Abstract

本明細書に記載の主題は、腫瘍特異的抗原に結合するとともに改変されたT細胞の活性を活性化する受容体タンパク質を発現する遺伝子改変またはキメラ抗原受容体T細胞(例えば、CAR−T)の肝動脈注入を介して肝臓関連がん及び転移を治療する方法に関する。
一態様では、キメラ抗原T細胞受容体タンパク質(CAR)を発現する免疫応答性細胞を含む組成物を対象の肝動脈に注入することを含む、肝転移を有すると診断された対象における肝転移を治療する方法であって、このキメラT細胞受容体タンパク質は、肝臓中の転移性細胞上に発現される抗原に結合する、方法が提供される。
いくつかの実施形態では、免疫応答性細胞はT細胞であり、この方法は、対象の血清から細胞を採取することを含む。他の実施形態では、最低108、109または1010個の細胞が、対象の血清から採取される。さらに他の実施形態では、この方法は、採取した細胞から末梢血単核細胞(PBMC)を単離し活性化して、自己T細胞の集団を生成することをさらに含む。

いくつかの実施形態では、この方法は、対象の肝臓における腫瘍負荷を減少させる。他の実施形態において、腫瘍負荷の減少は、陽電子放射断層撮影法(PET)、磁気共鳴画像法(MRI)または生検を用いて測定される。さらに他の実施形態では、治療期間の1〜8週間後、または約1、2、3、4、5、6、7または8週間後に腫瘍負荷を測定する。さらに他の実施形態では、CAR−T細胞の用量を注入する前に、腫瘍負荷に関して減少を測定する。

研究は、頑強なT細胞応答を有する肝転移(LM)患者が有意に改善された結果を有することを実証したが、ほとんどのLM患者は、有効な肝内抗腫瘍免疫を達成できない(Katzら,2003,Ann Surg Onc,20:946−955)。特異的な腫瘍抗原を標的とするように設計された高度に特異的な免疫療法産物であるキメラ抗原受容体改変T細胞(CAR−T)は、特に切除不能な腫瘍と診断された患者において、必要な抗腫瘍免疫を提供することを約束している。抗原特異的CAR−T細胞の投与は、白血病の早期臨床試験において有望な結果を示しているが、胃腸がん患者の主要な死因であるCEA発現腺がんLMに対するCAR−T技術の成功した適応は未だに達成されていない。したがって、CAR−T技術がLMの治療のための実行可能な選択肢であることを示すために、本明細書に記載のように、第I相臨床試験を設計し、実施した。さらに、臨床研究は、LMに対するCAR−T治療の有効性を制限しうる全身注入によるCAR−Tの非効率的な肝内送達に対処している。以下に記載する内容は、CAR−T肝動脈注入(HAI)を試験して、CAR−TのLMへの直接的な領域的送達が、LMの治療において安全かつ有効であることを示す研究である。要約すれば、データは、CAR−T HAIが良好に耐容され、腫瘍細胞の死滅の証拠と関連していることを実証し、CAR−TのLMへの肝動脈注入(HAI)がそれを必要とする患者におけるLMを実際に効果的に治療できることを示している。

増殖及び遺伝子改変の前に、肝転移の治療を必要とする対象からT細胞源が得られる。T細胞は、末梢血単核細胞、骨髄、リンパ節組織、臍帯血、胸腺組織、感染部位由来の組織、腹水、胸水、脾臓組織、及び腫瘍を含む多くの供給源から得ることができる。本開示の特定の態様では、当技術分野で利用可能な様々なT細胞株を使用することができる。T細胞は、Ficoll(商標)分離のような、当業者に既知の様々な技術を使用して、対象から採取した血液の単位から得ることができる。好ましい実施形態において、個体の循環血液由来の細胞は、アフェレーシスによって得られる。アフェレーシス産物は、通常、T細胞、単球、顆粒球、B細胞、他の有核白血球が含まれるリンパ球、赤血球、及び血小板を含有する。アフェレーシスによって収集された細胞を洗浄して血漿画分を除去し、その後の処理工程のために細胞を適切な緩衝液または培地に入れることができる。当業者であれば容易に理解されるように、洗浄工程は、製造者の指示に従い半自動フロースルー遠心分離機を用いるなど、当業者に既知の方法によって達成することができる。洗浄後、細胞は、種々の生体適合性緩衝液、例えば、Ca非含有、Mg非含有PBS、または緩衝液を含むまたは含まない他の生理食塩水などに再懸濁され得る。あるいは、アフェレーシス試料の望ましくない成分を除去し、細胞を培地に直接再懸濁することができる。
の後、必要に応じて繰り返される。細胞を精製して末梢血単核細胞(PBMC)、あるいは本明細書ではリンパ球リッチ(PBL)と呼ぶ、を回収する。リンパ球は、50ng/mlのOKT3及び3000IU/mLのIL−2への曝露によって活性化される(Walkerら,1993,4:659−680)。活性化細胞に、10μg/mLプロタミンを用いて、上記のように組換えキメラCARを含むレトロウイルスの高力価上清で形質導入する(Cornettaら、1989,23:187−194)。翌日、この手順を繰り返して形質導入細胞の画分を増加させる。形質導入の2日後に、少量の細胞を導入遺伝子の発現のためのフローサイトメトリーによって分析する。形質導入された細胞の割合が10%未満である場合、細胞はさらに2回の形質導入を受け、形質導入された細胞のパーセンテージについてフローサイトメトリーによって再度分析され得る。適切な数のT細胞が形質導入された(例えば、>10%)場合、活性化条件(上記)下で、細胞は例えば1.2〜1.5×106/mLで培養される。活性化された細胞の残りの部分は、形質導入の結果が分かるまで維持され、必要に応じて第2の試みに使用される。形質導入したT細胞を培養中で増殖させ、増殖/形質導入パラメーター(倍加時間、全細胞数、形質導入%、T細胞活性化指標)を監視する。細胞を凍結保存する前に、10%DMSO、20%ヒト血清及び3000IU/mlのIL−2を添加する。細胞を採取する時期の決定は、採取後の培養で十分な細胞が将来の注入のために増殖するよう、十分な総細胞の存在に基づいている。通常は、T細胞活性化から用量の採取までの間隔は、2〜3週間の範囲である。増殖の間、キメラ受容体を発現するT細胞の存在を記録するためにフローサイトメトリーを実施する。他の試験には、CEA+及びCEA標的に対する生存性、繁殖不能性(sterility)、及び標準的な細胞傷害性アッセイが含まれる。用量を構成するのに十分な形質導入効率及び細胞数を達成するために、患者T細胞で3回まで試みが為される。いくつかの実施形態では、種々の形質導入から増殖した細胞をプールして用量を達成する。
を感染させるために使用する。感染後、PG13細胞をフローサイトメトリーによりCAR導入遺伝子の発現についてアッセイする。次いで、細胞を、FACSによるCAR導入遺伝子の安定発現のために選別して、マスター作業細胞バンクを確立し、安全性、無菌性、同一性及び複製コンピテントレトロウイルス(RCR)の不在について試験する。形質導入した細胞を凍結して、患者の用量を構成する量にする。いくつかの実施形態では、適切な量のCAR−T細胞を、約50〜100mL、75〜100mL、75〜125mL、または約50mL、75mL、100mL、または135mLの等張性及び薬学的に許容される溶液を含む1つのバッグ内の液体窒素気相中に保存する。いくつかの実施形態では、溶液は、約15〜25%、15%、20%または25%のアルブミンを含有する。他の実施形態では、溶液は、約5〜15%、5%、10%または15%ジメチルスルホキシド(DMSO)を含有する。好ましい実施形態では、溶液容量は、約100mLであり、適切な数の細胞、約20%のアルブミン及び約10%のDMSOを含有する。いくつかの実施形態では、投与前に細胞生存率を維持するために、約400,000〜500,000IUまたは約450,000IUのIL−2を凍結前にバッグに添加する。溶液を投与前に解凍する。
本明細書に開示される研究及び結果は、肝動脈注入によるCAR発現免疫応答性細胞の投与が、肝臓に転移したがんを治療する際の治療効果を提供し得ることを示す。本明細書に記載されるように改変されて、肝臓転移、具体的にはCEAを発現する転移性細胞を標的とする細胞の投与は、例えば小分子化学療法剤の注入と比較してかなり複雑なプロセスである。結腸にあるような正常細胞は、CEAを発現する。健常細胞の破壊を最小化または防止するために、改変CAR−T細胞による正常細胞の接触を最小化または排除することが重要である。CAR−T細胞は、様々なサイトカインを分泌することができる免疫細胞であり、有害な副作用に寄与する。以下に、この問題に取り組む方法、具体的には、改変された免疫応答性細胞の健常細胞への曝露を最小限に抑え、改変された細胞の罹患細胞との接触を最適化するための手段を記載する。
タンパク質は、配列番号5に対して少なくとも90%、95%、98%または99%同一であるアミノ酸配列を含むCEAタンパク質に特異的に結合する。
患者は、CAR−T治療期間の間、例えば毎週または2週間ごとにCAR−T注入を受ける。CAR−T治療期間は、2、3、4、5、6 7、8、9、または10週間であってもよく、あるいは2〜10、4〜9、2〜8、2〜6、2〜4、3〜6、4〜8または4〜6週間続いてもよい。CAR−T治療期間の開始(0日目)は、治療される患者(あるいは代わりに、治療されるべき患者ではない対象)から血液細胞が採取される日である。CAR−T細胞の注入を受ける患者にはまた、IL−2も投与されうる。IL−2は、注入後のCAR−T細胞の生存を促進するが、発熱、吐き気、嘔吐、及び/または頻脈などの有害な副作用を引き起こさないかまたは増強しないIL−2の用量を使用することが好ましい。いくつかの実施形態では、IL−2は、CAR−T治療期間の全期間にわたって連続して投与される。このような連続注入は、ポンプ貯蔵器を使用して実施することができ、患者を歩行可能にする中心静脈カテーテルまたは他の方法によって投与することができる。IL−2注入は、CAR−T注入の開始の1、2または3時間前に開始することができ、または最初のCAR−T注入開始時または注入最中に、または、最初のCAR−T注入の完了後、約1、2または3時間以内に投与することができる。
肝転移(LM)を有する8人の患者が最初に登録され、本明細書に記載の抗CEA CAR−T細胞で治療される第1相臨床試験を実施した。6人の患者がプロトコルを完了した。患者をコホート1とコホート2に分けた。0日目にすべての患者から細胞を採取し、抗CEA CAR−T細胞を生成するために使用した(抗CEA scfv−CD8α−CD28/CD3ζCAR−T細胞)。14日目、28日目、42日目に、コホート1の各患者は、14日目に108個の細胞、28日目に109個の細胞、42日目に1010個の細胞の注入を受けた。コホート2については、14日目、28日目、42日目に、各患者は1010個の細胞の注入を受けた。コホート2の患者はまた、14日目に開始し、55日目または56日目に終了するIL−2の75,000IU/kg/日の用量の連続注入を受けた。約56日目に、コホート1及び2における患者のMRI及びPET分析を56日目に行った。プロトコルを完了した6人の患者からのデータは、抗CEA CAR−TのHAIが全身性IL−2注入の有無に十分耐容性であることを示した。IFNγのスパイクは、系IL−2の有無にかかわらず、すべての患者において24時間から48時間後に発生することが確認された(図6参照)。放射線学的な部分的または完全な反応はなかったが、6人の患者のうち1人は安定した疾患を有し、少なくとも24ヶ月の追跡期間にわたって生存していた。

選択的内照射療法(SIRT)は、切除不能ながんと診断された患者に一般に使用される放射線療法の一形態である。SIRTは、その標的臓器、組織または腫瘍の損傷または死をもたらす治療用線量の電離放射線をその標的に効果的に送達するために、器官、組織
または腫瘍のような標的に放射性マイクロスフェアとして投与される。
キメラ抗原受容体T細胞注入の治療有効性は、免疫抑制、例えば腫瘍死滅細胞の抑制ま
たは抗腫瘍サイトカインの発現の減少をもたらす因子によって影響される可能性がある。がんの存在下での腹腔内空間の免疫環境の影響を考慮し、それに応じてキメラ受容体T細胞治療を受ける患者を治療することが重要である。
以下の実施例は本質的に例示的なものであり、限定することを意図するものではない。
実施例1
ヒトCAR−T細胞の生産
以下により詳細に記載するように、6人の患者(本明細書では患者番号1、4、5、6、7及び8と呼ぶ)を、その表面にCEA抗原を発現する転移性細胞を特異的に標的とす
るCAR−T細胞の肝臓注入で治療した。抗CEA scfv−CD8α−CD28/CD3ζ(タンデム)キメラ抗原受容体を、先に記載したようにMFGレトロウイルス骨格にクローニングした(FDA BB IND 10791)(Emtageら,Clin
Canc Res,14:8112−8122、その全体が参照により本明細書に組み込まれる)。簡単に述べると、タンデム分子は、CEAに特異的に結合するモノクローナル抗体のhMN14 sFv(配列番号1)、CD8ヒンジセグメント、CD28細胞外ドメイン、膜貫通ドメイン及び細胞質ドメイン、ならびにζ細胞質ドメインを含む。得られたキメラ構築物をレトロウイルスベクターにクローニングし、制限消化及び配列決定によって確認した。臨床レトロウイルスベクター上清をPG13細胞を用いて産生させて、先に記載したようにテナガザル白血病ウイルスシュードタイプウイルス粒子を生成した(Beaudoinら,2008,J Virol Methods,148:253−259)。すべての臨床バッチは、インディアナ大学のベクター生産施設(Indiana
University vector production facility)(インディアナ州、インディアナポリス)で調製し、使用するまで−80℃で保存した。

よって測定した。ルシフェラーゼ発現CEA+腫瘍細胞を96ウェル丸底プレート中で様々な比率で抗CEA CAR−Tと混合し、各ウェルからの生物発光の消失を測定した(Karimiら,2014,PLoS One,9:e89357)。形質導入T細胞を培養し、IL−2(500IU/mL)の存在下で増殖させ、CAR発現レベルを形質導入の48時間後に調べた。
臨床研究デザイン
第I相臨床試験(NCT01373047、RWH 11−335−99)を実施した。この研究は、従来の全身療法の平均2.5(範囲2〜4)ラインで進行した切除不能CEA+腺がんLMを有する8人の患者を登録した(表2)。

ベースライン時に、右大腿動脈アプローチを介してマッピング血管造影を行った。他の潜在的な肝外灌流源に加えて、胃十二指腸及び右胃動脈は、マイクロコイルで塞栓形成された。CAR−T注入については、同じ動脈アクセス手順を実施し、60ccシリンジを介して2cc/秒未満の速度で、100ccの総容量で細胞を手で注入した。最初の50cc後及びCAR−T注入の完了時に、較正されたコントラスト速度を有する血管造影を実施して、保存された動脈流を確認した。可能であれば、適切な肝動脈に注入液を送達した。右または左の肝動脈のいずれかが固有肝動脈から生じなかった異常な肝動脈解剖所見の場合、葉の体積計算に基づいて用量を分割した。そのような場合、両方の葉への比例したCAR−T送達を確実にするために、分割用量を左右の肝動脈に別々に送達した。
注入後のCAR−T細胞の輸送
第1のCAR−T HAIの前及び最終HAIの時点でLM及び正常肝臓をサンプリングするために、CT誘導経皮的生検を得た。LM生検、正常肝生検、及び末梢血試料中のCAR−T(CAR+/全リンパ球%)の割合をフローサイトメトリーによって決定した。例えば、患者7の試料は、正常肝臓リンパ球の1.8%がCAR−TのHAIに続いてCAR+であり、腫瘍内リンパ球の7.6%がCAR+であることを実証した。注入後のLM生検標本におけるCAR+細胞はCD3+であることが確認された。末梢血、正常肝
臓及びLMのCAR−T集団データをすべての患者について決定した。CAR−Tは、6人中5人の正常な肝臓に比べて、LMにおいて、より豊富であった。患者5では、CAR−Tは、最終CAR−T注入の12週間後にマイクロ波アブレーション処置中に得られた試料中に1.4%のLMリンパ球を含むことが見出された。4人の患者では、CAR−Tは末梢血では検出されなかったが、最終注入時に患者7及び患者8に一時的に存在し、3日後に検出レベルを下回った。最終的な注入後2日目に採取した末梢血サンプルについて定量PCRを実施し、患者7のみがベースラインに対してCAR DNAの測定可能な増加(1.1倍)を示した。抗CAR抗体は、CAR−T注入後の患者血清では検出されなかった。
治療活性
最後のフォローアップでは、治験を完了した重度な前治療した6人の患者のうち5人が、疾患の進行のために死亡した(表3)。
IL−2投与による血清IFNγ濃度及びCEA応答の相関
血清IFNγレベルは、複数の時点でELISAによって測定した。IFNγのスパイクは、全身のIL−2を伴わず、または全身のIL−2を伴い、24時間〜48時間後にすべての患者に起こることが認められた(図6;点線の垂直線はCAR−T注入時点を示し、第1のデータ点はCAR−T注入前のベースライン値を示す)。血清CEA変化を各
患者のIFNγのピーク変化と比較した(図7、上部)。ピークIFNγレベルとCEA変化との間の逆相関は有意であった(R=−0.94、p=0.02)。すべての患者のHAI CAR−T用量は、IL−2(600,000IU)の量を含んでいた。連続的な全身IL−2曝露及び最大のCAR−T用量を有する3人の患者(患者番号6,7及び8)は、最良のCEA応答及び最も高い平均IFNγレベルを有した(P=0.03、図7、下部)。
安全データ
試験を完了したすべての患者において、いずれかの原因に起因するいずれかの等級の有害事象(AE)が観察された(表4)。コホート1における用量は、1010個の細胞で計画された最大のHAI CAR−T注入レベルに達した。コホート1ではCAR−Tの用量の減少は必要ではなく、したがって、コホート2の全患者は、IL−2の補助を受けて1010レベルで3回の投与を受けた。等級4または5の有害事象はなかった。4人の患者で発熱性AEが観察された。患者7は、40℃(104°F)の温度ピークを有する等級3の発熱及び頻脈を経験した。彼女の全身IL−2注入の50%の投与量を減少させた後、患者7で発熱と頻拍が消散した。注目すべきことに、患者7はまた、20%のピーク及び絶対数3,740/mlで末梢好酸球数の増加を経験した。IL−2注入とレフラー(Loeffler)症候群の他の特徴を伴う心筋血栓症との間の報告された関連性を考慮して(Junghansら,2001,New Eng J Med,344:859−860)、我々は正常な心エコー図と心電図を得た。彼女の好酸球数は、特別な介入なしに正常限界に戻った。
安全データ
患者にHAIを介して1010個の抗CEA scfv−CD8α−CD28/CD3ζCAR−T細胞を投与する、フェーズI臨床試験を(上記の実施例に記載されているように)実施する。T細胞を0日目に各患者から単離し、トランスフェクトし、抗CEA scfv−CD8α−CD28/CD3ζ構築物を発現するように選択する。その後、各患者に14日目、21日目及び28日目に1010個の細胞を投与する。44日目(最終細胞注入後の2週間の中断後)に、SIR−Spheres(登録商標)含有用量を各患者に注入する。
して腫瘍体積の大きさに応じて変化する所定量のSIR−Spheres(登録商標)を受ける。総肝臓容積の25%未満、25%〜50%または50%より大きい腫瘍を有する患者には、2ギガベクレル(GBq)、2.5GBq、または3GBqの90Y活性に相当するSIR−Spheres(登録商標)が与えられる。肺−肝臓の呼吸スキャン(lung−liver breaththrough scan)により、マイクロスフェアの10%超が肝臓を通過して肺に留まることが示された患者に対して提供される。投与される90Y活性の量は、1%ごとに2%減少し、肺−肝臓のブレークスルー率は10%より大きい。患者は、SIR−Spheres(登録商標)治療後の同日または翌日のいずれかに退院する。
Claims (20)
- 対象の肝転移を治療する方法であって、
キメラ抗原受容体タンパク質を発現するキメラ抗原受容体T細胞(CAR−T細胞)を含む組成物を前記対象の肝動脈に注入することを含み、前記キメラ抗原受容体タンパク質は、肝臓中の転移性細胞上に発現する抗原に結合する、方法。 - 前記注入ステップの前に、肝動脈及び近くの血管をマッピングするために血管造影を実施することをさらに含む、請求項1に記載の方法。
- 前記肝臓中に流れ込まない血管を閉塞させることをさらに含む、請求項2に記載の方法。
- 前記注入ステップ中に血管造影を実施することをさらに含み、前記血管造影は、前記注入ステップ中に肝内血行動態健常性(intrahepatic hemodynamic integrity)を監視する、請求項1〜3のいずれか1項に記載の方法。
- 前記キメラ抗原受容体タンパク質が、前記転移性細胞の表面上に発現されるがん胎児性抗原(CEA)に特異的に結合する、請求項1〜4のいずれか1項に記載の方法。
- 前記キメラ抗原受容体タンパク質が配列番号1を含む、請求項1〜4のいずれか1項に記載の方法。
- 前記対象の肝動脈内に第2の治療剤を投与することをさらに含む、請求項1〜4のいずれか1項に記載の方法。
- 前記第2の治療剤がIL−2である、請求項7に記載の方法。
- 前記第2の治療剤の前記投与が、前記免疫応答性細胞を含む前記組成物の前記注入の前、前記注入の間、または前記注入の後に実施される、請求項8に記載の方法。
- 前記CAR−T細胞を含む前記組成物が、1週間に1回、2週間に1回、3週間に1回、または4週間に1回、前記対象の肝動脈に注入される、請求項1〜4のいずれか1項に記載の方法。
- 前記CAR−T細胞を含む前記組成物の前記対象の肝動脈への前記注入が、108〜1010個のCAR−T細胞を肝動脈に注入することを含む、請求項1〜4のいずれか1項に記載の方法。
- 前記注入前記組成物が、前記注入前の血清中のCEAのレベルと比較して、血清CEAの40%〜50%の減少をもたらす、請求項1〜4のいずれか1項に記載の方法。
- 対象における肝転移を治療する方法であって、
前記対象の肝動脈に、キメラ抗原受容体タンパク質(CAR)を発現するCAR−T細胞を含む組成物を注入することを含み、前記CARタンパク質はCEAに結合する、方法。 - 前記キメラ抗原受容体タンパク質が配列番号1を含む、請求項13に記載の方法。
