1,2−ジミリストイル−sn−グリセロ−3−ホスホコリン(DMPC)、ジステアロイル−sn−グリセロ−3−ホスホエタノールアミン−N−メトキシ(ポリエテングリコール)(DSPE−PEG2000)、および1,2−ジステアロイル−sn−グリセロ−3−ホスホエタノールアミン−N−フォレート(ポリエチレングリコール)(葉酸−DSPE−PEG2000)は、Avanti Polar Lipids Inc.(AL、USA)から購入した。オレイン酸コレステリル(CO)はSigma−Aldrich Co.(MO、USA)から得た。1,1’−ジオクタデシル−3,3,3’,3’−テトラメチルインドトリカルボシアニンヨウ化物ビス−オレエート(DiR−BOA)およびポルフィリン−脂質(ピロ−脂質(pyro−lipid)と省略されるピロフェオホルビド(pyropheophorbide)−脂質)を以前に報告されたプロトコルによって調製した(16)。アポA−1模倣R4Fペプチド(R4F)、Ac−FAEKFKEAVKDYFAKFWDは、GL Biochem Ltd.(上海、中国)から購入した。細胞培養培地であるイーグル最小必須培地(EMEM)はATCC(American Type Culture Collection、Manassas、VA)から得た。ウシ胎仔血清(FBS)、トリプシン−エチレンジアミン四酢酸(EDTA)溶液およびHoechst 33258は全てGibco−Invitrogen Co.(CA、USA)から購入した。
USPVの合成
脂質膜を窒素下でクロロホルム中の液体混合物の蒸発によって調製した。USPVについての脂質混合物は、0.9μmolのポルフィリン−脂質、2.1μmolのDMPCおよび0.3μmolのオレイン酸コレステロールからなる。カーゴ負荷粒子について、モデル薬物として作用する3mol%のDiR−BOAを脂質混合物に加え、PEG化USPV製剤(PEG−USPV)について、1%のDSPE−PEG2000を脂質混合物中に加え、葉酸受容体標的化USPV(葉酸−PEG−USPV)について、1%の葉酸−DSPE−PEG2000を脂質混合物中に加えた。完全に乾燥させた脂質膜を1.0mLのPBS緩衝液(150mM、pH7.5)で水和し、40℃にて30サイクル、低周波数(30秒のオン/30秒のオフ)で超音波処理した(Bioruptor(登録商標))。R4Fペプチド(2.3mg、5mg/ml)を再水和した溶液中で滴定し、ペプチド溶液を添加すると、濁ったエマルションが透明になった。混合物を4℃にて一晩振盪し続けた。溶液を12,000rpmにて20分間遠心分離し、続いて上清を0.1μmの膜(Millex(登録商標)、Sigma−Aldrich)で濾過した。
USPVのサイズ分布およびζ電位は動的光散乱(ZS90 Nanosizer、Malvern Instruments)によって測定した。日立(日本)H−7000電子顕微鏡を備えた透過型電子顕微鏡(TEM)を使用して、粒子の形態およびサイズを決定した。
インタクト/クエンチ試料としてPBSまたは破壊/非クエンチ試料としてPBS中の0.5%のトリトンX−100のいずれかでUSPVを希釈した。インタクトおよび破壊されたUSPVの吸収スペクトルを、UV/Vis分光光度計Cary 50(Agilent、Mississauga、ON)によって測定し、Fluoromax−4蛍光光度計(Horiba Jobin Yvon、USA)(励起:420nm、発光:600〜800nm、スリット幅:5nm)を使用することによってそれらの蛍光を測定した。以下の式:(1−FIインタクト/FI破壊)×100%(FIインタクトおよびFI破壊はそれぞれインタクト試料および破壊試料の蛍光強度を表す)によって蛍光クエンチング効率を算出した。
SOSGアッセイを使用してUSPV(インタクトおよび破壊の両方)の1O2生成を測定した。簡潔に述べると、SOSG(1O2センサグリーン試薬、Molecular Proves,Inc.)溶液をメタノール(5mM)中で新たに調製し、6μMの最終SOSG濃度となるように、PBS中のインタクトまたは0.5%のトリトンX−100中の破壊されたUSPV(1μMの最終ピロ濃度)と混合した。0.5J/cm2から10J/cm2の光フルエンスで671nmにて発光ダイオードのアレイを用いて試料を処理し、次いで504nmでの励起および525nmでの収集によってSOSG蛍光を測定した。この発光ウインドウ内にポルフィリン蛍光の寄与はなかった。
U87GFPおよびU87luc細胞を、10%のFBSを有するイーグル最小必須培地(EMEM、ATCC(登録商標))中で培養した。USPV対ポルフィゾームの細胞取り込みを比較するために、定量的細胞取り込み研究をU87神経膠腫細胞で実施した。簡潔に述べると、U87GFP細胞を、インキュベーションの24時間前に106細胞/ウェルにて6ウェルプレートに播種し、10μMのポルフィリン濃度にてポルフィゾームおよびUSPVと37℃で3時間インキュベートした。PBSで3回リンスした後、細胞をトリプシン処理し、懸濁液を4000rpmで5分間遠心分離した。次いで、細胞ペレットを500μLの溶解緩衝液に再懸濁し、氷上で1時間インキュベートした。溶液を10,000rpmで10分間遠心分離し、蛍光分光計によりポルフィリンの蛍光測定のために上清を回収し、ポルフィリン分子の細胞取り込みを定量した。ポルフィゾームに対するUSPVの蛍光活性化をさらに調べるために、細胞インキュベーション後のポルフィリン蛍光変化を時間とともにモニターするために共焦点イメージングを行った。5×104細胞/ウェルを、インキュベーションの24時間前に8ウェルチャンバスライドに播種した。細胞を10μMのポルフィリン濃度にてポルフィゾームおよびUSPVと37℃で3時間インキュベートし、PBSで3回リンスし、新たな細胞培養培地中で再インキュベートした。細胞を、培地交換の直後、および3時間後、6時間後に、共焦点顕微鏡法(Olympus FluoView 1000、レーザー633nm、Em)によって画像化した。
動物準備および腫瘍モデル
全ての動物実験は、University Health Networkのガイドラインを遵守して実施した。動物研究は、同所性9Lluc神経膠肉腫のU87GFPおよびU87luc神経膠腫保有ヌードマウスで行った。nu/nuヌード雌性マウスは、Harlan Laboratoryから購入し、University Health Networkの動物研究センター(Animal Research Centre)にて維持した。モデルを確立するために、動物を、ケタミン、キシラジンおよびアセプロマジン(80mg/kg、5mg/kgおよび2.5mg/kg)それぞれの腹腔内注射により麻酔する。Dremelツールを使用して左半球に直径1mmのバリ穴を作製し、硬膜を露出させるがそれをそのまま残す。3μLの培地中の5×104個のU87細胞または1×104個の9L細胞を左半球に注射する。腫瘍サイズは、磁気共鳴イメージング(MRI)によって毎週モニターする。実験は、腫瘍が直径4〜5mmに達した接種後約18日に行った。
USPV、PEG−USPVおよび葉酸−PEG_USPVを、2.5mg/kgの用量でBALB/cマウスに静脈内注射した(n=4)。注射前および注射後(5分、30分、1時間、2時間、4時間、8時間、12時間、24時間および48時間)、連続して血液をマウスの下肢静脈から採取した。血液を室温で30分間置き、血漿を分離し、次いで12,000rpmの速度で10分間遠心分離した。上清の蛍光をSpectrofluorometer(HORIBA Scientific Inc.)により測定し、血中のポルフィリン量(励起420nm、発光675nm、スリット幅:5nm)を算出した。次いで各時点でのポルフィリン量をGraphpad Prism(登録商標)により分析し、粒子の半減期を計算した。
インビボでのUSPVの特定の腫瘍取り込みおよび画像誘導薬物送達能力を調べるために、全身投与の後、蛍光イメージングを実施した。腫瘍保有マウスに、USPV投与前の3日間、低蛍光食餌(Harlan Tekland(登録商標)、製品番号TD.97184)を与えた。次いでUSPV−DiR−BOAを、ポルフィリン含量に基づいて10mg/kgの用量にて尾静脈を介して注射した。ピロ(pyro)シグナルについて575−605nmの励起/680−750nmの発光フィルタおよびDiR−BOAシグナルについて725−755nmの励起/780nmのロングパス発光フィルタでMaestroイメージングシステム(CRI、USA)を使用して蛍光画像を獲得した。注射の24時間後に、頭皮を有するかまたは有さず、頭蓋を開いた状態でインビボで蛍光イメージングを実施した。動物を屠殺した後、脳ならびに心臓、肺、肝臓、脾臓、腎臓、副腎および筋肉を含む主要器官を採取し、エキソビボで蛍光イメージングに供した。FMT(蛍光分子トモグラフィー、PerkinElmer VisEn FMT 2500 LX Quantitative Tomography System、VisEn Medical Inc、Bedford、MA)イメージングおよびインビボ共焦点顕微鏡イメージング(Leica FCM1000、Cellvizio(登録商標) Technology、励起:660nm、発光:689−900nm)も、腫瘍保有脳で実施した。9Lluc−およびU87luc神経膠腫保有マウスについて、ルシフェラーゼ溶液をイメージングの10分前に腹腔内注射した。生物発光イメージングも、インビボおよびエキソビボの両方で実施した。
USPVのPDT有効性をU87GFP腫瘍保有マウスについて調べた。4つの群が含まれた:何も処置をしていないブランク対照群;PDTレーザーのみ;USPV注射のみ;USPVプラスPDTレーザー治療。腫瘍が直径1〜1.5mmに達したとき、ポルフィリン含量に基づいて計算して5mg/kgの用量にてUSPVを動物に静脈注射した。注射後24時間で、マウスを2%(v/v)イソフルランで麻酔し、腫瘍に671nmレーザー(DPSS LaserGlow Technologies、Toronto、カナダ)を照射した。レーザー強度は、処置領域として9mm直径および3.5mm直径のスポットサイズで50mW/cm2として測定された。37.5J/cm2および50J/cm2の光線量をこの研究において適用した。レーザーのみの群およびPDT処置の群についての腫瘍の温度変化を赤外線カメラ(Mikroshot、LUMASENSE Technologies)を使用してモニターし、平均および標準偏差について各処置群においてn=5で計算した。
腫瘍境界を規定するために、エキソビボ蛍光イメージング後に脳を液体窒素中で凍結させ、次いでLeica CM3050Sクライオスタットを使用して5μm厚さのスライドに切断した。University Health NetworkのPathology Research Program Laboratoryにおいて標準的な方法によってH&E染色を実施した。切片を、20倍の明視野顕微鏡によって観察し、撮影した。治療効果を評価するために、各処置群からの脳を採取し、処置の24時間後に10%ホルムアルデヒド中で固定した。H&E染色およびTUNEL染色を行い、続いて上記と同じ標準プロトコルで分析した。
凍結したスライドをDAPI含有マウント溶液でマウントし、405nm(DAPIチャネル)、491nm(GFPチャネル)および633nm(Cy5.5チャネル)の励起波長でOlympus FV1000レーザー共焦点走査顕微鏡(Olympus、東京、日本)およびQuorum WaveFX Spinning Disk Confocal(Yokogawa、日本)によって画像化した。
他の文献(17、18)に記載されている方法を使用してVX−2口腔扁平上皮細胞がんモデルを開発した。簡潔に述べると、新たに安楽死させたウサギから滅菌条件下で腫瘍を採取し、ハンクス平衡塩溶液(HBSS、Sigma)に入れ、滅菌HBSSで2回洗浄し、小切片に切断し、使用するまで−80℃にて保存した。単一の腫瘍細胞懸濁液を得るために、腫瘍片を解凍し、細かく刻み、70μmのセルストレーナーを通して圧縮した。300μLの高密度単細胞懸濁液(約5×106/mL)を、麻酔したニュージーランドホワイトウサギ(2.8〜3.3kg)の頬筋(頬の領域)に注射する。
腫瘍サイズが1.5〜2.0cmに達した腫瘍誘導の約2週間後、端部の耳静脈においてウサギにカテーテルを介して64Cu−USPVを静脈内注射した(ポルフィリンについて0.33mg/kg、約5mCi)。動脈血を注射の5分、0.5時間、1時間、4時間、8時間、21時間、30時間後に採取した(n=4)。血漿の放射能をガンマカウンター(Wizard 1480:PerkinElmer Inc.、MA、USA)で濃度の関数として測定した。クリアランス半減期を対数線形回帰によって決定した。
64Cu−USPV(ポルフィリンについて0.33mg/kg、約5mCi)の注射後24時間に、ウサギを麻酔し、5mLのオムニパーク(Omnipaque)350(GE Healthcare、Mississauga ON)の注射後、MicroPETシステムでのPETイメージング(Focus 220:Siemens、ミュンヘン、ドイツ)、およびmicroCTシステムでのCTイメージング(Locus Ultra:GE Healthcare、UK)に供した。PET/CT画像を登録し、Amira(FEI Visualization Sciences Group、Bordeaux、フランス)を使用して統合した。対象のボリュームを、Inveon Research Workplace(Siemens、ミュンヘン、ドイツ)を用いて、統合したCT画像上に描き、64Cu−USPVの標準取り込み値(SUV)を登録画像から定量した。
PET/CTイメージング後、腫瘍、リンパ節、唾液腺、肺、心臓、肝臓、筋肉、脾臓および腎臓を含むウサギの器官を切除し、秤量し、ガンマカウンターで放射能を測定した。器官取り込みを、各ウサギについて試料の全動物質量の百分率当たりの注射用量の百分率(SUV)として計算した。イエローフィルター設定(励起:575−605nm;発光:≧645nm検出、200ms曝露時間)でMaestro(Caliper Life Sciences、MA、USA)を用いてエキソビボ蛍光イメージングを実施した。
凍結した組織切片を固定し、DAPI、H&E及びパン−サイトケラチン染色でそれぞれ処理した。染色切片の高解像度画像を走査レーザー共焦点顕微鏡(TISSUEscope 4000、Huron Technologies)で取得した。
VX−2ウサギでのリアルタイム蛍光誘導手術を、4mg/kgのUSPVの静脈内注射の24時間後に、インハウス蛍光イメージング内視鏡システム(650±20nm励起、700±25nm発光)を用いて実施した。非蛍光結節が動物の手術台に残るまで、蛍光で誘導して腫瘍および疑わしいリンパ節を解剖した。
4群のVX−2ウサギを処置研究に含めた:ブランク対照(n=3);PDTレーザーのみ(n=3);USPV注射のみ(n=3);USPVプラスPDTレーザー処置(n=4)。腫瘍サイズが約300mm3に達したとき、USPV群およびUSPV−PDT群(4mg/kgのポルフィリン用量)についてUSPVをウサギに静脈内注射した。PDT処置のために、ウサギを麻酔し、注射後24時間に2段階のPDT処置に供した。第1の段階は、125J/cm2の光線量、200mWのレーザー出力および直径15mmの照射領域での腫瘍の外面上への直線レーザー照射(671nm)であった。レーザー照射の間の腫瘍の温度変化を、赤外線カメラを使用してモニターした。第2の処置段階は、120J/cm2の光線量および100mWのレーザー出力で腫瘍の内部から照射するために腫瘍内への光ファイバーケーブル(9mmの拡散レーザーファイバー)の挿入を含んだ。処置後、ウサギをケアの標準的なプロトコルに従って置き、腫瘍成長をmicroCT走査で連続的にモニターした。腫瘍サイズが5000mm3に達したとき、終末手術をウサギに実施した。4匹全てのUSPV−PDTウサギは処置の約30日後に腫瘍を含まないことが見出された。処置有効性のさらなる評価のためにPDTの34〜36日後にそれらを安楽死させた。
スチューデントのt検定(両側)を使用して、TUNELおよび毒性研究における異なる群間の差の統計的有意性を決定した。0.05未満のP値を有意とみなした。
USPVの合成および特性付け
本発明者らは、DMPCを有するポルフィリン脂質のリン脂質単層によって封入され、18アミノ酸のアポA−1模倣ペプチドによって拘束された、コレステリルオレイン酸の疎水性コアを有する超小型ポルフィリン小胞(USPV)を作製した。本発明者らは、USPVの構造および光物理的特性が、DMPCに対するポルフィリン−脂質の割合に依存することを見出した。図1に示すように、DMPCに対するポルフィリン脂質の割合を増加させると、ポルフィリン蛍光クエンチングが増強され、粒子サイズが増大した。この割合が30%を超えると、高い蛍光クエンチング(>95%)が達成され、粒子サイズは依然として30nm以下に制御された。30%モルのポルフィリン−脂質/70%モルのDMPCを有するUSPVを、さらなる応用研究のための最適USPVとして選択した。なぜならそれは、安定した単分散USPVについての最大ポルフィリン脂質を含有し、良好なサイズ(<30nm、図2)を有し、効果的な蛍光クエンチングを示したからである。
ピロフェオホルビド−脂質(ピロ−脂質)の吸光度スペクトルに基づいて、推定USPV吸光係数ε680は7.8×107cm−1M−1であった。この光吸収の増強はUSPVにおける高密度のポルフィリン環境を示す。CDスペクトルにより、USPVのαヘリックス構造が確認された(図3)。
USPVの光学特性は、同じポルフィリン濃度にてPBS中のインタクトな粒子およびトリトンX−100中のその構造破壊試料の蛍光および一重項酸素生成を比較することによって調べた。図4に示すように、ポルフィゾームについて観察されたものと同様に、高密度のポルフィリン環境は、USPVの蛍光生成および一重項酸素生成を極めて阻害した。USPVの蛍光は、ナノ構造破壊試料と比較して100倍クエンチされた。広範囲の光フルエンス(0.5〜10J/cm2)でPDTレーザー(671nm)を照射すると、USPVはナノ構造破壊試料と比較して一重項酸素生成が2〜3倍少なかった。したがって、インタクトなUSPVは光線力学的に不活性であるが、それは、ナノ構造が破壊されたときPDT活性になる。
小型粒子が細胞内取り込みに有益であるかどうかを調べるために、細胞溶解緩衝液中でポルフィリン蛍光シグナルを測定することによって、USPVおよびポルフィゾームの細胞取り込みをU87神経膠腫細胞において調べた。ポルフィゾームと比較して、USPVは、同じ濃度のポルフィリンによるインキュベーション後、U87細胞において約10倍高い取り込みを示した(図5a)。細胞におけるポルフィリン蛍光活性化を共焦点試験によってさらに評価した。時間がかかるプロセスであり、細胞内のポルフィリン蛍光が徐々にクエンチングしなくなることが証明されている細胞内のポルフィゾーム破壊とは異なり、USPVとの3時間のインキュベーションの直後にU87細胞において強いポルフィリン蛍光を観察し(図5b)、蛍光シグナルは時間とともにさらに著しく向上するとはなかった。全体として、これらのデータにより、小型USPVが細胞内局在化だけでなく、細胞内の光特性活性化も促進することが示唆された。
USPVの薬物動態プロファイルを調べるために、血中クリアランス研究を健常なマウスで実施した。3つの群をこの研究に含んだ:USPV、PEG−USPV(PEG化USPV)および活性ターゲティングFR−USPV(葉酸受容体標的化USPV)。血清中のポルフィリン濃度を、蛍光測定を使用して投与後の異なる時点で測定した。図6に示すように、PEG化と関係なく、USPVおよびPEG−USPVの両方は、同様の好ましい遅い循環半減期(それぞれUSPVについて9.9時間およびUSPV−PEGについて9.5時間)を有し、インビボ循環の改善のためにPEG化が必要ではないが、PEG化はインビボでリポソーム構造の安定性を改善するためにほとんどのリポソーム構造に不可欠であることが示された。興味深いことに、ポルフィゾーム製剤中の葉酸−脂質の包含が粒子のインビボ循環時間を短くしたという本発明者らの以前の観察と相反して(10)、FR−USPVは著しく延長した遅い半減期を示した(FR−ポルフィゾームについて4時間未満に対して葉酸−USPVについて13.3時間)。EPR効果がナノ粒子の腫瘍蓄積において重要な役割を果たすので、この長期にわたる循環は、組織への血液循環からの直接的なナノ粒子の浸潤に有益であり、ターゲティング疾患領域における粒子の保持を高める。したがって、より効果的なターゲティング送達およびより効果的な光特性(蛍光および一重項酸素生成)活性化が、葉酸−ポルフィゾームと比較してFR陽性がん種におけるFR−USPVについて予想される。
本発明者らは、最近、40nm以下のポルフィリン脂質ナノディスクを開発し、小型ナノディスクが、腫瘍のコラーゲンが豊富なマトリクスを通る拡散において、大型のポルフィゾーム(130nm)と比較して拡散係数の5倍の増加を示したことを実証した。ここで本発明者らは、ポルフィゾームよりも小型のUSPVのインビボ送達の利点を調べた。9Lluc神経膠腫を有するマウスに、200nmolのポルフィリン濃度にてUSPV(21nm)およびポルフィゾーム(130nm)を注射し、マウス頭蓋を投与後24時間に麻酔下で除去して、インサイチュで蛍光画像のために腫瘍を曝露した。図7、中央の列に示すように、USPVおよびポルフィゾームの両方は蛍光イメージングによって周囲の健常な脳から腫瘍を明確に描写することができ、これはBLIイメージングによって画定された腫瘍部位(図7、左の列)と十分に一致する。しかしながら、USPVを投与した腫瘍からの蛍光シグナルは、ポルフィゾームを投与した腫瘍よりもはるかに強く、腫瘍特異的蓄積を増強する超小型USPV(30nm未満)の利点を示唆している。9Lluc神経膠腫腫瘍におけるUSPVの腫瘍蓄積の特異性はエキソビボ脳組織イメージングによってさらに実証され(図8a)、脳における蛍光コアはH&E組織切片によって描かれた腫瘍領域と十分に並んだ(図8b)。本発明者らはさらに、凍結した脳組織切片の共焦点イメージングを使用して顕微鏡レベルにおけるUSPVの腫瘍特異的取り込みを検証し、強いポルフィリンシグナルを、対側脳領域ではなく、腫瘍周辺領域においてのみ観察した(図8C)。
USPVは、脂質単層で囲まれた疎水性コアを有するコア−シェルナノ構造を有するので、疎水性の生物活性化合物の負荷および安全な送達の好適な可能性を有する。この研究では、近赤外蛍光疎水性色素、DiR−BOAを薬物代用物として使用して、USPVの薬物負荷能力および送達挙動を調べた。USPV製剤(0.9μmolのポルフィリン−脂質、2.1μmolのDMPCおよび0.3μmolのCO)中に0.5molのDiR−BOAを添加することにより、DiR−BOAは、85%の負荷効率で粒子にうまく負荷された。22.5nmのサイズを有する得られたUSPV(DiR−BOA)(図9a)は、最小のサイズ変化およびごく少量のDiR−BOA漏出を30日にわたって観察したので、4℃にてPBS中でかなり安定であった。次いで本発明者らは、同所性U87神経膠腫保有マウスにおいてUSPV(DiR−BOA)のインビボでの挙動を調べた。USPV(DiR−BOA)を注射してから24時間後のマウスを、麻酔下で頭蓋除去手術に供し、CRI Maestro(商標)イメージングシステムを使用してポルフィリンチャネル(Ex:615nm、Em:680−750nm)およびNIR薬物代用チャネル(Ex:750nm、Em:780−950)においてそれぞれ蛍光画像化した。図10aに示すように、ポルフィリンおよびDiR−BOAシグナルの両方は腫瘍中に高濃度で存在しており、これにより、健常な脳を近くに保ちながら腫瘍境界が明確に描かれる。さらに、これらの2つの蛍光シグナルは十分に共局在化されており、これは、USPV(DiR−BOA)が、腫瘍内に選択的に薬物代用物の安定で、効果的な送達を可能にすることを示唆している。より興味深いことに、この非常に効果的な送達は、非蛍光対側脳細胞を保ちながら、ディープレッドロングパスフィルタを用いてインビボでの蛍光共焦点顕微鏡法によって顕微鏡レベルで腫瘍細胞の蛍光検出を可能にした(図10b)。USPVの腫瘍特異的蓄積をさらに検証するために、その組織の生体内分布を、動物の屠殺時に蛍光イメージングによって調べた。図10cに示すように、神経膠腫腫瘍および肝臓のみが、ポルフィリンおよびDiR−BOAの強い蛍光シグナルを示したのに対して、他の器官は無視できるほどの蛍光を示し、このことはUSPV(DiR−BOA)の非常に高い腫瘍特異的取り込みを実証した。ほとんどのナノ粒子の送達と同様に、USPVの高い肝臓取り込みは、おそらく肝胆道クリアランスによるものであった。しかし、ポリフィゾームを含むほとんどのナノ粒子の送達と異なり、USPVの非常に低い脾臓取り込みは、細網内皮系による濾去からの「回避」に起因するその超小型のサイズの恩恵を受けているようである。検出した組織の全てにおけるポルフィリン蛍光とDiR−BOA蛍光との間の十分な相関関係(図10c)はさらに、種々の組織における蓄積のための全身送達においてUSPV(DiR−BOA)が構造的にインタクトなままであることを実証した。全体として、これらのデータは、1)USPVが、最小限の前漏出(pre−leakage)および標的外効果でがん療法のための高度な腫瘍選択性および効果的な薬物送達系を提供すること、2)安定な送達特性に起因して、蛍光などのUSPVのポルフィリンシグナルが、薬物送達を追跡して治療計画を導くために使用できることを示唆している。
ポルフィリンは多くの金属にとっての重要なキレート剤であるので、非常に安定な金属錯体を形成する(20)。本発明者らの研究により、ナノ粒子のインビボ結果のPETイメージングを可能にする、ポルフィゾームのポルフィリン−脂質に対する放射性銅64(64Cu)の安定なキレート化を実証した(11−12)。同様の標識化アプローチを使用して、本発明者らは、高い64Cu標識効率(>95%)で64CuをプリフォームUSPV内に組み込むことに成功し、次にその送達挙動を調べた。図11に示すように、64Cu−USPVは卵巣がん転移を選択的にピックアップすることを可能にし、転移腫瘍は非常に明るいPETシグナルを示したのに対して、卵管などの周囲組織は最小のシグナルを示した。PETイメージングにより可能となる腫瘍特異的ピックアップは、腫瘍細胞からの生物発光シグナルと十分に相関した、エキソビボ組織ポルフィリン蛍光イメージングによってさらに確認された。転移組織は組織学的分析によってさらに識別された。したがって、本質的にUSPVの64Cu標識化により、ナノ粒子送達の非侵襲性および正確な追跡ならびにさらにその腫瘍優先的蓄積に起因する腫瘍の検出が可能となり、したがって臨床応用への転換のための大きな可能性が示される。
腫瘍の外科的除去は依然として臨床診療において神経膠腫治療の主流であり、結果は患者の生存に影響を与える。外科処置における主な課題は断端陽性を定義することである。不十分な手術は腫瘍の局所再発およびサルベージ療法の失敗をもたらし、過剰な切除は重要な神経機能の喪失につながる。したがって、切断縁の正確な描写は、手術の間、脳について不可欠である。本発明者らは、全身投与の24時間後、固有のポルフィリン蛍光およびDiR−BOAシグナルによって腫瘍を可視化し、周囲の健常な脳から腫瘍領域を描写するためのUSPV(DiR−BOA)の能力を実証した。本発明者らは次に、蛍光誘導神経膠腫手術におけるその潜在的用途を調べる。臨床状況を模倣するために、脳内深く(上面から5mm)に腫瘍を播種した同所性U87GFP神経膠腫マウスモデルを利用した。図12aに示すように、USPV(DiR−BOA)の注射から24時間後、固有のポルフィリン蛍光もDiR−BOA蛍光も、Maestroイメージングシステムを使用してインタクトな脳の上面から観察されなかったが、両方の蛍光シグナルはFMTイメージングによって明確に検出できた。図12bに示した切断プロセスの後、神経膠腫腫瘍を脳の上部を除去することによって露出させた。底部が固形腫瘍実体を含んでいたので、最小の腫瘍残存を有するそのような上部を手術床とみなした。図12cに示すように、ポルフィリンおよびDiR−BOAシグナルの両方は、腫瘍組織が腫瘍細胞のGFP蛍光と十分に相関したので、それらを正確に可視化し、定義することができた。次いでポルフィリン蛍光組織を収集し、組織学的分析および凍結組織切片のために送った。図13に示すように、H&E染色は組織のがん細胞形態を明らかにした。一方、凍結した組織スライドは、顕微鏡レベルでGFPシグナル(腫瘍細胞から)およびポルフィリンシグナル(USPVから)の両方を示し、USPVがイメージング誘導手術のための腫瘍を描写する能力をさらに確証した。さらに、USPVのポルフィリン蛍光は、MRI走査によっても検出できなかった、4mmから1mm未満のサイズの範囲のマウスの脳にわたって散在しているU87luc腫瘍の多焦点を同定することができた。図14に示すように、除去された蛍光焦点は腫瘍細胞の固有の生物発光シグナルを明確に示した。まとめると、これらの結果は、蛍光誘導神経膠腫手術のための良好な手段を提供する、腫瘍同定のためのUSPVの高い特異性および感度を実証した。
USPVの蛍光および一重項酸素生成の両方は、インタクトなナノ構造において高度にクエンチされ、腫瘍内蓄積後に迅速に回復され得る。本発明者らは、インビボでPDTについてのUSPVの潜在的な用途を広範に調べた。USPVの蛍光活性化はナノ構造破壊および一重項酸素活性化の評価のための有用な指標として役立ち得る。前述のように、神経膠腫腫瘍は注射後24時間でポルフィリン蛍光の有意な増加を示した。次いで本発明者らはレーザー照射のためにこの時点を選択した。簡潔に述べると、レーザー照射(671nm、50mW/cm2)は、4mg/kgのポルフィリン用量でのUSPV注射の24時間後、50J/cm2または37.5J/cm2の光フルエンスで小さな皮膚切開を通して経頭蓋(trans−cranium)により適用した。レーザー照射の間の腫瘍温度は熱カメラによってリアルタイムにモニターした。処置の24時間後に、動物を屠殺し、脳組織を組織学的分析およびTUNEL染色のために調製した。レーザー照射のみを受けている神経膠腫腫瘍を有するマウスおよびUSPVのみを受けている神経膠腫腫瘍を有するマウスを、それぞれレーザー対照およびUSPV対照とした。全てのレーザー処置群について腫瘍温度の有意な上昇はなく(約27℃で一定のままであった)、光熱効果が処置に貢献しなかったことが示された(図15)。50J/cm2または37.5J/m2の光フルエンスのいずれかにおけるUSPV−PDT後の腫瘍組織はH&E染色において濃縮した核および細胞構造の損失を示したのに対して、USPVおよびレーザー対照由来の腫瘍組織は影響を受けていないままであり(図16)、USPVにより可能な効果的なPDTならびにUSPVおよびレーザー照射のみの非侵襲性を示した。TUNEL染色はさらに、USPV−PDTが50J/cm2群について75.4%のTUNEL陽性細胞で明らかな細胞アポトーシスを誘導し、37.5J/cm2群について82.1%のTUNEL陽性を確認し(図17)、対照群について有意な細胞アポトーシスは観察されなかった。さらに、USPV−PDT群の周辺脳組織において観察可能な組織学的変化およびアポトーシスは存在せず、USPV−PDTによって引き起こされる副作用が無視できるほどであることを示している。したがって、USPVは、正常な健康状態を保ちながら、非常に低い光線量にて腫瘍特異的PDTを可能にし、それにより、安全なPDT治療プロトコルを提供する。
HNC患者の低い生存率は遅い疾患の診断および高い再発率に起因する。現在のHNC病期診断は適切な治療管理のための診断の不十分な精度および低い感度に悩まされている。PET、蛍光イメージングおよびPDTの固有の多様性を伴うUSPVは、HNC病期診断の精度を高め、HNC管理に革命をもたらす大きな可能性を提供し得る。腫瘍誘導後に局所リンパ節への転移を一貫して発生できる臨床的に関連するVX−2口腔がんのウサギモデルを使用して、本発明者らは、HNC診断および管理についてのUSPVの能力を調べた。
VX−2ウサギにおける64Cu−USPVの血中クリアランスプロファイルを2コンパートメントモデルに適合すると、最大で27.7時間の遅い半減期を有利に示した(図18a)。したがって、PETイメージングを、その生物学的半減期および放射性核種半減期(64Cu t1/2=12.7h)を一致させるために64Cu−USPV(0.34mg/kgのポルフィリン、約5mCi)の静脈内注射の24時間後にVX2ウサギにおいて実施した。PET/CT共登録画像(図18b、図19)に示されるように、腫瘍およびセンチネルリンパ節(SLN)は高いコントラストで明確に識別可能であった。レンダリングされた画像と一致して、腫瘍およびSLNは、周囲の筋肉のものと比較して対象のPET容積(VOI)測定値から定量された有意に高い標準取り込み値(SUV)を示し、それらはそれぞれ3.58±0.53、2.57±0.53および0.35±0.02(n=5、P<0.05、図18c)であった。
腫瘍および転移リンパ節におけるUSPVの選択的蛍光活性化を利用して、本発明者らは、腫瘍を有するウサギにおける原発性腫瘍およびSLNの外科的切除のための蛍光術中ガイダンスとしてUSPVの能力を評価した。図21aに示すように、腫瘍(皮膚はインタクトである)は、インビボでの蛍光イメージングシステム下で周辺組織と比較して可視化のために十分に蛍光性であった。外科的診査の間に皮弁を持ち上げると、腫瘍が露出し、ポルフィリン蛍光によって明確に描写された(図21b)。蛍光によって誘導して、検査の周囲の全ての疑わしい悪性腫瘍を外科的に除去した。手術床は無視できるほどの蛍光シグナルを示し、完全な腫瘍切除を示唆した(図21c)。切除された組織は組織学的分析によって悪性であることが確認された(図21d)。組織の組織学的スライドにおけるポルフィリン蛍光は、がん細胞形態および陽性PanCK染色と十分に対応し、USPV蛍光が細胞レベルでかなりの特異性および精度で原発性腫瘍を強調したことを示した(図21d)。同様に、USPV蛍光はまた、インビボで排出SLNを描写した(図21e)。注目すべきことに、原発性腫瘍からSLNおよび局所リンパ節までのリンパ管網は蛍光シグナルによって極めて詳細にマッピングされた(図21f)。リンパ管網の方向(拡大画像、図21fの位置1〜5)に従って、二次陽性リンパ節およびリンパ管の広がりパターンを識別した。組織学的研究により、リンパ節における転移が確認され、PanCK陽性領域において強いポルフィリン蛍光が観察され、転移領域におけるUSPVの取り込みが示唆された(図21g)。全体的に、USPV蛍光は、原発性腫瘍および悪性リンパ節だけでなく、局所リン管網も明確に描写し、これにより、切除および病理学的分析の前にHNC患者のリンパ節分類に役立ち、悪性リンパ節を明らかにする可能性があり得る。
HNCウサギにおいてUSPV−PDTの長期間の治療効果を評価した。300mm3の平均腫瘍サイズを有する腫瘍保有ウサギを、ブランク対照(n=3)、レーザーのみの対照(n=3)、USPVのみの対照(n=3)およびUSPV−PDT群(n=4)を含む4つの群に分類した。図22aに示すように、腫瘍全体の領域を照射するために2段階のレーザー照射戦略をUSPV注射の24時間後にPDTについて使用した。レーザー処置の間、有意な温度増加がないことにより、熱的効果が隣接する健常組織に対して意図しない副作用を引き起こし得るという懸念を除いて、処置の熱的効果がないことが確認された(図23)。USPV−PDTは、PDT後24時間から開始して治療後26日まで腫瘍周辺の瘢痕化を引き起こした。最終的に、全てのUSPV−PDTウサギは処置の34日後に触診可能な腫瘍を有さなかった(図22b)。治療後の腫瘍体積を3D再構成microCT画像の体積測定によって定量的に測定した。USPV−PDT群は、治療後1週間以内にわずかな腫瘍サイズの増加を示し、これは、PDTによって引き起こされる予想される炎症反応および浮腫に起因する可能性が高い(図22c)。しかしながら、PDT後34日目に腫瘍が検出されなくなるまで、腫瘍サイズはPDT後6日目から徐々に低下した。対照的に、レーザー照射またはUSPV投与単独のいずれかを受けた対照群はブランク対照と同様に指数関数的な腫瘍増殖を示し、それらのいずれもあらゆる治療効果を誘導しなかったことが示された(図22d、図24)。対照群はそれぞれ、ブランク対照について6日目、レーザー対照について8日目、およびUSPV対照について9日目にエンドポイント(腫瘍体積>5000mm3)に達した(図5d)。USPV−PDTにより可能な完全な腫瘍アブレーションは、病理学的分析によってさらに確認された。このことは、終末外科手術において元の腫瘍領域から切除された組織が、その陰性PanCK染色に加えて、病的細胞形態を示さなかったことにより実証された(図22e)。注目すべきことに、直接的なレーザー照射を受けなかったが、USPV−PDT群のリンパ節はPDTの14日後からサイズが徐々に減少したことが示された(図25)。USPV−PDT群からの全てのリンパ節は、病変およびPanCK染色分析によって証明されるようにPDT後34日で転移が認められなかった(図22f)。これらの結果は、中咽頭、鼻咽頭、下咽頭のような外科的にアクセスできない、または重要な解剖学的構造に隣接するHNCサブタイプについて、および再発症例について、USPV−PDTが、治療効果を増加させ、長期間の毒性を減少させるための放射線治療および化学療法に対する代替の手法として役立ち得ることを強く示唆している。USPV−PDTは、非常に効果的であり、高度に局在化するように見え、健常な組織機能の維持を可能にする。
ウサギに対するUSPV−PDTの毒性を定期的に血液検査によって評価した(図26a)。処置後のウサギの肝機能は、アルカリホスファターゼ(ALP)を除いて、有意な変化がなく正常を維持し、アルカリホスファターゼ(ALP)は、処置の1週間後に正常な範囲内(68.1±8.66〜43.5±9.67U/L)で中程度の減少を示し、時間と共にベースラインレベル(正常な範囲12〜98U/L)に戻った。赤血球レベルは治療後安定なままであり、内因性ポルフィリン(heme)の生理学的調節に干渉しないことを示している。白血球数もまた影響を受けないままであり、USPVによって免疫原性効果が引き起こされなかったことを示唆している。USPV−PDTウサギに対する死後の組織学的分析は、心臓、肺、肝臓、脾臓、副腎または筋肉において異常な細胞形態を示さなかった(図26b)。これらの結果は、USPVにより可能なPDT治療が安全な治療手法であることを示唆している。
