抗体のFab部分上のN結合型糖鎖に結合し得る分子に前記サンプルの抗体を接触させること、及び、結合抗体を収集すること、及び、
もしあるなら、収集抗体を、翻訳後修飾エピトープを含むペプチド、好ましくは、アセチル化リシンエピトープ、シトルリン化又はホモシトルリン化エピトープを含むペプチドに接触させること、を備える。

(実施例1)
(材料と方法)
〔患者サンプル〕
ACPAグリコシル化を全IgGグリコシル化と比較する実験のため、9名のACPA陽性RA患者から血漿(n=6)及び滑液(n=3)サンプルをLUMC(ライデン大学メディカルセンター)のリウマチ科の外来診察室で収集した。全てのRA患者は関節リウマチ分類基準(1987年、米国リウマチ学会)を満たした。
ライデンで収集したサンプルのACPA-IgG及びIgGを、既述のように[3]、高速タンパク質液体クロマトグラフィー(A¨KTA、GEヘルスケア製)で精製した。簡単に説明すると、サンプルを、ビオチン化CCP2-シトルリン-HiTrap-ストレプトアビジンカラムが後に直列に接続されているビオチン化CCP2-アルギニン-HiTrap-ストレプトアビジンカラム(GEヘルスケア製)に載せた。フロースルー(FT)画分及びACPA溶出画分を、さらに、HiTrapプロテインG上に載せ、その後、HiTrapプロテインAカラム上に載せた(両方ともGEヘルスケア製)。ACPA枯渇IgG(対照IgG)及びACPA-IgGの精製画分をサイズ排除クロマトグラフィーにより濃縮脱塩した。
ACPAを一般的なIgGと比較するための構造分析を、単離ACPA-IgG及びIgGのF(ab)2又はFc断片で行った。F(ab)2又はFc断片は、IdeS(FabRICATOR、ジェノビス(Genovis)製)での抗体消化により生成し、IgG-Fc/CH1 CaptureSelect affinity beads(サーモフィッシャー製)により精製した。(ACPA)-IgGのF(ab)2又はFc断片由来のN糖鎖をPNGase Fを用いて溶液中に遊離させた。糖鎖を、2アミノ安息香酸(2-AA)で標識し、親水性相互作用クロマトグラフィー固相抽出(HILIC-SPE)により精製し、マトリックス支援レーザー脱離イオン化飛行時間型質量分析計(MALDI-TOF-MS)及び超高速液体クロマトグラフィー(UHPLC)により解析した。
UHPLCデータをChromeleon 7で分析した。このソフトはクロマトグラムの曲線下面積を計算する。糖鎖ピーク及びグリコシル化由来特性を既述の通り[4]明らかにした。ガラクトシル化、シアル化、及びフコシル化の百分率、並びに、IgGのバイセクティングN-アセチルグルコサミン残基の頻度を、計算した。加えて、(GP19からGP24までの合計)÷(GP1からGP14までの合計)×100%という式を用いて、Fabグリコシル化百分率を計算した。統計分析はGraphPad Prism 6を用いて行った。ノンパラメトリック・ペアード・ウィルコクソン検定をp<0.05の有意水準で適用した。
最近、発明者等は、RA患者から得たACPA-IgGが、非自己反応性IgGに比べて、10~20kDa高い分子量を示すことを発見した。この特徴は、また、ACPA-IgGをリコール抗原に対する抗体又は他の疾患特異的自己抗体から際立たせた。構造分析は、ACPAの(超)可変ドメイン(F(ab)ドメイン)でのN結合型糖鎖の百分率がこの観察結果の原因であることを示した。N結合型糖鎖が位置する正確な部位の解明は、タンパク質のN結合型グリコシル化に必要なN結合型コンセンサス配列が、生殖細胞系にコードされておらず、体細胞超変異の際に導入されることを明らかにした[5]。ACPA上に存在するN結合型Fab糖鎖の構造分析は、Fab結合型糖鎖の組成が、Fc結合型糖とは異なり、より重要なことに、これらが高度にシアル化されていることを示した(図1)。さらに、定量に基づき、発明者等が評価したところ、血清中に存在するACPA分子の90%超がF(ab)糖鎖を持っており、この百分率は滑液中のACPAではずっと高い。
(材料と方法)
〔患者サンプル〕
9名のACPA陽性RA患者から血漿(n=6)及び滑液(n=3)サンプルをライデン大学メディカルセンターのリウマチ科の外来診察室で収集した。全てのRA患者は、関節リウマチ分類基準(1987年、米国リウマチ学会)を満たし、書面によるインフォームドコンセントを受けた[9]。治療は、疾患修正性抗リウマチ薬、生物学的薬剤、及び糖質コルチコイドを含んでいた。
TFA、SDS、リン酸水素二ナトリウム・二水和物、HCL、グリシン、β-メルカプトエタノール、酢酸、及びNaClは、メルク(独国、ダルムシュタット)から購入した。50%水酸化ナトリウム及びノニデットP-40代替品、ヒアルロニダーゼ(ウシ精巣由来、タイプIV)、EDTA、2-アミノ安息香酸、ボラン-2-ピコリンコンプレックス、水酸化アンモニウム、DMSO、及びギ酸は、シグマ・アルドリッチ(米国、セントルイス)から得た。Trisはロシュ(米国、インディアナ)から購入し、Laemmli緩衝液をバイオ・ラッド(米国、カルフォルニア)から得た。Peptide:N-glycosidase F(PNGase F)をロシュ・ダイアグノスティックス(独国、マンハイム)から、2,5-ジヒドロキシ安息香酸をブルカー・ダルトニクス(独国、ブレーメン)から、及びHPLC SupraGradient ACNをバイオソルブ(蘭国、ファルケンスワールト)から購入した。全体を通して、MQ(ミリQ脱イオン水、R>18.2MΩcm-1、Millipore Q-Gard 2 system、ミリポア、蘭国、アムステルダム)を用いた。CaptureSelect anti-IgG Fc affinity matrix及びanti-CH1 affinity matrixをライフ・テクノロジーズ(蘭国、ライデン)から購入した。空のスピンカラム(閉じたスクリューキャップ、挿入されたプラグ、大型10umフィルター付属)はMoBiTec(独国、ゲッティンゲン)から提供された。PBSをビー・ブラウン(独国、メルズンゲン)から、IdeS酵素(商品名:FabRICATOR)をジェノビス(典国、ルンド)から得た。CCP2アルギニン(対照)及びCCP2シトルリンペプチドは、J.W.Drijfhout博士(蘭国、ライデン大学メディカルセンター(LUMC)、IHB科)の好意により提供された。
ACPA-IgG及びIgGを、既述のように[5]、高速タンパク質液体クロマトグラフィー(A¨KTA、GEヘルスケア製)で精製した。サンプルを、ビオチン化CCP2-シトルリン-HiTrap-ストレプトアビジンカラムが後に直列に接続されているビオチン化CCP2-アルギニン-HiTrap126-ストレプトアビジンカラム(GEヘルスケア製)に載せた。フロースルー(FT)及びACPA溶出画分を、さらに、HiTrapプロテインG上に載せ、その後、HiTrapプロテインAカラム上に載せた(両方ともGEヘルスケア製)。その後、アイソタイプ1、2、及び4の精製IgG及びACPA-IgGを、サイズ排除クロマトグラフィー(ZebaSpin Desalting Column、7K MWCO、Pierce Thermo Scientific)により製造者の説明に従い濃縮脱塩した。
組換ストレプトコッカスIdes酵素を用いてACPA-IgG及びACPA枯渇IgGを特異的に切断しFc部分及びF(ab’)2部分を得た。既述のように[5]簡略化のために供給元のプロトコルを調整した。簡単に説明すると、各サンプルについて、(ACPA)-IgG抗体30μgを、遠心エバポレーター下で乾燥させ、IdeS 30Uを含む消化バッファー(50mM リン酸ナトリウム、150mM NaCl、5mM EDTA)200μLを添加し、オーバーナイトにわたり37℃でインキュベートすることで、消化した。その後、10μMフィルタースピンカラム上に載せたanti-IgG Fc affinity matrix(ビーズスラリー)でのアフィニティークロマトグラフィーによりF(ab’)2からFc部分を分離した。Fc断片を、100mMギ酸でビーズから溶出させ、2M Trisで中和した。F(ab’)2ドメインを捕捉するために、Fc精製から生じたFT画分を、anti-IgG Fc affinity matrixと同様のプロトコルを用いて、anti-IgG-CH1 affinity matrixで精製した。溶出画分を、2M Trisで中和し、サイズ排除クロマトグラフィー(Zeba Spin Desalting Column、7kDa MWCO、Pierce Thermo Scientific)により脱塩した。精製後、精製Fc及びF(ab’)2サンプル6μgを、SDS-PAGEにより純度分析し、bicinchoninic acid Protein Assay Reagent(Pierce Thermo Scientific)により定量した。糖鎖分析のため、サンプルを真空遠心により乾燥させた。
RA患者由来の単離ACPA-IgG及びIgGの、F(ab)2若しくはFc断片又は全分子のいずれかについて、構造分析を行った。加えて、(ACPA)-IgGのドデシル硫酸ナトリウム-ポリアクリルアミドゲル電気泳動(SDS-PAGE)を行った。(ACPA)-IgGの、N糖鎖型の、全分子、F(ab)2断片、及びFc断片をPNGase Fを用いて溶液中に遊離させ、一方、重鎖及び軽鎖(HC/LC)糖鎖をPNGaseによるゲル内消化後に得た。糖鎖の標識を、サンプル(25μL溶液)と、2-アミノ安息香酸(2-AA、48mg/mL)の15%氷酢酸含有DMSO溶液12.5μLと、2-ピコリンボラン(107mg/mL)のDMSO溶液12.5μLとを混合することにより、行った。この混合液を、65℃で2時間インキュベートし、室温まで冷やし、精製前に85%ACNに希釈した。2-AA標識糖鎖を、いくらかの修正を加えた上で既述の通り[10]、コットンチップを用いてHILIC SPEにより精製した。簡単に説明すると、各サンプルについて、コットン500μgを、200μLピペットチップ内に詰め、MQ 150μLを3回、その後、85%ACN 0.1%TFA 150μLを1回、及び、85%ACN 150μLを2回ピペッティングすることにより、コンディショニングした。サンプル(85%ACN溶液)を、25回ピペッティングすることで、反応混合液内にロードした。チップを、85%ACN 0.1%TFA 150μLを1回、及び、85%ACN 150μLを2回の組み合わせ3回で、3回洗浄した。最終的に、2-AA標識糖鎖を、MQ 30μLでコットンから溶出させ、MALDI-TOF-MS及びUHPLCで同定した。MALDI-TOF-MS分析では、コットンHILIC SPEにより精製した糖鎖サンプル2μLを、Bruker AnchorChip plate(800μm anchor、ブルカー・ダルトニクス、独国、ブレーメン)上、スポット内で、2,5-ジヒドロキシ安息香酸マトリックス(20mg/mL、50%ACN、50%水の溶液)1μLと混合し、周囲温度で乾燥するに任せた。測定は、UltrafleXtreme MALDI-TOF-MS(ブルカー・ダルトニクス)のlinear negative modeでFlexControl 3.4 software(ブルカー・ダルトニクス)を用いて行った。peptide calibration standard(ブルカー・ダルトニクス)を用いて外部較正を行った。各スペクトルについて、m/z1000から4000の質量窓を用い、最低5000回のレーザーショットを重ねた。UHPLC分析では、精製2-AA標識N糖鎖溶液5μLを、1.7μm 2.1x100 mm Acquity UHPLC BEH Glycan column(ウォーターズ)と蛍光検出器とを取り付けたDionex Ultimate 3000(サーモ・フィッシャー・サイエンティフィク)でのHILIC-UHPLCにより分離・分析した。分離は、0.6mL/分の流速、60℃で行った。勾配形成のために2種の溶液(溶液A:ACN、溶液B:100mMギ酸アンモニウムpH4.4(ギ酸を水酸化アンモニウムでpH4.4まで緩衝化することで調整))を用いた。カラムを、0.5分間、85%の溶液Aで平衡化した。その後、サンプルを、75%Aに載せ、10分間、85%Aで洗浄することにより、過剰な蛍光試薬をカラムから溶出させた。分離勾配は、75%Aから始め、30分かけて、63%まで直線的に減少させた。その後、カラム内に、4分間、40%Aを、0.4mL/分の流速で流し、その後、再平衡化のために、10分間、85%Aを流した。蛍光検出では、330nmを励起に用い、420nmで発光を記録した。結果として得られたクロマトグラムを、Chromeleon version 7.1.2.1713(サーモ・フィッシャー・サイエンティフィク)を用いて分析した。最終的に、糖ペプチド水準での(ACPA)-IgGのFc結合型グリコシル化を分析するために、抗体を、トリプシンで消化し、既述の通り[11]、LC-MSで分析した。
UHPLCデータをChromeleon 7で分析した。このソフトはクロマトグラムの曲線下面積を計算する。糖鎖ピーク及びグリコシル化由来特性を既述の通り[4]明らかにした。ガラクトシル化(非ガラクトシル化 G0、モノガラクトシル化 G1、ジガラクトシル化 G2)、シアル化(非シアル化 N、モノシアル化S1、ジシアル化S2)、フコシル化(F)の百分率、並びに、IgGのバイセクティングN-アセチルグルコサミン(GlcNAc、B)残基の頻度を、以下のように、計算した。G0=GP1+GP2+GP4+GP5+GP6、G1=GP7+GP8+GP9+GP10+GP11+GP16、G2=GP12+GP13+GP14+GP15+GP17+GP18+GP19+GP21+GP22+GP23+GP24、N=GP1+GP2+GP4+GP5+GP6+GP7+GP8+GP9+GP10+GP11+GP12+GP13+GP14+GP15、S1=GP16+GP19、S2=GP21+GP24、F=GP1+GP4+GP6+GP8+GP9+GP10+GP11+GP14+GP15+GP16+GP18+GP19+GP23+GP24、及びB=GP6+GP10+GP11+GP13+GP15+GP19+GP22+GP24。LC-MS糖鎖特性分析は既述の通り[11]である。LC-MSデータの処理については、観察した検体チャージ状態毎の最初の3つのアイソトープの合計強度を、以前記した通り[12]、理論質量周辺の±0.06Da及び手動抽出平均保持時間周辺の±20秒の窓内で抽出した。MALDI-TOF-MSによる糖鎖同定は、既述の通り[13]、明らかにした。統計分析はGraphPad Prism 6を用いて行った。ノンパラメトリック・ペアード・ウィルコクソン検定をp<0.05の有意水準で適用した。
〔IgG及びACPA-IgGにより発現されたN糖鎖の定量〕
発明者等は、RA患者により生産されたACPA-IgGが、他のIgG(自己)抗体に比べて、可変領域内で広範にNグリコシル化されていることを示した[5]。ここで、発明者等は、ACPA-IgG及びその断片のグリコシル化の包括的な定量化及び定性化を行い、非シトルリン特異的IgG(即ち、ACPAが枯渇したもの、以降では、対照IgGと呼ぶ)のものと比較した。この目的のため、(ACPA)-IgGをアフィニティークロマトグラフィーにより精製し、その糖鎖を、図9に示したスキームに従い、UHPLC、MALDI-TOF-MS、及び/又はLC-MSにより、分析した。精製後、(ACPA)-IgGの純度を還元条件下でSDS-PAGEにより評価した(図10A)。期待の通り、対照IgGは重鎖及び軽鎖(HC及びLC)に対応する2つの電気泳動バンドにより特徴づけられ、一方、ACPA-IgGは既述の通り(13)より分子量が高い複数のHC及びLCバンドを示した。IgGのHC及びACPA-IgGのHC1の両方からの遊離N糖鎖は、UHPLCでは典型的なFc結合型糖鎖プロファイルを示し(21、25)、一方、IgGのLC及びACPA-IgGのLC1ではN糖鎖は検出されなかった(図10B)。対照的に、ACPA-IgGのLC2から遊離したN糖鎖は、異なるプロファイルを示し、高度にシアル化された二分岐糖形態の存在を示した(図10B)。同様に、ACPA-IgGのHC2及びHC3由来のグリコシル化プロファイルは、Fcドメインに通常は存在しない追加の糖鎖の存在とともに、Fc糖鎖の混合物の存在を示した(図10B)。
ACPA-IgGの追加HCバンド、即ち、HC2及びHC3で検出された糖鎖パターンが本当にIgG可変領域のグリコシル化を反映しているかを確かめるために[14]。発明者等は、(ACPA)-IgG及びその断片のNグリコシル化(合計/Fc/Fab)又は糖ペプチドのNグリコシル化(Fcのみ)を調べた(図9)。まず、発明者等は、(同じ提供者からの)ACPA-IgG及び対照IgGのFc及びF(ab’)2断片から遊離されたN糖鎖の構造を分析・比較した。(ACPA)-IgGのFc断片由来のNグリコシル化プロファイルは、さまざまな数の分岐ガラクトース(0から2)及びシアル酸(0から1)残基を有する、二分岐の、しばしばコアの、フコシル化コンプレックス型種から構成された典型的なFc結合型N糖鎖構造を示した(図11A)。また、Fc結合型糖鎖の一部は、バイセクティングGlcNAcを含有していた。比較的高い割合の無ガラクトシル化糖鎖(G0)が既述の通り[11]観察されたことに注意されたい。(ACPA)-IgGのF(ab’)2断片から遊離したN糖鎖は、バイセクティングGlcNAc及び/又はコアフコースを持っているかもしれない高度にガラクトシル化及びシアル化された二分岐の糖形態から構成されていた(図11B)。まとめると、これらの結果は、IgG(自己)抗体のFc及びFab断片に結合した糖鎖種が、ACPA-IgGのFabドメインに結合した糖鎖内の高度シアル化糖鎖種の目立つ存在とは異なることを示す。
発明者等は、患者から単離したACPA-IgGのFc結合型N糖鎖が、他のIgG分子のものよりも、ガラクトシル化及びシアル化の水準がより顕著な減少を呈し、一方、コアフコシル化の程度の増加を呈することを示している[11、15]。一致して、本研究で精製したACPA IgGのFc糖鎖が、対照IgGのものと比較して、より低水準のシアル化(SがACPA-IgGでは12%[IQR9-16%]に対し対照IgGでは16%[IQR13-17.5%])と、より高頻度のコアフコシル化(FがACPA-IgGでは99.3%[IQR98.7-99.7%]に対しIgGでは91.8%[IQR90.3-99.7%])とを示す。加えて、一方、発明者等のデータは、ACPA-IgGのFab結合型N糖鎖と対照IgGのFab結合型N糖鎖との間の重要な違いを明らかにした(図12)。特に、ACPA-IgGのFabのN糖鎖は、ジガラクトシル化種(G2がIgGでは73%[IQR69.5-80%]に対しACPA-IgGでは84%[IQR74-87%])及びジシアル化された259種(S2がIgGでは27%[IQR19-30%]に対しACPA-IgGでは44%[IQR34-48.5%])の頻度が高いことを示し、ガラクトースに対するシアル酸の割合の増加(SA/GalがACPA-IgG及びIgGについて36%[IQR33-37%]対30%[IQR27-31.5%])も例証となる。加えて、発明者等は、9サンプル中の7サンプルでコアフコース及びバイセクティングGlcNAc残基の水準が高くなっていることを発見した。全体として、ACPA-IgG及び対照IgGのFabドメイン由来の糖鎖構造の間で、Fc部分由来の糖鎖の間でよりも強い糖鎖の差異が観察された。
発明者等は、ACPA-IgG及びACPA枯渇IgG上に存在するFabグリコシル化の量を定量した。Fabグリコシル化の水準を評価するために、糖鎖を、ACPA-IgG及びACPA枯渇IgGから遊離させ、MALDI-TOF-MSにより特徴づけし、その相対量をUHPLCにより測定した。全IgGの糖鎖プロファイルはFc結合型N糖鎖により支配され(G0F、G1F、及びG2F)、一方、ACPA-IgGの全糖鎖プロファイルはFab結合型N糖鎖が大量であることを示した(G2FBS1、G2FS2、及びG2FBS2)(図13A)。重要な点として、Fc断片又はF(ab’)2断片のいずれかについて特異的なこれらの糖形態の数々の同定、及び、全抗体分子から遊離させたこれらの糖形態の定量により、ACPA-IgG又はACPA枯渇IgGのいずれかでのFab糖鎖の全体頻度を判定することが可能となった(図13B)。全てのACPA-IgGサンプル(n=9)は、対照IgGに比べて、Fabグリコシル化の頻度が増加することを示した。ACPA枯渇IgGのメジアンFabグリコシル化水準は、提供者間で大きな違いをもって、17%[IQR12%-26%]と評価された。対照的に、ACPA-IgGのメジアンFabグリコシル化は93%[IQR77-123%]に達した。まとめると、これらのデータは、ACPA-IgGのメジアンFabグリコシル化が対照IgGのものよりも5倍分高いことを示す。
発明者等は、滑液由来のACPA-IgGが、血清由来のACPA-IgGよりも炎症誘発性のFcグリコシル化プロファイルを示すことを示した[16]。この観察結果を考慮して、発明者等は、ACPA-IgG及び対照IgGのFab結合型糖鎖構造及び/又はFabグリコシル化水準でも差異が生じると仮説を立てた。血漿のACPA-IgG(n=6)のFab結合型糖鎖に比べて、滑液由来のACPA-IgG(n=3)Fab糖鎖は、ガラクトシル化、シアル化、及びバイセクティングGlcNAcの水準が低くなる傾向を示した。同様の傾向が、血漿IgGと比較した滑液対照IgGの糖鎖プロファイルでも観察された。発明者等は、次に、血漿(ACPA)-IgGのFabグリコシル化の水準及び滑液の対応する値を定量した。図13Cに示すように、滑液ACPA-IgGで血漿ACPA-IgGと比べてFabグリコシル化の水準が有意に高いことがわかった(138%対80%)。こうした差異が対照IgGでは観察されなかった(20%対20%)ことに注意されたい。まとめると、これらの観察結果は、定量的な意味で、滑液由来のACPA-IgGで血液由来のACPA-IgGと比べてFabグリコシル化の水準がずっと顕著であることを示す。
(材料と方法)
〔ACPAへのIgGの又は抗CarP IgGのFab部分のN結合型グリコシル化の決定〕
ビオチン化CCP2又はアルギニン対照ペプチドを、異なる蛍光色に標識したストレプトアビジン四量体にコンジュゲートする。蛍光活性化セルソーティング(FACS)により、四量体陽性B細胞をソートする。2種類の異なる方法でB細胞受容体(BCR)配列を決定する。ACPA特異的B細胞を単離する一般的な方法は、Kerkman等,6月2日,2015;Ann. Rheum. Dis 0:1-7;doi:10.1136/annrheumdis-2014-207182に記載されている。
単一細胞ソートの方法でシーケンシングした、82%(23/28)のIgG、0%(0/3)のIgM、及び0%(0/1)のIgAのACPA抗体が、Fab部分にNグリコシル化部位を有していた。多細胞ソート及び次世代シーケンシングの方法では、94%(17/18)のIgG、40%(2/5)のIgM、及び100%(9/9)のIgAのACPA抗体が、Fab部分にNグリコシル化部位を有していた。比較すると、健常提供者から得た全B細胞のBCRレパートリーの配列分析は、約9%の抗体のみがNグリコシル化部位を含有することを示す。このデータから、ACPA配列がFab領域にNグリコシル化部位を有する百分率は健常個体由来の他の配列での百分率よりも有意に高い。
〔ACPAはアセチル化ペプチド(リシン及びオルニチン)を認識・結合できる〕
(材料と方法)
モノクローナル及びポリクローナルのACPA抗体が異なるPTMを有する変異ビメンチンペプチドに結合できるか否かを決定した。Rispens博士(Sanquin)に提供されたモノクローナルACPA E4 IgG1[17]を、PTM改変ビメンチンペプチドへの反応性について分析した。RA患者由来のポリクローナルACPAは、ゲルろ過カラムにより予め精製したものであり、ACPA2.93及び2.77が精製された。
モノクローナルACPA E4は、シトルリン、アセチル化リシン、及びアセチル化オルニチンの翻訳後修飾を有する変異ビメンチンペプチドに対して反応性である。RA患者由来のポリクローナルACPA2.93は、シトルリン及びアセチル化オルニチンの翻訳後修飾を有する変異ビメンチンペプチドに対して反応性である。ポリクローナルACPA2.93は、より程度は低いものの、アセチル化リシン及びホモシトルリンにも反応性である。また、RA患者由来のポリクローナルACPA2.77は、アセチル化オルニチン、アセチル化リシン、シトルリン、及び、より程度は低いものの、ホモシトルリンの翻訳後修飾を有する変異ビメンチンペプチドに対して反応性である(図15)。
モノクローナルACPA E4及びRA患者から得たポリクローナルACPA(2.93及び2.77)は、シトルリン、アセチル化リシン、及びアセチル化オルニチンの翻訳後修飾を有する変異ビメンチンペプチドに対して反応性である。これらの異なるアミノ酸への結合は、ACPAが異なるPTMに対して交差反応性であることを示す。

(材料と方法)
〔患者及び健常個体〕
末梢血サンプルをRA罹患ACPA陽性患者から得た。患者は、ライデン大学メディカルセンター(LUMC)のリウマチ科の外来診療室で募集され、書面によるインフォームドコンセントを受けた。健常提供者サンプルを、同種幹細胞移植のために収集された材料の残りから取得し、既述の通り[1]、シーケンシングした。
ACPA発現B細胞を、既述の通り[2]、末梢血単核細胞(PBMC)から単離した。破傷風毒素(TT)特異的B細胞を、AnaTagTM Labeling Kit(サーモフィッシャー製)で調整した直接標識TT(スタテン・セラム・インスティチュート製)を用いて、単離した。細胞を、既述の通り[3]、10個の細胞のプール内で、又は、単一細胞として、ソートした。1つの患者サンプルを、以下の方法の両方で処理した。培養上清でのACPA-IgGの存在をELISAにより評価した[2]。
プールとしてソートされた細胞を、Triton X-100(シグマ製)を用いて直に溶解させ、その後、mRNA単離を行った。単一細胞培養物由来のmRNAを、TRIzol(サーモフィッシャー製)を用いて単離した。いずれの単離手法の場合も、その後、既述の通り[4]、cDNAを合成した。
修正[1]を加えた免疫グロブリン配列のアンカリングリバーストランスクリプション及びネステッドPCR増幅(Anchoring Reverse Transcription of Immunoglobulin Sequences and Amplification by Nested PCR;ARTISAN PCR)を用いてIg転写物を増幅した。プールした細胞のPCR産物は、PacBio RSII system(パシフィック・バイオサイエンス、米国、カリフォルニア、メンローパーク)でシーケンシングした。単一細胞培養物から得たPCR産物は、サンガーシークエンシング[5]でシーケンシングした。配列データを、Geneious R9.1.5[6]及びIMGT (High)V-QUEST tools[7]で分析した。
〔ACPA BCR配列でのNグリコシル化部位の局在〕
シトルリン特異的B細胞のBCR配列は、Nグリコシル化部位の頻度が目覚ましいことを示す。それらがSHM割合から独立していることは、可変領域のN糖鎖が発生及び/又は成熟の際にACPA発現B細胞に選択的な利益を与えることを示唆する。この可能性への洞察をより深めるために、発明者等は、部位の分布を調べ、そのパターンを健常提供者B細胞受容体(BCR)レパートリーで同定されたNグリコシル化部位と比較した。ACPA-IgG配列について、発明者等は、部位がCDR1領域で優勢であり、ACPA-IgGのCDR3領域では比較的に欠如しているという観察結果を得た。これらの結果は、ACPA-IgGのNグリコシル化部位分布が、CDR3領域を避け、CDR1領域での糖鎖のいくらかの選好を示すことを示唆する[8]。より具体的には、IgG重鎖、κ軽鎖、及びλ軽鎖のV遺伝子を詳細に評価することで、BCR配列の特定の位置上での部位の富化、及び、他の位置上での部位の欠如をみることができる。IgGの重鎖及びκ軽鎖のV遺伝子を考慮すると、位置29及び77上での部位の富化及びCDR3領域のいくつかの位置での部位のより低い量という同様のパターンがみられる。λ軽鎖では、位置37、51、56での部位の欠如があるようにみえるが、これらの位置は健常個体から得たBCR配列では高度に存在する(図19)。

〔ACPA-Fab糖鎖の検出方法〕
〔方法1〕
発明者等は、精製ACPA-IgGのハイエンドUHPLC及び質量分析を用いた(図22)。
方法2(図3)では、ACPAを上述の通りCCP2コートマイクロビーズ(上)を用いて捕捉する。ACPA IgG分子上のF(ab)糖鎖の存在を計算するために、2種の異なるELISAを用いる。SNAベースレクチンによりシアル化ACPAを可視化するための第1のELISAを図3(下)に示す。第2のELISAでは、ACPA IgGの量を計算するために、ベシル(Bethyl)製のキットHuman IgG ELISA Kit、E88-104)を用いる。両方のELISAで、まず、プレートを10μg/ml過ヨウ素酸処理ヤギ抗ヒトIgG捕捉抗体でコートする。200mM過ヨウ素酸を、4℃、30分間、インキュベートし、RT、1時間、ヤギ抗ヒト抗体上に存在するシアル酸を破壊した。プレートを、PBS0.05%tweenバッファーで3回洗浄し、その後、RT、1時間、1%BSA-PBS(やはり20mM過ヨウ素酸でオーバーナイトにわたり4℃で処理済み)でブロックした。プレートを洗浄したあと、溶出させたACPA溶出物(図3、上)を、両方のプレートに加え、RT、1時間、インキュベートした。インキュベーション及び洗浄の後、一方のプレートを、2μg/mlのビオチン化SNAとともに、RTで、1時間、インキュベートし、他方のプレートを、ヤギ抗ヒトIgG HRPとともに、RTで、1時間、インキュベートした。インキュベーション後、再び、プレートを洗浄した。その後、ABTSをヤギ抗ヒトIgG HRPともに予めインキュベートしたプレートに加え、吸光度を415nmで測定した。SNAとともに予めインキュベートしたプレートに、Strep-HRPを加え、RTで、1時間、インキュベートした。インキュベート後、吸光度を同様のアプローチで測定した。結果の分析について、両方のプレートは標準曲線を含んでいた。
方法3では、反対の戦略を用いる。まず、血清又は血漿由来の全IgGを前に記載したマイクロビーズアッセイと同様のアプローチにより単離した。その後、SNAアガロースビーズによりSNA上にIgGを固定する。最後に、SNA溶出画分及びフロースルー画分でCCP ELISAを用いてACPA-IgGを検出する(図4)。ACPA-IgG上に存在するジシアル化Fab糖鎖の量が高いために、CCP反応性の富化がSNA溶出画分では期待される。このアプローチは、ACPA-IgGについて実行可能であり、図4.2に示すように、ハイスループットなやり方で用いることができる。重要な点として、IVIGサンプルのUHPLC分析からSNA溶出画分でIVIGに含有されるFabグリコシル化IgG分子を富化できたことが示されたことから(図23)、SNAアガロースビーズが実際にこの構成でFabグリコシル化IgGを捕捉することを発明者等は確かめた。また、図4に示すデータは、このアプローチの実現可能性を示し、ACPAの存在又はACPAの高度グリコシル化Fabドメインを可視化する。プロテインGによるIgGの事前単離が、シアル酸分子を持ち且つ血清中に存在する他の分子によるSNAの『過剰充填』を防ぐために、重要であることに注意されたい。ただし、SNA上の血清充填量を調整することで、この方法を反対に用いることもできる。

Fab部分上にN結合型糖鎖を有する抗体が前記ペプチド又はタンパク質内の前記修飾タンパク質エピトープに、好ましくは、シトルリン、ホモシトルリン、又はアセチル化リシンを含む前記修飾タンパク質エピトープに結合したか否かを判定すること、
を含む、方法。
前記サンプルは、サンプリング時に関節リウマチを有しない個体のサンプルである、付記18に記載の方法。
AMPAが、好ましくは、シトルリン化タンパク質抗原抗体(ACPA)、リシンアセチル化タンパク質抗原抗体(AAPA)、及び/又は抗CarP抗体が、前記個体の類似早期サンプルで検出されたことを特徴とする、付記18又は19に記載の方法。
前記早期サンプル内の前記AMPAは、前記抗体のFab部分上のN結合型グリコシル化の存在の検査で陰性であった、付記20に記載の方法。
前記個体は、関節炎、好ましくは、関節リウマチを発症する危険性がある、付記18から21のいずれか1つに記載の方法。
糖鎖結合分子が、レクチン、好ましくは、シアル酸残基結合レクチン、好ましくは、SIGLECファミリーの一員、好ましくは、CD22又は前記レクチンのシアル酸結合部分である、付記18から22のいずれか1つに記載の方法。
前記レクチンは、2箇所のシアル酸残基、好ましくは、セイヨウニワトコ凝集素(SNA)に同時に結合し得る、付記23に記載の方法。
サンプル中での、関節リウマチ、シェーグレン症候群、又はAAVに関連する自己抗体、AMPAなど、の検出に有用なパーツのキットであって、前記キットは、前記自己抗体に結合し得るペプチド又はタンパク質、翻訳後修飾を含むペプチド又はタンパク質など、と、抗体のFab部分上のN結合型糖鎖に結合し得る分子とを含む、キット。
前記ペプチド、前記タンパク質、又は、N結合型糖鎖に結合し得る前記分子は、固体表面に結合している、付記25に記載のパーツのキット。
個体が、Fab部分上にN結合型糖鎖を有する、関節リウマチ、シェーグレン症候群、又はAAVに関連する自己抗体を含むか否かを判定する方法であって、前記方法は、
前記自己抗体に結合し得るペプチド又はタンパク質に前記個体のサンプルを含むB細胞を接触させることと、
前記ペプチド又はタンパク質に結合したB細胞を未結合B細胞から分離すること、
少なくとも、結合B細胞のB細胞受容体の、重鎖可変領域のCDR1及び/又は軽鎖可変領域のCDR1をコードする核酸をシーケンシングすること、及び、
前記核酸配列がN結合型グリコシル化コンセンサスアミノ酸配列をコードしているか否かを判定すること、
を含む、方法。
少なくとも重鎖可変領域の複数のCDR及び/又は軽鎖可変領域の複数のCDRをシーケンシングすることを含む、付記27に記載の方法。
Claims (24)
- 関節リウマチの危険性を評価するために個体の抗体含有サンプルを分析する方法であって、
前記方法は、前記抗体含有サンプル内の抗シトルリン化タンパク質抗体(ACPA)-IgGのFabグリコシル化の割合を決定することを備え、
前記抗体含有サンプルは、サンプリング時に、関節リウマチと診断されていない個体のサンプルであり、
前記個体は、前記ACPA-IgGのFabグリコシル化の割合が高いときに、関節リウマチを発症する危険性が高いと分類されることを特徴とする、方法。 - 関節リウマチの危険性を評価することを支援するために個体の抗体含有サンプルを分析する方法であって、
前記方法は、前記抗体含有サンプル内の抗シトルリン化タンパク質抗体(ACPA)-IgGのFabグリコシル化の割合を決定することを備え、
前記抗体含有サンプルは、サンプリング時に、関節リウマチと診断されていない個体のサンプルであり、
以上により、関節リウマチの危険性を評価することを支援することを特徴とする、方法。 - 前記ACPA-IgGのFabグリコシル化の割合が高いか否かを判定することを含む、請求項2に記載の方法。
- 前記抗体含有サンプルは、血液サンプル、血清サンプル、血漿サンプル、滑液サンプル、又は唾液サンプルである、請求項1から3のいずれか1項に記載の方法。
- N結合型糖鎖は、H5N4S2、H5N5F1S1、H5N4F1S2、H5N5S2、H5N5F1S2、及びH6N5F1S2から選択される、請求項1から4のいずれか1項に記載の方法。
- 前記抗体含有サンプルは、自己抗体についての早期抗体サンプルの検査が陽性であった個体の抗体含有サンプルである、請求項1から5のいずれか1項に記載の方法。
- 前記早期抗体サンプルの自己抗体の10%以下がそのFab部分上にN結合型糖鎖を含む、請求項6に記載の方法。
- 前記抗体は、Fab及びFc断片を生産するために、切断される、請求項1から7のいずれか1項に記載の方法。
- 前記抗体含有サンプルがACPAを含むか否かを判定する工程を備え、
当該工程が、シトルリン化タンパク質抗原に前記抗体含有サンプルを接触させることを含む、請求項1から8のいずれか1項に記載の方法。 - 前記方法が、レクチンを含む糖鎖結合分子に前記抗体含有サンプルを接触させること、及び、前記ACPAへの前記糖鎖結合分子の結合を判定すること、を含む、請求項1から9のいずれか1項に記載の方法。
- 前記糖鎖結合分子は、シアル酸残基結合レクチン、好ましくは、SIGLECファミリーの一員、好ましくは、CD22又は前記レクチンのシアル酸結合部分を含む、請求項10に記載の方法。
- 前記レクチンは、2箇所のシアル酸残基、好ましくは、セイヨウニワトコ凝集素(SNA)に同時に結合し得る、請求項11に記載の方法。
- 前記個体は、前記ACPA-IgGのFabグリコシル化の割合が基準より高いときに、関節リウマチを発症する危険性が高いと分類される、請求項1から12のいずれか1項に記載の方法。
- 前記個体は、前記抗体含有サンプル内のACPAの、10%より多く、20%より多く、30%より多く、50%より多く、又は55%より多くが、当該抗体のFab部分にN結合型グリコシル化を含むときに、関節リウマチを発症する危険性が高いと分類される、請求項1から13のいずれか1項に記載の方法。
- 関節リウマチの危険性を評価するために個体の抗体含有サンプルを分析するために使用するための組成物であって、
前記抗体含有サンプルは、サンプリング時に、関節リウマチと診断されていない個体のサンプルであり、
前記抗体含有サンプル内の抗シトルリン化タンパク質抗体(ACPA)-IgGのFabグリコシル化の割合が決定されることを特徴とする、組成物。 - 前記個体は、前記ACPA-IgGのFabグリコシル化の割合が高いときに、関節リウマチを発症する危険性が高いと分類される、請求項15に記載の組成物。
- 前記個体は、前記ACPA-IgGのFabグリコシル化の割合が基準より高いときに、関節リウマチを発症する危険性が高いと分類される、請求項16に記載の組成物。
- 前記個体は、前記抗体含有サンプル内のACPAの、10%より多く、20%より多く、30%より多く、50%より多く、又は55%より多くが、当該抗体のFab部分にN結合型グリコシル化を含むときに、関節リウマチを発症する危険性が高いと分類される、請求項15から17のいずれか1項に記載の組成物。
- レクチンを含む糖鎖結合分子をさらに含む、請求項15から18のいずれか1項に記載の組成物。
- 前記ACPAへの前記糖鎖結合分子の結合が決定される、請求項19に記載の組成物。
- 前記糖鎖結合分子は、シアル酸残基結合レクチン、好ましくは、SIGLECファミリーの一員、好ましくは、CD22又は前記レクチンのシアル酸結合部分を含む、請求項20に記載の組成物。
- 前記レクチンは、2箇所のシアル酸残基、好ましくは、セイヨウニワトコ凝集素(SNA)に同時に結合し得る、請求項21に記載の組成物。
- サンプル中で関節リウマチに関連する自己抗体の検出に有用なパーツのキットであって、
前記キットは、翻訳後修飾を含むペプチド又はタンパク質であって前記自己抗体に結合し得るペプチド又はタンパク質と、抗体のFab部分上のN結合型糖鎖に結合し得る分子とを含み、
前記抗体は、抗シトルリン化タンパク質抗体(ACPA)であり、前記自己抗体に結合し得る前記ペプチド又は前記タンパク質は、シトルリン化タンパク質抗原を含む、キット。 - 前記ペプチド、前記タンパク質、又は、N結合型糖鎖に結合し得る前記分子は、固体表面に結合している、請求項23に記載のキット。
