Abstract
Description
本発明者らは、髄膜炎B型ワクチンBEXSERO(登録商標)に使用されるものを含む、ワクチン抗原が、淋菌(NG)に対する殺細菌性抗体を誘導することを見出した(実施例1~7及び9を参照のこと)。
(i)NHBA抗原;
(ii)fHbp抗原;
(iii)NadA抗原;
(iv)GNA1030抗原;
(v)GNA2091抗原;
(vi)髄膜炎菌血清群A、C、W135、及び/又はYのうちの1つ以上に由来するコンジュゲート化莢膜糖と組み合わせた上記(i)~(v)のうちの1つ以上
のうちの1つ以上を含む免疫原性組成物を投与することによって、淋菌に対する免疫化を必要とする被験体を、淋菌に対して免疫化する方法を提供する。
(i)NHBA抗原;

本発明は、免疫原性組成物のレシピエントが、淋菌細菌による感染及び/又は疾患に対する防御を提供する免疫反応を開始するように、淋菌によって引き起こされる感染及び/又は疾患(例えば、淋病及び関連合併症、例えば、骨盤炎症性疾患、及び淋菌による無症候性感染)に対して被験体を免疫化するために使用される。
本発明は、淋菌に対して被験体を防御するために免疫原性組成物(例えば、ワクチン)を使用する。上記組成物は、免疫原性量の、上に提供された抗原又は抗原の組み合わせの少なくとも1つを含む。
NHBAは、遺伝子NMB2132(GenBankアクセション番号GI:7227388; 本明細書中で配列番号4)として、髄膜炎菌血清群B株MC58についての公表されたゲノム配列に含まれた[22]。それ以後、多くの株に由来するNHBAの配列が公表されている。例えば、NHBAのアレル型(タンパク質「287」と称される)は、参考文献23の図5及び15、並びに参考文献24の実施例13及び図21(その中の配列番号3179~3184)に見ることができる。NHBAの様々な免疫原性フラグメントも報告されている。このタンパク質は、ヘパリン結合タンパク質であると確認され、参考文献25においてNHBAと命名された。
NadA抗原は、遺伝子NMB1994(GenBankアクセション番号GI:7227256; 本明細書中で配列番号5)として、髄膜炎菌血清群B株MC58についての公表されたゲノム配列に含まれた[22]。それ以後、多くの株に由来するNadA抗原の配列が公表されており、ナイセリアアドヘシンとしてのタンパク質の活性は十分に実証されている。NadAの様々な免疫原性フラグメントも報告されている。このタンパク質は、アドヘシンであると確認され、参考文献26においてNadAと命名された。
fHbp抗原は、詳細に特徴付けられている。それはまた、タンパク質「741」(参考文献24における配列番号2535及び2536)、「NMB1870」、「GNA1870」[27~29]、「P2086」、「LP2086」、又は「ORF2086」[30~32]としても知られている。それは、多くの髄膜炎菌血清群にわたって発現され、これらにおいて、それはリポタンパク質である。fHbpのC末端免疫優性ドメイン(「fHbpC」)の構造は、NMRによって決定されている[33]。このタンパク質のこの部分は、8本鎖のβ-バレルを形成し、それらの鎖は、可変長のループによって連結されている。バレルの前には、短いα-ヘリックス及び柔軟なN末端尾部がある。このタンパク質は、H因子結合タンパク質であると確認され、参考文献34においてfHbpと命名された。fHbp抗原はまた、淋菌に存在し、髄膜炎菌fHbpと淋菌fHbpの主な違いは、N末端にある[10]。淋菌において、髄膜炎菌fHbpのホモログは、Ghfpとしても知られている。

髄膜炎菌血清群Bに由来する「GNA1030」タンパク質は、参考文献24において「953」として(その中の配列番号2917及び2918)、及び参考文献22において「NMB1030」として開示される(GenBankアクセション番号GI:7226269も参照のこと)。血清群A[38]における対応するタンパク質は、GenBankアクセション番号7380108を有する。
髄膜炎菌血清群Bに由来する「GNA2091」タンパク質は、参考文献24においてタンパク質936として(配列番号2883及び2884)、及び参考文献22において「NMB2091」として開示される(GenBankアクセション番号GI:7227353も参照のこと)。血清群A[38]における対応する遺伝子は、GenBankアクセション番号7379093を有する。
いくつかの態様及び実施形態では、本発明の免疫原性組成物は、ナイセリア(髄膜炎菌又は淋菌)OMVを含む。任意選択で、そのようなOMVを含有する実施形態では、組成物は、例えば可溶性形態で、OMV成分から分離された上に挙げた抗原(すなわち、NHBA、NadA、GNA1030、又はGNA2091)の少なくとも1つを含む。したがって、これらの組成物は、OMVを1つ以上の可溶性抗原と混合することによって調製され、これは、参考文献42及び43で採用された手法とは対照をなす。

組成物は、1つ以上のさらなる髄膜炎菌タンパク質抗原、例えば、HmbR、NspA、NhhA、App、Omp85、TbpA、TbpB、及び/又はCu,Zn-スーパーオキシドジスムターゼを含むことができる。
組成物は、1つ以上の非髄膜炎菌抗原(非髄膜炎菌抗原は、上に論じられる淋菌莢膜糖ではない)を含むことができる。例えば、組成物は、以下のうちの1つ以上を含むことができる: (a)PREVNAR及びSYNFLORIX製品におけるように、糖(典型的にはコンジュゲート化)などの、肺炎球菌(Streptococcus pneumoniae)に由来する抗原; (b)表面抗原HBsAgなどの、B型肝炎ウイルスに由来する抗原; (c)百日咳菌に由来する百日咳ホロトキシン(PT)及び線維状赤血球凝集素(FHA)などの、百日咳菌(Bordetella pertussis)に由来する抗原(任意選択でまた、パータクチン並びに/又は凝集原2及び3と組み合わせて); (d)ジフテリアトキソイドなどの、ジフテリア抗原; (e)破傷風トキソイドなどの、破傷風抗原; (f)インフルエンザ菌B(Hib)に由来する糖抗原、典型的にはコンジュゲート化されたもの; 及び/又は(g)不活性化ポリオウイルス抗原。
その抗原に加えて、本発明の免疫原性組成物は、典型的には、薬学的に許容可能な担体を含み、そのような担体の徹底的な議論は、参考文献121で利用可能である。
本発明における使用のための組成物は、好ましくは、少なくとも10%、例えば、≧20%、≧30%、≧40%、≧50%、≧60%、≧70%、≧80%、≧85%、≧90%、又はそれを超える、淋菌に対するワクチン効力を有する。参考文献15に説明されるように、淋病に対するワクチン接種は、淋菌に対する20%という低いワクチン効力(淋菌の抗生物質耐性株における実質的な増加を有する場合は10%)においてさえも費用対効果が高い可能性がある。
本発明の組成物は、一般に、被験体に直接投与される。直接的送達は、非経口注射によって(例えば、皮下、腹腔内、静脈内、筋肉内に、又は組織の間質空間へ)、又は他の任意の適切な経路によって達成してよい。筋肉内投与(例えば、大腿部又は上腕へ)が、好ましい。注射は針(例えば、皮下注射針)を介してもよいが、無針注射を代わりに使用してもよい。典型的な筋肉内投与体積は、0.5mlである。
本発明の実施は、他に示されない限り、当技術分野の技能の範囲内の化学、生化学、分子生物学、免疫学、及び薬理学の従来の方法を使用する。そのような技術は、文献に十分に説明されている。例えば、参考文献124~130などを参照のこと。
以下の実施例において、287(NHBA)への言及は全て、ΔGバリアントを指す。
動物及び免疫化プロトコル
6週齢の雌性CD1マウスを、1、21、及び35日目に腹腔内に20μgのタンパク質で免疫化した。抗原を、Al(OH)3に吸着させるか、又は1回目の用量では完全フロイントアジュバント、2回目及び3回目のブースター用量では不完全フロイントアジュバントと一緒に与えた。血液サンプルを、最後の用量の2週間後に収集し、血清学的分析に使用した。
淋菌F62株を、O.D.600=0.1からO.D.600=0.3まで1%アイソバイタルエックス(isovitalex)を補充したGC培地中で増殖させた。遠心分離後、細菌を、1mMのCaCl2及び0.15mMのMgCl2を加えたハンクス平衡塩類溶液(HBSS++)に再懸濁した。100μl/ウェルの細菌を、96ウェルプレートに播種し、遠心分離し、HBSS++に希釈したマウス血清を用いて再懸濁し、37℃で15~30分間インキュベートした。
淋菌F62株を、O.D.600=0.1からO.D.600=0.3まで1.5時間、1%アイソバイタルエックスを補充したGC培地中で増殖させた。細菌を希釈してO.D.600=0.1を得て、再度1:10に希釈し、以下の反応混合物と共に37℃で1時間インキュベートした: 125μlの希釈した細菌、25μlの淋菌吸着ヒト血清(外因性補体源として)、25μlの所望濃度の血清、及び75μlの5%BSA(HBSS中)。反応混合物を希釈し、細菌を、時間0、及び37℃で1時間のインキュベーション後に、GC+1%アイソバイタルエックスプレート上にプレーティングした。
6週齢の雌性CD1マウス(10匹の動物/群)を、1、21、及び35日目に、腹腔内に、アラムアジュバントと組み合わせた、BEXSERO(登録商標)(20μgのタンパク質抗原及び10μgのOMVに相当する1:2.5ヒト用量)、又はOMVnz(10μg)、又は組み換えMenBタンパク質抗原(rMenB)(各20μg)で免疫化した。アジュバント単独及び無関係の抗原(呼吸器合胞体ウイルス(RSV)に由来するタンパク質F)を、陰性対照として使用した。血清サンプルを、最初の免疫化の前及び最後の用量の2週間後に収集し、血清学的分析に使用した。
淋菌株FA1090を、およそ0.1のO.D.600からおよそ0.3のO.D.600まで1.5時間、1%アイソバイタルエックスを補充したGC培地中で増殖させた。細菌を、SBA緩衝液(dPBS、0.1%グルコース、1%BSA)中の血清の懸濁液で希釈し、外因性補体源としてのヒト血清(16%v/v)の存在下で、試験されるべき血清と共に37℃で1時間インキュベートした。次いで、細菌を、GC+1%アイソバイタルエックスプレート上にプレーティングした。
図4から分かるように、それぞれにおいて、示された3つの免疫化スキームが、BEXSERO(登録商標)、OMVnz、及びrMenBのそれぞれに対して生じた血清の淋菌に対する殺細菌活性を示す場合、BEXSERO(登録商標)、OMVnz、及びrMenBで免疫化したマウスに由来する血清プールのhSBA力価は、高い(≧16)。対照的に、アジュバント単独の力価は低く(2つの異なる免疫化スキームにおいて≦8)、殺細菌活性について統計学的に意味のある(すなわち、非バックグラウンド)力価についての閾値レベルに達しない。RSVについて測定されたhSBA力価は、試験された最低血清希釈度である4未満であり、グラフには示されていない。
hSBAアッセイにおいて観察された結果は、淋菌に対するBEXSERO(登録商標)、OMVnz、及びrMenBの免疫原性活性のさらなる間接的証拠を提供するための競合的SBAアッセイを実施することによって補強された。
6週齢の雌性CD1マウス(10匹の動物/群)を、1、21、及び35日目に腹腔内に、アラムアジュバントと組み合わせた20μgのタンパク質(287-953又は287)で免疫化した。無関係の抗原である呼吸器合胞体ウイルス(RSV)に由来するタンパク質Fを、陰性対照として使用した。血清サンプルを、最初の免疫化の前及び最後の用量の2週間後に収集し、血清学的分析に使用した。
淋菌株FA1090を、O.D.600=0.1からO.D.600=0.3まで1.5時間、1%アイソバイタルエックスを補充したGC培地中で増殖させた。細菌を、SBA緩衝液(dPBS、0.1%グルコース、1%BSA)中の血清の懸濁液で希釈し、ウサギ補体(5%v/v)の存在下で、試験されるべき血清と共に37℃で1時間インキュベートした。次いで、細菌を、GC+1%アイソバイタルエックスプレート上にプレーティングした。
淋菌株FA1090を、およそ0.1のO.D.600からおよそ0.3のO.D.600まで1.5時間、1%アイソバイタルエックスを補充したGC培地中で増殖させた。細菌を、SBA緩衝液(dPBS、0.1%グルコース、1%BSA)中の血清の懸濁液で希釈し、外因性補体源としてのヒト血清(16%v/v)の存在下で、試験されるべき血清と共に37℃で1時間インキュベートした。次いで、細菌を、GC+1%アイソバイタルエックスプレート上にプレーティングした。
図11から分かるように、hSBAアッセイにおいて、抗953免疫血清は、抗体力価>8を有する、淋菌株FA1090に対する殺細菌性抗体を誘導することができた。対照的に、アジュバント単独で免疫化した血清も、免疫前血清のいずれも、FA1090に対する殺細菌性抗体の統計学的に意味のある力価を誘導することができなかった。
hSBAアッセイにおいて観察された結果は、淋菌に対する936付属タンパク質の免疫原性活性のさらなる間接的証拠を提供するための競合的SBAアッセイを実施することによって補強された。

図14から分かるように、抗BEXSERO(登録商標)及び抗MenABCWY血清の両方についてのhSBAアッセイにおいて、同等の結果が見られる。両方のワクチンは、高いSBA力価(16~32)をもたらし、これは、淋菌に対する強い殺細菌性抗体反応を示す。グラフ中の各点は、個々のウサギに由来する免疫血清についての結果を表し、これらのデータは、両方のワクチンが、淋菌に対して均質な殺細菌反応を誘導することができ、個々の結果の大部分が32のSBA力価を生じることを示す。

図15から分かるように、免疫前血清(すなわち、dOMV FA1090で免疫化する前にマウスから収集した血清)又はRSV免疫血清について、hSBA結果は観察されなかった。対照的に、dOMV FA1019で免疫化したマウスに由来する免疫血清(すなわち、免疫化後にマウスから収集した血清)を用いたhSBA力価は、陽性であり、≧1024の極めて高い力価を生じた。
SBAアッセイにおいて観察された結果は、淋菌に対する淋菌OMVの免疫原性活性のさらなる間接的証拠を提供するための競合的SBAアッセイを実施することによって補強された。
CD1マウス(5匹の動物/群)を、1、22、及び36日目に、(i)BEXSERO(登録商標)(各タンパク質抗原について20μg及びOMVについて10μgに相当する1:2.5ヒト用量)、(ii)OMVnz(10μg)、(iii)OMVFA1090(10μg)、又は(iv)Al(OH)3アジュバント単独、のいずれかでワクチン接種した。
図17に示されるように、BEXSERO(登録商標)ワクチン及びOMVnzワクチンは、淋菌抗原OMV FA1090に対する交差反応性CD4+ T細胞反応を誘導することができた。OMV FA1090ベースのワクチンはまた、同種淋菌抗原OMV FA1090に対するCD4+ T細胞反応を誘導することができた。対照的に、これらのワクチンはどれも、大腸菌OMV抗原に対するT細胞反応を誘導することができず、これは、T細胞反応がワクチン特異的であることを実証する。同様に、アジュバント単独で免疫化した集団において、淋菌又は大腸菌OMV抗原のいずれに対しても顕著なCD4+ T細胞反応は生じなかった。
TNF及びIL-2は、Th0プロファイルに関連している。
IL-17は、Th17プロファイルに関連している。
IL-4及びIL-13は、Th2プロファイルに関連している。
IFN-γは、Th1プロファイルに関連している。

[3] 淋菌に対する免疫化を必要とする被験体を淋菌に対して免疫化するための医薬の製造における、以下:
(i)NHBA抗原;

[15] 前記組成物が、配列番号11に対して少なくとも85%の同一性を有するアミノ酸配列を含むfHbp抗原を含む、上記[6]~[9]のいずれかに記載の方法、免疫原性組成物、又は使用。

Claims (37)
- 以下:

- 前記組成物が、筋肉内注射によって被験体に投与される、請求項1~30のいずれか一項に記載の方法、免疫原性組成物、又は使用。
- 前記組成物の少なくとも3回の用量が、被験体に投与される、請求項1~31のいずれか一項に記載の方法、免疫原性組成物、又は使用。
- 被験体が、若者及び/又は大人である、請求項1~32のいずれか一項に記載の方法、免疫原性組成物、又は使用。
- 被験体は、淋菌による感染の危険性が増加している、請求項1~33のいずれか一項に記載の方法、免疫原性組成物、又は使用。
- 被験体が、髄膜炎菌に対して以前に免疫化されており、免疫原性組成物が、淋菌に対して防御するためのブースターとして投与される、請求項33に記載の方法、免疫原性組成物、又は使用。
