−症例の約54%を表し、少しの中央の摩耗及び中央のドームによって特徴付けられる凹状の形態と、
−症例の42%を表し、多かれ少なかれ部分的な後方転位による後方の外周の摩耗によって特徴付けられる両凹の形態と、
−あまり一般的ではなく、通常は異形成に起因する25°超の関節窩の過剰な後傾の形態をとる後傾の形態とである。
−長手方向軸(x−x)に沿って延びるとともに近位把持部及び遠位の操作ヘッドが設けられたステム状本体と、
−前記操作ヘッド内の切削要素と、
−前記操作ヘッドから同軸に突出する先端部と
を備えるタイプの、例えば肩プロテーゼの骨インサートを取り出す器具であって、
該器具は、
−前記操作ヘッド内に挿入されるとともに前記切削要素に作用して、該切削要素を、前記先端部内に隠れるように収容される休止位置から、前記先端部の側部開口を通して突出する動作位置まで角度を付けて変位させる押し出し機構であって、前記切削要素は、前記動作位置にあるときに前記長手方向軸に対して実質的に横断方向に延びる端部を有する押し出し機構と、
−前記弾操作ヘッドと前記ステム状本体との間に配置された弾性要素であって、前記操作ヘッド及び前記ステム状本体を互いから離れるように絶えず押しやるとともに前記切削要素を前記休止位置に向かって絶えず押しやる弾性要素と
をさらに備えることを特徴とする器具によって解決される。
−前記骨インサートが取り出される骨部分内に所定の直径の穴を形成するステップと、
−前記穴に、切削要素が設けられている器具の操作端を挿入するステップと、
−前記切削要素を、前記操作端内に隠れるように収容される休止位置から、前記操作端の側部開口を通して突出する動作位置まで、器具の長手方向軸に対して横断方向に移動可能に案内するステップと、
−前記器具を360度回転させることによって前記骨インサートの基部を深く切り込むステップと、
−器具を前記穴から取り出すステップと、
−ピン要素を前記穴に挿入するとともに固定するステップと、
−フライスを前記ピン要素上にセンタリングすることによって、所定の直径のインサートの外周をフライス削りするステップと、
−インサート及びインサートに取り付けられているピン要素を取り出すステップと

−骨インサート2が取り出される骨部分内に所定の直径の穴5を形成し、
−穴5に、切削要素30が設けられている器具9の操作端16を挿入し、
−切削要素30を、操作ヘッド15の先端部16内に隠れるように収容される休止位置から、前記先端部16の側部開口27を通して突出する動作位置まで、器具9の長手方向軸x−xに対して横断方向に移動可能に案内し、
−図7にはっきりと示されるように、器具9を360°回転させることによって骨インサート2の基部に深く切り込みを入れ、
−器具9を穴5から取り出し、
−ピン要素25を前記穴5に挿入するとともに固定し、
−フライス50を前記ピン要素25上にセンタリングすることによって、所定の直径の骨インサート2の外周をフライス削りし、
−骨インサート2及び骨インサート2に取り付けられているピン要素25を取り出す。

- −長手方向軸(x−x)に沿って延びるとともに近位把持部(19)が設けられたステム状本体(40)と、
−互いに向かってまたは離れるように移動するよう前記ステム状本体(40)に摺動可能に関連付けられた遠位の操作ヘッド(15)と、
−該操作ヘッド(15)内の切削要素(30)と、

該器具は、

- 前記弾性要素(34)は、前記操作ヘッド(15)の内側縁と、前記ステム状本体(40)の遠位端のより小さい直径によって形成される縁との間に画定される可変に延びる円筒形の環状ギャップ内で弾性的に延びるばねを含むことを特徴とする、請求項1に記載の器具。
