Abstract
Description
(1) 外科的方法であって、
主要構造体を患者の脊椎に取り付けることであって、前記主要構造体は、最上部の機器が装着された椎骨(UIV)と最下部の機器が装着された椎骨(LIV)との間に延在する、取り付けることと、
コネクタを第1の取り付け点に取り付けることであって、前記第1の取り付け点は、前記主要構造体の前記UIVよりも上位に配置された棘突起を含む、取り付けることと、
前記コネクタを前記患者の脊椎に対して位置決めすることと、
前記コネクタを第2の取り付け点に取り付けることであって、前記第2の取り付け点は、前記主要構造体の前記UIVに配置されているか又は前記UIVよりも下位に配置されている、取り付けることと、を含む、外科的方法。
(2) 前記第1の取り付け点が、前記主要構造体の前記UIVよりも上位に配置された複数の棘突起を含む、実施態様1に記載の方法。
(3) 前記第2の取り付け点が、前記主要構造体の第1及び第2の脊椎ロッドを含む、実施態様1に記載の方法。
(4) 前記第2の取り付け点が、前記主要構造体の第1及び第2の骨アンカーを含む、実施態様1に記載の方法。
(5) 前記コネクタが、本体を含み、前記本体が、そこから上位方向に延在する第1及び第2のアームを有する、実施態様1に記載の方法。
(7) 前記アームが、矢状面において湾曲しており、前記患者の脊椎に伸展力を加える、実施態様5に記載の方法。
(8) 前記コネクタを前記第1の取り付け点に取り付けることが、前記アーム上に形成された歯を付勢して前記棘突起と係合させることを含む、実施態様5に記載の方法。
(9) 前記コネクタを前記第1の取り付け点に取り付けることが、各アームの第1の部分を前記棘突起のそれぞれの側面と接触させて位置決めすることと、各アームの第2の部分を前記棘突起の上面と接触させて位置決めすることと、を含む、実施態様5に記載の方法。
(10) 前記コネクタを前記第1の取り付け点に取り付けることが、前記コネクタの前記アームを前記棘突起に縫合又はリベット留めすることを含む、実施態様5に記載の方法。
(12) 前記コネクタを前記患者の脊椎に対して位置決めすることが、(i)前記本体内に形成された凹部内で前記アームを前記本体に対して長手方向に並進させることと、(ii)前記本体内に形成された凹部内で前記アームを前記本体に対して傾けることと、のうちの少なくとも一方を含む、実施態様5に記載の方法。
(13) 前記コネクタを前記患者の脊椎に対して位置決めすることが、前記コネクタの係止機構を締めて、前記本体に対する前記アームの位置及び/又は向きを係止することを含む、実施態様5に記載の方法。
(14) 前記コネクタを位置決めすることが、前記コネクタの前記アームを互いに離間するように広げて、前記棘突起がそれらの間に挿入されることを可能にし、次いで前記アームを互いに向かって戻して、前記アーム間に画定された空間内に前記棘突起を捕捉することを含む、実施態様5に記載の方法。
(15) 前記コネクタを前記第1の取り付け点に取り付けることが、前記コネクタの前記アームに対してカラーを回転又は摺動させて、前記アームを前記棘突起に締め付けることを含む、実施態様5に記載の方法。
(17) 前記アームが、脊椎運動制限の程度を有し、前記脊椎運動制限の程度は、前記主要構造体の前記UIVよりも上位の患者の解剖学的構造の脊椎運動制限の程度よりも大きく、かつ前記主要構造体の脊椎運動制限の程度よりも小さい、実施態様5に記載の方法。
(18) 前記アームが、前記脊椎の屈曲を完全に防止することなく、そのような屈曲を制限する、実施態様5に記載の方法。
(19) 前記主要構造体が一次切開部を通して埋め込まれ、前記コネクタが、前記一次切開部を介して、前記患者の脊椎に対して位置付けられて、前記第1及び第2の取り付け点に取り付けられている、実施態様1に記載の方法。
(20) 前記一次切開部が、前記主要構造体の前記UIVに配置されているか又は前記UIVよりも下位に配置されている上位末端部を有する、実施態様19に記載の方法。
(22) 前記コネクタを前記第2の取り付け点に取り付けることが、細長い可撓性テザーを前記コネクタの1つ又は2つ以上の開口部に挿通させることと、前記テザーを前記主要構造体に取り付けることと、を含む、実施態様1に記載の方法。
(23) 前記主要構造体が装着された椎骨が、互いに融合されるか、又はそのような融合のために準備され、前記コネクタによって前記主要構造体が延在している椎骨は、融合されないか、又は融合のために準備されない、実施態様1に記載の方法。
(24) 前記主要構造体が、それぞれの骨アンカーによって第1及び第2の椎骨に取り付けられた剛性脊椎ロッドを含む、実施態様1に記載の方法。
(25) 前記コネクタが前記第1の取り付け点及び前記第2の取り付け点に取り付けられているとき、前記コネクタのいかなる部分も、前記患者の脊椎の任意の椎弓板の前方に配置されていない、実施態様1に記載の方法。

前記本体から上位方向に延在する第1及び第2のアームと、を含む、コネクタと、を備える、外科用システム。 - 前記少なくとも1つの取り付け点が、前記本体の両端部に配置された第1及び第2のロッド受容凹部を含み、前記本体が、前記第1のロッド受容凹部と前記第2のロッド受容凹部との間に延在する横断支柱を含み、前記第1及び第2のアームが、前記横断支柱から上方に延在する、請求項1に記載のシステム。
