Description
本発明は、乳癌の早期発見のための方法および装置に言及し、複数の血液マーカーの分析を伴う。
乳癌は、乳管および小葉を覆う上皮細胞の悪性増殖である。これは、一連の体細胞または生殖細胞変異の産物である個々の細胞が、制御または秩序なしにそれ自体を分裂する能力を獲得し、腫瘍を形成するまで繁殖させるクローン性疾患である。わずかな異常として始まるこの腫瘍は、隣接する組織に侵入し、最終的には体の他の部分に広がる。したがって、この可能性を防ぐために、効率的で早期の診断が必要である。
乳癌には主に2種類ある。乳房から乳首に乳を運ぶ管から始まる浸潤性乳管癌腫は、群を抜いて最も一般的であり、症例の約80%である。2番目は、症例の約10%である、浸潤性小葉癌腫であり、これは母乳を産生する小葉と呼ばれる乳房の部分から始まる。全体として見ると、残りの種類の乳癌は症例の10%を超えない。
乳癌に罹患する主な危険因子には、高齢、非常に早い年齢での初経、高齢での初回妊娠または出産歴がないこと、および家族背景が含まれる。症例の5%〜10%において、乳癌は遺伝性の遺伝子変異によって引き起こされる。
乳癌を発見するために、マンモグラム、高解像度トランスデューサーを用いた乳房超音波(超音波検査)、エストロゲンおよびプロゲステロン受容体検査、または磁気共鳴画像法などのさまざまな検査が使用される。確定的な乳癌の診断は、乳房生検によってのみ決定され得る。
先行技術では、乳癌の発症に対応する抗体を検出する血液分析による乳癌診断方法が記載されている。これらは、血清が血液から分離され、分離されたら、発見された抗体を文献EP2446272、WO9858978、およびWO2008032084に記載されているように分析する非侵襲的検査(血液分析)に関係する。
先行技術に記載されている別の例は、早期乳癌診断に言及する文献US2015/0024960である。より具体的には、乳癌を特異的に認識する抗体を含む血液中の乳癌の出現を診断するように構成された一群のバイオマーカーを指す。

文献[Bayo J,Castano MA,Rivera F,Navarro F.Analysis of blood markers for early breast cancer diagnosis.Clin Transl Oncol.2017 Aug 14]では、乳癌の早期診断のための一連のマーカーが公開され、これは本発明に最も近い先行技術であると考えられる。この文献およびそれが記載している研究の目的は、癌と診断された患者において、高いままであり、したがって推定乳癌予測マーカーとして使用することができる任意の血液指標の存在の可能性を決定することである。
一方、この研究では、マーカーは3つの群に分けられる:
i.臨床的および科学的観点から、乳癌に関して先行技術に記載されている多数の研究によって承認されている第1のマーカー群。この第1の群には、マーカーCEAおよびCA15.3が含まれる。

文献[Bayo J,Castano MA,Rivera F,Navarro F.Analysis of blood markers for early breast cancer diagnosis.Clin Transl Oncol.2017 Aug 14]では、63の症例および63の対照を含む症例設計および対照を用いて、分析的な観察疫学研究が設計された。症例は、手術が行われるのを待っている限局性乳癌(cT1−2およびcN0)と診断された患者である。これらの患者の選択基準は、(a)手術可能な乳癌と診断されていること、(b)以前に別の腫瘍を患ったことがないこと、(c)疾患が進行期または転移期にないこと、(d)以前にネオアジュバント癌治療を受けたことがないこと、かつ(e)研究に含まれることを受諾すること、であった。
対照群に関しては、慢性的な症状を患っていなかったこと、または任意の癌の病歴がなかったことを選択基準とした健康な女性として特徴付けられる。彼女らはまた、研究に含まれることを論理的に受諾しなければならなかった。
定量的特徴および記述的特徴の両方の統計分析に関して、主な目的に取り組むために、カイ二乗検定に基づく比例比較およびスチューデントのt検定に基づく平均比較が使用された。次に、Wald方法を使用した二項ロジスティック回帰分析を行い、ROC曲線方法論を実施して終了した。
2つのシリーズは似ていることがわかった。それにもかかわらず、対照シリーズは平均年齢が低く(57歳と比較して45歳)、したがって閉経の割合が低く、さらに主婦の群と比較して雇用率が高かった。しかしながら、症例シリーズでは、ビタミンDのレベルが低く、BMI(体格指数)が高かった。残りの特徴はバランスがとれている。

CA15.3、CEA、CA125、CA19.9、NSE、CYFRA21.1、α−FETOPROTEIN、および実験的乳癌マーカー(NGAL、EGFR y 8−OHDG)が決定されたら、比率(8−OHDG)/(EGFR)*100を定義する。群ごとのレベルを比較するために、マン・ホイットニーのU検定および多変量ロジスティック回帰(以降、Waldの)を使用して、8−OHDG/EGFR比に従って、単独で、および残りの評価されたマーカーと共にがんの確率を予測する。診断性能は、ROC曲線面積によって評価された(図1〜図3)。
より高いレベルのEGFRが、対照(p<0.001で5.097対5.81ng/ml)、症例の8OHDG(p<0.001で9.85対7.37ng/ml)において検出され、差は、8OHDG/EFGR比でより顕著であった(p<0.001で198対122)。
CEA、CYFRA、NSE、およびNGALマーカーは、患者のほうが高かった(p<0.05)が、単独での挙動では診断感度が不十分であった。さらに、年齢、閉経、仕事、BMI、および低レベルのビタミンDなどの要因が、この疾患のリスク変数であることが判明した。評価された比率のロジスティック回帰分析は、82.4%(実施例1を参照されたい)の性能を単独でもたらし、これは、この商を他のマーカーと組み合わせ、NSEおよびNGALを含む多変量予測式を得ることにより91.2%(実施例2を参照されたい)に上昇し、これは、異なるマーカーの相乗的相互作用により、最大92.8%まで改善することができる(実施例3を参照されたい)。

ここで、Bは推定パラメーターであり、ETは典型的誤差であり、GLは自由度であり、SIGMAは標準偏差である。最低標準偏差の3つのパラメーターが考慮される(8OHDG/EGFR、NSE、およびNGAL比)。
このように、評価された比率のロジスティック回帰分析は、p<0.0001で91.2%の性能をもたらした。

- プロセッサ(複数可)を備えることを特徴とする、乳癌の早期発見のための装置であって、前記プロセッサ(複数可)によって実行される場合、前記装置に請求項1に記載の方法を実行させる複数の命令を含む、装置。
