Abstract
Description
本出願は、2018年6月5日に出願された米国特許出願第16/000,378号の優先権の利益を主張するものであり、この開示は参照によりその全体が本明細書に組み込まれる。
主題開示は、内視鏡外科手術、より具体的には、内視鏡または腹腔鏡外科手術処置中の外科手術空洞内のガス組成物を制御するためのシステムおよび方法を対象とする。
腹腔鏡外科手技または「低侵襲」外科手技は、胆嚢摘除、虫垂切除、ヘルニア修復および腎摘出などの処置の実施において日常事になりつつある。このような手術の利点には、患者への外傷の減少、感染の機会の低減、および回復時間の減少が含まれる。腹部(腹膜)空洞内のこのような手術は、典型的には、トロカールまたはカニューレとして知られる装置を通して実施され、これは腹腔鏡器具の患者の腹腔内への導入を促進する。
Claims (24)
- 内視鏡外科手術処置の間に外科手術空洞内のガス組成物を制御するための方法であって、
a)患者の外科手術空洞からのガス流中の複数のガス種を監視することと、
b)前記外科手術空洞からの前記ガス流中の前記複数のガス種を測定することと、
c)前記外科手術空洞からの前記ガス流中で測定された前記ガス種がそれぞれ存在するかどうか、および/またはそれぞれの所望の範囲内にあるかどうかを判定することと、
d)前記ガス種のいずれかが前記それぞれの所望の範囲外にある場合、是正処置を取ることと、を含む、方法。 - 是正処置を取ることは、前記複数のガス種の一つ以上のガス種が前記それぞれの所望の範囲外にある場合に、前記外科手術空洞内のガス種の組成を前記それぞれの所望の範囲内に至らせるように、ガスを前記外科手術空洞に加えることを含む、請求項1に記載の方法。
- 是正処置を行うことが、非理想的なガス組成をユーザに警告することを含む、請求項1に記載の方法。
- 是正処置を取ることが、外部または内部の気腹装置に、前記外科手術空洞を二酸化炭素でフラッシュするよう指示することを含む、請求項1に記載の方法。
- 是正処置を取ることが、前記患者への危害を防止するために装置を無効にすることを含む、請求項1に記載の方法。
- 前記外科手術空洞からのガス流中の複数のガス種を監視することが、前記ガス流を連続的に監視することを含む、請求項1に記載の方法。
- 前記外科手術空洞からのガス流中の複数のガス種を監視することが、前記ガス流を連続的または定期的に監視することを含む、請求項1に記載の方法。
- 前記外科手術空洞からの前記ガス流が、前記外科手術空洞からのガスの排出から生じる、請求項1に記載の方法。
- 前記外科手術空洞からの前記ガス流が、前記外科手術空洞からのガスの再循環から生じる、請求項1に記載の方法。
- 前記外科手術空洞からの前記ガス流が、前記外科手術空洞からのガスの断続的な漏洩から生じる、請求項1に記載の方法。
- 是正処置を取ることが、ガス流を気腹装置またはガス再循環器から前記外科手術空洞内に送達することを含む、請求項1に記載の方法。
- 前記外科手術空洞からのガス流中で測定された前記ガス種がそれぞれ、それぞれの所望の範囲内にあるかどうかを判定することが、前記ガス種の組成が、指定されたレベルより低い二酸化炭素(CO2)の濃度を含むかどうかを判定することを含む、請求項1に記載の方法。
- 前記外科手術空洞内にガスを加えることが、二酸化炭素(CO2)の濃度が前記指定されたレベルよりも低い場合、前記外科手術空洞内の前記二酸化炭素(CO2)の濃度が前記指定されたレベルよりも高くなるように、二酸化炭素(CO2)を前記外科手術空洞内に加えることを含む、請求項12に記載の方法。
- 複数のガス種を監視することおよび前記ガス種が測定されたか判定することが、窒素(N2)がそれぞれの所望の範囲内にあるかどうか監視することおよび判定することを含む、請求項1に記載の方法。
- 複数のガス種を監視することおよび前記ガス種が測定されたか判定することが、酸素(O2)がそれぞれの所望の範囲内にあるかどうか監視することおよび判定することを含む、請求項1に記載の方法。
- 複数のガス種を監視することおよび前記ガス種が測定されたか判定することが、二酸化窒素(NO2)がそれぞれの所望の範囲内にあるかどうか監視することおよび判定することを含む、請求項1に記載の方法。
- 複数のガス種を監視することおよび前記ガス種が測定されたか判定することが、水蒸気(H2O)がそれぞれの所望の範囲内にあるかどうか監視することおよび判定することを含む、請求項1に記載の方法。
- 複数のガス種を監視することおよび前記ガス種が測定されたか判定することが、セボフルランがそれぞれの所望の範囲内にあるかどうか監視することおよび判定することを含む、請求項1に記載の方法。
- 複数のガス種を監視することおよび前記ガス種が測定されたか判定することが、メタン(CH4)がそれぞれの所望の範囲内にあるかどうか監視することおよび判定することを含む、請求項1に記載の方法。
- 複数のガス種を監視することおよび前記ガス種が測定されたか判定することが、キセノン(Xe)、アルゴン(Ar)、デスフルラン、イソフルラン、および/または一酸化炭素(CO)のうちの一つ以上がそれぞれ所望の範囲内にあるかどうか監視することおよび判定することを含む、請求項1に記載の方法。
- 前記ガス流中の前記複数のガス種を測定することが、複数のガス種センサからの情報に基づいて個々のガス種の流量を合計し、前記ガス流中のすべてのガス種の合計流量で除算することによって、各ガス種をモル百分率として計算することを含む、請求項1に記載の方法。
- 各ガス種を計算することが、複数のガス種センサからの情報に基づいて個々のガス種の流量を合計し、前記ガス流中のすべてのガス種の総流量で除算することによって、各ガス種をモル百分率として計算することを含む、請求項21に記載の方法。
