Abstract

本出願は、2018年9月27日に出願された米国仮出願第62/737,155号の優先権を主張し、該仮出願の全内容はこれにより参照により本明細書に組み込まれる。
がんなどの所与の状態を治療するための2つの薬物の投与は多数の潜在的な問題を生じさせる。2つの薬物の間のインビボ相互作用は複雑である。任意の単一の薬物の効果はその吸収、分布、および排除と関係する。2つの薬物が身体に導入された場合、各薬物は他方の吸収、分布、および排除に影響し、それゆえ他方の効果を変更させることがある。例えば、1つの薬物は、他の薬物の排除の代謝経路に関与する酵素の産生を阻害し、活性化させまたは誘導し得る(44)。一例では、グラチラマー酢酸塩(GA)およびインターフェロン(IFN)の併用投与は、いずれかの療法の臨床的有効性を妨げることが実験的に示されている(49)。別の実験において、IFN−βとの併用療法におけるプレドニゾンの添加はその上方調節因子効果をアンタゴナイズすることが報告された(48)。そのため、同じ状態を治療するために2つの薬物が投与される場合、それぞれが対象において他方の治療活性を補うのか、それに対して効果を有しないのか、それともそれに干渉するのかは予測することができない。

抗体およびタンパク質。対照IgG1、オマリズマブはNovartis(Basel、Switzerland)から購入した。ヒトSFRP2組換えタンパク質(SFRP2)は以前に記載されたように(23)調製され、ノースカロライナ大学チャペルヒル校のthe Protein Expression and Purification Core Labにより提供された。ヒト化SFRP2モノクローナル抗体(hSFRP2 mAb)は、以前に記載され、実施例4に記載されるように製造し、エンドトキシンから精製した。
1.治療有効量のSFRP2アンタゴニスト、CD38アンタゴニスト、および/またはPD−1アンタゴニストならびに治療有効量のPD−1アンタゴニストを含む、薬学的組合せ。

4.前記SFRP2モノクローナル抗体がヒトまたはヒト化抗体である、パラグラフ3に記載の薬学的組合せ。
5.前記PD−1アンタゴニストが、
a.特異的にPD−1受容体に結合してその活性化を阻害する抗体、もしくは抗体の抗原結合断片、または
b.特異的にPD−1リガンドに結合して前記PD−1リガンドの前記PD−1受容体への結合を阻害する可溶性形態のPD−1受容体

69.前記SFRP2モノクローナル抗体の周期投与が少なくとも3日間、少なくとも30日間、少なくとも42日間、少なくとも8週間、少なくとも12週間、少なくとも24週間または少なくとも6か月間継続される、パラグラフ62〜67のいずれか1つに記載の方法。
70.前記対象がヒト患者である、パラグラフ62〜69のいずれか1つに記載の方法。
Claims (69)
- 治療有効量のSFRP2、CD38、および/またはPD−1アンタゴニストならびに治療有効量のPD−1アンタゴニストをそれを必要とする対象に投与することを含む、がんの治療方法。
- 前記投与が同時または逐次的なものである、請求項1に記載の方法。
- 前記SFRP2、CD38、および/またはPD−1アンタゴニストが、
a.特異的にSFRP2、CD38、および/もしくはPD−1受容体に結合してその活性化を阻害する抗体、もしくは抗体の抗原結合断片、または
b.特異的にSFRP2および/もしくはCD38リガンドに結合して前記SFRP2、CD38、および/もしくはPD−1リガンドの前記SFRP2、CD38、および/もしくはPD−1受容体への結合を阻害する可溶性形態のSFRP2、CD38、および/もしくはPD−1受容体
である、請求項1に記載の方法。 - 前記SFRP2、CD38、および/またはPD−1アンタゴニストがSFRP2、CD38、および/またはPD−1モノクローナル抗体(mAb)である、請求項1に記載の方法。
- 前記SFRP2モノクローナル抗体がヒトまたはヒト化抗体である、請求項4に記載の方法。
- 前記PD−1アンタゴニストが、
a.特異的にPD−1受容体に結合してその活性化を阻害する抗体、もしくは抗体の抗原結合断片、または
b.特異的にPD−1リガンドに結合して前記PD−1リガンドの前記PD−1受容体への結合を阻害する可溶性形態のPD−1受容体
である、請求項1に記載の方法。 - 前記PD−1リガンドがPD−L1またはPD−L2である、請求項6に記載の方法。
- 前記PD−1アンタゴニストがPD−1モノクローナル抗体である、請求項1に記載の方法。
- 前記PD−1アンタゴニストがニボルマブである、請求項1に記載の方法。
- 前記PD−1アンタゴニストがペムブロリズマブ、アベルマブ、デュルバルマブ、セミプリマブ、またはアテゾリズマブである、請求項1に記載の方法。
- 前記がんが乳がんである、請求項1に記載の方法。
- 前記がんが血管肉腫、肺がん、骨肉腫、黒色腫、非小細胞肺がん、または腎臓がんである、請求項1に記載の方法。
- 前記SFRP2、CD38、および/またはPD−1アンタゴニストの前記投与が前記PD−1アンタゴニストの前記投与に先行する、請求項1に記載の方法。
- 前記PD−1アンタゴニストの前記投与がSFRP2、CD38、および/またはPD−1アンタゴニストの前記投与に先行する、請求項1に記載の方法。
- 前記SFRP2、CD38、および/またはPD−1アンタゴニストが前記PD−1アンタゴニストに付随して投与される、請求項1に記載の方法。
- 前記PD−1アンタゴニストが前記SFRP2、CD38、および/またはPD−1アンタゴニストに付随して投与される、請求項1に記載の方法。
- 前記SFRP2、CD38、および/またはPD−1アンタゴニストが1日毎、1日1回より高い頻度または1日1回より低い頻度で投与される、請求項1に記載の方法。
- 前記SFRP2アンタゴニストが3日毎に1回、週毎に1回、2週毎に1回、3週毎に1回または4週毎に1回投与される、請求項1に記載の方法。
- 前記PD−1アンタゴニストが1日毎、1日1回より高い頻度または1日1回より低い頻度で投与される、請求項1に記載の方法。
- 前記PD−1アンタゴニストが3日毎に1回、週毎に1回、2週毎に1回、3週毎に1回または4週毎に1回投与される、請求項1に記載の方法。
- 前記PD−1アンタゴニストがニボルマブであり、かつ前記対象に投与される前記ニボルマブの量が3週毎の3mg/kg体重、2週毎の240mgまたは4週毎の480mgである、請求項1に記載の方法。
- 前記PD−1アンタゴニストがペムブロリズマブであり、かつ前記対象に投与される前記ペムブロリズマブの量が3週毎の200mgである、請求項1に記載の方法。
- 前記PD−1アンタゴニストがアベルマブであり、かつ前記対象に投与される前記アベルマブの量が2週毎の800mgである、請求項1に記載の方法。
- 前記PD−1アンタゴニストがデュルバルマブであり、かつ前記対象に投与される前記デュルバルマブの量が2週毎の10mg/kg体重である、請求項1に記載の方法。
- 前記PD−1アンタゴニストがセミプリマブであり、かつ前記対象に投与される前記セミプリマブの量が3週毎の250mgである、請求項1に記載の方法。
- 前記PD−1アンタゴニストがアテゾリズマブであり、かつ前記対象に投与される前記アテゾリズマブの量が2週毎の840mg、3週毎の1200mgまたは4週毎の1680mgである、請求項1に記載の方法。
- 前記対象が、SFRP2、CD38、および/またはPD−1アンタゴニスト療法を開始する前にPD−1アンタゴニスト療法を与えられている、請求項1に記載の方法。
- 前記対象が、PD−1アンタゴニスト療法を開始する前にSFRP2、CD38、および/またはPD−1アンタゴニスト療法を与えられている、請求項1に記載の方法。
- 前記対象が、第2の療法を開始する前に第1の療法を少なくとも8週間、少なくとも10週間、少なくとも24週間、少なくとも28週間、少なくとも48週間または少なくとも52週間与えられている、請求項27または28に記載の方法。
- 前記SFRP2、CD38、および/もしくはPD−1アンタゴニストならびに/または前記PD−1アンタゴニストの周期投与が少なくとも3日間、少なくとも30日間、少なくとも42日間、少なくとも8週間、少なくとも12週間、少なくとも24週間または少なくとも6か月間継続される、請求項1に記載の方法。
- 単独で摂取される場合のSFRP2、CD38、および/またはPD−1アンタゴニストの量のそれぞれ、ならびに単独で摂取される場合のPD−1アンタゴニストの量が、前記対象を治療するために有効なものである、請求項1に記載の方法。
- 単独で摂取される場合のSFRP2、CD38、および/もしくはPD−1アンタゴニストの量、単独で摂取される場合のPD−1アンタゴニストの量、または単独で摂取される場合の各そのような量のいずれかが、前記対象を治療するために有効なものではない、請求項1に記載の方法。
- 単独で摂取される場合のSFRP2、CD38、および/もしくはPD−1アンタゴニストの量、単独で摂取される場合のPD−1アンタゴニストの量、または単独で摂取される場合の各そのような量のいずれかが、前記対象を治療するのに有効性が低い、請求項1に記載の方法。
- 前記対象がヒト患者である、請求項1に記載の方法。
- 前記患者が、前記併用療法を与えられる前にPD−1アンタゴニスト療法を以前に与えられ、かつPD−1アンタゴニスト療法を与えることを中止された患者である、請求項1に記載の方法。
- 前記患者が以前にPD−1アンタゴニスト療法に応答しなかったか、または前記PD−1アンタゴニストが前記対象の治療に失敗した、請求項35に記載の方法。
- 治療有効量のSFRP2、CD38、および/またはPD−1アンタゴニスト、治療有効量のPD−1アンタゴニスト、ならびに前記キットの使用のための指示を含む挿入物を含む、がんを患う患者を治療するためのキット。
- ある量のPD−1アンタゴニスト、ならびにある量のSFRP2、CD38、および/またはPD−1アンタゴニストを含む、医薬組成物。
- ある量のPD−1アンタゴニスト、ならびにある量のSFRP2、CD38、および/またはPD−1アンタゴニストを本質的に含む、請求項38に記載の医薬組成物。

- 医薬の製造において使用するための、SFRP2、CD38、および/またはPD−1アンタゴニストならびにPD−1アンタゴニストの組合せ。
- 前記医薬ががんの治療、予防、またはその症状の緩和のためのものである、請求項45に記載の組合せ。

である、請求項47に記載の薬学的組合せ。 - 前記SFRP2、CD38、および/またはPD−1アンタゴニストがSFRP2、CD38、および/またはPD−1モノクローナル抗体(mAb)である、請求項47に記載の薬学的組合せ。
- 前記SFRP2モノクローナル抗体がヒトまたはヒト化抗体である、請求項47に記載の薬学的組合せ。
- 前記PD−1アンタゴニストが、
a.特異的にPD−1受容体に結合してその活性化を阻害する抗体、もしくは抗体の抗原結合断片、または
b.特異的にPD−1リガンドに結合して前記PD−1リガンドの前記PD−1受容体への結合を阻害する可溶性形態のPD−1受容体
である、請求項47に記載の薬学的組合せ。 - 前記PD−1リガンドがPD−L1またはPD−L2である、請求項51に記載の薬学的組合せ。
- 前記PD−1アンタゴニストがPD−1モノクローナル抗体である、請求項47に記載の薬学的組合せ。
- 前記PD−1アンタゴニストがニボルマブである、請求項47に記載の薬学的組合せ。
- 前記PD−1アンタゴニストがペムブロリズマブ、アベルマブ、デュルバルマブ、セミプリマブ、またはアテゾリズマブである、請求項47に記載の薬学的組合せ。
- SFRP2、CD38、および/またはPD−1アンタゴニストの前記治療有効量が0.1mg/kg体重〜100mg/kg体重である、請求項47に記載の薬学的組合せ。
- 前記SFRP2、CD38、および/またはPD−1アンタゴニストの前記治療有効量が0.2〜3、0.27〜2.70、0.27、0.54、1.35、または2.70mg/kg体重である、請求項47に記載の薬学的組合せ。
- SFRP2、CD38、および/またはPD−1アンタゴニストの前記治療有効量が10mg〜200mg、17mg、33mg、84mg、または167mgである、請求項47に記載の薬学的組合せ。
- PD−1アンタゴニストの前記治療有効量が0.1mg/kg体重〜100mg/kg体重である、請求項47に記載の薬学的組合せ。
- PD−1アンタゴニストの前記治療有効量が0.02〜1.2、0.027〜1.08、0.027または1.08mg/kg体重である、請求項47に記載の薬学的組合せ。
- PD−1アンタゴニストの前記治療有効量が1〜80、1.6〜67、1.6または67mg/kg体重である、請求項47に記載の薬学的組合せ。
- 治療有効量のSFRP2モノクローナル抗体(mAb)をそれを必要とする対象に投与することを含み、前記対象がCD38および/またはPD−1の増加した発現を有する、がんの治療方法。
