急性心不全後のヒト心臓に見出される状況と類似した、冠動脈結紮後の心筋梗塞(MI)の臨床マウスモデルを、開発した。心臓を優先的に形質導入する血清型9−MV9のアデノ随伴ウイルス(28、83、55)を、心臓に野生型TERT発現を特異的に送達するための遺伝子治療ベクターとして使用した。
FVB/N背景のオスのマウスは、MadridのCNIOの特定病原体未感染の障壁領域で生まれ、そこに収容された。すべての動物手順は、研究および動物福祉のためのCNIO−ISCIII倫理委員会(CNIO−ISCIII Ethics Committee for Research and Animal Welfare)(CEIyBA)によって承認され、欧州実験動物学会連合(FELASA)の提案に従って行った。
ウイルスベクターをMatsushita(48)によって説明されるように生成し、先に説明されるように精製した(49)。ウイルスゲノム粒子の力価を、定量的リアルアイムPCRによって決定した。ウイルストロピズムを評価するために、マウスの対照群に、先に記載した方法に従って、CMVプロモータの制御下でeGFPまたはTertを有する異なる濃度のウイルスを注入した。4週目に、注入後マウスを犠牲にし、病理分析またはeGFPおよびTert発現評価のいずれかに供した。eGFP免疫染色の定量化を、ヘマトキシリン染色細胞(肝臓および脳)の総数にわたる、ペルオキシダーゼ染色細胞の数、または総断面積(心臓)に対するペルオキシダーゼ染色面積を計数することにより決定した。異なる組織におけるテロメラーゼ発現を、先に記載したプロトコル(50)に従って、ウエスタンブロット分析、定量的RT−PCR、およびTRAP(テロメラーゼ反復配列増幅プロトコル)アッセイによって分析した。ウイルスゲノムコピー数を注入から4週間後の心臓DNA試料から、特異性AAV9プライマーを用いて、先に記載されるように評価した(51)。
離乳後、1つのケージ当たり5匹のマウスを収容し、非精製食(no 2018、Harlan)を不断給餌した。MIを、イソフルラン麻酔下で、恒久的左冠動脈前下降枝(LAD)結紮によってオスのマウスに誘発した。鎮痛剤であるブペノフィン(bupenorphine)を、本手順の前に腹腔内注入を介して投与した。疑似手術を、対照群および注入済みマウスに行った。疑似手術は、LADの結紮を除いて、同じ手術手順からなる。安楽死の日を、マウス生存を推定するために使用した。MI後生存の評価のために、マウスを観察し、LAD結紮後、最大6週間検査した。
経胸壁心エコーを、高解像度Vevo770を用いて、記載されるように、1〜2%のイソフルランで落ち着かせ、加熱パッドの上に置かれたマウスに行った(52)。マイクロPET画像法を、先に記載されるように行った(53)。
心臓を、リン酸塩で緩衝化した4%のホルムアルデヒド中に固定し、パラフィン中に包埋し、心臓スライサを使用して、底側、心室中部、および尖側領域から薄片(厚さ2mm)にした(Zivic instruments、HSMS001−1)。心臓スライド断面(5mm)をマッソン3色で染色した。梗塞サイズおよび線維化の長さを、瘢痕長さ対心室中部断面の総LV周長の平均比として計算した。間質性線維化面積を、ImageJソフトウェアを使用して、半定量的手段で、マッソン3色で染色した横断心臓面に行った。1つの心臓あたり3枚の非梗塞面積からの画像を分析した。
組織からの総RNAを、QiagenのRNeasyミニキットを用いて、製造業者の使用説明書に従って抽出した。処理前に、RNA試料をDNase Iで処理した。定量的リアルタイムPCRを、ABI PRISM 7700(Applied Biosystems)を使用して、以下のプライマーを使用して行った。

統計分析(スチューデントのt検定)を、デルタ−デルタCt値で行った。ウエスタンブロットを、次の抗体:抗h/TERT1(Calbiochem)、抗GAPDH(Sigma)を用いて、指示された組織から全細胞抽出物を作製した。定量化を、Scion Image Softwareを使用して行った。
パラフィン包埋組織切片に対してQ−FISH決定、および(末梢血中で)HT−qFISHを、先に記載されるように行った(54、55、56)。血液試料HT−QFISHに対して高スループットQ−FISH(HT−QFISH)TL分析を、記載されるように行った(56)。簡潔に、末梢血を、動物を安楽死する前に顔面静脈から抜き取り、赤血球を溶解し、白血球をプレーティングし、固定し、Tel−Q−FISHに供した。蛍光強度を、較正基準としてL5178−RおよびL5178−S細胞を使用して、Kbに変換し、これらは、それぞれ79.7kbおよび10.2kbの安定したTLを有する(62)。試料を、不偏の自動OPERA撮像システムでの較正基準の場合、2連または3連で分析した。
血液試料を、MIの6週間後にいずれの抗凝血剤もない管に採取した。試料を、20分間氷上で維持し、血清上清を冷凍し、冷凍/解凍劣化を最小化するために、分析するまで80℃で維持した(57)。マウス(描写されるマウスの群および数)における血清尿素濃度を、ABX Pentra400血清分析装置で決定した(Horiba Medical)。さらに、血清レベルの高スループット分析を、RodentMap v3.0(Myriad RBM)で測定した。
冷凍心臓試料からの全RNAをQiagen RNeasyキットで抽出し、RNA完全性をAgilent Bioanalyzerで分析し(7.8未満のRNA完全性指数を有する試料は廃棄した)、製造業者の使用説明書および(58)に従ってAgilentのMouse Genome DNAマイクロアレイを分析した。簡潔に、特異的に発現した遺伝子を、R limmaパッケージを使用して線形モデルを適用することにより得た(59)(Bioconductor project、http://www.bioconductor.org)。
対数順位検定を、異なるマウス群の生存曲線における統計的差異を計算するために使用した。不対tスチューデント検定を、mRNAおよびタンパク質発現レベル、TRAP、心機能パラメータ、瘢痕サイズ、ならびに間質性線維症の統計的有意性を計算するために使用した。マン・ホイットニーのU検定を、血清パラメータのために使用した。マッソン3色染色後の心臓観察またはFDG−PETスキャンのいずれかによる病理学的評価を、カイ二乗検定で計算した。
我々は、まず、異なるマウス組織中のマウステロメラーゼRNA構成要素(22)、Terc、ならびにラットにおけるTert発現(23)に関して先に説明されるのと同様に、マウスTert発現が生後1週間の間に心臓において低下するかどうかの判定に着手する。この目的を達成するために、我々は、1、3、7、および10日目に新生児の心臓を単離し、qRT−PCRによってマウスTert発現を判定した。我々は、生後7日からマウスTert mRNAレベルの著しい低下を観察した。
AAV9は、全身的に投与されたとき、心臓および肝臓を優先的に標的にすることで知られるが、肝細胞を標的にする効率は、心筋細胞を標的にする効率よりも約10倍低い(29)。我々は、肝臓、および他の組織の最小形質導入で、心臓を特異的に標的にするAAV9ベクターの最小量を見つけるために、この特異的形質導入効率を使用した。我々の実験条件下で、AAV9レポーターベクター(AAV9−CMV−eGFP)の投与量5x1011ug/マウスは、eGFP免疫組織化学的検査(IHC)によって示されるように、心臓細胞の60%超を形質導入することができたが、肝臓および脳の21個の細胞の1.2%未満を形質導入することができた。テロメラーゼベクター22(AAV9−CMV−Tert、以降、AAV9−Tert)の形質導入効率を推定するために、我々は、形質導入された心臓中の1個の2倍体ゲノム当たりのウイルスゲノムコピー数を、eGFPレポーターベクターまたはAAV9−Tertベクターのいずれかと比較した(図1)。我々は、1個の細胞当たり類似したウイルスゲノムコピー数を見つけ、心臓中でのAAV9−eGFPおよびAAV9−Tertの類似した形質導入効率を示した。eGFPおよび心筋細胞(ベータミオシン重鎖、β−MHC)または線維芽細胞(vimentin)のいずれかのマーカでの共免疫染色を使用して、我々は、心筋内で、AAV9が心筋細胞を優先的に標的にすることを見出した一方で(60%超のeGFP陽性心筋細胞)、線維芽細胞の感染の徴候を見出さなかった。Tertが心臓を特異的に標的にすることは、治療を受けていないマウスと比較して、治療を受けたマウスの心臓中にTert mRNAの量の約400倍の誘発を示したqRT−PCRによっても確認された一方で、Tert mRNA増加は、治療を受けたマウスの肝臓において、40倍低かった。心臓中のTert mRNA発現レベルは、空ベクターで治療したマウスと比較して、AAV9−Tertで治療したマウスにおいて200倍超の増加した発現によって示されるように、少なくとも2ヶ月の間、高いままであった。最後の、増加したTert mRNA発現は、治療を受けてないマウスおよびAAV9−空で治療した制御群と比較して、AAV9−Tertで治療した心臓における増加したTERTタンパク質発現および増加したテロメラーゼ活性と類似していた。一緒に、これらの結果は、我々が成体の心臓における特異的なTert発現を達成したことを示す。
胚発生以降からの心臓における恒常的Tert遺伝子導入発現は、心肥大を引き起こす(25)。したがって、我々はまず、成人期の間のAAV9−Tert治療が心臓形態に何らかの望ましくない影響を与えたかどうかの判定に着手する。心筋梗塞(MI)の非存在下のAAV9−Tert治療は、ウイルス投与の9〜10週間後に評価して、正常な心臓構造を変えなかったか、または心肥大のあらゆる徴候を生じなかった。心臓の正常な構造のさらなる裏付けとして、我々は、AAV9−Tertで治療した心臓中にベータミオシン重鎖(β−MHC)の正常な発現を見出した。これらの結果は、AAV9遺伝子治療を用いたTertの過剰発現が、心臓における恒常的Tert遺伝子導入発現に関して先に報告された悪影響のうちのいずれも有さないことを示す。
MI後の心不全の予防における成人期の間のテロメラーゼ活性化の治療可能性を評価するために、我々は、MI30によって誘発された心不全を再現することが先に示されている、冠動脈結紮後の心筋梗塞の真性(bona fide)マウス前臨床モデルを使用した。心臓特異的AAV9−10媒介導入遺伝子発現は、約2週間後にその最大に達し、それ以降は引き続き安定する(31)。発現は、静脈内注入の直後に既に出現するが、我々は、Tertレベルが最大であることを確実にするために、ウイルス投与の2〜3週間後に、MIを誘発することを決定した。さらに、我々は、ヒトにおいて主に成人期に起こる急性梗塞後の再生およびリモデリングプロセスに関心があったため、我々は、成体(1歳)マウスを使用した。実験下のすべてのマウスは、性別バイアスを避けるため、オスであった。
テロメラーゼ発現がどのようにしてMI後に低い死亡率をもたらすかを調査するために、心機能を、LAD結紮の1週間および3週間後に、二次元心臓超音波検査によって評価した。左心室収縮および拡張容積等の心臓容積が、同様に減少した心臓駆出率を伴う疑似手術(MIなし)マウス(図2B、C)と比較して、MI後に強く増加した(図2D)。重要なことには、心臓容積および駆出率の両方が、TERT遺伝子治療を受けたマウスにおいて大幅にレスキューされたが、空ベクターを受けたマウスにおいてはレスキューされなかった(図2B〜D)。
決定的なテロメア短縮化は、マウスにおいて心不全を引き起こす。具体的には、決定的に短いテロメアを有するテロメラーゼ欠損マウスは、心肥大を含む、ヒトにおける心不全の心臓表現型に似た心臓表現型を呈する(19)。テロメラーゼの主な機能は、決定的に短いテロメアを伸長することであるため、これより我々は、心臓面上で直接定量的テロメアQ−FISHを行うことにより、心臓テロメア長(TL)へのTert治療の影響を扱い始める。我々は、AAV9−Tertで治療した心臓が、空ベクター群と比較して、(主に、心筋細胞で形成される)陳旧性梗塞部位においてTLの純増を示すことを見出した。したがって、AAV9−Tertは、短期間に心筋中の短テロメア、続いて、心不全の関連リスクを減少させるための有力な手段である(TLはウイルス投与の9〜10週間後に測定した)。末梢血リンパ球中の短テロメアがCVDの増加した危険因子を示す予後値も有することが提唱されたため、我々は、末梢血単核細胞(PBMC)細胞のTLも測定した。我々は、対照群と比較して、AAV9−Tert注入マウスからの血球中のテロメア長のわずかな増加を検出し、我々の先の観察を裏付けた(26)。PBMCテロメア長における差異は、疑似手術マウスと、MIを経験し、AAV9−空またはウイルスなしを受けたマウスとの間に認められず、急性MIならびに炎症およびリモデリング等の後続の事象が、PBMCのTLに負の影響を与えないことを示唆した。これらの研究結果は、循環血液中の短テロメアの存在量を低下させることにより、テロメラーゼ遺伝子治療が追加の有益な効果を有し得、次いでこれが、CVDの既知の危険因子であり得ることを示唆する。
MI後の心臓におけるアポトーシスおよび増殖におけるTert治療の影響を研究するために、我々は、MIの6週間後に、活性カスパーゼ−3およびKi67陽性染色をそれぞれ示す細胞のパーセントを定量化した。我々は、治療に関係なく、アポトーシスが局所貧血後の初期反応であるという事実に一致して、MIの6週間後にアポトーシスを起こした細胞をほとんど見出さなかった(33)。MI後に、我々は、疑似手術マウスと比較して、AAV9−TertおよびAAV9−空群の両方で、陳旧性梗塞部位および梗塞部位において、増殖の増加(Ki67陽性細胞)を観察した。しかしながら、この増加は、AAV9−空と比較して、これらのマウスにおける線維性瘢痕形成の減少と一致して、AAV9−TERT群で大幅に減じられた。興味深いことに、Ki67および心筋細胞マーカであるトロポニンTを使用した共免疫染色は、空ウイルス群と比較してTERTで治療した群において大幅に増加した、梗塞周辺のKi67陽性心筋細胞の存在を示し、心臓治癒および再生の上昇を示唆した。
MI後の急性心不全からの保護におけるTertの役割をさらに理解するために、我々はまず、異なるマウス群における血清代謝産物の変化を研究した。駆出率の大幅な低下(図2D)に起因して、MIは、腎臓への血流を減少させ、最終的に、尿素血中レベルの上昇をもたらす糸球体濾過量の減少で、腎不全を誘発し得る。我々は、MIの6週間後に、血清尿素レベルがウイルスなしで、または空ベクターで治療したマウスにおいて大幅に上昇し、これは、AAV9−Tert群において著しくレスキューされたことを見出した(図4)。AAV9−Tertで治療したマウスにおけるMI後のより良好な腎機能は、これらのマウスにおける心機能の改善の我々の研究結果を裏付ける(図2)。

- 心筋梗塞と関連した病態の治療において使用するための、テロメラーゼ逆転写酵素(TERT)のコード配列を含む、核酸ベクター。
- 前記心筋梗塞と関連した病態は、心筋梗塞、心筋梗塞に起因する組織の損傷、心筋梗塞に起因する心筋の線維化、および心筋梗塞に起因する心機能の低下からなる群から選択される、請求項1に記載の核酸ベクター。
- 患者は、以前に、心筋梗塞事象を患っている、請求項1または2に記載の核酸ベクター。
- TERTは、配列番号1または配列番号3の配列を含む核酸配列によってコードされる、請求項1〜3のいずれかに記載の核酸ベクター。
- TERTは、配列番号2または配列番号4のアミノ酸配列を含む、請求項1〜4のいずれかに記載の核酸ベクター。
- TERTをコードする前記核酸配列は、前記コード配列の発現を促進する調節配列に作動可能に連結される、請求項1〜5のいずれかに記載の核酸ベクター。
- 前記ベクターは、非組み込みベクターである、請求項1〜6のいずれかに記載の核酸ベクター。
- 前記ベクターは、アデノ随伴ウイルス(adeno−associated virus)系非組み込みベクターである、請求項1〜7のいずれかに記載の核酸ベクター。
- 前記ベクターは、血清型9アデノ随伴ウイルス(AAV9)由来のアデノ随伴ウイルス系ベクターである、請求項1〜8のいずれかに記載の核酸ベクター。
- 前記アデノ随伴ウイルス系ベクターのカプシドは、前記血清型9アデノ随伴ウイルス(AAV9)のカプシドタンパク質からできており、前記カプシド内に含まれる前記核酸配列は、血清型2アデノ随伴ウイルスに対応する内部末端反復配列と両端で隣接している、請求項9に記載の核酸ベクター。
- 前記カプシド内に含まれる前記核酸は、TERTをコードする前記アミノ酸配列をコードする断片を含む、請求項10に記載の核酸ベクター。
