Claims (40)
- 免疫障害に罹患した対象を処置するための、インターロイキン2(IL−2)を含む組成物であって、前記IL−2が、
a)前記対象の血漿IL−2レベルを上昇させ、前記対象における、調節性Tリンパ球(Treg)の、従来型Tリンパ球(Tcon)に対する比(Treg:Tcon)を増大させる用量で、前記対象に、IL−2が持続的に投与されることを特徴とする誘導レジメンと;
b)その後、前記誘導レジメンの用量より高量であり、i)前記対象の血漿IL−2レベルを、さらに上昇させ、ii)Tregの、Tconに対する前記比を、さらに増大させる、IL−2の維持用量が、前記対象に持続的に投与されることを特徴とする少なくとも1つの維持レジメンと
を含む複数の可変IL−2用量として投与されることを特徴とする、組成物。 - 前記維持レジメンが、前記対象の血漿IL−2レベルを、前記誘導レジメンにより誘導されたピーク血漿IL−2レベルを超えて上昇させる、請求項1に記載の組成物。
- 前記血漿IL−2レベルが、IL−2の投与後1回または複数回、IL−2のタンパク質レベル、IL−2のタンパク質活性、IL−2の核酸レベル、Tregの増殖、Tregの活性、Tregのリン酸化STAT5レベル、TregのFOXP3レベル、およびTregのアポトーシスを解析することにより決定されることを特徴とする、請求項1または2に記載の組成物。

- 前記患者が、臨床的利益を享受し続ける限りにおいて、前記維持レジメンの前記持続的投与が、無期限の投与を含む、請求項1から11のいずれか一項に記載の組成物。
- 前記維持レジメンの前記持続的投与が、連続する少なくとも1〜42日間中に、1日1回の投与を含む、請求項12に記載の組成物。
- 前記IL−2が、薬学的に許容される製剤として投与されることを特徴とする、請求項1から13のいずれか一項に記載の組成物。
- 前記IL−2が、皮下、静脈内、腹腔内、および筋内からなる群より選択される投与経路により投与されることを特徴とする、請求項1から14のいずれか一項に記載の組成物。
- 前記IL−2が、皮下投与されることを特徴とする、請求項15に記載の組成物。
- 前記免疫障害が、移植片対宿主病(GVHD)、固形臓器の移植拒絶、血管炎、全身性エリテマトーデス(SLE)、1型糖尿病(T1D)、多発性硬化症(MS)、乾癬、関節リウマチ(RA)、炎症性腸疾患(IBD)、およびアレルギー性喘息からなる群より選択される、請求項1から16のいずれか一項に記載の組成物。
- 前記免疫障害が、cGVHDである、請求項17に記載の組成物。
- 前記対象の、全身ステロイドに対する応答が、不十分であった、請求項1から18のいずれか一項に記載の組成物。
- 前記対象が、ステロイドを含む、少なくとも2回の以前の全身療法にも拘らず、遷延性または再発性の慢性GVHDを有する、請求項1から19のいずれか一項に記載の組成物。
- 前記対象が、IL−2投与の前に、体外フォトフェレーシス(ECP)を受けていた、請求項1から20のいずれか一項に記載の組成物。
- 前記誘導レジメンおよび/または前記維持レジメンが、前記免疫障害を処置する、1または複数のさらなる治療をさらに含む、請求項1から21のいずれか一項に記載の組成物。
- 前記1または複数のさらなる治療が、ECPおよびTregからなる群より選択される、請求項22に記載の組成物。
- 前記Tregが、Treg以外のT細胞を含むTreg組成物として投与されることを特徴とする、請求項23に記載の組成物。
- 前記Treg組成物が、少なくとも1:2のTreg:Tcon比を有する、請求項24に記載の組成物。
- 前記Treg組成物が、T細胞を含む生体材料に対する、CD8+およびCD19+の共枯渇ならびにCD25+の陽性選択から得られたものである、請求項23から25のいずれか一項に記載の組成物。
- 前記Tregが、体重約1kg当たりの細胞0.1×106個〜体重1kg当たりの細胞1.0×106個の間で投与されることを特徴とする、請求項23から26のいずれか一項に記載の組成物。
- 前記対象自身のTregが前記対象に投与されることを特徴とする、請求項23から27のいずれか一項に記載の組成物。
- 造血幹細胞移植片が得られたのと同じ造血幹細胞ドナーに由来するTregが使用されることを特徴とする、請求項23から27のいずれか一項に記載の組成物。
- 前記Treg組成物が、70%超の総細胞生存率、陰性グラム染色、90%以上のCD4+ CD25+細胞、かつ/または50%以上のFoxP3+細胞を有する、請求項23から29のいずれか一項に記載の組成物。
- 前記Tregが、注入として投与されることを特徴とする、請求項23から30のいずれか一項に記載の組成物。
- 前記Tregが、IL−2の投与前に、IL−2の投与と共時的に、またはIL−2の投与後に投与されることを特徴とする、請求項23から31のいずれか一項に記載の組成物。
- 前記Tregが、IL−2の投与前に投与されることを特徴とする、請求項32に記載の組成物。
- 前記対象が、哺乳動物である、請求項1から33のいずれか一項に記載の組成物。
- 前記哺乳動物が、免疫障害の動物モデルである、請求項34に記載の組成物。
- 前記哺乳動物が、ヒトである、請求項34に記載の組成物。
