Abstract
Description
日本で最も罹患率の高いアレルギー疾患は花粉症(季節性のアレルギー性鼻結膜炎)である。スギ花粉症の罹患率は、1980年から2000年にかけて2.6倍増加し、20%近い数値となった。また、2006年から2007年にかけての福井県における調査では、スギ花粉症の罹患率が36.7%であった(1)(非特許文献1)。2000年に発表された日本のスギ花粉症の年間医療費は、休業などの間接費601億円を含み合計2860億円と推計されている(2) (非特許文献2)。しかし、スギ花粉症の場合、病気のために損失した労働/勉強時間(absenteeism)に加え、疾患により生産性が低下した状態(presenteeism)の影響が大きいと考えられる。例えば、スギ花粉症の市販薬に多く使用されている抗ヒスタミン薬による労働生産性障害は月間1450億円と見積もられている(3) (非特許文献3)(花粉の飛散時期は、北海道を除き、4-6か月)。
2011年の厚生労働省の調査において、医療機関を受診中の喘息患者は、104.5万人と推計されている(4) (非特許文献4)。世界共通の簡便な問診票 (International study of asthma and allergies in childhood: ISAAC) を用いた場合の日本全国の小児期(6-7歳)の喘息罹患率は、2008年の時点で19.9%となった(5) (非特許文献5)。なお、ISAAC問診票を用いた小児喘息罹患率は、英国、オーストラリアなどで20%を超えている。日本の喘息治療に係る医療の経済負担は1999年の厚生労働省調査によると、4517億円(医療機関を受診する喘息患者数119万6000人;患者1万人あたり37.8億円)と報告されている(6) (非特許文献6)。2007年の米国(人口3億人)の家庭へのアンケート調査では、喘息患者数が1300万人で、喘息治療に係る医療の経済負担は、休業、欠席、死亡等による損失額59億ドルを含み、年間560億ドル(約5兆6000億円)(医療の経済負担は501億ドルとなり、患者1万人あたり38.5億円で、日米でほぼ同額)と報告されている(7) (非特許文献7)。
アトピー性皮膚炎の罹患率は乳幼児期がピークで、日本の医師診断による調査において9.8 ~13.2%である。これらの数値は先進工業諸国と同様に高いレベルである(8) (非特許文献8)。
アトピー性皮膚炎の治療や労働生産性の障害による損失は、米国の調査で53億ドル(5300億円)(9) (非特許文献9)、ドイツ(人口8000万人)の調査では、年間15-35億ユーロ(約2000-4600億円)(10) (非特許文献10)と報告されている。アトピー性皮膚炎、接触皮膚炎、蕁麻疹など痒みを伴うアレルギー性皮膚疾患は、花粉症と同様に、疾患により生産性が低下した状態(presenteeism)の影響が大きいことが示されている(11) (非特許文献11)。
食物アレルギーの罹患率に関しては調査方法で解離が大きいが、医師診断では先進工業諸国で2%台という報告が多く、21世紀になっても増加している(12) (非特許文献12)。
アレルギー疾患の症状を緩和するために、様々な抗アレルギー薬(ケミカルメディエーター遊離抑制薬、ヒスタミンH1受容体拮抗薬、トロンボキサンA2受容体拮抗薬、ロイコトリエン受容体拮抗薬、Th2サイトカイン阻害薬など)が使用されているが、対症療法に過ぎず、根本的な治療に至っていない。
アレルギー疾患の大半は乳幼児期に発症する。さらに、成人期になると一度発症したアレルギー疾患が寛解することはほとんど期待できない、すなわち、一度獲得したアレルゲンに特異的なIgE抗体が陰性化することはほとんど期待できない。そのため、乳児期のアレルギー疾患の発症を予防することが重要である。特に、生後数ヶ月までの乳児期における湿疹・アトピー性皮膚炎の発症、および、それに引き続く様々なアレルゲンへの感作を予防することは、アレルギー・マーチ(アレルギー体質を有する個体は、乳児期に食物アレルギーやアトピー性皮膚炎が発症し、幼児期にダニアレルギー、喘息を発症し、学童期に花粉症、鼻炎を発症するという小児アレルギーの自然歴)の進展を抑制するためには、最も重要であると考えられている(14, 15)(非特許文献14、15)。そして、生後3-4ヶ月以前、特に生後1-2ヶ月に発症した湿疹・アトピー性皮膚炎は、3歳児の食物アレルギーの診断に強く相関(オッズ比6.6倍)するという報告(16)(非特許文献16)は、この考えを支持する。
本発明者らは、以下に示すように、胎児期(本明細書においては、「受胎後第2期~第3期」と定義する。)あるいは新生児期(本明細書においては、「出生後~1ヶ月」と定義する。)から乳児期(本明細書においては、「出生後1ヶ月~12ヶ月」と定義する。)までの間のいずれかの時期に抗IgE抗体を投与することにより、乳児期における免疫応答をIgEクラスに特異的に抑制する方法を発見した。このことにより、アレルゲンに特異的なIgE抗体の産生に引き続いておこる種々のアレルギー疾患(食物アレルギー(鶏卵アレルギー、牛乳アレルギー、ピーナッツ・アレルギー、小麦アレルギー等)、花粉症、アトピー性皮膚炎、喘息などが例として挙げられる)の発症が効率的に予防され、将来的に医療費が大幅に削減されることが期待できる。
上述の実施例をもって、発明を完成させた。
I.アレルギー発症に関する知見
食物アレルギーの新たな概念:二重抗原暴露仮説
経皮感作と食物アレルギー
食物アレルギー (food allergy) は、従来、腸管感作により発症すると考えられてきたが、近年、経皮感作の重要性が明らかになり、食物アレルギーの発症機序に関する概念が一新されつつある。
日本では、茶のしずく石鹸という小麦成分(加水分解小麦)を含有する石鹸の使用者に小麦アレルギーが発症し、社会問題になっていた(23)。この事例は、経皮暴露が食物アレルギーを誘発することを臨床的に初めて証明した事例である。

身体の外表を覆う皮膚は、外界と生体の境界をなすバリアとしての機能を持つと同時に、バリアの障害や侵入外来物質に対して監視機構の役割も果たしている(27)。皮膚の構造は、外側から表皮、真皮、皮下組織の3層に分かれ、最外層の表皮は、さらに、表層から角層、顆粒層、有棘層、基底層の4層に分けられる。表皮はその95%をケラチノサイト(表皮角化細胞)が占める。皮膚バリアは、主に角層と顆粒層のタイトジャンクション(tight junction)により構成される。
近年、アトピー性皮膚炎(様)の臨床像を呈する一部の患者に、皮膚バリア構成成分の遺伝子異常が同定されるようになった。SPINK5遺伝子にコードされたセリンプロテアーゼインヒビターLEKTIの不全によるNetherton症候群(28)や、KLK5の基質であるコルネオデスモシンが完全に欠損するpeeling skin症候群B型(29)は、生後早期にアトピー性皮膚炎様の皮疹が現れ、IgE上昇、食物アレルゲンに対する特異的IgE抗体上昇などの特徴がみられる。また、タイトジャンクションの接着分子であるclaudin-1遺伝子の一塩基多型とアトピー性皮膚炎の関係も報告された(30)が、最もアトピー性皮膚炎と強い相関が見いだされているのがフィラグリン(FLG)遺伝子変異である。
皮膚バリア障害を介して食物抗原の経皮感作が起こる機序については、以下のように考えられている。健常な皮膚では、通常500 ダルトン以上の大きな分子を通すことができないが、角層のバリアが障害されると、数十キロダルトンもある食物抗原が皮内に取り込まれる。それは、単に壊れたバリアを抗原が通り抜けるわけではない。表皮の様々なシグナルを受けて活性化したケラチノサイトからIL-1(interleukin 1)やTNF-α(tumor necrosis factor α)、TSLP(thymic stromal lymphopoietin)などのサイトカインが産生され、その作用により活性化されたランゲルハンス細胞が抗原を能動的に取得するのである(27, 35)。
経皮(経湿疹)感作予防
アレルギー疾患の中でもアトピー性皮膚炎は生後数か月以内に発症する。最近の研究により、乳児期のアトピー性皮膚炎の存在が食物アレルギーの発症リスクになることが分かった(16)。また、表皮に存在するが気道上皮には存在しないバリア機能タンパク質フィラグリンの遺伝子欠損はアトピー性皮膚炎、食物アレルギーのリスク要因であるのみならず花粉症や喘息についてもリスク要因である。つまり、アレルギー・マーチの主な原因は、乳児期のアトピー性皮膚炎の存在であるということができる(39)。
寝具等の家塵中のダニ抗原量は喘息発作頻度と相関する。ピーナッツ消費量の多い米国ではピーナッツ・アレルギーの頻度が高い。近年、日本でスギ花粉症が増加した大きな原因は、戦後植林したスギの樹木が花粉を多く飛散する樹齢になったことである(42)。このようにアレルギー疾患増加の原因として、アレルゲンの増加が寄与することは明らかである。
同じアレルゲンであっても、侵入経路によってはIgE抗体の産生が抑制されることがある。アトピー性皮膚炎の炎症を起こしている皮膚や喘息患者の気道粘膜からアレルゲンが侵入すると、感作されたマスト細胞の活性化を誘導するとともに、上皮間葉系組織から遊離されるサイトカインの影響により、IgE抗体の産生も増強される。しかし、健康な舌下や腸管などは制御性T細胞が生成しやすい環境であるため、アレルゲンが侵入すると制御性T細胞が作られ、乳幼児期である等のためにアレルゲンに対してナイーブであった場合は、免疫寛容が成立し、また、既にアレルギー疾患を発症している場合でもアレルギー反応を起こさない無害なIgG抗体が産生されるため、寛容状態になると考えられている。
多くの国で最も多いアレルギー原因食物である鶏卵に関しては、国立成育医療センターにおける臨床研究介入試験において、鶏卵を4カ月から摂取した児は、プラセボを摂取した児と比較し、1歳時点の鶏卵アレルギーの発症が80%減少したと報告された。この試験では、従来の方法と異なり、固ゆで卵を少量摂取から徐々に増量する介入方法とともに、一律に湿疹に対する早期治療を行ったことが特記されている(44)。
多くの疫学的調査結果から、環境中のエンドトキシン量が多い農村地域や非衛生的な地域で育った場合、その後の花粉症の発生率やアレルゲン特異的IgE抗体保有率が低くなることが報告されている(45)。
以上のエビデンスを踏まえ、乳児湿疹への早期介入とアレルギー原因物質の離乳早期からの経口摂取が食物アレルギーなどのアレルギー疾患の発症抑制に有効であることが示唆されつつある。
しかし、保湿剤による食物(鶏卵)抗原感作の抑制は認められなかったこと、および加熱鶏卵やピーナッツを離乳早期から摂取させることでこれらのアレルギーの発症を抑制できたものの、そのほかの抗原については、抑制効果は証明されていない。よって、将来的な医療費削減に十分な効果が期待できる喘息等を含めたアレルギー疾患発症予防方法を確立するためには、さらに安全で効率的な手段を確立することが必要である。
そして、上記の知見を総合して、発明者らは、「生後3-4ヶ月までのIgE抗体の産生を完全に制御することでアレルギー素因の獲得を阻止する」という戦略を立案した。本明細書における「アレルギー素因」とは、「アレルゲンに特異的なIgE抗体が陽性になること」と定義する。そして、その戦略の実現手段として、「妊婦に治療用抗IgE抗体を投与すれば、治療用抗IgE抗体が胎盤を通して胎児に移行し、胎児の体内で(ε鎖mRNAの発現抑制などを介して)IgE抗体の産生機構を抑制することにより、その妊婦から生まれてくる子の新生児期から乳児早期においてIgEクラスに特異的な免疫反応を阻害することができる(「IgE(免疫グロブリン)クラスに特異的なトレランス」あるいは「IgEクラスに特異的な免疫寛容」を成立させることができる)」という仮説を立てた。ここで、「免疫寛容」とは、一般的には、「特定抗原に対する特異的免疫反応の欠如あるいは抑制状態のこと」を指すが、本明細書における「IgEクラスに特異的な免疫寛容」とは、「全ての抗原に対してIgE(免疫グロブリン)クラスに特異的に抗体産生が欠如または抑制される状態のこと」と定義する。
ヒト化抗ヒトIgE抗体
1966年石坂らによりIgEが発見されて以来、それまで混とんとしていたアレルギーに関する研究は、免疫学の進歩とともに飛躍的に進展した(47)。
IgEをそのFCEL(低親和性IgEレセプター、FcεRII)およびFCEH(高親和性IgEレセプター、FcεRI)に結合させる上で重要な役割を果たすIgEドメインおよび特定の残基を同定し、その情報に基づき、これら2つのレセプターのうち一方のみに実質的に結合することができるが、これらレセプターのうち他方には実質的に結合することができないポリペプチドを設計した。これらポリペプチドは、識別結合ポリペプチドと称される。この発明の識別結合ポリペプチドは、IgEレセプターのための診断手順またはアレルギーなどのIgEに媒介された疾患の治療に有用である。該ポリペプチドはまた、レセプター結合に参画するIgEの領域に結合することのできる抗体を調製するのに有用である。この発明は、とりわけ、FCEL-結合IgEには結合することができるが、FCEH-結合IgEには実質的に結合することができない抗体に関する。この発明の態様において、アレルギーおよび他のIgEに媒介される疾患の診断または治療または予防に有用な変異体抗IgE抗体が提供される。
Omalizumabは、IgEのCε3と結合することにより、IgEがマスト細胞や好塩基球の表面にあるFcεRIに結合することをブロックする。その結果、抗原暴露が起こっても、IgEを介したマスト細胞や好塩基球での一連の反応が阻止され、アレルギー反応による喘息などの症状の発現を抑制すると考えられている(50)。
Omalizumabは抗体依存性細胞傷害活性が低いので、すでにアレルゲンに特異的なIgE抗体やメモリーB細胞が存在する成人に投与しても、血清IgE抗体価が低下することはないが、in vitro試験において、B細胞上の膜結合型IgEと反応し、ε-chainのmRNAの発現の抑制を介し、B細胞によるIgE抗体産生量を低下させることが報告されている (52, 53)。
Omalizumabは、完全ヒト抗体ではないため、残存したマウスのアミノ酸配列や抗原認識部位に対する抗体が生成することにより、ショック、アナフィラキシーが発現する可能性があるが、その発現頻度は、以下に示すように0.2%以下で、極めて低い。Omalizumab投与によるショック、アナフィラキシーは、日本国内の臨床試験では認められていないが、気管支喘息患者を対象とした海外成人臨床試験において、アナフィラキシー/アナフィラキシー様反応が、本剤投与軍群0.13%(7例/5367例)、プラセボを含む対照群0.03%(1例/3087例)に認められており、発生頻度は低いものの本剤投与群の方が高い傾向であった。また、気管支喘息患者を対象とした海外小児臨床試験では、本剤群0.2%(1例/624例)、プラセボ群0.3%(1例/302例)に認められている(55)。海外市販後の自発報告(56)においては、アナフィラキシーと報告された事象およびアナフィラキシーとは報告されていないがアナフィラキシーの可能性のある過敏症反応が、合計124名で認められており、本剤の推定処方患者数(約57300名)を基に算出した頻度は0.2%であった。ここで強調されるべきことは、Omalizumab投与に関連するアナフィラキシーの報告の頻度は、他の適用に使用されている他の生物製剤投与に関連するアナフィラキシーの報告の頻度に比べて、低いことである(57-61)。
Omalizumabは、遊離IgEと複合体を形成して、遊離IgEを減少させる。IgEは寄生虫感染に対する宿主防御機能に関与する因子の1つと考えられていることから、理論的には、寄生虫感染に対する感受性が高まる、と考えられる。このようなリスクについて検討するために、腸の寄生虫感染のリスクが高い喘息あるいは鼻炎患者137名を対象として、omalizumabの52週、無作為化二重盲検、プラセボ対照の臨床試験が実施された(62)。この1年間の臨床試験の結果、Omalizumab群はコントロール群と比べて、寄生虫感染のリスクの有意な増加は認められず、また、抗寄生虫療法に対する反応性の差は認められなかった。
非臨床試験において、ヒトに対する最大投与量の3.7 - 20倍のOmalizumabをカニクイザルに投与したところ、血小板数の減少が観察された。そこで、臨床試験や市販後調査結果を解析したところ、Omalizumabの血小板に対する作用は観察されなかった。
35のOmalizumabの臨床試験のすべてのindicationについて、悪性腫瘍に関する解析が行われた。Omalizumabを投与された患者5015名のうち25名(0.50%)において、コントロール群では、2845名のうち5名(0.18%)において、悪性腫瘍が報告された。Omalizumab投与群の1名を除いて、すべての悪性腫瘍は固形腫瘍あった。NIH SEERデータベースで比較を行った場合、Omalizumab投与群の悪性腫瘍の出現率は、一般的な集団における悪性腫瘍の罹患率と同等であったことから、Omalizumabは、悪性腫瘍の発生率を有意に増加させないことが判明した(63)。さらに、Omalizumabの市販後前向き観察コホート試験(EXCELS)においても、Omalizumab療法は、腫瘍発症リスクを増加させないという結果が得られた。
ヘモグロビン値、白血球数、血小板数、腎機能および肝機能に関する臨床検査値から、Omalizumab投与は、これらに対して影響を与えないことが明らかになった。
いくつかの臨床試験は、中等から重度、あるいは重度の持続性のアレルギー性喘息患者に対して、Omalizumabの上乗せ療法は、臨床的に有用であることを示した。それらの臨床試験における安全性データおよびOmalizumab特異的な懸念事項に関する臨床試験の結果を考え合わせると、Omalizumabの上乗せ療法は、中等から重度、あるいは重度のアレルギー性喘息患者に対して、有効で、しかも、忍容性が高いことが判明した(57)。
妊娠判明時にOmalizumabの投与を中止した場合
妊娠時に配偶者がOmalizumab治療を受けていた場合も含めて、47名で、臨床試験中に妊娠が報告された。そのうち、27名がOmalizumab治療群で、18名がコントロール群、他の2名は配偶者がOmalizumab治療を受けていた。Omalizumab群(27名)では、妊娠判明時にOmalizumabの投与を中止した結果、17名が正常出産、4名が人工妊娠中絶、6名が自然流産であった。Omalizumab非投与群(18名)では、8名が正常出産、1名が人工妊娠中絶、6名が自然流産、3名が不明であった。また、妊娠時に配偶者がOmalizumab治療を受けていた2名は、正常出産であった。これらの結果は、妊娠判明時にOmalizumabの投与を中止した場合には、有害な結果にならないことを示す(57)。
The omalizumab pregnancy registry, EXPECTは、Omalizumab投与後の母親、妊娠経過、および先天性異常を含む幼児の転帰を調査するために、FDAが計画した市販後前向き観察試験である。EXPECTは、妊娠の8週間以内前、あるいは、妊娠期間中にOmalizumabを1回以上投与された妊婦を対象とした前向観察試験であり、登録時、妊娠第一期、二期、三期、出産時、および出産後6か月ごとに18か月までのデータが収集された。転帰が判明している169の妊娠例(妊娠中のOmalizumabの平均投与期間は8.8か月)の転帰は、156の生産で160名の幼児(4組の双子含む)が誕生し、胎児死亡が1例、11例の自然流産、1例の人工中絶であった。152名の単生児のうち、22名(14.5%)は早産であった。出生時体重が分かっている147名の単生児のうち、16名(10.9%)は胎内発育遅延児であった。125名の満月産の幼児の中では、4名(3.2%)が低体重出生であった。発育異常が認められた幼児の中の7名(4.4%)は重大な障害を有していたが、異常に一定のパターンは認められなかった。以上の結果より、EXPECTで観察された重大な発育異常の有病率は、喘息の一般的な集団において報告されている有病率より、高くはないこと、および、Omalizumabは、喘息の一般的な集団のデータに比べて、早産児および胎内発育遅延児のリスクを増加させないことが判明した(64)。
母親から胎児へのIgGの移行には、Neonatal Fc receptor for IgG (FcRn)が重要な役割を果たすこと、および、ヒトではIgG1およびIgG4が最も移行しやすいことが知られている(65)。Omalizumabは、IgG1κ抗体である(49)ことから、Omalizumabを妊婦に投与すると、Omalizumabは胎児の体内へ移行すると考えられる。事実、カニクイザルを用いた動物実験では、Omalizumabの胎盤通過が認められたが、母体毒性や胎児毒性、催奇形性は認められなかった(55)。Correnらは、2009年の総説の中で、投与中に妊娠し、妊娠判明後に投与を中止した27例について、Omalizumabは悪影響を与えなかったと報告した(57)。また、EXPECT試験において、妊娠中にOmalizumabを投与された妊婦においては、喘息の一般的な集団のデータに比べて、重大な先天性障害の有病率、早産児および胎内発育遅延児のリスクを増加させないことが示された(64)。さらに、慢性特発性蕁麻疹を罹患している妊婦に対して、妊娠期間中Omalizumab治療を行った場合にも、正常出産で、身体的、精神心的な異常がなく、生育したことが報告されている(66)。
日本では、今から50年前にはアレルギー疾患患者はほとんどいなかったが、現在では、国民の3人に1人が何らかのアレルギーを持っているといわれている。アレルギーは、工業化・文明化など環境の変化と密接な関係があると考えられており、日本のほか、欧米などの先進国で大きな問題となっている。アレルギー疾患の大半は乳幼児期に発症する。また、その元となるアレルギー素因の獲得、即ち、種々のアレルゲンに対する特異的なIgE抗体の産生は新生児期から乳児期早期に始まる。アレルギー疾患発症を予防するために現代文明を捨てることは理論的には考えうるが、乳児死亡率が近代化以前のように悪化する可能性が高く現実的な選択ではない。
(1)子を妊娠している母及び/又は誕生後から乳児期までの間のいずれかの時期にある子に投与することを特徴とする、乳児期のアレルゲンに特異的なIgE抗体の産生を抑制するための、抗IgE抗体を含有する医薬。

〔実施例1〕
新生仔マウスにおけるOVA感作モデルの確立
妊娠マウスに抗マウスIgE抗体を投与し、新生仔マウスのアレルギー反応に対する影響を調べる評価系を確立するための基本的なスキームを図1に示す(68)。C57BL/6Jマウス(日本SLCより購入)を用い、抗原としては、卵白アルブミン(ovalbumin, OVA)を選択した。基本的には、出生2日および9日後に、抗原(OVA)とアジュバントを同時に投与することで2度の感作を行い、その7日後あるいは14日後に採血して血清中のOVA特異的IgE抗体の抗体価を評価する系である。まずは、この評価系を確立するために、アジュバントの種類、感作経路、抗原量、および解析のための採血時期に関して検討を行った。
以上の結果より、妊娠マウスに抗マウスIgE抗体を投与することによる仔マウスのOVAに特異的なIgE抗体の産生の抑制効果を評価するためのスキームを図6のように構築し、C57BL/6Jマウスを用いて実験を行った。すなわち、ヒトの場合、妊娠第二期から胎児の循環血中に母親のIgGが増加してくることを考え合わせて、妊娠12.5日と18.5日にマウス1匹あたり100 μgの抗マウスIgE抗体もしくはIsotype-matched control抗体を妊娠マウスに投与した。投与する抗マウスIgE抗体は、Omalizumabと同一のisotype (IgG1κ)であり、マウス喘息モデルにおける有効性が、Omalizumabのアレルギー性喘息患者における有効性と平行していることが報告されている(69)、精製Rat anti-mouse IgE抗体(R35-92: BD Bioscience)を選択し、isotype-matched control抗体には、精製Rat IgG1κ 抗体(R3-34: BD Bioscience)を選択した。仔マウスに対するOVAの抗原感作は、OVA(Sigma-Aldrich)とアラム(Alum: Imuject, Thermo Fisher Scientific)を1:1の割合で乳化させたOVA乳化液をOVA 5 μg/体重(g)となるように出生2日後および9日後に腹腔内投与することで実施し、採血は出生後23日目、OVA投与による体温低下試験は出生後28日に実施した。
次いで、仔マウスにおけるOVAに特異的なIgE抗体の産生能の完全な抑制状態の持続期間を明らかにするために、妊娠12.5日と18.5日にマウス1匹あたり100 μgの抗マウスIgE抗体もしくはIsotype-matched control抗体を妊娠マウスに投与し、仔マウスを4群に分けて異なる4つの時期(生後2日、16日、30日、44日後)に感作を開始する実験を行った(図13)。仔マウスにOVA 5 μg/体重(g)とアラムを腹腔内投与することによる感作は、Protocol Aでは生後0週(出生2日後および9日後)、Protocol Bでは生後2週(出生16日後および23日後)、Protocol Cでは生後4週(出生30日後および37日後)、Protocol Dでは生後6週(出生44日後および51日後)後から開始した(図13)。最終感作2週間後に、採血し、OVAに特異的なIgE抗体の抗体価およびOVAに特異的なIgG1抗体の抗体価を測定した(図14A、B)。その結果、図14Aに示すように、生後0, 2, 4および6週後のいずれの時期から感作した場合にも、isotype-matched control抗体を投与したマウスから生まれた仔マウスをOVAで感作した場合には、OVAに特異的なIgE抗体の抗体価の明らかな上昇が観察されたが、抗マウスIgE抗体を投与したマウスから生まれた仔マウスでは、OVAで感作してもOVAに特異的なIgE抗体の産生は完全に抑制されていた。一方、図14Bに示すように、いずれのProtocolの場合にも、抗マウスIgE抗体を投与したマウスから生まれた仔マウスおよびisotype-matched control抗体を投与したマウスから生まれた仔マウスをOVAで感作したところ、両仔マウスは、同程度のOVAに特異的なIgG1抗体を産生していた。
さらに、図17に示した直腸温を測定したC57BL/6の仔マウスについて、直腸温測定3日後に脾臓細胞を調製し、OVA (200 μg/mL)添加あるいは非添加の条件で、96時間培養後の培養上清中のIL-4、IL-13、IL-17およびinterferon-γ(IFN-γ)を、ELISAにより定量した。簡単に記述すると、直腸温測定3日後に脾臓を取り出し、溶血緩衝液(BioLegend)を用いた溶血処理後、70-μm細胞ろ過機(Corning)を用いて脾臓細胞懸濁液を調製した。脾臓細胞懸濁液は、10% FCS(Biological Industries)、 100 U/mL penicillin、100 mg/mL streptomicin(Thermo Fisher Scientific)含有RPMI1640(Nacalai Tesque)を培地として、48穴プレートを用いて、2 x 106 cells/mLの密度で、200 μg/mLのOVAの存在下、あるいは非存在下で培養した。96時間培養後、培養上清を回収し、培養上清中のサイトカインのレベルをELISAキット(Thermo Fisher Scientific)を用いて測定した。
次いで、抗IgE抗体を新生児に投与した場合の、OVAに特異的なIgE抗体の産生抑制効果およびその持続期間について、検討した。
C57BL/6 の新生児マウスに抗マウスIgE抗体(10 μg/mouse)あるいはisotype-matched control抗体を出生1日後に投与した。仔マウスにOVA 5 μg/体重(g)とアラムを腹腔内投与することによる感作は、G1 Groupでは生後0週(出生2日後および9日後)、G2 Groupでは生後2週(出生16日後および23日後)、あるいはG4 Groupでは生後6週(出生44日後および51日後)後から開始した。最終感作2週間後に、採血し、OVAに特異的なIgE抗体の抗体価を測定した(図19A)。その結果、図19Bに示すように、生後0, 2および6週後のいずれの時期から感作した場合にも、新生児期にisotype-matched control抗体を投与したマウスをOVAで感作した場合には、OVAに特異的なIgE抗体の抗体価の明らかな上昇が観察されたが、新生児期に抗マウスIgE抗体を投与したマウスでは、OVAで感作してもOVAに特異的なIgE抗体の抗体価は全く上昇しなかった。そして、これらのOVAに特異的なIgE抗体の産生抑制作用とその持続期間は、妊娠マウスに投与した場合(図14A)と同様であることが判明した。
上述の結果により、妊娠C57BL/6マウスへ抗マウスIgE抗体(Omalizumabと同じIgG1κ抗体)を投与することにより、仔マウスにおいて、アレルゲンの刺激によるT細胞由来のサイトカイン(IL-4, IL-13, IL-17, IFNγ)の産生や感染症の予防等に重要な役割を果たすアレルゲンに特異的なIgG1抗体の産生には影響を与えることなく、アレルギー反応の発症・進展に重要な役割を果たすアレルゲンに特異的なIgE抗体の産生がほぼ完全に抑制されることが明らかになった。さらに、アレルゲンに特異的なIgE抗体の産生のほぼ完全な抑制が、少なくとも6週間(ヒトの場合は3-4ヶ月に相当)継続することも判明した。一方、妊娠BALB/cマウスへ抗マウスIgE抗体を投与した場合には、アレルゲンに特異的なIgE抗体の産生の抑制の持続期間が非常に短かかったが、その原因の1つとして、BALB/cマウスは、C57BL/6マウスに比べて、アレルゲン感作時の総IgE抗体量が明らかに高く、抗IgE抗体がB細胞上のmIgEに結合する確率が低下していたことが考えられる。ヒトの乳児期は総IgE抗体量のレベルが低く維持されていること(72)を考え合わせると、ヒトの場合は、C57BL/6マウスの結果に近い結果になると考えられる。
本発明者らは、「妊婦に抗ヒトIgE抗体活性をもつOmalizumabなどの治療用IgG抗体を投与することで、胎盤組織に強く発現するFcRnを介して胎児にこの治療用IgG抗体を移行させ、抗ヒトIgE抗体を胎児B細胞の膜結合型IgE と結合させることにより、最終的に胎児期から乳児早期のIgE抗体の産生を阻害する(胎児期から乳児早期のIgEクラスに特異的な免疫反応を抑制する)」という乳児期のアレルギー疾患対策を立案した。そして、妊娠マウスを用いた評価系により、抗マウスIgE抗体(Omalizumabと同じIgG1κ抗体)を妊娠マウスに投与すると新生仔マウスのアレルゲンに特異的なIgE抗体の産生を特異的に、少なくとも6週間(ヒトでは3-4ヶ月間)、抑制することが可能であることを見出すことにより、乳児期のアレルギー疾患対策の妥当性を証明した。FcRnを介した治療用IgG抗体の胎児への移行の作用機序を考え合わせると、今回得られたマウスの結果は、ヒトへ外挿が可能であると考えられる。

Claims (21)
- 子を妊娠している母及び/又は誕生後から乳児期までの間のいずれかの時期にある子に投与することを特徴とする、乳児期のアレルゲンに特異的なIgE抗体の産生を抑制するための、抗IgE抗体を含有する医薬。
- 子を妊娠している母及び/又は誕生後から乳児期までの間のいずれかの時期にある子に投与することを特徴とする、胎児期から乳児期においてIgEクラスに特異的な免疫反応を抑制するための、抗IgE抗体を含有する医薬。
- 乳児期およびそれ以降のアレルギー疾患の発症を予防する、請求項1又は2記載の医薬。
- 子を妊娠している母及び/又は誕生後から乳児期までの間のいずれかの時期にある子に投与することを特徴とする、乳児期のアレルゲンに特異的なIgE抗体の産生を抑制するための、抗IgE抗体を含有する医薬とその子に対するアレルゲンを用いた免疫療法との併用。
- 子を妊娠している母及び/又は誕生後から乳児期までの間のいずれかの時期にある子に投与することを特徴とする、胎児期から乳児期においてIgEクラスに特異的な免疫反応を抑制するための、抗IgE抗体を含有する医薬とその子に対するアレルゲンを用いた免疫療法との併用。
- 乳児期およびそれ以降のアレルギー疾患の発症を予防する、請求項4又は5記載の併用。
- 子を妊娠している母及び/又は誕生後から乳児期までの間のいずれかの時期にある子に投与することを特徴とする、乳児期およびそれ以降の疾患の発症を予防するための、治療用抗体を含有する医薬。
- 子を妊娠している母及び/又は誕生後から乳児期までの間のいずれかの時期にある子に抗IgE抗体を医薬的に有効な量で投与することを含む、乳児期のアレルゲンに特異的なIgE抗体の産生を抑制する方法。
- 子を妊娠している母及び/又は誕生後から乳児期までの間のいずれかの時期にある子に抗IgE抗体を医薬的に有効な量で投与することを含む、胎児期から乳児期においてIgEクラスに特異的な免疫反応を抑制する方法。
- 乳児期およびそれ以降のアレルギー疾患の発症を予防する、請求項8又は9記載の方法。
- 子を妊娠している母及び/又は誕生後から乳児期までの間のいずれかの時期にある子に抗IgE抗体を医薬的に有効な量で投与することを含む、乳児期のアレルゲンに特異的なIgE抗体の産生を抑制する方法であって、その子にさらにアレルゲンを用いた免疫療法を施すことを含む前記方法。
- 子を妊娠している母及び/又は誕生後から乳児期までの間のいずれかの時期にある子に抗IgE抗体を医薬的に有効な量で投与することを含む、胎児期から乳児期においてIgEクラスに特異的な免疫反応を抑制する方法であって、その子にさらにアレルゲンを用いた免疫療法を施すことを含む前記方法。
- 乳児期およびそれ以降のアレルギー疾患の発症を予防する、請求項11又は12記載の方法。
- 子を妊娠している母及び/又は誕生後から乳児期までの間のいずれかの時期にある子に治療用抗体を医薬的に有効な量で投与することを含む、乳児期およびそれ以降の疾患の発症を予防するための方法。
- 子を妊娠している母及び/又は誕生後から乳児期までの間のいずれかの時期にある子に投与することを特徴とする、乳児期のアレルゲンに特異的なIgE抗体の産生の抑制に使用するための、抗IgE抗体。
- 子を妊娠している母及び/又は誕生後から乳児期までの間のいずれかの時期にある子に投与することを特徴とする、胎児期から乳児期においてIgEクラスに特異的な免疫反応の抑制に使用するための、抗IgE抗体。
- 乳児期およびそれ以降のアレルギー疾患の発症を予防する、請求項15又は16記載の抗IgE抗体。
- 子を妊娠している母及び/又は誕生後から乳児期までの間のいずれかの時期にある子に投与することを特徴とする、乳児期のアレルゲンに特異的なIgE抗体の産生の抑制に使用するための、抗IgE抗体であって、その子に対するアレルゲンを用いた免疫療法と併用される前記抗IgE抗体。
- 子を妊娠している母及び/又は誕生後から乳児期までの間のいずれかの時期にある子に投与することを特徴とする、胎児期から乳児期においてIgEクラスに特異的な免疫反応の抑制に使用するための、抗IgE抗体であって、その子に対するアレルゲンを用いた免疫療法と併用される前記抗IgE抗体。
- 乳児期およびそれ以降のアレルギー疾患の発症を予防する、請求項18又は19記載の抗IgE抗体。
- 子を妊娠している母及び/又は誕生後から乳児期までの間のいずれかの時期にある子に投与することを特徴とする、乳児期およびそれ以降の疾患の発症の予防に使用するための、治療用抗体。
