Description
The present invention relates to a laser treatment apparatus that performs treatment by irradiating a diseased tissue, which is a lesion, with a laser beam.
Conventionally, laser treatment has been performed in which a diseased tissue, which is a lesion, is irradiated with a laser beam, and thermal damage such as coagulation or transpiration is caused on the living tissue of the lesion by the energy of the laser beam.
As part of the laser treatment described above, the inventor first injects a laser light absorbing medium that absorbs laser light between the affected tissue to be irradiated with the laser light and the laser light, and the laser is injected into this medium. The treatment apparatus which gives a bigger thermal damage to an affected part tissue by irradiating light was proposed (refer patent document 1).
A treatment method using the laser treatment device described in
At the time of treatment, a hollow puncture needle through which a fiber for irradiating laser light is inserted is punctured into the affected tissue, an aqueous solution of a laser light absorbing medium is poured into the hollow portion of the needle, and simultaneously with the laser light irradiation, the laser light absorbing medium Is injected into the affected tissue.
In the treatment using the laser treatment apparatus described above, high-temperature steam is generated by the transpiration of living tissue, and this vapor causes thermal damage to the tip of the fiber and the needle. May occur.
The inventor forms a plurality of holes for injecting the aqueous solution of the laser light absorbing medium in the thick part of the outer peripheral portion of the needle in order to protect the fiber and the body tissue from the high temperature steam described above, and the steam is inserted into the fiber. A laser treatment device that escapes to the outside through a hole has been proposed (see Patent Document 2).
However, it is difficult to form a plurality of injection holes in the thick portion of a needle having a diameter of about 1 mm. Even if it can be formed, the needle becomes expensive and must be discarded for each treatment. It was difficult to put it into practical use as a puncture needle.
The present invention has been made in view of the above-described situation, and provides a laser treatment apparatus that can avoid thermal damage to a needle or a fiber, and further, explosion in a body tissue, and can produce a puncture needle at a relatively low cost. Hereinafter, a laser treatment apparatus according to an embodiment of the present invention will be described with reference to the drawings.
<Configuration of laser treatment device>
FIG. 1 shows a basic configuration of a laser treatment apparatus according to the present embodiment. The
Before explaining the configuration of the
In the present invention, indocyanine green (hereinafter referred to as “ICG”) is used as a medium for absorbing laser light. ICG is a safe drug that is used for liver function tests and circulatory function tests and has few serious side effects such as shock. ICG rapidly binds to human serum protein α 1 lipoprotein, is optically stabilized, and the maximum absorption starts immediately from 785 nm when dissolved in an aqueous solution by adding patient's blood to the ICG aqueous solution. Shifts to 805 nm and stabilizes.
A semiconductor laser is used as a light source of the laser oscillator. The oscillation wavelength is 810 ± 10 nm, which is almost the same as the maximum absorption wavelength of the human serum-added IGC aqueous solution. Therefore, when the human serum-added IGC aqueous solution is used, the laser beam is absorbed to the maximum extent by the IGC molecules, and the thermal action on the living soft tissue is maximized.
It is said that white-toned living tissue is poorly absorbed by laser light and, therefore, has a poor thermal effect. However, if IGC molecules are present on the tissue surface to be evaporated, the laser light is efficiently absorbed. . The living tissue is the same as the burning fuel, the temperature rises to 300 ° C. or higher, and the thermal effect increases. By interposing the ICG aqueous solution in this way, it is possible to overcome the difference in thermal action due to the difference in the absorption rate of the laser light due to the color tone of the living tissue.
Next, the function of each member constituting the
The
The
The vapor aspirator 4 sucks and discharges the vapor of the living tissue evaporated by the laser light through the hollow portion of the inner cylinder 81 (see FIG. 2) of the
The

<Configuration and function of puncture needle>
Next, the configuration and function of the
As described above, the
The
The
The
As shown in the cross-sectional view of FIG. 3, a
The
The
FIG. 4 shows a cross section of the
The
The inner diameter of the rear portion of the adapter
When the
A
An aqueous
A
As shown in FIG. 4, since the aqueous

<Configuration and function of three-way stopcock>
Although omitted in FIG. 1, a three-
As shown in FIG. 5, the three-
The

<Laser treatment procedure>
Next, with reference to FIG. 6 and FIG. 7, the procedure of the laser treatment by the
First, the
At the time of puncture, ultrasonic waves are radiated to the living
After the
Next, as shown in FIG. 6C, the
Next, as shown in FIG. 7A, the
Further, the
Next, as a pre-stage of laser treatment, as shown in FIG. 7A, a
First, the pump of the
If the supply of the aqueous solution WS is continued, as shown in FIG. 7A, the
Next, in FIG. 5, the
In a state where the
As described above, when laser light is irradiated from the tip of the
On the other hand, when the laser beam is irradiated with the
In the state shown in FIG. 7A, after the
When the internal pressure of the
After the irradiation of the affected part tissue with the laser beam LL, the integrated
As described above, in the laser treatment apparatus according to the present invention, by combining two members of the
In addition, in the laser treatment apparatus of the present invention, since the three members discarded for each treatment can be provided at a relatively low price, the financial burden on the patient can be reduced.
In the present embodiment, the
In the present embodiment, the semiconductor laser is used as the light source of the
Similarly, in this embodiment, ICG is used as the laser light absorbing medium. However, when a laser other than a semiconductor laser is used as the light source of the
