Abstract
Description

てんかん診断法
正確にてんかんを診断することは、臨床医が実際の発作を観察することが稀であることに加え、異なる症状を伴う多くの異なる種類の発作およびてんかん症候群が存在することにより、非常に困難であり、時間を消耗する。さらに、他の神経障害は、誤診断、著しく潜在的な有害事象を伴う不適切な治療、不正確な予後、および健康管理資源の著しい浪費をもたらす、発作のミミックであり得る。運動障害、例えば振戦、チック、ジスキネジー、失神発作/意識消失、一過性脳虚血発作(TIA)、睡眠障害/睡眠時異常行動、身体表現性精神障害などの臨床事象は、経験豊富な臨床医でさえ、発作と誤診し得る。発作の確定診断を行うということは、早めの治療、続く追跡検査および監視、相談ならびに支援をもたらす長期的な患者の健康および転帰にとって重要である。現在、発作およびてんかんの確定診断を得ることは、入院の評価および一連の高価な検査を必要とし得る患者に対して、高価で不便である。てんかんの診断は、長年にわたり、広く高価で、多くの場合扱いにくい医学的神経学的評価に依存し、その評価は、以下を含む。
・「黄金の基準」検査は、脳波検査法(EEG)である。発作が記録の間、確認される場合、発作活性は、EEG上の急激にスパイクする波として現れる。脳病変、すなわち、腫瘍、脳卒中は、正常な脳の電位リズムが緩慢になることを導き得る。EEGの困難さは、典型的には事後アッセイとして、すなわち、臨床事象が終了した後に実施されることであり、実際には正常となり得る。連続EEGは、病院で行われる24時間EEGで、延長されたパターンを得て、起こっている場合に発作を「捕獲」しようとする。これには、高価な入院を必要とし、臨床事象が入院中に起こることを確実に分かる方法はない。明らかに、これは、多くの患者が、事象が終了した後にEDに来院し、病歴情報のみが収集されるので、外来および救急部(ED)の環境での看護者に対して、著しくロジスティックな課題を提供し、単回発作の確定診断は、本質的に不可能であり、よくて経験的である。EEGは、高々約30〜50%の感度(正確に同定される陽性の割合を測定)である。
・最初の発作様事象、事象の期間および頻度、ならびに発症の年齢を取り巻く環境を決定する病歴。多くの場合、患者または看護者は、正確な診断が必要なレベルの詳細を得ることができない。欠落した情報は、たびたび、看護者が正確な記録を保持し続けられないか、または保持できないかのいずれかの事実により、発症の時期および事象の期間を含む。発作様事象は、最初の反応は、患者を世話すること、タイミングの追跡または診断に必要な他の情報を保持しないことである、患者、看護者、および応答者の外傷性の事象である。
・総血球数(CBC)、化学代謝パネル(CMP)、および毒性スクリーニング検査を含む、実験研究。これらは、「発作」そのものを診断しないが、その代わりに神経機能障害を説明する手掛かりを提供し得る。プロラクチンレベルの測定は、不確実である。EEGは、いくつかの種類の脳障害を評価するために使用される。
・MRIは、脳の画像またはスキャンを創出するために使用される技術である。MRIスキャンは、ヒトの脳構造を試験するために使用され得る。MRIスキャンは、それ自体、人がてんかんかどうかを決定することはできないが、他の情報を考慮する場合、てんかんが発作様事象の、可能性が高い原因かを臨床医が決定する助けとなり得る。
・PETスキャンは、臨床医に体内の細胞がどのように機能するかについての追加の情報を示すことで、発作様事象の原因である脳の部分を突き止めるために使用され得る。PETスキャンがいくつかのケースで役に立つと同時に、てんかんとは関連のない異常を示すことも多く、診断プロセスの一部とならないことが多い。
・腰椎穿刺は、脊髄周囲の液体が針吸引を通して取り出され、実験室で分析される手順である。これは、発作様事象の原因である髄膜炎または脳炎などの感染症を除外するために実施される。
適時診断:未解決の必要性
診断プロセスは、臨床事象がてんかん発作として正確に示される前に数か月を要し、多くの場合、臨床医療は、裏付けに基づくが確たる証拠には基づかない、主に経験的であり、多くは、過少なまたは過剰な診断および治療のいずれかの結果となる。ゆえに、適時で正確な発作診断は、未解決の医学的必要性のままである。診断プロセスが長いだけでなく、てんかん診断法に対して米国単独で年度間に総計150億ドル超の健康管理システムでの大きな負担が存在する。ゆえに、実質的な臨床環境それぞれにおいて発作の臨床的評価でのクリティカルギャップは、単回または再発性事象のいずれに対しても使用して、脳活動での相動性変化および緊張性変化の両方を同定することができる、発作に対する正確な診断血液検査である。多くの臨床シナリオは、発作に対する臨床診断検査が患者の臨床状態を説明するのに非常に貴重であるように考えられ得る。1)個人は、自宅で倒れた後、EDに搬送される、2)個人は、街路で混乱し、徘徊しているところを発見される、3)入院患者は、無反応の短いエピソードまたは精神状態の変化を有する、4)EEG、CT、およびMRIが容易に入手できない第3世界の国々での患者。利用できる結果で臨床事象を即時的かつ確定的に説明する簡易血液検査は、診断の大変な進歩となり、さらなる試験を、てんかんに向かう(「含める」)またはてんかんから離れる(「除外する」)方向に導き、資源、時間および費用を節約することができる。要するに、発作に対する簡易血液検査は、大きな革新であろう。
炎症および発作の関連
発作は、発作が始まる脳組織において、炎症反応を誘導する。例えば、腫瘍壊死因子アルファ(TNFα)、インターロイキン−8(IL−8)、インターロイキン−6(IL−6)、およびインターフェロンガンマ(IFNγ)などの重大な炎症促進性サイトカインおよびケモカインの発現を含む、難治性のてんかん患者の切除された脳標本での強い炎症反応であり得る。TNFα、IL−8、IL−6、およびIFNγの上昇したレベルはまた、脳の炎症プロセスが、発作の病原、および慢性てんかん焦点の確立に寄与するとする考えを強調する、マウス発作モデルにおいて検出されている。これらのサイトカインの多くは、発作直後でも発作患者の脳脊髄液において検出されている。いくつかのサイトカイン受容体サブタイプの発現はまた、ニューロンおよび星状細胞で上方制御され、これは、活性化された細胞内シグナル伝達の機構があることを示唆し、脳内のサイトカインのオートクリンおよびパラクリン作用を強調する。サイトカイン、ならびに、グルタミン酸およびGABAなどの古典的な神経伝達物質の間の機能的な相互作用は、これらの相互作用がニューロンの興奮性状態における確立されたサイトカイン媒介の変化の基礎となり、したがって発作を促進する可能性を示唆する。急性発作が、血液脳関門透過性の増加を誘導できる明らかな証拠もある。効果は、活性化T細胞およびマクロファージの脳組織への通過を促進し、脳内の炎症反応を促進し、脳特異的炎症性サイトカインおよびケモカインの末梢血管への漏出を助長することが示されている。

いくつかのたんぱく質の説明で示される通り、いくつかのマーカーは、てんかんにおいて以前に調査されることはなく、他のマーカーは、動物モデルにおいて、または切開脳組織か、もしくは死後の脳組織において調査されている。したがって、本研究は、てんかん患者に関連する患者血清において検出することができる、いくつかの新規のバイオマーカーを提供する。加えて、バイオマーカーの組合せは、てんかん患者を同定するための新規の機構を提供する。

試験方法
血液を、ヒトの神経学的患者、または正常対照から、抗凝固剤としてK2EDTAを含有するラベンダートップバキュテナー採血管に収集した(BD Biosciences)。採血管を8回反転し、次いで遠心分離する前に4℃で10〜15分間、湿った氷上に置いた。血液を4℃で10分間、1000RCFで遠心分離した。血漿上清を、無菌の2mlマイクロチューブ(Sarstedt、I型)内で等分し、−70℃〜−80℃で凍結した。
4X MSD Read Buffer Tを、滅菌した逆浸透H2Oで2回希釈した。プレートを、150μLのRT 2X Read Buffer Tを各ウェルに添加することにより展開し、次いで、MSD Discovery Workbench4.0ソフトウェアを備えたMSD Sector Imager2400で直ちに読み取った。MSD Discovery Workbench4.0ソフトウェアを、血漿サンプルのたんぱく質濃度を決定するために使用し、タンパク質値の最終濃度を各アッセイに対する希釈係数を乗算することにより決定した。
患者登録
EvoScoreが、発作患者を効果的かつ正確に診断し、診断の閾値を確立するために使用され得るかを決定する治験を実施した。全ての入院対象および外来対象は、18歳以上であり、インフォームドコンセントを認知的に得ることができた。18〜20歳の対象から同意を得て、法的権限をもつ代理人から彼らの代理として同意を得た。これらの研究に対する民族または性別の制限はなく、全ての的確な患者を動員して、選択の偏りがないことを確実にした。
個人の事象診断(IED)および患者診断(PD)の説明

予測モデルおよびスコア

アルゴリズムの目的、閾値、および利用できる結果
本明細書に記載の血液に基づく診断検査(例えば、EvoScore)を含む、アルゴリズムをスコア付けすることは、以下の方法により開発された:(a)発作か否かの個人の事象診断のために、分類木、ならびに、分類木分析により定義されるリスク群を含み得る、回帰分析および/または多重ロジスティック回帰、ならびに、(b)てんかんか否かの患者診断のために、分類木分析により定義されるリスク群を含む、ロジスティック回帰および多重ロジスティック回帰。

診断および治療の併用アプローチ
個人の対象に対する最大限の治療上の恩恵を達成するために、任意の特定の時間に、対象の疾患負担を特に定量化し、評価する、疾患活性での治療の効果を決定する、および、将来的な転帰を予測することができることが重要である。本教示の実施形態は、急性および慢性疾患の対象において、疾患活性の正確な臨床評価に対する複数の血清バイオマーカーを同定する。
キット

実施例10:EEGおよびEvoScore
EEGは、感度が37%、特異度が99%、PPVが98%、およびNPVが66%であることが示された。本発明らは、EEGは、発作事象を見逃し得ることで、てんかん患者の著しい偽陰性、および、対応する潜在的な過少な治療をもたらし得ることを見出した。本発明らの結果は、本試験の認識される範囲が反映される(25〜56%の感度、78%〜98%の特異度)。EvoScoreは、相動性変化対EEGを評価した場合、より良好な感度および負の予測値、ならびにほぼ同等の特異度および正の予測値を示した。

(c)てんかんに対して対象を治療するための指示を、対象、または、対象に対する治療を引き受ける開業医に提供すること
