Abstract
Description

本発明は、米国国立衛生研究所により与えられたEY017392及び国防総省により与えられたW81XWH-14-1-0496の下で政府の支援を受けて行われた。政府は本発明に関して一定の権利を有する。
本発明は、Itga-9に特異的に結合する精製された結合剤(またはリガンド)、すなわち、「抗Itga-9治療剤」を提供する。抗Itga-9治療剤は、抗体でもよいし、小分子薬剤でもよい。抗Itga-9治療剤である薬剤の例としては、モノクローナル抗体またはポリクローナル抗体、ペプチド、小分子、低分子干渉RNAなどが挙げられる。
本明細書で使用する場合、用語「抗体」は、scFv、ヒト化、完全ヒト化またはキメラ化抗体、単鎖抗体、ダイアボディ、及びFc領域を含まない抗体の抗原結合性断片(例えば、Fab断片)を含む。ある特定の実施形態では、抗体はヒト抗体またはヒト化抗体である。「ヒト化」抗体は、3つのCDR(相補性決定領域)のみを含み、ヒト抗体配列の対応するフレームワーク及び定常領域に組換えで連結された、各ドナー抗体可変領域由来の少数の慎重に選択された「フレームワーク」残基(可変領域の非CDR部分)を含むこともある。「完全ヒト化抗体」は、ヒト由来の抗体遺伝子のみを有するように遺伝子操作されているマウス由来のハイブリドーマ中で、またはヒト由来の抗体遺伝子のファージディスプレイライブラリーからの選択によって、作出される。

要約
目的。リンパ経路は移植拒絶反応を媒介する。本発明者らは、最近、リンパ管はリンパ脈管新生の間に角膜中に管腔弁を発生し、これらの弁はインテグリンアルファ9(Itga-9)を発現し、リンパ流動の方向付けにおいて重要な役割を果たすことを報告した。本研究では、本発明者らは、同種異系角膜移植モデルを使用して、Itga-9の遮断が移植後に弁形成を抑制することができるかどうか、及びこの効果が移植片のアウトカムにどのように影響するかを調べた。

動物

以前に報告されているように(Zhang H,Grimaldo S,Yuen D,Chen L.Combined blockade of VEGFR-3 and VLA-1 markedly promotes high-risk corneal transplant survival.Investigative ophthalmology & visual science 2011;52:6529-6535、Kang GJ,Ecoiffier T,Truong T,et al.Intravital Imaging Reveals Dynamics of Lymphangiogenesis and Valvulogenesis.Scientific reports 2016;6:19459)、完全にミスマッチしたC57BL/6(ドナー)とBALB/c(レシピエント)の間で同所性角膜移植を行った。基本的に、2mm径のマイクロキュレット(Katena Products Inc.、Denville、NJ)でドナーの中央角膜に印をつけ、バナス剪刀(Storz Instruments Co、San Dimas、CA)でこれを切除した。1.5mm径のマイクロキュレットを用いて、レシピエントの移植床を同様に調製し、8つの断続11-0ナイロン縫合糸(Sharpoint;Vanguard)でドナーボタンをレシピエント床中に固定した。抗菌性軟膏剤を手術の終わりに塗布した。
以前に報告されているように11、レシピエントマウスを無作為化して、ハムスターもしくはマウスのItga-9抗体(6.4μg;Dr.Toshimitsu Uede、Hokkaido Universityの好意により提供された)またはそのアイソタイプ対照のハムスターIgG(Jackson ImmunoResearch、West Grove、PA、USA)のいずれかで結膜下注射を行った。移植当日及びその後手術してから最大で8週まで、結膜下注射を週2回行った。
移植手術後、3日目にすべての眼を最初に調べ、7日目に角膜縫合糸を除去した。標準的なスキームに従って、眼部細隙灯顕微鏡検査によって8週間にわたって週2回移植片を評価した。基本的に、移植片の不透明化の程度は、0(透明及びコンパクトな移植片)~5+(前眼房の全体的な不明瞭化をともなう最大混濁)の間で等級付けした。3週後に2+またはそれ以上の混濁スコアを、または2週目に3+またはそれ以上の混濁スコアを有する移植片を拒絶されたとみなした。

角膜移植後のリンパの弁形成に対するItga-9遮断の効果
移植によって誘導される角膜のLG及びVGに対するItga-9遮断の効果を最初に研究した。Itga-9中和体またはアイソタイプ対照のいずれかを、手術日から開始して週2回結膜下注射した。図1Aで示すように、Itga-9遮断抗体による処置の後に、角膜リンパ管は、対照条件と比較して有意に少ない弁を含んでいた。反復実験からの要約データを図1Bに示す(左パネル;P<0.05)。しかし、図1B(右パネル)に示すように、この処置はLGには効果がなかった。弁の数量対リンパ浸潤領域の比に関するさらなる解析によって、対照条件と比較して、処理したものにおいて、このパラメーターの有意な低減が明らかになった(図1C;P<0.05)。

ophthalmology & visual science 2014;55:1876-1883、Ecoiffier T,Yuen D,Chen L.Differential distribution of blood and lymphatic vessels in the murine cornea.Investigative ophthalmology & visual science 2010;51:2436-2440)。この現象が移植関連LGにも顕在化するかどうか、及びこれがItga-9処置の影響を受けるかどうかを調べるために、図2Aに例示するように、鼻及び側頭の4分円を比較することによって、LGの極性に対するItga-9遮断の効果を次に調べた。結果は、処置群及び対照群の両方において、リンパ管は鼻側により多く分布しており、Itga-9遮断は、角膜のLGのこの極性に対して効果がないことを示した(図2B及び図4)。R Studioプラットフォーム(R Studio Inc.、Boston、MA)を用いて構築した線形混合モデルを使用するさらなる関連解析もLGの極性分布は角膜領域のみと関係があり、抗Itga-9遮断とは関係がないことを確認した。
角膜移植片の生存に対するItga-9遮断の効果をさらに評価するために、処置群及び対照群の両方において移植片を調べ、手術してから最大で8週まで週2回それらの生存率を評価した。図3に示すように、結果は、この処置による、移植片の生存の有意な促進を示した。対照群と処置群の両方における移植片拒絶は、移植してから約2.5週後に始まったが、カプラン・マイヤー生存曲線(P<0.05)によって解析した場合に、8週間の研究の終わりまで、有意に高いパーセンテージの移植片が処置群において生存した。
本研究では、Itga-9は角膜移植誘導性VGに決定的に関与し、その分子遮断はこのプロセスを効果的に抑制できることを初めて示した。この治療ストラテジーは、角膜のLGまたは鼻分布のその極性に影響を及ぼさないことも示した。さらに重要なことは、脈管それ自体ではなくリンパ弁を低減させることによって、より高い移植片生存率を達成することが可能あったことを示す最初の証拠が提示されることである。

さらなる研究によって、ltga-9とVEGFR-3の複合遮断は、危険性の高い角膜移植の後にリンパ弁及び脈管形成を阻害すること(図6、緑色で示される脈管及び弁)及び移植片の生存を促進することが示された(図6B、カプラン・マイヤー生存曲線)。
目的。本発明者らは、最近、炎症性リンパ脈管新生の間に、角膜リンパ管がインテグリンアルファ-9(Itga-9)陽性弁を発生することを報告した。本研究の目的は、インビボでの角膜のリンパ弁形成におけるItga-9の役割及びインビトロでのリンパ管内皮細胞(LEC)の機能をさらに調べることである。
リンパ網はほとんどの組織に広がり、組織液恒常性及び免疫監視において重要な機能を果たす。炎症、がん転移及び移植拒絶反応を含めた幅広い疾患及び状態がリンパ機能障害と関係がある1~5。リンパ系と関係がある疾患の蔓延にもかかわらず、依然として、リンパ系障害に利用可能な治療はほとんどない。したがって、これは、新しい治療ストラテジーに対して差し迫った需要がある分野である。
動物

以前に述べられているように16、縫合糸設置モデルを使用して、弁形成をともなう角膜炎症性LGを誘導した。手短に言えば、3つの11-0ナイロン縫合糸(AROSurgical、Newport Beach、CA、USA)を、前眼房中に入り込むことなく角膜実質中に設置した。続いて、マウスを無作為化して、6.4lgのハムスター抗マウスItga-9遮断抗体18またはアイソタイプ対照ハムスターIgG(Jackson ImmunoResearch、West Grove、PA、USA)のいずれかを、縫合後0日目から始めて2週間にわたって週2回結膜下注射した。実験は、各群に合計で10匹のマウスを用いて2回反復した。
以前に報告されているように16、19、実験を行った。手短に言えば、免疫蛍光染色のために、ホールマウント角膜をアセトン中に固定した。精製されたウサギ-抗マウスLYVE-1(Abcam、Cambridge、MA、USA)及びヤギ-抗マウスItga-9抗体(R&D Systems、Minneapolis、MN、USA)とともに試料を順次インキュベートし、これは、それぞれ、FITCコンジュゲートロバ-抗ウサギ及びCy3-コンジュゲートロバ-抗ヤギ二次抗体(Jackson ImmunoResearch)によって可視化した。試料をVector Shieldマウント媒体(Vector Laboratories;Burlingame、CA、USA)でマウントし、ZENデジタルイメージングソフトウェア(Carl Zeiss,Inc.、(Germany)を有するZeiss LSM 710 AxioObserver倒立共焦点顕微鏡を使用して調べた。LYVE-1pリンパ管の長さに沿ったItga-9p限局的な領域を弁として特定した。角膜あたりのリンパ弁の数を定量化し、スコアが100%であると定義される対照群に対して正規化することによって、パーセンテージスコアを得た16、19。
以前に記載されているように13、19、実験を行った。手短に言えば、ヒト新生児マイクロダーマルLECをLonza(Walkersville、MD、USA)から購入し、製造者の指示書に従って、EGM-2MV細胞培養培地(Lonza)中で維持した。LECの染色については、細胞をウサギ抗ヒト-Itga-9抗体(Novus Biologicals、Littleton、CO、USA)とともにインキュベートし、Cy3-コンジュゲートロバ-抗ウサギ二次抗体(Jackson ImmunoResearch)によって可視化した。DAPIマウント媒体(Vectashield;Vector Laboratories)で試料をマウントした。Zeiss Axioplan2落射蛍光顕微鏡(Carl Zeiss、Inc.)でデジタル画像を得た。
以前に報告されているように13、19、実験を行った。ヒトItga-9mRNA(50-AAG AAG AAA GTC GTA CTA TAG-30)に対して、特注設計のsiRNA二本鎖をQiagen(Valencia、CA、USA)が合成した。スクランブルsiRNA対照は、Ambion(Austin、TX、USA)から購入した。トランスフェクションは、製造者の指示書に従って、トランスフェクション試薬(RNAiMax;Invitrogen、Carlsbad、CA、USA)及びopti-MEM血清低減培地を用いて、5%CO2の加湿したエアーインキュベーターにおいて378Cで行った。
前述のように、及び定量的リアルタイムPCR実験(MIQE)ガイドラインの刊行物についての最小限の情報に基づいて13、19、20、実験を行った。siRNAトランスフェクションから48時間後に、QiagenのRNAeasyミニキットを用いて、LECから全RNAを抽出し、精製した。InvitrogenのSuperScript VILO cDNA合成キットを使用して、逆転写を行った。プライマー配列は以下通りであった:ヒトItga-9、フォワード50-CGG AAT CAT GTC TCC AAC CT-30及び50-TCT CTG CAC CAC CAG ATG AG-30;ヒトb-アクチン、フォワード 50-GAT CTG GCA CCA CAC CTT CT-30、リバース50-GGG GTG TTG AAG GTC TCA AA-3.0。CFX96配列検出システムを用いるSsoFast EvaGreen(Bio-Rad Laboratories,Inc.、Hercules、CA、USA)を使用して、リアルタイムPCRを行った。アクチンのD-Ctに対して正規化した標的遺伝子のD-Ct(閾値サイクル)からItga-9遺伝子の相対的発現を計算した。
LECのトランスフェクションから48時間後に、Guava ViaCountアッセイ(Millipore、Billerica、MA、USA)によってLECの生存率を評価した。Guava easyCyte HTサイトメーター(Millipore)及びInCyte2.6ソフトウェア(Millipore)を使用して、データを得た。Guava ViaCount試薬中の2つのDNA結合色素の差次的浸透性によって、生存細胞及び非生存細胞を評価した。一方の色素はすべての有核細胞を染色し(赤色蛍光チャネル)、一方で、他方の色素は非生細胞を染色する(黄色蛍光チャネル)。生細胞をゲートし(R1)、全集団に対するパーセンテージを計算した。実験は3回反復し、スコアが100%であると定義される対照条件に対してパーセンテージスコアを正規化した。
以前に記載されているように13、19、96ウェルプレート中にLECを播種した。Itga-9siRNAまたはスクランブルsiRNAのいずれかによるsiRNAトランスフェクションから48時間後に、製造業者のプロトコールに従って、Promega(Madison、WI、USA)のMTS増殖アッセイに細胞をかけた。アッセイは3連で行い、3回反復した。
以前に記載されているように13、Itga-9siRNAまたはスクランブルsiRNAのいずれかによるsiRNAトランスフェクションから48時間後に、フィブロネクチンでコーティングされた96ウェルプレート中にLECを播種した。プレートを378Cで1時間インキュベートし、2回洗浄し、HBSS中のカルセイン(1lg/mL)とともに室温で30分間インキュベートした。プレートをPBSで洗浄し、マイクロプレートリーダー(Spectramax M5e;Molecular Devices、Sunnyvale、CA、USA)で蛍光強度を測定した。アッセイは3連で行い、3回反復した。
Itga-9siRNAまたはスクランブルsiRNAのいずれかによるsiRNAトランスフェクションから48時間後に、10-lLのピペットチップを使用して、LEC単層内に直線的な傷をつけた。かき傷をつけてから0、24及び72時間後に微分干渉(DIC)位相画像を得て、細胞単層における創傷閉鎖を可視化した。TScratchprogram(Tobias Geback and Martin Schulz、ETH Zurich)を使用して、創傷治癒についてかき傷を解析した。創傷閉鎖の間の細胞遊走の可視化のために、TRITCコンジュゲート化ファロイジン(Millipore、Temecula、CA、USA)を使用して細胞を染色した。
以前に記載されているように13、19、Itga-9siRNAまたはスクランブルsiRNAのいずれかによるsiRNAトランスフェクションから48時間後に、固化したMatrigel(BD Biosciences、San Jose、CA、USA)を含む96ウェルプレート中にLECを播種した(2 3 104細胞/ウェル)。Zeiss Axio Observer A1倒立顕微鏡(Carl Zeiss,Inc.)を使用して、播種後24時間で管形成を画像化した。管の位相画像を得(Qcapture;Qimaging、Surrey、BC、Canada)、ImageJソフトウェア(http://ImageJ.nih.gov/ij/;米国国立衛生研究所、Bethesda、MD、USAによるパブリックドメインで提供される)を使用して、ウェルあたりの管の全長を計算した。アッセイは3連で行い、少なくとも3回反復した。
結果は平均6SEMとして報告し、Prismソフトウェア(GraphPad、La
Jolla、CA、USA)を使用して、異なる群の間の有意性レベルを決定するために、スチューデントのt検定を使用した。P<0.05である場合に、差異を統計学的に有意であるとみなした。
インビボにおける角膜のリンパ弁形成に対するItga-9遮断の効果
本発明者らは、最初に、炎症性LGにおける角膜のリンパ弁形成に対するItag-9遮断の効果を研究することに着手した。標準的な縫合糸設置モデルを使用して、LGをともなう炎症性角膜に形成される弁の数に対するItga-9中和抗体の結膜下送達の効果を評価した。図7Aに示すように、Itga-9遮断抗体による処置の後、角膜リンパ管は、対照条件と比較して有意に少ない弁を含んでいた。反復実験からの要約データを図7Bに示す(*P<0.05)。
リンパ弁は内皮細胞の2つの層に挟まれた細胞外マトリックスでできている。インビトロでリンパ管内皮細胞におけるItga-9の特定の役割をさらに調べるために、本発明者らは、次にヒトLEC培養系を用いた。図8Aに示すように、本発明者らは、最初に、免疫細胞蛍光顕微鏡解析によって、これらの細胞におけるItga-9の発現を確認した。次いで、siRNA媒介性遺伝子サイレンシングアプローチによって、これらのLECにおけるItga-9発現がダウンレギュレートされ得るかどうかを評価した(図8B)。リアルタイムPCR解析からの本発明者らのデータは、Itga-9siRNAによるトランスフェクションの後に、LECにおけるItga-9発現が約50%有意に低減することを示した。図9に示すように、Guava ViaCountアッセイを用いるフローサイトメトリー解析によって、トランスフェクションはLECの生存率に直接効果がないことも確認した。まとめると、これらのデータは、siRNAによるトランスフェクションは、インビトロにおいてLECにおけるItga-9の機能的役割を研究するのに使用することができることを示す。
本発明者らは、次に、siRNAアプローチを使用して、増殖に関するLEC機能におけるItga-9の役割をより深く探究し、評価した。Itga-9siRNAまたはスクランブルsiRNAのいずれかによるsiRNAトランスフェクションから48時間に、以前に報告されているように19、LECをMTS増殖アッセイにかけた。図10Aに示すように、本発明者らのデータは、対照条件と比較して、Itga-9siRNA処理がLECの増殖の有意な低減をもたらすことを示した(*P<0.05)。この結果は、Itga-9の関与がLECの増殖を調節することを示唆する。LECの接着に対するItga-9の欠乏の効果。インテグリンは、細胞外タンパク質への細胞接着に必須である。Itga-9がLECの接着に重要であるかどうかを調べるために、Itga-9siRNAまたはスクランブルsiRNAで細胞をトランスフェクトし、Itga-9リガンドのフィブロネクチンでコーティングしたウェル上で接着アッセイにかけた。図10Bに示すように、LECのItga-9欠乏は、フィブロネクチンへの細胞接着の有意な低減をもたらし(*P<0.05)、これによって、Itga-9は細胞外マトリックスとのLECの相互作用にも極めて重要であることが示される。LECの遊走に対するItga-9の欠乏の効果。細胞遊走は、インテグリンと細胞外マトリックスタンパク質の間の相互作用に主に依存しているプロセスであり、本発明者らは、次に、創傷治癒のかき傷アッセイを使用して、LECの遊走に対するItga-9の重要性を調べた。Itga-9siRNAまたはスクランブルsiRNAのいずれかによるsiRNAトランスフェクションの後に、72時間にわたってLECをモニターした。図11Aに示すように、Itga-9siRNAでトランスフェクトしたLECは、創傷後24及び72時間という研究した両時点で、スクランブルsiRNAでトランスフェクトした対照細胞よりも大きな開放創領域を示した(*P<0.05)。さらに、創傷治癒に対するItga-9の阻害効果は、対照群の細胞が視野の大部分にわたって増殖したより遅い時点で、より大きかった(図11B)。これらの結果は、Itga-9がLECの遊走に必須であることを明示する。
本発明者らは、3次元培養系を使用して、毛細管型の管に組織化するLECの能力に対するItga-9の欠乏の効果も調べた。Itga-9siRNAまたはスクランブルsiRNAのいずれかでトランスフェクションしてから48時間後に、固化した基底膜マトリックス、MatrigelにLECを播種して、管形成を可能にさせた。このアッセイの結果は、図12Aに示すように、LECにおけるItga-9の欠乏が、管の全長の劇的な低減をもたらすことを示した。反復実験からの要約データを図12Bに示す(*P<0.05)。これらの結果は、マクロスケール構造体へのLECの組織化におけるItga-9の重要な役割を意味する。
本研究では、本発明者らは、インビボの動物モデルとインビトロのヒト細胞モデルの両方を使用して、角膜のリンパ弁形成及びLECにおけるItga-9の必須の役割を示した。本発明者らは、本研究から2つの相互依存的な結論を導き出すことができる。第1に、Itga-9遮断抗体を使用する実験を通して示すように、インビボにおけるリンパ弁形成はItga-9によって堅固に調節されている。第2に、siRNAノックダウンアプローチによって明らかにされたように、Itga-9はインビトロにおけるLECプロセス、例えば、増殖、遊走及び接着に決定的に関与している。インビボのデータは、炎症性角膜におけるリンパ弁形成を妨げるための新しいストラテジーを示唆する一方で、インビトロの結果は、LECの様々な機能におけるItga-9の役割の根底にあると考えられるメカニズムに関して、より多くの詳細を提供し、これらは、一緒に弁形成のプロセスを編成する。細胞外マトリックスへの細胞固定は、主としてインテグリンに基づく結合を介して媒介され、細胞内環境と細胞外環境の間に強い結び付きをもたらす11。インテグリンは、a及びbサブユニットに依存して、細胞外マトリックスタンパク質に特異性を有する。Itga-9に対する主なリガンドのうちの1つは、ヒトがん細胞モデルで報告されているように21、フィブロネクチンである。ヒトLECを用いる本研究では、Itga-9標的化siRNAを使用して、本発明者らは、Itga-9の欠乏の際に、フィブロネクチンへのLECの接着が有意に阻害されることを示すことができる。さらに、細胞遊走の中心にあるものは細胞-マトリックス接着複合体であり、これは、主として、細胞外マトリックスを細胞内アクチンに連結するインテグリンからなる22。LECの遊走におけるItga-9の役割を特定するために、本発明者らは、創傷治癒かき傷アッセイによって、細胞遊走に対するItga-9の消失の効果を調べた。本発明者らは、Itga-9を欠くLECは、時間の経過とともにかき傷表面にわたって完全に増殖した対照LECと比べると、かき傷のすべてにわたって決して十分に遊走しないことを示すことができる。増殖データとまとめると、細胞外マトリックスへのLECの結合に関連するプロセスにおけるItga-9の重要性は、疑いの余地がなくなる。これらの基本的なプロセスに関するさらなる研究は、治療介入のための新しい標的を明かす可能性がある。さらに、本発明者らのインビトロデータは、Itga-9が欠乏したLECは、それらが培養中に毛細管型の管を形成する能力を失ったことを示す。しかし、Itga-9遮断抗体を用いる本発明者らのインビボ処置レジメンは、リンパ管の有意な低減は示さないが、弁の有意な低減は示す。それにもかかわらず、この矛盾現象は、発生の間の非眼性組織におけるリンパ弁形成に関する以前の研究でも観察されている。特に、Bazigou et al23は、Itga-9ノックアウトマウスにおけるリンパ弁形成を調べ、Itga-9ホモ接合体において、弁の数の減少を報告したが、リンパ管の数の減少は報告しなかった。中和抗体を用いる本発明者らのインビボデータについての1つの考えられる説明は、Itga-9が脈管よりも角膜のリンパ弁でより高発現しており16、17、これが、抗Itga-9処置に対して弁をより感受性にし得るということである。これは、さらなる研究の根拠となる。まとめると、これらの結果は、脈管を乱すことなくリンパ弁形成を操作するための理想的な標的としてItga-9を指定する。疾患管理において弁と脈管の両方の介入が必要とされる場合に、弁/脈管標的化アプローチの組み合わせが必要であることも示唆され、これはまた、さらなる研究を要求する。リンパ管は免疫反射弓の求心性アームを構成し1、その管腔弁は脈管内部のリンパ(免疫細胞を含む)流動を導くように機能するので、角膜の内部に形成される脈管及び/または弁の数を操作することによって、免疫応答の程度ならびに様々な段階での疾患の進行及び消退をおそらく妨げることができることが推測される。要約すると、本研究は、角膜の弁形成への新しい洞察を提示するだけでなく、眼の内部または外部に生じる幅広いリンパ系疾患のためのItga-9に基づく新しい療法のさらなる開発に、いくらかの手掛かりを与える可能性がある。
