Abstract
Description
本出願は、2016年10月27日出願の米国仮特許出願シリアル番号第62/413,952号の優先権を主張し、同号は参照によりその全体が本明細書に組み込まれている。
本発明は、サンプル分析技術、より具体的には、例えば血液サンプル中のがん細胞から得られた核酸を検出及び分析して、特定の患者においてどの療法が最も有効であるか判断するための方法及びシステムに関する。
単純な血液検査又は「液体生検」を使用して、稀少な循環性腫瘍細胞(CTC)、エキソソーム、及び細胞フリー核酸、例えば細胞フリーデオキシリボ核酸(DNA)又はリボ核酸(RNA)等の存在を検出することができれば、がんのモニタリングが大幅に強化される可能性があり、侵襲的な腫瘍生検を必要とすることなく、腫瘍細胞数、遺伝組成、及び薬物応答パラメーターの即時サンプリングが実現する。したがって、初期のがん検出において、CTC、エキソソーム、及び細胞フリーDNA又はRNAを検出することは、がんの治療に一大変革を引き起こす可能性があり、根治的治療が予期される場合、がん転移前のステージにおいて浸潤がんの診断が可能となる。
本開示は、所定の患者内の特定の検出されたがんを治療するのにどのがん療法が最も有効であり得るか予測するために、例えば、標準的な血液サンプルにおいて、稀少なCTC、エキソソーム、及び/又は細胞フリーRNAから、腫瘍特異的RNAに関係するデータについて、考え得る最高の感度を取得する方法及び使用に関する。特に、新しい方法は、CTC及び/又はエキソソームが夾雑しているWBCから完全に単離されることを必要とせず、むしろ例えば、10,000個に達する又はそれ超のWBCを含有する生成物中に1個のCTC又はエキソソームほどの僅かな量をも確実に検出することができる。新しいアッセイ方法は、(1)血液から高品質RNAを含むインタクトなCTC及びエキソソームを一貫して取得することができる単離システムと、(2)健康な患者の血液中には存在しない腫瘍型それぞれに対する特定のがん系統のRNAマーカーに着目した液滴に基づくデジタルポリメラーゼ連鎖反応(PCR)アッセイ方法とを組み合わせる。この新しい方法は、どの治療剤が各患者に見出される特定のがん型を有効に治療する可能性が最も高いか判定するのに使用可能である。
本開示は、所定の患者における特定の種類のがん治療において、所定の抗がんレジメンが有効に働くかどうかを予測するのに役立つ、血液サンプル中のがん細胞、例えばCTC、血液サンプル中のがん細胞に由来するエキソソーム、又はがん細胞に由来する細胞フリーRNAに由来するRNAから情報を取得する方法及びシステムに関する。これらの方法及びシステムでは、単離技術の力、例えばウルトラハイスループットマイクロ流体技術、例えば、陰性枯渇技術(negative depletion technique)、例えば、血液サンプル中の非タグ化CTCを単離するための造血細胞の陰性枯渇を使用する技術等と、分析技術、例えば特定のがん系統のRNAマーカーに重点が置かれた液滴に基づくデジタルポリメラーゼ連鎖反応(PCR)アッセイ等が併用される。特別なアッセイ方法(本明細書に記載する新しい予測的な分析法ではない)は、PCT WO2016154600にさらに詳細に記載されており、参照によりその全体が本明細書に組み込まれている。
単離技術が、例えば、ウルトラハイスループットマイクロ流体技術、例えば国際PCT出願WO2015/058206、及びOzkumurらの「Inertial Focusing for Tumor antigen−Dependent and −independent Sorting of Rare Circulating Tumor Cells」、Sci. Transl. Med.、5巻: 179ra47頁(2013)に記載されているいわゆる「CTC−iChip」等を使用して血液サンプルからCTCを濃縮するのに使用される。CTC−iChipは、血液サンプル中のWBCが磁気ビーズで標識され、次にサンプルが2つの濃縮ステージによって処理されるような、CTC抗原非依存性アプローチを使用する。第1のステージは、小型の柔軟な細胞/粒子(RBC、血小板、未結合の磁気ビーズ、及び血漿)を取り除く一方、より大型の細胞(CTC及びWBC)を保持する決定論的横置換法(deterministic lateral displacement)を使用する。第2のステージは、慣性集束(inertial focusing)を使用してすべての細胞を細い流体流中に移動させ、次に磁場を使用して集束流からビーズ標識されたWBCを引き出し、高度に濃縮されたCTCを残す。CTC−iChip生成物は、全血10mlから、<500,000個のRBC、<5,000個のWBC、及び可変数のCTCを典型的には含有する。
新しいアッセイ方法は、単離システム、例えばマイクロ流体陰性枯渇システム等を使用する部分的に純粋なCTCの単離から開始して、液滴デジタルPCR装置から得られたデータの分析までを含む。10の主要なアッセイステップが存在し、そのいくつかは、一般的により良好な結果をもたらすが、任意選択である:
1.例えば、患者又は対象由来の血液中に存在するCTC及びその他の細胞を含む生成物を血液サンプルから単離するステップ;
2.稀少な細胞を含有する生成物の容積を低減させるステップ(任意選択);
3.生成物からリボ核酸(RNA)分子を、例えば細胞溶解により単離するステップ、及び単離されたRNAから、例えば生成物に含有される細胞から放出されたRNAのRT−PCRにより、cDNA分子を溶液中で生成するステップ;
4.RT−PCRステップ期間中に合成されたcDNAの浄化ステップ(任意選択);
5.遺伝子特異的標的化事前増幅プローブを使用して、例えば、Fluidigm BioMark(商標)Nested PCRアプローチ、又は非特異的な全トランスクリプトーム増幅法を使用して、例えば、ClontechのSMARTer(商標)アプローチを使用して、cDNAを事前増幅するステップ(任意選択);
6.cDNA分子を、例えばPCR試薬と共に個々の液滴中に封入するステップ;
7.CTCに由来するcDNAと特異的に結合し、且つその他の細胞に由来するcDNAには結合しないように構成されたレポーター基の存在下、各々の液滴中でcDNA分子を、例えばPCRを使用して増幅するステップ;
8.液滴中にCTC由来のcDNA分子が存在することを示す指標としてレポーター基を含有する液滴(例えば「陽性の」液滴)を検出するステップ;
9.例えば、患者又は対象における特定の疾患の存在を判定するために、検出された液滴中のCTCを分析するステップ;及び
10.がん細胞、例えばCTC内の特定のがん特異的又は系統特異的遺伝子の発現が、健常ドナー対照により決定される低バックグラウンドレベル(2.5のレベルに設定)よりも高いことを検出し、特定の抗がんレジメンが、その特定の患者の特定の腫瘍に対して有効であると予期されるかどうか判定するステップ。
1.CTC単離
患者血液を、CTC−iChip、例えばバージョン1.3M又は1.4.5Tにかけ、サンプルを、氷上の15mLコニカルチューブに収集する。CTC−iChipを、これまでの記載に従い設計及び作製した(Ozkumurら、「Inertial Focusing for Tumor Antigen−Dependent and −Independent Sorting of Rare Circulating Tumor Cells」、Science Translational Medicine、5巻(179号):179ra47頁(DOI:10.1126/scitranslmed.3005616) (2013))。
生成物中の単離されたすべての細胞を完全に溶解するために、代表的な開始点である数ミリリットルから約100μlの最終容積に生成物の容積を低減させることが好ましい。これは、例えば、生成物を遠心分離し、そして細胞の溶解、cDNAの生成に備えてプルロニックバッファー中で再懸濁することにより達成され得る。この時点で、サンプルは、RNAlater(商標)(ThermoFisher)を添加することにより長期保管用として処理可能であり、その後に液体窒素中での瞬間冷凍、及び−80℃での保管が続く。
RNA単離ステップは、RT−PCRに備えて細胞から全RNA分子を完全に放出させる処理にとって重要である。ワンステップ、チューブ内反応が、標準的な移送ステップ中に遭遇する可能性がある細胞及びRNAの喪失リスクを最低限に抑えるのに使用可能である。例えば、ペレット化した生成物にRT−PCRマスターミックスを直接添加し、ピペッティングして完全に溶解し、1:1のRNA:cDNA比を目標とするキットプロトコールに基づく反応を実施することにより、qRT−PCRキット用のInvitrogen、SuperScript III(登録商標)First−Strand Synthesis Supermix(登録商標)を使用することができる。cDNAが合成されたら、残留するすべてのRNAを取り除くために、RNase Hが反応に適用される。或いは、後のステップにおいてサンプルの全トランスクリプトーム事前増幅を実施したければ、cDNAは、独自開発のオリゴヌクレオチド及び逆転写酵素を使用するSMARTer(商標)Ultra Low Input RNA Kitプロトコールを使用して合成可能である。
やはり任意選択であるが、プロセス内の別の有用なステップは、cDNAを含有する溶液から溶解試薬を除去することと関係する。cDNAを含有する溶液をddPCR装置に移されると、刺激性の界面活性剤の存在が、ddPCR方法で使用される液滴の不安定化を引き起こすおそれがある。界面活性剤の除去は、例えば、可逆的固相固定化法(Solid Phase Reversible Immobilization)(SPRI)の使用によって実現可能である。この技術は、コーティングされた磁気ビーズを使用して特定のサイズ範囲のcDNAにまず結合させ、次に界面活性剤を含有する上清の除去、及び最終的にddPCR装置にインプットするための純粋なcDNAの溶出を可能にする。RT−PCRの浄化に加えて、SPRIプロセスでは、プロセスのddPCRフェーズに入り込む非標的cDNA分子の数を減少させ、次にバックグラウンド及びノイズを低減させるcDNAのサイズ選択も実現する。
cDNAの事前増幅は、液滴PCRステップにおいて検出可能であるテンプレート分子の数を増加させ、したがってシグナル対ノイズ比を改善し、陽性の読み出しにおける信頼度を増強する任意選択のステップである。これは、例えば転移前の疾患の早期検出の文脈等において、CTCにおいて低レベルで発現しているマーカーの検出、及びごく少数のおそらくはアポトーシス性のCTCを含有するサンプルの分析にとって非常に強力なアプローチであり得る。これら2つのアプローチは、d−CTCアッセイのワークフローに適用されるように改良されてきた。Fluidigm BioMark(商標)Nested PCRプロトコールに基づく特異的標的増幅法(STA)は、液滴PCRステップで使用されるプローブが標的とする領域を増幅するように特別に設計されたプライマーの使用に立脚する(表2を参照)。これらのプライマーは、健常対照においてノイズを増加させることなく効率的で特異的な増幅を保証するために、その各蛍光プローブと関連して慎重に設計及び試験された。或いは、SMARTer(商標)Ultra Low Input RNA Kitプロトコールに基づく全トランスクリプトーム増幅法は、ランダムプライマーを使用する、WBCに見出される転写物及びCTCに見出される転写物の両方を含む生成物内のあらゆる転写物の増幅に立脚する。
cDNAが合成され、夾雑している界面活性剤を精製により除くと、溶液中のcDNA分子の全凝集物+qPCR試薬は、例えば、液滴製造装置、例えば、1サンプル当たり20,000個の液滴を生成するBioradの自動液滴発生装置等の液滴発生装置により、多くの微細な区画化反応に分割される。各反応は、非水性の液体、例えば油(PCR安定的、例えば、ベンダー提供の独自処方)の極めて小さな液滴からなり、該液滴は、遺伝子特異的プライマーを含むTaqman型PCR試薬、及びオリゴヌクレオチドプローブ、及び少量のサンプルを含有する。液滴の生成が完了すると、サンプルは、膨大な数の個別のPCR−迅速反応を含有するエマルジョンからなる。
標準PCRサイクリングは、qPCRサイクリング条件を使用して、全エマルジョンサンプル上で実施される。反応は45サイクルまで行って、個々の液滴−PCR容積の大半がエンドポイントに到達することを保証する。反応はTaqman型qPCR試薬を用いて実施され、そしてqPCR条件下でサイクル稼働するものの、サンプルの蛍光強度はPCRサイクリング期間中に測定されず、むしろ次のステップで測定されるので、これは重要である。
個々の分割されたPCRのそれぞれは、任意の蛍光測定が実施される前にエンドポイントに十分到達しているので、個々の液滴のそれぞれは、完全に蛍光性の液滴であるか、又は実質的に蛍光を全く含まない。これは、すべての陽性(蛍光性)及び陰性(非蛍光)の液滴について、それらの単純な羅列を可能にする。
アップストリームRT−PCRは、RNA:cDNAの比として1:1を目標とするので、各陽性の液滴は、単一起源のRNA転写物を表すはずである。この解釈は、個々の液滴の数が標的cDNA分子の数をはるかに上回ることに依拠する。新しいプロセスでは、その一方において、本発明者らは、単一のCTCが単離、溶解され、いくつかの数のRNA転写物を放出し、次にそれが1:1でcDNAに逆転写され、分割され、PCR増幅され、そして羅列される可能性について検討する。
最後のステップには、特定の抗がんレジメンがその患者の特定の腫瘍に対して有効であると予期されるかどうか判断するために、がん細胞、例えば、CTC内の特定のがん特異的又は系統特異的遺伝子の発現が、健常ドナー対照により決定される低バックグラウンドレベル(例えば、血液1mL当たり転写物2.5個のレベルに設定)を上回ることを検出するステップが含まれる。
上記で議論したように、正常な血球が周囲を取り巻く状況において、がん細胞に対して極めて特異的な遺伝子転写物の同定は、新しい方法の中核をなす。多くの遺伝子はがん細胞内でより高度に発現していることが公知であるが、これらの遺伝子の大多数についても、血液を含む正常な組織内ではその発現が少なくとも制限されているのが一般的である。極めて高い感度がこのアッセイには必要とされることから、正常な血球中にはシグナルが完全に存在しないことが、血流中の腫瘍細胞の信頼性が高い同定にとって必須である。
下記の表3は、遺伝子の名称((Genbank ID)及び配列識別番号(配列番号)を含む)のリストを、それが選択的であるがん型と共に提供する(Br:乳房、Lu:肺、Li:肝臓、Pr:前立腺、Panc:膵臓、Mel:メラノーマ)。さらに、デジタルPCR産物を最適に可視化するための蛍光プライマープローブの組成と共に(例えば、タグ化プローブ用の6−FAM(商標)(青色の蛍光標識)又はHEX(商標)(緑色の蛍光標識)、及びZEN−31ABkFQクエンチャー)、最適化されたプライマーセットを遺伝子毎に列挙する(プライマー1及び2)。
CTCに由来するRNAの最低量からの腫瘍特異的mRNAの検出を改善するために、本発明者らは、微量のCTC−Chip生成物から多くの異なる遺伝子転写物を試験することができるマルチプレックスアッセイを確立した。このアッセイは、複数の独立した遺伝子の使用によるより高い感度/特異性と、インプットテンプレートの量には限りがある(したがって、複数の反応に希釈されるべきでない)という事実を組み合わせる。本発明者らのアッセイには、反応当たり4つの遺伝子が含まれ、各遺伝子は、蛍光コンジュゲートプライマーについて異なる比を選択することにより、二次元空間内にユニークに溶解する。したがって、単一の反応において、テンプレートを希釈することなく、4つの遺伝子転写物を独立して測定することができる。異なるがんについて、最大4つの異なる反応を実施し(すなわち、最大20個の異なる遺伝子転写物)、ネスト化されたRT−PCRデジタルアッセイを適用することにより、実施可能な反応の数の制限が無くなる。
転移性前立腺がんを有する実質的にすべての患者が、アンドロゲン遮断療法(androgen deprivation therapy)(ADT)の後に初期の臨床応答を経験する。腫瘍が去勢抵抗性を発現すると、患者の半分は、強力なアンドロゲン合成阻害剤アビラテロン(例えば、ZYTIGA(登録商標))による治療の後に持続的な第2の寛解を有する一方、他の半分は短期的な応答を有するに過ぎず、したがって代替療法又は併用療法から利益を享受する。CTC由来のシグネチャーが、ADT後の抗アンドロゲン療法に対する応答について予測マーカーを提供するか試験するために、一次治療においてアビラテロン療法を開始した、転移性のCRPCを有する患者25例について前方視的に評価した。
外科的切除又は放射線照射可能な時期に上皮がんを早期検出すれば、治癒の絶好の機会がもたらされ、がん播種性転移が最低限度である状況においてアジュバント化学療法を投与すれば、治癒の実現において、確立された転移性疾患を治療するよりもはるかに有効である。早期検出が重要であるのと全く同様に、適切な抗がん療法を選択することも重要である。本明細書に記載する新しい方法は、d−CTCアッセイ方法を使用して特定の種類のがんについて早期検出を実現するだけでなく、ふさわしい参照標準と組み合わせれば、患者の腫瘍遺伝子発現プロファイルに応じて、特定の患者における異なる治療レジメンの予測された有効性を判定及び比較するのに使用可能である。
下記の材料及び方法を下記実施例で使用した。
本実施例は、本明細書に記載する方法で使用可能である一般的なデジタルCTCアッセイプロトコールを提示する。この一般的なプロトコールの異なる態様は下記の実施例2で使用した。
2.サンプルを、4℃でスピンダウンする。上清をデカンテーションし、そしてSUPERase(商標)In(DTT非依存性RNAse阻害剤)+RNALater(登録商標)安定化溶液(RNAsesを阻害することによりRNAの分解を防止する)をペレットに添加する。サンプルを急速冷凍し、さらなる処理まで−80℃に置く。サンプルは−80℃で安定である。
3.以下に記載する実施例で使用したcDNA合成に対して、RNAの精製について2通りの異なる処理プロトコールが存在する。
a.サンプルを氷上で解凍した。
b.界面活性剤(NP40、Tween20)を使用してサンプルを直接溶解した。
c.溶解したサンプルをcDNA合成用として直ちに採取した(Superscript III)。
d.cDNA合成後、SPRI(Agencourt AMPure(登録商標)XPビーズ)クリーンアップによりサンプルを精製して、界面活性剤及び<100bpsのヌクレオチドのすべてを除去した。
a.サンプルを氷上で解凍した。
b.サンプルをRNeasy Qiagen Micro Kit上で処理した。従来のQiagen勧告法と比較してプロトコールに若干変更を施した。バッファーRLT(溶解バッファー)につき大きめの容積、並びにより高いETOH濃度を使用した。RNALater(登録商標)をサンプルに添加するのでこれらの改変を行った。
c.cDNA合成後−SPRI(Agencourt AMPure XPビーズ)クリーンアップによりサンプルを精製して、界面活性剤及び<100bpsのヌクレオチドのすべてを除去した。
4.cDNA(アプローチ1又は2から合成した)は、2通りの異なる方法で処理することができる:
a.cDNAをddPCR用として直接使用した;又は
b.cDNAをFluidigm BioMark(商標)Nested PCRアプローチ(ネステッドPCRで使用される遺伝子に由来するプライマーを事前に妥当性確認した)を使用して増幅した。増幅したcDNAを希釈した。
5.cDNA(ステップ4a又は4bに由来)、プローブ用のBiorad Supermix(商標)、1つ又は複数のプライマー(目的とする遺伝子に対する;最多4個の異なるプライマー(FAM及びHEX)を多重化することができる)を総容積22μlに添加した。
6.液滴を生成した(1ウェル当たり約15,000〜18,000個の液滴)。
7.液滴サンプルをPCRマシンに導入した。PCR条件はBioradの推奨例とは異なった。すべての液滴が同一温度に達することを保証するために、スローランプよりはむしろステップダウンを使用した。これは、RainDance及びBioradの両社が使用するものとは異なる。より良好な結果(すなわち、陽性の液滴と陰性の液滴との間でのより多くのシグナル及びより多くの分離)は、グラジエントよりはむしろステップダウンにより取得可能である。
8.PCR後、ddPCRマシンにおいて陽性の液滴を計測した。
9.TIBCO(登録商標)Spotfire(登録商標)分析ソフトウェアを使用してデータを収集及び分析した。
試薬:
・プローブ用のBiorad ddPCR(商標)Supermix(dUTPを含まない)
・IDTプライマー/プローブ(20×又は40×)
・cDNA(細胞株について1ng/μl)
・ヌクレアーゼフリーの水
・エッペンドルフセミスカート96ウェルプレート(このプレートのみがマシンに適合する)。

遺伝子特異的プライマーに対して複数の患者、又は複数の遺伝子に対して多重化プライマーを試験する際には、ddPCR supermix及びプライマーを含むマスターミックスをウェル内に一定量分取し、その後患者cDNAを各ウェルに添加し、そして十分に混合した。
前立腺がんと診断された患者は、2つの治験審査委員会承認プロトコール、DF/HCC05−300又はDF/HCC13−209のうちの一方に対してインフォームドコンセントを提出した。転移性前立腺がんの患者、及び限局性前立腺がんの患者を含め、患者はCTC分析用として20mLの血液を供与した。ホルマリン固定化、パラフィン包埋した患者由来の原発腫瘍組織を切片化し、RNA抽出に供した後、液滴デジタルPCR用として処理した(下記参照)。
これまでの記載に従い、マイクロ流体CTC−iChipを使用して白血球を枯渇させた後に、CTCを新鮮な全血から単離した。RNAの品質が高いインタクトなCTCの回収量を最大化するために、血液サンプルを患者から収集してから4時間以内に処理した。検査室において新鮮血サンプルを入手した後、CTC単離の合計時間は、約2.5時間であった。手短に述べると、全血サンプルを、CD45(R&D Systems、クローン2D1)、CD66b(AbD Serotec、クローン80H3)、及びCD16(Janssen Diagnostics)に対するビオチン化抗体でスパイクとして添加し、その後、Dynabeads MyOneストレプトアビジンT1(Invitrogen)と共にインキュベーションして磁気標識化及び白血球の枯渇を実現した。CTC−iChipにより全血を処理し、そして氷上で濃縮したCTC生成物を収集した後、細胞を4750rpmで遠心分離し、RNAlater(登録商標)(Ambion)の存在下、液体窒素中で急速冷凍してRNAの完全性を保持した。
CTCサンプルを、RNeasy Plus Micro Kit(Qiagen)を使用してRNA抽出に供し、その後にSuperScript III First−Strand Synthesis System(Life Technologies)を使用して逆転写した。96ウェルプレート中で、cDNA及びプライマー/プローブを、プローブ用のddPCR Supermix(Bio−Rad)と合わせ、そしてBio−Rad社の自動液滴発生装置にロードした。変性とアニーリングステップとの間で70℃ステップダウンを含む改変された45サイクルPCRを使用して液滴を増幅した。熱サイクリング後、増幅された液滴を、QX200 Droplet Reader System(Bio−Rad)を用いて蛍光により検出した。
ddPCRアッセイの検出限界を試験するために、CTC−iChipを使用して一連の細胞スパイキング実験を実施した。単一のLNCaP細胞を、10μmエッペンドルフTransferMan(登録商標)NK2移送チップを使用してKolliphor P188バッファー中で操作し、そして男性健常ドナーの血液中にスパイクとして添加した。スパイクしたサンプルを、上記のように処理するために調製し、CTC−iChipにかけた。RNA及びcDNAを単離し、そしてCTC−iChip生成物から調製し、そして反応1及び2を使用してddPCRに供した。
蛍光に基づくイメージング、及び夾雑している血球と混ざり合ったCTCのスコアリングにおける固有の限界を踏まえ、本発明者らは、RNAに基づくデジタルPCR定量法が、よりハイスループット、より高感度、及びより特異的な読み取りを提供し得るか試験した。血液サンプル由来の造血細胞のマイクロ流体(CTC−iChip)枯渇はCTCについて104〜105倍の精製を実現する一方、処理全血1mL当たり約500個のWBCが残留する。精製されたCTC中のRNAの品質が高ければ、稀少なcDNAテンプレートが脂質液滴中に封入され、その後に陽性の液滴のPCR増幅及び蛍光スコアリングが続く、新規で極めて強固なデジタル液滴PCR技術の適用が可能となる。血液由来CTCのマイクロ流体による全細胞単離と、CTC由来転写物のRNAに基づくデジタルPCR(d−CTCアッセイ)とを組み合わせれば、転移性前立腺がんをモニタリングするための極めて特異的なマーカーとして、前立腺組織系統特異的mRNAを使用することができる(図1)。
転移性前立腺がんの患者12例を対象に、限局性前立腺がんの患者8例、男性健常血液ドナー34例(19例が>50歳;15例が<50歳)、及び女性対照5例と比較して、d−CTC検出戦略を試験した。8つのマーカーすべてにわたる観測シグナルを図7Aに示す。年齢がマッチした男性対照19例(>50歳)、及び転移性前立腺がんの患者12例を使用して、8遺伝子のそれぞれについてシグナル閾値を、対照のメジアンの上方2標準偏差に設定し、また遺伝子毎にシグナル強度が異なることを踏まえ、CRPC患者と年齢がマッチした対照との間のメジアン差異に比例してそのそれぞれに重み付けを行い(実施例1を参照)、これによりデジタルCTCスコアを導出した。陽性のデジタルCTCスコアが、健常男性血液ドナーの0/34と比較して、転移性前立腺がんの患者の11/12(92%)に存在した(図7B)。これらの厳密な基準の下、限局性前立腺がんの患者12例のいずれも、検出可能なCTCスコア(図7B)を有さなかった。興味深いことに、転移性前立腺がんの患者をモニタリングする際に、最高の特異性を有するスコアリング基準を設定したが、限局性のがんを有する個体数例において、低レベルのデジタルシグナルが存在した。健常個体では、>50歳の男性が、<50歳の男性よりも高いバックグラウンドシグナルを有し、また女性対照ではシグナルが実質的に存在しなかった(図7B)。
ミスセンス突然変異の再発は前立腺がんにおいて稀ではあるが、2つの特定のRNA融合転写物がこの腫瘍型に特徴的である。CTCにおける前立腺系統に基づく転写物の定量を補完するために、本発明者らは、50%のケースで存在するTMPRSS2−ERG融合転写物と、抗アンドロゲン療法に対する抵抗性のマーカーを構成するAR−V7 RNAスプライスバリアントの両方について、液滴PCRアッセイを開発した。両試験は、対照血液検体中にスパイクとして添加した前立腺細胞株に適用し、その後にCTC−iChip精製を行ったとき、極めて特異的且つ高感度であった(図8A及び8B)。転移性前立腺がんの男性由来の血液サンプルに適用したとき、mCRPC患者13例中5例(38%)がTMPRSS2−ERG転位を有し、11例(85%)がAR−V7スプライスバリアント有し、及び3例(23%)がそのCTC内に両方の転写物を有した(図8C)。年齢がマッチしたドナー12例から得た血液サンプルは、両方の転写物について陰性であった(図8D)。予期した通り、CTCがTMPRSS2−ERGについて陽性であった男性は、そのマーカーに対して概ね一致したアーカイブ原発腫瘍を有した(図8E)。対照的に、CTC由来のAR−V7シグナルは、マッチした原発性前立腺がんには実質的に存在せず(図8F)、進行したCRPCにおいて現われるマーカーとしてのその特徴と整合する。
転移性前立腺がんを有する実質的にすべての患者が、アンドロゲン遮断療法(ADT)の後に初期の臨床応答を経験する。腫瘍が去勢抵抗性を発現すると、患者の半分は、強力なアンドロゲン合成阻害剤アビラテロンによる治療の後に持続的な第2の寛解を有する一方、他の半分は短期的な応答を有するに過ぎず、したがって代替的療法又は併用療法からベネフィットを享受する。CTC由来のシグネチャーが、ADT後の抗アンドロゲン療法に対する応答について予測マーカーを提供するか試験するために、一次治療においてアビラテロン療法を開始した、転移性のCRPCを有する患者25例を前方視的に評価した。
ホルモン受容体陽性(「HR+」)疾患を有する患者において、例えばCDK4/6阻害剤と組み合わせて、エストロゲンシグナル伝達経路を標的とする薬物、例えばER阻害剤(例えば、タモキシフェン)、選択的ERディグレーダー(「SERD」、例えばフルベストラント等)、及びエストロゲンの産生を遮断するアロマターゼ阻害剤(AI)(例えば、アナストロゾール、レトロゾール、及びエキセメスタン)等で治療したときに、エストロゲンシグナル伝達と関連した6遺伝子抵抗性シグネチャー(「RS」)が持続的に発現していれば、進行までの時間(TTP)の短縮、及び全生存期間(OS)の不良(p=0.02(OS)、p=0.003(TTP))を含む有害な転帰と相関する。
患者は、CTC収集について治験審査委員会承認プロトコールを介して同意した(DFHCC 05−300)。アッセイの初回臨床ベンチマーキングでは、合計78例の固有患者から末梢血液10〜20ml(平均17ml)を延べ85サンプル収集した。これには、ステージIにつき23例、ステージIIにつき24例、及びステージIIIにつき8例の固有患者から得た治療前サンプル、並びに固有のステージIV患者23例由来の治療中サンプル30例が含まれる。女性健常ドナー(HD)に由来するサンプル33例を、血液バンク(平均容積9ml)から取得した。
RBC、WBC、及び血小板のネガティブ選択によって、全血から得たCTCを濃縮するためのCTC−iChip技術についてはすでに上記した。手短に述べると、8〜20mlの全血を、WBCマーカーCD45(R&D Systems、クローン2D1)、CD66b(AbD Serotec、クローン80H3)、及びCD16(Janssen Diagnostics)に対するビオチン化抗体と共にインキュベートした。Dynabeads MyOneストレプトアビジンT1(Invitrogen)を次に添加してWBCをタグ化した。その後、血液をCTC−iChip中に供給し、そこでサイズに基づく分離法によってRBC及び血小板を除去し、一方、WBCを磁気により枯渇させた。CTC濃縮生成物を遠心分離し、RNA−later(Ambion)内で保存し、長期保管用として急速冷凍した。
乳房CTC(AGR2、CXCL13、CXCL14、EFHD1、FAT1、FAT2、MGP、MUC16、PGR、PIP、PRAME、SCGB2A1、SERPINA3、SFRP1、SFRP2、TMPRSS4、WFDC2)に対する17個のマーカーを、文献調査、及び社内マイニング、及び乳がんにおいて発現しているが、全血中では発現していないマーカーに関するGTeX(登録商標)及びOncomine(登録商標)を含む公的に利用可能なデータセットによって選択した。乳がんアッセイを完成させるのに使用した特異的遺伝子及びIDTプローブを表6に列挙する。
L536R、Y537C、Y537N、Y537S、及びD538G ESR1突然変異に対して特異的なプローブはこれまでに公表されている。それらの増幅効率、並びにそれらの各野生型プローブの増幅効率を合成配列上で試験した(データ図示せず)。本発明者らは、健常ドナー血液中の増加しつつあるBRx−68細胞を顕微操作し、次にそれを上記のように処理することにより、Y537SがCTC濃縮IFD生成物からのcDNA中に存在する突然変異を検出する能力を有することを立証した。18サイクルWTAを、SMART−Seq v4 Ultra Low Input RNA Kit(Clontech)により、製造業者のプロトコールに従って、抽出したRNAの1/3を使用して実施した;1反応当たり1μlの未希釈WTA生成物を使用した。患者サンプルを同様に処理した;1プローブ当たり少なくとも5例の健常ドナーサンプルを試験した後、プローブ特異性を100%で立証した。ESR1突然変異の存在に対するカットオフを、>3の陽性の液滴で立証した。
本発明者らの初期の試験コホートにおける、各マーカーの特異性及び感度、並びに異なるがんステージに対する総CTCスコアを立証するために、受診者動作特性曲線分析を実施した。分析を、ROCRパッケージを使用してRで実施した。特定のスクリプトは要請に応じて入手可能である。AUCの有意性を立証するために、Wilcoxon検定を実施した。ステージIVのがんにおける特異性及び感度を、新しいセットの健常ドナー及びTRACKコホートからの治療前のサンプルを使用して妥当性確認した。
夾雑している正常血球のバックグラウンド内の乳がん細胞を検出するためにRNA発現シグネチャーを開発するために、最初に、正常な乳房組織、乳がん、及び全血に由来するRNA−Seq及びマイクロアレイ遺伝子発現データセットを上記のように分析した。発現が血球中には実質的に存在しないが、乳房由来の組織において強く発現している17個のマーカーを最終的に選択した。マーカーとして、乳房系統特異的転写物(PGR、SCGB2A1、PIP)及び乳がん中で高度に発現している転写物(MGP、EFHD1)、並びに内分泌シグナル伝達(SERPINA3、WFDC2)、内分泌薬抵抗性(AGR2)、がんの増殖及び転移(MUC16、TMPRSS4)、細胞シグナル伝達(FAT1、FAT2、SFRP1、SFRP2)、上皮由来サイトカイン(CXCL13、CXCL14)、及びがん胎児性抗原(PRAME)に関わる遺伝子が挙げられる。

Claims (19)
- 対象における特定のがんに対する抗がん剤の潜在的有効性を判定する方法であって、
前記対象に由来する血液サンプルから循環性腫瘍細胞(CTC)を単離することと、
CTC由来のRNAをcDNAに変換することと、
前記cDNAを個々の液滴中に封入することと、
CTCに由来するcDNAと特異的に結合し、且つ血液中のその他の細胞に由来するcDNAとは結合しないように構成されたレポーター基の存在下で、前記cDNAを各液滴中で増幅させることと、
一連の系統マーカーのどれが前記血液サンプル中の前記CTC内で発現しているか判定することと
を含み、特定の1つ又は複数の系統マーカーの発現レベルから、特定の抗がん治療レジメンに対する無増悪生存率及び全生存期間が予測される、方法。 - 前記対象における特定のがんに対する特定の抗がん治療レジメンの潜在的有効性が、前記対象の系統マーカーのうちの特定の1つ又は複数の発現レベルを、前記特定のがんに対する前記特定の抗がん治療レジメンについて確立された参照標準と比較して、前記対象が前記特定の抗がん治療レジメンにより改善するかどうか判断することにより決定される、請求項1に記載の方法。
- 前記対象が前立腺がんを有し、任意の治療を開始する前にアッセイされた前記対象の特定の系統マーカーが、がんを有さない健常ドナーの評価により決定されるバックグラウンドノイズレベルを上回り上昇したレベルのFOLH1(PSMA)及びHOXB13を含む場合、患者はアビラテロンのみで治療した場合には改善しないと予測する、請求項1に記載の方法。
- 前記対象が、アビラテロン及び別の抗前立腺がん療法の併用療法をさらに処方される、請求項3に記載の方法。
- 前記対象が、ホルモン受容体陽性(「HR+」)乳がんを有し、エストロゲンシグナル伝達経路を標的とする薬物による治療後の3〜4週間においてアッセイされた前記対象の特定の系統マーカーが、PIP、SERPINA3、AGR2、SCGB2A1、EFHD1、及びWFDC2遺伝子について、がんを有さない健常ドナーの評価により決定されるバックグラウンドノイズレベルを上回り上昇したレベルの1つ又は複数、例えば6つすべてを含む場合、患者は、前記エストロゲンシグナル伝達経路を標的とする薬物のみで治療した場合には改善しないと予測する、請求項1に記載の方法。
- 前記薬物が、ER阻害剤(例えば、タモキシフェン)、選択的ERディグレーダー(例えば、フルベストラント)、及びエストロゲンの産生を遮断するアロマターゼ阻害剤(AI)(例えば、アナストロゾール、レトロゾール、及びエキセメスタン)を含む、請求項5に記載の方法。
- 前記対象が、前記エストロゲンシグナル伝達経路を標的とする薬物及び別の抗乳がん療法の併用療法をさらに処方される、請求項5又は請求項6に記載の方法。
- RNAを単離する前に生成物の容積を低下させることをさらに含む、請求項1から7のいずれか1項に記載の方法。
- cDNA分子を封入する前に、cDNA含有溶液から夾雑物を除去することをさらに含む、請求項1から8のいずれか1項に記載の方法。
- 単離したRNAからcDNA分子を生成することが、前記単離したRNA分子の逆転写(RT)ポリメラーゼ連鎖反応(PCR)を実施することを含む、請求項1から9のいずれか1項に記載の方法。
- 前記液滴のそれぞれの内部でcDNA又はcDNA分子を増幅することが、各液滴中でPCRを実施することを含む、請求項1から10のいずれか1項に記載の方法。
- 個々のcDNA分子を封入することが、個々の液滴中のPCR試薬を前記cDNA分子と共に封入すること、及び非水性液体の少なくとも1000個の液滴を形成することをさらに含む、請求項1から11のいずれか1項に記載の方法。
- 前記レポーター基が、蛍光標識を含む、請求項1から12のいずれか1項に記載の方法。
- 前記液滴のそれぞれの内部でcDNA分子を増幅するのに使用されるプローブ及びプライマーが、表3に列挙されている選択されたがん遺伝子と関連する1つ又は複数のプローブ及びプライマーに対応する、請求項1から13のいずれか1項に記載の方法。
- 選択されたがん選択的遺伝子が、前立腺がん選択的遺伝子を含む、請求項14に記載の方法。
- 前記選択されたがん遺伝子が、乳がん選択的遺伝子を含む、請求項14に記載の方法。
- 前記選択されたがん遺伝子が、肺がん、膵がん、肝がん、及びメラノーマのうちの1つ又は複数に対して選択的な遺伝子を含む、請求項14に記載の方法。
- 前記CTCが、転移性又は原発性/限局性のがんに起因する、請求項1から17のいずれか1項に記載の方法。
- 血液サンプル中の循環性腫瘍細胞(CTC)から得られるcDNA分子を増幅及び検出するための、及び一連の系統マーカーのどれが前記血液サンプル中の前記CTCにおいて発現しているか判定するための、表3に列挙されている1つ又は複数の選択されたがん遺伝子と関連するプローブ及びプライマーの使用であって、特定の1つ又は複数の系統マーカーの発現レベルが、特定の抗がん治療レジメンに対する無増悪生存率及び/又は全生存期間について予測的である、使用。
