いくつかの実施形態では、本発明は、タンパク質及びタンパク質をコードする遺伝子構築物を、それらを免疫応答が誘導され得る免疫原として特に有効にするエピトープと共に提供することによって、改善されたワクチンを提供する。それに応じて、ワクチンは、治療または予防免疫応答を誘導するために提供され得る。
ジカワクチンアプローチ
図2に示されるように、ジカ抗原発現構築物をそこに示される骨格を用いて生成した。発現カセットをCMVプロモーターの後方に、かつトレイリングポリアデニル化テールを用いて挿入した。カセットは、prME、NS1、及びカプシドを含む、図3に示される抗原に対するコード配列を含むことができる。
図5及び6に示されるように系統発生分析を行った。星は、コンセンサスprME配列の配列番号3の位置を示す。このコンセンサスprMEは、図7に従った発現ベクター中のクローニング部位に挿入されることが示される。
図9A及び9Bに示されるように発現したタンパク質をウエスタンブロット分析によって特性化し、それは、抗フラビウイルス抗体への特異的結合を示す。
次いで、図10A及び10Bに示されるようにタンパク質を精製した。

マウスのジカprMEワクチン接種は、ジカ-エンベロープ抗原と反応する陽性抗体応答を引き起こした。図13及び14を参照されたい。
ジカウイルスのプレ膜+エンベロープタンパク質を標的とする新規の合成DNAコンセンサスベースのワクチンが、本明細書に記載される。構築物発現の確認後、電気穿孔を通してマウス及び非ヒト霊長類を免疫化し、ワクチン接種された動物における中和活性と共に、細胞性免疫及び液性免疫の両方の誘導を示す。IFN-α/β R-/-マウスにおいて、単回または2回のいずれかの注入免疫化が、この致死的な攻撃接種モデルにおける体重減少または死亡に対して100%防御性であった。これは、人体試験に承認された最初のジカウイルスワクチンを表す。
10%ウシ胎仔血清(FBS)ならびに1%ペニシリン及びストレプトマイシンを補充したダルベッコ改変イーグル培地(DMEM;Gibco-Invitrogen)中で、ヒト胚腎臓(HEK)293T(American Type Culture Collection(ATCC)#CRL-N268,Manassas,VA)及びVero CCL-81(ATCC #CCL-81)細胞を維持し、コンフルエンス後に継代した。10%ウシ胎仔血清(FBS)ならびに1%ペニシリン及びストレプトマイシンを補充したイーグル最小必須培地(MEM;Corning-cellgro)中で、神経細胞性腫瘍細胞株SK-N-SH(ATCC HTB-11)及びU87MG(ATCC HTB-14)を維持し、コンフルエンス後に継代した。ジカウイルス株MR766(Dr.Susan Weissより寄贈)及びPR209(Bioqual,MD)の両方がVero細胞内で増幅し、ストックをVero細胞に対する標準的なプラークアッセイによって滴定した。
ジカ-prM+EnvプラスミドDNA構築物は、膜(prM)及びエンベロープ(E)タンパク質の全長前駆体をコードする。コンセンサス戦略を使用し、現在のジカprM+Eタンパク質配列の整列によってコンセンサス配列を決定した。ヒトにおける発現の強化のためにワクチンインサートを遺伝的に最適化し(すなわち、コドン及びRNA最適化)、発現を促進するためにIgEリーダー配列を付加した。構築物を商業的に合成し(Genscript,NJ)、次いで、以前に記載されているように(Muthumani et al.,2015,Sci Trans Med 7:301ra132)、サイトメガロウイルス最初期プロモーターの制御下で修飾pVax1発現ベクターにサブクローン化した。最終構築物は、ZIKV-prMEワクチンと名付けられ、対照プラスミド骨格は、pVax1である。加えて、MR766及び2016 Brazilin発生株由来のprM及びEnv遺伝子をコードする多くの他の一致したDNA構築物もまた、さらなる評価のために設計した。Inovio(Plymouth Meeting,PA)によってDNA構築物の大規模増幅を実施し、精製したプラスミドDNAを免疫化のために水中で製剤化した。アガロースゲル電気泳動を介して、DNAインサートのサイズを確認した。MEGAバージョン5ソフトウェアを使用したClustalWを用いた多重整列(Muthumani et al.,2015,Sci Trans Med 7:301ra132)によって、系統発生分析を実施した。
マウス免疫原性研究:インビボEP送達を用いて前脛骨筋に送達される全容積20~30μLの水中25μgのDNAで、雌C57BL/6マウス(6~8週齢)及びIFNAR-/-マウス(5~7週齢)を免疫化した(n=4)。免疫化の直後に、同じ部位にCELLECTRA適応定電流EPデバイス(Inovio Pharmaceuticals,PA)を用いてインビボEPを送達した。三つ又CELLECTRA低侵襲デバイスを筋肉に約2mm挿入した。26ゲージの固体ステンレス鋼電極からなる三角形3電極アレイを通じて方形波パルスを送達し、1秒の遅延によって分離した52マイクロ秒/パルスにわたって、2つの0.1アンペア定電流パルスを送達した。EPの使用に関するさらなるプロトコルは、すでに詳述されている。マウスを2週間の間隔で3回免疫化し、最終免疫化の1週間後に屠殺した。細胞性及び液免疫応答の分析のために各免疫化後に血液を採取した(Muthumani et al.,2015,Sci Trans Med 7:301ra132)。アカゲザル免疫原性研究:5匹のアカゲザルを2mgのZIKV-prMEワクチンを用いて、4週間置いて2回、2つの部位においてID免疫化した。マウス免疫化に関して記載される同じデバイスを使用して、EPをすぐに送達した。
IFNAR-/-マウスを3つの群に分けた。マウスの第1の群を1回免疫化し、免疫化の2週間後に106PFUのZIKV PR209を用いて攻撃接種した。マウスの第2の群を2週間の間隔で2回免疫化し、第2の免疫化の1週間後に106PFUのZIKV PR209を用いて攻撃接種した。マウスの第3の群を2週間の間隔で2回免疫化し、第2の免疫化の1週間後に2×106PFUのZIKV PR209を用いて攻撃接種した。攻撃接種後、動物を計量し、皮下に位置する温度チップによって体温を毎日測定した。加えて、1日2回、疾病の臨床兆候(可動性の低下、猫背の姿勢、後肢指背歩行(部分麻痺)、一方の後肢または両方の後肢の麻痺)に関して動物を観察した。愛護に基づく安楽死の基準は、20%の体重減少または任意の異常な臨床兆候の観察からなった。
インビトロ発現研究のために、製造業者のプロトコル(Agilent)に従ってGeneJammer試薬を使用して、トランスフェクションを実施した。簡潔に、35mm皿において細胞を50%コンフルエンスまで成長させ、1ugのジカprMEワクチンを用いてトランスフェクトした。トランスフェクションの2日後に細胞を採取し、リン酸緩衝生理食塩水(PBS)で2回洗浄し、細胞溶解緩衝液(Cell Signaling Technology)で溶解した。すでに記載されているように(Muthumani et al.,2015,Sci Trans Med 7:301ra132)、ウエスタンブロットを使用して、採取した細胞溶解物からのジカpreM+Envタンパク質の発現を検証した。
脾細胞の単細胞懸濁液を全てのマウスから調製した。簡潔に、10% FBS(R10)を補充した5mLのRPMI 1640中で、マウスからの脾臓を個々に採取し、次いで、Stomacher 80パドルブレンダー(A.J.Seward and Co.Ltd.)を用いて高速で30秒間処理した。処理した脾臓試料を、45mmナイロンフィルタを通して濾過し、次いで、4℃で10分間、1500rpmで遠心分離した。細胞ペレットを、室温で5分間、5mLのACK(アンモニウム-塩化物-カリウム)溶解緩衝液(Life Technologies)中に再懸濁させ、次いで、PBSを添加して反応を停止した。試料を4℃で10分間、1500rpmで再び遠心分離した。細胞ペレットを、1×107細胞/mLの濃度で、R10中で再懸濁させ、次いでELISpotアッセイ及びフローサイトメトリ分析で使用する前に45mmナイロンフィルタに通した(Muthumani et al.,2015,Sci Trans Med 7:301ra132)。RMに関して、血液(各時点において20mL)をEDTA管中で採取し、Accuspin管(Sigma-Aldrich,St.Louis,MO)を用いて、標準的なフィコール-ハイパック手順を使用して、末梢血単核細胞(PBMC)を単離した。
簡潔に、96ウェルELISpotプレート(Millipore)を抗マウスIFN-γ捕捉Ab(R&D Systems)で覆い、4℃で一晩インキュベートした。翌日、プレートをPBSで洗浄し、PBST+1% BSAで2時間遮断した。pZV-prM+Envで免疫化されたマウスからの20万個の脾細胞を各ウェルに添加し、培地単独(陰性対照)、PMA/イオノマイシンを有する培地(陽性対照)、または9個のアミノ酸による15mer重複からなり、ジカエンベロープタンパク質(Genscript)の長さに及ぶペプチドプール(1μg/mL)を有する培地の存在下で、5% CO2中37℃で一晩インキュベートした。24時間後、細胞を洗浄し、次いで、ビオチン化抗マウスIFN-γ Ab(R&D Systems)を用いて4℃で一晩インキュベートした。洗浄後、ストレプトアビジン-アルカリホスファターゼ(R&D Systems)を各ウェルに添加し、次いで、室温で2時間インキュベートした。プレートを洗浄し、次いで、5-ブロモ-4-クロロ-3′-インドリルリン酸p-トルイジン塩及びニトロブルーテトラゾリウムクロリド(色原体呈色試薬、R&D Systems)を添加した。最後に、プレートを蒸留水で洗い流し、室温で乾燥させ、スポット形成単位を自動ELISpot読み取り装置(CTL Limited)によって定量化し、未処理の値を100万個の脾細胞当たりのSFUに正規化した。RM試料に関して、サルIFN-γ用のELISPOTPRO(MABTECH)を製造業者によって説明されるように使用し、20万個のPBMCをペプチドプールで刺激し、プレートを洗浄し、以前に記載されているように(Muthumani et al.,2015,Sci Trans Med 7:301ra132;Mallilankaraman et al.,12011,PLoS Negl Trop Dis 5:e928)、スポットを発現させ、計数した。
酵素結合免疫吸着アッセイ(ELISA)を使用して、すでに記載されているように(Muthumani et al.,2015,Sci Trans Med 7:301ra132)、マウス及びRM血清の力価を決定した。簡潔に、1μg/mLの精製ジカエンベロープタンパク質を使用して、4℃で一晩、96ウェルマイクロタイタープレート(Nalgene Nunc International,Naperville,IL)を覆った。少なくとも1時間PBS中10% FBSで遮断した後、プレートを0.05% PBST(PBS中Tween20)で4回洗浄した。免疫化されたマウス及びRMからの血清試料を1% FBS、0.2% PBST中で順次に希釈し、プレートに添加し、次いで、室温で1時間インキュベートした。プレートを0.05% PBST中で再び4回洗浄し、次いで、室温で1時間、マウス血清に対して1:35000の希釈で、HRP共役抗マウスIgG(Sigma)を用いてインキュベートした。RM血清に関して、抗サルIgG HRP(Southern Biotech)を室温で1時間、1:5000の希釈で使用した。製造業者の使用説明書(Sigma Aldrich)に従って、SIGMAFAST(商標)OPD(o-フェニレンジアミン二塩酸塩)錠を添加することによって、結合した酵素を検出した。1N H2SO4を添加して、15分後に反応を停止した。次いで、プレートを450nmの光学濃度で読み取った。全てのマウス血清及びRM血清試料を二つ組で分析した。Frey et alによって記載される方法(Frey et al.,1998,J Immunol Methods 221:35-41)を使用して、エンドポイント力価を決定した。
MR766及びVero細胞を含むプラーク減少中和試験(PRNT)は、すでに記載されている(Sun et al.,2006,J Infect Dis 193:1658-65)。簡潔に、マウスまたはRM血清を血清無含有DMEM中で順次に希釈し(1:10~1:1280)、37℃で2時間、等容積のMR766ジカウイルス(100pfu)でインキュベートした。混合物をVero細胞の融合層に添加し、2時間吸着のために37℃でそのままにした。2×DMEM培地:軟寒天(1:1)重層を細胞上に添加し、プレートを37℃で5日間インキュベートした。寒天重層をウェルから除去し、細胞を4%パラホルムアルデヒドで固定し、1×PBSで洗浄し、クリスタルバイオレット溶液で染色し、1×PBSで洗浄し、プレートを乾燥させた。24ウェルプレート内で行われたアッセイにおけるプラークを手動で計数した。96ウェルプレート内で行われたアッセイにおけるプラークを自動Immunospot読み取り装置(CTL Limited)でスキャンし、試料ウェル中のプラークならびに陰性対照(DMEMのみ)及び陽性対照(100pfuのMR766ジカウイルスのみ)中のプラークを、ELISpot Readerが備わった自動ソフトウェアを使用して計数した。プラーク減少のパーセントを以下のように計算した。%減少=100×[1-(各希釈に対するプラークの平均数/陽性対照ウェル中のプラークの平均数)]。GraphPad Prismソフトウェアを使用して、%プラーク減少対各個々の血清希釈のログ変換の非線形回帰分析を実施して、ワクチン接種応答後のピークにおける実際の50% PRNT力価の線形補間を促進した。50%中和における中央値及び四分位範囲を各中和標的に対して、全体的にかつワクチン処置群によって計算し、幾何平均力価も計算した。力価は、プラーク数の50%減少をもたらす最高希釈の逆数を表す。
脾細胞を96ウェルプレート(2×106/ウェル)に添加し、タンパク質輸送阻害剤カクテル(ブレフェルジンA及びモネンシン)(eBioscience)の存在下で、37℃/5% CO2で5時間、ジカpreM及びエンベロープでプールされたペプチドを用いて刺激した。細胞刺激カクテル(加えてタンパク質輸送阻害剤)(ホルボール12-ミリステート13-アセテート(PMA)、イオノマイシン、ブレフェルジンA及びモネンシン)(eBioscience)を陽性対照として、及びR10培地を陰性対照として使用した。次いで、製造業者の使用説明書(BD,San Diego,CA)によって記載されるように、全ての細胞を表面及び細胞内タンパク質に対して染色した。簡潔に、蛍光色素共役抗体を用いた表面染色の前に、FACS緩衝液(0.1%のアジ化ナトリウム及び1% FCSを含有するPBS)中で細胞を洗浄した。製造業者のプロトコルに従ってBD Cytofix/Ctyoperm TM(BD,San Diego,CA,USA)を使用して、細胞をFACS緩衝液で洗浄し、固定し、かつ透過化し、続いて細胞内染色を行った。以下の抗体を表面染色のために使用した。LIVE/DEAD Fixable Violet Dead Cell染色キット(Invitrogen)、CD19(V450;クローン1D3;BD Biosciences)、CD4(FITC;クローンRM4-5;ebioscience)、CD8(APC-Cy7;クローン53-6.7;BD Biosciences);CD44(BV711;クローンIM7;Biolegend)。細胞内染色のために、以下の抗体を使用した。IFN-γ(APC;クローンXMG1.2;Biolegend)、TNF-α(PE;クローンMP6-XT22;ebioscience)、CD3(PerCP/Cy5.5;クローン145-2C11;Biolegend);IL-2(PeCy7;クローンJES6-SH4;ebioscience)。LSRIIフローサイトメータ(BD Biosciences)を使用して全てのデータを収集し、FlowJoソフトウェア(Tree Star,Ashland,OR)を使用して分析した。
Graphpad,Prism 4(Graphpad software,Inc.San Diego,CA)を統計分析のために利用した。Log10変換をエンドポイント結合ELISA力価及び全ウイルスPRNT50力価に適用した。
ジカprM(前駆体膜)及びE(エンベロープ)遺伝子(ZIKV-prME)のコンセンサス配列を、ヒトにおける感染を引き起こした、1952~2015年の間に単離された様々なジカからのprM及びE配列を使用して生成した(図16A)。追加の修飾後、ジカ-prMEコンセンサス配列をpVax1ベクターにクローン化し、効率の高い免疫グロブリンE(IgE)リーダーペプチド配列の付加を含む最適化を行って、そのインビボ発現を改善した(図16B)。エンドヌクレアーゼ制限消化及び遺伝子配列決定を使用して、最終ワクチンプラスミドを検証した(図16C)。84トランスフェクションの48時間後、ワクチンでトランスフェクトされた293T細胞から、ウエスタン分析及び間接免疫蛍光アッセイを実施することによって、プラスミドからのジカ-prMEタンパク質の発現を確認した(図16D及び16E)。
ジカ-prMEプラスミドワクチンが細胞免疫応答を誘導する能力を評価した。記載されるように(Muthumani et al.,2015,Sci Trans Med 7:301ra132)、筋肉内注入、続いて、92の送達部位における電気穿孔(EP)によって、2週間の間隔で3回、対照プラスミド骨格(pVax1)またはジカ-prMEプラスミドワクチンのいずれかを用いて、5匹のC57/BL6マウスの群を免疫化した。第3の注入の1週間後に動物を屠殺し、各動物から採取したバルク脾細胞を、それらがジカ-Envを包含するペプチドプールへのエクスビボ曝露後にインターフェロン-γを分泌する能力に関して、標準的な酵素結合immunospotアッセイで評価した。アッセイの結果は、ジカ-prMEで免疫化されたマウスからの脾細胞が、複数のジカ-Envペプチドプールでの刺激後に明確な細胞免疫応答をもたらしたことを示す(図17A)。全ジカ-prMEタンパク質に及ぶ15mer(11個のアミノ酸による重複)からなる22個のペプチドプールを使用した、マトリクス形式でのマッピング分析ELISpotによって、最も強力な細胞性応答(複数可)を誘発したジカ-Envの領域(複数可)を評価した。図17Bに見られるように、いくつかのプールは、高いT細胞応答を誘導したが、ペプチドプール15は、106応答当たり最高のSFUを誘導した。マッピングデータは、配列に対する1つの優性prMEエピトープ『IRCIGVSNRDFVEGM(配列番号18)』を明らかにした。免疫エピトープデータベース分析リソースIDEPコンセンサスツールを使用して、列挙された優性ペプチドが1つのH2-Db制限エピトープを含有することを確認し、この抗原の効果的な処理を示唆する。
pVax1を受けたマウスよりも、160がジカ-prMEワクチンを受けたマウスにおいて有意に高かった。本実験で使用したジカ-prMEワクチンによって誘導された中和抗体は、PRNT50力価=456を有した。ウイルスプラークの代表的な写真は、1:100の希釈の血清に関して下部に示される。
皮内(ID)免疫化によってNHPを免疫化し、続いて電気穿孔し、これは、この方法がDNAワクチンによって抗原特異的液性免疫応答を増強し得ることを示す以前の研究に基づく。アカゲザル(RM;n=5/群)に、EPで2.0mgのワクチンプラスミドIDを投与し、0日目(第1の免疫化前の免疫化前)、2週目(第1の免疫化の2週間後)、6週目(第2の免疫化の2週間後)にRMから血清及びPBMCを採取した。ワクチンによって誘導された細胞免疫応答を測定するために、図17Aで使用したジカ-Eペプチドプールを用いてエクスビボで刺激された6週目のPBMCに、ELISpot分析を実施した。結果は、ジカprME免疫化が全てのRMにおいて抗ジカT細胞応答を追加刺激し、免疫前血清における応答と比較してそれらの抗原認識を広げたことを示す(図20A)。
ジカで誘導された疾病及び免疫の機構は、はっきりと定義されず、ジカ感染に対する免疫応答の仮説的な病原性に対する防御は、今のところ不明瞭である。マウスのほとんどの株は、ジカ感染に耐性を示すが、IFN-α/β受容体(IFNAR)を欠いたマウスは、感染及び疾病にかかりやすく、6~7日間の攻撃接種16以内に最も死亡したことが分かった。コンセンサスジカ-prMEプラスミドワクチンがこのマウス株において細胞性及び液性免疫応答を誘導する能力を調査した。空の対照プラスミドを用いて、またはコンセンサスジカ-prMEプラスミドを用いて、2週間の間隔で3回、EPによってIFNARマウスの群を免疫化した。0、14、21、及び35日目に、免疫化されたマウスから血清を採取し、最終免疫化の1週間後に、脾細胞をマウスから採取した。ワクチンで免疫化されたIFNARマウスからの脾細胞は、ELISpotアッセイにおける106個の細胞当たりのSFUレベルによって示されるように、明確な細胞免疫応答をもたらした(図22A)。rZIKA-Envを捕捉抗原として使用するELISAからの結果は、動物が14日目までに検出可能な抗ジカ血清IgGを有したことを示し、これらのレベルを後続の採取時間において追加刺激した(図22B)。ワクチン接種されたマウスからの血清は、エンドポイント力価によって示されるように、有意なレベルの抗体を含有した(図22B)。結果は、コンセンサスジカ-prMEワクチンで免疫化されたIFNARマウスが、抗ジカ細胞性及び液性免疫応答を可能にすることを示し、この潜在的な攻撃接種モデルにおけるワクチン防御に関するさらなる研究を支持する。
本研究において、ZIKVのプレ膜+エンベロープタンパク質(prMEnv)を標的とする新規の合成DNAワクチンを生成し、インビボ有効性に関して評価した。プラスミド構築物の初期のインビトロ発生及び評価研究後に、電気穿孔を介した強化DNA送達を通して、このprMEnv DNAベースの免疫原を用いて、マウス及び非ヒト霊長類を免疫化した。ワクチン接種された動物は、抗原特異的細胞性及び液性免疫ならびに中和活性をもたらすことが分かった。インターフェロン(IFN)-α/β受容体を欠いたマウス(指定されたIFNAR-/-)において、このDNAワクチンを用いた免疫化は、インビボウイルス攻撃接種後に、脳組織内のウイルス病理を予防することに加えて、感染に関連した体重減少または死亡に対して100%の防御を誘導した。加えて、非ヒト霊長類抗ZIKV免疫血清の受動的移入は、後続のウイルス攻撃接種に対してIFNAR-/-マウスを防御した。病原性マウスモデルにおけるこのZIKVワクチンのこの初期研究は、ZIKV感染におけるprMEを標的とする免疫応答の重要性を裏付け、このワクチンアプローチに対する追加の研究が、ヒトにおけるZIKV対照に関連性があり得ることを示唆する。
10%ウシ胎仔血清(FBS)ならびに1%ペニシリン及びストレプトマイシンを補充したDMEM(ダルベッコ改変イーグル培地;Gibco-Q3 Invitrogen)中で、ヒト胚腎臓293T(American Type Culture Collection(ATCC)#CRL-N268,Manassas,VA,USA)及びVero CCL-81(ATCC #CCL-81)細胞を維持し、コンフルエンス後に継代した。ジカウイルス株MR766(Dr Susan Weissより寄贈)及びPR209(Bioqual,MD)の両方がVero細胞内で増幅し、ストックをVero細胞に対する標準的なプラークアッセイによって滴定した。National Institutes of Health,Wistar及びPublic Health Agency of Canada IACUC(Institutional Animal Care and Use Committee)のガイドラインに従って、5~6週齢の雌C57BL/6(The Jackson Laboratory)及びIFNAR-/-(MMRRCリポジトリ-The Jackson Laboratory)マウスを収容し、温度制御された光サイクルの施設で処理/ワクチン接種した。
膜(prM)+エンベロープ(E)及びカプシドタンパク質の全長前駆体をコードするZIKV-prMEプラスミドDNA構築物を合成した。コンセンサス戦略を使用し、現在のZIKV prMEタンパク質配列の整列によってコンセンサス配列を決定した。ヒトにおける発現の強化のためにワクチンインサートを遺伝的に最適化し(すなわち、コドン及びRNA最適化)、発現を促進するためにIgEリーダー配列を付加した。構築物を商業的に合成し(Genscript,NJ,USA)、次いで、以前に記載されているように(Muthumani et al.,2016,Sci Transl Med 7:301ra132)、サイトメガロウイルス最初期プロモーターの制御下で修飾pVax1発現ベクターにサブクローン化した。最終構築物は、ZIKV-prMEワクチンと名付けられ、対照プラスミド骨格は、pVax1である。加えて、MR766(DQ859059.1)及び2016 Brazilin(AMA12084.1)発生株由来のprM及びE遺伝子をコードする多くの他の一致したDNA構築物もまた、さらなる評価のために設計した。Inovio Pharmaceuticals Inc.(Plymouth Meeting,PA,USA)によってDNA構築物の大規模増幅を実施し、精製したプラスミドDNAを免疫化のために水中で製剤化した。アガロースゲル電気泳動を介して、DNAインサートのサイズを確認した。MEGAバージョン5ソフトウェアを使用したClustalWを用いた多重整列(Muthumani et al.,2016,Sci Transl Med 7:301ra132)によって、系統発生分析を実施した。
インビボ電気穿孔送達を用いて前脛骨筋に送達される全容積20~30μLの水中25μgのDNAで、雌C57BL/6マウス(6~8週齢)及びIFNAR-/-マウス(5~6週齢)を免疫化した。DNA注入の直後に、同じ部位にCELLECTRA適応定電流電気穿孔デバイス(Inovio Pharmaceuticals)を用いてインビボ電気穿孔を送達した。三つ又CELLECTRA低侵襲デバイスを筋肉に約2mm挿入した。26ゲージの固体ステンレス鋼電極からなる三角形3電極アレイを通じて方形波パルスを送達し、1秒の遅延によって分離した52μs/パルスにわたって、2つの0.1アンペア定電流パルスを送達した。電気穿孔の使用に関するさらなるプロトコルは、すでに詳述されている(Flingai et al.,2015,Sci Rep 5:12616)。マウスを2週間の間隔で3回免疫化し、最終免疫化の1週間後に屠殺した。細胞性及び液免疫応答の分析のために各免疫化後に血液を採取した(Muthumani et al.,2016,Sci Transl Med 7:301ra132)。アカゲザル免疫原性研究:5匹のアカゲザルを2mgのZIKV-prMEワクチンを用いて、5週間の間隔で2回、2つの部位において皮内で免疫化した。マウス免疫化に関して記載される同じデバイスを使用して、電気穿孔をすぐに送達した。
インビトロ発現研究のために、製造業者のプロトコル(Agilent)に従ってGeneJammer試薬を使用して、トランスフェクションを実施した。簡潔に、35mm皿において細胞を50%コンフルエンスまで成長させ、1μgのZIKV-prMEワクチンを用いてトランスフェクトした。トランスフェクションの2日後に細胞を採取し、PBSで2回洗浄し、細胞溶解緩衝液(Cell Signaling Technology)で溶解した。すでに記載されているように(Muthumani et al.,2016,Sci Transl Med 7:301ra132)、ウエスタンブロットを使用して、ZIKV-prMEをワクチン接種されたマウスからの抗フラビウイルスまたは免疫血清のいずれかを使用して、採取した細胞溶解物からのZIKV-prMEタンパク質の発現ならびにマウス及びRM血清の免疫特異性を検証した。簡潔に、3~12% Bis-Tris NuPAGEゲル(Life Technologies)に、5μgまたは1μgのZIKVエンベロープ組み換えタンパク質(rZIKV-E);トランスフェクトされた細胞溶解物または上清、及びOdysseyタンパク質分子量マーカー(製品番号928-40000)を搭載した。MOPS緩衝液中で50分間、200Vでゲルを実行した。iBlot 2 Gel Transfer Device(Life Technologies)を使用して、タンパク質をニトロセルロース膜上に移した。膜を室温で1時間、PBS Odyssey遮断緩衝液(LI-COR Biosciences)で遮断した。ワクチン発現を検出するために、抗フラビウイルス型抗原(MAB10216-Clone D1-4G2-4-15)抗体を1:500に希釈し、マウス及びRMからの免疫血清を、0.2% Tween 20(Bio-Rad)を有するOdyssey遮断緩衝液中で1:50に希釈し、4℃で一晩膜を用いてインキュベートした。膜をPBSTで洗浄し、次いで、室温で1時間、マウス血清に対して1:15,000の希釈で、マウス血清及びフラビウイルス抗体に対して適切な二次抗体(ヤギ抗マウスIRDye680CW;LI-COR Biosciences)、ならびにRM血清に対してヤギ抗ヒトIRDye800CW(LI-COR Biosciences)を用いてインキュベートした。洗浄後、膜をOdyssey赤外線画像装置(LI-COR Biosciences)上で撮像した。
免疫蛍光アッセイに関して、細胞をカバースリップ上で成長させ、5μgのZIKV-prMEワクチンでトランスフェクトした。トランスフェクションの2日後、細胞を4%パラホルムアルデヒドで15分間固定した。次いで、室温で1時間、PBS中で希釈した正常ヤギ血清を用いて、非特異的結合を遮断した。次いで、スライドをPBS中で5分間洗浄し、続いて4℃で一晩、1:100の希釈で、免疫化されたマウスまたはRMからの血清を用いてインキュベートした。上記のようにスライドを洗浄し、室温で1時間、1:200の希釈で、適切な二次抗体(マウス血清に対してヤギ抗マウスIgG-AF488、及びRM血清に対してヤギ抗ヒトIgG-AF488;Sigma)を用いてインキュベートした。洗浄後、DAPIを有するFlouroshield封入培地(Abcam)を添加して、全ての細胞核を染色した。その後、カバースリップを載置し、スライドを顕微鏡(EVOS Cell Imaging Systems;Life Technologies)下で観察した(Muthumani et al.,2016,Sci Transl Med 7:301ra132)。加えて、Vero、SK-N-SH、またはU87-MB細胞を、4チャンバ組織培養処理ガラススライド上で成長させ、0.01のMOIで、RM免疫血清(1:200)を用いて/用いずに予めインキュベートしたZIKV-MR766またはPR209を用いて感染させ、記載されるように(Rossi et al.,2016,J Rop Med Hyg 94:1362-9)、pan flavirus抗体を使用して、ZIKV感染後4日目に染色した。
病理組織学のために、ホルマリン固定パラフィン包埋された脳組織を厚さ5μmの矢状断面に分割し、Superfrost顕微鏡スライド(Fisher Scientific)上に配置し、37℃で一晩支持した。2回のキシレン変化を使用して切片を脱パラフィンし、100%、90%、次いで70%エタノール中に浸漬することによって再水和した。切片を、2分間ハリスヘマトキシリン(Surgipath)を使用して核構造に対して染色し、続いて、1%酸アルコール(Surgipath)中で分化し、2分間Scott’s tap waterで処理した。続いて、切片を、2分間エオシン(Surgipath)を使用して細胞質構造に対して対比染色した。スライドを70%、90%、及び100%エタノールで脱水し、キシレン中で除去し、Permount(Fisher Scientific)を使用して載置した。
脾細胞の単細胞懸濁液を全てのマウスから調製した。簡潔に、10% FBS(R10)を補充した5mLのRPMI 1640中で、マウスからの脾臓を個々に採取し、次いで、Stomacher 80パドルブレンダー(A.J.Seward and Co.Ltd.)を用いて高速で30秒間処理した。処理した脾臓試料を、45mmナイロンフィルタを通して濾過し、次いで、4℃で10分間、1500gで遠心分離した。細胞ペレットを、室温で5分間、5mLのACK(アンモニウム-塩化物-カリウム)溶解緩衝液(Life Technologies)中に再懸濁させ、次いで、PBSを添加して反応を停止した。試料を4℃で10分間、1500gで再び遠心分離した。細胞ペレットを、R10中で再懸濁させ、次いでELISpotアッセイ及びフローサイトメトリ分析で使用する前に45mmナイロンフィルタに通した(Muthumani et al.,2016,Sci Transl Med 7:301ra132)。RMに関して、血液(各時点において20mL)をEDTA管中で採取し、Accuspin管(Sigma-Aldrich,St.Louis,MO,USA)を用いて、標準的なフィコール-ハイパック手順を使用して、PBMCを単離した。5ミリリットルの血液も、血清単離のために各時点において血清管に採取した。
脾細胞を96ウェルプレート(2×106/ウェル)に添加し、タンパク質輸送阻害剤カクテル(ブレフェルジンA及びモネンシン;eBioscience)の存在下で、37℃/5% CO2で5時間、ZIKV-preMでプールされたペプチドを用いて刺激した。細胞刺激カクテル(加えてタンパク質輸送阻害剤;PMA(ホルボール12-ミリステート13-アセテート)、イオノマイシン、ブレフェルジンA及びモネンシン;eBioscience)を陽性対照として、及びR10培地を陰性対照として使用した。次いで、製造業者の使用説明書(BD Biosciences,San Diego,CA,USA)によって記載されるように、全ての細胞を表面及び細胞内タンパク質に対して染色した。簡潔に、蛍光色素共役抗体を用いた表面染色の前に、FACS緩衝液(0.1%のアジ化ナトリウム及び1% FBSを含有するPBS)中で細胞を洗浄した。製造業者のプロトコルに従ってBD Cytofix/Ctyoperm TM(BD Biosciences)を使用して、細胞をFACS緩衝液で洗浄し、固定し、かつ透過化し、続いて細胞内染色を行った。以下の抗体を表面染色のために使用した。LIVE/DEAD Fixable Violet Dead Cell染色キット(Invitrogen)、CD19(V450;クローン1D3;BD Biosciences)、CD4(FITC;クローンRM4-5;eBioscience)、CD8(APC-Cy7;クローン53-6.7;BD Biosciences);CD44(BV711;クローンIM7;BioLegend)。細胞内染色のために、以下の抗体を使用した。IFN-γ(APC;クローンXMG1.2;Biolegend)、TNF-α(PE;クローンMP6-XT22;eBioscience)、CD3(PerCP/Cy5.5;クローン145-2C11;BioLegend);IL-2(PeCy7;クローンJES6-SH4;eBioscience)。LSRIIフローサイトメータ(BD Biosciences)を使用して全てのデータを収集し、FlowJoソフトウェア(Tree Star,Ashland,OR,USA)を使用して分析した。
簡潔に、96ウェルELISpotプレート(Millipore)を抗マウスIFN-γ捕捉Ab(R&D Systems)で覆い、4℃で一晩インキュベートした。翌日、プレートをPBSで洗浄し、PBST+1% BSAで2時間遮断した。免疫化されたマウスからの20万個の脾細胞を各ウェルに添加し、培地単独(陰性対照)、PMA/イオノマイシンを有する培地(陽性対照)、または9個のアミノ酸による15mer重複からなり、ZIKV prMEタンパク質(Genscript)の長さに及ぶペプチドプール(1μg/mL)を有する培地の存在下で、5% CO2中37℃で一晩インキュベートした。24時間後、細胞を洗浄し、次いで、ビオチン化抗マウスIFN-γ Ab(R&D Systems)を用いて4℃で一晩インキュベートした。洗浄後、ストレプトアビジン-アルカリホスファターゼ(R&D Systems)を各ウェルに添加し、次いで、室温で2時間インキュベートした。プレートを洗浄し、次いで、5-ブロモ-4-クロロ-3′-インドリルリン酸p-トルイジン塩及びニトロブルーテトラゾリウムクロリド(色原体呈色試薬、R&D Systems)を添加した。最後に、プレートを蒸留水で洗い流し、室温で乾燥させ、SFUを自動ELISpot読み取り装置(CTL Limited)によって定量化し、未処理の値を100万個の脾細胞当たりのSFUに正規化した。RM試料に関して、サルIFN-γ用のELISPOTPROキット(MABTECH)を製造業者によって説明されるように使用し、20万個のPBMCをペプチドプールで刺激し、プレートを洗浄し、以前に記載されているように(Muthumani et al.,2016,Sci Transl Med 7:301ra132)、スポットを発現させ、計数した。
ELISAを使用して、すでに記載されているように(Muthumani et al.,2016,Sci Transl Med 7:301ra132)、マウス及びRM血清の力価を決定した。簡潔に、1μgの精製rZIKV-Eタンパク質を使用して、4℃で一晩、96ウェルマイクロタイタープレート(Nalgene Nunc International,Naperville,IL,USA)を覆った。少なくとも1時間PBS中10% FBSで遮断した後、プレートを0.05% PBST(PBS中Tween20)で4回洗浄した。免疫化されたマウス及びRMからの血清試料を1% FBS中で順次に希釈し、プレートに添加し、次いで、室温で1時間インキュベートした。プレートを0.05% PBST中で再び4回洗浄し、次いで、室温で1時間、マウス血清に対して1:35,000の希釈で、HRP共役抗マウスIgG(Sigma)を用いてインキュベートした。RM血清に関して、抗サルIgG HRP(Southern Biotech)を室温で1時間、1:5,000の希釈で使用した。製造業者の使用説明書(Sigma-Aldrich)に従って、SIGMAFAST OPD(o-フェニレンジアミン二塩酸塩)基質溶液を添加することによって、結合した酵素を検出した。1N H2SO4を添加して、15分後に反応を停止した。450nmの光学濃度をSynergyプレートリーダー上で読み取った。全てのマウス及びRM血清試料を二つ組で分析した。すでに記載されている方法(Frey et al.,1998,J Immunol Methods 21:35-41)を使用して、エンドポイント力価を決定した。
MR766及びVero細胞を含むPRNTは、すでに記載されている(Sun et al.,2006,J Infect Dis 193:1658-65)。簡潔に、熱不活性化マウスまたはRM血清を血清無含有DMEM中で順次に希釈し(1:10~1:1280)、37℃で2時間、等容積のZIKV MR766(100PFU)でインキュベートした。混合物をVero細胞の融合層に添加し、2時間吸着のために37℃でそのままにした。2×DMEM培地:軟寒天(1:1)重層を細胞上に添加し、プレートを37℃で5日間インキュベートした。寒天重層を除去し、細胞を4%パラホルムアルデヒドで固定し、1×PBSで洗浄し、クリスタルバイオレット溶液で染色し、1×PBSで洗浄し、プレートを乾燥させた。24ウェルプレート内で行われたアッセイにおけるプラークを自動Immunospot読み取り装置(CTL Limited)でスキャンし、試料ウェル中ならびに陰性対照(DMEMのみ)及び陽性対照(100PFU MR766 ZIKVウイルスのみ)ウェル中のプラークを、ELISpotリーダーが備わった自動ソフトウェアを使用して計数した。プラーク減少の割合を以下のように計算した。%減少=100×{1-(各希釈に対するプラークの平均数/陽性対照ウェル中のプラークの平均数)}。GraphPad Prismソフトウェアを使用して、%プラーク減少対各個々の血清希釈のログ変換の非線形回帰分析を実施して、ワクチン接種応答後のピークにおける実際の50% PRNT力価の線形補間を促進した。50%中和における中央値及び四分位範囲を各中和標的に対して、全体的にかつワクチン処置群によって計算し、幾何平均力価も計算した。力価は、プラーク数の50%減少をもたらす最高希釈の逆数を表す。
ジカ攻撃接種研究のために、IFNAR-/-マウス(n=10/群)を、ジカ-prMEワクチンまたはpVax1を用いて1回または2回免疫化した。マウスは、15日目(単一の免疫化群)、または第2の免疫化の1週間後の21日目(2つの免疫化群)に、1×106PFUまたは2×106PFUのZIKV-PR209ウイルスのいずれかを有した。また、IFNAR-/-マウス(n=10/群)の追加の群を1回免疫化し、15日目に2×106PFUのZIKV-PR209ウイルスを用いて攻撃接種した。攻撃接種後、動物を計量し、皮下に位置する温度チップによって体温を毎日測定した。加えて、1日2回、疾病の臨床兆候(可動性の低下、猫背の姿勢、後肢指背歩行(部分麻痺)、一方の後肢または両方の後肢の麻痺)に関して動物を観察し、ウイルス負荷決定のために採血した。愛護に基づく屠殺の基準は、20%の体重減少または一方もしくは両方の後肢の麻痺からなった。
処理されたマウスからの脳を4℃で1週間RNAlater(Ambion)中に浸漬し、次いで-80℃で保管した。次いで、脳組織を計量し、ステンレス鋼ビーズを有するTissueLyser(Qiagen)を使用して、30サイクル/秒で6分間、2mLクライオバイアル中600μLのRLT緩衝液中で均質化した。RNeasy Plusミニキット(Qiagen)を用いて、血液からウイルスRNAも単離した。ZIKV特異的リアルタイムRT-PCRアッセイを、対象の動物からのウイルスRNAの検出のために利用した。TaqMan Fast Virus 1-Step Master Mix(Applied Biosystems)を有するプライマーZIKV 835及びZIKV 911cならびにプローブZIKV 860FAMを使用して、RNAを逆転写及び増幅した。各プレートに対して標準曲線を並行して生成し、ウイルスゲノムコピー数の定量化のために使用した。StepOnePlus Real-Time PCR System(ABI)ソフトウェアバージョン2.3を使用して、サイクル閾値(Ct)の値を計算し、複製のうちの少なくとも1つに対するCt値≦38を、すでに記載されているように(Lanciotti et al.,2008,Emerg Infect Dis 14:1232-9)、陽性と見なした。出血前は、このアッセイにおいて陰性であった。
抗体力価の倍の増加の差を、マン-ホイットニー分析を使用して比較した。GraphPad,Prism 4(GraphPad Software,Inc.San Diego,CA,USA)を使用して、統計分析を実施した。全ての分析に対して、P<0.05を有意と見なした。Log10変換をエンドポイント結合ELISA力価及び全ウイルスPRNT50力価に適用した。
ZIKV prM(前駆体膜)及びEnv(エンベロープ)遺伝子(ZIKV-prME)のコンセンサス配列を、ヒトにおける感染を引き起こした、1952~2015年の間に単離された様々なZIKVからのprM及びEnv配列を使用して生成した。追加の修飾後、ZIKV-prMEコンセンサス配列をpVax1ベクターにクローン化し、効率の高い免疫グロブリンE(IgE)リーダーペプチド配列の付加を含む最適化を行って、そのインビボ発現を改善した(図24A)。Discovery Studio 4.5上の相同モデル及び可視化を使用して、ZIKV-エンベロープ配列の最適整列を実施した。参照モデルは、PDB 5JHM及びPDB 5IZ7を含んだ。prME抗原上の特異的領域に対応する整列した残基を、可視化目的でモデルにおいて標識化した(図24B)。最適化されたコンセンサスワクチン選択は、概して、本研究において分析された複数のZIKV株に対して保存的または半保存的である。EDE特異的中和抗体の構造研究は、これらの認識決定基がエンベロープ-ダイマー表面における血清型不変部位に見られ、それが、融合ループの露出した主鎖ならびに2つの保存グリカン鎖(N67及びN153結合グリカン)を含むことを明らかにした(Rouvinski et al.,2015,Nature 520:109-13)。これらの2つのグリコシル化部位は、他のフラビウイルスにおいて高度に保存されない。さらに、ZIKVは、N67結合グリコシル化部位を有さず、N154結合グリコシル化部位(デングのN153結合グリコシル化部位と同等)は、単離されたZIKV株のうちのいくつかにおいて存在しない。したがって、コンセンサス設計の一部として、このグリコシル化部位を除外して構築物を設計した。この部位におけるグリコシル化の欠如は、ZIKV-エンベロープタンパク質へのEDE1型広域中和抗体(bnAb)の改善された結合と関連付けられている(Rouvinski et al.,2015,Nature 520:109-13)。
ZIKV-prMEnvプラスミドワクチンが細胞免疫応答を誘導する能力を評価した。筋肉内(i.m.)注入、続いて、送達部位における電気穿孔を通して、2週間の間隔で3回、対照プラスミド骨格(pVax1)またはZIKV-prMEプラスミドワクチンのいずれかを用いて、4匹のC57BL/6マウスの群を免疫化した(図25A)。第3の注入の1週間後に動物を屠殺し、各動物から採取したバルク脾細胞を、それらがZIKV-prMEを包含するペプチドプールへのエクスビボ曝露後にインターフェロン-γ(IFN-γ)を分泌する能力に関して、ELISpotアッセイで評価した。アッセイの結果は、ZIKV-prMEで免疫化されたマウスからの脾細胞が、複数のZIKV-Eペプチドプールでの刺激後に細胞免疫応答をもたらしたことを示す(図25B)。全ZIKV-prMEタンパク質に及ぶ15mer(11個のアミノ酸による重複)からなる22個のペプチドプールを使用した、マトリクス形式でのELISpotアッセイによって、最も強力な細胞性応答(複数可)を誘発したZIKVEnvの領域(複数可)を評価した。いくつかのプールは、高いT細胞応答を示したが、ペプチドプール15は、スポット形成単位(SFU)の最高値を呈した(図25C)。このマトリクスマッピング分析は、優性prMEエピトープ『IRCIGVSNRDFVEGM(配列番号17)』を明らかにした(aa167-181)。免疫エピトープデータベース分析リソースツールを利用した分析を通して、このペプチドがH2-Db制限エピトープを含有することを確認し、このハプロタイプにおいて、抗原が効果的に処理されることを裏付ける。
これらの研究の開始時に、特異的抗ZIKV免疫応答を評価するために使用可能な市販の試薬はなかった。したがって、必要に迫られて、組み換えZIKV-エンベロープタンパク質(rZIKV-E)を生成して、本研究で実施されるアッセイを支持した。この試薬を生成するために、ZIKV-prMEワクチンコンセンサス抗原に基づくコンセンサスZIKV-エンベロープ配列を、pET30a Escherichia coli発現ベクターにクローン化した(図32A)。rZIKV-E抗原をE.coli培養物中で産生し、ニッケルカラムクロマトグラフィを使用して精製し、SDS-PAGEを使用して分析し、それは、抗His tag抗体を使用したウエスタン分析によって検出され得る、rZIKV-Eでトランスフェクトされた細菌からの溶解物中に、予測されたサイズの過剰発現されたタンパク質を示した(図32B)。ZIKV-prMEワクチンで免疫化されたマウスからの血清は、ELISA(酵素結合免疫吸着アッセイ;図32C)において捕捉抗原として使用したrZIKV-Envに結合した。複数のフラビウイルスのエンベロープタンパク質に反応する市販の抗体(指定されたpanflavivirus)もまた、rZIKV-Eに結合した。ウエスタン分析は、ZIKV-prMEnvで免疫化されたマウスからの免疫血清が、rZIKV-Eを特異的に認識したことを実証した(図32D)。これらのデータは、生成されたrZIKV-Eが、ZIKV-prMEnvをワクチン接種されたマウスからの免疫血清と特異的に反応したことを示し、したがって、この組み換えタンパク質をさらなる免疫原性研究のために使用した。
コンセンサスZIKV-prMEnvワクチンがマウスにおける液性免疫応答を誘導する能力を評価した。25μgの空の対照pVax1またはコンセンサスZIKV-prMEnvワクチンプラスミドのいずれかを用いて、2週間の間隔で3回、電気穿孔を介した送達を通して、4匹のC57BL/6マウスの群を筋肉内(i.m.)で免疫化した。免疫化された各マウスから血清を得て、捕捉抗原として固定化rZIKV-Eを使用して、ELISAによって、ZIKV特異的IgG応答に関して試験した。抗ZIKV特異的IgGの有意な増加が21日目に観察され、血清IgGレベルにおけるさらなる追加刺激が35日目に認められた(図26A)。ワクチン接種された動物からの60日目の血清は、高いZIKV特異的抗体応答が最終追加刺激後に長期間維持されたことを示す。最も重要なことには、ワクチン接種されたマウスからの血清が、エンドポイント力価によって示されるように非常に高いレベルのrZIKV-E特異的抗体を含有した(図26B)。ウエスタン分析によってrZIKV-E(エンベロープ)を検出する能力(図26C)、及び免疫蛍光アッセイによってZIKV(MR766株)感染細胞を染色する能力(図26D)に関して、ZIKVprMEnvプラスミドを接種されたマウスからのプールされた血清をスクリーニングすることによって、ワクチンによって誘導された抗体の特異性の追加の評価を実施した。これらの分析の両方からの結果は、ワクチンによって誘導された液性応答の特異性を確認した。
ZIKVで誘導された疾病及び免疫の機構は、はっきりと定義されず、ZIKV感染に対する免疫応答の仮説的な病原性に対する防御は、今のところ不明瞭である(Rossi et al.,2016,J Rop Med Hyg 94:1362-9)。マウスのほとんどの株は、ZIKV感染に耐性を示すが、IFN-α/β受容体(IFNAR-/-)を欠いたマウスは、感染及び疾病にかかりやすく、攻撃接種後6~7日以内に最も死亡したことが分かった(Lazear et al.,2016,Cell Host Microbe 19:720-30)。コンセンサスZIKV-prMEプラスミドワクチンがこのマウス株において細胞性及び液性免疫応答を誘導する能力を調査した。対照pVax1プラスミドまたはZIKV prMEワクチンプラスミドワクチンのいずれかを用いて、2週間の間隔で3回、電気穿孔を介した送達で、5~6週齢の雌IFNAR-/-マウス(n=4)をi.m.で免疫化した。0、14、21、及び35日目に、免疫化されたマウスから血清を採取し、最終免疫化の1週間後(35日目)に、脾細胞をマウスから採取した。ワクチンで免疫化されたマウスからの脾細胞は、ELISpotアッセイにおける106個の細胞当たりのSFUレベルによって示されるように、明確な細胞免疫応答をもたらした(図33A)。rZIKV-Eを捕捉抗原として使用するELISA分析からの結果は、14日目までの検出可能な抗ZIKV血清IgG(約1:1,000の力価)を示し、これらのレベルを後続のワクチン接種で追加刺激し、結合抗体力価は、少なくとも1:100,000に達した(図33B及び33C)。比較すると、免疫化後35日目の試料に対するPRNT50力価は、1:60であった。結果は、コンセンサスZIKV-prMEnvワクチンで免疫化されたIFNAR-/-マウスが、抗ZIKV細胞性及び液性免疫応答を生成することが可能であることを示し、この病原性攻撃接種における推定的ワクチン効果のこのモデルにおけるさらなる研究を支持する。
より低い電圧の皮内形式での強力な免疫応答を示す近年の研究(Hutnick et al.,2012,Hum gene Ther 23:943-50;Broderick et al.,Mol Ther Nucleic Acids 1:e11)に基づき、皮内電気穿孔を使用して、NHPを皮内免疫化によって免疫化した。アカゲザル(RM;n=5/群)に、電気穿孔を用いて2.0mgのワクチンプラスミドを皮内投与し、各動物に4週間置いて2回ワクチン接種した。血清及び末梢血単核細胞(PBMC)を、0日目(免疫化前)及び6週目(第2の免疫化の2週間後)に採取した。ZIKV-prMEnvペプチドプールを用いてエクスビボで刺激された免疫化前及び6週目のPBMCのELISpot分析は、ZIKV-prMEnv免疫化がRMにおいて堅固な抗ZIKV T細胞応答を誘導したことを示した(図27A)。
予備的研究において、5~6週齢のIFNAR(-/-)マウス(n=10)を、皮下(s.c.)、腹腔内(i.p.)、頭蓋内、または静脈内(i.v.)経路のいずれかによって投与される1×106プラーク形成単位(PFU)のZIKV-PR209分離株を用いて攻撃接種した。攻撃接種後、全ての動物を、体重の日常測定、ならびに後肢虚弱及び麻痺などの瀕死状態の他の兆候の検査を含んだ、感染の臨床兆候に関して監視した。接種後の最初の4日間、マウスの全身外観の変化は観察されなかった。しかしながら、4日目以降、群の各々におけるマウスは、全体的な活性の低下、可動性の低下、及び猫背の姿勢を示し、多くの場合、後肢虚弱、水分摂取の低下、及び明らかな体重減少を伴った。ウイルス攻撃接種経路に関わらず、動物は6日目~8日目の間に感染で死亡した(図35A~35E)。これらのデータに基づき、このモデルにおけるZIKV-prMEを介した防御を評価するための後続の研究は、攻撃接種のためにs.c.経路を使用した。
次に、ZIKV-prMEnvをワクチン接種されたRMからの免疫血清の移入がIFNAR-/-マウスにおけるZIKV媒介病因を予防するかどうかを試験した。この目的で、6週目のRMからの150μg当量のIgG(PRNT50≒1/160)を、ZIKVウイルス攻撃接種の1日後に、IFNAR-/-マウスに養子導入した。対照マウスの2つの群を含み、一方の群は、RMからの免疫前血清が投与され、もう一方の群は、リン酸緩衝生理食塩水(PBS)が投与された。PBSまたは対照血清が投与されたマウスは、感染の過程中に元の体重の15~25%が減少し、全てが感染の6~8日後に死亡した。RMからのワクチン免疫血清を感染しやすいマウスに移入したとき、動物は、3及び4日目に体重が減少したが、その後、5日目に体重が戻り始め、最終的に80%が感染性の攻撃接種を生き延び(図30A)、NHP血清移入がウイルス攻撃接種後のZIKV感染の臨床症状に対する防御を付与する能力を実証する(図30B)。ZIKVを用いた攻撃接種に対する防御に対する免疫血清移入の効果を評価するために実施された反復実験において、ZIKV-prME免疫血清レシピエント間の生存は、80~100%に及んだ。これらの研究は、抗ZIKVワクチン免疫血清が、必要とされる適応性抗ZIKV免疫応答の非存在下で、ZIKV感染に対する有意な防御を付与する能力を有したことを示す。
近年のZIKVの蔓延及びヒトにおけるその関連した病因によって、深刻な問題が生じている。現在、この新しい感染性病原体に対する認可されたワクチンまたは治療薬は存在していない。ごく最近、実験用ZIKVワクチンの採取が非病原性動物感染モデルにおいて攻撃接種後のウイルス負荷を低下させることが示されている(Larocca et al.,2016,Nature 536:474-8;Abbink et al.,2016,Science 353:1192-32)。これらのデータは期待が持てる。この点で、追加のモデルにおいてジカを標的とする追加の新規のワクチンアプローチを検査することが重要である。ここで、新規のコンセンサスZIKV-prM及びE抗原を発現させるために設計された合成DNAワクチンを、マウス及び非ヒト霊長類における電気穿孔によって強化された免疫化後の免疫原性に関して評価した。ZIKV-prME DNAワクチン接種が、マウス及びNHPに両方において免疫原性であり、抗原特異的T細胞ならびに結合抗体及び中和抗体を生成したことが観察された。独自に、近年記載されているように(Trimble et al.,2016,Lancet 386:2078-88)、NHPを皮内経路による電気穿孔を通してZIKV-prMEで免疫化し、それは、i.m.電気穿孔よりも低い電圧及び小さいトランスフェクション領域を使用する。そのようなアプローチのさらなる研究は、臨床状況において利点を提供し得る。
アカゲザルを、2つの部位のそれぞれにおいて1mgとして0及び4週目に投与された2mgのZIKV-prMEプラスミドを用いて、皮内(i.d.)で免疫化し、免疫化の直後に皮内電気穿孔(EP)した。PBMCを免疫化前及び6週目に単離し、ELISPOTアッセイのために使用して、ZIKV-prMEペプチドを用いた刺激に応答するIFN-g分泌細胞を検出した(図36A)。1回の免疫化を受けたNHP及び2回の免疫化を受けたNHPは、全prMEタンパク質を包含する6つのペプチドプールに対して100万個のPBMC当たり得られたIFN-g産生細胞の増加を示し(図36B及び図36C)、ZIKV-prMEワクチン接種後のZIKV特異的細胞免疫応答の誘導を実証する。図37に示されるように、抗ZIKV抗体応答は、NHPのZIKV-prMEワクチン接種によって誘導される。
ジカ-001臨床プロトコル
最初の第1相試験は、デングウイルス未投与成人においてID投与、続いてEPされたGLS-5700の安全性、耐性、及び免疫原性を評価するための非盲検、投与量決定試験であり、米国及びカナダの3つの現場で実施した。
ジカ-001の2つの群がある(表1)。参加者(1群当たりn=20)に、2つの用量レベル:1mgまたは2mgのDNA/用量のうちの1つで、GLS-5700を投与する。0.1mLのID注入としてワクチンを投与し、続いてCELLECTRA(登録商標)-3Pデバイスを用いてEPする。参加者は、0、4、及び12週目に三角筋部に1回または2回の注入を受ける(3回のワクチン接種の連続)。
a.年齢18~65歳;
b.参加に同意することができ、同意説明文書(ICF)に署名している;
c.全ての研究手順に従うことが可能であり、かつその意思がある;
d.妊娠の可能性がある女性は、医学的に有効な避妊法(経口避妊、バリア法、殺精子剤など)を使用すること、または登録から最後の注入後3か月間無精子のパートナーを有すること、もしくは妊娠を引き起こすことが医学的に不可能なパートナーを有することに同意する。
e.性的に繁殖力があると見なされる性的に活発な男性は、研究中にいずれかのバリア式避妊法を使用することに同意しなければならず、かつ最後の注入後少なくとも3か月使用し続けるか、または永久的に不妊であるか、もしくは妊娠することが医学的に不可能なパートナーを有することに同意する;
f. 正常なスクリーニングECGまたは臨床的に有意な所見がないスクリーニングECG;
g.スクリーニング検査は、正常限度内であるか、またはグレード0~1の所見のみを有しなければならない;
h.臨床的に有意な免疫抑制疾患または自己免疫疾患の既往歴なし。
i.デングウイルスワクチン接種または病気の既往歴なし、または黄熱ワクチン接種の既往歴なし。
j.スクリーニング検査評価によるベースラインにおいてデング血清陰性
k.現在、またはこの4週間以内、免疫抑制剤(コルチコステロイド、低用量のメトトレキサート、または10mg/日未満の用量もしくはステロイド用量当量のプレドニゾンを含有する吸入、局所皮膚、及び/または点眼薬を除外する)を服用していない。
a.現在、または第1の投与の30日以内のいずれかの治験化合物の投与;
b.参加者がプラセボを投与されたことが検証されている場合を除き、ジカウイルスの治療または予防のための治験製品を以前に投与されている;
c.第1の投与の4週間以内のいずれかのワクチンの投与;
d.第1の投与の4週間以内のいずれかのモノクローナルまたはポリクローナル抗体の投与
e.第1の投与の3か月以内のいずれかの血液製剤の投与;
f.研究の過程中の妊娠または授乳または妊娠の計画;
g.デングウイルスに対する陽性の血清学的結果(任意の血清型)、または過去のいずれかの時点におけるデングウイルスもしくは黄熱ウイルスワクチン接種のいずれかの投与の既往歴;
h.HIV、B型肝炎表面抗原(HBsAg)、または治験責任医師もしくはメディカルモニターによって決定される潜在的に伝染性の感染病に対する陽性の血清学的試験;
i.C型肝炎に対する陽性の血清学的試験(除外:持続的ウイルス学的応答の確認を伴う治療の成功);
j.1.5を超えるクレアチニンによって測定される腎疾患のベースラインエビデンス(慢性腎疾患ステージII以上);
k.グレード2クレアチニンを除く、グレード2以上の異常を伴うベースラインスクリーニング検査(複数可);
l.慢性肝疾患または肝硬変;
m.血液学的悪性腫瘍を含む免疫抑制疾患、固形臓器または骨髄移植の既往歴;
n.進行中の、または予想される付随する免疫抑制療法(コルチコステロイド、低用量のメトトレキサート、または10mg/日以上の用量もしくはステロイド用量当量のプレドニゾンを含有する吸入、局所皮膚、及び/または点眼薬を除外する);
o.インフリキシマブ、アダリムマブ、エタネルセプトなどのTNF-α阻害剤を用いた進行中の、または予想される治療;
p.群割り当ての4週間以内の事前の大手術または任意の放射線療法;
q.任意の早期興奮症候群、例えば、ウォルフ・パーキンソン・ホワイト症候群;
r.心臓ペースメーカーまたは自動植え込み式除細動器(AICD)の存在
s.計画された注入部位(複数可)の20cm以内の金属インプラント;
t.計画された注入部位(複数可)における臨床的に有意な医学的状態としてのケロイド瘢痕形成または肥厚性瘢痕の存在。
u.刑事被告人または身体的もしくは精神的疾患のいずれかの治療のために強制的に拘留(強制収監)される参加者;
v.治験責任医師の意見によって、研究要件の順守または免疫学的エンドポイントの評価を妨げる積極的な薬物またはアルコールの摂取または依存;あるいは
w.ジカウイルス関連研究のための試料の保管及び将来の使用を認める意思がない
x.治験責任医師の意見によって、参加者の安全または任意の研究エンドポイントの評価に影響を及ぼす可能性がある任意の病気または状態。
第2の第1相試験は、プエルトリコのデング血清陽性成人においてID投与、続いて電気穿孔されたGLS-5700の安全性、耐性、及び免疫原性を評価するためのプラセボ対照、二重盲検研究であった。研究の第一目的は、デング血清陽性の健康な成人対象においてID注入、続いてEPによって投与されたときのGLS-5700の安全性及び耐性を、最終ワクチン投与から12週目まで評価することであった。
対象(1群当たりn=80)を無作為化して、2mgのDNA/用量のGLS-5700(GLS-5700は、SSC中で製剤化される)またはプラセボ(SSC、GLS-5700のための組成緩衝液)のうちのいずれかを投与する。2回の0.1mLのID注入としてワクチンまたはプラセボを投与し、続いてCELLECTRA(登録商標)-3Pデバイスを用いてEPする。対象は、0、4、及び12週目に三角筋部にワクチン接種を受ける(3回のワクチン接種の連続、表2)。
a.年齢18~65歳;
b.参加に同意することができ、同意説明文書(ICF)に署名している;

j.C型肝炎に対する陽性の血清学的試験(除外:持続的ウイルス学的応答の確認を伴う治療の成功);
k.1.5を超えるクレアチニンによって測定される腎疾患のベースラインエビデンス(慢性腎疾患ステージII以上);
l.グレード2クレアチニンを除く、グレード2以上の異常を伴うベースラインスクリーニング検査(複数可);
m.慢性肝疾患または肝硬変;
n.血液学的悪性腫瘍を含む免疫抑制疾患、固形臓器または骨髄移植の既往歴;

0日目及び4週目にpZV-prME DNAで患者を免疫化した(図39~42)。結合レスポンダーの割合を決定するために、ジカエンベロープタンパク質で覆われたプレート内で指示された希釈において血清をインキュベートし、二次抗体は、全IgG応答を検出した。
