDACにおいて無効であった。COMBAT試験のコホート1において、BL-8040(CXCR4阻害剤)およびペムブロリズマブの二重併用は、安全であり、第二選択(2L)患者において有望な7.5カ月間の全生存(OS)中央値を示した(Hidalgo, A., et al., A Phase 2a Trial to Assess the Safety and Efficacy of BL-8040 and Pembrolizumab in Patients with Metastatic Pancreatic Adenocarcinoma (PDAC). 2019年9月の時点で、22人の患者が登録され、そのうち15人が評価可能である(すなわち、併用の少なくとも1回の用量を受け、ベースライン後のCTを有する)。年齢中央値68歳、ECOG≦1、および60%男性。15個のSAEが10人の患者により報告された。2人の対象は、SAEにより中止された。評価可能な集団についてのRECISTv1.1による最良の応答(実施例6)は、4人の部分奏効(PR)および8人の安定な疾患(SD)の患者、15人のうち病勢コントロールを有する合計12人の対象を示した。PFSおよびOSの中央値はまだ到達していなかった。特に、SDおよびPRを有する全ての患者は、CA 19-9の最初の増加、その後、減少を生じた。腫瘍縮小は、CA 19-9の一過性増加中に始まった。
BL-8040、ペムブロリズマブ、および化学療法の三重併用での進行中のCOMBAT試験コホート2からの予備データは、有望な全奏効率(ORR)(4/15)および病勢コントロール率(DCR)(12/15)を示している。
Claims (25)
- 必要とする対象において転移性膵臓腺癌を処置する方法であって、配列番号1に記載のペプチド、抗PD-1、および化学療法のそれぞれの治療的有効量を前記対象に投与すること、それにより、前記転移性膵臓腺癌を処置することを含む、方法。
- 必要とする対象において転移性膵臓腺癌を処置することにおける使用のための、配列番号1に記載のペプチド、抗PD-1、および化学療法のそれぞれの治療的有効量。
- 転移性膵臓腺癌の処置における使用のための、配列番号1に記載のペプチド、抗PD-1、および化学療法のそれぞれの治療的有効量を含む製造品。
- 前記抗PD-1がペムブロリズマブである、請求項1~3のいずれか一項に記載の方法または治療的有効量または製造品。
- 前記化学療法が複数の化学療法剤を含む、請求項1~4のいずれか一項に記載の方法または治療的有効量または製造品。
- 前記化学療法が、イリノテカン、フルオロウラシル(5-FU)、およびロイコボリン(LV)を含む、請求項1~5のいずれか一項に記載の方法または治療的有効量または製造品。
- 前記イリノテカンがリポソームに封入されている、請求項1~6のいずれか一項に記載の方法または治療的有効量または製造品。
- 前記イリノテカンがOnivyde(登録商標)である、請求項1~7のいずれか一項に記載の方法または治療的有効量または製造品。
- 前記ペプチドが皮下に(SC)投与される、請求項1~8のいずれか一項に記載の方法または治療的有効量または製造品。
- 前記ペプチドが1.25mg/kgの用量で投与される、請求項1~9のいずれか一項に記載の方法または治療的有効量。
- 前記抗PD-1が静脈内に(IV)投与される、請求項1~10のいずれか一項に記載の方法または治療的有効量。
- 前記抗PD-1が200mgの用量で投与される、請求項1~11のいずれか一項に記載の方法または治療的有効量。
- 前記化学療法が静脈内に投与される、請求項1~12のいずれか一項に記載の方法または治療的有効量。
- 前記処置が、前記ペプチドでの単剤療法期間、それに続く、前記ペプチド、前記抗PD-1、および前記化学療法での併用療法を含む、請求項1~13のいずれか一項に記載の方法または治療的有効量。
- 前記化学療法および前記抗PD-1での前記併用療法が8日目に開始される、請求項1~14のいずれか一項に記載の方法または治療的有効量。
- 前記併用療法が、前記化学療法を2週間ごとに繰り返し、前記抗PD-1を3週間ごとに繰り返すものである、請求項14または15に記載の方法または治療的有効量。
- 前記ペプチドでの前記併用療法が、10日目に開始され、48時間離れた連続していない日に週2回である、請求項15に記載の方法または治療的有効量。
- 前記単剤療法が1~5日目に毎日実施される、請求項1~17のいずれか一項に記載の方法または治療的有効量。
- 前記処置がさらに、抗ヒスタミン剤、および任意で、鎮痛剤を含む、請求項18に記載の方法または治療的有効量。
- 前記併用療法が、35回の処置まで継続する、請求項1~19のいずれか一項に記載の方法または治療的有効量。
- 前記対象が、前記転移性膵臓腺癌に対する第一選択処置後である、請求項1~20のいずれか一項に記載の方法または治療的有効量。
- 前記第一選択処置が、ゲムシタビンに基づいた化学療法を含む、請求項21に記載の方法または治療的有効量。
- 前記転移性膵臓腺癌が切除不能である、請求項1~21のいずれか一項に記載の方法または治療的有効量。

- 前記転移性膵管腺癌が膵管内乳頭粘液性新生物を含む、請求項24に記載の方法または治療的有効量。
