Abstract
Description
本発明は、抗原CD123に特異的な新世代のキメラ抗原受容体(CAR)(多重鎖CARと称される)に関する。このようなCARは、免疫細胞の特異性および反応性を、腫瘍抗原CD123を発現する悪性細胞に再指向させるためのものである。これらのCARを構成するアルファ、ベータおよびガンマポリペプチドは、最適なシグナル伝達を付与する天然受容体により近いフレキシブルなアーキテクチャを形成する膜近傍位置においてアセンブルするように設計される。本発明は、特に免疫療法に使用するための、前記多重鎖CARをコードするポリヌクレオチド、ベクター、および表面においてそれらを発現する単離された細胞を包含する。本発明は、癌、特に急性骨髄性白血病(AML)を処置するための効率的な養子免疫療法戦略への道を開く。
エクスビボで生成された自己の抗原特異的T細胞の移入を含む養子免疫療法は、ウイルス感染および癌を処置するための有望な戦略である。養子免疫療法のために使用されるT細胞は、抗原特異的T細胞の増大または遺伝子操作を通したT細胞の再指向のいずれかによって生成され得る(Park, Rosenberg et al. (2011) Treating Cancer with Genetically Engineered T Cells. Trends Biotechnol. 29(11): 550−557)。ウイルス抗原特異的T細胞の移入は、移植に関連するウイルス感染および稀なウイルス関連悪性疾患の処置のために使用されている、よく確立された手法である。同様に、腫瘍特異的T細胞の単離および移入は、黒色腫の処置において成功が示されている。
したがって、宿主にとってより効率的で低毒性な新たなCD123ターゲティング治療の開発が必要である。

(a)好ましくは細胞培養物由来のまたは血液サンプル由来のT細胞を提供すること;

本発明の好ましい実施形態によれば、抗CD123多重鎖CARを持つ免疫細胞は、化学療法薬(特に、プリンヌクレオチド類似体(PNA))に対して耐性であるように操作され、養子免疫療法および化学療法を組み合わせるための癌処置に適切である。プリンヌクレオチド類似体は、多くの癌処置(特に、白血病)の化学療法組成物に含まれる。それは、AMLにおける標準治療として特に使用されている。最も広く使用されているPNAは、単独または組み合わせのクロファラビン、フルダラビンおよびシタラビンである。PNAは、デオキシシチジンキナーゼ(dCK)活性を有する酵素[EC2.7.1.74]によって、一リン酸、二リン酸および三リン酸PNAに代謝される。それらの三リン酸形態、特にクロファラビン(clorofarabine)三リン酸は、DNA合成でATPと競合し、アポトーシス促進剤として作用し、トリヌクレオチド産生に関与するリボヌクレオチドレダクターゼ(RNR)の強力な阻害剤である。

(d)場合により前記プリン類似体薬物の存在下で、工程c)で得られた人工免疫細胞を増大させること;および
(e)前記抗CD123多重鎖CARを前記免疫細胞に導入すること
の1つまたは複数を含む。
T細胞は、T細胞の遺伝子改変の前であっても後であっても、例えば、米国特許第6,352,694号;米国特許第6,534,055号;米国特許第6,905,680号;米国特許第6,692,964号;米国特許第5,858,358号;米国特許第6,887,466号;米国特許第6,905,681号;米国特許第7,144,575号;米国特許第7,067,318号;米国特許第7,172,869号;米国特許第7,232,566号;米国特許第7,175,843号;米国特許第5,883,223号;米国特許第6,905,874号;米国特許第6,797,514号;米国特許第6,867,041号;および米国特許出願公開第20060121005号に記載されている方法を一般に使用して、活性化させ増大させることができる。T細胞は、インビトロまたはインビボで増大させることができる。
別の実施形態では、以前に記載されているような異なる方法によって得られる単離された細胞または前記単離された細胞に由来する細胞株は、医薬として使用され得る。別の実施形態では、前記医薬は、CD123陽性細胞に関連する病変と診断された患者における癌または感染症を処置するために使用され得る。別の実施形態では、本発明の前記単離された細胞または前記単離された細胞由来の細胞株は、癌、特にAMLの処置のための医薬の製造に使用され得る。
(a)以前に記載されている方法のいずれか1つによって入手可能な免疫細胞を提供すること;
(b)前記形質転換免疫細胞を前記患者に投与すること
の少なくとも1つを含む方法に依拠する。
ドナーは、健常個体または疾患を患っている個体を意味する。
一般的方法
国際特許出願第PCT/EP2015/055848号の文献に開示されているすべての方法は、参照により本明細書に組み込まれる。
フィコール密度勾配媒質(Ficoll Paque PLUS/GE Healthcare Life Sciences)を使用して、EFS(Etablissement Francais du Sang,Paris,France)によって提供されるバフィーコートサンプルから、T細胞を精製した。PBMC層を回収し、市販のT細胞濃縮キット(Stem Cell Technologies)を使用して、T細胞を精製した。20ng/mLヒトIL−2(Miltenyi Biotech)、5%ヒト血清(Sera Laboratories)およびDynabeadsヒトT活性化剤CD3/CD28を補充したX−Vivo(商標)−15培地(Lonza)中、1:1のビーズ:細胞比(Life Technologies)で、精製T細胞を活性化した。活性化後に、細胞を成長させ、20ng/mLヒトIL−2(Miltenyi Biotec)および5%ヒト血清(Sera Laboratories)を補充したX−Vivo(商標)−15培地(Lonza)中で維持した。
最初、MOLM13細胞株は、初期骨髄異形成症候群(MDS、過剰な芽球を伴う不応性貧血、RAEB)後の1995年に再発した急性骨髄性白血病AML FAB M5aを有する20歳の男性の末梢血から樹立された。MOLM13−Luc細胞株およびdckノックアウトMOLM13−Luc細胞株(クロファラビン、フルダラビンまたはシタラビン耐性MOLM13−Luc細胞株)を樹立するために、デオキシシチジンキナーゼ活性(dcK−EC2.7.1.74)を有する酵素を発現する遺伝子(すなわち、ヒトデオキシシチジンキナーゼ遺伝子(NCBI Gene ID:1633))を特異的にターゲティングする低頻度切断エンドヌクレアーゼをコードする核酸配列で、ならびにGFPおよびホタルルシフェラーゼをコードするレンチウイルス(amsbio LVP438−PBS)で、MOLM13細胞(DSMZ ACC 554)をトランスフェクトした。
組換えレンチウイルスベクターによるT細胞の形質導入を、T細胞の精製/活性化の3日後に行った。Vectalys SA(Toulouse,France)によって、HEK−293細胞におけるゲノムおよびヘルパープラスミドのトランスフェクションによって、レンチウイルスベクターを生産した。形質導入当たり106個の細胞を使用して5の感染多重度で、形質導入を行った。ヒトCD123タンパク質の細胞外ドメインとマウスIgG1 Fc断片(LakePharma製)との融合物からなる組換えタンパク質を使用して、T細胞の表面におけるCAR検出を行った。該タンパク質のマウスFc部分をターゲティングするPE結合二次抗体(Jackson Immunoresearch)を用いて、CAR分子に対するこのタンパク質の結合を検出し、フローサイトメトリーによって分析した。
96ウェルプレート(細胞40,000個/ウェル)において、T細胞を、等量のCD123タンパク質発現または非発現細胞と共にインキュベートした。最終容量100μlのX−Vivo(商標)−15培地(Lonza)中で、共培養物を5%CO2、37℃で6時間維持した。細胞刺激の間、共培養の開始時に、1μg/mlの抗CD49d(BD Pharmingen)、1μg/mlの抗CD28(Miltenyi Biotec)および1×Monensin溶液(eBioscience)と共に、蛍光抗CD107a抗体(APC結合、Miltenyi Biotec製)を追加することによって、CD107a染色を行った。6時間のインキュベーション期間後に、細胞を、固定可能な生存色素(eFluor780、eBioscience製)および蛍光色素結合抗CD8(PE結合、Miltenyi Biotec)で染色し、フローサイトメトリーによって分析した。
96ウェルプレート(細胞100,000個/ウェル)において、T細胞を、10,000個の(様々なレベルのCD123を発現する)標的細胞および10,000個のコントロール(CD123neg)細胞と共に同じウェルにおいてインキュベートした。CAR+T細胞(mcCAR+T細胞またはscCAR+T細胞)と共に共培養する前に、標的細胞およびコントロール細胞を蛍光細胞内色素(CFSEまたはCell Trace Violet、Life Technologies製)で標識した。共培養物を5%CO2、37℃で4時間インキュベートした。このインキュベーション期間後に、細胞を、固定可能な生存色素(eFluor780、eBioscience製)で標識し、フローサイトメトリーによって分析した。各細胞集団(標的細胞またはCD123negコントロール細胞)の生存を決定し、特異的細胞溶解の%を計算した。mRNAトランスフェクションの48時間後に、細胞傷害性アッセイを行った。
−動物飼育および実験手順は、実験動物の管理および使用に関するフランスおよび欧州の規制およびNRCガイドにしたがって、Oncodesign(Dijon,France;http://www.oncodesign.com/)が行った。
−それぞれAMLおよびクロファラビン耐性AMLマウスモデルとして、MOLM13−ルシフェラーゼ細胞またはクロファラビン耐性MOLM13−ルシフェラーゼ細胞をマウスに静脈内(iv)注射した。

CD123特異的多重鎖CARの実施例
A.多重鎖CARの設計(図2、図3、図4)
CD123抗原をターゲティングする多重鎖CARを、IgEに対する高親和性受容体(FcεRI)に基づいて設計した。肥満細胞および好塩基球上において発現されるFcεRI(図1)は、アレルギー反応をトリガーする。それは、単一のαサブユニット、単一のβサブユニットおよび2つのジスルフィド結合γサブユニットから構成される四量体複合体である。αサブユニットは、IgE結合ドメインを含有する。βおよびγサブユニットは、シグナル伝達を媒介するITAMを含有する。あらゆる多重鎖CARにおいて、FcRα鎖の細胞外ドメインを削除し、それぞれ表5においてμと称される各scFvで置換し、FcRβ鎖および/またはFcRγ鎖のCD8αヒンジ(配列番号:2)およびITAMを削除した。得られた構造は、表6に詳述されている構造を有していた。
ポリシストロン性mRNAのエレクトロポレーション後に、ヒトT細胞において、多重鎖CARを発現させ得る。
エレクトロポレーションの24時間後に、多重鎖CARをコードするポリシストロン性mRNAを使用して操作したヒトT細胞を、標的(ダウディ)またはコントロール(K562)細胞と共に24時間共培養した。次いで、上清を回収し、TH1/TH2サイトカインサイトメトリービーズアレイキットを使用して分析して、T細胞によって産生されたサイトカインを定量した。アッセイにより、多重鎖CARを発現するヒトT細胞は、コントロール細胞との共培養ではなくCD123発現標的細胞との共培養後に、IFNγ、IL8およびIL5を産生することが示された。
エレクトロポレーションの24時間後に、多重鎖CARをコードするポリシストロン性mRNAを使用して操作したヒトT細胞を、標的(ダウディ)またはコントロール(K562)細胞と共に4時間共培養した。次いで、フローサイトメトリーによって標的細胞を分析して、それらの生存を分析したところ、CD123多重鎖CARを発現する異なる細胞が、コントロール細胞ではなくCD123発現標的細胞を溶解することが示された。
また、2つのCARを使用して、腫瘍樹立マウスモデルにおける抗腫瘍インビボ実験を行った。非形質導入ヒトT細胞の静脈内(iv)注射の7日前に(cdk wtまたはcdk KO)MOLM13−ルシフェラーゼ細胞を、または異なる用量の抗CD123 mcCAR+T細胞(104〜5×106個)(TCR KO薬剤耐性人工細胞)を免疫欠損マウスに静脈内(iv)注射した。結果は、マウスにおける抗CD123 mcCAR+T細胞の用量依存的な抗腫瘍活性を示している。
得られたデータは、TCR KO CD123 CAR T細胞の抗腫瘍活性が、TCR KO CD123 CAR T細胞のものと類似することを示している。これらの細胞で処置した個体では、細胞と比較して、KO CD123 CAR T細胞の寛容性の劇的な改善(TCR発現細胞と比較してGVHDが約50%〜80%減少)が測定されている。
Claims (28)
- −細胞外CD123リガンド結合ドメインに融合された高親和性IgE受容体(FcεRI)のアルファ鎖由来の膜貫通ポリペプチド
を含む、CD123特異的多重鎖キメラ抗原受容体(mc CAR)。 - −シグナル伝達ドメインに融合されたFcεRIのガンマまたはベータ鎖由来の第2の膜貫通ポリペプチド
をさらに含む、請求項1に記載のCD123特異的多重鎖キメラ抗原受容体(mc CAR)。 - −共刺激ドメインを含むFcεRIのガンマまたはベータ鎖由来の第3の膜貫通ポリペプチド
をさらに含む、請求項2に記載のCD123特異的多重鎖キメラ抗原受容体(mc CAR)。 - FcεRIの前記アルファ鎖に融合された前記CD123リガンド結合ドメインが、CD123に対する特異性を付与する重鎖(VH)および軽鎖(VL)を含む一本鎖可変断片(scFv)である、請求項1〜3のいずれか一項に記載のCD123特異的多重鎖キメラ抗原受容体。
- 前記VHが、配列番号:13、15、17、19、21および23から選択される1つと少なくとも90%の同一性を示すポリペプチド配列を含む、請求項4に記載のCD123特異的多重鎖キメラ抗原受容体。
- 前記VLが、配列番号:14、16、18、20、22および24から選択される1つと少なくとも90%の同一性を示すポリペプチドを含む、請求項1に記載のCD123特異的多重鎖キメラ抗原受容体。
- FcεRIの前記アルファ鎖が、CD8α、IgG1またはFcRIIIαタンパク質由来のヒンジによって、前記細胞外リガンド結合ドメインに融合されている、請求項1に記載のCD123特異的多重鎖キメラ抗原受容体。
- 前記ヒンジが、配列番号:2と少なくとも90%の同一性を示すポリペプチド配列を含む、請求項1に記載のCD123特異的多重鎖キメラ抗原受容体。
- FcεRIのガンマまたはベータ鎖に融合された前記シグナル伝達ドメインが、TCRゼータ鎖、FCεRβ鎖、FcεRIγ鎖に由来し、または免疫受容活性化チロシンモチーフ(ITAM)を含む、請求項2〜8のいずれか一項に記載のCD123特異的多重鎖キメラ抗原受容体。
- 前記シグナル伝達ドメインがCD3ゼータに由来する、請求項9に記載のCD123特異的多重鎖キメラ抗原受容体。
- 前記シグナル伝達ドメインが、配列番号:10と少なくとも90%の同一性を示すポリペプチド配列を含む、請求項10に記載のCD123特異的多重鎖キメラ抗原受容体。
- 前記第2または第3のポリペプチドが、CD27、CD28、4−1BB、OX40、CD30、CD40、PD−1、ICOS、リンパ球機能関連抗原−1(LFA−1)、CD2、CD7、CD8、LIGHT、NKG2C、B7−H3、CD83に特異的に結合するリガンド、およびそれらの任意の組み合わせから選択される共刺激分子の細胞質ドメイン由来の共刺激ドメインを含む、請求項3〜11のいずれか一項に記載のCD123特異的多重鎖キメラ抗原受容体。
- 前記共刺激ドメインが4−1BBに由来し、配列番号:6と少なくとも90%の同一性を示すポリペプチド配列を含む、請求項12に記載のCD123特異的多重鎖キメラ抗原受容体。
- 前記共刺激ドメインがCD28に由来し、配列番号:7と少なくとも90%の同一性を示すポリペプチド配列を含む、請求項12に記載のCD123特異的多重鎖キメラ抗原受容体。
- 請求項1に記載のCD123特異的多重鎖キメラ抗原受容体をコードするポリペプチドであって、表6において抗CD123 7G3、抗CD123 Old4、抗CD123 26292、抗CD123 32716、抗CD123 Klon43、抗CD123 12F1と称される全長アミノ酸配列と少なくとも80%の同一性を示すポリペプチド配列を含む、ポリペプチド。

(a)免疫細胞を提供すること;
