本明細書に開示するように、本発明者らは、FLT3−ITD陽性AML細胞および患者試料におけるPRL−3の役割を調べた。本発明者らは、AML細胞におけるFLT3−Src−STAT5シグナル伝達によるPRL−3の制御を記載する。PRL−3発現は、AML患者において、FLT3−ITD突然変異と正に相関した。PRL−3過剰発現は、c−Jun癌原遺伝子の活性化および細胞増殖と関連した。最後に、本発明者らは、AML患者における、PRL−3発現上昇およびより短い全生存の間の臨床的関係を記載し、そしてPRL−3発現上昇をAMLの独立の予後マーカーとして特徴付ける。白血病マウスモデルにおいて、PRL−3ターゲティング免疫療法を用いて、白血病誘発におけるPRL−3の非常に重要な役割が明らかになり、PRL−3がAMLの潜在的な療法ターゲットでありうることが示唆された。

PRL3ポリヌクレオチドの変異体、相同体および誘導体
本文書に記載するヌクレオチド配列に関連する用語「変異体」、「相同体」または「誘導体」には、配列の置換、変動、修飾、交換、配列からのまたは配列への1(またはそれより多い)ヌクレオチドの欠失または付加のいずれも含まれる。ポリヌクレオチドは、非癌性組織におけるPRL3に比較して、一部切除されるかまたは伸長していてもよい。
治療される患者は、任意の動物またはヒトであってもよい。患者は、好ましくは、非ヒト哺乳動物、より好ましくはヒト患者である。患者は男性または女性であってもよい。患者は癌を有してもよいし、または有すると推測されてもよい。

無進行生存時間は、疾患が悪くならない時間の長さである。
診断は、疾患、例えば癌の同定を指す。本明細書に記載する方法を用いて、癌を検出することも可能である。これらを用いて、特定の癌のサブタイプまたはサブクラスを診断してもよい。
本明細書に開示する方法は、PRL3の検出および/または定量化を伴う。本明細書において、検出は、定量化を伴わないPRL3の測定を指す。PRL3ヌクレオチドおよびタンパク質の検出および定量化のための方法は、当該技術分野に周知であり、そして当業者によって容易に認識されるであろう。

癌または白血病は、PRL3発現癌または白血病である。すなわちPRL3陽性癌または白血病である。
白血病は、芽細胞と呼ばれる未成熟白血球の異常な増加によって特徴付けられる血液または骨髄の癌のタイプである。白血病の治療は、化学療法、医学的放射線療法、またはホルモン治療を伴う。

本明細書に記載するのは、白血病の治療法および抗PRL3抗体を用いた治療法を含む治療法である。抗PRL3剤および抗PRL3抗体を含む、こうした方法で使用するための剤もまた、癌のための薬剤製造におけるこうした剤の使用とともに開示する。
本発明の側面にしたがった薬剤および薬学的組成物を、限定されるわけではないが、非経口、静脈内、動脈内、筋内、腫瘍内、口腔および鼻を含むいくつかの経路による投与のために配合することも可能である。薬剤および組成物を液体または固体型で配合してもよい。液体配合物を、ヒトまたは動物の身体の選択した領域への注射による投与のために配合してもよい。
PRL3に結合するであろう抗体がすでに知られている。例えば、Jie Liら、2005に開示される通りである。
細胞株および初代患者試料
TF−1およびMV4−11細胞をATCC(アメリカンタイプカルチャーコレクション、ヴァージニア州マナサス)から購入した。MOLM−14細胞株を自社で得た。熱不活化10%ウシ胎児血清(Hyclone Laboratories, Inc.、ユタ州ローガン)を補充し、そして2ng/mlヒトIL−3(R&D System Inc.、ミネソタ州ミネアポリス)を補充したRPMI 1640(Invitrogen、カリフォルニア州カールスバッド)中で、TF−1細胞を培養した。TF1−ITDおよびTF1−PRL−3細胞を先に記載されるように調製した(Kimら、2005;Zhouら、2011)。シンガポール国立大学病院の新規に診断されたAML患者から、書面のインフォームドコンセントとともに、骨髄(BM)芽細胞を得た。この研究は、シンガポール国立大学施設内倫理委員会(IRB)によって認可された。
FLT3阻害剤(PKC412)、MEK阻害剤(U0126)、p38 MAPK阻害剤(SB203580)、およびJNK阻害剤(SP600125)をLC Laboratories(マサチューセッツ州ウォバーン)から購入した。FLT3阻害剤(CEP−701)およびSrcキナーゼ阻害剤(SU6656およびPP2)を、Sigma(ミズーリ州セントルイス)より購入した。FLT3、STAT5、JAK2、Src、ERK、c−Jun、pFLT3、pJAK2、pSTAT5、pSrcおよびp−c−Junを、Cell Signalling Technologies(マサチューセッツ州ビバリー)より購入した。GAPDH抗体をMillipore(マサチューセッツ州ビレリカ)より得た。抗CD45−APCおよびLightShift化学発光EMSAキットを、Pierce Biotechnology, Inc.(イリノイ州ロックフォード)より得た。マウス抗PRL3抗体は、以前報告されたようにハイブリドーマクローン318より得た(Liら、2005)。分泌アルカリホスファターゼ(SEAP)レポーターアッセイキットをClontech(カリフォルニア州パロアルト)より、そしてPhospha−LightTMをApplied Biosystems(マサチューセッツ州ベッドフォード)より購入した。
製造者の指示にしたがって、RNeasyミニキット(Qiagen、カリフォルニア州チャツワース)を用いて、AML患者の骨髄細胞から総RNAを抽出した。逆転写酵素III(Invitrogen、カリフォルニア州カールスバッド)によって総RNAからcDNAを合成し、そして以前記載されるように、PCRによって増幅した(Quentmeierら、2003)。RT−PCRのプライマーセットを要約した;FLT3−ITD、5’−GCAATTTAGGTATGAAAGCCAGC−3’および5’−CTTTCAGCATTTTGACGGCAACC−3’、PRL−3、5’−GGGACTTCTCAGGTCGTGTC−3’および5’−AGCCCCGTACTTCTTCAGGT−3’、そしてβ−アクチン遺伝子は5’−GTGGGGCGCCCCAGGCACCA−3’および5’−CTCCTTAATGTCACGCACGATTTC−3’であった。PCR産物を5%ポリアクリルアミドゲル上で分析し、エチジウムブロミドで染色し、そして次いで、GelDoc画像化装置(BioRad Inc.、カリフォルニア州ハーキュルス)で視覚化した。

AP−1レポーターベクター(AP1−SEAP)をClontechから購入した。TF1−GFP細胞およびTF1−PRL−3細胞を200ngのAP1−SEAPベクターでトランスフェクションし、そして24時間インキュベーションした。培養上清を収集し、65℃で30分間加熱し、そして以下のようにアルカリホスファターゼ活性に関してアッセイした; 30μlの上清を120μlのアッセイ緩衝液と5分間インキュベーションし、その時点で、1:20希釈したCSPD基質を添加し、そしてTECANマイクロプレート読み取り装置(Maennedorf、スイス)上で試料を読み取った。
AML細胞(1x104)を、100μl増殖培地中、96ウェル組織培養プレートの各ウェル内に植え付け、そしてCellTiter96水性細胞増殖アッセイキット(Promega)を用いて、72時間、異なる阻害剤とともに植え付けた後、生存細胞を測定した。簡潔には、20μl MTS混合物のアリコットを、アッセイの示す時間で添加し、そして37℃で2時間反応を行った。そしてTECANマイクロプレート読み取り装置を用いて、490nmの波長で、吸光度を読み取った。細胞数および各細胞株由来の吸光度の間の直線関係を確立した後、獲得された吸光度を細胞数に変換した。
ヨウ化プロピジウム(PI)で染色し、そしてフローサイトメトリー(BDLSR11、Becton Dickinson、カリフォルニア州サンノゼ)によって分析することによって、培養細胞のDNA含量を定量化した。簡潔には、細胞をPBSで採取し、そして70%冷エタノールを用いて4℃で30分間固定した。細胞をPBSで洗浄し、そして次いで、500μのPI染色溶液中に再懸濁し、そして室温で30分間インキュベーションした。次いで、細胞周期に関して試料を調べ、そして分析した(Modfit LT2.0、Becton Dickinson)。
サイトカイン補充なしに48時間培養した後、TF1−GFPおよびTF1−PRL−3細胞を、PBSで採取した。細胞をPBSで2回洗浄し、そしてアネキシン−Vおよび7−AAD染色溶液と室温で30分間インキュベーションした。染色後、細胞をFACS分析に供した。
PRL−3ノックダウン細胞株を生成するため、pRS−PRL−3−shRNA(OriGene Technologies、メリーランド州ロックビル)をTF1−ITD細胞内にトランスフェクションした。生じたPRL−3−KD細胞株を、ピューロマイシンで選択し、そしてウェスタンブロット分析によって確認した。TF1−ITD PRL−3−KD細胞を、マウス注射のために用いた。
先に記載されるような(Guoら、2008)すべての動物研究は、施設動物飼育使用委員会(IACUC)によって認可されてきている。本発明者らは、シンガポール科学技術研究所動物施設センター(A* STAR)のポリシーにしたがった。生物資源センター(A* STAR、シンガポール)からBalb/cヌードマウスを得た。ヌードマウスに、1x106 TF1−ITD細胞を静脈内注射した。3日後、マウスをランダムに3つの処置群に分け;IgG(偽処置、n=11)、PRL−3 mAb(PRL−3処置、n=11)、およびFLT3 mAb(FLT3処置、n=11)を週2回注射した。生存研究のため、IgG処置(n=7)、またはPRL−3 mAb処置(n=7)したマウスを用い、そして毎日観察した。TF−1細胞を注射したマウスを、TF1−ITD細胞を注射したマウスの対照として用いた。臓器を単離し、そして実験の最後に、巨視的転移に関して調べた。(Guoら、2008)。
ヌードマウスに、1x106 TF−1、TF1−ITD、またはTF1−ITD PRL−3 KD細胞を静脈内注射した。TF1−ITD細胞を注射したマウスを2群に分け、そして対照IgGまたはPRL−3 mAbで週2回処置した。実験終了時、骨髄細胞を単離し、そしてヒト特異的造血細胞表面マーカー、CD45−APC抗体(Pierce、Thermo Scientific)で染色し、そしてフローサイトメトリーによって分析した。
本研究に用いたすべてのヒトAML患者のデータセットの詳細を以下に要約した。総数5つの独立のAML患者データセットを分析した:1)英国ベルファストのAffymetrix U133Plus2アレイ上で分析した本発明者らの未公表データセット(コホート1); 2) GEOアクセス可能GSE1159データセット(Valkら、2004); 3) GEOアクセス可能GSE6891データセット(Verhaakら、2009); 4) GEOアクセス可能GSE15434データセット(Kohlmannら、2010);および5) GEOアクセス可能GSE12417データセット(Metzelerら、2008)。図10は、各データセットにおいて利用可能な情報、および生データを用いたこの報告中の図の番号を示す。規準化のため、RおよびBioconductorを用いて、データセットを前プロセシングした。PRL−3高レベルおよび低レベルを区別するため、カットオフポイントとして中央値を用いた(2つのPRL−3プローブ; 206574および209695の平均)。SPSS19.0(IBM、シンガポール)を用いて、統計分析を行った。Fisherの直接検定、または適用可能な場合、カイ二乗検定によって、PRL−3発現およびFLT3−ITD突然変異状態の間の相関を分析した。PRL−3発現および生存時間の間の関連を、ログランク検定によって比較されるカプラン−メイヤー分析によって分析した。p<0.05を有意と見なした。p>0.05の除去限界を伴う後ろ向き条件付段階的選択を含むコックス回帰分析を行って、AML患者生存に関する独立の予測因子を同定した。
PRL−3過剰発現およびFLT3−ITD突然変異の間の相関を調べるため、FLT3−ITD突然変異を持つまたは持たないAML患者由来の19の骨髄試料を分析した。AMLにおけるPRL−3上方制御の発生率は、FLT3−ITD突然変異を持たない場合の12症例のうち3例(25%)に比較して、FLT3−ITD突然変異と有意に関連する(7つの症例のうち5つ、71.4%)ことが見出された(Fisherの直接検定、p<0.05;図1A)。同様に、2つのFLT3−ITD陽性細胞株(MOLM−14およびMV4−11)において、高いPRL−3発現が観察された(図1A)。同じ患者由来のPRL−3 mRNAの定量的リアルタイムPCR分析によって、より高いPRL−3 mRNA発現は、FLT3−ITD突然変異を有するAML患者と関連することを裏付けた(図11)。この知見を拡張して、本発明者らは、総数221人のAML患者からなる、本発明者らの未公開ベルファスト/MILEデータセット(コホート1)を分析した。このうち、正常核型を持つ101人の患者を用いて、PRL−3発現レベルおよびFLT3−ITD突然変異状態の間の関係を分析した。FLT3−ITD陰性AML患者のわずか10%が、「非常に高い」PRL−3を発現し(カイ二乗検定、p<0.001)、一方、40%のFLT3−ITD陽性患者は、「非常に高い」PRL−3(黒いブロック、図1B、a)を発現した。本発明者らの観察は、PRL−3発現が、3つの独立のデータセットにおいて、FLT3−ITD突然変異に関して陰性である患者に比較して、FLT3−ITD突然変異に関して陽性であるAML患者において、有意により高いことが観察される、3つの独立の公的に入手可能なAML患者データベース(GSE1159 n = 285、GSE6891 n=521、およびGSE15434 n=251)においてさらに実証された(図1B、b〜d; p<0.001)。要約すると、4つの別個のAML患者のコホートの本発明者らの分析は、総数1158人のAML患者において、FLT3−ITD突然変異および高いPRL−3発現の間の強い関連を示す。
恒常的に活性であるFLT3シグナル伝達が、PRL−3発現の上方制御に関与しているかどうかを調べるため、本発明者らは、FLT3阻害剤を用いて、FLT3受容体活性を遮断し、そしてFLT3−ITD突然変異の下流シグナル伝達分子を調べた。STAT5は、FLT3−ITDの非常に重要な下流ターゲットであることが知られているため(Mizukiら、2000)、本発明者らは、FLT3特異的阻害剤; PKC412またはCEP−701(Odgerelら、2007; Smithら、2004)での処理後、STAT5発現レベルを試験した。それぞれの阻害剤は、TF1−ITDおよびMOLM−14細胞株において、用量依存方式で、FLT3およびSTAT5のリン酸化を減少させ、そしてPRL−3タンパク質レベルの対応する減少を生じた(図2A)。本発明者らは、次に、FLT3−ITD誘導性PRL−3発現が、STAT5の2つの別個の上流活性化因子であるJAKまたはSrc(Robinsonら、2005; Spiekermannら、2003)によって仲介されうるかどうかを調べた。FLT3阻害剤での処理後、ホスホおよび総JAK2レベルは影響を受けておらず(図2B)、一方、Srcの活性化型(pSrc Y416)は、処理後、強力に下方制御された。重要なことに、Src不活性化は、用量依存方式で、STAT5リン酸化の減少に緊密に対応した(図2B)。FLT3−ITD陽性 AML細胞における、Src仲介性STAT5リン酸化の役割を調べるため、AML細胞を2つの別個のSrcキナーゼ阻害剤、SU6656およびPP2(Blakeら、2000; Namら、2002)で処理した。Src阻害は、STAT5リン酸化およびPRL−3発現レベルの両方を減少させ(図2C)、Src仲介性STAT5リン酸化およびPRL−3発現の間の相関を明らかにした。
PRL−3がどのように上方制御されうるかを理解するため、ヒトPRL−3プロモーター領域を転写因子データベース(TRANSFAC)によって分析して、ありうる転写因子結合部位を予測した(Wingenderら、1996)。TRANSFAC プログラムは、PRL−3の上流プロモーター領域にいくつかの推定上の転写因子を同定し、これには、2つのSTAT5コンセンサス結合配列、TTCN(3)GAA(Seidelら、1995)が含まれた(図3A)。PRL−3の転写制御因子としてのSTAT5の役割を評価するため、本発明者らは、これらのSTAT5結合配列に対応する2つのビオチン化プローブ、S1およびS2を設計し、そしてTF−1(PRL−3非発現)またはTF1−ITD(PRL−3発現)細胞のいずれかに由来する核抽出物を用いて、ゲル移動度シフトアッセイ(EMSA)を行った(図1C、レーン1、4)。TF1−ITD細胞由来の核抽出物は、プローブS1(−5.4kb)に対するロバストなレベルの特異的なDNA結合活性を示したが、プローブS2(−18.4kb)に対しては示さず、一方、親TF−1細胞由来の核抽出物は、プローブS1またはS2との観察可能なDNA結合活性を持たなかった(図3B)。STAT5結合配列を含有する非標識競合オリゴヌクレオチドは、結合シフトアッセイ中、標識プローブを効率的に置換可能であった(図3C)。このタンパク質/DNA複合体におけるSTAT5の関与をさらに確証するために、ビオチン化プローブS1を用いて、ストレプトアビジン−アガロース・プルダウンアッセイを行った。EMSA結果と一致して、STAT5抗体を用いたウェスタンブロット分析は、STAT5が、複合体において、プローブS1に結合する転写因子であることを確認した(図3D)。
PRL−3は、腫瘍発生において、重要な役割を果たすことが報告されてきている(Guoら、2006; Matsukawaら、2010)。したがって、本発明者らは、多様な発癌性転写因子、例えば腫瘍発生を駆動する周知の転写因子であるAP−1に対するPRL−3過剰発現の分子的結果を調べた(Eferl & Wagner、2003)。このため、本発明者らは、TF1−PRL−3(GFP−PRL−3を過剰発現するTF−1細胞)およびTF1−GFP対照細胞を用いて、AP−1プロモーターの制御下でSEAPレポーター遺伝子を含有するpAP1−SEAPベクターを用いた、SEAP(分泌アルカリホスファターゼ)アッセイを行った。図4Aに示すように、TF1−PRL−3細胞は、TF1−GFP対照細胞に比較した際、SEAP活性に>2.5倍の増加を示し、PRL−3がAP−1発現を誘導可能であることを示した。TF−1白血病細胞株からのこの観察が、他の固形腫瘍細胞株に適用可能であるかどうかを調べるため、2つの結腸直腸癌細胞株、DLD−1およびHCT116を調べた。一貫して、PRL−3の過剰発現は、それぞれ、DLD−1細胞およびHCT116細胞において、それぞれ、AP−1活性の>6.5倍および>2.5倍の増加を導いた(図13)。
PRL−3過剰発現の生物学的転帰を調べるため、TF−1細胞におけるPRL−3機能の獲得を調べた。TF−1は、細胞増殖および生存を維持するために、培地においてIL−3またはGM−CSFなどのサイトカインの補充を必要とする、サイトカイン依存性細胞株である(Linら、2007)。サイトカインなしでは、TF−1−GFPベクター対照細胞は、劣ってしか増殖せず(図5A)、そして48時間の時点で、22.8%のサブG1アポトーシス集団を示した(図5B、左パネル)。しかし、PRL−3を過剰発現しているTF−1細胞(TF1−PRL−3細胞)は、細胞増殖に関してサイトカイン非依存性となり、そして細胞数は、同じ時点で、TF1−GFP対照細胞のほぼ2倍に増加した(図5A)。さらに、図5Bに提示するように、TF1−PRL−3細胞は、サイトカイン補充が欠如しているにもかかわらず、はるかにより少ないサブG1アポトーシス集団を有した(1.7%、図5B、右パネル)。サイトカイン補充の非存在下でのPRL−3の抗アポトーシス活性を研究するため、本発明者らは、TF1−GFP対TF1−PRL−3細胞株に対して、アネキシン−Vおよび7−アミノアクチノマイシンD(7−AAD)染色後、蛍光活性化細胞ソーティング(FACS)分析を行った。サイトカイン補充を伴わず48時間培養した後、TF1−PRL−3細胞(6.8%)におけるより、TF1−GFP細胞において、より多くのアポトーシス集団(31%)が観察され(図5C)、PRL−3が、抗アポトーシス性の役割を果たし、そしてTF1−PRL−3 AML細胞において細胞増殖を維持しうることが示唆された。
AML細胞株におけるPRL−3機能の喪失を調べるため、本発明者らは、内因性FLT3−ITDおよびPRL−3の両方を高発現する2つのサイトカイン非依存性細胞株(MOLM−14およびMV4−11)において、PRL−3をノックダウンした(図1C)。PRL−3の枯渇後、多様な時点で細胞生存度を評価した(図6A a、B a)。興味深いことに、siRNAによってPRL−3をサイレンシングすると、48時間で、偽ノックダウン細胞に比較して、MOLM−14細胞において〜64.5%、そしてMV4−11細胞において〜66.7%の細胞数減少を生じた。さらに、細胞周期分析によって、PRL−3除去細胞における細胞数減少は、MOLM−14およびMV4−11細胞株において、G1増加およびS期集団減少(↑G1/S↓)と相関することが示された。G1/S集団の比は、MOLM−14偽ノックダウン細胞において46.6%/41.1%であり、そしてMOLM−14 PRL−3 KD細胞において69.7%/21.6%になった(図6A、b)。同様に、MV4−11偽ノックダウン細胞におけるG1/S集団の比は、MV4−11 PRL−3 KD細胞では、51.4%/36.8%から80.5%/14.6%にシフトした(図6B、b)。したがって、PRL−3の枯渇は、細胞がG1からS期に進入するのを遅延させ、PRL−3が、MOLMおよびMV4−11細胞株の両方において、G1からS期の遷移を促進する際に役割を果たし、細胞増殖を促進する可能性を暗示する。しかし、PRL−3の枯渇は、細胞周期分析において提示されるように、アポトーシスに影響を及ぼさなかった(図6A b、B b)。結果は、PRL−3ノックダウン後、サブG1細胞集団の観察可能な増分がないことを示した。これはさらに、アネキシン−Vおよび7−AAD染色を用いたアポトーシス分析によって確認された。FACS分析は、siRNAによるPRL−3のサイレンシングが、両細胞株において、アポトーシス集団の実質的な増分を示さないことを示した。これらの結果は、MOLM−14およびMV4−11サイトカイン非依存性細胞において、PRL−3の役割が、主に、G1−S遷移を促進する際のものであることを示す。
本発明者らの結果は、これまでのところ、FLT3およびPRL−3が、相乗的にAML細胞増殖を駆動可能であることを示した。FLT3阻害剤を用いた臨床試験が、一次または二次薬剤耐性(Wiernik、2010)およびAML薬剤耐性におけるPRL−3の関与(Zhouら、2011)を示してきていることを考慮すると、本発明者らは、本明細書において、PRL−3(細胞内ホスファターゼ)発現AML細胞を、PRL−3抗体を用いてターゲティングすることによる、代替戦略の開発を試みた。本発明者らおよび他の研究者らは、抗癌免疫療法のための細胞内腫瘍性タンパク質に対する抗体療法の実現可能性を立証してきている(Daoら、2013; Guoら、2011; Guoら、2012)。PRL−3のin vitroの役割が、in vivoでのFLT3−ITD駆動AML腫瘍負荷と相関するかどうかを確かめるため、本発明者らは、AML細胞の側方尾静脈注射を用いた白血病マウスモデルを開発した。PRL−3モノクローナル抗体(mAb)(Li Jieら、2005)を続いて用いて、PRL−3発現が上昇したTF1−ITD AML細胞をターゲティングした(図1C、レーン4)。TF1−ITD細胞を注射したBalb/cヌードマウスを3つの処置群に分けた: 1. IgG抗体偽処置(IgG処置、n=11); 2. PRL−3 mAb(PRL−3 mAb処置、n=11);または3. FLT3 mAb(FLT3 mAb処置、n=11)。12〜14日に渡って、IgG、PRL−3またはFLT3 mAbを週2回投与した後、PRL−3 mAb処置マウスは、肝臓および脾臓サイズの有意な減少を示し(図7A、a)、これは腫瘍負荷の減少の指標である。肝臓および脾臓重量が、未処置(IgG対照)群の、それぞれ、72.8%および59.3%に減少した(p<0.001、図7A、b)。注目すべきことに、PRL−3 mAb処置は、FLT3 mAb処置と類似の結果を生じた(図7A a、b)。以前、FLT3 mAb処置は、FLT3白血病マウスモデルにおいて、有効性を持つことが立証されている(Liら、2004)。本明細書の結果は、FLT3−ITD駆動性AML進行において、PRL−3の役割を実証し、そして以前調べた他のPRL−3陽性癌タイプに加えて、PRL−3陽性AML患者を治療して、PRL−3抗体療法の新規使用を示す(Guoら、2012)。
コホート1(n=221)および公的に入手可能なデータセットGSE12417(n=163)(Metzelerら、2008)を用いて、AML患者におけるPRL−3遺伝子発現および全生存の間の相関を分析した。単変数コックス回帰分析によって、高レベルのPRL−3発現は、コホート1(HR=1.327、95% CI=1.057〜1.664、p=0.015)およびGSE12417コホート(HR=1.81、95% CI=1.20〜2.74、p=0.005)の両方において、より短い生存と関連した。本発明者らは、PRL−3の予後値が、コホート1中、正常核型を有するAML患者(n=101)(HR=1.576、95% CI=1.151〜2.156、p=0.005)において、細胞遺伝合併症を有するAML患者(n=120)(HR=1.298、95% CI=0.927〜1.818、p=0.129)におけるよりも、高いことに注目した。本発明者らはしたがって、以下の3つのコホートを用いたカプラン−メイヤー生存分析によって、正常核型を持つこうした患者におけるPRL−3および生存の間の関係に焦点をあてた: 1. コホート1(n=101)、2. GSE 6891(n=227)、および3. GSE12417(n=163)。コホート1においては、高レベルのPRL−3発現は、正常核型を持つ患者(ログランク検定、n=101、p=0.028;図8A)における低いPRL−3発現レベルを持つ患者(平均生存時間=60ヶ月、95% CI=39〜81ヶ月)に比較して、より短い生存と有意に関連した(平均生存期間=26ヶ月、95% CI=13〜40ヶ月)。したがって、他の2つの独立のコホート、GSE6891およびGSE12417において、正常核型を持つAML患者のカプラン−メイヤー生存分析もまた、高レベルのPRL−3 mRNA発現がより短い生存時間と有意に関連することを明らかにした(それぞれ、p<0.001およびp=0.025;図8B〜C)。総合すると、これらの結果は、PRL−3発現が正常核型を有するAML患者において、より劣った全生存と関連することを示唆する。
PRL−3がAML患者における生存に関する独立の予後マーカーであるかどうかを評価するため、性別、年齢、細胞遺伝リスク群、核型、FAB群、FLT3突然変異状態、NPM突然変異状態、およびPRL−3 mRNA発現を含むパラメータを用いて、コホート1(n=221)において、多変数コックス回帰を行った(図9)。重要なことに、高PRL−3 mRNA発現(p=0.001、HR=1.577、95% CI=1.199〜2.073)を、年齢(p<0.001)、細胞遺伝リスク群(中程度、p=0.001;有害、p<0.001)およびFLT3−ITD突然変異(p=0.001、赤)に加えて、患者生存に関する独立予測因子として同定した。一貫して、同様の多変数分析を用いたGSE6891データセット(n=521)(Verhaakら、2009)の検査は、PRL−3高発現またはFLT3−ITD突然変異が、患者生存に関する独立の予測因子であることを立証した。このデータセットにおいて、本発明者らは、高レベルのPRL−3発現(HR=1.488、95% CI=1.194〜1.855、p<0.001)またはFLT3−ITD突然変異(HR=1.389、95% CI=1.094〜1.764、p=0.007)のみが、患者生存に関する独立の予測因子であることを示すことを見出した。別個のデータセットに由来するこれらの一貫した結果は、総合的に、PRL−3の高レベル発現が劣った生存と関連することを示し、そしてALM患者に関する他の既知の臨床的に適切な予後マーカーとは独立に、重要でそして新規の予後マーカーとしてのPRL−3発現レベルを強調する。
本研究において、本発明者らは3つの主な知見を提示する:1)AML細胞増殖をin vitroで促進する際のPRL−3過剰発現の分子機構、2)動物モデルにおける腫瘍負荷を減少させるための、PRL−3発現AML細胞をターゲティングする非慣用的な療法として、PRL−3抗体を用いる新規アプローチ、ならびに3)AML患者における高PRL−3発現および劣った生存の間の臨床的関連。総合すると、本発明者らの知見は、PRL−3が、FLT3−ITD突然変異を有するAML患者に関して、劣った生存を予測する、推定上の新規療法ターゲットおよび予後マーカーでありうることを示唆する。
Claims (31)
- 癌を有する個体の予後を決定するための方法であって、個体から得た試料におけるPRL3の発現、量または活性の調節を検出する工程を含む、前記方法。
- 個体の生存率を予測するための方法である、請求項1記載の方法。
