Abstract

難聴は、伝音性又は感音性のいずれかのタイプに分類され、伝音難聴及び感音難聴の両方を同一の耳が併せ持っている場合は一般的に混合難聴との名称が用いられる。伝音難聴は、音が外耳から中耳に効果的に伝達されない機械的な原因による場合が多い。判別力は殆ど影響されず、殆どの場合は手術又は正確に処方された増幅によって解決することができる。
(難聴検査)
患者の難聴レベルは、オージオグラムを用いて測定することができる。図1に示されているオージオグラムは、純音聴力検査(PTA)、自動式聴力検査又はコンピュータ式聴力検査の結果を示すグラフである。これは、難聴のタイプ、程度及び形態を示すものである。
(耳鳴りの測定)
耳鳴りマッチ(TM)、耳鳴りラウドネスマッチ(TLM)及び最小マスキングレベル(MML)を含む音響心理評価は、対側耳、又は、両耳に差がない場合は両側耳における耳鳴りの周波数レベル及び耳鳴り周波数の強度レベルを判定することによって決まる。両者の測定はそれぞれ、ヘルツ(Hz)及びデジベル(dB)で表される。MMLを行う場合は、広帯域ノイズを採用し、それぞれの患者に質問して耳鳴りが聞こえなかったときを判定する。MML及びTLMは1dBのステップを用いて判定される。
(逆オージオグラムフィルタ)
耳鳴り患者が経験する錯覚音の高低はしばしば、難聴周波数に相関している(すなわち、難聴ディップ:4kHzで30dB、マッチする耳鳴り:4kHzで20dBである)。本発明の一側面では、蝸牛及び聴覚神経の損失帯域における残りの帯域幅は、患者の難聴によって特徴付けられた帯域ブースト(有色)ノイズを用いた増幅を行うことによって刺激される。具体的には、患者の(オージオグラムによって特徴付けられる)難聴を利用して、カスタマイズされた帯域ブーストノイズの生成に利用される所定の補正プロフィールを作成する。一実施形態では、所定の補正プロフィールを利用して、「逆オージオグラム」による帯域ブーストフィルタを作成し、この帯域ブーストフィルタを帯域ブーストノイズの生成に利用する。ここで、この工程について、3個の例示的な例を参照しながら説明する。
実施例1では、難聴及び/又は耳鳴りの疑いのある患者1に対して、純音聴力検査を行い、聴力のタイプ、程度及び形態を(先述したように)定量化した。耳鳴りマッチ(TM)、耳鳴りラウドネスマッチ(TLM)及び最小マスキングレベル(MML)を含む心理音響評価を(先述したように)行った。

(実施例4)
本発明の実施形態に従った方法及び装置を、持続する難治性耳鳴りの治療及び症状緩和のために使用する効果についての臨床パイロット研究を行った。研究結果を以下に説明する。
(題材及び方法)
研究の目的は、持続する難治性耳鳴りの自覚的及び他覚的な評価に対する、聴覚及び触覚による多モードの神経調節の効果を判定することであった。これは、16週にわたる研究であった。被験者はスクリーニングを4週間、治療を10週間を受け、さらに治療後2週間の追跡検査を受けた。この試験は、4週間の導入期間のベースライン数値を定め、10週間にわたる治療の結果をベースライン数値と比較して、試験期間中の装置の使用及び耐性を評価することを目的としたものであった。
(不適任性及び辞退)
この特定の研究に加わるには適任でないとプレスクリーンで見なされた参加者は、かかりつけ医(GP)に回されて正式な断り状を受け取る。本研究の開始後に中途辞退する被験者については、治療企図(ITT)法によって分析した。患者は、研究への参加は完全に自主的であり、理由を言うことなく何時でも研究を自由に中途辞退することができた。募集工程は、患者に参加を熟考するための十分な時間を与えた。
(偏見の最小化)
無記名の参加や、他覚的及び自覚的なゴールド標準の結果測定を用いることによって、偏見を最小化した。

前治療段階は、治療の開始前の4週間の導入期で構成された。この段階では、2週間毎、すなわち、0週、2週及び4週にベースライン測定を行いサンプリングした。治療段階は、10週間の期間で構成された。この段階では、被験者は、自宅で、推奨されたとおり1日60分間装置を使用した。全ての被験者について、装置の使用のログが埋め込みSDリーダカードに残された。先述したように、他覚的及び自覚的な試験を、エンロールされた検査において、2週間毎に研究期間にわたって行った。
(結果測定の評価及びコンプライアンス)
重要な結果測定は、研究期間を通して臨床環境における「レビュー」検査で評価された。ログ技法や、アンケートによって研究終了時に評価された許容度を用いて、被験者のコンプライアンスを測定した。
(自覚的結果測定)
耳鳴り障害目録(THI)は、耳鳴りの測定のための25項目の問診表である。被験者は、2週間毎及びレビュー検査直前にTHI問診表に記入した。THIのスコアは「改善」から「悪化」まで5段階の重症度に分類される。
(他覚的結果測定)
耳鳴りマッチ(TM)は、耳鳴りの周波数の高低を判定する音響心理評価である。耳鳴りラウドネスマッチ(TLM)は、耳鳴りの強度を判定する音響心理評価である。最小マスキングレベル(MML)は、耳鳴り[45]をマスキングするために必要な最小レベルのノイズを判定する音響心理評価である。TM、TLM及びMMLによる評価を2週間毎にレビュー検査で患者に行った。
(器具)
本研究では、本発明の実施形態に従った聴覚刺激装置及び触覚刺激装置を使用した。この非侵襲性の装置は、ハイファイヘッドホンを通じて聴覚刺激を耳に伝達すると同時に、32個の経皮的電気刺激器のアレイを通じて触覚パターンを舌に伝達することができる。
(結果及び分析)
(分析人口及びコンプライアンス)
治療企図(ITT)に基づく人口から得られたデータに対して統計分析を行った。被験者のデータは、以下のコンプライアンス及び最低適用条件が満たされた場合に適格であるものと見なされた:1日最低30分間又は毎週3.5時間の合計使用量;ゼロ超の最小レベルの刺激;予定の日時から1週間以内のレビュー検査。
(人口統計およびベースライン特性)
前治療段階において、ベースライン測定及び基本人口統計データ(年齢/性別)を得た。
(難聴プロフィール)
BS EN 60645−1(IEC 60645−1)及び関連のBS EN ISO 389(ISO 389)シリーズ規格に従って較正したGN Otometrics Madsen社のAstera臨床オージオメータを用いて、左耳及び右耳の難聴プロフィールをそれぞれ測定した。難聴を、正常、軽度、軽度〜軽中度、軽中度、軽中度〜高度、高度の重症度に分類した。重症度の分布を以下の表に要約した。
(耳鳴りプロフィール)
スクリーニングではTHI、MML、TLM及びTMのスコアを用いて患者の耳鳴りプロフィールを測定した。サマリ統計を以下の表に示す。
THI、MML、TLM及びTMのスコアの経時的変化を測定することによって、持続する難治性耳鳴りの他覚的及び自覚的な評価に対する、聴覚及び触覚による多モードの神経調節の効果を判定した。スコアは、スクリーニングV0、ベースラインV2、そして検査期間の間2週間毎に得た。ベースラインV2(4週/治療1週目)及びV7(14週/治療10週目)の主効果の比較と、ベースラインV2及びV4(8週/治療4週目)の暫定効果の比較とを行った。被験者がまだ治療を受けていないスクリーニング検査V0及びベースラインV2の比較によって、プラセボ/文脈効果を調べた。治療の最終週V7(14週/治療10週目)及びV8(16週/治療後2週目)の比較によって、治療の短期的な効果を測定した。
(最小マスキングレベル[MML])
MMLのスコアの経時的変化を図7に示した。繰り返しANOVA全体が統計学的に有意であった(p値<0.001)。また、対t検定によるベースラインV0及びV7(14週/治療10週目)の比較も有意であった(p値<0.001)。MMLスコアは、ベースラインV2の平均値47.4(SD[標準偏差]=2.54、95%Cl[95%信頼限界]:42.3〜52.6、N[数]=39)からV7(14週/治療10週目)の38.8(SD=2.7、95%CI:33.4〜43.34、N=39)に低下した。また、暫定効果(ベースラインV2からV4[8週/治療4週目]までの間のMMLの平均変化)も有意であり(p値=0.0088)、ベースラインV0の48.15(SD=2.69、95%CI:42.66〜53.64、N=33)からV4(8週)の43.79(SD=3.13、95%CI:37.4〜50.16、N=33)まで低下した。MMLスコアにはプラセボ/文脈効果のいくつかのエビデンスがあったが、有意ではなかった(p値=0.01)。スクリーニング検査V0及びベースラインV2の間で、平均MMLスコアはV0の50.8(SD=2.3)からV2の46.7(SD=2.2)(N=54)に変化した。
(耳鳴りラウドネスマッチ[TML])
TLMスコアの経時変化を図8に示す。繰り返しANOVA全体は統計的に有意であった(p値<0.001)。対t検定によるベースラインV2及びV7(14週/治療10週目)の比較も有意であった(p値=0.001)。TLMスコアは、ベースラインV2の平均値45.3(SD=2.5、95%CI:40.2〜50.4、N=39)からV7(14週/治療10週目)の38.1(SD=2.75、95%CI:32.5〜43.6、N=33)に低下した。また、暫定効果(ベースラインV2からV4[8週/治療4週目]までの間のTMLの平均変化)も有意であり(p値=0.045)、ベースラインV2の44.63(SD=2.61、95%CI:39.31〜50、N=33)からV4(8週/治療4週目)の40.18(SD=3.28、95%CI:33.5〜46.85、N=33)に低下した。TLMスコアにはプラセボ/文脈効果のエビデンスはなかった。スクリーニング検査V0及びベースラインV2の間の平均変化は1ポイント未満の42.9(SD=2.68)から43.4(SD=2.1)までの変化であり、有意ではなかった。
(耳鳴り障害目録[THI])
THIスコアの経時変化を図9に示す。繰り返しANOVA全体は統計的に有意であった(p値<0.001)。対t検定によるベースラインV2及びV7(14週/治療10週目)の比較も有意であると判定された(p値<0.001)。THIスコアは、ベースラインV0の平均値34.3(95%CI:27.3〜41.2、N=46)からV7(14週/治療10週目)の24.9(95%CI:19.8〜30.7、N=42)に低下した。また、暫定効果(ベースラインV2からV4[8週/治療4週目]までの間のTHIの平均変化)も有意であり(p値=0.0052)、ベースラインV2の34.42(95%CI:27.5〜41.3、N=50)からV4(8週/治療4週目)の31.12(95%CI:24.2〜38.1、N=50)に低下した。THIスコアには大きなプラセボ/文脈効果が見られた。平均THIスコアはスクリーニング検査V0の41.1(SD3.04)からベースライン検査V2の34.2(SD3.2)(N=54)まで低下し、この変化は統計的に有意であった(p値<0.001)。
(耳鳴りマッチ)
TMスコアの経時変化を図10に示す。繰り返しANOVA全体で値が低下する傾向がいくらか見られたが、有意ではなかった。それぞれの検査のサマリ値を以下の表に示す。
本研究は、目に見える有効性の早期のエビデンスを実証しており、この新規な介入が耳鳴り治療における有望な成果であることを示唆している。患者グループは、他覚的測定値における統計的に有意な平均改善率を示し、ベースライン検査(V2/4週)及び治験終了時(V7/14週)の間で最小マスキングレベルでは8.6dBの低下、耳鳴りラウドネスマッチでは7.2dBの低下を示した。これらの結果は、同様の他覚的測定を用いたその他の研究(ニューロモニクス療法による2か月で7.68dBの低下)に有利に匹敵するものである。同様に、患者グループは、THIの自覚的測定においても統計的に有意な改善を示した。ここで評価されている介入は、その他の治療とは違って、心理学的カウンセリングを含まないことを考慮すれば、これは非常に重要な結果である。これは、その他の単独(カウンセリングなし)の技術(ANM)が用いられた研究に有利に匹敵する。
Claims (16)
- 音処理部、触覚刺激部及び音響伝達部を有する耳鳴り治療システムであって、
前記音処理部は、
音響信号を受信するためのプロセッサ入力と、
前記音響信号を分析し、前記音響信号を表す複数の駆動信号を前記音響信号から生成するように、また、所定の補正プロフィールに従って前記音響信号をスペクトル補正し、補正音響信号を生成するように構成されたデジタル信号プロセッサと、
を有し、
前記触覚刺激部は、
対象に触覚刺激を与えるための個々に駆動可能な刺激器アレイと、
前記デジタル信号プロセッサによって生成された前記複数の駆動信号を受信し、それぞれの駆動信号をそれぞれの刺激器に送信するための刺激部入力と、
を有し、
前記音響伝達部は、前記デジタル信号プロセッサによって生成された前記補正音響信号を受信するための音響伝達部入力を有することを特徴とするシステム。 - 請求項1記載のシステムにおいて、当該システムは、デジタル信号処理機器 及び刺激伝達機器を有し、
前記デジタル信号処理機器は、
前記デジタル信号プロセッサと、
前記生成された駆動信号及び前記補正音響信号を電子ファイル形式で記憶するための電子ファイルライタと、
を有し、
前記刺激伝達機器は、
前記触覚刺激部と、
前記音響伝達部と、
前記デジタル信号処理機器によって生成された電子ファイルを読み取り、前記ファイルを音響信号及び複数の駆動信号に変換し、当該信号を前記音響伝達部及び前記触覚刺激部それぞれに送信するための電子ファイルリーダと、
を有することを特徴とするシステム。 - 請求項1又は2に記載のシステムにおいて、前記音処理部はさらに、前処理モジュールを有し、当該前処理モジュールは、
第1音響信号成分を受信するための第1前処理入力と、
第2音響信号成分を受信するための第2前処理入力と、
前記第1音響信号成分及び前記第2音響信号成分を組み合わせて前処理音響信号とするための加算器と、
前記前処理音響信号を前記プロセッサ入力に送信するための前処理出力と、
を有することを特徴とするシステム。 - 前処理モジュールを有する音処理部を有する耳鳴り治療システムであって、
前記前処理モジュールは、
第1音響信号成分を受信するための第1前処理入力と、
第2音響信号成分を受信するための第2前処理入力と、
前記第1音響信号成分及び前記第2音響信号成分を組み合わせて前処理音響信号とするための加算器と、
前記前処理音響信号を前記音処理部のプロセッサ入力に送信するための前処理出力と、
を有し、
前記音処理部はさらに、
前記前処理部からの前記前処理音響信号の出力を受信するためのプロセッサ入力と、
所定の補正プロフィールに従って前記前処理音響信号をスペクトル補正し、補正音響信号を生成するように構成されたデジタル信号プロセッサと、
前記デジタル信号プロセッサから前記補正音響信号を受信するためのプロセッサ出力と、
を有することを特徴とするシステム。 - 請求項4記載のシステムにおいて、当該システムはさらに、ノイズ発生器を有し、当該ノイズ発生器は、音響信号を生成し、当該音響信号を前記第1前処理入力に送信するように構成されていることを特徴とするシステム。
- 請求項1〜5のいずれかに記載のシステムにおいて、前記デジタル信号プロセッサはさらに、前記所定の補正プロフィールに応じて較正された帯域ブーストフィルタを有し、前記デジタル信号プロセッサは、前記音響信号を前記帯域ブーストフィルタに通過させて前記補正音響信号を生成することによって、前記音響信号をスペクトル補正するように構成されていることを特徴とするシステム。
- 請求項1〜6のいずれかに記載のシステムにおいて、前記所定の補正プロフィールは、耳鳴りを煩う患者の逆オージオグラムに基づいていることを特徴とするシステム。
- 請求項1〜7のいずれかに記載のシステムにおいて、前記補正音響信号は少なくとも、患者の聴力が低下しているスペクトル範囲にわたるスペクトル帯域幅を占める第1成分信号を含むことを特徴とするシステム。
- 請求項8記載のシステムにおいて、前記第1成分信号は、前記患者の聴力が低下している前記スペクトル範囲を1オクターブ以上超えて広がるスペクトル帯域幅、より好適には、難聴の最小周波数未満及び難聴の最大周波数超にそれぞれに対して約半オクターブ以上広がるスペクトル帯域幅、好適には、難聴の最小周波数未満及び難聴の最大周波数超にそれぞれに対して約4分の1オクターブ以上広がるスペクトル帯域幅を占めることを特徴とするシステム。
- 請求項7記載のシステムにおいて、前記補正音響信号は、当該補正音響信号が前記患者に聞こえるとき、少なくとも前記信号 に対する白色ノイズ成分を含む音響信号であるとの認識を誘発するようなスペクトル帯域幅を占めることを特徴とするシステム。
- 請求項1〜10のいずれかに記載のシステムにおいて、前記補正音響信号は少なくとも、2kHz〜6kHz、より好適には500Hz〜8kHz、さらに好適には125Hz〜20kHz、最も好適には125Hz〜40kHzのスペクトル帯域幅を占める第1成分信号を含むことを特徴とするシステム。
- 請求項3又は4に記載のシステムにおいて、前記第2音響信号成分は、人間である聞き手の注意を惹き付けることができ、任意選択的には、楽曲の記録又は人間の会話の記録であることを特徴とするシステム。
- 請求項2記載のデジタル信号処理機器又は刺激伝達機器。
- 音響信号を処理するための方法であって、
前記音響信号を分析するステップと、
第1出力信号を含み、電極アレイの駆動に適した、前記音響信号を表す複数の駆動信号を前記音響信号から生成するステップと、
所定の補正プロフィールに従って前記音響信号をスペクトル補正し、第2出力信号を含む補正音響信号を生成するステップと、
を含み、
前記第1出力信号及び前記第2出力信号は、信号出力装置に送信されるか、又は、電子ファイル形式に変換されて電子的に記憶され、
当該方法はさらに、任意選択的には、第1音響信号成分及び第2音響信号成分を組み合わせて前処理音響信号とすることによって前記音響信号を前処理するステップを含むことを特徴とする方法。 - 音響信号を処理するための方法であって、
第1音響信号成分及び第2音響信号成分を組み合わせて前処理音響信号とすることによって前記音響信号を前処理するステップと、
所定の補正プロフィールに従って前記音響信号をスペクトル補正し、補正音響信号を生成するステップと、
