Abstract
Description
The present invention relates to a catheter provided with a reinforcing body in a shaft.
In recent years, for example, in the treatment of acute myocardial infarction or angina pectoris, percutaneous transluminal coronary angioplasty for improving blood flow by expanding a lesion (stenosis) of a coronary artery with a balloon catheter has been performed. ing. A balloon catheter may be used to improve other blood vessels, bile ducts, trachea, esophagus, urethra, and other lesions formed in the body lumen.
Generally, a catheter such as a balloon catheter has a long and flexible shaft, and an operator operates the proximal end side of the catheter to smoothly advance the long shaft into a tortuous blood vessel. It is necessary to pass the tip smoothly through a hard stenosis. For this reason, it is desirable that the pushing force by the operator from the proximal end side can be reliably transmitted to the distal end side.
In the prior art, the shaft is configured to have a highly flexible distal shaft whose distal end is connected to the balloon, and a proximal shaft that is connected to the proximal end of the distal shaft and has higher rigidity than the distal shaft. Some have made it easier for the pushing force applied to the hand during operation to be transmitted to the tip side. However, in the structure in which the distal shaft and the proximal shaft are simply connected in this way, the rigidity of the catheter changes abruptly at the connection portion between the distal shaft and the proximal shaft, so when the catheter passes through a tortuous blood vessel. There is a problem that stress concentrates on the portion where the rigidity has suddenly changed and kinks are likely to occur.
Therefore, by disposing a flexible elongated reinforcing body from the inside of the distal end of the proximal shaft to the inside of the distal shaft, the sudden change in rigidity at the connecting portion between the distal shaft and the proximal shaft is alleviated. A configuration that improves kink resistance is known (see, for example, Patent Document 1 below).
By the way, in order to keep the shaft lumen large, the conventional reinforcing body has been used that has a small cross-sectional area at the tip and a sharp tip. However, a catheter using such a reinforcing body may pierce the blood vessel wall by breaking through the shaft when the catheter is bent. Therefore, it is considered that the tip of the reinforcing body is joined and fixed to the shaft. However, since the above-mentioned reinforcing body has a thin tip, a sufficient bonding area to the shaft cannot be secured, and the tip of the reinforcing body is difficult to be fixed to the shaft. Therefore, with such a reinforcing body, it is difficult to efficiently prevent the reinforcing body from breaking through the shaft.
The present invention has been made in consideration of such problems, and in a catheter in which a reinforcing body is disposed in a shaft, a catheter capable of effectively preventing the reinforcing body from breaking through the shaft is provided. With the goal.
In order to achieve the above object, the present invention comprises a hollow shaft and an inner tube disposed in the shaft, wherein the shaft has a distal end shaft and a distal end portion is a proximal end portion of the distal end shaft. A proximal end shaft having a rigidity different from that of the distal end shaft, and a reinforcing body is disposed in the distal end shaft and in the proximal end shaft, and the reinforcing body includes the distal end shaft and the distal end shaft. A main body portion extending across the proximal shaft, and a distal end enlarged portion provided at a distal end portion of the main body portion and disposed between the distal shaft and the inner tube, and the distal end enlarged portion Is three-dimensionally formed by curved thin wire elements, and has a larger diameter than the tip of the main body. Here, “the tip enlarged portion is three-dimensionally formed by curved thin wire elements” means that the tip enlarged portion of the reinforcing body is not a planar shape existing in one plane, but the axis ( The thin wire element is curved so as to extend in the directions of the Y axis and the Z axis perpendicular to each other and perpendicular to the X axis). It is not on the same plane. The curved fine line element includes a bent fine line element.
According to the catheter configured as described above, since the distal end enlarged portion is provided in the reinforcing body, the contact area between the distal end portion of the reinforcing body and the distal end shaft or the proximal end shaft increases. Thereby, it can suppress suitably that a reinforcement body breaks through a front-end | tip shaft. In addition, since the distal enlarged portion is a bent thin wire element and has flexibility, the flexibility of the catheter in the portion where the distal enlarged portion is disposed can be suitably maintained.
The catheter includes a balloon that is connected to a distal end of the distal shaft and can be expanded and contracted by an expansion fluid, and the lumen of the distal shaft and the lumen of the proximal shaft have a flow of the expansion fluid. The distal end enlarged portion may have a communication path that communicates the distal end side and the proximal end side of the distal end enlarged portion. According to this configuration, since the distal end enlarged portion has the communication passage, the flow of the expansion fluid is not hindered when the balloon is expanded and contracted. That is, since the reinforcing body has the communication path at the tip enlarged portion, the reinforcing body has a space that does not have the reinforcing body in the cross section of the shaft, and the shaft lumen can be kept large. Therefore, the balloon can be expanded and contracted without any trouble.
In the above catheter, the distal end enlarged portion may have a random structure in which the thin wire elements are bent at random.
In the above catheter, the thin wire element may have a spiral shape at the distal end enlarged portion.
In the above catheter, the distal end enlarged portion may be disposed between the inner tube and the distal shaft without surrounding the inner tube.
In the catheter described above, the enlarged distal end portion may surround the inner tube.
In the above catheter, the distal end enlarged portion may be in contact with an inner circumferential surface of the distal shaft or an outer circumferential surface of the inner tube. Thereby, a front-end | tip enlarged part can be arrange | positioned stably.
In the above catheter, the distal end enlarged portion may be in contact with both the inner peripheral surface of the distal shaft and the outer peripheral surface of the inner tube. Thereby, a front-end | tip enlarged part can be arrange | positioned more stably.
In the above catheter, the distal end enlarged portion may be fixed to at least one of an inner peripheral surface of the distal end shaft and an inner peripheral surface of the inner tube. Since the reinforcing body has an increased contact area with the inner peripheral surface of the tip shaft or the inner peripheral surface of the inner tube at the tip enlarged portion, the tip enlarged portion can be stably fixed to the tip shaft or the inner tube. it can.
In the catheter, at least one of the distal end shaft and the inner tube may be provided with a stopper that engages with the distal end enlarged portion. With this configuration, the tip enlarged portion can be easily positioned at a predetermined position in the tip shaft.
According to the catheter of the present invention, it is possible to effectively prevent the reinforcing body from breaking through the shaft.
Hereinafter, preferred embodiments of the catheter according to the present invention will be described with reference to the accompanying drawings.
FIG. 1 is a partially omitted schematic diagram showing the configuration of a
The present invention is also applicable to catheters other than PTCA dilatation catheters, for example,
As shown in FIG. 1 (and FIG. 2), the
In this embodiment, the
The
The
The
As shown in FIG. 2, the

The
Therefore, the
The
Inflow (introduction) of the expansion fluid into the

The
The reinforcing
The reinforcing
The
The
The distal end enlarged
The tip enlarged
The distal end enlarged
Further, the distal end enlarged
In this case, the distal end enlarged
The length of the distal end enlarged portion 34 (the length along the longitudinal direction of the shaft 12) is preferably shorter than the length of the
The distal end enlarged
FIG. 3A is a schematic cross-sectional view of the distal-end
In the case of FIG. 3A, the distal end enlarged
FIG. 3B is a schematic cross-sectional view of the distal-end
The tip enlarged
FIG. 3C is a schematic cross-sectional view of the distal-end
FIG. 4A is a schematic cross-sectional view of the tip enlarged
As yet another configuration example, the distal end enlarged

The
The treatment using the
Next, under X-ray fluoroscopy, the
Next, the expansion fluid is sucked from the inside of the
In this case, according to the
Further, in the case of the present embodiment, the distal end enlarged
In the case of the present embodiment, the distal end enlarged
In the case of this embodiment, the distal end enlarged
In the
As shown in FIG. 5A, the
In place of the
As shown in FIG. 5B, the
As a modification of the
As shown in FIG. 5C, the
In the configurations of FIGS. 5A to 5C, the distal end enlarged
In order to obtain the configuration of FIG. 5A or FIG. 5B, the reinforcing
