抗がん抗原T細胞受容体(TCR)を発現するよう遺伝学的に操作された細胞を用いる養子細胞療法(ACT)は、一部のがん患者において有益な臨床反応をもたらし得る。それにもかかわらず、がんや他の病気を、TCRを操作した細胞を使用して、首尾よく広範に治療することへの障害が残っている。例えば、がん抗原を特異的に認識するTCRは、同定すること及び/又は患者から単離することが困難であり得る。従って、がん反応性TCRを得る方法の改善についてのニーズがある。
本発明の一実施形態は、がん特異的突然変異によってコードされる突然変異アミノ酸配列に対し抗原特異性を有するTCR又はその抗原結合部分を単離する方法であって、それぞれの遺伝子が、突然変異アミノ酸配列をコードするがん特異的突然変異を含有する、患者のがん細胞の核酸中の1以上の遺伝子を同定すること;患者の自己抗原提示細胞(APC)を、突然変異アミノ酸配列を提示するよう誘導すること;患者の自己T細胞と、突然変異アミノ酸配列を提示する自己APCとを共培養すること;(a)突然変異アミノ酸配列を提示する自己APCと共培養されて、且つ(b)患者によって発現される主要組織適合性複合体(MHC)分子の関係で提示される突然変異アミノ酸配列に対し抗原特異性を有する、自己T細胞を選択すること;及び選択された自己T細胞から、TCR又はその抗原結合部分をコードするヌクレオチド配列を単離することを含み、TCR又はその抗原結合部分は、がん特異的突然変異によってコードされる突然変異アミノ酸配列に対し抗原特異性を有する、方法を提供する。

Jones et al.,Science 330:228−231(2010)に記載の通り、凍結保存した腫瘍組織(OCTに包埋)及び正常末梢血細胞の全エクソーム配列決定を、Personal Genome Diagnostics(PGDx、Baltimore、MD)により行った。配列の、それぞれの塩基での、明確で質の高い平均読み取り数は、腫瘍及び正常の(PBMCの)DNAについて、それぞれ155及び160であった。
患者3737を、制度審査委員会(IRB)承認のプロトコル:「転移性消化管がんにおけるリンパ球枯渇療法後の、短期間培養自己腫瘍浸潤リンパ球を用いた第II相試験(治験登録ID:NCT01174121)」に登録した。該プロトコルは、消化管がん患者における、ex vivoで増幅した自己腫瘍浸潤リンパ球(TIL)の養子移入の安全性と有効性を評価するために計画した。

DNeasy blood and tissue kit(Qiagen)を用いて、末梢血、T細胞及び凍結した腫瘍組織から単離したゲノムDNAに対して、immunoSEQ,Adaptive Biotechnologies(Seattle,WA)により、TCR−Vβディープシークエンシングを行った。サンプル当たりの、実益性のある(productive)TCRの読み取り総数は、279,482〜934,672の範囲であった。TCR頻度の算出においては、TCRの、実益性のある再配列のみを用いた。

自己フェレーシス試料を解凍し、5%の当所ヒト血清、10μg/mlゲンタマイシン(CellGro)、100U/mlペニシリン及び100μg/mlストレプトマイシン、1.25μg/mlアンフォテリシンB(Fungizone)並びに2mM L−グルタミン(いずれもLife Technologiesから)を添加した、RPMI及びAIM−V培地の50/50混合物からなるT細胞培地中、2×106細胞/mlに設定した。2×106細胞(1ml)を、レトロウイルスによる形質導入前の2日間、50ng/mlの可溶性OKT3(Miltenyi Biotec)及び300IU/ml rhu IL−2(Chiron)を含む24ウェルプレート中で刺激した。一過性レトロウイルス上清を生成するために、リポフェクタミン2000(Life Technologies)を用いて、Vβ22陽性ERBB2IP突然変異特異的TCRをコードするレトロウイルスベクターMSGV1(1.5μg/ウェル)及びエンベロープをコードするプラスミドRD114(0.75μg/ウェル)を、レトロウイルスパッケージング細胞株293GP(6ウェルのポリ−D−リジンコーティングプレート1ウェル当たり1×106細胞、トランスフェクションの前日にプレーティング)に共トランスフェクションした。トランスフェクション後42〜48時間で、レトロウイルス上清を回収し、DMEM培地で1:1に希釈し、レトロネクチンでコーティングした(10μg/ml,Takara)非組織培養処理6ウェルプレートに向けて、32℃で2時間、2,000gで遠心分離した。次いで、活性化T細胞(ウェル当たり2×106、IL−2を含有するT細胞培地中0.5×106細胞/ml)を、レトロウイルスプレートへ、300gで10分間スピンした。活性化T細胞を、一晩形質導入し、プレートから取り出し、IL−2を含有するT細胞培地中で更に培養した。GFP及びモックの形質導入コントロールを、形質導入実験に含めた。細胞は、通常、レトロウイルス形質導入の10〜14日後にアッセイした。
本実施例は、それぞれの遺伝子が、突然変異アミノ酸配列をコードするがん特異的突然変異を含有する、患者のがん細胞の核酸中の1以上の遺伝子を同定する方法を実例で示す。
本実施例は、患者の自己APCを、突然変異アミノ酸配列を提示するよう誘導する方法;患者の自己T細胞集団と、突然変異アミノ酸配列を提示する自己APCとを共培養する方法;及び(a)突然変異アミノ酸配列を提示する自己APCと共培養されて、(b)患者によって発現されるMHC分子の関係で提示される突然変異アミノ酸配列に対し抗原特異性を有する、自己T細胞を選択する方法を実例で示す。
−1BBの上方制御により実証される通り(表4A及び4B)、CD4+T細胞集団において反応性が優勢であった。表4A及び4Bは、非特異的刺激剤OKT3と共に培養したDC、又は緑色蛍光タンパク質(GFP)RNA若しくは、全エクソーム配列決定によって同定された種々の突然変異をコードする、表示したタンデムミニ遺伝子(TMG)コンストラクトをトランスフェクションしたDCとの共培養後に、フローサイトメトリーによる検出で、表示した表現型を有するとされた3737−TILのパーセンテージを示す。モックトランスフェクションした細胞は、核酸の添加なしで、トランスフェクション試薬のみで処理した。データを、生きているCD3+細胞にゲートした。
737−TILと、DMSO、突然変異(mut)ALK又は突然変異ERBB2IPの25アミノ酸長ペプチドで一晩パルスした、自己B細胞又はHLA−DQ遺伝子座が一部適合する、同種異系EBV−B細胞とを共培養した(図2D)。
本実施例は、実施例2で同定したERBB2IP特異的CD4+T細胞のTCR−Vβ22鎖で(そのα鎖と対応させて)遺伝的に改変した自己オープンレパートリー末梢血T細胞(autologous open repertoire peripheral
blood T cells)が、突然変異ERBB2IPペプチドに対する特異的反応性を付与したことを示す。
B細胞とを、6時間共培養した。表6は、フローサイトメトリーによって測定した、共培養後に細胞内IFN−γを産生する(IFN−γ+)か、又は細胞内IFN−γを産生しない(IFN−γ−)、CD4+でVβ22+及びVβ22−のクローンのパーセンテージを示す。データは、同一のTCR−Vβ配列を共に有する2クローンのうちの代表例である。
本実施例は、実施例2で同定した自己細胞を用いて、がんを治療する方法を実例で示す。
には、最大の30%減退に達した(図3C)。患者は、細胞注入後約13ヶ月間、疾患の安定化を経て、その後、疾患の進行は肺のみで認められ、肝臓では認められなかった。

アミノ酸配列の805位の変化に対応する、コード配列の2414位のヌクレオチドであった。定量的RT−PCRによって決定された通り、元の肺病変及び再発肺病変の両方において、比較的高レベルのERBB2IP発現が観察され(図4C)、サンガー配列決定により、すべての腫瘍病変において、ERBB2IP突然変異の存在が確認された。
本実施例は、突然変異反応性Th1細胞の、実施例4の抗腫瘍応答への寄与を実証する。
するために、>95%がVβ22+ERBB2IP突然変異反応性Th1細胞(約1200億の突然変異反応性細胞)で構成されるTIL産物を作製し、患者3737に養子移入した。
実施例8〜10についての材料及び方法を以下に示す。
すべての患者の材料は、国立がん研究所の施設内治験審査委員会が承認した臨床試験の過程で得た。患者2359及び患者2591を、Dudley et al.,J.Clin.Oncol.,26:5233−9(2008)に記載されている臨床試験(それぞれ、試験登録ID:NCT00096382及びID:NCT00335127)に登録した。患者に切除術を施し、それからTIL株と腫瘍細胞株の両方を樹立した。本研究
に使用するTILは、Dudley et al.,J.Immunother.,26:332−42(2003)に記載の方法により作製した。簡潔に述べると、腫瘍断片を切除し、IL−2含有培地中で培養した。増幅したTIL培養物の数を、自己又はHLA適合腫瘍の認識についてスクリーニングし、急速増幅プロトコル(REP)(Riddell et al.,Science,257:238−41(1992))を用いて、反応性TILの数を、IL−2、抗CD3抗体及び放射線照射したフィーダー細胞により、患者注入用に大量増幅した(Riddell et al.,Science,257:238−41(1992))。一部少数のTILに第2のREPを施した。T細胞及び腫瘍細胞を、共培養アッセイ用に、200μLの培地(5%ヒト血清を添加したAIM−V培地)を含む96ウェルプレート中1:1の割合で、16時間(hr)培養した。
該方法は、Robbins et al.,Nat.Med.,19:747−52(2013)に記載されている。ゲノムDNA精製、ライブラリー構築、約20,000のコード遺伝子のエクソーム捕獲(capture)、並びに腫瘍試料及び正常試料の次世代配列決定を、Personal Genome Diagnostics(Baltimore,MD)で行った。要約すると、腫瘍試料及び正常試料由来のゲノムDNAを断片化し、Illumina TRUSEQ library construction(Illumina,San Diego、CA)に用いた。エクソン領域は、Agilent SURESELECT 50Mb kit(version 3)を、製造業者の指示書(Agilent,Santa Clara、CA)に従って溶液中に捕捉した。HISEQ 2000 Genome Analyzer(Illumina)を用いて、各フラグメントの各末端から100塩基を得るペアエンド配列決定を行った。配列データを、参照用のヒトゲノム配列にマッピングし、配列改変を、5,000万塩基超の腫瘍DNA及び正常DNAの比較によって決定した。各試料について80億塩基超の配列データを得たが、該塩基の多くは、捕獲されたコード領域由来であった。腫瘍試料及び正常試料において、4300万塩基超の標的DNAを解析し、正常DNA試料及び腫瘍DNA試料における各塩基で、平均42〜51の読み取りを得た。
黒色腫試料由来の非同義突然変異を、全エクソーム配列決定データから同定した。同定された突然変異アミノ酸6残基(そのN−及びC−末端両側に、両側の12アミノ酸が隣
接する)を含有するポリペプチドをコードするタンデムミニ遺伝子コンストラクトを合成し(Integrated DNA Technologies,Coralville、Iowa)、次いで、IN−FUSION Advantage PCR Cloning Kit(Clontech)を用い、製造業者の指示書に従って、pcDNA3.1発現ベクターにクローニングした。
腫瘍細胞株及びペプチドでパルスした標的細胞に対する応答を、15μg/mlの抗IFN−γモノクローナル抗体1D1K(Mabtech、Inc.,Cincinnati,OH)でコーティングした96ウェルPVDF膜フィルタープレート(EMD Millipore,Billerica,MA)を用いて、IFN−γELISPOTアッセイで定量した。結合したサイトカインを、1μg/mlのビオチン化抗IFN−γ抗体7−B6−1(Mabtech)を用いて検出した。HLA−A*0201、HLA−A*0205又はHLA−C*0701を発現するHEK293細胞を、ペプチドで、37℃で2時間パルスした。以下のペプチド:MART−1:AAGIGILTV(配列番号54)、突然変異KIF2C:RLFPGLTIKI(配列番号55)、突然変異POLA2:TRSSGSHFVF(配列番号56)を使用した。T細胞を、標的細胞又は50ng/ml PMA及び1μMイオノマイシンを含有する培地(PMA/I)と一晩共培養した。105T細胞あたりのスポット数を算出した。
本実施例は、TIL2359が、ミニ遺伝子ライブラリースクリーニングによって評価される通り、突然変異抗原を認識することを実例で示す。

本実施例は、TIL2591が、ミニ遺伝子ライブラリースクリーニングにより同定された突然変異抗原を認識することを実例で示す。
TIL2591が、DW−6変異体の各々(WT POLA2断片をコードするコンストラクトを除く)をトランスフェクションしたCOS−7細胞を認識することが示された(図6C)。これらの知見を検証するために、COS−7細胞に、HLA−C*0401、HLA−C*0701又はHLA−C*0702 cDNAと共に、WT又は突然変異の全長POLA2 cDNAコンストラクトのいずれかをトランスフェクションした。TIL2591 T細胞は、HLA−C*0701と突然変異POLA2コンストラクトをトランスフェクションした標的細胞のみを認識し、対応するWT転写産物ではしなかった(図6D)。POLA2コード領域のヌクレオチド1258における、CからTへのシングルの変化により、WT POLA2タンパク質の420位での、フェニルアラニンのロイシンへの置換がもたらされた。サンガー配列決定により、ゲノムDNA及びMel2591 RNAに由来するcDNAの両方が、1258位にWT及び突然変異ヌクレオチドの両方を含有する一方、患者2591のPBMCから単離したゲノムDNAは、WT配列に対応することが示された、このことにより、これが、Mel2591細胞におけるヘテロ接合性体細胞突然変異を表すことが示された。
本実施例は、ミニ遺伝子ライブラリースクリーニングにより同定した、消化管(GI)がんに存在する突然変異抗原に対して反応性のT細胞を同定する方法を実例で示す。

Claims (20)
- がん特異的突然変異によってコードされる突然変異アミノ酸配列に対し抗原特異性を有するT細胞受容体(TCR)又はその抗原結合部分を単離する方法であって、
それぞれの遺伝子が、突然変異アミノ酸配列をコードするがん特異的突然変異を含有する、患者のがん細胞の核酸中1以上の遺伝子を同定すること;
患者の自己抗原提示細胞(APC)を、突然変異アミノ酸配列を提示するよう誘導すること;
患者の自己T細胞と、突然変異アミノ酸配列を提示する自己APCとを共培養すること;(a)突然変異アミノ酸配列を提示する自己APCと共培養されて、且つ(b)患者によって発現される主要組織適合性複合体(MHC)分子の関係で提示される突然変異アミノ酸配列に対し抗原特異性を有する、自己T細胞を選択すること;及び
選択された自己T細胞から、TCR又はその抗原結合部分をコードするヌクレオチド配列を単離することを含み、TCR又はその抗原結合部分は、がん特異的突然変異によってコードされる突然変異アミノ酸配列に対し抗原特異性を有する、方法。 - 患者の自己APCを、突然変異アミノ酸配列を提示するよう誘導することが、突然変異アミノ酸配列を含むペプチド又はペプチドプールでAPCをパルスすることを含み、プール中の各ペプチドは異なる突然変異アミノ酸配列を含む、請求項1の方法。
- 患者の自己APCを、突然変異アミノ酸配列を提示するよう誘導することが、突然変異アミノ酸配列をコードするヌクレオチド配列をAPCに導入することを含む、請求項1の方法。
- 自己APCに導入されるヌクレオチド配列がタンデムミニ遺伝子(TMG)コンストラクトであり、各ミニ遺伝子が異なる遺伝子を含み、各遺伝子が突然変異アミノ酸配列をコードするがん特異的突然変異を含む、請求項3の方法。
- 腫瘍の複数断片を患者から得ること、該複数断片からの自己T細胞のそれぞれを別々に、突然変異アミノ酸配列を提示する自己APCと共培養すること、及び複数断片のそれぞれからのT細胞を、突然変異アミノ酸配列に対する抗原特異性に関して別々に評価することを更に含む、請求項1〜4のいずれか1項の方法。
- 突然変異アミノ酸配列に対し抗原特異性を有する自己T細胞を選択することが、突然変異アミノ酸配列に対し抗原特異性を有する自己T細胞を選択的に増殖させることを含む、請求項1〜5のいずれか1項の方法。
- 突然変異アミノ酸配列に対し抗原特異性を有する自己T細胞を選択することが、プログラム細胞死1(PD−1)、リンパ球活性化遺伝子3(LAG−3)、T細胞免疫グロブリン及びムチンドメイン3(TIM−3)、4−1BB、OX40、及びCD107aのいずれか1以上を発現するT細胞を選択することを含む、請求項1〜6のいずれか1項の方法。
- 突然変異アミノ酸配列に対し抗原特異性を有する自己T細胞を選択することが、(i)突然変異アミノ酸配列を提示するAPCとの共培養に際して、ネガティブコントロールによって分泌される1以上のサイトカインの量と比較して、より多くの量の1以上のサイトカインを分泌するか、又は(ii)突然変異アミノ酸配列を提示するAPCとの共培養に際して、1以上のサイトカインを分泌するネガティブコントロールT細胞の数と比較して、少なくとも2倍の多さの数のT細胞が1以上のサイトカインを分泌するT細胞を選択することを含む、請求項1〜7のいずれか1項の方法。
- 1以上のサイトカインが、インターフェロン(IFN)−γ、インターロイキン(IL)−2、腫瘍壊死因子アルファ(TNF−α)、顆粒球/単球コロニー刺激因子(GM−CSF)、IL−4、IL−5、IL−9、IL−10、IL−17及びIL−22を含む、請求項8の方法。
- がん細胞の核酸中の1以上の遺伝子を同定することが、がん細胞の全エクソーム、全ゲノム、又は全トランスクリプトームを配列決定すること含む、請求項1〜9のいずれか1項の方法。
- がん特異的突然変異によってコードされる突然変異アミノ酸配列に対し抗原特異性を有するTCR又はその抗原結合部分を発現する細胞集団を調製する方法であって、
請求項1〜10のいずれか1項の方法により、TCR又はその抗原結合部分を単離すること、及び
単離されたTCR又はその抗原結合部分をコードするヌクレオチド配列を末梢血単核細胞(PBMC)に導入して、TCR又はその抗原結合部分を発現する細胞を得ること
を含む、方法。 - TCR又はその抗原結合部分を発現するPBMCの数を増幅することを更に含む、請求項11の方法。
- 請求項1〜10のいずれか1項の方法により単離された、TCR又はその抗原結合部分。
- 請求項11又は12により調製された、単離された細胞集団。
- (a)請求項13のTCR若しくはその抗原結合部分、又は(b)請求項14の、単離されたT細胞集団、及び
医薬的に許容可能な担体
を含む、医薬組成物。 - 哺乳動物におけるがんの治療又は予防において使用するための、請求項13のTCR若しくはその抗原結合部分、請求項14の単離された細胞集団、又は請求項15の医薬組成物。
- がんが上皮癌である、請求項16に記載の使用のための、TCR若しくはその抗原結合部分、単離された細胞集団、又は医薬組成物。
- がんが胆管がん、黒色腫、結腸がん又は直腸がんである、請求項16に記載の使用のための、TCR若しくはその抗原結合部分、単離された細胞集団又は医薬組成物。
- PBMCが患者に対して自己である、請求項16〜18のいずれか1項に記載の使用のための、単離された細胞集団、又は単離された細胞集団を含む医薬組成物。
- PBMCが患者に対して同種異系である、請求項16〜18のいずれか1項に記載の使用のための、単離された細胞集団、又は単離された細胞集団を含む医薬組成物。
