Description
PEG分子によりPEG化されている方法を提供する。
CBS遺伝子は、ヒト第21染色体上にq22.3において存在する(スコブビー(Skovby)ら著、ヒューマン・ジェネティクス(Hum.Genet.)、第65巻、p.291~294、1984年;ムンケ(Munke)ら著、アメリカン・ジャーナル・オブ・ヒューマン・ジェネティクス(Am.J.Hum.Genet.)、第42巻、p.550~559、1988年;それらの内容は、参照により全体として本明細書に組み込まれる)。ヒトCBSをコードする核酸配列およびそれによりコードされるアミノ酸配列はジェンバンク(GenBank)アクセッション番号L19501を介して利用可能であり、それらの配列は内容が参照により全体として本明細書に組み込まれる米国特許第5,523,225号明細書にも開示されている。CBSをコードするゲノムDNAの核酸配列は、配列データ、例えば、ジェンバンク(GenBank)およびコロラド大学デンバー校ウェブページ(クラウス(Kraus)研究室)におけるデータを介しても公的に利用可能である。
一実施形態において、本明細書に記載の組成物および方法は、CBSDHを治療するために使用することができる。CBSDHに関連する病因性突然変異の3つの群(1)ピリドキシン応答性突然変異、(2)C末端CBS突然変異体および(3)古典的ホモシスチン尿症が存在する。
一実施形態において、PEG化htCBSまたはPEG化htCBS突然変異体は、ホモシスチン尿症の治療のためのCBS酵素補充療法(ERT)の一部として使用される。PEG化htCBSおよびhtCBS突然変異体の投与は、欠損酵素のその天然細胞内コンパートメント中への導入を必要とし得ない。
一実施形態において、細胞外および細胞内平衡の変更は、血漿ホモシステインの減少、例えば、限定されるものではないが、約5%、10%、15%、20%、25%、30%、33%、35%、40%、45%、50%、52% 55%、60%、65%、67%、69%、70%、74%、75%、76%、77%、80%、85%、90%、95%または95%超の減少であり得る。非限定的な例として、血漿ホモシステインの減少は、約75%の減少であり得る。

一実施形態において、PEG化htCBSまたはPEG化htCBS突然変異体は、非経口投与により対象に投与することができる。

実施例1.実験手順
A.PET29A(+)ベクター中の配列最適化トランケートヒトCBSの構築
開示が参照により全体として本明細書に組み込まれる国際公開第2014120770号において既に記載のとおり、細菌発現について全長(551aa)ヒトCBSコード配列を最適化し、ジェンスクリプト米国有限会社(GenScript USA Inc)(ニュージャージ、米国)によるEcoRV制限酵素により消化した後のpUC57ベクター中にクローニングした。次いで、プライマーA1およびA2を使用するPCRによりCBS配列を増幅させてトランケート酵素(aa1~413)をコードする配列を生成した。次いで、制限酵素NcoIおよびXhoIによりPCR産物を消化し、同一酵素により消化されたpET-28a(+)ベクター中にライゲートした。pET-28a(+)の最適なNcoI部位中へのクローニングは、CBS野生型配列と比較してGからCへの突然変異をもたらす。プライマーB1およびB2を利用する部位特異的突然変異誘発キット(ストラタジーン(Stratagene)、カリフォルニア、米国)を使用して野生型配列(htCBS)を再生成した。プライマーC1およびC2を使用することにより同一方針を使用してC15S突然変異体(TからAへの突然変異;ヒトCBSC15)を生成した。シーケンシングにより全ての配列を確認した。DE3細菌中への形質転換およびIPTGによる誘導を要求するpET-28a(+)ベクター中の上流T7プロモータにより、トランケートCBSの発現を制御する。表1は、プライマーおよびそれらの配列識別子を記載する。
DE3細菌、すなわち、大腸菌(E.Coli)BL-21(DE3)またはHMS174(DE3)中に、トランケートヒトCBSをコードする配列を保有するpET-28a(+)ベクターを形質転換し、30μg/mlのカナマイシンを有する5mlのルリア・ベルターニ(LB)培地中で、回転シェーカ上で275RPMにおいて37℃においてカナマイシン耐性クローンからの細菌を一晩増殖させた。30ug/mlのカナマイシンを有する100mlのテリフィックブロス(TB)培地に1mlの一晩培養物を添加し、一晩増殖させた。次いで、0.001%のチアミン-HCl pH8.0、0.0025%のピリドキシン-HCl pH8.0、0.3mMのδ-ALA pH8.0、150μMの塩化鉄、30ug/mlのカナマイシンを含有する1リットルのTB培地に、その10mlを添加した。次いで、OD600が約0.6~0.7の値に達するまで30℃において回転シェーカ上で275RPMにおいて培養物を増殖させ、1mMのIPTGの添加によりタンパク質発現を誘導した。さらに16時間、発酵を継続した。4℃における10分間の6000RCF遠心分離により細胞を回収し、氷冷0.9%のNaClにより洗浄し、上記のとおり再遠心分離し、-80℃において冷凍した。次いで、細胞ペレットに、1グラムのペレット当たり4.45mlの溶解緩衝液(20mMのNaH2PO4、pH=7.2、40mM NaCl、0.1mMのPLP)を添加し、ダウンス型ホモジナイザ中でそれをホモジナイズし、リゾチーム(最終2mg/ml)により処理し、ロッキングプラットフォーム上で4℃において1時間インキュベートし、超音波処理して粘度を低減させ、53000RCFにおいて遠心分離した。次いで、-80℃において可溶性分画を含む上清を貯蔵した。多段階クロマトグラフィー手順を介して溶解物をプロセシングした。コアプロセスは、アニオン性交換捕捉カラム(DEAEセファロース(Sepharose)-FF)と、それに続く親和性カラムからなる。これは、約90%の純度を達成する。1または2つのポリッシングクロマトグラフィーステップの使用により、>99%の最終純度が達成される。最終カラム溶出物は、PBS中に透析した。
基質としてのC14標識セリンを用いる放射性同位体アッセイにより、CBS活性を測定した。0.1Mのトリス-HCl pH=8.6、10mMのL-セリン、0.5mMのPLP、0.5mg/mlのBSAおよび0.3μCi(純粋酵素用)または0.45μCi(血漿用)のL-[C14(U)]-セリンを含有する85μlの反応混合物に、希釈緩衝液(0.1Mのトリス-HCl pH=8.6、1mMのDTT、10μMのPLP、0.5mg/mlのBSA)中の10μl(合計490ng)の純粋htCBSまたは10μlの血漿を、添加した。37℃において5分間、試料をインキュベートし、5μlの0.2MのHcy(最終濃度10mM)の添加により反応を開始させた。37℃におけるインキュベーションの30分後、グレード3CHRワットマン(Whatman)(ニュージャージ、米国)紙上にアッセイ混合物の20μlのアリコートをアプライした。それぞれの反応時間が30分間になるようにアッセイ開始および得られた混合物のサンプリングを調整した。下降ペーパークロマトグラフィを使用して標識基質(Ser)から放射性産物(Cth)を単離した。2-プロパノール/ギ酸/H2O(75:5.7:18.9 v/v)中の一晩の下降ペーパークロマトグラフィにより、C14-セリンから、この反応から形成されたC14-シスタチオニンを分離した。5mlのオプティ-フルオル(Opti-fluor)シンチレーションカクテル(パーキンエルマ(PerkinElmer)、マサチューセッツ、米国)中に浸したストリップにクロマトグラムを切断し、ベックマン(Beckman)LS-3801シンチレーションカウンタでカウントすることにより、マーカシスタチオニンの区域中の放射能(マーカレーンをニンヒドリンにより染色することにより検出される)を測定した。ブランクとして酵素不含試料を使用してバックグラウンド放射能をモニタリングし、次いでそれぞれの試料から差し引いた。純粋な酵素について、比活性値はCBS1mg当たりの酵素単位(1μmolのシスタチオニン/時間を産生する酵素の量)として表現し、血漿試料について、血漿1μl当たりの単位として表現する。薬物動態(PK)試験において、これはmU/μl血漿と命名された。
ネイティブPage(バイオラッド(Bio-Rad)、カリフォルニア、米国)を使用してタンパク質試料を分離した。次いで、染色溶液(100mMのトリス-HCl pH=8、20mMのL-システイン、50mMの2-メルカプトエタノール、0.1mMのPLPおよび0.2の硝酸鉛)中で15~30分間、ゲルをインキュベートした。7%の酢酸中でゲルを浸すことにより反応を停止させた。
(アレン(Allen)ら著、1993年、メタボリズム(Metabolism)、第42巻、p.1448~1460)に既に記載のとおり安定同位体希釈液体クロマトグラフィー質量分析により、血漿代謝産物ホモシステイン、シスタチオニンおよびシステインを測定した。(マクリーン(Maclean)ら著、2010年b、モレキュラー・ジェネティクス・アンド・メタボリズム(Mol.Genet.Metab.)、第101巻、p.153~162)に記載のとおり、合計の非タンパク質結合ホモシステインおよび他のアミノチオールの、ならびに組織中のアミノ酸の計測を実施した。安定同位体希釈液体クロマトグラフィー質量分析により代謝産物を測定した。
NOFコーポレーション(NOF Corporation)(東京、日本)からポリエチレングリコール分子を購入した。製造業者の説明書に従ってPEG化を実施した。例えば、htCBS(5mg/ml)のSH基へのPEGマレイミド誘導体のカップリングは、100mMのリン酸緩衝液pH=6.5中で4℃において一晩実施した。PEG分子およびCBSタンパク質間のモル比は、10:1または5:1であった。
PBS pH7.4を使用してPEG化htCBS C15Sを配合し、少なくとも約5mg/mlに濃縮し、典型的には、10~25mg/ml濃度に濃縮した。-80℃において(1.2ml)アリコート中で酵素を貯蔵した。それぞれの投薬日について、1mg/mlの最終濃度へのPBS中への希釈により使い捨て新鮮酵素溶液を調製した。投薬の消費時に残留するいかなる溶液も廃棄した。
全ての動物手順は、AAALAC認可(番号00235)、公衆衛生局保証(番号A3269-01)およびUSDAライセンス許諾(番号84-R-0059)機関であるコロラド大学デンバー校IACUCにより承認された。ジャクソン・ラボラトリ(Jackson Laboratory)(メイン、米国)からC576BL/6JおよびCBSノックアウト(KO)マウスを入手した。実験室内で事前にヒューマンオンリ(HO)マウスを生成した(マクリーン(Maclean)ら著、2010年b、モレキュラー・ジェネティクス・アンド・メタボリズム(Mol.Genet.Metab.)、第101巻、p.153~162)。押出成形標準食2918(ハーラン(Harlan)、カリフォルニア、米国)に供して動物を維持し、食料および水へのアクセスは無制限とした。
mix)(品目番号4369016)を使用してトリプリケートでシングルプレックスでDNAの20ngの試料をランさせた。標準曲線法を使用してアプライド・バイオシステムズ(Applied Biosystem’s)7500ファスト・インスツルメント(Fast Instrument)上で増幅を実施した。ホモ接合性1コピーキャリブレータとしてアプライド・バイオシステムズ(Applied Biosystems’)Tert(品目番号4458366)またはTfrc(品目番号4458368)コピー数参照アッセイを使用した。アプライド・バイオシステムズ(Applied Biosystem’s)アッセイMr00299300を使用してNeo遺伝子を検出した。
野生型カニクイザル(Macaca fascicularis)を6つに区切られたペン中で収容し、食料および水へのアクセスを無制限とした。12時間の明/暗サイクル、温度、および湿度を連続的にモニタリングし、制御した。野生の聴覚バックグラウンド音声および充実した可視的自然映像の両方の形態が充実した追加の環境を毎日提供した。試験前に全てのサルを全身健康についてスクリーニングした。
htCBSのPEG化形態、例えば、20NHS PEG-htCBS C15Sの注射後、種々の時点においてゲルを有するキャピジェクト(Capiject)T-MLHGリチウムヘパリンチューブ(12.5IU)(テルモ・メディカル・コーポレーション(Terumo Medical Corporation)、ニュージャージ、米国)中に、顎下腺サンプリング用に設計された使い捨てランセットを使用して有意識下の試験動物の顎下腺静脈から血液を回収した。1200Gにおける10分間の遠心分離後に血液試料から血漿を回収し、-80℃において1.5mLチューブ中で貯蔵した。
血中の酵素の経時的変化の間に生じる吸収、分布および排泄期に対応する一連の指数項に曲線を分解する非コンパートメント曲線ストリッピングモデルを使用して薬物動態(PK)パラメータを計算した。曲線ストリッピングアプローチは、薬物の堆積期が見かけの一次キネティクスに従うことを想定し、それは片対数プロットの末端部における線形性により証明される。これらの計算は、それぞれの投与経路についての平均血漿酵素活性を使用してPKソリューションズ(PK Solutions)2.0(サミット・ソリューションズ(Summit Solutions)、モントローズ、コロラド)と呼ばれるモデリングプログラムを使用して行った。SCまたはIP経路についての曲線下面積(AUC)をIV投与後に観察されたAUCにより割ることによりバイオアベイラビリティを計算した。
HPLC水(サーモフィッシャ(ThermoFisher))によりそれぞれの試料(300μg)を、非PEG化CBS対照を含め、80μLに希釈した。次いで、0.24MのCAPS緩衝液(シグマ(Sigma))pH11.5により試料を1:1(v/v)で混合し、37℃において22時間インキュベートしてPEGを除去した。アミコン超30K遠心フィルタ(Amicon Ultra 30K Centrifugal filter)(ミリポア(Millipore))に脱PEG化試料を移し、320μLの8MのグアニジンHCI溶液(シグマ(Sigma))を添加した。フィルタを13,000rpmにおいて10分間スピンした。このプロセスを2回繰り返して試料を8MのグアニジンHCl中に緩衝液交換し、残留遊離PEGを除去した。
L.組織病理学的分析のための試料の調製
17~19日目、PBS注射またはPEG化htCBSコンジュゲート、例えば、20NHS PEG-htCBS C15S処理KOマウスを屠殺し、光学顕微鏡検査による組織学的分析のために肝細胞を加工した。組織学的評価は盲検様式で実施した。肝臓を摘出し、PBS(pH7.4)中4%のパラホルムアルデヒドにより24時間固定した。組織学的観察のための組織ブロックをトリミングし、エタノール系、次いでアセトン、アセトン-キシレン混合物、およびキシレンにより脱水し、次いでパラフィン中で包埋した。並行して、非極性脂質の検出のため、ドライアイスにより冷却した石油エーテル中で、パラホルムアルデヒドにより固定された小組織ブロック(約4mm×2mm)を急速凍結させ、-50℃において貯蔵した。キシレン中で4μm厚のパラフィン切片を脱パラフィン化し、イソプロピルアルコールステップ後にエタノール(96%、70%、60%)により再水和した。ヘマトキシリンおよびエオシン(HおよびE)により組織切片を染色して組織病理学的変化を評価した。線維症の検出のためにマッソントリクローム染色を実施した。脂肪症は、10μm厚であり、ライカ・クライオミクロトーム(Leica Cryomicrotome)(CM1850)により切断された固定凍結切片中の非極性脂質の検出のためのオイルレッドO染色を使用して確認した。オリンパス(Olympus)デジタルカメラ(DP70)を備えるニコン(Nikon)光学顕微鏡(E800)により、切片を視認し、撮影した。
DEXA分析に、小動物、例えば、マウスの走査のために具体的に設計されたPIXImusII DEXAスキャナ(GEルナ・メディカル・システムズ(GE Lunar
Medical Systems))を使用した。この装置により、末端組織抽出手順前のマウスの体組成(除脂肪量、脂肪量および骨密度)の迅速で効率的な測定が可能となった。DEXA計測の4時間前にマウスを絶食させて胃腸管から食料を除去した。次いで、60mg/kgのケタミンおよび15mg/kgのキシラジンのIPボーラス注射を使用してマウスを麻酔し、DEXA装置中のトレイ上に配置した。計測を行った後(3~5分間)、DEXA装置からマウスを取り出し、最終手順、例えば、組織抽出において使用した。
1×PBS pH7.4(4%のPFA)中の4%のパラホルムアルデヒドによりマウスを灌流固定に供して眼球壁を保存した。眼球を手術により慎重に摘出し、4%のPFA中に浸し、眼球後部中で視神経直下で小さい穴を慎重に作製して4℃において一晩、固定剤が眼球の内部に浸透し、構造を固定するのを可能とした。濾過1×PBSpH7.4を使用して試料を3回洗浄した。洗浄後、眼球後部を毛様体上皮の位置まで慎重に切除して毛様体小帯を露出させた。8%のBSAブロッキング溶液中で振とうさせながら2時間、試料を配置した。ブロッキング後、試料に4%のBSA中で1:50希釈されたMAGP1抗体(シグマ(Sigma))を添加し、4℃において一晩貯蔵した。翌日、1×PBS pH7.4を使用して試料を6回洗浄し、試料にアレクサ(Alexa)488二次抗体(1:200、インビトロジェン(Invitrogen))を添加し、室温において浸とうさせながら2時間インキュベートした。試料を6回洗浄し、メチルグリーン(核染色)により15分間対比染色した。
A.非修飾htCBSは循環中の短い保持時間を示す
循環に投与される薬理学的活性物質の薬物動態特性は、ADMEの天然機序により大きく影響を受ける。注射分子の急速なクリアランスは、治療効力に大きく影響し得、したがって、より長い循環半減期が望まれ、それは順次、副作用を最小化するより低頻度の投与またはより小用量につながり得る。血漿循環中のhtCBSの薬物動態を測定するため、皮下(SC)、静脈内(IV)または腹腔内(IP)経路を介してC57BL/6Jマウスに5mg/kgの単回用量を注射した。IVまたはIP経路を介するhtCBSの投与は、注射の最初の4時間後の間に最大比活性値を示した(それぞれ最大123および76mU/μLの観察ピーク血漿レベル;表2は、曲線下面積(AUC)値を示す)。血漿試料から計算された薬物動態パラメータ(表2)は、htCBSについての半減期が2.7時間であったことを示し、ほぼ6つの半減期後(20時間未満)にhtCBSレベルが検出不能であるはずであることを示唆する。
CBSの薬物動態に対するPEG分子の影響を決定するため、低分子量(2kDa)直鎖PEGにより、ならびに高分子量(40kDa)4アーム分枝鎖PEG(それぞれME020MAおよびGL4-400MAと命名)によりhtCBSをPEG化した。表3に示されるとおり、PEG化は酵素の比活性に影響を及ぼさなかった。表3は、PEG化および非PEG化htCBSのクーマシー染色SDS-PAGEの結果を示し、対応する比活性値は、PEG化が酵素の活性に影響を与えないことを示す。マウスにAに記載のとおりSC注射し(n=4~5)、ME200MA-またはGL4-400MA-PEG化htCBSを非PEG化htCBSと比較した。
CBSDHのためのhtCBSの使用の最終目的は、毒性tHcy負荷を低下させること、シスタチオニンおよびシステインレベルを上昇させることである。これらの変化は、CBSDH症状の発症を予防、回復または遅延させることが予期される。しかしながら、ホモシスチン尿症研究は、長きにわたり好適な動物モデルの欠落により阻まれてきた。マウス遺伝子について完全なノックアウトであるマウスは、出生後2~3週間以内に死亡する。HO(ヒューマンオンリ)マウスは、他のノックアウトマウスと異なる。それというのも、マウス遺伝子がノックアウトされていることに加え、それらはそれらの成体期までの生存を可能とする低レベルのヒト遺伝子を発現するためである(マクリーン(Maclean)ら著、2010年b、モレキュラー・ジェネティクス・アンド・メタボリズム(Mol.Genet.Metab.)、第101巻、p.153~162)。HOマウスは、Hcy、メチオニン、S-アデノシルメチオニン、およびS-アデノシルホモシステインの重度の上昇を示し、付随的なシステインの血漿および肝レベルの減少を呈する。したがって、これらのマウスは、いくつかの態様において、ヒトにおいて生じる疾患を再現する特徴を示す。
非PEG化htCBSを5mg/kgにおいて1週目および3週目(治療のセット間は10日間)で5日連続で投与したHOマウスは、血清ホモシステインレベルの変化を実証しなかった。図2は、HOマウスにおける平均ホモシステインおよびシスタチオニンレベルを示す。非PEG化htCBSによる代謝モジュレーションのこの欠落は、ポリエチレングリコール分子の共有結合によりhtCBSを修飾する必要性をさらに裏付ける。
HOマウスを使用してPEGhtCBS投与の長期効果をモニタリングした。PEGhtCBS酵素は、7.5mg/kgの用量において、最初の2週間については週2回(月曜日および木曜日)、および次の6週間については週3回(月曜日、水曜日および金曜日)、8匹のHOマウスに投与した。注射の24時間(火曜日)および72時間(月曜日)後に血液試料を回収した(図4A~4C)。図4Aは、血漿tHcyが0時間における212μMから、注射24時間後に62~103μMの範囲内の平均に低減し、最終の毎週の注射72時間後に141~189μMの範囲であったことを示す。28日目以降、注射72時間後の値は0時間の値よりも有意に低かった(P≦0.02)。シスタチオニンおよびシステインレベルも同様に好ましい影響を受けた。注射24時間後、シスタチオニンは6μMから30μMほど高くまで増加し、システインは一定して200μM超であった(図4C)。興味深いことに、24日目以降、シスタチオニン濃度の変動は、処理の最初の3週間と比較してほとんど目立たなかった。血漿代謝産物レベルの分析に加え、注射マウスからの肝臓、腎臓および脳組織試料も同様に分析して非タンパク質結合ホモシステイン(遊離およびジスルフィド結合形態)のレベルを測定した。図4Dに示されるとおり、長期注射動物において、肝臓、腎臓、および脳組織についてそれぞれ44%、63%および47%の低減が実証された。したがって、PEGhtCBSの長期反復注射はホモシステイン、シスタチオニンおよびシステイン血漿レベルに有意に影響する。
A.C15S突然変異htCBSは凝集および均等なPEG化パターンを示さない
htCBS酵素は全長タンパク質と比較して凝集傾向が低いことが既に報告されたが(フランク(Frank)ら著、2006年、バイオケミストリー(Biochemistry)、第45巻、p.11021~11029)、それは四量体およびより高次の凝集物を依然として形成する。これは免疫防御機序を刺激し得、例えば、アミロイド沈着(デスーザ(D’Souza)ら著、2014年、「アミロイド:ジ・インターナショナル・ジャーナル・オブ・エクスペリメンタル・アンド・クリニカル・インベスティゲーション、ジ・オフィシャル・ジャーナル・オブ・ジ・インターナショナル・ソサエティ・オブ・アミロイドーシス(Amyloid:the international journal of experimental and clinical investigation.The official journal of the International Society of Amyloidosis)」、第21巻、p.71~75)をもたらし得るだけでなく、精製、取扱、およびコンシステンシーにも影響し得る。図5A(内容が参照により全体として本明細書に組み込まれる国際公開第2014120770号の図5Aとして既に開示)は、htCBSの異なるバッチが異なる二量体/四量体比を示し、より高次の凝集物の程度が変動したことを示すクーマシー染色ネイティブPAGEを示す。図5B(内容が参照により全体として本明細書に組み込まれる国際公開第2014120770号の図5Bとして既に開示)は、ゲル内活性アッセイであり、それらの凝集物がCBS活性を示すことを実証した。図5C(内容が参照により全体として本明細書に組み込まれる国際公開第2014120770号の図5Cとして既に開示される)は、凝集が非一貫性PEG化産物を引き起こすことを示す。PEG化は、変動するPEG化の程度を反映すると仮定される2つの区別されるバンドをもたらした。それというのも、凝集が利用可能なPEG化部位を隠蔽するように作用し得るためである。したがって、バッチ28(図5A)のより目立つ凝集に起因して、そのPEG化は、SDS-PAGE上で、より目立つ下方バンド、すなわち、より低PEG化のものをもたらした一方(図5C)、四量体および二量体の両方から構成されるバッチ112(図5A)のPEG化は、バッチ28と比較して目立つ上方バンドをもたらした(図5C)。図5Dは、還元剤トリス(2-カルボキシエチル)ホスフィン(TCEP)を用いる両方の酵素バッチのインキュベーションが四量体およびより高次の凝集物のほとんどを二量体形態に変換させたことを示すクーマシー染色ネイティブゲルを示す。これらのデータは、凝集がhtCBSの表面上の露出システインにより駆動されることを意味する。したがって、TCEP処理は、より広範なPEG化ももたらすことが考えられた。それというのも、より多くの二量体がより高いMW形態から生成され、htCBSの表面上の追加のPEG化部位の露出を生じさせるためである。実際、図5Eに示されるとおり、クーマシー染色SDS-PAGE上の上方/下方PEG化バンド間の比は、TCEPの不存在下でのPEG化と比較してTCEP処理後に上方バンドに有意にシフトする。したがって、htCBS凝集は、少なくとも部分的に、分子内ジスルフィド架橋の形成により駆動されると考えられる。したがって、C15をセリンに突然変異させ、C15ShtCBS突然変異体を生成した;C15は、CBSの表面上の最も反応性のシステインであることが以前示された(フランク(Frank)ら著、2006年、バイオケミストリー(Biochemistry)、第45巻、p.11021~11029)。図5F(内容が参照により全体として本明細書に組み込まれる国際公開第2014120770号の図6Aとして既に開示)は、2つの主要バンド(二量体および四量体)がhtCBSについて観察される一方、C15ShtCBS突然変異体が単一バンドを示すクーマシー染色ネイティブゲルを示し、それは予測されるとおり、TCEPによる処理によっては影響を受けない。この観察は、HPLCサイズ排除クロマトグラフィー(HPLC-SEC)によりさらに立証された。ヤッラ(Yarra)SEC-3000、300*7.8mmサイズ排除カラム(フェノメネクス(Phenomenex)、カリフォルニア、米国)上でCBS調製物を分離した。カラムを較正し、100mMのリン酸ナトリウムpH=6.8中で1ml/分の流量において室温において操作した。HPLC-SEC分析、図5G(内容が参照により全体として本明細書に組み込まれる国際公開第2014120770号の図7Aおよび図7Bの一部として既に開示)は、htCBS C15S(図5G、左パネル)およびhtCBS二量体/四量体比(図5G、右パネル)の差を示す。二量体C15S htCBS突然変異体についての単一ピークは8.62分において溶出した一方、htCBSについての主要ピークは7.9分において溶出し、四量体に対応した。図4Hは、異なるバッチ間のhtCBS C15SのPEG化の再現性およびhtCBSとの比較を示すクーマシー染色SDS-PAGEゲルを示す。C15S htCBS中の凝集物の不存在は、SDS-PAGE上で現れるほぼ単独で1つのPEG化バンドを有する異なるバッチ間のより一貫したPEG化パターンをもたらした。インビボおよびインビトロの両方でのPEGhtCBSおよびPEGC15S間の直接比較は、性能の差を示さなかった。
ベタインおよび組合せ療法の効果を評価するため、ベタイン、PEGC15S、およびベタイン+PEGC15S処理の組合せを実施し、トランススルフレーション経路の代謝産物を比較した。
A.早期死亡の低減
完全なCBSノックアウトマウス(KO)の大多数は、出生後2~3週間以内に死亡する。PEGC15Sを週2回注射されたKOマウス対PBSを注射されたマウスのカプランメイヤー生存曲線をプロットした。21日目までベタイン水でマウスを維持し、KO仔マウスをレスキューする注射酵素の能力を試験した。KO仔マウスにPEGC15SまたはPBSを週2回注射した。21日目(離乳日)まで飲料水中のベタインで全てのマウスを維持した。図8Aに示されるとおり、21日目、PBS注射群においてマウスの18%のみが生存し、対照的に酵素が注射されたマウスの93%が生存した。PBS注射動物は24日目まで生存しなかった一方、酵素注射群については86%の生存率が記録された。24日目以降、酵素注射動物の数も降下し始め、29日目に45%、35日目に33%の生存率であったが、追加の以下の実施例に示されるとおり、異なるPEG分子および投薬レジメンの使用はそれらの結果に大きく影響することが観察された(例えば、図11Aおよび11C)。
KOマウスが重度の肝損傷を罹患することは既に実証された(マクリーン(Maclean)ら著、2010年a、モレキュラー・ジェネティクス・アンド・メタボリズム(Mol.Genet.Metab.)、第101巻、p.163~171;ワタナベ(Watanabe)ら著、1995年、米国科学アカデミー紀要(Proc.Natl.Acad.Sci.U.S.A.)、第92巻、p.1585~1589)。したがって、KOマウスの生存延長は、CBS処理後の肝疾患の改善と相関させることができる。したがって、PEGC15Sを35日間注射された動物からの肝切片の組織学的分析を実施した。36日目に動物を屠殺し、光学顕微鏡観察による組織学的分析のために肝試料を加工した。PBS注射動物は35日目まで生存しなかったが、死亡直後の2匹の動物から肝試料を採取し、加工した。実験は盲検様式で実施した。肝臓を摘出し、PBS(pH7.4)中4%のパラホルムアルデヒドにより24時間固定した。組織学的観察のための組織ブロックをトリミングし、エタノール系、次いでアセトン、アセトン-キシレン混合物およびキシレンにより脱水し、次いでパラフィン中で包埋した。並行して、非極性脂質の検出のため、ドライアイスにより冷却された石油エーテル中で、パラホルムアルデヒドにより固定された小組織ブロック(約4mm×2mm)を急速冷凍させ、-50℃において貯蔵した。キシレン中で4μm厚のパラフィン切片を脱パラフィン化し、イソプロピルアルコールステップ後にエタノール(96%、70%、60%)により再水和した。ヘマトキシリンおよびエオシン(H&E)により組織切片を染色して組織病理学的変化を評価した。線維症の検出のためにマッソントリクローム染色を実施した。脂肪症は、10μm厚であり、ライカ・クライオミクロトーム(Leica Cryomicrotome)(CM1850)により切断された固定凍結切片中の非極性脂質の検出のためのオイルレッドO染色を使用して確認した。オリンパス(Olympus)デジタルカメラ(DP70)を備えるニコン(Nikon)光学顕微鏡(E800)により、切片を視認し、撮影した。
20NHS PEG-htCBS C15Sの生化学的特徴は、その分子のインビボ投与の効果に関する試験前に決定した。
N末端シーケンシングを使用して非修飾htCBS C15Sの5つのバッチを分析した。得られた配列は全てのバッチについて同一であり、参照配列と比較し、それらは全て、最初のメチオニン残基を欠いた。htCBS C15SのN末端が、野生型CBSの配列に見出されるとおりPSETPであり、MPSETPでないことが観察された。
20NHS PEG-htCBS C15Sの3つのロットのPEG化部位をマッピングした:RC-6-67A(6.3mg/ml)、RC-6-67B(6.7mg/ml)、およびRC-8-14(3.8mg/ml)。アルカリ中で試料を脱PEG化し、還元し、アルキル化し、Asp-Nエンドペプチダーゼにより消化し、LC/UV/MSにより分析した。
SDS-PAGEおよびネイティブPAGEゲルを実施してME-400MAおよびME-200GSにコンジュゲートしているhtCBSについてのPEG化パターンの再現性を比較した。結果は、区別されるPEG-CBS種間の変動比を示し、ME-400MAのマレイミドPEG分子を使用した場合に不完全なPEG化を示した。NHSエステルPEG ME-200GSについてのPEG化の変動程度は、SDS-PAGE(図9A)およびネイティブPAGEゲル(図9B)上で分離不能なPEG化種の高分子量「スメア」を生成した。
ペプチドマッピングの最初のステップは、ME-400MAによりPEG化されているhtCBS C15Sの2つの試料:RC-6-13プール1およびRC-6-13プール2中のそれぞれのシステイン含有ペプチドを説明することであった。RC-6-13プール1(BSL試料番号1127)は、ホモPEG化種を表す(同一残基上でPEG化されている可能性が最も高い)。RC-6-13プール2(BSL試料番号1128)はヘミPEG化種を表し、したがって、SDS-PAGE上で、PEG化および非修飾バンドのほぼ等しい比率が観察された。この実施例は、最もアクセス可能なシステイン(C15)がセリンにより置き換えられている場合、ME-400MAの付着の局在を決定した。
本実施例は、20kDa直鎖NHSエステル活性化PEG(ME-200GS)または40kDa直鎖マレイミド活性化PEG(ME-400MA)のいずれかによりPEG化されているhtCBS C15Sの薬物動態(PK)および薬力学(PD)特性を特徴付けし、比較する。DEAEセファロース(sepharose)上でPEGC15Sのそれぞれを精製し、1×滅菌PBS中でバイオマックス(BioMax)(登録商標)100カートリッジ上のタンジェンシャルフロー濾過(TFF)を使用して緩衝液交換した。
C15S(20NHS PEG-htCBS C15S)を6mg/mlに濃縮した。
時間活性曲線から計算されたPKパラメータは、ME-200GSコンジュゲートについて表10に、およびME-400MAコンジュゲートについて表11に概略する。片対数プロットは静脈内群について線形であり、したがって、htCBS C15S酵素の全ての形態が一次キネティクスにより血漿からクリアランスされたことを裏付けた(データ示さず)。
PEG-htCBS C15Sの全吸収のわずかな減少により説明される可能性が高い。
PKパラメータは、本明細書において比較されたhtCBS C15Sの2つのPEG化形態(ME-200GSおよびME-400MA)について認識可能に異なるとは観察されなかった。それぞれの酵素の10mg/kgのIVおよびSC投与についての比活性対時間曲線は、図10Bに示されるとおり、類似することも観察された。IV投薬後の活性は実線として示し、SC投与後のものは点線として示す。丸(IV)および三角(SC)は20NHS PEG-htCBS C15Sを投与されたマウスについての回収時間を表し、菱形(IV)および四角(SC)は400MA-htCBS C15Sを投与されたマウスについての回収時間を表す。
PD応答は、一般に、ピーク血漿レベルに正比例して生じ、半減期に比例して減少することが観察された。しかしながら、ピーク応答の用量関係は、5mg/kgにおいて最大レベルに接近することが観察され、その結果、10または15mg/kgの用量が図10Dに示されるホモシステインおよびシスタチオニンレベルの両方により決定されたとおりピーク効力のわずかな増加をもたらすことが観察された。排出の波状パターンが排出期のそれぞれにおいて観察され、それはそれぞれの曲線において同一であり、その結果、血漿活性は48時間においてトラフに到達し、次いで72時間においてリカバーし(または類似であり)、結局96時間において再度降下する結果となった。htCBS C15SのSC投薬後に観察されたこの異常な排出パターンは、早期の急速およびより低速の後期の吸収期からなる二相吸収期のクリアランスに起因し得ると考えられる。
本実施例は、HOマウスにおいて長期外因投与されるME-200MA0B、ME-200GS、GL4-400MA、ME-400MA、またはME-050GSのいずれかによりPEG化されているhtCBS C15Sの薬力学的効力および潜在的免疫原性応答を評価し、比較する。マウスゲノム中にヒトタンパク質を遺伝子操作し、HOマウスの場合のように新生児動物中で発現させる場合、免疫応答は、ヒト応答に対する臨床的関連性を有する可能性がより高い。産業界のためのガイダンス、治療タンパク質産物の免疫原性評価(Guidance for Industry,Immunogenic assessment of therapeutic protein products)、CEBR(2014年8月)参照。HOマウスは少量のヒトCBSタンパク質を発現し、完全ノックアウトまたは野生型マウスよりもPEGhtCBS酵素に対するそれらの適応免疫応答に関してヒト患者に類似する可能性が高い。
本明細書に記載のKOマウスモデルにおいて生存試験を実施して種々のPEG化htCBS C15Sコンジュゲートの効力を比較した。
ME-200MA0B(n=24)、ME-400MA(n=31=13F+18M)、またはME-200GS(n=28=14F+14M)によりPEG化されている7.5mg/kgのhtCBS C15Sを週3回注射されたKOマウス、および対照としてPBSを注射されたKOマウス(n=44)の群のカプランメイヤー生存曲線をプロットした。授乳中の母マウスをベタイン水で維持し、離乳後、ベタインを有さない常用水に供して仔マウス(21日目)を配置した。KO仔マウスをレスキューする注射酵素の能力を試験した。
C15(C15S/400MA)または20NHS PEG-htCBS C15S(C15S/200GS)を注射されたものの約97%が生存した。
実施例1の実験手順を使用して週末ウオッシュアウト後(トラフ)および最後の注射の24時間後(ピーク)に約4~5週齢における生存KOマウスにおける代謝産物レベルを計測した。
KOマウスの新生児死亡を予防するための初回の35日間(5週間)の長期処理後、20NHS PEG-htCBS C15Sにより処理されたKOマウスを2つの群に分けた。一方の群(5W)は、初回処理を別として、いかなる処理も受けることを停止させた一方、他方の群(連続)は、120日齢まで同一のレジメンを用いて継続した(7.5mg/kg、SC、週3回)。
20NHS PEG-htCBS C15Sを7.5mg/kgの用量において週3回投与されたKOマウス(雄n=16および雌n=11)の総重量および重量増加の量を測定し、同様に処理された+/-健常リッターメイト(雄n=14および雌n=13)と比較した。21日目から36日目まで離乳後のマウスにおいて総重量および重量増加を計測した。
ホモシスチン尿症のI278T-/-ホモ接合性マウスモデルは、他のホモシスチン尿症マウスモデルと異なり、典型的な代謝産物プロファイルに加えて特異的表現型形質、例えば、顔面脱毛症を示すことが観察された。顔面脱毛症は、顔面および頭部の体毛の損失として定義される。顔面脱毛症の発症は約90日間(W12)であり、典型的には、約120日齢(W17)において完全に発生した。
顔面脱毛症のいかなる徴候も示さない3W齢マウスを2つの群に分けた。第1群(n=3匹の雄)に、200MA0B PEG-htCBS C15Sを7.5mg/kgで6週間の期間、週3回注射した。実験前に身体外観を記録し、週1回モニタリングし、次いで6W注射レジメン後に週2回モニタリングした。代謝産物レベルは、実験前および最後の注射の24時間後、すなわち、6W注射レジメン後に確認した。
C15Sの投与がI278Tマウスにおける顔面脱毛症の発症を遅延させた証拠を提供する。
第2群(n=3;2匹の雄、1匹の雌)に7.5mg/kgの200MA0B PEG-htCBS C15Sを連続注射し、3W齢から41W齢まで皮下投与した。重量を最初に週1回モニタリングした。実験前に身体外観を記録し、週1回または初回6W注射レジメン後に週2回モニタリングした。代謝産物レベルは、実験前および初回6W注射レジメント(regiment)の最後の注射の24時間後に確認し、次いで週1回または週2回追跡した。
少なくとも120D(W17)齢であり、視認可能で完全に発生した顔面脱毛症を有するI278Tマウスを、2つの群に分けた。約1年齢までA群(n=6;3匹の雄、および3匹の雌)に400MA PEG-htCBS C15Sを週3回注射し、B群(n=4;2匹の雄、2匹の雌)に20NHS PEG-htCBS C15Sを週3回注射した。処理開始前に代謝産物レベルを計測し、試験の間、注射24時間後に計測し、重量および身体外観を最初に週1回モニタリングし、後に週2回モニタリングした。
PEG-htCBS C15Sを投与されたものについてより良好であったことを提供する。
C15S(A群)よりもHcyレベルをかなり減少させ、Cysレベルを増加させることが観察されたことを示す。
PEG-CBSの投与は、PEG部分に関わらず、上記のとおり非処理I278T-/-ホモ接合性マウスに典型的な表現型形質である顔面脱毛症の改善をもたらした。6週間のERT施与は、顔面脱毛症の発症の遅延をもたらした。顔面脱毛症の発症前のマウスへの連続ERT施与はその発生を完全に予防し、そのような動物は非罹患健常I278T+/-ヘテロ接合性マウスと区別不能であった。顔面脱毛症が既に完全に発生したマウスへの連続ERT施与は、それらの身体外観の有意な改善をもたらした。全ての場合において、顔面脱毛症に対する好ましい効果は、血漿代謝産物レベルの少なくとも部分的な(Hcy)または完全な(Cys)正常化に伴い、またはそれらにより引き起こされた。
2日齢から7.5mg/kgの20NHS PEG-htCBS C15SまたはPBSを週3回皮下(SC)注射され、17~19日齢において安楽死させたKOマウスからの肝切片について組織学的分析を実施し、日齢が一致する+/-健常対照マウスの肝臓と比較した。
17~19日目に動物を安楽死させ、光学顕微鏡による組織学的分析のために肝試料を加工した。図13Aに示されるとおり、PBS注射KOマウスの肝実質は、矢印により示される中程度から重度の脂肪症、病巣肝細胞壊死、および吸収炎症反応を示す肝小葉のわずかに不規則なアーキテクチャを有することが観察された。同様に図13Aに示されるとおり、健常ヘテロ接合性マウスの肝実質および20NHS PEG-htCBS C15S処理KOマウスは、規則的な肝臓のアーキテクチャおよび最小の核大小不同を有することが観察された。
B.電子顕微鏡観察
3000倍の倍率における電子顕微鏡観察も使用して健常ヘテロ接合性(+/-)マウス、PBS処理KOマウス、および400MA PEG-htCBS C15S処理KOマウスからの肝組織も観察した。図14Aに電子顕微鏡観察の取り込み画像を示す。図14Aにおいて、黒色矢印は粗面小胞体(ER)を示し、白色矢印はミトコンドリア(MT)を示す。黒色アスタリスクは細胞質グリコゲンを示し、二重黒色アスタリスクは核を示す。白色アスタリスクは脂肪滴を示す。
野生型マウス(対照)、非処理ホモ接合性-/-罹患KOマウス、および400MA PEG-htCBS C15S処理KOマウスの群において血漿および組織代謝産物を計測した。処理群に、400MA PEG-htCBS C15Sの皮下注射を、2日齢から7.5mg/kgの用量において週3回施与した。最後の注射の24時間後に血漿および組織を回収した。表15は、血漿中のHcy、Cys、Cth、およびMetのレベルを示す。
アルゼット(ALZET)(登録商標)浸透圧ポンプは、新鮮およびナイーブHOマウスにおいて長期の期間、20NHS PEG-htCBS C15Sの安定した制御送達を提供する。反復皮下注射に代えて1時間当たりの酵素の容量を連続分注するマイクロ浸透圧アルゼット(ALZET)(登録商標)ポンプの4つの異なるモデル:モデル1002(0.25μl/h)、モデル2002(0.5μl/h)、モデル2004(0.25μl/h)、およびモデル2006(0.15μl/h)を使用してME-200GSによりPEG化されているhtCBS Cl5Sを投与した。
C15Sの比活性を比較するグラフである。モデル2002ポンプが移植されたHOマウスの血漿中のCBS活性は、モデル1002が移植されたマウスと比較してほぼ2倍であることが観察された。しかしながら、モデル2002ポンプが移植されたマウスの血漿中のCBS活性の量の2倍は、モデル1002ポンプが移植されるとともにモデル2002群と比較してほぼ半分の血漿CBS活性を有するマウスにおいて達成されたものと類似する血漿代謝産物の改善をもたらした。
実施例1のプロトコルを使用する二重エネルギーx線吸収測定(DEXA)を使用してKOおよびI278Tマウスにおける20NHS PEG-htCBS C15Sの長期連続投与の効果を分析した。
3つのマウス群についてDEXAにより骨密度(BMD)および骨塩量(BMC)を分析した:+/-健常ヘテロ接合性対照(n=4M+6F)、罹患KO対照(n=5M+13F)、および20NHS PEG-htCBS C15S(C15S/ME-200GS)処理KOマウス(n=6M+15F)。20NHS PEG-htCBS C15S処理マウスに、2日目から約5ヵ月まで7.5mg/kgを週3回皮下(SC)投与した。全ての-/-KOマウスを初回5W(2~35日齢)について処理して新生児死亡を予防した。
20NHS PEG-htCBS C15S処理KOマウスにおいて、試験過程にわたり週2回、Hcy、Cth、およびCysの血漿レベルを計測し、図16Fに示す。41日目(真のゼロ時間を表すのではなく、注射72時間後の代謝産物レベルを表す)において最初に血液試料を採取し、次いで、45日目において開始して2週間ごとに試料を採取した。Hcyのレベル(中段線)は、45日目における約180μMから、45日目における約90μMに減少し、試験全体にわたりそれらのレベルにおいて安定したままであることが観察された。Cysのレベル(上段線)は、処理により45日目における100μM未満から45日目における約200μMに正常化し、試験過程にわたり持続した。Cthのレベル(下段線)は、試験の継続時間全体にわたり約50μMで変動した。
3つのI278Tマウス群についてDEXAにより骨密度(BMD)および骨塩量(BMC)を分析した:+/-健常ヘテロ接合性対照(n=10M+11F)、-/-罹患ホモ接合性I278T対照(n=6M+8F)、および20NHS PEG-htCBS C15S処理I278Tマウス(n=6M+6F)。20NHS PEG-htCBS C15S(C15S/ME-200GS)処理I278Tマウスに、6ヵ月齢から13ヵ月齢まで7.5mg/kgを週3回皮下(SC)投与した。図17A、図17B、図17C、図17D、および図17Eは、それぞれ、3つのマウス群間のBMD(g/cm2)、BMC(g)、全質量(g)、除脂肪量(g)、および脂肪含量(%)を比較するグラフである。表21は、対照群と比較した処理群の差の有意性を決定するために使用されるP値を提供する。
PEG-htCBS C15S処理I278Tマウスとの間で区別不能であることが観察された。したがって、20NHS PEG-htCBS C15SによるI278Tマウスの処理も深刻な骨粗鬆症を予防し、体組成(全質量、除脂肪量および脂肪含量)を改善した。
上記のものと同一のマウス/群において血漿代謝産物を測定し、互いに比較した。図17Fに、代謝産物Hcy、Cth、Cys、およびMetの血漿レベルを示す。左側のバーは+/-健常対照についての代謝産物のレベルを表し、中央のバーは-/-罹患対照についての代謝産物のレベルを表す。右側のバーにより20NHS PEG-htCBS C15S処理マウスにおけるそれぞれの代謝産物のレベルを表す。表22は、+/-健常対照、-/-罹患対照、および20NHS PEG-htCBS C15S処理I278Tマウス間の血漿代謝産物レベルの差についての有意性を決定するために使用されるP値を示す。
I278Tマウスへの200MA0B PEG-htCBS C15Sの投与は、肝グルコース代謝、酸化還元状態、およびメチル化潜在性ならびに脂質代謝を改善することが観察された。肝機能は非罹患であることが観察され、炎症サイトカインおよび血漿脂質は処理により正常化されることが観察された。
200MA0B PEG-htCBS C15Sによる長期処理後のI278Tマウスにおいて血漿バイオマーカを計測して肝機能、炎症、および血漿脂質レベルを分析した。3つのI278Tマウス群を分析した:+/-健常ヘテロ接合性対照(n=3)、-/-罹患ホモ接合性対照(n=3)、および200MA0B PEG-htCBS C15S処理I278Tマウス(n=3)。200MA0B PEG-htCBS C15S処理マウスに、6ヵ月齢から13ヵ月齢まで7.5mg/kgを週3回皮下(SC)投与した。アラニンアミノトランスフェラーゼ(ALT)、アスパラギン酸アミノトランスフェラーゼ(AST)、およびアルカリホスファターゼ(ALP)の酵素活性;サイトカインIL-12(p40)、IL-12(p20)、IL-13、G-CSF、MCP-1、およびTNF-αの濃度;ならび血漿脂質(総、HDLおよびLDLコレステロール、ならびにトリグリセリド)の濃度を血漿中で計測した(結果示さず)。肝機能は、AST、ALTおよびALP血漿活性が参照範囲内であるため、非罹患であることが観察された。炎症性サイトカイン、特にIL-12(p40)およびIL-13は非処理I278Tマウスにおいて上昇し、処理群については正常化されることが観察された。血漿脂質パネルは、非処理I278Tマウスが健常+/-対照と比較して有意に高い総、HDLおよびLDLコレステロールを有する一方、かなり減少したトリグリセリドを有することを示した。脂質プロファイルは、200MA0B PEG-htCBS C15S処理I278Tマウスにおいて完全に正常化された。
上記のバイオマーカに加え、核磁気共鳴(NMR)メタボロミクスを実施して同一マウスの肝組織を分析してI278Tマウスに対する200MA0B PEG-htCBS C15Sによる長期処理の効果を調査した。親水性および親油性溶媒中に肝組織からの代謝産物を抽出し、NMRを使用して別個に定量した。
I278T(+/-)健常ヘテロ接合性対照マウス、非処理-/-罹患ホモ接合性対照マウス、および20NHS PEG-htCBS C15S処理-/-ホモ接合性I278Tマウスにおいて血漿中の代謝産物レベルの変化を計測した。20NHS PEG-htCBS C15S(C15S/ME-200GS)処理マウスに、2日齢から4ヵ月齢まで7.5mg/kgを週3回皮下(SC)投与した。図19Aに6週間から4ヵ月までの血漿代謝産物レベルの変化を示す。マウスを2日齢から処理したため、注射72時間後に開始してレベルを計測した。
I278Tマウスにおいて食餌試験を実施し、それらを分析した。10週齢のマウスの血漿代謝産物:+/-I278T(対照)、-/-I278T(陰性対照)、および20NHS PEG-htPBS(OT-58)により処理された-/-マウス。それぞれの群をさらに下位群に分け:1群を常用食(8.2g/kgのMet TD.01084食+亜鉛水)(REG)に供し、1群をメチオニン制限食(0.5g/kgのMet TD.110591食+ベタイン水)(MRD)に供した。W5の週齢において処理を開始した。
A.単回投与薬物動態
ラットにおける20NHS PEG-htCBS C15Sの薬物動態を評価するため、野生型スプラーグ・ドーリ(Sprague Dawley)ラットに静脈内(IV)または皮下(SC)経路を介して単回用量の20NHS PEG-htCBS C15Sを注射した。表23に示されるもののように、同一のまたは極めて類似する試験設計を使用して3つの別個の単回投与PK試験を組み合わせた。第3(反復投与)のPK試験(第7~9群)の設計は、最初の投与後のデータ抽出およびしたがって単回投与PK試験(第1~6群)とのその組合せを可能とするように調整した。表23に列記される指定時間において血漿を回収し、実施例1に詳述される方法を使用してCBS活性について分析した。
長期毒性試験を実施するための種としてラットをさらに評価するため、4mg/kg(低)、8mg/kg(中)、または24mg/kg(高)の20NHS PEG-htCBS C15Sのいずれかの48時間ごとの合計9回のSC注射を受ける3つのスプラーグ・ドーリ(Sprague Dawley)ラット群において20NHS PEG-htCBS C15Sの薬物動態を測定した。表23に試験設計を概略する。実験は24日間の長さにわたり実施した。血漿を回収し、実施例1に詳述される方法を使用してCBS活性について分析した。
長期毒性試験のための非ヒト霊長類モデルを評価するため、およびアロメトリックスケーリングを介してヒト有効用量を推定するため、野生型カニクイザル(Macaca fascicularis)において20NHS PEG-htCBS C15Sの薬物動態および薬力学試験を実施した。
単回投与PK試験について、それぞれの4(n=2M+2F)匹のサルの3つの実験群を使用して実験を実施した。第1群は、IV注射を介して2mg/kgの20NHS PEG-htCBS C15Sを受けた。IV注射0、0.083、0.16、0.25、0.5、1、2、4、8、24、72、120、および192時間後において血液を回収した。第2および第3群は、SC注射を介して2mg/kgまたは6mg/kgの20NHS PEG-htCBS C15Sのいずれかを受けた。SC注射0、1、2、4、8、24、48、72、96、120、192、240、および336時間後において血液を回収した。CBS活性について回収された血漿を分析し、実施例1に記載の方法を使用して硫黄アミノ酸代謝産物(ホモシステイン、シスタチオニンおよびシステイン)レベルを測定した。
C15Sのバイオアベイラビリティは約80%であると計算され、それは5mg/kgのSC用量が投与されたHOマウスにおけるバイオアベイラビリティ(83%)とほぼ同一である。サルにおける血漿からの20NHS PEG-htCBS C15Sの排出半減期は、IVデータセットから67時間、およびSCデータセットからの73時間であると計算された。cmaxは、2mg/kgのSC用量において40.8mU/μLおよび6mg/kgのSC用量において114.9mU/μLであると計算された。
4匹のサル(メイン下位群における1M+1Fおよびリカバリー下位群における1M+1F)の3つの群が1mg/kg(低)、3mg/kg(中)、または10mg/kg(高)の20NHS PEG-htCBS C15Sのいずれかの72時間ごとの合計6回連続のSC用量を受ける反復投与実験を実施した。メイン下位群において最初の注射の0、32、72、104、144、176、216、248、288、320、360、392、および408時間後においてサルから血液を回収した一方、リカバリー下位群の動物から最初の注射の432、456、480、528、および696時間後の追加の時間において採血した。血漿を回収し、実施例1に記載の方法を使用してCBS活性および硫黄アミノ酸(ホモシステイン、シスタチオニン、システイン、ランチオニン、およびホモランチオニン)レベルについて分析した。
PEG-htCBS C15S活性の半数の減少に要求される時間は一定であった(データ示さず)。排出半減期は、16日目における最後の投与後にリカバリー動物において推定され、49~64時間であり、すなわち、20NHS PEG-htCBS C15Sの単回SC投与後にサルにおいて観察された半減期(73時間)に類似し、それは複数回投与が排出速度を変化させない証拠であった。
PEG-htCBS C15S活性を示すことが見出された(単回投与サルデータ参照)。低(1mg/kg)、中(3mg/kg)、高(10mg/kg)用量について72時間空けて6回の注射を受けたサルにおける結果を図23Bに提示する。Cthのレベルは、血漿中の20NHS PEG-htCBS C15S活性のピークおよびトラフと十分相関し、用量依存的であることも観察された。ホモシステインおよびシステインについて用量群間で明らかな差は観察されなかった(データ示さず)。
内部標準としてのシスタチオニンM+8(13C2D6)を使用するCBS活性の産物としてのシスタチオニンM+4の測定のためにLC/MS/MSを使用して開発された生物分析的方法が開発され、サルおよびヒト血漿試料の両方についてバリデートされており、したがって、それを使用して薬物動態および毒性試験ならびに臨床試験からの試料中のCBS活性を測定することができる。以下の改変で本質的に実施例1に記載のとおり酵素アッセイを実施した。重水素化D,L-ホモシステインM+4を使用して反応混合物中で未標識L-セリンとの反応を開始させた。37℃における30分間のインキュベーション後、6NのHClを使用する酸性化により酵素反応を停止させ、EZ:ファースト(faast)キット(フェノメネクス(Phenomenex)、米国)を使用して反応の標識産物(シスタチオニンM+4)を抽出した。手短に述べると、チューブ中で氷上で50μlの試料(較正標準、QC試料または酵素反応)を混合し、100μlの内部標準溶液(シスタチオニンM+8、0.25ng/μl)および250μlの水により光から保護した。ミックスをMCXカートリッジカラム(ウォーターズ(Waters))により抽出した。溶出媒体(メタノール中6Nのアンモニア)を45℃において蒸発乾固させた。乾燥残留物をEZ:ファースト(faast)キットの溶出媒体中で溶解させ、誘導体化し、キットの製造業者のプロトコルに従って脂質-脂質抽出した。抽出プレートにより固相抽出を自動化してスループットを増加させ、時間を節約した。酵素の起源(血漿試料中に存在するhtCBS C15SもしくはC15S/ME-200GSまたは対照または血漿試料中の微量の内因性サル/ヒトCBS)にかかわらず、計測可能な濃度のシスタチオニンM+4が観察された。較正範囲にはかなり留意した。それというのも、発生した測定値は、定量された濃度範囲(5.00~1500nM)のより高い部分に存在したためである。
ほとんどの場合、薬物動態応答を誘発するために要求される薬物の血漿レベルは、受容体または酵素標的の種特異的配列変動に起因して種にわたり異なる。例えば、ヒトタンパク質に対する抗体は、標的タンパク質配列の種差に起因してネズミおよびヒト標的タンパク質についての極めて異なる親和性を有し得る。20NHS PEG-htCBS C15Sの場合、生物学的標的は種にわたり同一であるアミノ酸、Hcyである。さらに、標的組織への薬物の分布は種にわたり変動し得、種特異的生物分布に起因して薬物血漿レベルにより容易に予測することができない。しかしながら、20NHS PEG/C15Sについての「標的組織」は、血漿中の薬物動態パラメータの種間比較からの外挿ヒト値の予測に高程度の確実性を与える血漿である。
A.ヒトにおける仮定反復投与PK曲線
図24Fは、定常状態血漿レベルへの接近の間、週1回投与される1mg/kgのSC用量におけるヒトにおける20NHS PEG-htCBS C15Sについての予測PK値ならびに定常状態における予測ピークおよびトラフレベル(淡灰色)を示す。図24Gは、定常状態血漿レベルへの接近の間に週2回投与される1mg/kgのSC用量におけるヒトにおける20NHS PEG-htCBS C15Sについての予測PK値ならびにこの投薬間隔におけるヒトにおける予測ピークおよびトラフレベル(淡灰色)を示す。表32は、週1回および週2回投与される0.33、0.66、および1mg/kgのSC用量におけるヒトにおける20NHS PEG-htCBS C15Sの予測ピークおよびトラフ血漿レベルを比較する(ヒトにおける80%のバイオアベイラビリティを想定)。
