Abstract
Description
本出願は、2016年5月12日付けで出願された米国特許出願第62/335,561号に基づき、その優先権を主張するPCT出願であり、その全体が完全に記載されたものとして本明細書に組み込まれる。
敗血症の原因は、一般的に、感染と考えられている。しかしながら、専門的に言えば、敗血症は、免疫系が感受性を有する病原体関連分子パターン(PAMP)に起因する。これらは、Toll様受容体(TLR)、C型レクチン受容体(CLR)および核酸モチーフに応答する細胞質内パターン認識受容体(cPRR)を誘発する。加えて、死細胞片からのダメージ関連分子パターン(DAMP)も炎症性サイトカインを誘発する。
本発明の方法および構造の発明の特徴を例示するために、CRISPR(クラスター化された規則的に間隔を空けられた短いパリンドローム反復(clustered regularly interspaced short palindromic repeat))の細胞年数の概念が、本明細書にわたり使用されることになる。この概念に関して、絶対的に完璧なCRISPR系は決してないと仮定されている。意図しないDNAを改変するオフターゲット事象の確率は、一部の極めて低い数値×発現レベルに、細胞の数×CRISPR系がそれらの細胞において活性である年数を掛けた値である。
CHYSEL系は、翻訳後にタンパク質の分割を可能にする特有のアミノ酸配列をウイルスが有していると認識された後に開発された。この革新により、単一のプロモーターから等モル量の様々なタンパク質を発現させることが可能になった。これは、その発現を大いに簡単にすることができる。
遺伝子治療一般、特に「クラスター化された規則的に間隔を空けられた短いパリンドローム反復」(CRISPR)遺伝子治療の開発を妨げる2つの問題がある。これらの問題は、ヒト体内の十分な細胞へのDNAまたはRNAの送達と、長過ぎるCRISPR発現である。
遺伝子治療患者であるジェシー・ゲルシンガー(Jesse Gelsinger)は、遺伝子治療薬を送達するために肝静脈に投与された用量3.8×1013個のアデノウイルス粒子が原因で死亡した。その結果として、遺伝子治療を達成するために注入されるビリオンの通常の限界は、1012個である。本発明は、この患者の死を引き起こした症候群を予防することを意図している。本発明者は、大学院を通じてこの問題を認識していたが、他のほとんどの者と同様に、その解決が難しいこと、さらに、それを解決する方法は、アデノウイルスカプシド、アデノ随伴ウイルス(AAV)カプシド、レンチウイルスカプシドなどより免疫原性が低い代替の送達方法を発見することであると認めていた。今日に至るまで、このストラテジーに対する支持者がなお多く存在する。しかしながら、先の開示である米国特許出願第13/298,251号において、本発明者は、細胞内で作用すると思われる抗HIV抗体をコードする遺伝子を送達する遺伝子治療法を使用してHIVを治療することを記載した。この特許の送達の態様は、人体におけるトランスフェクションに関与する数に焦点を合わせたものである。免疫系の炎症性の態様や、それを制御するための方法、および、人工的に作り出された敗血症と感染で起こる敗血症との間の応答の相違は、発明者の心中では明らかであった。残念なことに、免疫系の過剰活性化に対してヒト免疫学が唯一無二であることに気付いている者は、がんを治療するための微生物の方法を取り扱う医師や科学者を含めてもごく少数である。結果として、ほとんどの専門家は、必要なものと現存の方法を使用可能なものとの間のギャップの規模を認識していない。これに対処するためには、免疫系の炎症サイクルを阻害するかまたはそれ以外の方法で抑制もしくは抑止するモジュレーターを用いて免疫系を制御することが必要である。
CRISPRは、それが何を行うかによっては実に効率的であるが、DNAに対するオフターゲットの変化の問題がある。オリジナルのCAS9より優れた、非常に低いオフターゲット率を有する酵素がある。CAS9酵素が活性である時間が長ければ長いほど、潜在的な問題を伴うオフターゲットの変化が生じる確率がより高くなる。おそらくその可能性は極めて低いが、それでもなお懸念がある。体は、天の川銀河中の星の数を100分の1にしたほどの多数の細胞を有することから、非常に低い確率の事象でも比較的起こりやすい。そのため、特定のCRISPR系による活性の細胞年数を最小化することが望ましい。ほぼ完全な改変が全てのトランスフェクトされた細胞で起こるのに、数日かからないという仮説がある。
対処すべき第1の問題は、どのように実際のMOTをヒトで使用されるために100もの高さにするかである。関与するメカニズムの性質を考慮すれば、実際のMOTの100は、形式的なMOTがin vivoで10,000〜100,000であることを要することを示唆すると予想される。以下で教示されるように、炎症を十分に阻害するには、一連の免疫系モジュレーターが必要である。
a.3−[4−(6−(3−(ジメチルアミノ)プロポキシ)ベンゾ[d]オキサゾール−2−イル)フェノキシ]−N,N−ジメチルプロパン−1−アミン
b.6−[3−(ピロリジン−1−イル)プロポキシ)−2−(4−(3−(ピロリジン−1−イル)プロポキシ)フェニル]ベンゾ[d]オキサゾール
c.ヒドロキシクロロキン
一次免疫刺激のリスクは、生きた動物中で、特にヒトの場合、細胞に核酸を送達するのに使用される核酸送達媒体(NADV)に起因する。一般的にこれらは、現存のウイルス、例えばアデノウイルス(AV)、アデノ随伴ウイルス(AAV)、ヒト免疫不全ウイルス(HIV)などの組換え体である。
上記の薬剤および生物製剤を使用する方法は、薬剤との中和作用により免疫系の活性化を低減または排除するように努めるものである。結果として、人工の敗血症/多臓器不全の兆候への応答は、薬剤用量が不十分であると予想される開示された方法の不可欠な部分であってもよい。本明細書の残りに関して、症候群は単に敗血症と称されることになるが、最も実用的な目的の観点でもそれで十分であり、その起こるべき危険を十分伝えるものである。敗血症の誘発は、本事例ではNADVと見込まれる抗原の投与用量、ならびに肝機能状態および可能性のある未知の要因に依存する。患者は、慎重にモニターされるべきであり、評価は、遺伝子治療薬送達プロトコルの前に行うべきである。
a.24時間で1mg/kgから開始して5mg/kgまでのIVポリミキシンB硫酸塩の投与。この抗生物質のLD50は不明である。げっ歯類は、1kg当たり100mg以上に対して耐性を有する。イヌは、8mg/kgで死に始める。5mg/kgがおそらくガイドラインである。腎臓の機能に特別な注意を払い患者を慎重にモニタリングすることにより、緊急の状況ではそれより多くの用量が許容されてもよい。
b.DMOGの注射または経口投与。
c.IL−10の注射。この根拠は、動物実験において、原因となった感染がなくなるまで敗血症を抑えることにおいてIL−10が有効であることが示されていることである。
d.グレリンの注射。グレリンは、並外れた作用で敗血症から救うことが示されている。
e.アダリムマブ、エタネルセプト、インフリキシマブ、DOI、またはTNF−αアンタゴニストとしての他のものの投与。DOI用量は100〜500マイクログラム/kgの範囲内であり、承認された薬剤ではない。
f.抗炎症性ステロイドの投与。
このようなことが起こる可能性は低いが、患者にこれらの薬剤を投与するにつれ、その期間が長くなる場合、患者の状態が、重篤な放射線中毒を受けているヒトの状態に類似してくる可能性がある。例えば、患者が予想外の肝臓障害を有する場合、または遺伝子治療薬の接種から抗原性物質を取り除くために患者に異常に長い時間を与えることが必要な場合、患者は、好中球、NK細胞およびT細胞の減少を認めることもある。これは、エポジェン、グレリンおよび/または抗生物質ならびに他の放射線中毒の薬物標準治療と共に、それらを投与することを必要とする場合がある。感染を予防するために、消化管の運動性の維持を確実するように注意を払うべきである。この状況のときに患者が感染に罹患した場合、1.6〜2.2バールで1時間の高圧酸素治療を開始し、患者が明らかによくなるまで繰り返すべきである。
患者が、処置の一部の要素に対してアナフィラキシー反応を起こし得ることが考えられる。この状況に対処するための標準的な材料が利用可能であり、手元にあることが好ましい。(例えば、エピネフリン、コルチコステロイド、抗ヒスタミン剤、気道用の器具)。コルチコステロイドは免疫系応答を劇的に妨害するため、エピネフリンおよび抗ヒスタミン剤が好ましい。化学療法でなされる方法と類似した予防薬としての抗ヒスタミン剤の投与は、ここで記載された手順を妨害しないと予想される。
本出願の目的に関して、CRISPRカセットは、CRISPR系が活性化するために必要とされる全ての遺伝子、またはその系に対する任意の部分を指す。この用例におけるCRISPRカセットは、1つ以上のプラスミドを包含していてもよいし、または全てが1つのプラスミド中にあってもよい。この用例において、プラスミドはまた、初代大腸菌(E. coli)ゲノム中に発現カセットを作り出し、トポイソメラーゼまたは同様の酵素を使用して大腸菌遺伝子から発現カセットを切り出すような系も包含する。
本発明は、臨床的な治療の重要な2分野、CRISPR遺伝子治療および一般的な遺伝子治療に適用される。どちらの分野も社会への高い重要性を有しており、この分野における改善は大きな利益を提供することができる。それゆえに、前述のものは、本発明の原理の単なる例示とみなされる。さらに、当業者であれば数多くの改変および変化を容易に思いつくと予想されるため、示され説明された正確な開示に本発明を限定することは望ましくなく、したがって、全ての好適な改変および均等物は、本発明の範囲によって決まり、その範囲内に含めることができる。
前述の発明は、医学や、家畜を用いる農作業における遺伝子治療の分野の範囲内で製造および適用することができる物理的な産物である。
(パート1の好ましい実施態様)
本発明のパート1の好ましい実施態様は、免疫系を阻害するための薬剤の送達を備える方法であり、本方法は、以下のステップを備える。
a.NFκBの阻害剤。
b.阻害剤、または、IRF3、TLR3、TLR4およびTLR7/8。毒性がより低いため、SSRI、セルトラリンが好ましい。他のSSRI薬剤またはSNRI薬剤も、この目的のために機能することを証明してもよい。
c.必要な場合、TLR9阻害剤。
d.TNFα阻害剤。
e.mTOR阻害剤。
f.生きた動物の細胞に核酸を送達するためのNADV。
本発明のパート2の好ましい実施態様は、以下の構造を備える。
a.CpG−ODNを含有する発現ベクタープラスミド。
b.TetR系依存性のプロモーターを有する発現ベクタープラスミド。
c.プロモーターと、その間にCHYSELリンカーを有する複数のタンパク質コード配列とを有する発現ベクタープラスミド。

(項目3)
前記mTOR阻害剤は、ラパマイシン、テムシロリムス、エベロリムスおよびデフォロリムスからなる群から選択される、項目2に記載の方法。

選択的セロトニン再取り込み阻害剤(SSRI)を投与するステップをさらに備え、
前記送達するステップは、それぞれの前記投与するステップの後に行われる、項目1に記載の方法。
(項目5)
前記SSRIは、シタロプラム、エスシタロプラム、フルオキセチン、フルボキサミン、パロキセチン、ダポキセチン、インダルピン、ジメリジン、セリクラミンおよびパヌラミンからなる群から選択される、項目4に記載の方法。
(項目6)
セロトニン−ノルエピネフリン再取り込み阻害剤(SNRI)を投与するステップをさらに備え、
前記送達するステップは、それぞれの前記投与するステップの後に行われる、項目1に記載の方法。
(項目7)
前記SNRIは、ベンラファキシン、シブトラミン、デュロキセチン、アトモキセチン、デスベンラファキシン、ミルナシプランおよびレボミルナシプランからなる群から選択される、項目6に記載の方法。
(項目8)
前記TNFα阻害剤は、アダリムマブ、エタネルセプト、インフリキシマブまたは2,5−ジメトキシ−4−ヨードアンフェタミンからなる群から選択される、項目1に記載の方法。
(項目9)
前記NFκB阻害剤は、エクテイナシジン743、ジギトキシン、ウアバイン、ボルテゾミブ、クロモマイシンA3、エメチン、フルオロサラン、ナラシン、レスタウルチニブ、トリブロンサラン、ビチオノールおよびダウノルビシンからなる群から選択される、項目1に記載の方法。
(項目10)
前記IRF3阻害剤は、セルトラリン、トリフルオペラジンまたはフルフェナジンからなる群から選択される、項目1に記載の方法。
(項目11)
前記TLR9阻害剤は、3−[4−(6−(3−(ジメチルアミノ)プロポキシ)ベンゾ[d]オキサゾール−2−イル)フェノキシ]−N,N−ジメチルプロパン−1−アミン、6−[3−(ピロリジン−1−イル)プロポキシ)−2−(4−(3−(ピロリジン−1−イル)プロポキシ)フェニル]ベンゾ[d]オキサゾールまたはヒドロキシクロロキンからなる群から選択される、項目1に記載の方法。
(項目12)
前記NADVは、ウイルスカプシドである、項目1に記載の方法。
(項目13)
前記NADVは、ポリプレックスである、項目1に記載の方法。
(項目14)
前記NADVは、エンベロープ型ポリプレックスである、項目1に記載の方法。

a.前記テトラサイクリン誘導プロモーターの制御下で2つ以上の遺伝子配列を連結する1つ以上のシス作用性ヒドロラーゼエレメント(CHYSEL)配列をさらに備え、

Claims (19)
- 動物の細胞に核酸配列を送達する方法であって、
a.前記動物に、
i.腫瘍壊死因子アルファ(TNFα)阻害剤、
ii.核内因子κB(NFκB)阻害剤、
iii.インターフェロン調節因子3(IRF3)阻害剤、
iv.Toll様受容体9(TLR9)阻害剤、
を備える免疫系モジュレーターを投与するステップと、
b.前記動物の細胞内に、前記核酸配列を送達するステップと、
を備え、
c.前記送達するステップは、前記投与するステップの後に行われ、
d.前記核酸配列は、核酸送達媒体(NADV)を使用して送達される、方法。 - ラパマイシンの機構的標的(mTOR)阻害剤を投与するステップをさらに備え、
前記送達するステップは、それぞれの前記投与するステップの後に行われる、請求項1に記載の方法。 - 前記mTOR阻害剤は、ラパマイシン、テムシロリムス、エベロリムスおよびデフォロリムスからなる群から選択される、請求項2に記載の方法。
- 選択的セロトニン再取り込み阻害剤(SSRI)を投与するステップをさらに備え、

- 前記NADVは、ウイルスカプシドである、請求項1に記載の方法。
- 前記NADVは、ポリプレックスである、請求項1に記載の方法。
- 前記NADVは、エンベロープ型ポリプレックスである、請求項1に記載の方法。
