特に断りのない限り、本明細書で使用するすべての技術用語、及び、科学用語は、当該技術分野(例えば、生理学、分子生物学、生化学など)に属する当業者が、一般に理解するものと同じ意味を有する。
本開示の方法は、対象における聴覚障害の処置、及び/または、予防に関する。用語「聴覚障害」は、本開示に関連して、音を識別、認識、または、理解することを不可能にする障害を指すために使用している。例示的な聴覚障害として、難聴、耳鳴り、及びメニエール病、細菌性、及び、ウイルス性耳感染症、聴覚過敏症、及び、内リンパ水腫などの中耳、内耳、及び、前庭系の疾患がある。ある例では、聴覚障害という用語は、内耳、及び/または、中耳障害におけるニューロン、及び/または、有毛細胞、または、それらのシナプス結合の喪失を特徴とする障害を指すために使用している。
本開示に関連して使用する用語「超粒子」(「SP」)は、細孔のネットワークを含む凝集粒子のことを指す。当該細孔のネットワークは、治療用ペイロードを運搬するための大きな細孔容積と表面積とを兼ね備えた超粒子を提供する。細孔容積と表面積が大きくなると、超粒子が運搬できる治療用ペイロードの量を増やすことができるので有利である。ある例では、本開示の超粒子は、凝集ナノ粒子である。
超粒子は、耳での細胞の生存を促すために、十分な量で、かつ、十分な期間にわたって、内耳に治療薬を送達することができる。本開示の超粒子は、様々な治療用ペイロードを含むことができる。当該「治療用ペイロード」は、聴覚障害を処置する上で有用なあらゆる作用物質とすることができる。作用物質の例として、ポリヌクレオチド、抗体、モノクローナル抗体、抗体断片、抗体薬物コンジュゲート、タンパク質、生物学的に活性なタンパク質、融合タンパク質、組換タンパク質、ペプチド、ポリペプチド、合成ポリペプチド、ワクチン、治療用血清、ウイルス、ポリヌクレオチド、幹細胞などの細胞、または、それらの一部、ならびに、小分子などの生物学的産物がある。
超粒子は、対象への投与に適した医薬組成物として製剤し得る。例示的な医薬組成物は、単独で、または、医薬として許容される担体、希釈剤、または、賦形剤と組み合わせて超粒子を提供し得る。これらの組成物において、治療有効量の治療用ペイロードを、対象の耳に送達するのに十分な量で、超粒子を提供する。特定の投与経路に応じて、例えば、Remington’s Pharmaceutical Sciences(Mack Publishing Co.N.J.USA,1991)に記載されている通りにして、当該技術分野で公知の様々な許容可能な担体を使用し得る。
本開示の超粒子は、その他の治療的介入と組み合わせて、処置を改善することができる。例えば、超粒子は、補聴器と組み合わせて投与し得る。別の例では、超粒子は、聴覚障害、難聴、または、炎症あるいは感染などの関連する病状の処置において有用な1つ以上のさらなる薬物(複数可)と組み合わせて投与し得る。ある例では、さらなる薬物(複数可)は、当該超粒子と同じ製剤で投与し得る。別の例では、さらなる薬物(複数可)は、別の製剤で投与し得る。この例では、超粒子は、その他の薬物(複数可)と、連続して、または、同時に投与し得る。さらに、さらなる薬物(複数可)は、全身、筋肉内、または、経口など、超粒子とは別の代替経路を介して投与し得る。
本開示は、耳を介して対象の細胞、組織、または、臓器に治療用ペイロードを送達する方法も含んでおり、当該方法は、本開示の超粒子を、対象の耳に投与することを含む。
本開示の超粒子を、キットまたはパックで提供することができる。例えば、本明細書に開示した組成物は、聴覚障害を処置するための指示書面と一緒に、適切な容器に梱包し得る。ある例では、組成物を、点耳器、または、充填済注射器などの単回投与容器で提供し得る。
実験動物
本研究では、いずれかの性別の7匹の若年成体有色モルモット(平均550g)を使用した。すべての手順は、National Institutes of Health(NIH)Guidelines for the Care and Use of Laboratory Animalsに従って、適切な倫理委員会の承認を受けており、また、Code of Practice of the National Health and Medical Research Council of Australiaに従った。
この研究では、耳鏡検査で正常な外耳を有していた動物だけを使用した。外科的処置を行う前に、聴力状態を、麻酔下(ケタミン、60mg/kg(Parnell Australia)、及び、キシラジン、4mg/kg(Ilium,Australia)、筋肉注射)で評価した。聴覚脳幹反応(ABR)を測定して、前述した手順(Wise et al.(2005)J Comp Neurol.487,147−165;Wise et al.(2010)Mol Ther.18,1111−1122)を使用して、聴力を喪失する前と後でのモルモットの聴覚状態を評価した。麻酔をした当該モルモットを、音を減衰させた部屋で、温度を37℃に維持した温感パッドに置き、そして、針電極を、頭蓋頂、首のうなじ、及び、腹部に置いた。ABRは、最大で100dBピーク相当(p.e.)の音圧レベル(SPL)の強度で、目盛り付スピーカーから送り出したクリック音を測定した。このABRを増幅し、コンピューターに記録し、そして、0dB〜100dB p.e.SPLの範囲の刺激強度で得た200回超の検査での平均を取った。聴力閾値を決定し、そして、両耳で正常な聴力閾値を有するモルモットだけ(ABR閾値<50dB p.e.SPL)を、研究で使用した。
重度の両側性SNHLをもたらす前記した手順(Shepherd et al.(2005)J Comp Neurol.486,145−158;Wise et al.(2010)Mol Ther.18,1111−1122;Landry et al.(2011)Hear Res.282,303−313)を使用して、動物の聴力を耳毒的に喪失させた。ガス麻酔下(02で1〜2%のイソフルラン、1L/分)で、右頸静脈を露出させ、そして、カニューレを挿入した。温かいハルトマン溶液で希釈をしたフルセミド(130mg/kg、Ilium,Australia)を、ゆっくりと注射した。当該静脈を縛り、そして、切開部をシアノアクリレートで閉じた。次に、3mlのハートマン溶液に溶解した耳毒性アミノグリコシド硫酸カナマイシン(420mg/kg、Sigma−Aldrich,USA)を、皮下注射した(s.c.)。難聴手順を行った約4日後に、上記したガス麻酔薬を使用して、ABRを再測定して、難聴を確認した。50dBを超える聴覚閾値の増加は、重度〜最重度のSNHLを示す。記録したすべての閾値は、100dB p.e.SPLを超えており、これらは、提示した最大強度のものであった。
メソ多孔質シリカ(MS)ナノ粒子を、Wang et al.(2010)Chem Mater.22,3829−3831に従って調製した。MSナノ粒子を、5wt%の粒子濃度で、Milli−Q水に分散させ、そして、短時間の超音波処理をして、安定なコロイド懸濁液を形成した。次いで、当該MSナノ粒子分散液の0.5〜2.0μLアリコートを、パラフィンフィルムで事前に覆った平坦な表面に塗布した。これらの液滴を、気流下で乾燥させて、毛細管力作用を介して、当該MSナノ粒子を、メソ多孔質シリカ超粒子(MS−SP)へと集合するように促した。当該MS−SPのサイズは、当該液滴に使用するナノ粒子分散液の量で制御した。
MS−SPを、100μlのMilli−Q水で6回の濯ぎを行う前に、100μlのエタノール(80vol/vol%)に4時間浸漬して滅菌した。次に、当該MS−SPを、0.1M リン酸緩衝生理食塩水(PBS)で、1回洗浄した。それぞれの実験動物のために、8つのMS−SPを、15μlのBDNF(Geneway、BDNF ヒトタンパク質、カタログ番号10−663−45078)溶液(1mg/mlのBDNF)が入ったエッペンドルフチューブに入れ、そして、それらを、室温で、時折に手作業で震盪させて、3日間インキュベートして担持させた。
聴覚を喪失させて1週間後に、これらの動物に、無菌手術技術を使用して、両側人工内耳手術を施した。ケタミン(60mg/体重kg)とキシラジン(4mg/体重kg)とで筋肉内注射を行って、モルモットに麻酔をかけた。耳介後部手法を使用して、水疱を露出させ、そして、小さな穴を開けて、蝸牛の基底回転を露出させた。0.8mmのダイヤモンドドリルを使用して、蝸牛切開術(約800g)を行い、そして、蝸牛から骨片を除去するために穏やかな吸引を加えた(図1)。SPを、滅菌生理食塩水に懸濁し、そして、21ゲージのポリウレタンカテーテル(Optiva,Medex Medical,UK)を使用して、8個のSPを、蝸牛の基底回転に配置した。1つの蝸牛に対してBDNF−SPを移植し、そして、もう一方の蝸牛に対して、同一の外科的手法を使用して、無担持のSP(コントロールSP)を移植した。BDNF−SPを移植した側を、無作為化した。移植に続いて、筋肉の小片を蝸牛切開部の上に置いて、蝸牛を密閉した。創傷箇所を、周囲の筋肉を2層に縫合し、そして、ステープルで皮膚の切開部を留めて閉じた。外科手術の後と、その翌日に、ハルトマン溶液(10ml/kg、s.c.)、抗生物質Baytril(Bayer,Germany)(0.1mg/kg、s.c.)、及び、鎮痛薬Temgesic(Reckitt−Benckiser,UK)(50mg/kg、s.c.)を与えて、回復を促した。
28日の処置期間の後に、これらの動物に対して、ペントバルビタール(150mg/kg、腹腔内)の過剰投与を行い、そして、0.9%NaCl(37℃)、続いて、10%中性緩衝ホルマリン(10%NBF、4℃)を、経心臓的に灌流させた。蝸牛を解剖し、そして、その頂部に、小さな穴を開けた。正円窓、及び、卵円窓を、25ゲージの針で切開し、そして、当該蝸牛を、室温で、シェーカーにて、1時間、10%NBFで事後固定した。次いで、これらの蝸牛を、室温で、10%エチレンジアミン四酢酸(EDTA)を含むPBSに入れて、脱灰した。脱灰した後に、蝸牛を、前述したようにして(Landry et al.(2011)Hear Res.282,303−313)、Tissue−Tek O.C.T.低温切開片化合物(Sakura,Japan)に埋設する前に、15%、次いで、30%のショ糖溶液で低温保護し、そして、−80℃で保存した。蝸牛を、CM1900UVクライオスタット(Leica,Germany)を使用して、−22℃で、蝸牛軸平面で、12gに切り分け、そして、Superfrost−Plusスライド(Menzel−Glaser,Braunschweig,Germany)に取り付けた。代表的な一連の蝸牛の切片を、マイヤーのヘモトキシリンとパットのエオシン(H&E)で染色をして、ローゼンタール管内の一般的な定性検査を行い、そして、SGN密度を測定した。
すべての定量を、実験グループを知らされていない単一の試験官が行った。Axio Lab顕微鏡と、ソフトウェア(Zeiss,Germany)とを使用して、切片の表示と画像化を行った。ローゼンタール管の面積を、ImageJ V1.46(NTH,USA,http://rsb.info.nih.gov/ij/index.html)を使用して、それぞれの蝸牛領域(領域1〜8。図1を参照されたい)について測定した。SGNを、ローゼンタール管内で特定し、そして、計数した。以前に公開された手法(Wise et al.(2011)Neurotherapeutics.8,774−787)を使用して、明確な核を示すSGNだけを計数した。データを、72μmの間隔で、5つの連続しない切片から回収し、細胞が、重複して計数されないようにした。当該SGNの密度を、5つの切片での平均として、それぞれの蝸牛領域で決定した。先端の蝸牛領域(領域6〜8)では、一般的に、ローゼンタール管が狭くなっており、したがって、領域6〜8のSGN密度データを結合した。
移植したSPに対する免疫応答を定量するために、蝸牛における線維症及び新骨の成長の程度を、少なくとも400μmの距離で隔てた3つの異なる箇所にあるヘマトキシリン及びエオシン染色切片で測定した。前述した技術(Wise et al.(2011)Neurotherapeutics.8,774−787)を使用して、炎症細胞、または、組織成長が占める鼓室階の面積の割合として、組織反応を定量した。要するに、鼓室階の画像を捕捉し、そして、面積を測定した。ImageJの「モーメント」アルゴリズムを使用して、画像の閾値処理を行って、組織反応を同定した。鼓室階の面積を測定し、そして、鼓室階において組織反応が占める割合を計算した。蝸牛内の合計で6つの箇所のデータを測定し、そして、蝸牛あたり3つのデータポイントを、下部基底領域(Ai−Aiii)で平均し、そして、3つのデータポイントを、上部基底領域(Bi−Biii)で平均した(図1Aを参照されたい)。
SGN密度と組織反応との統計的比較は、BDNF−SPを移植した蝸牛のデータを、コントロールSPを移植した反対側のコントロール蝸牛のデータと比較して行った。p<0.05レベルの有意性を利用する反復測定(RM)分散分析(ANOVA)を使用して、統計的有意性を決定し、そして、Holm−Sidak法を使用して事後比較を行った。
実施例2
聴覚障害のある動物は、100dB SPL p.e.を超えるABR閾値を示した。(データは示さず)。組織学的分析は、コルチ器官が完全に崩壊していることを確認しており、このことは、聴覚を喪失すると、有毛細胞が完全に失われてしまい、また、支持細胞が変性して、一般的に、完全に平らな上皮が存在する、ことを示す(図2)。聴覚を喪失させる技術の効果は、両耳の間で同様なSNG損失とは対称的であり(Xu et al.(1993)Hear Res.70,205−215)、このことは、反対側の蝸牛が、SGNの生存を向上させるためにデザインした実験技術のための信頼可能なコントロールとして機能する、ことを示している。
実施例3
蝸牛の切片を調べて定量して、BDNF−SP処置がSGNの生存に及ぼす影響を決定した。図2は、BDNF−SPを移植した蝸牛(左側パネル)、及び、コントロール−SPを移植したコントロール蝸牛(右側パネル)から得た蝸牛領域1〜5(R1〜5)の代表例を示す。アミノグリコシド難聴では、クリック音に対して50dBを超える両側の閾値シフトが認められ(データは示さず)、そして、蝸牛全体でコルチ器官が完全に失われていた(図2)。
実施例4
シナプス変性症の動物モデルにおける機能的欠損に関して、最も一般的に報告されている変化は、聴覚刺激によって活性化を受ける聴覚ニューロンの数が少なくなる結果として現れる、惹起した聴覚反応の振幅の縮小である。しかしながら、図3のデータは、行動実験で初めて、かつ、下丘での神経記録から、騒音に曝露した後の蝸牛シナプス変性症の動物での変調音刺激の検出に欠陥があることを示している。蝸牛シナプス変性症の知覚障害は、現実的であり、そして、行動課題と電気生理学的記録とを使用して測定可能であり、また、重要なことに、失われたシナプスを修復するニューロトロフィン療法の評価を可能にする。
実施例5
SP薬物送達システムの臨床的利用可能性の前提条件は、ヒトへの移植に対して安全であることである。薬物送達のためのSPの安全性を決定するために、BDNF−SP、及び、コントロール−SPの移植に対する組織反応を、蝸牛内で定量した(図1を参照されたい)。組織反応は、新しい組織が占める鼓室階の割合を測定して定量を行い、そして、そのデータを、図5に示す。
実施例6
本開示のSP担体システムは、蝸牛への薬物送達に特異的に適応する、ことを特徴としている。第一に、SPのサイズが比較的に大きい(約500μm)ので、蝸牛への外科的移植を行う際の取り扱いが容易になり、また、SPが移植部位から広範に拡散することがなくなった。蝸牛から中枢神経系、及び/または、前庭末端器官への小さな粒子または細胞の分散は、蝸牛内での標的SGNへの治療用担持物を減らしてしまうので、問題である。第二に、当該SPは、治療用ニューロトロフィン(例えば、BDNF)の持続放出を提供することができ、このことは、長期間の薬物送達が可能であることを意味する。第三に、当該SPは、治療薬を多量のペイロードに担持させることができ、かつ、これらの薬物を治療レベルで送達することができる。第四に、当該SPの細孔へのBDNFの固定化は、インビボでの変性からタンパク質を保護するのに役立ち、そのため、その生物活性を維持するが、ミニポンプに保管すると、タンパク質の経時的な不安定性が問題になる(Zuccato and Cattaneo(2009)Nat Rev Neurol.5,311−322)。ニューロトロフィンは、インビボで半減期は比較的に短く(血漿に注入すると約数分、そして、CSFに投与すると約1時間)、この点は重要である。第五に、当該SP技術は、それぞれの薬物タイプに最適な効果を発揮するように調整した放出特性を付与することが可能な複数の治療用ペイロードを使用することができる。最後に、当該SPシステムは、例えば、人工蝸牛に沿って、蝸牛に向けてSPを移植する、または、(蝸牛の外側の)正円窓膜にSPを移植する、という選択肢を臨床医に提供する。当該正円窓膜は、小分子、及び、薬物を透過する(Plontke et al.(2008)Otol Neurol.29,410− 406;Plontke et al.(2007)Laryngoscope.117,1191−1198)。難聴を処置するために治療化合物を蝸牛に送達するための現在の臨床的手法は、中耳腔に薬物を注入し、そして、当該正円窓膜を介した薬物の受動拡散に依存している。しかしながら、この技術では、薬剤の多くが耳管で短時間の内に亡失するので、蝸牛に薬剤を送達する技術の有効性を制限してしまう。同様に、体積に余裕がないので、蝸牛への薬物の直接注射に制限が生じてしまい、このことが、治療レベルの薬剤を等浸透圧媒体で提供することを難しくしている。体積が大きすぎると、蝸牛の外側、または、蝸牛水管を介して大半の化合物を亡失してしまう。したがって、SPが提供する持続放出プロファイルは、正円窓膜への薬物担持SPの移植が、蝸牛への薬物進入を改善するとの予測を示唆している。
実施例7
SGNの生存と、人工内耳の性能との間には強い相関関係があり、生存しているSGNの数が多いほど、人工内耳の性能は向上する(Seyyedi et al.(2014)Otol Neurol.35,1446−1450)。したがって、SGNの生存を向上させ、かつ、電気的閾値を下げる臨床的に応用可能なニューロトロフィン送達戦略の開発は、現代の機器の人工内耳機能の改善をもたらす可能性が高いであろう。さらに、進行しているSGNの変性と細胞損失は、蝸牛内でより正確な神経活性化を提供することを目的とする現在の集中戦略の臨床的実施において、重大な障害である(Goldwyn et al.(2010)Hear Res.268,93−104;Zhu et al.(2012)Hear Res.283,45−58)。SPの予防、または、SGN損失の減少、及び、SGN線維の再成長の促進は、蝸牛内での神経活性化の精度を向上させるようにデザインした将来の刺激戦略の臨床結果を改善し得る。したがって、SPは、人工蝸牛と組み合わせて移植することができる。
実施例8
超粒子が放出するニューロトロフィンの分布を確立するために、ニューロトロフィン−3に付着させた放射性タグ(ヨウ素125)を使用して、実験を行った。放射標識したニューロトロフィンが、蝸牛の広範囲に(頂端蝸牛領域に向けて)及んでいることは明らかであり、このことは、分布が基底回転(超粒子を移植した箇所)に局在化するものではない、ことを示す(図7B及び7C)。
7日後、蝸牛内のピークBDNF外リンパ濃度、775ng/ml(n=2)
実施例9
インビボ試験を実施して、移植の4時間後、3日後、及び、7日後に、耳に移植した超粒子で送達したニューロトロフィンの薬物ペイロードとクリアランスを決定した。その目的は、時間の経過とともに、蝸牛に残存するニューロトロフィンの量を決定することであった。
実施例10
メソ多孔質シリカナノ粒子(MS−NP)を、Cui et al.2015,ACS Nano,9,1571−1580に記載されている方法を用いて製造した。1.1gの臭化セチルトリメチルアンモニウム(CTAB)を、攪拌しながら、50mlのMilli−Qに完全に溶解した。次に、4.3gのポリ(アクリル酸)溶液(PAA、Mw=250kDa、35wt%の水溶液)を、透明な溶液が得られるまで、室温(25℃)で、20分間、激しく攪拌しながら加えた。次いで、3.5mlの水酸化アンモニウム溶液(28〜30%)を、激しく攪拌しながら溶液に加えて、乳白色の懸濁液を得た。次いで、20分間攪拌した後に、4.46mlのオルトケイ酸テトラエチル(TEOS)を加えた。この溶液を、さらに15分間攪拌した後に、テフロンで密封したオートクレーブに移し、そこで、90℃で、48時間放置した。

メソ多孔質シリカ(MS)ナノ粒子を、Wang et al.(2010)Chem Mater.22,3829−3831に従って調製した。MSナノ粒子を、Milli−Q水に、5wt%の粒子濃度で分散させ、そして、短時間の超音波処理をして、安定なコロイド懸濁液を形成した。次いで、当該MSナノ粒子分散液の0.5〜2.0μlのアリコートを、パラフィンフィルムで事前に覆った平坦な表面に塗布した。液滴を気流下で乾燥させて、毛細管力作用を介して、MSナノ粒子を、メソ多孔質シリカ超粒子(毛細管力MS−SP)に集まるように促した。毛細管力MS−SPのサイズは、当該液滴中で適用されるナノ粒子分散液の体積で制御した。
実施例12
80mgのMS−NP粉末を、2mlのアルギン酸ナトリウム塩溶液(30mg mL−1を含む水)に加えた。当該MS−NPが、アルギン酸ナトリウム塩溶液に均一に分布するまで、得られた溶液を、1時間、超音波処理した。
実施例13
MS−SP(LMS−SPalg)を、実施例3で説明した方法を使用して製造した。650℃で、30時間焼成して、アルギン酸ナトリウム塩を除去した。このステップで、すべての有機成分を除去し、そして、メソ多孔質シリカナノ粒子(MS−SP)、そして、微量のカルシウムと塩化ナトリウムだけが残存した。次いで、MS−SPに、ペイロードを担持させた。これらの粒子では、プロセスAで生成した粒子と比較して、薬物担持性能が顕著に改善した:約7.8μgタンパク質/粒子。
リゾチーム
滅菌したSPを、100μLのFITC−リゾチーム溶液(0.2mg mL−1を含むMilli−Q水)と共にインキュベートして、インビトロ放出試験のために、FITC−リゾチーム担持SPを調製した。リゾチームは、ニューロトロフィンBDNFを模倣するのに適したモデルタンパク質であり(リゾチームは安価で、かつ、容易に入手できるが、BDNFは高価である)、これは両者が、同様の物理化学的性質を有する(リゾチーム、Mw=14.3±0.5KDa、RH=18.9±0.25Å、及び、pI=11;BDNF、Mw=13KDa、RH=24.0±3.2Å、及び、pI=10)ためである。
次に、MS−SPに、フルオレセイン標識リゾチーム(FITC−リゾチーム)を担持させた。図12に示すように、pH値が4から10に高くなると、MS−SPは、約−7.6mVから約−33.9mVの範囲の負のゼータ電位を示した。したがって、正に帯電したリゾチームとBDNFは、静電駆動力を利用して、MS−SPに担持させることができる。共焦点顕微鏡画像(図13a、b)は、MS−SPの表面に担持させたFITC−リゾチームを示した。しかしながら、MS−SPのサイズは非常に大きい(直径が数百マイクロメートルである)ので、標準的なレーザー走査型共焦点顕微鏡は、SPの内部構造の画像化には適していない。しかしながら、外科用メスでMS−SPを破砕すると、内部の画像化が可能となり、また、MS−SPの多孔質内部構造でもFITC−リゾチームが認められる、ことが明らかになった(図13c)。
次に、3日間のインキュベーション時間で、異なる担持濃度のFITC−リゾチームを使用して、異なるタイプのSPの担持能力を調べた(図10)。一般的に、FITC−リゾチームの濃度が大きくなると、薬物担持量も増えた。それらの結果は、アルギン酸塩を含むNMS−SPalg、SMS−SPalg、及び、LMS−SPalgは、アルギン酸塩を除去した(同等の濃度の)NMS−SP、SMS−SP、及び、LMS−SPよりも大きな担持能力を有していた、ことを示している。これは、中性pH値付近で正に帯電したFITC−リゾチームと、負に帯電したアルギン酸塩との間の静電相互作用の高まりに起因し得る。加えて、低いFITC−リゾチーム担持濃度(<0.4mg mL−1)では、LMS−SPalgは、非多孔質MS−SPalg、及び、SMS−SPalgと同様の薬物担持能力を有していたが、当該濃度が大きくなると(>0.4mg mL−1)、NMS−SPalg、及び、SMS−SPalgと比較して、より多くのFITC−リゾチームを、LMS−SPalgに担持できた。アルギン酸塩を除去したMS−SPについても、同様の傾向が認められた:LMS−SPは、NMS−SPやSMS−SPよりも、多くのFITC−リゾチームを担持できた。これらの結果は、(LMS−SP、及び、LMS−SPalgでの)大きな多孔質構造が、おそらくは、さらなる表面を提供して、よって、粒子の外表面と(SP構造内の)粒子内部の領域とを、薬物で完全に飽和させるときに、担持と能力を改善する、重要な要因であることを示す。加えて、当該担持能力は、MS−SPの直径にも依存しており、大きなMS−SP(1000μm)は、小さなMS−SP(200μm)よりも、大きな担持能力を有する(図14)。

Claims (45)

- 前記超粒子が、BDNFと、ニューロトロフィン−3、ニューロトロフィン−4、CNTF、及び、GDNF、ステロイド、または、抗生物質からなる群から選択する治療用ペイロードとを含む、請求項4に記載の方法。
- 前記ステロイドが、デキサメタゾンである、請求項5に記載の方法。
- 前記治療用ペイロードが、ニューロトロフィンである、請求項1、4、または、5のいずれか1項に記載の方法。
- 前記超粒子が、少なくとも2つの治療用ペイロードを含み、前記治療用ペイロードの1つが、神経栄養ペプチドである、請求項7に記載の方法。
- 前記超粒子を、前記対象の中耳腔に投与する、請求項1〜8のいずれか1項に記載の方法。
- 前記超粒子を、前記対象の正円窓または卵円窓に投与する、請求項9に記載の方法。
- 前記超粒子を、前記対象の蝸牛へ移植して投与する、請求項1〜8のいずれか1項に記載の方法。
- 少なくとも2つ、少なくとも3つ、少なくとも4つ、少なくとも5つ、少なくとも10個、少なくとも20個の超粒子を、前記対象の耳に投与する、請求項1〜11のいずれか1項に記載の方法。
- 前記超粒子が、異なるペイロードを有する、請求項12に記載の方法。
- 第1の超粒子が、治療用ペイロードを含み、かつ、第2の超粒子が、異なる治療用ペイロードを含む、請求項13に記載の方法。
- 超粒子を、前記対象の両耳に投与する、請求項1〜14のいずれか1項に記載の方法。

- 前記難聴が、感音性難聴(SNHL)を特徴とする、請求項27に記載の方法。
- 前記難聴が、老人性難聴、騒音誘発性、疾患誘発性、遺伝的、毒素誘発性、または、外科的誘発性のものを特徴とする、請求項27または28に記載の方法。
- 前記疾患誘発性難聴が、脳卒中、自己免疫疾患、メニエール病、ウイルス感染、細菌感染、骨硬化症、または、がんに起因する、請求項27または28に記載の方法。
- 前記遺伝性難聴は、ダウン症候群が原因である、請求項27または28に記載の方法。
- 前記毒素誘発性難聴は、がんを処置するために使用した薬物が原因である、請求項27または28に記載の方法。
- 前記難聴は、騒音誘発性難聴を特徴とする、請求項27に記載の方法。
- 前記外科的誘発性難聴が、生体工学機器の外科的移植を原因とする、請求項27または28に記載の方法。

- 前記難聴が、SNHL、または、騒音誘発性難聴である、請求項38に記載の方法。
