Abstract
Description
GR−MNパッチの針は、4℃でのin−situ光重合により、NVP、DMAEA、3APBA、およびEGDMAのインスリンをプレロードしたモノマー混合物から製造されたインスリンロードグルコース応答性ポリマーマトリックスで構成されていた。NVPは、周囲条件で液体であり、他のモノマーを溶解するための溶媒として機能できるため、主要なモノマーとして選択される。得られたMNは、20×20配列に配置され、各針はピラミッド型で、ベースの辺が400μm、高さが900μmであった。その後、パッチのベースは、柔軟な市販のUV硬化性材料を使用してさらに調製された。GR−MNパッチの蛍光画像は、ローダミンB標識インスリンが各針の先端領域に均一に分布していることを明らかにした(図1C)。さらに、in−situ光重合法により、インスリンのカプセル化効率がほぼ100%になり、潜在的な臨床使用のターゲットであるMNに対して20wt%の高いロード能力が得られた。さらに、引張圧縮機を使用して、MNの破砕強度が0.75N/針であることが決定され(図2A)、これは、破壊することなく皮膚に浸透するのに十分である。
PBA基はグルコースの検出のために以前使用されていた。グルコースへの選択的結合に不可欠なパラメータは、PBA基のpKaである。3APBAのpKaを減少させて、生理学的pHでのグルコース認識能力を高めるために、ルイス塩基DMAEAを導入して、プロトン化されたジメチルアミノ基を介した静電引力(B−−N+)によってホウ酸エステルを安定化した。得られた3APBA/DMAEA含有ポリマーマトリックスのグルコース結合能力を、様々なグルコース濃度を有するPBS緩衝液中でのインキュベーションによって測定した。典型的な高血糖状態(400mg/dL)でのポリマーマトリックスによる結合グルコースの量は、正常血糖状態(100mg/dL)で結合されたものより5.7倍多かった(図2D)。
GR−MNのin vivo性能は、ストレプトゾトシン(STZ)誘発1型糖尿病マウスモデルで評価された。糖尿病マウスをランダムにグループ化し、GR−MNおよび対照としての非応答性架橋MN(CR−MN)パッチ(インスリン用量:0.5mg)を含む様々なサンプルに経皮的に曝露した(図5A)。処置されたマウスのBGLを経時的にモニターした。予想通り、CR−MNおよびGR−MNで処置されたマウスのBGLはすべて200mg/dL未満に減少した(図5B)。ただし、CR−MNで処置したグループでは正常血糖状態を維持できず、BGLは4時間後に高血糖状態に戻った。対照的に、GR−MNは、スマートなグルコース応答性により、おそらく10時間以上にわたってターゲット範囲内(<200mg/dL)でBGLを調節することが示された。酵素免疫測定法(ELISA)による血漿インスリン測定は、持続的な正常血糖BGLレベルと一致して、GR−MNグループでより高い連続したインスリン放出を示した(図5C)。
げっ歯類モデルと比較して、ミニブタのグルコース代謝はヒトのシステムにより類似している。さらに、ブタの皮膚は、一般的な構造、厚さ、毛のまばらさ、コラーゲンおよび脂質組成物の点で、ヒトの皮膚の優れたモデルと見なされてきた。したがって、GR−MNのin vivo性能は、STZ誘発1型ゲッチンゲンミニブタで評価された。糖尿病のミニブタは、麻酔下で7mgのインスリン用量のCR−MNまたはGR−MNで経皮的に処置され、その後、通常は毎日2回の食事が与えられた。図6Aに示されるように、MNパッチは、ミニブタの皮膚に効果的に浸透することができる。ミニブタBGLのリアルタイムで持続的な記録を達成するために、連続グルコースモニタリングシステム(CGMS、Dexcom、米国)がミニブタ実験に統合された。CR−MNおよびGR−MNで処理されたブタの両方のBGLは、2時間後に正常血糖に減少した(図6Bおよび図6C)。午後の食事の後、CR−MNで処置されたブタはすぐに高血糖状態へ上昇した。しかし、GR−MNで処置されたブタのBGLはわずかな増加を示し、食事時間後にすぐに通常の血糖状態に戻った。BGLは、2日目の次の食事まで一晩減少した状態に維持できる。投与中にBGLは40mg/dL(CGMSの検出下限)未満に減少したが、ミニブタの正常血糖値の範囲が低いため、低血糖症の症状は観察されなかった(ブタでの40〜80mg/dL対ヒトでの80〜120mg/dL)。
現在のシステムは、血糖降下状態でグルカゴンの迅速な放出を必要とするDMAEAと3APBAの比率を調整することにより、低血糖を処置するためのグルコース応答性グルカゴン送達にも利用できる。グルコース濃度がPBA濃度よりも低い場合、1つのグルコース分子が2つのPBA分子と反応してビス複合体を形成し、DMAEAと3APBAの比率が0:10のポリマーマトリックスの収縮につながる。図8に示すように、グルカゴンの放出速度は、血糖降下状態(50mg/dL)では速かったが、正常血糖(100mg/dL)および高血糖症条件(200および400mg/dL)では低かった。このようなポリマーマトリックスは、生物医学的応用と同じ方法を使用して、マイクロニードルパッチと統合することができる。
MNベースの経皮インスリン技術は、糖尿病患者の継続的で便利な痛みのない処置を実現するために活用されてきた。グルコース応答性製剤の追加の統合は、BGLの改善された調節に大きく期待できる。しかしながら、そのようなインスリンパッチの橋渡しにおける1つのボトルネックは、臨床使用のためのインスリンの限られたロード容量に関する。この目的のために、このスマートインスリンパッチは、ナノ粒子などのグルコース応答性製剤を内部に埋め込む代わりに、グルコース応答性部分としてPBA基からなるポリマーマトリックス全体を使用して開発された。in−situ光重合戦略に基づくMN加工プロセスは、インスリンの生物活性を維持するための有機溶媒の使用と高温を回避しながら、簡単かつ効率的である。in vivo研究により、パッチ内にロードされたインスリンの生物活性は、室温で8週間以上維持できることが確認された。
材料
別段の明記がなければ、すべての化学物質はSigma−Aldrichから購入し、受領したままの状態で使用した。Norland Optical Adhesive 81(NOA 81)はNorland Products、Inc.から購入した。3−(アクリルアミド)フェニルボロン酸は米国のBoron Molecularから購入した。脱イオン水は、Millipore NanoPure精製システム(18.2MΩcm−1より高い抵抗率)によって調製された。
GR−MNパッチは、UV照射下でのin−situ重合によって調製された。光開始剤を含む液体モノマーは、最初に、架橋剤としてエチレングリコールジメタクリレート(EGDMA、0.5mol%)および光開始剤としてIrgacure 2959(1mol%)を含むN−ビニルピロリドン(NVP)モノマー液体に、2−(ジメチルアミノ)エチルアクリレート(DMAEA)と3−(アクリルアミド)フェニルボロン酸(3APBA)を特定の比率で溶解することによって調製した。その後、インスリンをプレロードしたモノマー(20wt%)をピペットでMNモールド表面に直接堆積させた。次に、型を真空下に10分間置き、液体をマイクロニードル型に充填した。過剰な溶液を除去した後、型をUVランプ(100W、365nm、Blak−Ray、USA)の下に4℃で20分間置き、光重合を開始した。その後、UV硬化性ベース材料(NOA 81)をその型の上に添加し、さらにUV光下で10分間硬化させて、パッチのベースを形成した。得られたパッチを型から注意深く分離し、さらなる研究のために室温で密封された6ウェル容器に保存した。CR−MNは同様のプロセスで調製されたが、DMAEAと3APBAは添加していない。
ストレプトゾトシン(STZ)誘発成人糖尿病マウス(オスC57B6、Jackson Lab、米国)で、糖尿病処置のためのグルコース応答性インスリンパッチのin vivo有効性を評価した。血漿相当グルコースをClarity GL2Plusグルコースメーターを用いてマウスの尾静脈血サンプル(〜3μL)から測定した。マウスに使用したパッチは、ピラミッド型のマイクロニードルの11×11配列で、ベースの辺が300μm、高さが700μmであった。各グループの5匹のマウスを選択して、CR−MN、またはヒト組換えインスリンをロードしたGR−MN(インスリン用量:マウスあたり0.5mg)で経皮的に処置した。それぞれのマウスのグルコース値を経時的にモニターした。in vivoで血漿インスリン濃度を測定するために、25μLの血液サンプルを、示された時点でマウスの尾静脈から採取した。血清を分離し、アッセイまで−20℃で保存した。血漿インスリン濃度は、Human Insulin ELISAキットを製造元のプロトコル(Invitrogen、米国)に従って使用して測定した。
到着から6か月齢の3頭のオスのゲッチンゲンミニブタ(Marshall BioResources、米国)を使用した。STZ注入(150mg/kg)によってミニブタに糖尿病を誘発した。回復の7日後、1型糖尿病モデルの確立の成功は、連続グルコースモニタリングシステム(CGMS、Dexcom、米国)を使用して血糖値をモニタリングすることによって確認された。すべてのミニブタ(体重:25kg〜30kg)は、投与前に一晩絶食させた。ミニブタは、各ブタに対して7mgのインスリン用量で脚部位でCR−MNおよびGR−MNで経皮的に処置された。ミニブタに使用したパッチは、ピラミッド型のマイクロニードルの20×20配列で、ベースの辺が400μm、高さが900μmであった。血糖値はCGMSを使用して継続的にモニターされ、実験中は通常1日2回の食事が提供された。
提示されたすべての結果は平均±s.d.である。スチューデントのt検定を使用して統計分析を実行した。P値が0.05未満の場合、実験群と対照群との間の差は統計的に有意であると見なされた。
マイクロニードル(MN)の機械的強度は、MNをステンレス鋼プレートに押し付けることによって測定された。初期ゲージは、MNチップとステンレス鋼板の間で2.00mmに設定され、ロードセル容量は10.00Nであった。MNに向かう上部のステンレス鋼板の移動速度は0.1mm/sであった。MNの破損力は、針が座屈し始める力として記録された。
サンプルを、様々なグルコース濃度(50mg/dL、100mg/dL、400mg/dL)で、1mLのPBS溶液(NaCl、137mM;KCl、2.7mM;Na2HPO4、10mM;KH2PO4、2mM;pH7.4)中で、37℃で4時間インキュベートした。サンプルを取り出した後、溶液中のグルコースの残留量を、Clarity GL2Plusグルコースメーター(Clarity Diagnostics、フロリダ州ボカラトン)を使用して測定した。濃度は、グルコース標準曲線を使用して較正された。
グルコース応答性マトリックスのグルコース応答性を評価するために、サンプルを、様々なグルコース濃度(0mg/dL、100mg/dL、および400mg/dL)で、1mLのPBS溶液中で、37℃でインキュベートした。所定の時点で、50μLの培地を除去し、Coomassie Plusタンパク質アッセイを使用して、放出されたインスリンの量を調べた。Infinite 200 PROマルチモードプレートリーダー(Tecan Group Ltd.、スイス)で595nmの吸光度を検出し、インスリン標準曲線を使用してインスリン含有量を較正した。グルコース値の周期的変化に適応する能力をテストするために、サンプルを最初に100mg/dLグルコースを含むPBSバッファーで15分間インキュベートした。その時点で、サンプルを取り出し、続いて400mg/dLのグルコースを含むPBS緩衝液中でさらに15分間インキュベートした。このサイクルを数回繰り返した。放出されたインスリンは、上記と同じ方法を使用して測定された。

Claims (10)
- インスリンロードコポリマーを含むマイクロニードルパッチであって、前記インスリンは高血糖環境で前記マイクロニードルから解離する、マイクロニードルパッチ。
- 前記コポリマーが、N−ビニルピロリドン(NVP)、ポリ(ビニルアルコール)(PVA)またはメタクリレートPVA(m−PVA)、2−(ジメチルアミノ)エチルアクリレート(DMAEA)、3−(アクリルアミド)フェニルボロン酸(3APBA)、エチレングリコールジメタクリレート(EGDMA)、2−(ジメチルアミノ)エチルメタクリレート、および/または4−(ブロモエチル)フェニルボロン酸を含むモノマーを含む、請求項1に記載のマイクロニードルパッチ。
- 前記コポリマーが、前記インスリンロードポリマーを架橋するマイクロニードルに組み込まれた架橋剤をさらに含み、前記架橋剤は、エチレングリコール、ポリビニルアルコール(PVA)、メチルアクリレートPVA(m−PVA)、メチレンビスアクリルアミド、ポリ(エチレングリコール)ジアクリレート、およびエチレングリコールジメタクリレート(EGDMA)からなる群から選択される、請求項1または2のいずれかに記載のマイクロニードルパッチ。
- 架橋剤がEGDMAを含む、請求項3に記載の前記マイクロニードルパッチ。
- 前記コポリマーが、ポリ(N−ビニルピロリドン−co−2−(ジメチルアミノ)エチルアクリレート−co−3−(アクリルアミド)フェニルボロン酸を含む、請求項1〜4のいずれかに記載の前記マイクロニードルパッチ。
- 請求項1〜5のいずれかに記載の前記マイクロニードルパッチを含む自己調節インスリン送達システム。
- 対象における高血糖症を処置する方法であって、前記対象に請求項1〜5のいずれかに記載の前記マイクロニードルパッチを投与することを含む、方法。
- 前記高血糖症が糖尿病の症状である、請求項7に記載の方法。
- 対象における糖尿病を処置する方法であって、前記対象にインスリンロードコポリマーを含むマイクロニードルパッチを投与することを含み、前記コポリマーは、N−ビニルピロリドン(NVP)、ポリ(ビニルアルコール)(PVA)またはメタクリレートPVA(m−PVA)、2−(ジメチルアミノ)エチルアクリレート(DMAEA)、3−(アクリルアミド)フェニルボロン酸(3APBA)、エチレングリコールジメタクリレート(EGDMA)、2−(ジメチルアミノ)エチルメタクリレート、および/または4−(ブロモエチル)フェニルボロン酸を含み、前記コポリマーはさらにインスリンを含み、前記インスリンは、高血糖環境で前記マイクロニードルから解離する、方法。
- 前記コポリマーが、ポリ(N−ビニルピロリドン−co−2−(ジメチルアミノ)エチルアクリレート−co−3−(アクリルアミド)フェニルボロン酸を含む、請求項9に記載の方法。
