Abstract
Description
多発性硬化症(MS)は、世界中で百万を超える人々が冒されている神経学的疾患である。それは、若年および中年成人における神経学的障害の最も多い原因であり、対象およびその家族、友人ならびに健康管理を担う団体に与える身体的、心理的、社会的および家計上の影響が大きい。
現在、再発性MS(RMS)における使用が承認された疾患修飾薬がいくつか存在する。これらには、非経口薬のインターフェロンベータ1−a(例えば、Avonex(登録商標)、Rebif(登録商標)、Plegridy(登録商標))、インターフェロンベータ1−b(Betaseron(登録商標)、Extavia(登録商標))、グラチラマー酢酸塩(Copaxone(登録商標))、ミトキサントロン(Novantrone(登録商標))、ナタリズマブ(Tysabri(登録商標))およびアレムツズマブ(Lemtrada(登録商標));ならびに経口薬のフィンゴリモド(Gilenya(登録商標))、テリフルノミド(Aubagio(登録商標))およびフマル酸ジメチル(Tecfidera(登録商標))が含まれる。これらの薬物の大半は、免疫調節薬として働くと考えられている。ミトキサントロンおよびナタリズマブは、免疫抑制薬として働くと考えられている。しかし、それぞれの作用機序は部分的にしか明らかにされていない。
シポニモド
シポニモドは、スフィンゴシン1−リン酸(S1P)受容体モジュレーターと呼ばれる薬物群に属する。前記群の別のメンバーは、RRMSを治療するために市販されているフィンゴリモド(Gilenya(登録商標))である。
ラキニモドは、RRMSの治療用の経口製剤として提案されている。それは2つの第三相試験(すなわち、BRAVO試験およびALLEGRO試験)で試験され、RRMSを治療する際に0.6mgの用量で1日1回、24か月にわたって経口投与すると、有効性を示した。
MSなどの所与の病態を治療するために2種の薬物を投与すると、潜在的な問題がいくつか生じる。両薬物間の生体内での相互作用は複雑である。いかなる単一の薬物の作用も、その吸収、分布、および排泄と関係付けられる。2種の薬物が体内に導入されると、薬物はそれぞれ、他方の吸収、分布、および排泄に影響を及ぼし、したがって他方の作用を変化させることができる。例えば、一方の薬物は、他方の薬物の消失の代謝経路に関与する酵素の産生を抑制し、活性化し、または誘導することがある。
(i)前記対象における前記二次性進行型多発性硬化症を診断するステップと、
(ii)シポニモドとラキニモドを含む治療有効量の組み合わせを前記対象に投与するステップと
を含む方法に関する。
ならびにその薬学的に許容される塩、多形体、溶媒和物および/または水和物を含むと理解される。本明細書ではシポニモドは、IUPAC名1−{4−[1−((E)−4−シクロヘキシル−3−トリフルオロメチル−ベンジルオキシイミノ)−エチル]−2−エチル−ベンジル}−アゼチジン−3−カルボン酸(BAF312)である。本発明の好ましい実施形態において、シポニモドはシポニモド遊離塩基またはシポニモド塩の形で存在する。シポニモドの薬学的に許容される塩の例としては、塩酸塩、臭化水素酸塩および硫酸塩などの無機酸塩、酢酸塩、フマル酸塩、ヘミフマル酸塩、マレイン酸塩、安息香酸塩、クエン酸塩、リンゴ酸塩、メタンスルホン酸塩およびベンゼンスルホン酸塩などの有機酸塩、または適切な場合、ナトリウム、カリウム、カルシウムおよびアルミニウムなどの金属の塩、トリエチルアミンなどのアミンの塩、ならびにリシンなどの二塩基性アミノ酸の塩が挙げられる。好ましい実施形態において、シポニモドはシポニモドヘミフマル酸塩である。
ならびにその薬学的に許容される塩、多形体、溶媒和物および/または水和物を含むと理解される。本明細書ではラキニモドは、IUPAC名5−クロロ−N−エチル−4−ヒドロキシ−1−メチル−2−オキソ−N−フェニル−1,2−ジヒドロキノリン−3−カルボキサミドである。本発明の好ましい実施形態において、ラキニモドはラキニモド遊離酸またはラキニモド塩の形で存在する。ラキニモドの薬学的に許容される塩の例としては、ナトリウム、カリウム、カルシウムおよびアルミニウムなどの金属の塩が挙げられる。一実施形態において、ラキニモドはラキニモドナトリウムである。
本発明は、同時、異時または逐次使用のための配合製剤または医薬組成物などのシポニモドとラキニモドを含む組み合わせに関する。
本発明はまた、シポニモドおよびラキニモドと、少なくとも1種の薬学的に許容される担体とを含む医薬組成物にも関する。
驚くべきことに、シポニモドとラキニモドを含む組み合わせは、治療用のEAEモデルにおいて、すなわちMOG/CFA/百日咳毒素の注射によってEAEが誘導され、臨床症状が明らかになった後のC57/Bl6マウスへの投与によって、疾病負荷を改善し、相乗的にリンパ球の数を減少させるのに有効であることが明らかになった。このモデルは、損傷を受けたCNS(例えば、疾患の発生後/再発が起こった後)に対するMS治療のためのモデルであるので、このモデルは特に後期RRMS患者またはSPMS患者に関連があり、臨床的に正常な患者における再発予防には関連がない。したがって、シポニモドを単独で使用した療法に比べて、シポニモドとラキニモドを含む組み合わせは、(i)炎症性事象、例えばSPMS患者のCNSにおける髄膜炎を和らげることができ、かつ/または(ii)SPMS患者における再発の回数および/もしくは重症度を有効に低減する可能性がある。
動物モデルからのデータ
実施例1.1:マウスにおける脳脊髄液(CSF)中のシポニモドのレベル
雌性C57Bl/6マウスを1日3mg/kgのシポニモドを8日間経口投与して治療した。最後に投与してから8時間後、動物を屠殺し、血液、脳およびCSF中のシポニモドのレベルを測定した。
雌性C57Bl/6マウスをCFA中200μgの組換えMOG1−125で皮下免疫し、続いて100ngの百日咳毒素を1回腹腔内注射した。2日後、さらに100ngの百日咳毒素を腹腔内注射により投与した。第1の臨床症状が出て3日後、マウスを媒体、0.3mg/kgのシポニモド、10mg/kgのラキニモドまたは0.3mg/kgのシポニモド+10mg/kgのラキニモドにランダム化し、前記の1日用量で29日間治療した。C57/Bl6マウスにおけるMOG/CFA/百日咳毒素によって誘導された実験的自己免疫性脳脊髄炎モデルがQuancard et al(Chem Biol. 19(9):1142-51, 2012)に記載されている。
雌性C57Bl/6マウスをCFA中200μgの組換えMOG1−125で皮下免疫し、続いて100ngの百日咳毒素を1回腹腔内注射した。2日後、さらに100ngの百日咳毒素を腹腔内注射により投与した。第1の臨床症状が出て3日後、マウスを媒体、0.3mg/kgのシポニモド、10mg/kgのラキニモドまたは0.3mg/kgのシポニモド+10mg/kgのラキニモドにランダム化し、前記の1日用量で30日間治療した。重度のCNS神経学的疾患および組織損傷が起こった後に治療を開始した。さらに詳細には、その後、後肢麻痺によって特徴づけることができる重度CNS損傷が既に起きていた。
雌性C57Bl/6マウスをCFA中200μgの組換えMOG1−125で皮下免疫し、続いて100ngの百日咳毒素を1回腹腔内注射した。2日後、さらに100ngの百日咳毒素を腹腔内注射により投与した。マウスに、媒体、0.3mg/kgのシポニモド、10mg/kgのラキニモドまたは0.3mg/kgのシポニモド+10mg/kgのラキニモドを最長21日間投与した。免疫細胞がCNSに入り、その結果麻痺が生じた後に治療を開始し、組織損傷を限定した。
雌性C57Bl/6マウスをCFA中200μgの組換えMOG1−125で皮下免疫し、続いて100ngの百日咳毒素を1回腹腔内注射した。2日後、さらに100ngの百日咳毒素を腹腔内注射により投与した。免疫細胞がCNSに浸潤して、組織損傷を開始した後、マウスを媒体、0.3mg/kgのシポニモド、10mg/kgのラキニモドまたは0.3mg/kgのシポニモド+10mg/kgのラキニモドにランダム化し、前記の1日用量で21日間治療した。
臨床試験
二次性進行型多発性硬化症患者におけるシポニモドとラキニモドの組み合わせの有効性および安全性を評価する多施設ランダム化二重盲検並行群間比較プラセボ対照可変投与期間試験。
a)主要目的
主要目的は、EDSSで測定した場合のSPMS患者における3か月確認障害進行停止期間を遅延させる上での、プラセボと比べたシポニモドとラキニモドの組み合わせの有効性を示すことである。
b)重要な副次的目的
第1の副次的目的は、時限25フィートの歩行試験(T25−FW)におけるベースラインから少なくとも20%の3か月確認悪化停止期間を遅延させる上での、プラセボと比べたシポニモドとラキニモドの組み合わせの有効性を示すことである。
第2の副次的目的は、試験の目的を達成するために、T2病変容積のベースラインからの増加を低減する上での、プラセボと比べたシポニモドとラキニモドの組み合わせの有効性を示すことである。
c)別の副次的目的としては、下記が挙げられる。
・EDSSで測定した場合の6か月確認障害進行停止期間(time to 6-month confirmed disability progression)を遅延させる上での、プラセボと比べたシポニモドとラキニモドの組み合わせの有効性を評価すること、
・確認された再発の頻度を低減する際に、年率換算再発率(ARR)で評価して、プラセボと比べたシポニモドとラキニモドの組み合わせの有効性を評価し、かつ初回再発までの期間および無再発患者の割合を評価すること、
・プラセボと比べたシポニモドとラキニモドの組み合わせの患者報告アウトカム(patient reported outcome)の多発性硬化症歩行スケール(MSWS−12)に及ぼす効果を評価すること、
・通常のMRIで測定される炎症性疾患活動性および疾病負荷(Gdエンハンスメントを呈するT1病変、新たなまたは増大するT2病変、脳容積)に関して、プラセボと比べたシポニモドとラキニモドの組み合わせの有効性を評価すること、
・シポニモドとラキニモドの組み合わせ対プラセボの安全性および忍容性を評価すること。
d)探索的目的としては、下記が挙げられる。
・プラセボと比べたシポニモドとラキニモドの組み合わせの以下の患者報告アウトカムに及ぼす効果を評価すること:
・多発性硬化症インパクトスケール(MSIS−29)で測定される健康関連生活の質(QoL)
・EQ−5Dで測定される健康関連生活の質(QoL)
・プラセボと比べたシポニモドとラキニモドの組み合わせの有効性を確定した認知機能検査で探索すること:
・連続聞き取り加算検査(PASAT)
・符号数字モダリティー検査(SDMT)
・簡易視空間記憶検査改訂版(BVMTR)
・急性病変の慢性ブラックホールへの進展において、MRIによって、プラセボと比べたシポニモドとラキニモドの組み合わせの有効性を評価すること、
・プラセボと比べたシポニモドとラキニモドの組み合わせの有効性をMSFC zスコアで評価すること、
・下記までの時間を遅延させる際に、プラセボと比べたシポニモドとラキニモドの組み合わせの有効性を評価すること:
・時限25フィートの歩行試験(T25W)におけるベースラインから少なくとも20%の3か月確認悪化、または
・EDSSスコアで測定した場合の3か月確認障害進行、または
・両手のうちのどちらか一方(利き手または非利き手)において、ナインホールペグテスト(9−HPT)におけるベースラインから少なくとも20%の3か月確認悪化。
・障害進行終点と薬物濃度/リンパ球数との間の関係を探索すること、
・選択された安全性パラメータと薬物濃度/リンパ球数との間の関係を探索すること、
・シポニモドおよびラキニモドの薬物動態を評価すること。
試験母集団は、EDSSスコア3.0〜6.5、年齢18歳と60歳の間、二次性進行型疾患経過を伴うMS(SPMS)と診断された外来患者からなる。
これは、SPMS患者におけるランダム化多施設二重盲検プラセボ対照並行群間比較試験である。患者は、シポニモドとラキニモドの組み合わせ投与群またはプラセボ投与群にランダム化される。
(a)ラキニモドと、
(b)シポニモドと、
(c)二次性進行型多発性硬化症の治療におけるそれらの同時、異時または逐次使用のための指示書と、
(d)成分(a)および(b)を入れるための少なくとも1つの容器と
を含むキット。
(i)前記対象における前記二次性進行型多発性硬化症を診断するステップと、
(ii)シポニモドとラキニモドを含む治療有効量の組み合わせを前記対象に投与するステップと
を含む方法。

- 二次性進行型多発性硬化症の治療、予防またはその進行の遅延における使用のためのシポニモドとラキニモドを含む組み合わせ。
- 前記組み合わせ中のラキニモドの量が0.1〜1.2mgであり、前記組み合わせ中のシポニモドの量が0.1〜2.0mgである、請求項1に記載の組み合わせ。
- 前記組み合わせ中のラキニモドの量が0.2〜0.5mgであり、前記組み合わせ中のシポニモドの量が0.5〜1.5mgである、請求項1に記載の組み合わせ。
- 医薬として使用するためのシポニモドとラキニモドを含む組み合わせ。
