Abstract
Description
[02] 本発明は、中心静脈圧の特定方法と、中心静脈圧を特定するように構成された装置とに関する。
[04] 中心静脈圧(CVP:central venous pressure)とは、平均大静脈又は右房圧を指し、これは三尖弁の狭窄がないかぎり、右室拡張末期圧と等しい。CVPが高いほど、右室の受動的拡張期充満が大きい。正常な心臓に関するスターリングの心機能曲線によれば、右室の充満が大きいと、それに続く拍動での右室1回排出量がより大きくなる。CVPは、大気圧を基準とした水銀柱ミリメートル(mmHg)又は水柱センチメートル(cm H2O)で表現される(1.36cm H2O=1.0mmHg)。ほとんどの患者において、CVPにより測定された平均右房圧は平均左房圧(LAP:left atrial pressure)と近似している。拡張末期では、左房圧は左室拡張末期圧(LVEDP:left ventricular end diastolic pressure)と等しいと仮定され、これ自体は左室拡張末期容量(LVEDV:left ventricular end diastolic volume)を反映していると仮定される。それゆえCVPは、心機能の最適化における重要パラメータである左室前負荷を反映している。しかしながら、梗塞、すなわち弁の障害又は肺疾患のある患者の場合、左右の心室は独立して機能するかもしれない。これらの比較的稀なケースでは、左室前負荷は肺動脈カテーテル(PAC:pulmonary artery catheter)を使って肺毛細血管「楔入」圧を測定することによって推定すべきであり、それは、そのほうがCVPより心左側への静脈還流をよりよく示すからである。
[14] 本発明の実施形態は、中心静脈圧を特定するための非侵襲的、非移植式、自己管理型テストを提供することにより、現在の中心静脈圧モニタリングの限界に対処するものである。システムは、壁内外圧差の変更、静脈容量の光学的計測、及び解剖学的計測を用いて中心静脈圧を特定する。システムは、中心圧の絶対的計測を提供でき、又は中心静脈圧のある時間にわたる相対的変化をモニタするために使用できる。壁内外圧差の変化は、中心静脈圧計測のための末梢静脈血管容量の検出可能な変化を生じさせるために使用される。壁内外圧差の変化は、血管内圧又は血管外圧力の変化により誘発させることができる。システムは、所定の壁内外圧差の変化が存在する中での静脈容量の変化を非侵襲的に計測する。壁内外圧差の変化と血管静脈容量の変化との間の関係が解剖学的計測と共に使用されて、患者の中心静脈圧が特定される。中心静脈圧の変化は心血管機能と相関関係があり、心不全患者の効果的な管理に使用できる。

[62] 心不全の原因から、心血管機能の変化は全体的な体液状態の変化に関連付けられ、中心静脈圧又は慢性的左室圧上昇に反映される。慢性的左室圧上昇の特定は心不全患者の心血管評価の重要なメトリクスであるが、現在の特定方法は家庭用には使用できない。侵襲的な計測は中心静脈圧の正確な推定を提供するが、これらは通院患者にとっては実用的でない。移植方式も利用可能であるが、高額である。超音波又はエコー検査法による方法は中心圧力の上昇を推定できるが、時間がかかり、訓練されたオペレータが必要となる。
[64] 本発明では、末梢静脈圧(PVP:peripheral venous pressure)は、肺動脈閉塞カテーテルと左房との間に存在する体液連続性と同様に、連続的血液コラムによりCVPに結合される「上流の」静脈可変値を反映していることが認識される。Synder CL, Saltzman D, Happe J, Eggen MA, Ferrell KL, Leonard AS. [66] 末梢細静脈及び静脈は、壁の薄い、膨大な数の脈管であり、循環する血液の略3分の2を収容している。静脈系は胸部コンパートメントのための可変的血液タンクの役割を果たし、心充満圧に影響を与える。図4に示されるように、圧力が静脈容量に与える影響はゼロ〜10mmHgの間で特に急峻であり、これは壁の薄い静脈が容易に変形するからである。壁内外圧差がゼロより低いと、静脈はダンベル形状に虚脱し、何れの流れも辺縁経路に限定される。壁内外圧差が1mmHgのとき、静脈はほとんど虚脱し、薄い楕円形状を有する。圧力が10mmHgに向かって上昇すると、楕円形状はどんどん丸くなり、静脈はわずか数mmHgの圧力変化で大きな容量変化に対応できる。10〜15mmHgを超えると、形状は完全に丸くなり、壁内の引き伸ばされたコラーゲンが比較的伸張不能となるため、容量は15mmHgを超える圧力に応答しにくくなる。最大の伸展性(容量変化/圧力変化)は約4mmHgで見られる。静脈系に関して、伸展性は約100ml/mmHgと推定され、これは動脈系の50倍を超える。
[69] 明瞭にするために、図4の伸展性曲線が図5に作図しなおされており、ここで、圧力は壁内外圧差として表現され、横軸の左側に正の壁内外圧差がある。完全な静脈虚脱は、壁内外圧差がゼロ又はゼロ未満のときに起こる。それゆえ、図5はx軸に上肢の動きによる壁内外圧差の変化を示し、減少する容量がy軸である。
[74] 上記の例は、CVP特定のための簡略化された理想的なケースを表している。実際には、人の生理学及び計測プロセスに幾つかの微妙な差異があり、本発明の実施形態はCVPの正確な特定のためにこれを緩和する。これらの生理学的な複雑さと方法論的課題を以下に述べる。
[76] 心臓から上肢への血液流は、上肢がどの位置にあっても発生する。系は閉じた系であるため、上肢への動脈血の量は、血管の大きさの変化又は容量の変化を除き、上肢から出る静脈血の量と等しくなければならない。硬質のチューブを想定すると、肢を通る循環は実際に、U字管のサイフォンを通る流れに似ており、硬質のサイフォンを通る流れは、それが垂直か、水平か、又は上下逆かにかかわらず、同じである。血管が完全に硬質であったとすれば、重力は循環全般に影響を与えない。しかしながら、系は硬質の脈管で構成されておらず、自律神経系が上肢を通る流れの調整に積極的に関与する。血管の変化及び自律神経系による変化は、Hickeyらにより特徴付けられている。Hickey, M,, Phillips, J.P., & Kyriacou, P.A. Journal of Clinical Monitoring and Computing。上肢を下げると、毛細血管圧は血管収縮により制御され、前毛細血管抵抗が増大する。しかしながら、静脈は静水圧の上昇により拡張する。それに加えて、手指の末端には多数の動静脈吻合があり、それが上肢を通る血流を容易にし、体温調節に直接かかわる。図7は、Hickeyらの論文から複製したものであり、これらの生理学的変化を示している。上肢が下の位置にあると、毛細血管前動脈の血管収縮が起こり、動静脈吻合部を通って静脈系へと血液を有効に循環させる。要約すれば、上肢がCVPレベルより低いと、毛細血管の流れが限定され、動静脈吻合部の流れが増大して、静脈が拡張する。フォトプレティスモグラフィ(PPG:photoplethysmogram)を使って組織の光学的計測を行う場合、信号のAC(拍動)成分は静脈拍動がより小さいために小さくなり、その一方で信号のDC(平均)の吸収率は、組織内の血液量の全体的な増大により大きくなる。上肢を挙げると、自律神経系が毛細血管流を保持しようとし、前毛細血管レベルで血管拡張が起こる。動静脈吻合部を通る流れが減少する。この生理学的変化は、大気圧が静脈圧より高いことによって静脈が虚脱を始め、その結果、壁内外圧差がゼロになると発生する。この虚脱は、後毛細血管抵抗が減少することにより、全身血管抵抗を増加させる。それゆえ、上肢がCVPレベルより高いと、毛細血管の流れが増大し、静脈が虚脱する。光学的PPG信号のAC成分が大きくなり、PPG信号のDC吸収率成分が減少する。
[82] 本発明の実施形態が緩和させ、又は補償する第2の生理学的事実は、血管系が、静水圧の変化を含む壁内外圧差の変化に対して瞬時に応答しないという点である。上肢を下向き位置から上向き位置へと移動させると、静脈は中心静脈圧より高い位置で瞬時には虚脱せず、それは血液が流出静脈へと移動するのに時間がかかるからである。静脈系は逆行流を防止するために静脈弁を備える可変径脈管で構成される。これらの弁には、開放圧のほか、被検者ごとの生理学的な差異によるコンプライアンスの違いがある。静脈系の解剖学的構成の結果、応答は減衰又は遅延され、正確な計測結果を得るためにこれらを緩和又は補償しなければならない。この影響を最小化するための方法を以下に説明する。
[84] 静脈排出と充満との間の非対称性は、CVPの特定における重要な要素である。静脈弁は逆行流を防止するため、静脈は動脈の流れから再充満されなければならず、それゆえ、充満時間は典型的に排出時間より長い。例えば、上肢を下の位置から上の位置へと移動させたとき、静脈虚脱に関係する影響の主要なものは静水圧の変化であり、数秒で排出される。しかしながら、上肢を挙上位置から下方位置に移動させた場合、静脈は瞬時には伸展せず、これは上肢の静脈容量を動脈からの流入により有効に再充填させなければならないからである。上肢をある位置から下の位置に移動させたとき、上肢の静脈区画を十分に再充満させるための時間は30秒程度でありうるが、血管の形態及び現在の心拍出量によって異なる。したがって、静脈容量を壁内外圧差の変化により変化させる場合、容量変化の方向、すなわち排出か充満かを適切に考慮しなければならない。
[86] ほとんどの光学的組織計測は、光学計測システムを組織上に設置することにより実行される。静脈系は顕著に低圧であり、典型的には10cm H2O(0.14psi)であるという事実から、光学システムが壁内外圧差に影響を与えないように注意することが必要である。光学システムを組織と接触するように設置した場合、静脈壁内外圧差へのあらゆる影響を計測方法に有効に組み込まなければならない。局所的な壁内外圧差の変化の影響は、非接触型の光学システムを使用することにより最小化できる。このようなシステムは、壁内外圧差に対する影響を最小化するように設計され、非侵襲的かつ非侵入式に静脈容量を有効に特定する。
[88] 前述のように、頸静脈圧を中心静脈圧特定の方法として利用する可能性はオペレータごとのばらつきによって限定され、これは解剖学的な目印の繰返し可能な同定と被検者の姿勢に大きく関係する。非侵襲的、自己管理的に中心静脈圧を正確かつ繰返し可能に特定するには、解剖学的目印の査定を行い、被検者の適切な体位を確実に決定し、又はこれらの組合せを行うシステムが必要である。中心静脈圧計測システムをある被検者の反復計測に使用する場合、このシステムは比較の根拠として所定の高さレベルの単純な変化を使用できる。このような場合、被検者の繰返し可能な体位の決定が制御すべき重要なパラメータとなる。システムをクリニックの現場で複数の患者に対して使用する場合、システムは患者の体位の決定に関連して解剖学的寸法を特定しなければならない。これらの問題に対処するためのシステムと方法を以下に説明する。
[90] 以下の例は、正確なCVP特定に伴う課題を示す。各例において、PPGセンサを手首に、また場合によっては指先に設置した。手首の位置では、軽量PPGセンサを表在静脈上に直接、接触圧を最小化するために接着剤を使って取り付けた。壁内外圧差の変化は、上肢挙上により達成させた。図8Aは、ゆっくりとした上肢挙上中のPPG手首データの一例を示す。当初、光吸収率の変化は挙上しても比較的平坦であるが、胸骨上窩に関して略0cm H20において、吸収率信号は急速に下降し始め、静脈を虚脱させる。高さに関する吸収率の導関数を図8Bに示す。導関数の、破線で示される突出した負のピークは、静脈の充満状態から虚脱状態への遷移の中心を示す。遷移が生じる高さは、末梢静脈圧及び、したがって中心静脈圧に関係する。
[92] 図9は、センサ位置として手指の末端毛細血管を使用した場合の複雑さを示す。上肢を0度(まっすぐ下)から180度(上)まで回転させるときの手指末端部及び手首での光吸収率信号が示されている。手首は、上肢挙上に対して予想通りの応答を示しており、すなわち、静脈が完全に虚脱すると、吸収率の減少は安定状態に到達する。それに対して、指先位置は、より複雑なパターンを示す。当初は上肢挙上量の増大により吸収率が低下するが、自律的動脈血管拡張の大きな影響によって安定状態に到達せず、これは信号のAC(拍動)成分の増大から明らかである。全体的な血液信号は略22秒で最下点に到達するが、その後、静脈及び動脈系の反対の応答により変動する。
[94] 図10は、上肢挙上実験中の接触圧の末梢静脈圧の特定に対する実質的な影響を示す。非常に軽量な接触圧縮をPPGセンサの周囲に加えると、静脈虚脱の圧力は壁内外圧差の減少によって10cm H2Oより大きく低下する。PPGセンサ上のバンドをきつくすると、静脈虚脱点はさらに低下する。
[96] 静脈の排出と虚脱は瞬間的なプロセスではないため、上肢の移動率又は壁内外圧差の変化速度は可変値であり、これを補償し、又はそれ以外に制御しなければならない。図11は、検出された遷移高さが上肢移動率に依存することを示している。静脈が虚脱するまでにかかる時間により、より速い移動によってCVP特定の正確さが低下する。
[98] 静脈系内の容量は、図9からわかるように、上肢挙上及び上肢下垂に対して対称な応答を示さない。手首と指先のどちらの場合も、上肢挙上時の吸収率の変化は速い。しかしながら、上肢下垂に対する応答ははるかに緩慢であり、これはその静脈区画を再充満しなければならないからである。手首位置で、上肢が下向き(0度)位置に戻ってから吸収率信号がベースラインの値に戻るのに20秒を超える時間がかかる。
[100] 光学計測システム
[101] 光学計測システムは、単一の信号源と検出器の形態から複数波長/複数センサの形態まで、様々な形態をとることができる。例示のために、4つの計測システムについて説明する。
[109] 血管内圧力変化は、複数の方法で実現できる。静水圧の変化は、上肢挙上量の変化を含む複数の方法で生成させることができる。上肢の挙上により、心臓から垂直方向の変位で圧力は約0.77mmHg cm-1だけ低下し(Gavish and Gavish 2011)、その一方で、手を下げると局所的動脈圧は同じ分だけ上昇する。
[116] 静脈の壁内外圧差の変化は、外圧の変化により実現できる。このような外圧変化の例は、上肢をウォータバスに漬けること、静圧、又は物理的物体による静脈の物理的圧縮である。これらの種類の外圧印加は、物理的圧力測定法と一致している。静脈系の圧力が非常に低いことから、従来の物理的圧力測定法は非常に高レベルの精度が必要となるため、自己管理型に適さない。この用途のための幾つかの実施形態の重要な要素は、非接触式の圧力測定法の使用である。非接触式の圧力測定法では、壁内外圧差の変化を生じさせるための使われる力は動(速度)圧、静圧、又はそれらの組合せである。

[126] 幾つかの例示的な実施形態において、地面、心臓、又はその他の所定の基準点に関するセンサの位置を特定することが望ましい可能性がある。センサ位置の特定プロセスは、被検者に取り付けられた計測システム(後述)を通じて、又は被検者を観察することにより実行できる。解剖学的寸法を特定するための上述の構造化された光又は3Dカメラシステムは、センサ位置の特定にも使用できる。その他の方法には、風景撮影のための外部カメラと被検者に取り付けられるマーカを含むモーションキャプチャシステムの使用が含まれる。光学受動方式は静脈センサ上の再帰反射マーカを使用し、これをカメラにより追跡できる。光学能動方式は、LEDマーカを使用する。どちらの方法も、マーカ又は発光ダイオード等を静脈感知システムに含めることにより容易に実装できる。
[128] 手指、手、又は手首上の物体の位置の特定能力は、慣性計測ユニット(IMU:inertial measurement unit)システムを介して実現できる。加速度計とジャイロスコープを含む典型的なIMUシステムは、3D空間内の物体の角度位置を測定することができ、これは上肢の振り等の制御された運動条件での物体の位置を推定するために使用できる。IMUをカメラと共に使用することにより、さらに正確さを高めることができる。この方式には幾つかの種類があるが、最もよく知られているのはTango(試験分野では以前はProject Tangoと呼ばれていた)である。Tangoは、Googleが開発し、制作した技術的プラットフォームであり、コンピュータビジョンを使ってスマートフォン、タブレット、ウォッチ等のモバイル機器が、GPS又はその他の外部信号を使用せずに、周囲の世界に関するそれ自体の位置を検出できるようにするものである。Project Tangoは、環境内の機器の位置と向きを特定できる。ソフトウェアは、3種類の機能性、すなわち(1)環境の視覚的特徴と加速度計及びジャイロスコープのデータと共に使用して、空間内の機器の運動を密接に追跡するモーショントラッキング、(2)後で再使用し、その他のProject Tnago機器と共有し、注釈、命令、又は関心対象点などのメタデータにより強化できるマップ内の環境データを保存するエリア学習、及び(3)環境内の距離、大きさ、表面を検出する深さ認識、を統合することにより作動する。これらは共同で、「6自由度」(向きの3軸と動きの3軸)の機器に関するデータを生成し、機器の位置を絶対座標空間内で把握できるようにする。
[132] 末梢静脈の位置を利用するCVPの計測には、測定位置と右房の適当な目印との間の高さの関係を特定しなければならない。胸郭内の右房の位置は、人によって大きさが異なり、目に見える目印がないため、特定が難しい。今日測定されているように、訓練された医療関係者が解剖学的目印を触診することにより、心臓の位置を特定する。phlebostatic axisは右房の大まかな位置であり、図21に示されるように、中腋窩線と胸骨右側の第4肋間腔から引かれた線との交点にある。従来、右房位置の特定は、解剖学的なばらつき、触診エラー、及び測定方法の相違により、エラーが生じやすい。
[136] 反復計測のために、計測位置と心臓との間の高さの関係を再現するか、又は前回の計測からの変化を1〜2cmの正確さで特定することが望ましい。人体の関節の数を考えると、このような再位置決めの作業には多くの微妙な差異があり、かなり複雑である。例えば、以下のシナリオを考える。被検者は試験のために同じ硬い椅子と同じテーブルを使用するが、被検者は前傾している。胴部の角変位により、被検者が体を起こした姿勢で座っていた前回の測定に関して心臓の位置はより低い位置に下がる。同様に、横に傾いているか、単に椅子にドスンと座ることで、心臓の位置は幾数センチメートルずれる可能性がある。正確に心臓の高さを再位置決めし、又は心臓がどれだけ動いたかを特定するプロセスは、心臓が外が見える表面にあるのではなく、大きさの異なる胸腔の中にあるという事実により、さらに複雑となる。
[149] 制御された上肢挙上による静脈画像化
[150] 制御された上肢挙上を利用するシステムと方法は様々な方法で実装でき、以下は1つの例示的な実施形態の説明である。図28は、解剖学的計測を行うための静脈イメージングシステムと3次元イメージングシステムの複合的使用を示す。図28Aにおいて、被検者がシステムに近付いて、座ると、正面カメラ1201を使って被検者の解剖学的寸法と目印が捕捉される。光学システム1202は、静脈容量の特定ができるような方法で組織を画像化する。するとシステムは、所定の制御された方法での上肢挙上中に画像を取得できる。図28Bは、上肢が中心静脈圧より高い地点まで挙上されていることを示す。
[155] 制御された上肢挙上は、被検者主導の運動でも実装できる。1つのシナリオでは、手首用の光学評価システム、例えば前述のような腕時計のバンドを遠隔投射システムと組み合わせることができ、それが使用者が自分の手をそこまで移動させなければならない位置を表示する。このようなシステムは、図31に示されている。被検者には、視覚的又は聴覚的合図を受け取ってからのみ、手を次に表示された標的まで挙上させるように知らせることができる。投射システムは、カメラを使って壁(又はその他の投射面)までの距離を特定し、被検者が適切に移動したことを確認できる。あるいは、腕時計のバンドに取り付けられた光検出器を使って、被検者が手を適切な位置まで挙上したことを特定できる。視覚的フィードバックのほか聴覚的フィードバックも、適切な移動と位置決めを示すために被検者に提供できる。手首用システムは、無線又はBluetooth接続を通じて投射システムと通信できる。
[158] 図33Aは、静脈虚脱の特定のために動(速度)圧を利用する中心静脈圧計測システムの実施形態の図である。動圧は、空気流の方向と速度の変化の結果である。このシステムでは、被検者が手を動かす必要がなく、壁内外圧差を体系的に変化させるような方法で組織に対する外力を変化させる。動作中、システムは静脈イメージングシステムを使って、制御されたエアストリームにより影響を受ける領域内の1つ又は複数の計測部位を分離させることができる。このような方法により、上肢挙上から生じた自律神経による変化の影響が最小化される。非接触式のトノメータシステムは、システムに関する被検者の心臓の高さを評価するための3Dカメラを含むことができる。静脈容量の実際の計測は、脈管から計測された吸収率変化、光学システムが静脈高さ特定のために側面と整列されている場合は高さの変化、及び脈管寸法の変化の検査により特定できる。繰返し可能な計測を容易にするために、システムは、図33Bに示されるように、位置合わせペグの形態のフィンガアラインメントシステムを含むことができる。照明、画像捕捉、及び空気管理のための機器が図33Cに示されている。手の照明は、光学ソース2601により行われ、空気は2602により案内される。システムはまた、光源を手の下に設置することにより、透過照明も可能にできる。
[161] システムは、壁内外圧差を変化させるために静圧又は略静圧機構を使用できる。このシステムは、使用者に手を入口又は開口部から筐体の中に入れさせることにより、動作する。ボックスには入口ポートがあり、これは手をボックスに入れることはできるが、同時に手首周囲に残る空間が最小となるような大きさである。図35エラー!出典不明。はこのような設計を示す。幾つかの実施形態において、手首の周囲の空間は、アイリス絞りを含む複数の機構により最小化できる。アイリス絞りは、光学系において円形の開口を体系的に閉じるために一般的に使用される。手首の周囲を密閉するために使用さる機構は、手からの静脈流をCVP計測エラーが生じるように妨害するようなものであるべきではない。適当な機構は2017年11月17日に出願されたPCT特許出願の米国特許出願第17/62356号に記載されており、これを参照により援用する。測定エラーは、手首に接触しない、手首の静脈のない組織とのみ接触する、又は典型的な静脈圧より低い圧力で手首と接触するシステムを使用することにより削減できる。図35は、被検者の手首が、それを筐体に入れる際にテーブル又はその他の表面に載せることができることを示している。遠位前腕部において、静脈流のための一次表在導管は尺側及び橈側皮膚静脈であり、これらはそれぞれ前腕の内側面と外側面に沿っている。それゆえ、遠位前腕部の手掌表面には大きな静脈がなく、接触圧をこの面に加えても、CVPの計測に影響を与えない。
[167] 静圧及び動圧システムの他の例示的な実施形態は、空気圧を使って手首の空気カフを作ることである。毛細血管と心臓との間のある位置における壁内外圧差を体系的に変更できることによって、幾つかの計測の選択肢が得られる。具体的には、制御された静脈還流機構を作れることにより、中心静脈圧を計測するための血管内圧の操作が可能となる。
[170] 例示的な実施形態の光学トノメータは、組織と接触して設置できる1つ又は複数の光ファイバの使用に基づく。光ファイバは、所定の方法で組織と相互作用し、異なるレベルの力が組織に加えられるようにすることができる。力を組織に加えるための機構は光ファイバを介して行われ、これは計測モダリティも表す。光ファイバは、ファイバにより発せられる圧力による圧力上昇によって吸収率の変化を計測できる。それ以上の吸収率の変化が起こらなかった、又は最大変位が発生した圧力点により、静脈虚脱と壁内外圧差ゼロを特定できる。壁内外圧差をゼロにするのに必要な、その結果による力と心臓に関する手の高さにより、中心静脈圧の特定が可能となる。自己管理型テストを創出するために、複数のピンを使って手の表面に有効に適合させる子供用玩具によく似た方法で、光ファイバの分散アレイを利用できる。その結果として得られる光ファイバのアレイをイメージング分光計に再び送信し、評価できる。図39は、光学トノメータの例示的な実施形態の図である。
Claims (34)
- 個人の中心静脈圧を特定する装置であって、
(a)前記個人の肢の静脈内の壁内外圧差を変化させるための圧力機構と、
(b)前記肢の計測領域内の血圧に応答し、複数の壁内外圧差の各々での血液量の計測値を特定するように構成された光学測定システムと、
(c)解剖学的基準点に関する前記計測領域の高さを特定する高さ特定機器と、
(d)前記特定された高さから、及び壁内外圧差と血液量との関係から中心静脈圧を特定するように構成された分析システムと、を備える装置。 - 前記分析システムは、前記特定された高さから、及び、前記特定された血液量の計測値が壁内外圧差がゼロに到達したことを示す前記壁内外圧差から、中心静脈圧力を特定するように構成される、請求項1に記載の装置。
- 前記圧力機構は、前記静脈への外部圧力を変化させる機構を含む、請求項1に記載の装置。
- 前記圧力機構は、
(a)密閉機構を有し、前記静脈の周囲より高い圧力を付加するように構成された加圧可能筐体と、
(b)前記静脈に複数の圧力を付加するように構成された制御機構と、を備える、請求項3に記載の装置。 - 前記密閉機構は、前記付加された圧力を使って係合するシールを含む、請求項4に記載の装置。
- 前記制御システムは、前記筐体を出る空気の流速を変化させることにより、前記静脈に複数の圧力を付加する、請求項5に記載の装置。
- 前記光学計測システムは、
(a)光を前記静脈へと案内するように構成される照明システムであって、前記光は脱酸素化ヘモグロビンにより吸収される1つ又は複数の波長を含む照明システムと、
(b)前記1つ又は複数の波長に対して感度を有する光学感知機器と、を備える、請求項1に記載の装置。 - 前記1つ又は複数の波長は530nmより長い波長を含む、請求項7に記載の装置。
- 前記光学感知機器は、前記肢内の組織から反射した光を収集するように構成される、請求項7に記載の装置。
- スペクトル反射した光が前記光学感知機器に到達するのを阻止するように構成される第1及び第2の偏光板をさらに含む、請求項9に記載の装置。
- 前記光学感知機器は、前記肢内の前記組織を透過した光を収集するように構成される、請求項7に記載の装置。

- 個人の中心静脈圧を特定する非侵襲的方法において、
前記個人の肢の計測領域を、圧力管理システムを使用して変化する外的圧力に前記肢をさらすことにより、又は前記肢を前記個人の心臓に関する測定部位の高さが変化するような複数の形態で設置することにより、又は抵抗呼吸により、又はそれらの組合せにより、複数の壁内外圧差にさらすことと、
光源と感光検出器を使って、前記肢の計測領域における静脈の虚脱と、静脈虚脱が発生した前記壁内外圧差を光学的に特定することと、
前記個人の心臓の右房と前記計測領域との間の垂直方向の分離を特定することと、
静脈虚脱が発生した前記壁内外圧差から、及び前記垂直方向の分離から、前記中心静脈圧を特定することと、を含む方法。 - 個人の中心静脈圧を特定する方法であって、
(a)加圧可能筐体を提供し、前記肢を前記筐体内に設置し、前記肢の周囲にシールを提供することと、
(b)解剖学的基準に関する前記肢の計測領域の高さを特定することと、
(c)前記加圧可能筐体を使って前記肢を複数の外的圧力にさらすことと、
(d)前記肢の計測領域内の血液の存在に応答する光学測定システムを提供し、前記光学計測システムを使って、前記計測領域内の血液量の計測値を特定することと、
(e)前記血液量の計測値と、対応する外的圧力との関係から、及び前記特定された高さから、前記中心静脈圧を特定することと、を含む方法。 - 前記加圧可能筐体を使って前記肢を複数の外的圧力にさらすことは、前記肢を時間と共に増大する複数の圧力にさらすことを含む、請求項22に記載の方法。
- 前記加圧可能筐体を使って前記肢を複数の外的圧力にさらすことは、前記肢を変調された圧力にさらすことを含む、請求項22に記載の方法。
- 前記変調された圧力は1Hz〜0.05Hzの間の周波数を有する、請求項24に記載の方法。
- 前記変調された圧力は、10cm H2O未満の変調振幅を有する、請求項24に記載の方法。
- 前記加圧可能筐体を使って前記肢を複数の外的圧力にさらすことは、前記肢を、時間と共に離散増分で変化する複数の圧力にさらすことを含む、請求項22に記載の方法。
- 前記加圧可能筐体を使って前記肢を複数の外的圧力にさらすことは、前記肢を時間と共に線形に変化する複数の圧力にさらすことを含む、請求項22に記載の方法。
- 前記分析システムは、前記静脈の画像から前記静脈の大きさ、容量、又は血液成分を特定するように構成される、請求項13に記載の装置。
- 前記複数の壁内外圧差は、
(a)前記肢を、前記個人の心臓に関る計測部位の高さが変化するように複数の形態で設置すること、
(b)前記個人が前記計測領域を前記心臓に関する複数の位置に位置付けるようにさせることであって、少なくとも幾つかの位置は前記心臓に関して異なる垂直位置にあること、
(c)前記個人が前記計測領域を含む上肢、手指、又は下肢を、垂直平面内の所定の回転を通じて移動させること、
(d)前記個人が前記計測領域を含む上肢、手指、又は下肢を、固定された基準又は前記個人に通信される基準により示される複数の所定の位置へと移動させること、
(e)前記個人が前記計測領域を含む上肢、手指、又は下肢を、固定された基準又は前記個人に通信される基準により示される複数の所定の位置へと移動させ、前記移動が完了したことをセンサを使って確認すること、
(f)前記個人が前記計測領域を前記心臓に関する複数の位置に位置付けるようにさせることであって、少なくとも幾つかの位置は前記心臓に関して異なる垂直位置にあり、各移動は現在位置の光学計測が安定化してから促されること、のうちの1つ又は複数により得られる、請求項21に記載の方法。 - 前記複数の壁内外圧差は、前記計測領域を含む肢の動きを制限するモーションガイダンス装置を使用することと、前記個人が前記計測領域を前記モーションガイダンス装置により規定される複数の位置へと移動させることとによって得られる、請求項1に記載の方法。
- 前記複数の壁内外圧差は、光を表面に投射することであって、投射される光のパターンは前記計測部位の複数の位置を規定することと、前記個人が前記計測部位を前記複数の位置の各々へと移動させるように促すこととにより得られる、請求項1に記載の方法。
- 前記複数の壁内外圧差は、
(a)前記肢を圧力管理システムを使って変化する外的圧力にさらすこと、
(b)前記計測領域を複数の外的圧力にさらすこと、
(c)前記計測領域を複数の空気圧にさらすこと、
(d)前記計測領域を複数の物理的圧力にさらすこと、により得られる、請求項1に記載の方法。 - 前記複数の壁内外圧差は、前記個人の手を受け入れるように構成された筐体と、容器と通信するガス供給手段とを使用すること、及び前記ガス供給手段を制御して、前記容器内の前記個人の手にかかるガス圧を変化させて、複数の壁内外圧差を提供できることにより得られる、請求項1に記載の方法。
