血液脳関門(BBB)は、全身投与されたリソソーム酵素(例えば組み換えSGSH)の中枢神経系への送達を妨げる深刻な障壁である。本明細書に記載の方法および組成物は、治療的に意義のあるレベルの、ムコ多糖症III型(MPS−III)で欠損している酵素、例えば、SGSH、NAGLU、HGSNAT、GNSを、BBBを超えてCNSへ送達する上で重要な因子に対処する:1)MPS−IIIで欠損している酵素が内因性BBB輸送体上の輸送体を介してBBBを通過できるようにするための、該酵素の修飾、2)BBB輸送をもたらすために必要な修飾をした後も、酵素活性を保持することによる、全身投与された修飾酵素のCNS内への取り込み量および速度。本明細書に記載の方法および組成物の様々な態様は、(1)内因性BBB受容体の細胞外ドメインに対する免疫グロブリン(重鎖または軽鎖)に、介在配列有りまたは無しで融合した酵素(すなわち、SGSH活性を有するタンパク質)を含む融合抗体を提供することにより、そして(2)CNSにおける取り込みおよび比活性に基づいて、該融合抗体の治療的に有効な全身用量を確立することにより、これらの因子に対処する。いくつかの態様において、内因性BBB受容体に対する抗体は、ヒトインスリン受容体に対する抗体(HIR Ab)である。
本明細書で用いられる「治療」または「治療する」としては、治療効果および/または予防効果を達成することが挙げられる。治療効果は、治療される原因となる障害または病状の根絶または改善を意味する。例えば、MPS−IIIAに罹患した個体において、治療効果としては、障害の進行の部分的もしくは完全な停止、または障害の部分的もしくは完全な好転が挙げられる。また、治療効果は、患者が依然として病状に罹患している可能性があるという事実があっても、患者において改善が認められるように、原因となる病状に関連する1つ以上の生理的または精神的症状の根絶または改善によっても達成される。治療の予防効果としては、病状の予防、病状の進行の遅延(例えば、リソソーム貯蔵障害の進行の遅延)、または病状が発生する可能性の低減が挙げられる。本明細書で用いられる「治療する」または「治療」は、予防を含む。
一態様において、本明細書は、内因性BBB受容体/輸送体上の受容体媒介輸送を介して血液脳関門(BBB)を通過できる免疫グロブリンと融合した、MPS−IIIで欠損している酵素(例えばSGSH)を利用する、組成物および方法を提供する。標的化のための典型的な内因性輸送体は、BBB上のインスリン受容体である。BBBインスリン受容体は、血中インスリン、および、HIRMAbのような、ある種のペプチド模倣モノクローナル抗体(MAb)の脳内への輸送を仲介する。内因性リガンドまたはペプチド模倣MAbのいずれかにより、標的となり得る他の内因性輸送体としては、BBBトランスフェリン受容体、BBBインスリン様成長因子(IGF)受容体、BBBレプチン受容体、またはBBB低密度リポタンパク質(LDL)受容体が挙げられる。該組成物および方法は、SGSHを、末梢血から血液脳関門を超えてCNS内へ輸送するために有用である。本明細書において、「血液脳関門」は、末梢循環と脳および脊髄との間の障壁を指し、該障壁は、脳の毛細血管内皮細胞膜内の密着結合により形成され、脳内への分子の輸送を制限する極めて密着した障壁を作り出す:BBBは、分子量60Daの尿素程度の小さい分子でさえ制限できるほど密着している。脳内の血液脳関門、脊髄内の血液脊髄関門、および網膜内の血液網膜関門は、中枢神経系(CNS)内の隣接する毛細血管関門であり、まとめて血液脳関門またはBBBと称される。
グルコースまたはアミノ酸のような、血中のある種の内因性低分子は、水溶性であり、ある種のBBB担体系上の担体介在輸送(CMT)により、BBBをまだ透過することができる。例えば、グルコースは、GLUT1グルコース輸送体上のCMTを介してBBBを透過する。L−DOPAのような治療用のアミノ酸を含むアミノ酸は、LAT1大型中性アミノ酸輸送体上のCMTを介してBBBを透過する。同様に、血中のある種の内因性巨大分子、例えば、インスリン、トランスフェリン、インスリン様成長因子、レプチン、または低密度リポタンパク質は、ある種のBBB受容体系上の受容体介在トランスサイトーシス(RMT)により、BBBを透過することができる。例えば、インスリンは、インスリン受容体上のRMTを介してBBBを透過する。トランスフェリンは、トランスフェリン受容体上のRMTを介してBBBを透過する。インスリン様成長因子は、インスリン様成長因子受容体上のRMTを介してBBBを透過し得る。レプチンは、レプチン受容体上のRMTを介してBBBを透過し得る。低密度リポタンパク質は、低密度リポタンパク質受容体上の輸送を介してBBBを透過し得る。
MPS−IIIで欠損している酵素(例えばSGSH)を、CNSへ送達する非侵襲的なアプローチの一つは、SGSHを、インスリン受容体のECDに選択的に結合する抗体と融合することである。それにより、BBB上に発現したインスリン受容体は、BBBを超えてSGSHを輸送するベクターとして働くことができる。ある種のECD特異抗体は、内因性リガンドを模倣し得ることから、特異的受容体系上の輸送を介して細胞膜障壁を通過する。そのようなインスリン受容体抗体は、図1に概略的に示すように、分子的「トロイの木馬」または「TH」として働く。SGSHは、単独では通常、血液脳関門(BBB)を通過しない。しかし、SGSHをTHと融合すると、IRのような、脳内のBBBと脳細胞膜の両方に発現している内因性BBB受容体上の輸送により、該酵素は、BBBおよび脳細胞膜を通過できる(図1)。
組み換えSGSHの全身投与(例えば、静脈注射による)が、MPS−IIIAに罹患した患者のCNSにおけるSGSHの欠損を救うことは期待できない。SGSHはBBBを通過せず、BBBを越える酵素の輸送の欠如により、末梢投与された後、CNSにおいて意義のある治療効果を有することが妨げられる。しかし、本発明者らは、HIR AbなどのBBBを通過する抗体とSGSHが(例えば、共有結合性のリンカーにより)融合すると、この酵素が、非侵襲的な末梢投与経路(例えば、静脈内投与、動脈内投与、筋肉内投与、皮下投与、腹腔内投与、または経口投与でさえ)をたどって、血液からCNSに入ることができるようになることを見いだした。HIR Ab−SGSH融合抗体の投与は、末梢血から脳内へのSGSH活性の送達を可能にする。本明細書は、CNSにおけるSGSH欠損症を治療するために治療的に有効なHIR Ab−SGSH融合抗体の全身用量を決定することについて記載する。本明細書に記載されているように、HIR Ab−SGSH融合抗体の適切な全身用量は、HIR Ab−酵素融合抗体のCNS取り込み特性および酵素活性の、定量的な決定に基づいて確立される。
組み換えNAGLUの全身投与(例えば、静脈注射による)が、MPS−IIIAに罹患した患者のCNSにおけるNAGLUの欠損を救うことは期待できない。NAGLUはBBBを通過せず、BBBを越える酵素の輸送の欠如により、末梢投与された後、CNSにおいて意義のある治療効果を有することが妨げられる。しかし、本発明者らは、HIR AbなどのBBBを通過する抗体とNAGLUが(例えば、共有結合性のリンカーにより)融合すると、この酵素が、非侵襲的な末梢投与経路(例えば、静脈内投与、動脈内投与、筋肉内投与、皮下投与、腹腔内投与、または経口投与でさえ)をたどって、血液からCNSに入ることができるようになることを見いだした。HIR Ab−NAGLU融合抗体の投与は、末梢血から脳内へのNAGLU活性の送達を可能にする。本明細書は、CNSにおけるNAGLU欠損症を治療するために治療的に有効なHIR Ab−NAGLU融合抗体の全身用量を決定することについて記載する。本明細書に記載されているように、HIR Ab−NAGLU融合抗体の適切な全身用量は、HIR Ab−酵素融合抗体のCNS取り込み特性および酵素活性の、定量的な決定に基づいて確立される。

本明細書に記載の二機能性の融合抗体は、それらの別々の構成タンパク質の活性、例えば、BBBを通過できる抗体(例えばHIR Ab)と、BBB上の内因性受容体の細胞外ドメイン(例えばIR ECD)との結合、および、MPS−IIIで欠損している酵素(例えばSGSH)の酵素活性を高い割合で保持していることが見いだされている。本明細書に記載の任意のタンパク質をコードするcDNAおよび発現ベクターの構築、ならびに、それらの発現および精製については、当業者に周知されており、本明細書の例えば実施例1〜3、ならびにBoadoら(2007), Biotechnol Bioeng 96:381-391、米国特許出願番号11/061,956および米国特許出願番号11/245,710に詳細に記載されている。
本明細書は、治療的有効量の、本明細書に記載の融合抗体を全身投与することにより、BBBを越えてCNSへ、有効用量の、MPS−IIIで欠損している酵素(例えばSGSH)を送達する方法について記載する。いくつかの態様において、本明細書の融合抗体はHIR Ab−SGSHである。本明細書に記載されているように、HIR Ab−SGSH融合抗体の送達のための適切な全身用量は、そのCNS取り込み特性およびSGSH比活性に基づいている。SGSH欠損症に罹患した対象へのHIR Ab−SGSH融合抗体の全身投与は、CNSへSGSHを非侵襲的に送達する有用なアプローチである。さらに、本明細書は、治療的有効量の、本明細書に記載の融合抗体を全身投与することにより、対象におけるMPS−IIIを治療する(例えば、治療的に処置する)方法について記載する。いくつかの態様において、治療は治療的処置である。いくつかの態様において、治療は、MPS−IIIの1つ以上の症状を軽減する。いくつかの態様において、治療は、MPS−IIIの1つ以上の症状を、約5%、10%、15%、20%、25%、30%、40%、50%、60%、70%、75%、80%、90%、95%、99%、または100%軽減する。
スライ症候群とも称されるMPS−VIIで変異しているリソソーム酵素は、β−グルクロニダーゼ(GUSB)である。MPS−VIIは、脳内のグリコサミノグリカンの蓄積をもたらす。GUSB酵素はBBBを通過しないため、MPS−VIIの酵素補充療法(ERT)は、脳の治療に有効ではないようである。BBBを通過するヒトGUSBを再設計する試みにおいて、HIR Ab−GUSB融合タンパク質プロジェクトを開始した。

ゴーシェ病(GD)で変異しているリソソーム酵素は、β−グルコセレブロシダーゼ(GCR)である。GDの神経細胞障害性の形態はCNSに影響を及ぼし、これにより、脳内でGCR酵素活性が欠如するため、脳細胞内でリソソーム内封入体が蓄積することになる。GDの酵素補充療法(ERT)は、GCR酵素がBBBを通過しないことから、脳の治療に有効ではない。BBBを通過させるためにヒトGCRを再設計する一環として、HIR Ab−GCR融合タンパク質プロジェクトを、酵素活性について設計し、発現し、試験した。39アミノ酸のシグナルペプチドを含まない、ヒトGCRタンパク質(NP_000148)のアミノ酸Ala40−Gln536に対応するヒトGCR cDNAを、営利DNA作製会社でカスタム合成した。GCB cDNAを、シグナルペプチドからTGAストップコドンまでを含まない、GCBのオープンリーディングフレームを含む1522ヌクレオチド(nt)で構成した。5’末端には、StuI制限エンドヌクレアーゼ(RE)配列を付加し、3’末端には、GCR mRNAの3’−非翻訳領域からの14ntフラグメントの後にHindIII RE部位を続けた。GCR遺伝子内の内在するHindIIIおよびStuI部位を、アミノ酸配列を変化させないように突然変異させた。GCR遺伝子を、StuIおよびHindIIIを用いて、供給業者により提供されたpUCプラスミドから切り離して、HIR Ab重鎖をコードする真核細胞発現プラスミドのHpaIおよびHindIII部位に挿入し、この発現プラスミドをpCD−HC−GCRと命名した。この発現プラスミドは、HIR AbのHCとGCRとの間の3アミノ酸のリンカー(Ser−Ser−Ser)により、HIR Abの重鎖(HC)のカルボキシル末端と、39アミノ酸のGCRシグナルペプチドを含まないヒトGCRのアミノ末端を融合した、融合タンパク質を発現する。DNAシーケンシングにより、pCD−HC−GCR発現カセットを同定した。発現カセットを、2134ntのCMVプロモーター配列、2,889ntの発現カセット、および367BGHポリA配列を含む、5,390ntで構成した。プラスミドに、19アミノ酸のIgGシグナルペプチド、443アミノ酸のHIRMAb HC、3アミノ酸のリンカー(Ser−Ser−Ser)、および、該酵素シグナルペプチドを含まない497アミノ酸のヒトGCRからなる、963アミノ酸のタンパク質をコードさせた。GCR配列は、ヒトGCR(NP_000148)のAls40−Gln536と100%同一であった。グリコシル化されていない重鎖融合タンパク質の予想される分子量は104,440Daであり、予想される等電点(pI)は8.42である。
MPS−IIIAで変異しているリソソーム酵素は、SGSHである。MPS−IIIAは、脳内にヘパラン硫酸の蓄積をもたらす。Hemsley et al. (2009)(Examination of intravenous and intra-CSF protein delivery for treatment of neurological disease」, Eur. J. Neurosci., 29:1197-1214)に記載されているように、SGSH酵素がBBBを通過しないため、MPS−IIIAの酵素補充療法は脳の治療に有効ではない。BBBを通過することができ、かつ酵素活性を示すことができる、二機能性の分子を開発するために、SGSHをHIR Abと融合した。一態様において、成熟SGSHのアミノ末端を、HIR Abの各重鎖のカルボキシル末端と融合した(図2)。

チャイニーズハムスター卵巣(CHO)細胞を、1xHTサプリメント(ヒポキサンチンおよびチミジン)を含む無血清HyQ SFM4CHOユーティリティ(Utility)培地(HyClone)中で培養した。CHO細胞(5x106の生細胞)を、5μgのPvuIで線状にしたTV−HIRMAb−SGSHプラスミドDNAと共にエレクトロポレーションした。次いで、細胞−DNA懸濁液を氷上で10分間インキュベートした。BioRadのCHO細胞用のプリセット・プロトコール、すなわち、15msecおよび160ボルトのパルスを有する方形波で細胞をエレクトロポレーションした。エレクトロポレーション後、細胞を氷上で10分間インキュベートした。細胞懸濁液を50mL培地に移し、1ウェル当たり125μlで4x96ウェルプレートに蒔いた(1ウェル当たり10,000細胞)。1研究当たり、計10回のエレクトロポレーションおよび4,000ウェルを行った。
HIR細胞外ドメイン(ECD)に対する融合タンパク質の親和性を、ELISAで測定した。以前Coloma et al. (2000) Pharm Res, 17:266-274に記載されているように、無血清培地(SFM)中で、HIR ECDで永続的にトランスフェクトされたCHO細胞を培養し、コムギ胚芽凝集素アフィニティーカラムを用いて、HIR ECDを精製した。Nunc−Maxisorb 96ウェルディッシュにHIR ECDを蒔き、ビオチン化抗ヒトIgG(H+L)ヤギ二次抗体、次いでアビジンおよびビオチン化ペルオキシダーゼ(Vector Labs、Burlingame、CA)を用いて、HIR ECDへの、HIR AbまたはHIR Ab−SGSH融合タンパク質の結合を検出した。非線形回帰分析により、HIR AbまたはHIR Ab−SGSH融合タンパク質の、最大の結合の50%を与える濃度であるED50を決定した。HIRへの結合のED50は29±3ng/mLであり、HIRへのHIR Ab−SGSH融合タンパク質の結合のED50は107±17ng/mLである(図12)。HIR AbのMWは150kDaであり、HIR Ab−SGSH融合タンパク質のMWは325kDaである。従って、分子量の差を標準化した後の、HIR ECDへの、キメラHIR AbまたはHIR Ab−SGSH融合タンパク質の結合は同等であり、ED50はそれぞれ、0.19±0.02nMと0.33±0.05nMであった(図12)。これらの知見は、SGSH分子がIgGの両重鎖のカルボキシル末端と融合されているにもかかわらず、HIRに結合するHIR Ab−SGSH融合タンパク質の親和性が保持されていることを示している。
成熟ヒトSGSHは、HIR AbのHCのカルボキシル末端と、3アミノ酸のリンカー、(図9で下線を付した)Ser−Ser−Serで融合している。任意の数のリンカーのバリエーションがSer−Ser−Serリンカーの代わりに用いられる。3アミノ酸のリンカーは保持されてよいが、アミノ酸配列は代わりのアミノ酸、例えばGly−Gly−Gly、Ser−Gly−Ser、Ala−Ser−Gly、または20の天然アミノ酸の任意の数の組み合わせに変化してよい。または、リンカーは2、1、または0のアミノ酸に減らされる。0アミノ酸のリンカーの場合、SGSHのアミノ末端はHIR AbのHCのカルボキシル末端と直接融合されている。代わりに、リンカーの長さは3、4、5、6、7、8、9、10、11、12、13、14、15、または20アミノ酸まで伸長される。融合タンパク質の設計における最適なアミノ酸リンカーを決定するための複数の公的に入手可能なプログラムがあるため、そのようなリンカーは当該技術分野で周知である。頻繁に用いられるリンカーは、反復配列におけるGlyおよびSerの様々な組み合わせ、例えば(Gly4Ser)3、または他のバリエーションを含む。
SGSH酵素は、ヘパラン硫酸GAGからの硫酸基を加水分解し、これにより、細胞内の他の酵素よるGAGのその後の分解が可能となる。サンフィリッポA型線維芽細胞(MPS−IIIA線維芽細胞)において、GAGからの硫酸エステルの放出に対する、HIRMAb−SGSH融合タンパク質を用いた処理の効果を試験した。細胞を、Coriell Institute for Medical Research(Camden、NJ)から入手し、6ウェルクラスターディッシュ内でコンフルエントになるまで培養し、次いで、35Sを用いたパルス・チェイス実験を行った。パルスの段階では、コンフルエントな細胞を、リン酸緩衝生理食塩水(PBS)で洗浄し、加湿インキュベーター内で、10%透析ウシ胎児血清(FBS)を含む1mLの低濃度硫酸エステルハムF12培地、および、47μCi/mLの[35S]硫酸ナトリウムと、37℃で48時間インキュベートした。培地を吸引し、ウェルをPBSで洗浄して、加湿インキュベーター内で、細胞を、通常の10%FBSを含む放射能のないDMEM/F12培地(1mL/ウェル)、および、様々な濃度のHIRMAb−SGSH融合タンパク質と、37℃で48時間インキュベートした。培地を吸引し、細胞をPBSで洗浄した。37℃で4分間、0.4mL/ウェルの0.05%トリプシン/EDTAを用いて、細胞をウェルから取り出し、このステップにより、細胞表面に付着していた放射能は取り除かれることから、リソソームのGAGには取り込まれない。細胞を無血清培地中に懸濁してトリプシン反応を停止し、1000gで遠心分離する。上清を取り出して廃棄し、細胞ペレットを、0.4mLの1N NaOH中に、60℃で1時間可溶化する。ビシンコニン酸(BCA)タンパク質アッセイを用いて、タンパク質含有量を測定する。Perkin Elmer液体シンチレーションカウンターを用いて、Ultima−gold counting solution中で、35S放射能を測定した。CPM放射能をmgタンパク質ウェルで割り、CPM/mgタンパク質としてデータを報告した(表3)。本研究は、HIRMAb−SGSH融合タンパク質の準治療濃度(0.25〜0.5μg/mL)が、MPS−IIIA線維芽細胞において硫酸エステルでラベルされたGAGを、72%〜83%減少させることを示している。

アカゲザルにおいて、インビボでのHIRMAb−SGSH融合タンパク質のBBB透過を評価した。融合タンパク質のHIRMAbドメインは、アカゲザルのような旧世界霊長類のインスリン受容体と交差反応するが、リスザルのような新世界ザルのインスリン受容体、または、マウスのインスリン受容体とは交差反応しない。従って、脳へのインビボ送達は、アカゲザルにおいて評価した。[125I]−モノヨウ化−ボルトン・ハンター試薬を用いて、比活性が3.7μCi/μgとなるように、HIRMAb−SGSH融合タンパク質を放射性ラベルした。トリクロロ酢酸(TCA)による[125I]−HIRMAb−SGSH融合タンパク質の沈殿性は、−70℃で保管している間、ラベル後少なくとも8日間は>97%であった。ラベルをする前に、0.01M 酢酸ナトリウム/140mM NaCl/pH=5.5/0.001%Tween−80、およびAmicon Ultracel−30K遠心ろ過ユニットを用いて、融合タンパク質の緩衝液交換を行った。Sephadex G−25の1x28cmカラムを用いたゲルろ過、および、0.01M 酢酸ナトリウム/140mM NaCl/pH=5.5/0.001% Tween−80の溶出緩衝液を用いて、ラベルされたHIRMAb−SGSH融合タンパク質を精製した。MPI Research(Mattawan,MI)において、17.3kgの成熟雌アカゲザルを調査した。該動物に、静脈内ボーラスにより、30秒間にわたって、左大腿静脈に、1200μCiの[125I]−HIRMAb−SGSH融合タンパク質を静脈内(IV)注射した。HIRMAb−SGSH融合タンパク質の注入量(ID)は19μg/kgであった。筋肉内ケタミンにより該動物を麻酔した。すべての手順は、米国国立衛生研究所により承認、公布された、実験動物の管理と使用に関する指針に従い実施した。総血漿[125I]放射能(DPM/mL)、および、10%冷TCAで沈殿する血漿放射能を測定するため、静脈内薬物投与の2、5、15、30、60、90、および140分後に、大腿静脈血漿を採取した。該動物を安楽死させ、主要組織(心臓、肝臓、脾臓、肺、骨格筋、および大網脂肪)のサンプルを摘出、秤量し、放射能測定用に加工した。頭蓋を開け、脳を摘出した。放射能を測定するために、前頭皮質灰白質、小脳灰白質、および脈絡叢のサンプルを摘出した。Wizard model 1470 gamma counterを用いて、125I放射能について、血漿および組織サンプルを分析した。注射した融合タンパク質の比活性に基づいて、血漿中のTCAで沈殿可能な[125I]放射能(DPM/mL)をng/mLに換算し、融合タンパク質の血漿濃度に、二項指数方程式%ID/mL=A1e−k1t+A2e−k2tを当てはめた。滞留時間の中央値(MRT)、分布容積の中央値(Vc)、定常状態分布容積(Vss)、血漿濃度曲線下面積(AUC)、および全身クリアランス(CL)を算出するために、切片(A1、A2)および傾き(k1、k2)を用いた。非線形回帰分析には、BMDP Statistical Software(Statistical Solutions Ltd、Cork、Ireland)のARサブルーチンを使用した。データを1/(%ID/mL)2で重み付けした。

非特異的なヒトIgG1アイソタイプコントロール抗体の脳VD(20±6μl/グラム)に比べ、注射140分後の総脳ホモジネート中のHIRMAb−SGSH融合タンパク質の分布容積(VD)(782±36μL/グラム)は大きい(表5)。IgG1アイソタイプコントロール抗体の脳VDは、脳の血液容量内に隔離されており、かつ、BBBを通過しない分子の脳取り込みを表す。後血管上清中のHIRMAb−SGSH融合タンパク質のVD(666±71μL/グラム)は、脳の血管ペレット内のHIRMAb−SGSH融合タンパク質のVD(24±17μL/グラム)より大きく(表5)、このことは、HIRMAb−SGSH融合タンパク質の大部分が、BBBを通過し脳実質を透過したことを示している。後血管上清放射能のTCA沈殿が95.9±0.7%である(表5)ことから、後血管上清中の放射能は、ラベルされた代謝物ではなく、無傷なHIRMAb−SGSH融合タンパク質を表している。

サンフィリッポA型またはMPS−IIIAは、リソソーム酵素であるスルファミダーゼ(SGSH)をコードする遺伝子の欠損が原因で起こる、リソソーム貯蔵障害である。SGSHを欠くと、ヘパラン硫酸のような、ある種のGAGが細胞内に蓄積する。脳内のヘパラン硫酸の蓄積は、重度の行動障害、一般的に7歳までに言語障害、一般的に12歳までに車椅子生活を招く歩行障害、および、平均18歳での死亡を含む、MPS−IIIAの臨床症状をもたらす(Valstar et al.
