いくつかの態様において、検出は、ハイブリダイゼーションに基づくアッセイ、ウエスタンブロット、フローサイトメトリー、定量的リアルタイムポリメラーゼ連鎖反応(qRT-PCR)、クロマチン免疫沈降(ChIP)、FACS、バイサルファイトシーケンシング、免疫蛍光などによって実行される。
ここで用いられる用語「FMR1不活性化に関連する異常」は、FMR1遺伝子の転写不活性化からもたらされる疾患または異常を言う。一般的に、FMR1遺伝子の不活性化は脆弱X精神遅滞タンパク質(FMRP)の産生の喪失をもたらし、学習障害および認知障害、中程度から重度の精神遅滞、運動失調(例えば、協調の喪失)、振戦、記憶喪失、下肢の感覚の喪失(例えば、末梢ニューロパチー)、精神的および行動的変化、ならびに多嚢胞性卵巣症候群を包含する広範な発達問題を引き起こす。いくつかの態様において、FMR1不活性化に関連する異常は脆弱X症候群(FXS)である。
ここで用いられる用語「FMR1のエピジェネティックなモジュレーター」は、FMR1の転写活性を改変する薬を言う。例えば、いくつかの態様において、FMR1のエピジェネティックなモジュレーターはFMR1の転写活性を増大させる。増大した転写活性は一般的に、mRNAの増大した産生および/または増大したタンパク質翻訳(例えば、FMRPの翻訳)をもたらす。いくつかの態様において、FMR1のエピジェネティックなモジュレーターはFMR1のクロマチン状態を変化させる。エピジェネティックなモジュレーターはFMR1の転写活性を直接的に改変し得るか、またはFMR1遺伝子の発現および/もしくはエピジェネティックな状態を調節する別の因子(例えばタンパク質)と相互作用することによってFMR1転写活性を間接的に改変し得る。いくつかの態様において、FMR1のエピジェネティックなモジュレーターは、FMR1の転写活性を調節する別のタンパク質の発現レベルまたは活性(例えば機能)を阻害する。例えば、いくつかの態様において、サイレンシングされたFMR1はDNAメチルトランスフェラーゼ1(DNMT1)による増大したDNAメチル化を有する。それゆえに、いくつかの態様において、FMR1のエピジェネティックなモジュレーターはDNMT1活性または発現を阻害する薬である。いくつかの態様において、FMR1のエピジェネティックなモジュレーターは、核酸、ポリペプチド、小分子、または前述のいずれかの組み合わせであり得る。エピジェネティックなモジュレーターは、ここではエピジェネティックな修飾因子ともまた言われる。

特にN-[1-(2,3ジオレイルオキシ)-プロピル]-N,N,N-トリメチルアンモニウムメチルサルフェート(DOTAP)を包含するある種のカチオン性脂質は、本開示のFMR1のエピジェネティックなモジュレーター(例えば干渉RNA)と組み合わされるときに有利であり得る。

本願に記載される製剤に加えて、化合物はデポ剤調製物としてもまた製剤され得る。かかる持続性製剤は、好適なポリマー性または疎水性材料(例えば、許容される油中エマルションとして)もしくはイオン交換樹脂によって、または難溶解性の誘導体として、例えば難溶解性の塩として製剤され得る。
例えば、好適な液体または固体医薬調製物形態は、吸入のための水系もしくは食塩水溶液であるか、マイクロカプセル化されるか、コクリエート化(encochleate)されるか、微視的な金粒子上にコーティングされるか、リポソーム中に含有されるか、ネブライゼーションされるか、エアロゾルであるか、皮膚へのインプラントのためのペレットであるか、または鋭利な物体上に乾燥されて皮膚に刻み込まれる。医薬組成物は、顆粒、粉末、錠剤、コーティング錠剤、(マイクロ)カプセル、座薬、シロップ、エマルション、懸濁液、クリーム、点薬、または活性化合物の長引く放出を有する調製物をもまた包含し、その調製においては、賦形剤ならびに添加剤および/または助剤、例えば崩壊剤、バインダー、コーティング剤、膨潤剤、滑剤、香料、甘味料、もしくは可溶化剤が上に記載されているように習慣的に用いられる。医薬組成物は種々の薬物送達系への使用にとって好適である。薬物送達のための方法の簡略な総説は、Langer R (1990) Science 249: 1527-1533参照。これは参照によって本願に組み込まれる。
好適な緩衝剤は、酢酸および塩(1~2%w/v)、クエン酸および塩(1~3%w/v)、ホウ酸および塩(0.5~2.5%w/v)、ならびにリン酸および塩(0.8~2%w/v)を包含する。好適な保存料は、塩化ベンザルコニウム(0.003~0.03%w/v)、クロロブタノール(0.3~0.9%w/v)、パラベン(0.01~0.25%w/v)、およびチメロサール(0.004~0.02%w/v)を包含する。
好ましくは、本開示の医薬組成物は、化合物または混合物を錠剤、カプセル、もしくは丸薬として経口で、または非経口で、静脈内で、皮内で、筋肉内で、もしくは皮下で、もしくは経皮で投与可能にすることにとって好適な医薬的に許容されるキャリアまたは賦形剤を含有する。
いくつかの態様においては、FMR1のエピジェネティックなモジュレーターの治療有効量が標的組織または標的細胞に送達される。一般的に、FMR1はヒト胚において広く発現している。それゆえに、いくつかの態様においては、FMR1のエピジェネティックなモジュレーターの治療有効量が対象の脳、精巣、卵巣、食道、上皮、胸腺、眼、および/または脾臓に送達される。いくつかの態様においては、FMR1のエピジェネティックなモジュレーターの有効量が対象の中枢神経系(CNS)に送達される。いくつかの態様においては、FMR1のエピジェネティックなモジュレーターの有効量が対象のニューロン細胞、例えば分化したニューロン細胞に送達される。分化したニューロン細胞の例は、運動ニューロン、感覚ニューロン、末梢ニューロン、介在ニューロン、プルキンエ細胞、顆粒細胞、三極性ニューロン、ピラミッド型細胞、シャンデリア細胞、スピンドルニューロン、星状細胞、バスケット細胞、神経節細胞、および有毛細胞を包含するが、これに限定されない。
バッカル投与のためには、組成物は従来の様式で製剤された錠剤またはトローチの形態をとり得る。
代替的に、活性化合物は、使用前の好適な基剤、例えば無菌のパイロジェン不含水による戻しのための粉末形態であり得る。
他の送達系は、時間放出、遅延放出、または持続放出送達系を包含し得る。かかる系は化合物の繰り返しの投与を回避し得、対象および医師の便宜を増大させる。放出送達系の多くの型が利用可能であり、当業者に公知である。それらは、ポリ(ラクチド-グリコリド)、コポリオキサレート、ポリカプロラクトン、ポリエステルアミド、ポリオルトエステル、ポリヒドロキシ酪酸、およびポリ酸無水物などのポリマーベース系を包含する。薬物を含有する前述のポリマーのマイクロカプセルは例えばU.S.Pat.No.5,075,109に記載されている。送達系は非ポリマー系をもまた包含し、それらは、ステロール、例えばコレステロール、コレステロールエステル、および脂肪酸、または中性脂肪、例えばモノ、ジ、およびトリグリセリドを包含する脂質;ハイドロゲル放出系;シラスティック系;ペプチドに基づく系;ワックスコーティング;従来のバインダーおよび賦形剤を用いる圧縮錠剤;部分癒合インプラント;ならびに同類である。具体的な例は、(a)本開示の薬がマトリックス中の形態で含有される浸食型の系、例えばU.S.Pat.No.4,452,775、4,675,189、および5,736,152に記載されているもの、ならびに(b)活性成分がコントロールされた速度でポリマーから透過する拡散型の系、例えばU.S.Pat.No.3,854,480、5,133,974、および5,407,686に記載されているものを包含するが、これに限定されない。加えて、ポンプに基づくハードウェア送達系が用いられ得、それらのいくつかはインプラントのために適用される。
いくつかの側面において、本開示は、FMR1のエピジェネティックなモジュレーターとして機能する薬を同定する方法に関する。従って、いくつかの態様において、本開示はFMR1のエピジェネティックなモジュレーターを同定するための方法を提供し、方法は、不活性化したFMR1遺伝子を含む細胞を候補薬と接触させることと、細胞のFMR1発現レベルを検出することと、FMR1の発現レベルが候補薬との接触後にコントロール細胞に対して増大するときに、候補薬をFMR1のエピジェネティックなモジュレーターとして同定することとを含む。
抑制性のマークを蓄積することまたは活性化のマークを除去することを担う因子は、エピジェネティックにサイレンシングされたFMR1遺伝子を再活性化するための潜在的な標的である(図2)。次の表(表3)は、サイレンシングされたFMR1遺伝子上のエピジェネティックマークを蓄積または除去することを担う因子の非網羅的なリストである。
ゲノムワイドなライブラリーおよび/またはキナーゼもしくは転写因子サブライブラリーを用いる大スケールRNAiスクリーニングを実行して、FMR1のエピジェネティックなサイレンシングを媒介する新たな因子を同定する(図5)。このスクリーニングのために、不活性化したFMR1遺伝子の下流に位置するブラストサイジン(blasticidine)レポーター遺伝子(BlastR)を含有するレポーター細胞株を作製する。FMR1のエピジェネティックなモジュレーターによるFMR1遺伝子の再活性化は、BlastR遺伝子の発現を誘導し、薬物選択によるポジティブな候補の単離を可能にする。図5に示されているように、BlastR遺伝子はCRISPR/Cas9系を用いてFMR1遺伝子に挿入され得る。
FMR1のサイレンシングを媒介するエピジェネティックな制御因子を同定するために、転写抑制を媒介する33個の良くキャラクタリゼーションされたエピジェネティックな制御因子を対象とする162個のshRNAを含む小スケール短鎖ヘアピンRNA(shRNA)ライブラリーを構築した。それぞれのshRNAをレンチウイルスにパッケージングし、未分化のFXS-iPSC株(FXS848-iPS3細胞)に形質導入した。トランスフェクションの20日後に、mRNAを調製し、FMR1発現を定量的RT-PCR(qRT-PCR)によって分析した。ポジティブな結果は、コントロールの非サイレンシング(NS)shRNAによって得られたものと比較して、同じ標的を対象とする少なくとも2つの無関係なshRNAによるFMR1発現の統計的に有意な増大であると考えられる。図4Aの結果は、サイレンシングされたFMR1遺伝子の9つのエピジェネティックな制御因子を同定した:DNMT1、EZH2、RNF2(RING1Bともまた呼ばれる)、SUV39H1、KDM5C、KDM5D、HDAC5、HDAC10、およびSIRT5。これらの機能は表4に簡略に記載されている。便宜のために、FMR1サイレンシングを促進する下の因子はFMR1サイレンシング因子(FMR1-SF)と言う。
典型的には、エピジェネティックな制御因子は、それらが作用するプロモーターおよび/または遺伝子と安定に結びついている。9つのFMR1-SFがエピジェネティックにサイレンシングされたFMR1プロモーターと安定に結びついているかどうかを決定するために、一連のクロマチン免疫沈降(ChIP)実験を実行した。図7AのChIP実験は、9つのFMR1-SFの8つがFXS-iPSCにおいてはエピジェネティックにサイレンシングされたFMR1プロモーターに結合しているが、正常iPSCにおいてはしていないということを示している。FMR1-SFは恒常的に活性な遺伝子APRTのネガティブコントロールのプロモーターには結合していなかった。エピジェネティックにサイレンシングされたFMR1プロモーターに結びついていない単一のFMR1-SFはSIRT5であり、これはミトコンドリアに局在することが公知である。それゆえに、SIRT5はFMR1サイレンシングを促進するが、他のFMR1-SFとは違って、間接的に機能する。
FMR1-SFのいくつかには、良く記載された小分子阻害剤がある。図8Aは、エピジェネティックにサイレンシングされたFMR1が、DNAメチルトランスフェラーゼ阻害剤5-アザシチジンによる処置によってFXS848-iPS3細胞において再活性化され得たということを示しており、いくつかの以前の研究の結果と一致する。特に、エピジェネティックにサイレンシングされたFMR1は、EZH2(EPZ6438、GSK126)、SUV39H1(ケトシン(chaeotocin))、およびRNF2(PRT4165)の小分子阻害剤による処置に引き続いてもまた再活性化された。再活性化はFMR1のmRNA(図8A)およびタンパク質(図8B)レベル両方の分析によって観察され、再び野生型レベルの~15~20%であった。興味深いことに、ケトシン(chaeotocin)およびEPZ6438、ケトシン(chaeotocin)およびPRT4165、またはEPZ6438およびPRT4165などの2つの阻害剤による同時処置は、単一の阻害剤単独による処置と比較して、FMR1の増強された再活性化をもたらすということが見出された(図8C)。
上に記載されている実験は未分化のFXS-iPSCにおいて実行した。次に、FMR1-SFの同じセットの拮抗作用が、エピジェネティックにサイレンシングされたFMR1をFXS-NPCおよび有糸分裂後ニューロンにおいてもまた再活性化するかどうかを調べた。これらの後者は、FXSの特に妥当な細胞型である。それらの実験のために、上に記載されているFXS848-iPS3細胞に由来するFXS-NPC細胞株を用いた。9つのFMR1-SFのいずれか1つのノックダウンは、mRNA(図10A)およびタンパク質(図10B)レベル両方で、FXS848-NPCにおいてエピジェネティックにサイレンシングされたFMR1を再活性化した。エピジェネティックにサイレンシングされたFMR1は、mRNAおよびタンパク質レベルで、FXS848-NPCにおいて、5-アザシチジン、ケトシン、EPZ6483、GSK126、およびPRT4165を包含するFMR1-SFの小分子阻害剤によってもまた再活性化された(図10C~10D)。
次に、shRNAまたは小分子阻害剤によるFMR1の部分的な再活性化が、機能異常FXSニューロン表現型を「正常化する」ために十分であるかどうかを調べた。結果の生理的妥当性は、FXS-iPSC由来ニューロンにおけるFXSニューロン表現型のいくつかの定量可能な尺度を用いて決定した。
