Abstract
Description
血友病は、血友病A、血友病B(クリスマス病)及びフォンウィルブランド病を含む遺伝性血液障害の群に属する。
後天性血友病は、百万人ごとに1〜4人が罹患する希な自己免疫状態である。この状態において、血友病を持って生まれたのではない対象が、第VIII因子などの凝固因子のうちの1つに対して抗体を発生させる。妊娠及び関節リウマチなどの自己免疫疾患及び癌は、後天性血友病を発症するリスクを増大させ得ると考えられている。インヒビターの産生につながる根底にある免疫機構に違いはあるが、凝血因子補充療法に応答して産生されるFVIIIインヒビターと、後天性血友病において産生されるインヒビターの臨床的徴候は類似している。

対象は第VIII因子が欠乏している状態であり得る。特に、対象は血友病Aであり、第VIII因子補充療法を受けているか、もしくは受けようとしている患者であり得る。
あるいはまた、対象は、後天性血友病を有するか、またはこれに罹患するリスクを有する者であり得る。
第VIII因子インヒビターは、HLA-DR2を発現する個体においてより頻繁に見られる。従って、本発明の方法によって治療される対象はHLA-DR2陽性であり得る。

本発明のペプチドの中心部分を形成する配列DNIMVTFRNQASRPY及びPRCLTRYYSSFVNMEは、両方とも第VIII因子から誘導可能である。
本発明の第一の実施形態のペプチドは、下記ペプチドのうちの1種についての修飾形態である:
XXGDNIMVTFRNQASRPYGXX 又は
XXGPRCLTRYYSSFVNMEGXX
[式中、Xは、リジン又はグルタミン酸である。]
を有する。
T細胞エピトープは、自己のものであろうと外来性であろうと、任意の抗原への適応免疫応答において、中心的な役割を果たす。過敏性疾患(アレルギー、自己免疫疾患及び移植片拒絶反応を含む)においてT細胞エピトープが果たす中心的な役割は、実験モデルの使用を通して実証された。炎症性又はアレルギー性疾患を、アジュバントと組み合わせた(T細胞エピトープの構造に基づいた)合成ペプチドの注射によって誘導することが可能である。
(i)FVIII阻害性抗体、
(ii)FVIIIに特異的なCD4+T細胞
(iii)FVIII阻害性抗体を分泌する能力があるB細胞
のレベルの低下を調べることによって、in vivoでモニターすることができる。
(a)CD4+T細胞におけるアネルギーの誘導(その後のin vitroでのFVIII接種によって検出することができる)
(b)(i)増殖の低下、
(ii)IL-2、IFN-γ及びIL-4の産生の下方制御、及び
(iii)IL-10の産生の増加
を含む、CD4+T細胞集団の変化
を含む様々な技法によってモニターすることもできる。
本発明は、本発明による1種以上のペプチドを含む医薬組成物などの組成物にも関する。
(i)FVIII阻害性抗体、
(ii)FVIIIに特異的なCD4+T細胞
(iii)FVIII阻害性抗体を分泌するB細胞。
組成物は、溶液又は懸濁液としての注射剤として調製することができ、注射前に液体中に溶解又は懸濁させるのに適した固体形態を調製することもできる。この調製物は、乳化することもでき、又はペプチドは、リポソームに封入することができる。活性成分は、薬学的に許容され、活性成分と適合性の賦形剤と混合することができる。適した賦形剤は、例えば、水、生理食塩水(例えば、リン酸緩衝生理食塩水)、デキストロース、グリセロール、エタノールなど及びそれらの組合せである。
組成物が複数のペプチドを含む場合、それらを好都合に混合組成物又はカクテルの形態で一緒に投与することができる。しかしながら、同時、個別、連続又は組合せ投与のために、ペプチドをキットの形態で別々に提供することが好ましい状況があり得る。
血友病A(古典的血友病)は、第VIII因子の欠乏によって引き起こされる。
後天性血友病は、以前は正常な凝血を有した個体におけるFVIIIに対する自己抗体インヒビターの存在によって特徴付けられる。それは、年間の推定発症率が人口百万人当たり1〜3人である、希な状態である。後天性自己抗体インヒビターと関連している死亡率は、同種抗体を有する人における実質的により低い死亡リスクと比較して25%に達する。
i)例えばペニシリン又はスルホンアミドでの差し迫った治療、
ii)腫瘍又は他の悪性疾患の進行、
iii)差し迫った妊娠又は妊娠初期
のために後天性血友病を発症するリスクがあると考えられる患者に投与することができる。
FVIII由来ペプチドの溶解性を調査する
合計32種のペプチドを試験した:そのうちの16種は、FVIIIペプチドPRCLTRYYSSFVNME(「PRCLT」)に基づき、そのうちの16種は、FVIIIペプチドDNIMVTFRNQASRPY(「DNIMV」)に基づくものとした。ペプチドを、下記表1及び2に要約する。
臨床的に関連する濃度でのタグを付けたペプチドの水性溶媒への直接溶解
各試験ペプチド及び親ペプチド(PRCLT及びDNIMV)に関して、DMSO(対照)又はPBS中の4mg/mLで、2つのチューブを調製した。
タグを付けたペプチドは、アピトープとして働く
HLA-DR2トランスジェニックマウスを、親ペプチドPRCLT又はDNIMVで抗原刺激した。次いで、脾臓及びリンパ節を採取し、CD4精製後、細胞を、1ug/mlヒト組換えFVIIIで再刺激した。BW5147細胞との融合後、得られたクローンを増殖させ、DNIMV又はPRCLTへの特異性を「スクリーニングした」。これは、5×104DR2陽性抗原提示細胞(MGAR細胞系)及び10ug/ml PRCLT/DNIMV又は培地で48時間、ハイブリドーマ細胞をインキュベートすることによって行われる。次いで、上清を、ELISAによってIL-2産生に関して分析した。
Ex vivo T細胞寛容
修飾アピトープが親ペプチドへのT細胞応答を制御する能力があるかどうかを決定するために、雄HLA-DR2トランスジェニックマウスを、3×100μgペプチド(ペプチド配列番号1、2、17又は18)又は対照としてPBSで処置し、次いで、完全フロイントアジュバント中のそれぞれ100μgの親ペプチド(PRCLT及びDNIMV)で免疫化した。10日後、流入領域リンパ節及び脾臓を採取し、PRCLT又はDNIMVでin vitroで刺激した。結果を、図4(PRCLT)及び5(DNIMV)に示す。修飾ペプチド配列番号1又は配列番号2での処置は、PRCLTへのT細胞応答を低下させ、修飾ペプチド配列番号17又は配列番号18での処置は、DNIMVへのT細胞応答を低下させる。したがって、修飾ペプチドは、親ペプチドで免疫されたマウスからの免疫応答を低下させることができる。
Ex vivo T細胞寛容
親ペプチドへのT細胞応答の制御に対する修飾アピトープの効果が、FVIIIへの応答の制御にまで及ぶかどうかを決定するために、雌HLA-DR2トランスジェニックマウスを、3×100μgペプチド(ペプチド配列番号1又は配列番号17)で処置し、次いで、完全フロイントアジュバント中のそれぞれ100μgの親ペプチド(PRCLT及びDNIMV)で抗原刺激した。10日後、流入領域リンパ節を採取し、PRCLT、DNIMV又は組換えFVIIIでin vitroで刺激した。結果を、図6に示す。ペプチド配列番号1は、PRCLTペプチドへのT細胞応答を有意に(p<0.01)低下させた。ペプチド配列番号17は、DNIMVへのT細胞応答を有意に(p<0.02)低下させた。
PRCLT及びDNIMV修飾ペプチドの組合せの用量漸増での抗体産生予防
雄HLA-DR2トランスジェニックマウスを、用量漸増プロトコールに従って、ペプチド配列番号1及びペプチド配列番号17の組合せで処置した。組合せの6回の注射を、以下の用量で投与した:(組合せ中の各ペプチドに関して)0.1、1.0、10及び3×100μg。
PRCLT及びDNIMV修飾ペプチドの組合せの用量漸増を使用したEx vivo T細胞寛容
雄/雌DR2マウスを、実施例6に記載した用量漸増プロトコールに従って、ペプチド配列番号1/配列番号17の組合せで処置した。
治療動物モデルにおける、PRCLT及びDNIMV修飾ペプチドの組合せの用量漸増での中和抗体産生の抑制
HLA-DR2トランスジェニックマウスを、CFA中の3μg rFVIIIで抗原刺激し、免疫化の3週間後、用量漸増プロトコールに従ってペプチド配列番号1及びペプチド配列番号17の組合せで処置した。組合せの6回の注射を、以下の用量で投与した:(組合せ中の各ペプチドに関して)0.1、1.0、10及び3×100μg。対照動物を、同じ用量漸増プロトコールを使用して、関係のないFVIII DR2結合前立腺酸性ホスファターゼ(PAP)133〜152ペプチド(PCT/US2006/031961に記載された配列、配列番号15)で処置した。全ての動物に、rFVIII免疫化の5週間後及びペプチド処置の最後のラウンドの4日後に、IFA中の3μg rhFVIIIで追加免疫をほどこした。中和抗体の産生を、最初のFVIII免疫化の3週間後及びあらゆるペプチド処置の前、5週目(FVIII追加免疫化前及びペプチド処置後)及び7、9及び13週目での追跡調査期間中に、採取した血漿試料から分析した。血漿試料中の中和FVIII抗体レベルを、実施例6に記載したように決定した。

- FVIII由来の配列DNIMVTFRNQASRPYを含むペプチドであって、

を有し、かつ
(b)以下の配列:

- 対象が血友病Aを有し、第VIII因子補充療法及び/又はFVIIIバイパス療法を受けているか、あるいは受けようとしている、請求項8又は9記載の方法。
- 対象が後天性血友病を有するか、又はこれに罹患するリスクを有する、請求項8又は9記載の方法。
