本発明は、一般に、金属中毒の処置に関する。
偶発的に散乱されたか、放射線分散装置によって意図的に散乱されたか、原子力発電所の事故または原子力装置の爆発によって散布された放射性核種への曝露により、多くの人々が汚染され得る。内在化された放射性核種は毒性が高く、急性および慢性の放射線傷害の両方を生じ得るので、かかる汚染事象による公衆衛生上の影響は甚大になる。
本明細書中の実施形態は、処置を必要とする被験体を処置する方法であって、治療有効量の1つまたは複数の、1,2−HOPOキレート剤および3,2−HOPOキレート剤を含む薬学的組成物を、かかる処置を必要とする被験体に投与するステップを含む方法を提供する。この方法は、1種または複数種の既知または未知のアクチニドおよび/もしくはランタニド、またはその混合物に対して曝露、接触、または汚染されているか、曝露、接触、または汚染されることがある被験体に対して実施する場合に特に有用である。

放射性物質に関する大きな事故が起こると、集団が大規模な外部被曝に見舞われるだけでなく、放射性核種の拡散や内部汚染を抑制することもできないという結果を招く可能性がある。放射線および核に関するインシデントに対する緊急時応答を計画する場合、汚染した個体の曝露後処置の必要性だけでなく、かかる事象の余波として、汚染された領域および患者と向き合う職業人を防御するための予防手段の必要性も考慮しなければならない。安全性、投与の容易さ、ならびに複数の放射性核種に対する広範囲の有効性およびチャレンジのレベルなどの曝露後処置についての望ましい基準を満たすことに加えて、理想的な予防対策は、即効性がなければならず、性能を低下させる副作用が最少であるか全くあってはならず;軍の化学、生物、放射性物質、核、および爆発物における現行の対策に適合しなければならず;且つ、運搬上の負荷が最小である必要がある。
用語「緊急事態」は、以下を含む:(a)任意の原子力事故に起因する環境中への放射性同位体の偶発的な放出事象。(b)環境中への有害な核種の任意の偶発的放出。(c)核降下物(実験、診断、または治療を目的とした通常の過程において生じる核降下物が含まれる)。(d)ヒトまたは動物の被験体による放射性核種のあらゆる種類の偶発的な取り込みおよび保持。(e)揮発性の放射性核種に対する他のあらゆる種類の曝露。(f)あらゆる種類の放射性物質事故。
以前の研究により、キレート剤での迅速な処置またはわずかに遅延した処置(ほとんどの場合、汚染事象の最初の24時間以内)の後に放射性核種の排出が増強されることが明らかとなった。アクチニドでのヒト汚染のシナリオには、無数の未知の事項が含まれるが、不運なことに、処置を広範に実施することやインシデント後に迅速に大集団に利用できるようにすることはありそうにないようである。

いくつかの実施形態では、1種または複数種の本明細書中に提供したキレート剤を使用して、ガドリニウム系造影剤に関連するリスクを低減することができる。いくつかの実施形態では、キレーターを、ガドリニウム系造影剤の使用前、使用と共に、または使用後に投与することができる。1988年の米国における臨床診療への導入以来、ガドリニウム系造影剤(GBCA)は、磁気共鳴画像法(MRI)に多大な影響を及ぼしていた。GBCAは、米国で各年実施される3000万回のMRIスキャンの半分超で使用されていると推定される。造影剤は安全と見なされており、関連する有害事象率は、全世界で100,000投与あたり14.4投与と報告されている。腎臓学者がGBCAの投与を腎性全身性線維症(NSF)と関連付けた最初の重篤な有害作用が報告されたのは使用から18年後のことで、これにより、2006年に米国食品医薬品局(FDA)は警告を発するよう促された。NSFの正確な病態生理学は依然として知られていないが、キレート化リガンドからのGdイオンの解離が主要病因として受け入れられている。キレートがin vivoで解離する時間を考慮すると、腎不全患者における排泄速度が低下しているので、この解離は腎機能が正常な患者よりも腎不全患者で起こる可能性が高い。文献に報告されたNSFのほとんどの症例が非イオン性直鎖ガドジアミド(オムニスキャン)の投与に関連しているが、報告には別の非イオン性直鎖薬剤であるガドベルセタミド(OptiMARK)による、およびイオン性直鎖薬剤であるガドペンテト酸ジメグルミン(マグネビスト)による実質的インシデントの記載もある。腎疾患患者の有効なスクリーニングおよび実質的な腎疾患を有する患者におけるGBCAの回避によって、またはより安定なGBCAの利用によってNSFの症例が極めて少数になったか存在しなくなったことと関連して、NSFは2009年以来ほぼ完全に排除されている。しかし、この2年間に、腎機能が正常な患者における神経組織内のGd沈着に関する多数の研究が発表されている。この沈着は、腎機能が正常な患者における非増強T1強調画像上の淡蒼球(globi pallidi)および/または歯状核内のシグナル強度の漸増が複数回のGBCA投与に関連するというMR画像化研究によって最初に想定された。NSFと同様に、この所見に最も関連する薬剤はガドジアミドであったが、この所見はその他の薬剤(ガドペンテト酸ジメグルミンが含まれる)を用いても示されている。
3,4,3−LI(1,2−HOPO)および5−LIO(Me−3,2−HOPO)の投与時間枠を調査することを目的とした2組の研究を本明細書中に示す。予防的な腹腔内処置または経口処置のための汚染から48時間前もの早期、および遅延腹腔内処置のためのチャレンジから7日後もの後期にプルトニウム排出の増強が記述される。これらの研究と以前に報告された研究との間の整合性を確実にするために、単回静脈内注射によって可溶性クエン酸プルトニウム(238Pu)による汚染を実施し、動物モデルとして若年成体雌スイス・ウェブスターマウスを選択した。これらの手順の利点は、組織試料および排泄物試料における正確な計数統計を得るために少量の放射線学的汚染物質しか必要としないこと、大量の放射性核種の在庫が回避されること、および取り扱う放射性物質の量が低減されることである。
2.1汚染物質およびリガンドの溶液
4M HNO3中の硝酸プルトニウム(238Pu)の保存溶液をEckert and Ziegler Isotope Products(Valencia,CA,USA)から購入し、注射液の調製のために使用した。汚染線量は、0.008Mクエン酸ナトリウムおよび0.14M NaCl(pH4)中に0.74kBq(1.16ng)の238Puを含む溶液の0.2mLアリコートからなる。以前に記載のように[19]、リガンド3,4,3−LI(1,2−HOPO)および5−LIO(Me−3,2−HOPO)を、Synthetech、Inc.(Albany,OR,USA)およびAlbany Molecular Research、Inc.(Albany,NY,USA)によってそれぞれ調製した。DTPAをSigma−Aldrich(St.Louis,MO,USA)から入手し、CaCO3およびNaOHを使用してCa−DTPAとして製剤化した。このCa−DTPAは、Hameln Pharmaceuticals gmbh(Hameln,Germany)から市販されていた以前に入手可能であった医薬品に類似する。選択された投薬量(Ca−DTPAについては30μmol/kg、3,4,3−LI(1,2−HOPO)については30または100μmol/kg、および5−LIO(Me−3,2−HOPO)については100または200μmol/kg)を0.5mLの0.14M NaClに含め、1N NaOHでpH7.4〜8.4に調整することによってリガンド溶液を調製した。投与前に全ての溶液を濾過滅菌した(0.22μm)。各溶液の濃度を、公開された方法の修正版に従った高速液体クロマトグラフィによって検証した。
記載のin vivo研究で使用した全ての手順およびプロトコールは、ローレンス・バークレー国立研究所の動物実験委員会によって審査および承認され、AAALAC認定施設で実施した。使用動物は、若年成体(遅延処置実験のためには86±6日齢、および予防的処置実験のためには90±3日齢)の雌(遅延処置実験のためには30.7±4.0g、および予防的処置実験のためには30.8±1.6g)のスイス・ウェブスターマウス(Simonsen Laboratories,Gilroy,CA,USA)であった。これらのマウスの全体の身体および組織組成、血漿、細胞外液、ならびに全身、主要な組織および器官の赤血球量(インタクトまたはマウスの総血液量の25〜40%を出血させた)は事前に決定している。マウスを、温度(18〜22℃)および相対湿度(30〜70%)を制御した12時間明周期下に維持し、水および飼料を自由に与えた。各マウス群を、尿および糞便を分離するための高吸収性の低灰分ペレット化セルロース床敷(ALPHA−dri(登録商標))を0.5cmの層として敷き詰めたプラスチック製のストックケージに収容した。温めた側尾静脈への静脈内(iv)注射、腹腔内(ip)注射、経口投与(po、胃挿管による)、および安楽死を、イソフルラン麻酔下で実施した。マウスの体重に基づいて処置用量体積を調整した(35gマウスに対応して0.5mLの体積を用いる)。遅延処置の影響を調べるために、5匹からなるマウス群に、単回用量のクエン酸プルトニウム(238Pu)をiv注射し、リガンド溶液またはコントロール食塩水溶液を、以下の汚染処置時間後に1回ip投与した:1時間、5時間、16時間、24時間、3日間、7日間。排泄物を、7日間にわたって毎日回収した。処置7日後に動物を安楽死させた。予防的処置の影響を調べるために、5匹からなるマウス群に、リガンド溶液またはコントロール食塩水溶液を以下の汚染前処置時間にipまたはpoで1回最初に投与した:−1時間、−6時間、−16時間、−24時間、−30時間、−40時間、−48時間。次いで、マウスに単回用量のクエン酸プルトニウム(238Pu)をiv注射し、排泄物を、3日間にわたって毎日回収した。汚染3日後(72時間)に動物を安楽死させた。マウスをその各ケージ上で頸椎脱臼によって安楽死させて死亡時に排泄した尿を回収し、直ちにプラスチックに包み、その後の解剖のために凍結した。
凍結したマウスの部分的解凍後、肝臓および腎臓を解剖し、腹部残余組織(ATR、インタクトな胃腸(GI)管、生殖器官、脾臓、膀胱、および腹部脂肪を含む)を除去した。肝臓、腎臓、ATR、および部分的に臓器摘出した屠殺体を、個別の試料として管理した。糞便試料を、尿で汚れたセルロース床敷から手作業で分離し、群の試料(ケージあたり1群)として処置した。全試料を100℃で乾燥させ、575℃で乾式灰化を行った。灰化試料を、濃HNO3で処理した。次いで、これらの酸性化溶液を希HNO3中でホモジナイズし、液体シンチレーションカウンティング(Packard Tri−CarbモデルB4430,Perkin Elmer)による放射性核種の検出のためにUltima Gold(Perkin Elmer,Shelton,CT,USA)と混合した。
全実験で汚染物質として放射性238Puを使用し、したがって、全ての組織および排泄物を放射能分析する代謝平衡研究として管理した;放射性化学物質の平均回収率は全て、95%注入用量(ID)を超えていた。実験データを、放射性核種の割合(注入した238Puに対するパーセント(%ID)として示す)として報告し、値は算術平均±SDである。体外排出研究において群間で値を比較する場合、用語「有意な」を、統計的意味で使用し、それは、一元配置分散分析(ANOVA)およびその後の適切な事後解析によってp<0.01を示す。ダネット多重比較検定を使用してキレート剤で処置した動物群と食塩水を投与した対応コントロール群とを比較した一方で、テューキー公正有意差(HSD)多重比較検定を使用してキレート剤で処置した全群間を対比較した。両検定を、99%信頼区間レベルで設定した。全統計解析を、GraphPad Prism 5(GraphPad Software,Inc.,San Diego,CA,USA)を使用して行った。
実験的体外排出剤3,4,3−LI(1,2−HOPO)および5−LIO(Me−3,2−HOPO)のうちの1つでの単回の非経口投与または経口投与によって促進されたプルトニウム排出の増強を、処置上の投与時間に応じて若年成体雌スイス・ウェブスターマウスにおいて調査した。第1の研究では、キレート化処置を汚染から1時間後〜7日後の種々の期間でチャレンジ事象後に非経口で1回施行し、処置7日後にマウスを安楽死させた。第2の研究では、汚染の48時間前〜1時間前の種々の期間に非経口または経口のいずれかで単回処置を予防的に行い、汚染事象の3日後にマウスを安楽死させた。両研究では、主要有効性評価項目は、食塩水処置コントロール群と比較した安楽死時点の放射性核種の体内含有量の減少であった。非経口処置症例について、比較はDTPA処置群で引き出すこともできる。両研究のin vivo部分を、インシデントを使用せずに行った。以前に報告された結果から予想されるように、実験的リガンド3,4,3−LI(1,2−HOPO)および5−LIO(Me−3,2−HOPO)の用量レベルでは、いかなる識別可能な有害作用も生じなかった。
汚染から1時間後と7日後との間にマウスを処置し、処置から7日後に安楽死させる研究のために、異なる剖検時間での体内総238Pu含有量および分布の結果を、統計解析に供し、図3に示す。汚染から1時間後〜16時間後の範囲の処置時点で、単回非経口用量の3,4,3−LI(1,2−HOPO)または5−LIO(Me−3,2−HOPO)のいずれかにより、食塩水処置コントロールと比較して、238Puの排出率が有意に増加し、かつ全身負荷量および組織含有量が明確に減少した。しかし、前述の時点にて3,4,3−LI(1,2−HOPO)でキレート化した群のみで238Pu含有量が減少し、DTPA処置後に認められた減少より有意に良好であった。さらに、3,4,3−LI(1,2−HOPO)は、汚染から24時間後〜7日後の遅延した時点で処置を施した場合に238Pu排出が有意に増強される唯一のキレート化の選択肢であった。3,4,3−LI(1,2−HOPO)はまた、全ての試験した処置時点で238Pu骨格含有量を有意に低下させる唯一のリガンドであった。体外排出有効性は、処置の遅延の増加とともに減少した。
汚染の1時間前と48時間前との間に予防的にマウスを処置するこの研究のために、汚染3日後の238Puの体内総含有量および分布の結果を統計解析に供し、図6に示す。図6のパネルAに示すように、5−LIO(Me−3,2−HOPO)またはDTPAの非経口投与は、非常に短い予防時間枠(それぞれ、1時間および6時間)で238Puの身体負荷量および組織負荷量を有意に低下させて有効であった。対照的に、3,4,3−LI(1,2−HOPO)は、汚染48時間前に1回注射した場合でさえ、顕著な予防活性を示した(図6のパネルB)。全ての動物は、3,4,3−LI(1,2−HOPO)で非経口処置した群が、対応する食塩水処置群およびDTPA処置群と比較して、全身、骨格、肝臓、軟部組織、および腎臓内の238Pu含有量を非常に有意に低下させることを示した。予想通り、体外排出有効性は、処置と汚染との間の遅延の増大に伴って減少した。それにもかかわらず、汚染48時間前の3,4,3−LI(1,2−HOPO)注射後に認められた有効性レベル(3日以内の50%までの238Puの排出)は、予防時間枠をさらに拡大することができることも示唆している。経口投与後の3,4,3−LI(1,2−HOPO)および5−LIO(Me−3,2−HOPO)の予防有効性も調べた(図6のパネルC)。6時間の予防時間枠においていずれかのリガンドで処置した群は、食塩水コントロール群と比較して、全身、骨格、および肝臓の238Pu含有量の有意な低下を示した。しかし、3,4,3−LI(1,2−HOPO)の経口生物学的利用能の低さは、より早期の処置に関する体外排出有効性の欠如を反映していた。
GBCAからのGd放出レベルの決定を目的としたヒトおよび動物における最近の研究は、主に、Gd検出に最適な方法としてMR画像化を使用して行われていた。MRIは非侵襲性の技術である一方で、分解能の増強は主にGdをキレート化した場合に達成され、このキレート化によって水素−プロトン緩和時間が短縮され、遊離Gdの検出感度が制限される。より以前の少数の研究では、マウスおよびラットにおける放射性標識GBCAの生体内分布、測定精度の改善、およびGd放出の予想範囲よりも広いことの証明について記載している。しかし、これらの研究は、γカウンティングによって検出される低エネルギーγ放射体である153Gd同位体を使用していた。ここでは、α放出体148Gdを使用する。次いで、この148Gdを、公開されたプロトコールにしたがった液体シンチレーションカウンティング(感度が遥かに高い方法)によって検出する。さらに、放射性トレーサの決定とMR測定との組み合わせにより、本発明者らは確実にin vivoで遊離Gdをキレート化Gdと区別することができる。最後に、高親和性キレーター3,4,3−LI(1,2−HOPO)へのアクセスにより、任意の他の既存のおよび利用可能なリガンドと比類のない体外排出が見込まれる。
Gdを含むMRI造影剤を受ける予定の被験体を同定する。被験体は、重篤な腎臓機能障害を有する。被験体に、有効な予防量の3,4,3−LI(1,2−HOPO)(1マイクロモル/Kgと1000マイクロモル/Kgとの間)を投与し、次いで、ある量のGd系MRI造影剤を受ける。MRIから2日後の被験体におけるGd造影剤の量は、被験体がキレート剤を投与されなかった場合より低い。
Gdを含むMRI造影剤(constrast agent)を受ける予定の被験体を同定する。被験体に、Gd系MRI造影剤を投与する。投与から7日以内に、有効量の3,4,3−LI(1,2−HOPO)を被験体に投与する。その後の数日にわたる被験体内のGd造影剤の量は、被験体がキレート剤を受けなかった場合より低い。
鉛、スズ、カドミウム、スカンジウム、およびイットリウムのうちの1つが含まれる重金属に曝露する可能性がある環境におかれる予定の被験体を同定する。被験体に、有効な予防量の3,4,3−LI(1,2−HOPO)(1マイクロモル/Kgと1000マイクロモル/Kgとの間)を投与する。次いで、被験体は、その後2日以内は、重金属をキレート化して被験体から排泄する能力が増強された状態で重金属への曝露に直面し得る。

過去にGdを使用した1回または複数回のMRIスキャンを受けた被験体を同定する。被験体に、有効量の3,4,3−LI(1,2−HOPO)(1マイクロモル/Kgと1000マイクロモル/Kgとの間)を、1用量または複数用量投与する。その後数日間にわたる被験体内のGdの量は、被験体がキレート剤を受けなかった場合より低い。
本実施例は、種々の分子(3,4,3−LI(1,2−HOPO)およびDTPAが含まれる)の金属除去有効性の評価である。試験した計画(arrangements)(化合物、汚染前または汚染後に、どのくらいの量、どのくらいの期間、化合物が投与されたなど)の概要を、表9.1に示す。

- MRI造影剤を被験体に投与する前または後に1,2−HOPOキレート剤を該被験体に投与するステップを含む方法。
- 前記被験体が、前記MRI造影剤を受ける予定である被験体と確認される、請求項16に記載の方法。
- 前記造影剤がGdを含む、請求項16に記載の方法。
- 造影剤の量が100〜600μmol/kgである、請求項18に記載の方法。
- 前記被験体が前記MRI造影剤を受ける前に、前記1,2−HOPOキレート剤を投与する、請求項16に記載の方法。
- 前記被験体が前記MRI造影剤を受けた後に、前記1,2−HOPOキレート剤を投与する、請求項16に記載の方法。
- 前記1,2−HOPOキレート剤が3,4,3−LI−1,2−HOPOである、請求項16に記載の方法。
- 前記被験体が重篤な腎臓機能障害を有する、請求項16に記載の方法。
