Abstract
Description
本出願は、2018年3月29日に出願され、参照により本明細書に組み込まれる係属中の米国仮特許出願第62/649838号に基づく優先権を主張する。
本明細書で使用される場合、「電気信号」という用語は、一般に、1つまたは複数のパラメータ、例えば、周波数、パルス幅および/または振幅によって特徴付けられ得る電気信号を指す。本明細書に開示される代表的な電気信号は、(1)約1.2kHz〜約100kHz、または〜約50kHz、または〜約25kHz、または〜約10kHz、または約1.5kHz〜約100kHz、または約2kHz〜約50kHz、または約3kHz〜約20kHz、または約3kHz〜約15kHz、または約5kHz〜約15kHz、または約3kHz〜約10kHz、または1kHz、2kHz、3kHz、4kHz、5kHz、10kHz、15kHz、20kHz、25kHz、50kHz、または100kHzの周波数;(2)約0.1mA〜約20mA、約0.5mA〜約10mA、約0.5mA〜約7mA、約0.5mA〜約5mA、約0.5mA〜約4mA、約0.5mA〜約2.5mAの振幅範囲内の振幅;(3)約1マイクロ秒以下〜約416マイクロ秒、約10マイクロ秒〜約333マイクロ秒、約10マイクロ秒〜約166マイクロ秒、約25マイクロ秒〜約166マイクロ秒、約25マイクロ秒〜約100マイクロ秒、約30マイクロ秒〜約100マイクロ秒、約33マイクロ秒〜約100マイクロ秒、約50マイクロ秒〜約166マイクロ秒の範囲のパルス幅;および/または(4)30マイクロ秒の陰極パルスと続く30マイクロ秒の陽極パルスとの間の20マイクロ秒の相間遅延、引き続いて別の20マイクロ秒の相間を含む、ゼロまたは非ゼロ相間遅延を有することができる。
本技術は、一般に、治療的結果を提供する治療的電気信号要素または成分を伴う波形を介して、メタボリックシンドローム、2型糖尿病(T2D)を含む血糖異常を治療する、および/またはHbA1cレベルを低下させるための脊髄調節ならびに関連するシステムおよび方法に関する。代表的なシステムおよび方法は、T2Dの文脈で以下に記載するが、このようなシステムおよび方法はまた、包括的な血糖異常、および/または患者の血糖異常に関連する他の症状にも対処し得る。本明細書に記載されるシステムおよび方法は、例えば特定の患者によっては、副作用となり得るまたはなり得ない感覚異常を一般に発生させることなく、血糖異常を治療することができる。その他の副作用には、望ましくない運動刺激もしくは遮断、および/または標的血糖異常以外の感覚機能への干渉が含まれ得る。代表的なシステムおよび方法は、調節信号が停止した後、少なくとも一定期間、血糖異常、および/またはHbA1cレベルの低下のための治療を提供し続ける。脊髄後柱、後角、後根、後根進入部および脊柱の他の特定の領域を調節して血糖異常を治療することに関していくつかの代表的なシステムおよび方法を以下に記載するが、かかる調節は、場合によっては、一般に、患者の脊柱の胸部領域(例えば、T2〜T12)、ならびに/あるいは他の神経学的構造、ならびに/あるいは他の神経学的組織、器官および/または組織の標的神経細胞集団を対象とすることができる。少なくともいくつかの代表的なシステムおよび方法では、前記範囲内の胸椎レベルに対して送達される治療信号が、複数の椎骨レベルで脊髄から分岐する脾臓神経に影響を及ぼし得る。
代表的なシステムは、埋込型または外部の信号発生器(またはパルス発生器)を含む。信号発生器は、電気信号を患者の標的神経細胞集団に向ける埋込型信号送達装置に結合可能である。代表的なシステムは、他の要素、例えば、電気信号が患者に送達されるのに応じて信号送達パラメータをプログラムまたは更新するための1つまたは複数の装置も含むことができる。
例1.刺激位置:T9〜T10;周波数:10kHz;パルス幅:30マイクロ秒;振幅:0.5〜5mA。

見出し3.0で上におよび本明細書の他の場所に記載される代表的なシステムを使用して、見出し4.0および本明細書の他の場所に記載される方法を行うことができる。
治療的調節信号を胸部神経細胞集団に印加することによって患者の血糖異常を治療するための方法およびシステムをすぐ下で論じる。
(1)正常な徐波周波数が約3波/分であり、電気信号が約20秒のオンサイクルおよび約20秒のオフサイクルを有し、患者の胃を神経支配する交感神経を阻害する。
(2)正常な徐波周波数が約12波/分であり、電気信号が約5秒のオンサイクルおよび約5秒のオフサイクルを有し、患者の十二指腸を神経支配する交感神経を阻害する。
(3)正常な徐波周波数が約11波/分であり、電気信号が約5.5秒のオンサイクルおよび約5.5秒のオフサイクルを有し、患者の空腸を神経支配する交感神経を阻害する。
(4)正常な徐波周波数が約8波/分であり、電気信号が約7.5秒のオンサイクルおよび約7.5秒のオフサイクルを有し、患者の回腸を神経支配する交感神経を阻害する、および
(5)正常な徐波周波数が約6波/分であり、電気信号が約10秒のオンサイクルおよび約10秒のオフサイクルを有し、患者の大腸を神経支配する交感神経を阻害する。
以下の例は、本技術の代表的なシステムおよび方法をさらに例示するために提供されており、本技術の範囲を制限するものとして解釈されるべきではない。一定の代表的なシステムおよび方法またはその特徴が言及される限り、それは単に例示目的のためであり、特に明記しない限り、本技術を制限することを意図するものではない。当業者であれば、本発明の能力を行使することなく、本技術の範囲から逸脱することなく、等価な手段を開発することができる。本技術の範囲内にとどまりながら、本明細書に記載される手順に多くの変形を加えることができることが理解されるだろう。このような変形は、ここに開示される技術の範囲内に含まれることが意図されている。よって、ここに開示される技術の代表的なシステムおよび方法を以下に記載する。
1.血糖異常を有する患者を治療する方法であって、
少なくとも部分的に、患者の血糖異常の症状に基づいて、少なくとも1つの埋込型信号送達装置を、約C8から約T12までの椎骨範囲内にある患者の脊髄の標的位置に近接して配置するステップと;
電気信号を埋込型信号送達装置を介して標的位置に向けて発するステップであって、電気信号は1.2kHzから100kHzの周波数範囲の内の周波数を有することを特徴とするステップと
を含む方法。
2.血糖異常が2型糖尿病(T2D)および/またはメタボリックシンドロームを含む、上記1に記載の方法。
3.標的位置が患者の脊髄の長軸方向正中線に沿っている、上記のいずれかに記載の方法。
4.少なくとも1つの埋込型信号送達装置がパドルリードである、上記のいずれかの1つに記載の方法。
5.電気信号が約10kHzの周波数を有する、上記のいずれかの1つに記載の方法。
6.電気信号が約30マイクロ秒のパルス幅を有する、上記5に記載の方法。
7.電気信号が、患者の感覚閾値の約20%から患者の感覚閾値の約90%の振幅を有する、上記6に記載の方法。
8.標的位置がT4からT6である、上記7に記載の方法。
9.電気信号が、患者の胃、肝臓、膵臓、および1つまたは複数の副腎からなる群から選択される器官に関連する1つまたは複数の患者の交感神経を阻害する、上記8に記載の方法。
10.1つまたは複数の交感神経が患者の腹腔神経節によって供給される、上記9に記載の方法。

第1の電気信号を前記少なくとも1つの第1の接点を介して前記第1の標的位置に向け、第2の電気信号を前記少なくとも1つの第2の接点を介して前記第2の標的位置に向け、前記第1および第2の電気信号はそれぞれ、1.2kHzから100kHzの周波数範囲の周波数を有するステップと
を含む方法。 - 前記少なくとも1つの第1の接点が第1の埋込型信号送達装置によって保有され、前記少なくとも1つの第2の接点が第2の埋込型信号送達装置によって保有される、請求項60に記載の方法。
- 前記少なくとも1つの第1の接点および前記少なくとも1つの第2の接点が単一の埋込型信号送達装置によって保有され、前記単一の信号送達装置がパドルを含む、請求項60に記載の方法。
- 前記少なくとも1つの第1の接点が前記患者の脊髄正中線の第1の側に配置され、前記少なくとも1つの第2の接点が前記患者の脊髄正中線の第2の側に配置される、請求項60に記載の方法。
- 前記少なくとも1つの第1の接点が前記正中線の前記第1の側に沿って長軸方向に配置された複数の第1の接点を含み、前記少なくとも1つの第2の接点が前記正中線の前記第2の側に沿って長軸方向に配置された複数の第2の接点を含む、請求項63に記載の方法。
- 前記第1の標的位置が第1の胸椎に近接し、前記第2の標的位置が前記第1の胸椎とは異なる第2の胸椎に近接する、請求項60に記載の方法。
- 血糖異常を有する患者を治療するためのシステムであって、
前記患者の脊髄領域の硬膜外腔に埋め込み可能な信号送達装置と;
前記信号送達装置に電気的に結合可能なパルス発生器と;
患者センサーと;
実行されると、
患者血糖値または患者インスリンレベルの少なくとも1つの指標に対応する前記患者センサーからの入力を受信し;
前記入力に応じて、前記パルス発生器が電気信号を前記埋込型信号送達装置に向ける少なくとも1つのパラメータを変更し、前記電気信号は1.2kHzから100kHzの周波数範囲の周波数を有する
機械可読命令を有する、前記患者センサーおよび前記パルス発生器に作動可能に結合された機械可読媒体と
を含むシステム。 - 前記患者センサーがインスリンセンサーである、請求項66に記載のシステム。
- 前記患者センサーが血糖センサーである、請求項66に記載のシステム。
- 前記患者センサーがHbA1Cセンサーである、請求項66に記載のシステム。
- 前記電気信号がオンサイクルおよびオフサイクルを有するデューティサイクル期間を有し、前記オンサイクルおよび前記オフサイクルが前記患者の正常な徐波周波数と相関している、請求項66に記載のシステム。
- 前記オンサイクルが約20秒であり、前記オフサイクルが約20秒であり、前記正常な徐波周波数が約3波/分である、請求項70に記載のシステム。
