Abstract
Description
本発明は、全体として、レトロウイルスに感染した対象の治療に関し、詳細には、HIV感染者の組換えガレクチンタンパク質を用いた治療に関する。
世界的には3500万人が、米国内では120万人近くが、AIDSの原因因子となるHIV−1に感染している。医療資源が豊富な、HIV感染者が治療を受けられる国では、抗レトロウイルス剤療法(ART)の登場により、罹患率と死亡率は大幅に低下している。とはいえ、強力な治療を何年も続けても現状の治療戦略では感染根絶には至らず、生涯にわたりARTを続ける必要がある。さらに、蓄積されたデータからは、HIV感染により「非AIDS」の心血管、腎臓、肝臓などの疾患が増幅されること、及びウイルスが抑制されている人でも免疫系に早期老化が見られ得ることが示唆されている。また、医療資源が限られた状況でもARTが提供できるよう大きな進歩はあったが、全世界的に生涯治療を実現するには大きな経済、政治、運営上の課題がある。こういった実情からも、感染者のHIV−1の根絶に向けた戦略の開発は重大事となっている。
本発明の組成物及び方法を説明する前に、本発明はここで説明する特定の組成物、方法、実験条件などに限定されないことを理解されたい。そのような組成物、方法、条件は、変化し得るからである。また、本明細書で使用する用語は、特定の実施形態を説明する目的しかなく、限定的なものではないことも理解されたい。本発明の範囲は添付の特許請求の範囲でのみ限定される。
ガレクチン−9を用いたHIV−1潜伏感染状態の強力な逆転
rGal−9は潜伏HIVの再活性化に有意な効果がある、という仮説を立てた。この仮説をめぐり、既に確立されているHIV潜伏「J−Lat」モデルで実験を行った。J−Lat細胞は、再活性化の指標である緑色蛍光タンパク質(GFP)をコードする潜伏中の転写可能なHIVプロウイルスを担持する。J−Lat5A8細胞(抗CD3/CD28刺激に感応するJ−Latバリアント)を組換えGal−9で刺激し時間を経過させる用量反応実験を行った。このデータから、rGal−9は、潜伏感染J−Lat細胞を用量反応的に再活性化し、その有効性はCD3/CD28単独刺激の場合と同等かそれ以上であることが明らかになった(図2A)。rGal−9とCD3/CD28の同時刺激は相乗的で、(GFP発現で表された)J−Lat細胞のHIV RNA発現率は、CD3/CD28単独刺激の場合のほぼ3倍増という結果であった(図2B)。J−Lat潜伏モデルにおける組換えガレクチン−9によるHIV発現のインビトロ再活性化。J−Lat細胞をフローサイトメトリー分析して、組換えガレクチン−9(rGal−9)による再活性化後の、HIVがコードするGFP発現の平均蛍光強度を評価した。(A)刺激から2、6、12、24時間後のrGal−9によるHIV再活性化の用量反応。ポジティブコントロールとしてビーズに結合させた抗CD3/CD28抗体を用いた。(B)100nMのrGal−9とaCD3/aCD28を組み合わせた刺激から24時間後の効果。結果は代表的な三連実験の平均値±標準偏差である。
宿主トランスクリプトームに及ぼすrGal−9治療の影響
rGal−9が潜伏HIVの再活性化に及ぼす影響の評価に加えて、rGal−9がヒトトランスクリプトームに及ぼす影響も決定しようと、具体的な焦点を(1)HIV転写及び潜伏に関与する遺伝子の発現、ならびに(2)HIVの制御とHIV感染細胞のクリアランスとに関与する免疫遺伝子、とした。2種のアプローチを用いた。「CuRe」(累積的制限)定量PCRアレイにより内在性免疫遺伝子発現に特化して測定し、遺伝子発現に対する全体的効果をRNA配列決定により総合評価した(図3)。ワークフローの一部として、GFP陽性細胞とGFP陰性細胞は、それぞれ再活性化(転写活性)HIVプロウイルスと潜伏(転写不活性)プロウイルスを含有した。
rGal−9治療はAPOBEC3シチジンデアミナーゼを含む抗HIV細胞内在性免疫を誘導する
rGal−9が抗HIV細胞内在性免疫防御をいかに調節するかを決定するために、遺伝子発現データを調べた(図4)。複数の論文に記載されている確立された42の抗HIV制限因子に及ぼす効果を調査した(Abdel−Mohsen et al.(2014)J Virol 88:763及びAbdel−Mohsen(2013)Retrovirology 10:106)。宿主ゲノムへの組込み前にHIVを阻害するいくつかの因子がかなり誘導されたが、HIV組込み後の抑制因子は誘導されなかった。これは、ショック・アンド・キルでHIVを根絶する枠組みで使用される剤としては理想的なシナリオである。特に関連深いものとしては、APOBEC3B、APOBEC3F、APOBEC3G、及びAPOBEC3Hを含むAPOBEC3シチジンデアミナーゼがCD4+T細胞においてrGal−9によって強力に誘導されたということが判明した。このことは、こういったAPOBEC3因子関連の既知の作用機構に基づき、rGal−9により再活性化したあらゆるウイルスは新細胞の感染時に極度に超変異して、生産されるウイルスすべてが複製不全となることを示唆している。このことは、ショック・アンド・キルのHIV治療の枠組みでは極めて適切である。というのは、特に薬物浸透性が最適とまでいかない組織では、ARTはウイルス複製を完全には阻止しないことが次第に明らかになりつつあるからである。ヒトシチジンデアミナーゼAPOBEC3G及びAPOBEC3Fは、複製中にレトロウイルス及びヘパドナウイルスのマイナス鎖DNAにおいてCからUへの変化を導入することにより、自然抗ウイルス防御機構として働く(結果として、ゲノムセンス鎖配列にGからAへの変異が生じる)。しかしHIV−1ゲノムは、宿主細胞におけるAPOBEC3のタンパク質分解を促進することによりこの防御に対し特異的に反作用する23キロダルトンのタンパク質Vif(ビリオン感染性因子)をコードする。Vif発現の非存在下では、APOBEC3はビリオンに組み込まれ、ウイルスゲノムはコード鎖における大がかりなGからAへの変異、つまり「超変異」を経て、一般的には1回の複製サイクル内で生存不能になる。観察された、rGal−9に反応したAPOBEC3G誘導レベル(最高55倍)(図4、図5)から、ウイルスVifタンパク質の拮抗効果は中和され、再活性化したウイルスは高濃度のAPOBEC3Gを内包し、子孫のビリオンはすべて複製不全となるのでは、と予測された。このシナリオは、本発明者らのグループなどがこれまでに発表し提唱している、APOBEC3Gを超生理的レベルにまで(たとえ2倍か3倍であっても)誘導するとHIV−1及び他のレトロウイルスをインビボで強力に中和できる、という知見と同じである。図4は、rGal−9が抗HIV細胞内在性免疫に及ぼす影響を説明するヒートマップを示す。表示のデータはカスタム化した定量PCRアレイを用いて収集した。観察結果は本発明者らのRNA配列決定データでミラーリングして、観察したパターンの妥当性を確認した。ベースライン(処理前)状態に対する倍率変化を示す。図5は、HIV潜伏感染細胞におけるrGal−9によるAPOBEC3G宿主シチジンデアミナーゼの誘導を示す。再活性化(転写活性)HIVプロウイルスと潜伏(転写不活性)プロウイルスを含むGFP陽性細胞とGFP陰性細胞を示す。平均値±標準誤差を示す。
rGal−9治療はNFkB経路コンポーネントを誘導する
活性化B細胞(NFkB)経路の核内因子カッパ軽鎖エンハンサーは、HIV−1転写にとって重要であることがわかっている。NFkB経路は、HIV転写開始のシグナル依存性活性化にとって重要であり、ヒストンアセチルトランスフェラーゼ(HAT)をウイルスの末端反復配列(LTR)プロモーターに動員できる。したがって、リアル(real)−9治療はNFkB経路の複数のコンポーネントを誘導して、HIV−1転写に及ぼすその強力な補強効果に貢献するのでは、と仮定した。予想どおり、本発明者らのRNA−seqデータ分析は、いくつかのNFkB経路遺伝子がNFkB治療により誘導されることを明白に実証している(図6)。データはRNA−seqを用いて作成した。平均値±標準偏差を棒グラフに示す。アステリスクはt検定に基づく統計学的有意差を示し、偽発見率の補正:*=FDR<0.05、**=FDR<0.01、***=FDR<0.001である。図7に示すように、rGal−9は選択的にヒストンデアセチラーゼ(HDAC)を阻害する。データはRNA−seqを用いて作成した。平均値±標準偏差を棒グラフに示す。アステリスクはt検定に基づく統計学的有意差を示し、偽発見率の補正:*=FDR<0.05である。
rGal−9は採取したばかりの初代細胞の生存度には最小限の影響しか及ぼさない
rGal−9が潜伏HIVの再活性化に及ぼす影響を評価することに加えて、リアル−9の毒性を評価し、rGal−9が初代(primary)細胞の生存度に及ぼす影響を決定することにした。潜伏HIVを強力に再活性化させるrGal−9の能力にもかかわらず、細胞生存度に及ぼす大きな無差別的影響によりインビボでの利用可能性が損なわれることにもなる。複数の健常コントロールから新鮮血100ccを得た。これはBlood Systems Research Instituteの健常ドナープログラムでHIVに感染していないドナーから採取したものである。この実験は、rGal−9が初代末梢血単核球(PBMC)及び精製CD4+T細胞の生存度に及ぼす影響が最小限であることを明白に実証しており、rGal−9のヒトへの適用の可能性がさらに高まった(図8)。大腸菌(Escherichia coli)(Epi300、Epicentre)をルリア培地またはクロラムフェニコール(20mg L−1)もしくはカナマイシン(50mg L−1)もしくはアンピシリン(50mg L−1)もしくはテトラサイクリン(10mg L−1)もしくはゲンタマイシン(20mg L−1)もしくは必要に応じてそれらの組合せを補充した寒天培地で増殖させた。図8に示すように、単離したばかりの初代CD4+T細胞は、少なくとも200nMの濃度までのrGAL−9治療に耐容性を示した。図8Aは、0.5%DMSOかまたは段階的に濃度を上げたガレクチン−9のいずれかで24時間処理した、3名の健常なドナー由来の生CD4+T細胞の割合を示す。図8Bは、フローサイトメトリーのデータからの代表的なセットを示す。ドットプロットは、FSC−SSCプロットである。FSCとSSCは、それぞれ前方散乱と側方散乱を示す。
rGal−9による潜伏HIVの強力なエクスビボ再活性化
ART抑制下のHIV感染者3名からCD4+T細胞を単離した。細胞は、DMSO0.5%(ネガティブコントロール)、aCD3/aCD28ビーズ(細胞とビーズの比率は1:1)、PMA(5nM)/イオノマイシン(1pM)、SAHA(1pM)、または濃度が異なるrGal−9(200nM、500nM、1pM、及び1.65pM)で24時間処理をしたかまたは処理をしなかった。細胞はまた、ロミデプシン(40nM)で4時間パルス処理し、2回洗って、(Gilead社が推奨するプロトコールにしたがい)5日間培養した。細胞関連全RNAを、Qiagen Allprep RNA/DNA/タンパク質キットを用いて、カラムでDNase処理し、抽出した。HIV細胞のRNAレベルを、定量PCR(qPCR)により保存性の高いLTR領域を対象に三連で実施して定量化した。細胞関連HIV RNAの倍率増加は、各時点において対応するDMSO処理コントロールに対し相対的に決定した。各ドナー及び条件の倍率変化は、2つの独立した測定による平均HIVコピー数に基づく。ボックスは範囲を表し、線は中央値を表す。ART抑制下のHIV感染者3名からCD4+T細胞を単離した。細胞の全RNAを、Qiagen Allprep RNA/DNA/タンパク質キットを用いて、カラムでDNase処理し、抽出した。HIV細胞関連RNAレベルを、定量PCR(qPCR)により保存性の高いLTR領域を対象に三連で実施して定量化した。細胞のHIV RNAの倍率増加は、各時点において対応するDMSO処理コントロールに対し相対的に決定した。各ドナー及び条件の倍率変化は、2つの独立した測定による平均HIVコピー数に基づく。ボックスは範囲を表し、線は中央値を表す。
HIV潜伏感染状態を逆転させるための細胞表面グリカンの操作
組換えガレクチン−9(rGal−9)が媒介するHIV潜伏感染状態の逆転に関する作用機構を特定するために実験を行った。感染細胞の表面から選択的にグリカンを除去することにより、本発明者らは、rGal−9がHIV潜伏感染状態を逆転させる能力には特定の細胞表面グリカンの存在(既に認識されているガレクチン−9タンパク質リガンドではなく)が不可欠であることを判明した(図10を参照のこと)。この結果は、グリカンがrGal−9のシグナル伝達において重要な役割を果たしていることを示す。図10A〜図10Dは、rGal−9がグリカンに依存してHIV転写及び再活性化を誘導することを示すグラフ及びフローサイトメトリー分析図を示している。図10Aは、抗Tim−3抗体、抗CD44抗体、または抗PDI抗体の投与が、J−Lat5A8細胞におけるrGal−9が媒介するHIVの再活性化に及ぼす効果である。抗体は、200nMのrGal−9を投与する30分前に加えた。α−ラクトースをポジティブコントロールとして用いた。図10B及び図10Cは、J−Lat5A8細胞を1μg/mlのツニカマイシンまたは酵素脱グリコシル化ミックスのいずれかでrGal−9刺激前に24時間かけて処理している。J−Lat細胞をフローサイトメトリーで分析して、HIVがコードするGFP発現を評価した。統計学的比較は、両側マン・ホイットニー検定を用いた。図10Dは、脱グリコシル酵素の組合せが、rGal−9が媒介するJ−Lat5A8細胞におけるHIV潜伏感染状態逆転に及ぼす影響である。N=PNGaseF(エリザベスキンギア・ミリコラ);O=O−グリコシダーゼ(肺炎連鎖球菌からの組換え);S=α−(2→3,6,8,9)−ノイラミニダーゼ(アルスロバクター・ウレアファシエンスからの組換え);B=β(1→4)−ガラクトシダーゼ(肺炎連鎖球菌からの組換え)+β−N−アセチルグルコサミニダーゼ(肺炎連鎖球菌からの組換え)。平均値±標準誤差を示す。
ブロモドメイン阻害物質を含む他の潜伏感染克服剤と相乗的に組み合わせたガレクチンの使用
この実施例では、HIV感染の治療または治癒を達成するために他の潜伏感染克服剤と相乗的に組み合わせたrGal−9の使用を示す。この実施例は、この概念を支持するデータを提供する。たとえば、rGal−9をブロモドメイン阻害物質である潜伏感染克服剤「JQ1」と同時投与すると、相乗的活性が見られる(図11を参照のこと)。これらのデータは、rGal−9が他のクラスのHIV潜伏感染克服剤と相乗的活性を示すであろうこと、及びHIV感染の治療のために複数の薬物カクテルに組み込むのに好適であろうことを例示している。
Claims (30)
- 細胞におけるヒト免疫不全ウイルス(HIV)潜伏感染状態を逆転させる方法であって、
それを必要とする対象にガレクチンタンパク質を投与し、これにより前記HIV潜伏感染状態を逆転させることを含む、前記方法。 - 前記ガレクチンタンパク質が、ガレクチン−1、ガレクチン−2、ガレクチン−3、ガレクチン−4、ガレクチン−5、ガレクチン−6、ガレクチン−7、ガレクチン−8、ガレクチン−9、ガレクチン−10、ガレクチン−11、ガレクチン−12、ガレクチン−13、ガレクチン−14、及びガレクチン−15からなる群より選択される、請求項1に記載の方法。
- 前記ガレクチンタンパク質が、ガレクチン−9または組換えガレクチン−9(rGal−9)である、請求項2に記載の方法。
- 前記ガレクチンタンパク質の投与後、前記HIVが超変異する、請求項1に記載の方法。
- 超変異した前記HIVが複製不全である、請求項4に記載の方法。
- 少なくとも1種のAPOBEC3シチジンデアミナーゼの発現及び少なくとも1種のNFκB経路コンポーネントの発現が誘導される、請求項1に記載の方法。
