Abstract
Description
TG2阻害剤とCK2阻害剤、好ましくはシステアミンとECGCの組み合わせに関しては、米国特許出願公開第2013/0310329号を参照とする。
ハムスターの子における腎線維芽細胞でCFTRを検出するために使用される抗ヒト嚢胞性線維症膜貫通コンダクタンスレギュレーター(CFTR)抗体(「抗体596」)、CFTR矯正薬(コレクター)「C18」(CF-106951;WO2007/021982参照。1-(ベンゾ[d][1,3]ジオキソール-5-イル)-N-(5-((2-クロロフェニル)(3-ヒドロキシピロリジン-1-イル)メチル)チアゾール-2-イル)シクロプロパンカルボキサミド)は、Cystic Fibrosis Foundation Therapeutics Chemical and Antibody Distribution Programs (嚢胞性線維症財団治療薬・抗体配布プログラム)(http://www.cff.org/research) から得た。John Riordan 博士(University of North Carolina Chapel Hill)から「596 CFTR抗体」が、Robert Bridges 博士(Rosalind Franklin University of Medicine and Science, Chicago, USA)からC18化合物が提供された。596 CFTR抗体は、アミノ酸1204から1211(すなわち、ヌクレオチド結合ドメイン2に位置する)を標的としている(方法と試薬・参考文献1)。フルオレセイン標識試薬S1P(FITC-S1P)は,Echelon Biosciences社(Cedarlane Laboratories社経由;Burlington, Canada)から購入した。ルマカフトールはSelleck Chemicals社(Cedarlane Laboratories社)から、組換え腫瘍壊死因子はSigma-Aldrich Canada社(Oakville, Canada, cat# T6674)から、プロテアーゼ阻害剤カクテルタブレット(Complete(登録商標);ウェスタンブロット溶解バッファーに使用)はRoche社(Mississauga, Canada)から購入した。プロテアーゼインヒビターカクテルタブレット(Complete(登録商標);ウェスタンブロット溶解バッファーに使用)はRoche社(カナダ、ミシソーガ)から購入した。その他の化学試薬はすべてSigma-Aldrich社から購入した。MOPSバッファーには、NaCl 145 mmol/L 、KCl 4.7 mmol/L、CaCl2 3.0 mmol/L、MgSO4 -7H2O 1.17 mmol/L、NaH2PO4 -2H2O 1.2 mmol/L、ピルビン酸 2.0 mmol/L、エチレンジアミン四酢酸(EDTA)0.02 mmol/L、3-モルホリノプロパンスルホン酸(MOPS)3.0 mmol/L、グルコース5.0 mmol/L の成分が含まれている。
Hospital for Sick Children(Toronto)で確立されたコロニーから、ΔF508 CFTR突然変異体(CFTRtm1EUR;本研究では「ΔF508」と示す)(方法と試薬・参考文献2)、CFTR遺伝子欠損体(CFTRtm1Unc;CFTR-/-と命名)、および相補的野生型コントロール同腹仔、のホモ接合の雄マウスを入手した(すべての CFTR-/-、CFTRΔF508、野生型同腹仔は混合系統)。CFTR-/-マウスは麻酔下で非常に死亡しやすいことがわかった。この死亡率は,脳動脈の分離を伴う実験(図3Cおよび5Cなど)では問題ではなかったが,脳血流や全身の血行動態の測定に大きな影響を与えた。CFTR-/-マウスとは異なり、CFTRΔF508変異マウスは麻酔中に死に至ることはほぼなかった。CFTRΔF508の突然変異は、CFTRの細胞膜への輸送を著しく低下させるため(方法と試薬・参考文献3)、CFTRΔF508マウスモデルではCFTRの活性が非常に低い(方法と試薬・参考文献2,4,5)。したがって、CFTRΔF508モデルは、脳血流や全身の血行動態の測定において、CFTR-/- マウスの適切な代替えとなる。CFTR-/-の死亡率の根本的な原因については追求しなかった。CFTR-/- もCFTRΔF508も、ストレスに対して不利に反応することが広く知られている。その例として、飼育環境や輸送事情は、死亡率を著しく増加させる2つの顕著なストレス要因である(方法と試薬・参考文献6)。興味深いことに、CFTRΔF508マウスは、CFTR-/-マウスよりも重篤な表現型を示さない(方法と試薬・参考文献5,6)。これは、CFTRΔF508動物に存在する少量の残存CFTR活性によるものと推定される(方法と試薬・参考文献2)。このことから、CFTR-/-マウスは動物施設内の外部環境ストレス要因(飼育環境、騒音、ハンドリングなど)により敏感に反応し、最終的に麻酔下での死亡率が高くなったのではないかと予想される。実験モデルとしてのCFTRΔF508マウスは、CFTRの機能低下に伴ういくつかの異常を持っている。腸の合併症は、CFTR突然変異の最も顕著な病理学的影響であり、これは出生後の死亡率の主要原因でもある(方法と試薬・参考文献6,7)。共通の消化器系問題には、濃粘液貯留、運動障害および腸閉塞の強い傾向、が含まれる。CFTRΔF508マウスは、野生型のマウスに比べて体重/サイズが40から50%小さいが、成人まで元気に成長する (方法と試薬・参考文献6,7)。他にもいくつかの野生型マウスとの違いが観察されているが、これらは比較的小さいと思われる(方法と試薬・参考文献6,7)。例えば、特定の組織(腎臓、胆嚢、鼻腔上皮など)では、塩素電流やナトリウム輸送の変化が認められるが、組織が組織学的に損なわれているとは思えないため、これらの違いの病理学的な意味は明らかではない(方法と試薬・参考文献6,7)。なお、CFTRΔF508マウス(および一般的なCFTR変異マウス)の嚢胞性線維症の表現型は、ヒトの嚢胞性線維症と比較してかなり軽度である。CFTRΔF508マウスの下気道は基本的に正常であり、下気道上皮の異常は見られず、チャレンジなしでは肺炎を自然に発症しない (方法と試薬・参考文献6,7)。膵臓、胆嚢、肝臓、胆管、男性生殖管、涙腺、顎下腺は明らかな病理組織を示さない(方法と試薬・参考文献5から7)。重度の嚢胞性線維症の表現型が見られないのは、マウスの特定の組織にCFTRではないカルシウム依存性塩素チャネル(CACC)が発現し、CFTRの欠損を補っていることが一因である(方法と試薬・参考文献7,8)。
既述のとおり(方法と試薬・参考文献9-11)、心エコーの測定は、平均動脈圧(MAP)の測定(Millar SPR-671 micro-tip mouse pressure catheter; Inter V Medical Inc, Montreal, Canada)とともに、30MHzの機械式セクタトランスデューサ(Vevo 770; Visual Sonics, Toronto, Canada)を用いて収集した。大動脈流量の時間流速積分値(VTI)は、大動脈基部のすぐ上でパルス波ドップラーを用いて測定した。大動脈の断面積(CSA)は、大動脈基部の寸法(ARD)から算出した。CSA = π(ARD/2)2. ストローク量(SV)、COおよびTPRは以下のように算出した。SV = CSA x VTI; CO = SV x HR; TPR = MAP/CO。
これまでに述べたように(方法と試薬・参考文献10-12),我々は脳血流(CBF)を評価するために,flow-sensitive alternating inversion recovery(FAIR)法(造影剤を用いずに反転パルスによるラベリングを利用する方法)磁気共鳴画像(MRI)技術を用いた(13)。撮像中、マウスは1.8%のイソフルランをノーズコーンから投与されて固定された。体温を維持するため、外部のヒーターポンプから水を転換する、埋め込まれたチューブをMRIは備えた。呼吸は空気圧式枕(SA Instruments, Stonybrook, USA)を用いてモニターし、イソフルラン濃度は呼吸数が一分当たり約50回分になるように調整した。
CBF = l (1/T1,ss - 1/T1,ns) (ml/(100g×min)
ここで,「ss」と「ns」はスライス選択型と非選択型の測定を示し,lは脳血液分配係数で,脳組織1gあたりの水分濃度と血液1mlあたりの水分濃度の比として定義される。この係数は、マウスでは約90ml/100gである(方法と試薬・参考文献14)。
本研究で用いたFAIR最適化は,前述の断熱反転を用いたsingle-shot echo planar imaging(シングルショットEPI)法であった。パラメータは、エコー時間12.5 ms、繰り返し時間17秒、25〜6825msの範囲で400ms刻みで反転時間18回、3mmのスライス選択型反転スラブ、18×18mmの視野、72×72のマトリクスで250μmの面内分解能、1mmのスライス厚、10分12秒のデータ取得時間であった。撮像は、前脳、中脳、後脳の垂直断面において、前循環、混合循環、後循環に対応して繰り返し行われた。FAIR画像は,大脳半球下の関心領域(ROI)、「グローバル」と定義する、と大脳皮質および皮質下実質組織,中脳皮質および脳室傍実質組織,後脳皮質および中脳実質組織に対応する局所的な関心領域(ROI)を手動で設定(MIPAV, NIH, Bethesda, MD; http://mipav.cit.nih.gov)して評価した。ROIは,スキャン内の動きを手動で補正できるように,T1強調信号画像上に直接描画した。ROIをパラメトリックCBFマップに登録し,高灌流血管や髄膜からの偏りがないことを確認した。T1回帰とCBF計算はMatlab(Mathworks, Natick, MA)を用いて行った。報告されているすべてのCBF値は、本研究で使用したイソフルランレベルのマウスCBF測定値として公表されている範囲内に収まっている(15)。EPIは、特に位相エンコード方向に磁化率に起因する歪みが生じやすい(16)が、今回のFAIRプロトコルでは垂直方向に相当する。図8に示すように,くも膜下出血(SAH)の手術モデルでは,術後の撮像タイムポイントで脳のすぐ近くに空気と組織の境界があるため,シミングを行ってもEPIの歪みを十分に抑えることができなかった。あるケースでは、歪みがひどく、ROIを大脳皮質のより外側に配置する必要があった(図8)。しかし、歪みはすべてのT1強調画像で一貫しているため、歪んだ領域が他の領域と重ならない限り、T1測定に偏りはない。各コホート内で比較的一貫したns-T1値(コホート間の標準偏差7%、コホート内での治療群の違いなし)が得られたことから、歪曲バイアスがCBF測定に与える影響はごくわずかであることが裏付けられた。
腸間膜動脈平滑筋細胞の分離・培養の手順は、(方法と試薬・参考文献10)に記載されている通りに行った。簡単に言うと、腸間膜動脈セグメントを分離し、小片に切断し、トリプシン、コラゲナーゼ、エラスターゼで消化した。得られた細胞懸濁液をリン酸緩衝生理食塩水で数回洗浄した後、10%牛胎児血清と1%ペニシリン-ストレプトマイシンを含むダルベッコ改変イーグル培地(DMEM)に播種した。細胞培養は37℃、5% CO2で維持し、106細胞の播種密度で分割した。野生型ヒトCFTR(方法と試薬・参考文献10,17)を安定的に発現するベビーハムスター腎臓線維芽細胞を、5%ウシ胎児血清および250μmol/Lメトトレキサート(CFTR導入遺伝子プロモーターを活性化する)を含むDMEM/F12培地で維持した。細胞は、37℃、5%CO2の標準的な細胞培養条件で維持した。
以前述べられてように(方法と試薬・参考文献10)一層の細胞(処理済み又は未処理)を1μmol/L S1P-FITCで60分間インキュベートし、その後、トリプシンで細胞を剥離し、氷冷したPBSで2回洗浄した後、35μmのセルストレーナーでろ過し、FACS DIVA version 6.1ソフトウェアで操作されるBecton-Dickinson社製FACS Cantoを用いて解析した。非標識S1Pで処理した一層の細胞をバックグラウンドコントロールとした。解析方法は、各細胞集団の平均蛍光強度(任意単位)を求め、これを取り込みの指標とした。
ヨウ化物流出量の測定は,既述の方法で行った(方法と試薬・参考文献18)。コンフルエントな細胞を、HEPESベースのローディングバッファー(136 mmol/L NaI, 3 mmol/L KNO3, 2 mmol/L Ca(NO3)2, 11 mmol/L glucose and 20 mmol/L HEPES; pH 7.4)中で1時間インキュベートすることにより、ヨウ化物を負荷した(標準的な細胞培養条件である37℃、5% CO2下で)。ヨウ化物負荷後、細胞はヨウ化物を含まない溶出バッファー(NaNO3をNaIに代えたもの)で4回洗浄した。上清のヨウ化物レベルは、ヨウ化物選択電極(Lazar Research Laboratories;Los Angeles, USA)を用いて定量し、NaI標準液で校正した。刺激前のヨウ化物レベルを測定した後、10μmol/L フォルスコリン、1mmol/L イソブチルメチルキサンチン、100μmol/L cpt-cAMPを含むカクテル(1% v/v DMSO中)で細胞を刺激した。刺激カクテルはPKAを強く活性化し、その結果、CFTRを最大限にリン酸化/活性化する。上清のヨウ化物レベルは、刺激後1分間隔で7回連続して測定した。以前の結果(方法と試薬・参考文献18)と同様に、溶出は最初の1分以内に明らかになり、60-120秒の間に確実に最大になった。したがって、最大流出速度は、刺激後60-120秒の間に測定された流出レベルを用いて算出した(方法と試薬・参考文献18)。
CFTRのウェスタンブロットは,既述の方法で行った(方法と試薬・参考文献12,18)。脳動脈溶解物は、50mM Tris(pH7.3)、150mM NaCl、2mM ETDA、0.1% Triton-X-100、0.1% SDSおよびプロテアーゼインヒビターを含むライシスバッファー中で動脈サンプルを粉砕して調製した。培養細胞溶解物の調製にも同じライシスバッファーを用いた。溶解後、遠心分離(13,500g、10分、4℃)を行い、不溶物を除去した。ポリアクリルアミド電気泳動の直前に、SDS(2%最終濃度)、グリセロール(2%最終濃度)、β-メルカプトエタノール(2%最終濃度)、ジチオスレイトール(2mM最終濃度)を追加した。タンパク質は、7%アクリルアミドゲルで電気泳動的に分離し、ポリビニリデンジフルオライド(PVDF)膜に転写した。膜を5%、無脂スキムミルク(1%Tween20を含むリン酸塩緩衝生理食塩水(PBST);137mM NaCl, 2.7mM KCl, 10mM Na2HPO, 1.76mM K2HPO4;pH7.4)で30から40分ブロックした。一次抗体の処理は以下のことを含む。(i)ウサギポリクローナル抗CFTR(5%ミルク/PBSTで1,000倍希釈、New England Biolabs Canada経由Cell Signaling Technology社、Whitby, Canada、cat#2269);脳動脈および血管平滑筋細胞溶解物。(ii)マウスモノクローナル抗ヒトCFTR(5%ミルク/PBSTで1:20,000倍希釈;「抗体596」);ベビーハムスター腎臓線維芽細胞溶解物。(iii)マウスモノクローナル抗α-チューブリン(5%ミルク/PBST中5,000倍希釈;クローンDM1A;New England Biolabs Canada経由Cell Signaling Technology;cat# 3873)。一次抗体は、ペルオキシダーゼで標識したロバ抗ウサギIgG(GE Healthcare Amersham (Piscataway, USA) cat# NA934)またはペルオキシダーゼで標識したヤギ抗マウスIgG(GE Healthcare Amersham cat# NA931)の二次抗体を結合させた。標準的な化学発光法(Westar ETA C; VPQ Scientific, Toronto, Canada)を用いて、X線フィルムの露光やデジタル画像の収集を行った(ChemiDoc; Bio-Rad Laboratories; Mississauga, Canada)。現像したフィルムは「Image J」ソフトウェア(NIHより無償提供)を用いてデンシトメトリーで評価し、デジタル画像は「Image Lab」ソフトウェア(Bio-Rad)で評価した。
定量PCRは、Applied Biosystems ViiATM 7 Real Time PCRシステムとPower SYBR(登録商標) Green PCR master mix(いずれもInvitrogen Life Technologies社製)を用いて行った。各プライマーセットは、特異性と相対効率が確実であるように厳密に検証された。遺伝子ターゲットは、逆転写から生成した1ngのcDNAとネガティブコントロールには水を用い、トリプリケートで評価した。PCRの増幅は、95℃で10分間の変性後、40サイクルの増幅(95℃で15秒+60℃で60秒)で行われた。増幅後、アンプリコンを融解し、得られた解離曲線から単一の産物であることを確認した。マウス組織における転写産物の発現レベルは、標準ハウスキーピング遺伝子ヒドロキシメチルビランシンターゼ(HMBS)に対するDCt値から算出した。HMBSによるノーマライゼーションの信頼性を確認するために、グルコース-6-リン酸デヒドロゲナーゼ(G6PD)に対するDCt値からも転写産物の発現量を算出したところ、同様の結果が得られた。ベビーハムスターの腎臓線維芽細胞を用いた実験では、ヒトCFTRとハムスターGAPDHに特異的なプライマーを使用し、後者はハウスキーピングノーマライゼーション遺伝子として使用された。
遅発性血管収縮と脳障害の発生は、実験的なマウスSAHモデル(SAH後2〜5日)(方法と試薬・参考文献12、19-21)では、臨床的に観察されるもの(SAH後4〜12日(方法と試薬・参考文献22、23))と比較して、より急速に進行する。傷害マーカーの評価には、SAH誘発後2日目の時点を選択した。これは、この時点で神経細胞の傷害と行動障害が顕著であることを示した過去の研究結果に基づくものである(方法と試薬・参考文献12)。SAH誘発後2日目に、動物をイソフルランで麻酔し、上行大動脈を介して脳をリン酸緩衝生理食塩水(PBS)で灌流した後、上行大動脈を介してリン酸緩衝パラホルムアルデヒド(4%、pH7.4)で灌流固定した。脳を直ちに解剖し、ブレグマの後方1mmの位置で冠状切片に切断した。これらの脳切片は、4%パラホルムアルデヒド(pH7.4)で48時間(4℃)後固定し、凍結保護のために10%スクロース(3時間)、続いて30%スクロースで一晩(4℃)インキュベートした。その後、脳切片をイソペンタンとドライアイス上でOCT(Sakura Finetek USA; Torrance, USA)に包埋した。「Tissue Path Superfrost Plus Gold」スライド(Fisher Scientific; Whitby, Canada)にクライオスタット切片(厚さ5μmの冠状切片)を集め、使用するまで-80℃で保存した。
すべてのサンプルを10分間プロテイナーゼ(20μg/ml、Promega社、Madison, USA)で前処理した後、PBS中の10%正常ロバ血清(Jackson ImmunoResearch Laboratories社、West Grove, USA)または1%ウシ血清アルブミンを含むPBS中の10%ヤギ血清(Invitrogen Life Technologies社、Burlington, Canada)を用いた標準的な60分間のブロッキング工程を行った。脳切片を、ウサギ抗ヒト活性型切断カスパーゼ-3(クローンC92-605;1000希釈;BD Biosciences Canada;Mississauga Canada)モノクローナル抗体とインキュベートし、Alexa Fluor 488ヤギ抗ウサギIgG(Invitrogen)抗体と結合させた。どちらの抗体もCan Get Signal(登録商標)immunoreaction enhancer solutionで希釈し、室温で1時間インキュベートした。検体を洗浄し、CC MountOで固定した。
脳切片を1%NaOH / 80%エタノール(5分)、70%エタノール(2分)、蒸留水(2分)、0.06%過マンガン酸カリウム(10分)で連続的にインキュベートした。脱イオン水で洗浄した後、脳切片を0.1%酢酸中の0.0004%Fluoro-jade B(Histo-Chem Inc.、Jefferson、USA)で染色した(15分)。その後、脱イオン水で洗浄し、乾燥させ、キシレンに1分間浸漬して洗浄した。スライドはDPX mounting medium(Sigma)を用いて固定した。
デジタル免疫蛍光画像は、Zeiss ZEN 2010ソフトウェアと併用して、Zeiss LSM 710共焦点レーザー走査顕微鏡を用いて取得した(血管画像)。オーバーレイは、自由に使用できるImageJ 1.44pソフトウェア(National Institutes of Health, USA)を用いて構築した。2人の独立した評価者が、既述のように盲検下でカスパーゼ-3およびFluoro-Jade(フルオロジェイド)陽性細胞を計数した(方法と試薬・参考文献12)。各動物について、ブレグマから1mm後方の単一冠状脳スライス(200倍)の、左右の側頭葉および頭頂葉を含む皮質領域を計数した。各評価者が計数した陽性細胞の数を平均し、平均陽性細胞数/動物とし、これを統計解析に用いた。
脳をヘパリン処置した生理食塩水で経心腔的灌流し,分離して,市販のRapid GolgiStainキット(FD NeuroTechnologies Inc; Columbia, USA)を用いて処理/染色した.このキットは,GlaserとVan der Loosが最初に記載したGolgi-Cox染色法を修正したものを使用している(方法と試薬・参考文献24).簡単に説明すると、分離した脳組織サンプルを、塩化水銀、重クロム酸カリウム、クロム酸カリウムを含むキット付属の溶液に、暗所で8日間浸漬し、続いて、組織保護液で6日間インキュベートし、その後、脳サンプルを洗浄し、4%低ゲル化温度アガロースに包埋し、冠状面で切片を作成(厚さ150μm、Campden Instruments 7000smz-2 vibrating microtome)し、ゼラチンコートしたスライドグラスに固定した。その後、キット付属の現像・染色試薬を用いて、メーカーの説明書に従って染色作業を完了した。神経細胞と樹状突起セグメントは、高解像度画像取得用の電動X、Y、Zフォーカスを備えたNikon Eclipse Ti2顕微鏡(Nikon Instruments Europe; Amsterdam, The Netherlands)で、立体解析学を基にしたNIS Elements ARソフトウェアを用いて画像化した。我々は、Sholl (方法と試薬・参考文献25)が考案した定量的な手法を用いて、前頭皮質から皮質錐体細胞の基底樹状突起と尖端樹状突起のネットワークの両方を解析した。Image Jソフトウェアと半自動のSimple Neurite Tracerプラグイン(いずれもNIHより無償提供)をつなげて用いて、200倍に拡大したハイパースタック画像から1つの皮質の神経細胞の樹状突起ネットワークを特定し、デジタル的に分離した。神経細胞体の中央をポインターでマークし、樹状突起の形態をSholl解析(https://imagej.net/Sholl_Analysis, version 3.7.4参照)により、5μm間隔、最大半径300μmで描写した。この解析では、樹状突起の形態を、樹状突起の交点(すなわち枝分かれ)と樹状突起の長さの観点から特徴づける。各処理グループについて、4匹のマウスから1匹のマウスにつき2から4個のニューロンを盲検下で分析した。樹状突起のスパイン密度(樹状突起に見られる小さな突起の数)を評価するために、3枝目の樹状突起セグメントを100倍に拡大して撮影した。スパイン密度は、細胞体から20〜50μm、細胞体から50〜80μm、細胞体から60〜100μmの各セグメントについて、セグメントあたりのスパインの数として測定した。この 「sliding measurement 」法により、樹状突起の枝の長さに応じてスパイン密度がどのように変化するかを評価することができる。盲検化された条件下で、各処理群において、1群3から4匹のマウスから一匹のマウスたり2から4個の錐体皮質神経細胞(1個の神経細胞につき2から4個の3番目の分枝)のスパイン密度を測定した。
上述のとおり、CFTR矯正薬C18および抗ヒトCFTR抗体(「596」と命名)は、Cystic Fibrosis Foundation Therapeutics Chemical and Antibody Distribution Programs)を通じて入手した。その他の試薬はすべて市販されているものを使用した。
トロント大学のInstitutional Animal Care and Use Committees およびUniversity Health Network(UHN)が,すべての動物飼育および実験プロトコルを承認した。市販のオスの野生型マウス(2から3ヶ月、C57BL/6N)をCharles River Laboratories社(Montreal, Canada)から購入した。トロントのHospital for Sick Childrenで確立されたコロニーから、ΔF508 CFTR変異体のホモ接合の雄マウス(CFTRtm1EUR;本研究では「ΔF508」と呼称)(方法と試薬・参考文献11)、CFTR遺伝子欠損(CFTRtm1Unc;CFTR-/-と呼称)のホモ接合の雄マウスと、相補的な野生型コントロールの同腹仔を入手した(CFTR-/-、CFTRΔF508、同腹子はすべて混合系統)。すべてのマウスは、標準的な14時間:10時間の明暗サイクルで飼育され、通常の餌を与えられ、自由に水を飲むことができた。
HFは左前下行枝冠動脈の外科的結紮により誘発された(方法と試薬・参考文献3)。簡単に説明すると、マウスをイソフルランで麻酔し,20ゲージの血管カテーテルで挿管し,室内空気で通風した。無菌状態で、胸郭と心膜を開き、左前下行枝冠動脈を7-0絹縫合糸(Deknatel; Fall River, USA)で永久結紮した。偽手術を行った対照群では、胸郭と心膜を開いたが、左前下行枝冠動脈は結紮しなかった。処置後、胸部を閉じ、マウスが自発的に呼吸した時点で抜管した。 梗塞後4〜6週目に後大脳動脈(PCA)を分離した。
よく特徴が知られている実験的SAHのモデル(方法と試薬・参考文献6)を使用した。簡単に説明すると、マウスに麻酔(イソフルラン)をかけ、頭部を定位脳手術用フレームに固定した後、前頭皮の正中線に沿って7mmの切り込みを入れ、ブレグマの4.5mm前方の頭蓋骨に0.9mmの穴を開けた。脊髄針を気管支槽まで進め、80μlの動脈血を10秒かけて頭蓋内の空間に注入した。注入した血液は、注入直前に別の野生型ドナーマウスから(心臓穿刺により)採取したもので、抗凝固剤は含まれていなかった。注射後、頭皮の切開部を閉じた。ブプレノルフィン(0.05mg/kg、0.5〜1.0mlの容量)を1日2回投与した(SAHの外科手術直後に開始)。擬似手術を受けた動物には、同じ手術を行い、血液の代わりに滅菌生理食塩水を注入した。SAH誘発後2日目に嗅覚脳動脈を分離した。
以前述べられたように(方法と試薬・参考文献3,6)、マウスの嗅覚器官(前大脳動脈の第一枝)とPCAを慎重に解剖し、マイクロピペットにカニュレーションし、in vivo長にのばし、45mmHgに加圧した、マウスの骨格筋の微小循環の小さい径の動脈を精巣拳筋から切離し、カニューレを挿入して60mmHgに加圧した。すべての機能的実験は、3モルホリノプロパンスルホン酸(MOPS)バッファー生理食塩水を用いて37℃で行い、灌流は行わなかった。フェニレフリンに対する血管運動反応(後大脳動脈は5μmol/L、精巣拳筋動脈は10μmol/L)により、各実験の開始時と終了時に血管の生存率を評価した。フェニレフリンで25%以上の収縮が見られなかった動脈は除外した。
筋原性反応は、壁貫通部の圧力を20mmHgから80mmHg(嗅覚動脈)または100mmHg(後大脳動脈および精巣拳筋の微小循環の小さい径の動脈)へと段階的に20mmHgずつ上昇させることで誘発された。それぞれの段階において、一度定常状態に到達したら(5分)、血管径(diaactive)を測定した。処理が必要な血管(CFTR(inh)-172など)は、MOPS中の試薬と30分間インキュベートし、その後、その試薬の存在下で筋原性反応を評価した。すべてのdiaactive測定の終了後、MOPSバッファーをCa2+フリーのものに交換し、各圧力ステップでの最大の受動血管径(diamax)を測定した。筋原性緊張は,それぞれの壁内圧における最大径に対する収縮率として次のように算出した: tone (% of diamax) = [(diamax - diaactive)/diamax]x100,ここで,diaactiveはCa2+を含むMOPSでの血管径,diamaxはCa2+を含まないMOPSでの血管径。フェニレフリンに対する血管運動反応の解析でも同様の計算を行ったが、この場合、diaactiveは薬剤投与後の定常状態での血管径、diamaxはCa2+フリーの状態で測定した最大径を表す。
脳灌流の評価には,既述のように,非破壊的な磁気共鳴画像(MRI)法(FAIR技術)が用いられた(方法と試薬・参考文献6)。簡単に説明すると、FAIR技術では、灌流を、加速したT1シグナルの緩和として分離する。MRIシグナル(Bruker Corporation Biospec 70/30 USR; Ettlingen, Germany)を,前脳,中脳,後脳の垂直断面(前循環,混合循環,後循環に対応)から取得した。FAIR画像は,標準化されたアルゴリズムと画像処理工程(MIPAV; National Institutes of Health, Bethesda, USA; http://mipav.cit.nih.gov/)を用いて,指定された関心領域を評価した(領域の配置は図8に示す)。
浮腫は,7 Tesla micro-MRIシステム(Bruker Corporation Biospec 70/30 USR; Ettlingen, Germany)を用いて,定量的なT2マッピング(方法と試薬・参考文献12)により評価した。T2マッピングでは,前脳から後脳までをカバーする厚さ1 mmの連続した9枚の2次元軸スライスで定量的なT2マップを作成した。T2マップは,Bruker ソフトウェアを用いて,エコー時間12msから384msのT2強調画像から,ボクセルごとに対数変換した信号強度とエコー時間を線形回帰して作成した。T2値は,MIPAVソフトウェアを用いて,前脳,中脳,後脳のT2マップ内に手動で関心領域を設定して抽出した。
神経学的機能は、先に述べられたように、修正したGarciaスコアを用いて評価した(方法と試薬・参考文献6)。神経学的評価は、自発的活動、四肢の自発的運動、前足の伸展、よじ登り、身体の受容性感覚、振動子の接触に対する反応の6つの領域からなる。SAHの2日後に2人の盲検観察者が神経学的評価を行った。最大スコアは18で、神経機能が正常であることを示している。
すべてのデータは平均値±標準誤差で表され、nは独立した測定(すなわち、サンプル、血管評価または実験対象)の数である。統計的な比較として、2つの独立したグループの比較には、正確なp値を計算するノンパラメトリックなMann Whitney検定を利用した。複数の独立したグループの比較には,ノンパラメトリックな一元配置分散分析(one-way ANOVA)(Kruskal Wallis)を用い,その後,Dunnの多重解析を行った。筋原性反応および用量反応関係を評価するために、データを二元配置分散解析(two-way ANOVA)で解析し、Tukeyの 多重解析を行った。P<0.05で有意差があると判断した。図6に示したデータはすべて盲検下で収集した。本研究で示したその他のデータは盲検を必要とせず、盲検下で収集したものではない。

これまでの研究で、CFTRが脳動脈の筋緊張を顕著に制御していることが明らかになり、脳動脈のCFTRタンパク質発現と脳灌流の間に強い相関関係があることが特定されている(非特許文献4,6)。したがって、脳血管CFTR活性の欠損(細胞表面でのCFTR発現低下によるS1P輸送の低下)を改善することが、脳の筋原性反応の低下、ひいては脳血流の低下を正常化するための有効な治療戦略であるというメカニズム概念が提唱されている(非特許文献4)。脳動脈のCFTR活性と脳灌流の因果関係を明らかにするため,CFTR変異マウスを用いた。CFTR-/-マウスは実験的なストレスに非常に敏感で、正確で再現性のある心エコーやCBF(Cerebral Blood Flow、脳血流(脳内血流))の測定ができなかった。そこで、細胞表面のCFTRの発現と活性が最小である、より安定したCFTRΔF508マウス(非特許文献11)を使用した。これは、機能喪失表現型の脳動脈筋緊張を血行動態パラメーターに関連付けるためにCFTRΔF508マウスを使用するという本発明者の戦略的決定を裏付けである。CFTRノックアウトマウスからの後大脳動脈(PCA)反応に関する以前に公開された特徴とCFTRΔF508の表現型を直接比較するために、後大脳動脈(PCA)の血管運動反応を評価した(非特許文献4)。予想通り、CFTRΔF508マウスのPCAは、パラメーターと比較して筋緊張の増加を示し(図1A)、その増加の大きさはCFTR-/-マウスで見られたものと定量的に類似している(非特許文献4)。フェニレフリンの反応は、筋緊張の亢進により上方にシフトしている(図1B)。しかし、EC50値には差がなく(log EC50WT:-5.83±0.21、n=5、3匹のマウスから;log EC50CFTRΔF508:-6.00±0.04、n=6 、4匹のマウスから;Mann-Whitney test P=N.S.)、基底の緊張での差(すなわち、45mmHgでの筋緊張;図1C)を補正することで曲線の差は解消される。したがって、ΔF508変異は筋原性反応を増加するが、一般的な収縮性は変化させない。CFTRΔF508マウスでは、CBFが有意に低下しているが(図1D)、心拍出量(Cardiac Output (CO)、図1E)も全末梢(血管)抵抗(Total Peripheral Resistance (TPR)、図1F)も変異の影響を受けていない。これらの観察結果から、脳灌流障害は明らかに血管に起因するものであると考えられる。
ハイスループットの薬物スクリーニングにより同定されたCFTRを調節する薬剤が、嚢胞性線維症の症状を緩和に実験的にも臨床的にも用いられている。C18は、野生型CFTRと直接相互作用して安定化させ、それによって細胞膜でのCFTRの発現を増加させることが以前に実証されているので(非特許文献15)、本実施例では、C18を試験に用いた。
Meissnerら(非特許文献4)がすでに報告しているように、HFを引き起こすと、脳動脈のCFTRタンパク質の発現が著しく低下し(図3A)、これはPCA筋緊張の著しい亢進と一致する(図3B)。図2のデータから予測されるように、C18をin vivoで投与(1日3mg/kg、2日間)すると、HFマウスから分離した脳動脈でCFTRの発現が回復し(図3A)、付随してPCAの筋原性が正常化する(図3B)。基底の緊張の減少により、筋緊張の減衰はフェニレフリン反応性の予想された変化に関連するが、EC50値に差はない(log EC50 Sham:-6.30±0. 14, n=6 、4 匹のマウスから; log EC50HF:-6.25±0.07, n=5 、3 匹のマウスから; log EC50 HF+C18:-5.94±0.21, n=5 、4 匹のマウスから; Kruskal Wallis test P=N.S.)、基底の緊張の差を補正すると、曲線は重なり合う(図11)。C18の投与は、偽手術を受けたマウスのPCAの筋緊張やフェニレフリン反応に影響を与えない(図12)。本発明は、CFTRノックアウトマウスから分離したPCAを用いて、C18がCFTRを標的にして筋緊張の減衰効果を媒介することを確認した。予想通り、CFTRノックアウトマウスのPCAは、in vivoでのC18投与に影響されない筋緊張の亢進を示し(図3C)、フェニレフリン反応はC18投与に影響されない(図13)。
これまでの研究では、SAHがHFと同様の脳血管表現型(i)脳動脈CFTRタンパク質の発現が低下する、(ii)脳動脈の筋緊張が亢進する、(iii)脳灌流が低下する、を持つことが示されている(非特許文献6)。HFの場合(図3)と同様に、in vivoでC18を投与(1日3mg/kg投与、2日間)すると、SAHを発症したマウスの脳動脈におけるCFTRの発現が回復し(図5A)、付随して嗅覚脳動脈の筋緊張が正常化する(図5B)。基底緊張が低下するため、筋緊張の低下は予想されたフェニレフリン反応性の基底の変化に関係するが、EC50値に差はなく(log EC50 Sham:-5.52±0. 23, n=5、3匹のマウスから; log EC50SAH: -6.01±0.16, n=6、6匹のマウスから ; log EC50 SAH+C18:-6.01±0.31, n=5、4 匹のマウスから;Kruskal-Wallis test P=N.S.)、基底緊張の差を補正すると、曲線は重なり合う(図14)。C18の投与は、偽手術を受けたマウスの嗅覚大脳の筋緊張やフェニレフリン反応に影響を与えない(図15)。CFTRノックアウトマウスから分離した嗅覚脳動脈を用いて、本発明は、C18のCFTRを標的とする筋緊張の低下効果を示すことを確認した。予想通り、CFTRノックアウトマウスの嗅覚脳動脈は、in vivoでのC18投与に影響されない筋緊張の亢進を示し(図5C)、これらの動脈のフェニレフリン反応性はC18投与によって影響されない(図16)。重要なことは、in vivoでC18を投与すると、CBFが回復し(図5D)、嗅脳動脈の筋緊張の正常化(図5B)と再び相関したことである。
C18のデータは、嚢胞性線維症の治療でFDAに承認されている臨床的に適切なC18アナログであるCFTR矯正薬 ルマカフトールとCFTRポテンシエーター(増強薬) イバカフトール(すなわち、Orkambi(登録商標))を併用した介入によって補完される。ルマカフトールは、マウスとヒトの両方のCFTRの発現を増加させる能力があることが最初に確認され、C18で観察されたように(図2)、ルマカフトールは、マウスの脳動脈(マウスに1日3mg/kgを2日間注射)およびヒトのCFTRを安定的に発現しているベビーハムスターの腎臓線維芽細胞でCFTRタンパク質の発現を増加する(図6)。いずれの場合も、CFTR mRNAの発現には影響がなく、非転写性のメカニズムであることがわかった(図6)。SAHにおける今回のC18データ(図5)と同様に、ルマカフトールをin vivoで投与(1日3mg/kg、2日間)すると、SAHマウスの脳動脈におけるCFTR発現が回復し(図6)、同時に嗅覚脳動脈の筋緊張(図6)と脳灌流(図6)も正常化した。ルマカフトールはフェニレフリン反応性に影響を及ぼさず(図17)、その結果、EC50値に違いはなかった(log EC50Sham:-5.79±0.09、n=7、4匹のマウスから、log EC50SAH:-5.93±0.19、n=6、3匹のマウスから、log EC50SAH+Lum:-5.90±0.18、n=7、4匹のマウスから、Kruskal-Wallis test P=N.S.)。
SAHによる神経細胞の損傷は、標準的な組織学的手法(フルオロジェイド染色、活性化カスパーゼ-3染色)と簡単な神経学的テスト(修正 Garcia スコア)で容易に特徴づけることができる(非特許文献6)。これらの方法を用いて、正常な筋原性反応と脳血流(CBF)の回復が、神経学的結果の改善と相関するかどうかを調べた。確かに、C18とルマカフトールは、活性化カスパーゼ3とフルオロジェイド染色で評価したように、SAHにおける神経損傷を劇的に減少させた(図7)。C18投与マウスの神経損傷の減少は、神経機能の改善と相関している(図7E)。具体的には、SAHマウスは偽手術をしたマウスよりも修正Garcia神経機能テストのスコアが低かったが、C18を投与したSAHマウスは偽手術をしたマウスと同等の神経機能スコアを示した。
TG2阻害剤とCK2阻害剤、好ましくはシステアミンとEGCGの組み合わせに関しては、米国特許出願公開第2013/0310329号を参照とする。
Claims (17)
- 脳血管状態の予防および/または治療におけるCFTRモジュレーターの使用。
- 前記CFTRモジュレーターがCFTR矯正薬(コレクター)およびCFTR増幅薬を含む群から選ばれることを特徴とする、請求項1に記載のCFTRモジュレーターの使用。
- 前記CFTRモジュレーターが、C18(VRT−534)、ルマカフトール(VX−809)、テザカフトール(VX−661)、4,4’,6−トリメチルアンゲリシン、VRT−768、VRT−422、VRT−325、CFpot−532、Copo−22、002_NB_28(DBM228)、DBM_003_8Cl(DBM308)、システアミン、ECGCおよびそれらの二つ以上の混合物からなる群から選ばれることを特徴とする、請求項1または2に記載のCFTRモジュレーターの使用。
- 前記CFTRモジュレーターが、C18、ルマカフトールおよびそれらの混合物からなる群から選ばれることを特徴とする、請求項3に記載のCFTRモジュレーターの使用。
- 前記CFTRモジュレーターが、C18であることを特徴とする、請求項4に記載のCFTRモジュレーターの使用。
- 前記CFTRモジュレーターが、ルマカフトールであることを特徴とする、請求項4に記載のCFTRモジュレーターの使用。
- 前記脳血管状態が、脳灌流低下または脳灌流不全の状態であることを特徴とする、請求項1から6の何れか1項に記載のCFTRモジュレーターの使用。
- 前記脳灌流低下が、心臓病、心不全、くも膜下出血、突発性感音難聴、血管性認知症、動脈性高血圧、虚血性脳卒中、出血性脳卒中、糖尿病、およびアルツハイマー病からなる群から選ばれる状態に関連することを特徴とする、請求項8に記載のCFTRモジュレーターの使用。
- 脳血管状態の予防および/または治療のための方法であって、少なくとも一つのCFTRモジュレーターをそれを必要とする対象に有効量投与することを含む、脳血管状態の予防および/または治療のための方法。
- 前記脳血管状態が、脳灌流低下であることを特徴とする、請求項9に記載の方法。
- 前記脳灌流低下が、心臓病、心不全、くも膜下出血、突発性感音難聴、血管性認知症、動脈性高血圧、虚血性脳卒中、出血性脳卒中、糖尿病、およびアルツハイマー病からなる群から選ばれる状態に関連することを特徴とする、請求項10に記載の方法。
- 前記対象が、哺乳類、好ましくはヒトであることを特徴とする請求項9から11のいずれか1項に記載の方法。
- 前記少なくとも一つのCFTRモジュレーターが、CFTR矯正薬およびCFTR増幅薬を含む群から選ばれる、請求項9から12のいずれか1項に記載の方法。
- 前記CFTRモジュレーターが、C18(VRT−534)、ルマカフトール(VX−809)、テザカフトール(VX−661)、4,4’,6−トリメチルアンゲリシン、VRT−768、VRT−422、VRT−325、CFpot−532、Copo−22、002_NB_28(DBM228)、DBM_003_8Cl(DBM308)、システアミン、ECGCおよびそれらの二つ以上の混合物からなる群から選ばれることを特徴とする、請求項9から13のいずれか1項に記載の方法。
